Abstract: This article examines the situation of self-catering accommodation in the light of tourist experience in Switzerland. Swiss tourism is currently declining, and so are traditional forms of accommodation such as hotels. At the same time there is evidence that the demand for self-catering holiday properties is increasing. Yet very little is known about this sector in Switzerland. The present paper attempts to identify the number of Swiss self-catering properties and presents the results of interviews and an on-line survey, which shed light on this industry and allows the situation in Switzerland to be compared with that in other countries. The number of holiday homes in Switzerland has increased steadily since the 1980s, and could provide a valuable source of rented property and local income. The Swiss self-catering industry is highly seasonal, although rental properties are mostly available all year round. Swiss accommodation is relatively small and expensive, partly because the rent usually includes laundry and other services, not offered in other countries. Owners often rent to offset the cost of upkeep, rather than to make a profit, and this attitude is reflected in the fragmented and rather unprofessional nature of marketing, representation and communications in the sector. A number of recommendations are made for developing the industry, based on the findings. [Copyright &y& Elsevier]

The present study was conducted to assess the extent to which the treatment of patients who take one or more cardiovascular drugs regularly is changed during hospitalisation and over the course of the subsequent two months after release from hospital. In order to elucidate the question more exactly, data was collected on 107 patient after a hospital stay on the internal medicine ward of a university hospital and on 107 patients who had been hospitalised in two non-university hospitals. The average number of changes in medication in patients in the university setting was 2.7 and in patients in the non-university setting it was 2.2. The treatment of patients who were hospitalised for cardiovascular complications was switched more often than that of patients whose circulation was stable at admission. Over the course of the subsequent two months after release, the attending general practitioners (GP) switched the medication at a much lower frequency than the hospitals had done. Within one specific drug class there was no more frequent changes in medication during the hospital stay as afterwards. A drug was discontinued in 107 patients in the university setting and in 124 cases in the two non-university hospitals. The same drug was prescribed again by the treating GP in 30 and 40 patients, respectively, after release. The results of the study show that treatment with drugs that have an effect on the patient's understanding of his illness with regard to its severity may be likely to cause doubts about the effectiveness of the drug and whether the therapeutic decisions that were made by the doctors for medical or other reasons were correct. Therefore, it makes more sense to avoid unnecessary changes in medication whenever possible and only then in unavoidable cases with a clear medical indication.

Cardiovascular diseases represent the major cause of mortality in industrialized countries, including Switzerland, and affect a large percentage of hospitalized patients and patients seen in outpatient clinics. Recently, a decline in the incidence of cardiovascular diseases has been observed, associated with a general improvement in prognosis. This has been attributed to progress achieved in the treatment of many cardiovascular diseases, and to a better identification and treatment of various risk factors. Differences in the clinical manifestations of cardiovascular diseases have been observed between men and women. Women seem to be protected until the time of menopause. In this review, various epidemiological aspects of cardiovascular diseases in men and women are discussed.

Little is known concerning the influence of remote prior coronary artery bypass grafting (CABG) on the outcome of patients with acute myocardial infarction (AMI). Therefore, this study evaluated 2,494 patients with AMI of whom 219 (8.8%) had a history of CABG a mean of 7.1 +/- 3.7 years before the index AMI. Compared with all other patients, those with a history of CABG had an increased prevalence of a history of prior AMI (153 [70%] vs 547 [24%]), congestive heart failure (48 [22%] vs 236 [10%]), and angina pectoris (165 [75%] vs 787 [35%]), all p < 0.001. There was no difference in age, but patients with prior CABG were more often men (192 [88%] vs 1,702 [75%], p < 0.001). During the hospitalization for AMI, patients with prior CABG had more recurrent ischemic pain (100 [46%] vs 732 [32%, p < 0.001]), and more frequently developed non-Q-wave AMI (72 [33%] vs 514 [23%], p < 0.01). In-hospital mortality did not differ among patients with or without prior CABG (15 [7%] vs 195 [9%]). At hospital discharge, more patients with prior CABG had complex ventricular ectopic activity on 24-hour ambulatory electrocardiographic monitoring (48 of 74 [65%] vs 327 of 797 [41%], p < 0.0001), and radionuclide ejection fraction < 0.45 (53 of 99 [54%] vs 430 of 1,024 [42%], p < 0.01). Among patients undergoing coronary angiography during the first 2 months, multivessel coronary artery disease was more prevalent among patients with prior CABG (78 of 107 [73%] vs 508 of 959 [53%], p < 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)
