Preserving the outcome of orthodontic treatment is both important and challenging. However, there is insufficient evidence regarding the best way to ensure long-term treatment outcome. The aim of this study was to evaluate a pre-fabricated chain retainer (PFCR) in terms of: ability to maintain satisfactory treatment outcomes; periodontal and dental health; complications; and patient satisfaction. Overall, 130 patients who had completed orthodontic treatment with a fixed appliance during the period 2016–2019 (follow-up time range, 24–55 months) at a specialist orthodontic clinic in Varberg, Sweden and who had a PFCR in the lower jaw were invited to take part in the study. Little's irregularity index (LII) was recorded on dental casts. Caries, gingivitis, calculus, probing pocket depth, and gingival retractions were registered during clinical examinations. Patient satisfaction and retainer complications were evaluated using a questionnaire. In total, 76 patients (58.5%) agreed to participate. All patients, except for one, had their retainer still in place, and the complication rate was 40%. The LII scores were in the range of 0–4 mm (mean, 1.42 mm). At the retainer site, 82% had calculus, 74% had gingivitis, 1% had pocket depth >4 mm, 10.5% had gingival retractions >2 mm, and 0% had caries. All the patients expressed satisfaction with their retainer. PFCRs exhibit characteristics similar to those of traditional bonded retainers in terms of complications, stability, side-effects and patient experience. Smoking has been linked with both increased and decreased risk of COVID-19, prompting the hypothesis of a protective role of nicotine in the pathogenesis of the disease. Studies of the association between use of smokeless tobacco and COVID-19 would help refining this hypothesis. We analysed data from 424,386 residents in the Stockholm Region, Sweden, with information on smoking and smokeless tobacco (snus) use prior to the pandemic obtained from dental records. Diagnoses of COVID-19 between February and October 2020 were obtained from health-care registers. We estimated the risk of receiving a diagnosis of COVID-19 for current smokers and for current snus users relative to non-users of tobacco, adjusting for potential confounders (aRR). The aRR of COVID -19 was elevated for current snus users (1.09 ;95%CI = 0.99–1.21 among men and 1.15; 95%CI = 1.00–1.33 among women). The risk for women consuming more than 1 can/day was twice as high as among non-users of tobacco. Current smoking was negatively associated with risk of COVID-19 (aRR = 0.68; 95% CI = 0.61–0.75); including hospital admission (aRR = 0.60; 95% CI = 0.47–0.76) and intensive care (aRR = 0.43; 95% CI = 0.21–0.89). The hypothesis of a protective effect of tobacco nicotine on COVID-19 was not supported by the findings. The negative association between smoking and COVID-19 remains unexplained. Aim: Sweden´s first multidisciplinary children's advocacy centre (CAC) was founded in 2005 as a collaborative practice between child protection services, the legal system and health care in response to police‐reported child abuse. CACs were introduced in the county of Skåne in 2007. The aim of the study was to describe the health of children investigated at the CAC in Lund, and to examine whether the CAC model of collaboration responded to the healthcare needs of these children. Methods: All children aged 0–17 years investigated at the CAC in 2015 were included in this retrospective study. We reviewed the CAC files and the children's medical and dental records from one year prior to, until one year after their assessment at the CAC. Results: Our review of the medical and dental records (n = 298) showed a high prevalence of mental, dental and physical ill‐health. After the CAC joint meeting, only 1% of the children were referred for a medical examination and 4% for a focused forensic evaluation. Conclusion: Our study demonstrates limitations in the CAC process in responding to extensive health issues of the young victims of crime. Objective: The aim was to study prevalence of xerostomia, hyposalivation and quality of life among caries active younger adults.Materials and methods: A questionnaire regarding oral and general health, xerostomia and quality of life was mailed to 134 caries active (CA) and 40 caries inactive (CI) patients, 25-50 years of age (mean age 39.9 ± 6.2 years) treated at a Swedish Public Dental Service clinic, regarding oral and general health, xerostomia and quality of life. Caries data and unstimulated whole salivary flow rates were obtained from dental records.Results: The overall response rate was 69%. Dental records confirmed that CA patients had more decayed teeth over time than CI patients (p < .001). The CA group reported worse oral health (p < .001) and general health (p < .01), more xerostomia (p < .001) and lower salivary flow rate (p < .01) compared to CI patients. Xerostomia was inversely related to unstimulated whole salivary flow rates as well as to oral and general health (p < .01). There were no differences between groups in quality of life.Conclusion: Younger caries active adult patients reported significantly more xerostomia and hyposalivation compared to caries inactive patients. Background In dentistry, epidemiological studies are important for establishing high-quality care for the individual patient as well as for socio-economic reasons. Objective The aim of this epidemiological study was to evaluate the prevalence of impacted maxillary canines in a geographical region in which interceptive treatment is implemented systematically. Furthermore, the aim was to study the age and gender of the patients, and the location and surgical technique used for the impacted maxillary canine. Method The study was based on 54 716 adolescents in the Region Västra Götaland, Sweden who were born in the period 1996–98. All patients in the three cohorts who had impacted maxillary canines treated with surgical exposure or surgical removal were identified in the dental record system used in the region. Results The prevalence of impacted maxillary canines when interceptive treatment was systematically implemented was 1.1% (N = 601). Overall, the cohorts of patients with impacted canines comprised 65% girls and 35% boys. Most of the canines were palatally impacted and the most common surgical technique was closed exposure. Conclusion The prevalence of impacted maxillary canines in a geographical area in which interceptive treatment is systematically implemented is lower than that reported previously. Objective: Many patients with temporomandibular disorders (TMD) seem to go undetected within primary dental health care. Primarily we evaluated if the implemented intervention increased the clinical decision-making for TMD patients; secondarily we evaluated if other factors could be identified that predicted performed or recommended TMD treatment.Material and Methods: This case-control study was carried out within the Public Dental Health service in Västerbotten County, Sweden. An intervention based on a decision-tree with three screening questions for TMD (3Q/TMD) was implemented during 2015 in four clinics and compared with the remaining county. A total of 400 individuals were selected-200 3Q-positives and 200 3Q-negatives. The 3Q/TMD consists of Q1-frequent jaw pain, Q2-frequent pain on function, and Q3-frequent catching and/or locking of jaw. The 3Q/TMD answers were analyzed in relation to TMD treatment and any TMD related decision that was collected from the digital dental records.Results: The intervention did not increase the frequencies of traceable clinical decisions among patients with TMD.Conclusions: Despite the implemented intervention aimed, the indicated undertreatment of patients with TMD remains. Future studies are still needed to gain a deeper understanding of the clinical decision-making process for TMD patients in general practice dentistry. [ABSTRACT FROM AUTHOR]

Since 2008, FRAMM has been a guideline for caries prevention for all 3- to 15-year-olds in the Västra Götaland Region in Sweden and a predominant part is school-based fluoride varnish applications for all 12- to 15-year-olds. The aims were to evaluate dental health-economic data among 12- to 15-year-olds, based on the approximal caries prevalence at the age of 12, and to evaluate cost-effectiveness. Caries data for 13,490 adolescents born in 1993 who did not take part and 11,321 adolescents born in 1998 who followed this guideline were extracted from dental records. Those with no dentin and/or enamel caries lesions and/or fillings on the approximal surfaces were pooled into the "low" subgroup, those with 1-3 into the "moderate" subgroup and those with ≥4 into the "high" subgroup. The results revealed that the low subgroup had a low approximal caries increment compared with the moderate and high subgroups during the 4-year study period. In all groups, there were statistically significant differences between those who took part in the guideline and those who did not. The analysis of cost-effectiveness revealed the lowest incremental cost-effectiveness ratio (ICER) for the high subgroup for decayed and/or filled approximal surfaces (DFSa) and approximal enamel lesions together and the highest ICER for the low subgroup for DFSa alone. To conclude, the FRAMM Guideline reduced the caries increment for adolescents with low, moderate and high approximal caries prevalence. Background: It has been suggested that dental caries should be regarded as a chronic disease as many individuals repeatedly develop new caries lesions. How this is perceived by caries active patients is unclear. The aim of this study was to measure patient-reported attitudes and negative experiences related to caries and dental treatment. Methods: A questionnaire was mailed to 134 caries active (CA) and 40 caries inactive (CI) adult patients treated at a Swedish public dental service clinic. The questionnaire included items regarding patient-reported oral health; attitudes towards caries and efforts to prevent them; and negative experiences related to caries and dental treatment. Questionnaire data were supplemented with data on caries and caries prophylaxis from patients' dental records. Exploratory factor analysis was conducted on items related to patients' perceptions of problems to see whether scales could be created. Experiences, perceptions and dental records of CA and CI patients were compared. Results: The overall response rate was 69%. Dental records confirmed that CA patients had significantly more decayed teeth per year and a longer period of caries-active time than CI patients. Factor analysis resulted in 3 distinct scales measuring problems related to caries; 1) caries-related information; 2) negative experiences; and 3) negative treatment/ staff attitudes. A fourth scale measuring perceived problems related to caries was also created. The CA group reported significantly more problems related to caries and dental treatment, received significantly more caries-related information, and reported significantly more negative treatment experiences compared to CI patients. Conclusions: Caries prophylaxis methods need to be improved in order to better meet the needs of caries active patients and to create a more positive experience with dental care. Objective.To investigate the dental health of Swedish children and adolescents with reference to age, gender and residence.Material and methods.Electronic dental records from 300,988 3–19-year-olds in one Swedish region were derived in a cross-sectional study in years 2007–2009. The DMFT system was used. Age was categorized into 3–6/7–9/10–12/13–15/16–17/18–19-year-olds and residence into ‘metropolitan’, ‘urban’ and ‘rural’ areas. ANOVA, generalized linear regression models and Fisher’s exact test were used.Results.Among 7–9-year-old children, nine out of 10 were free from fillings and manifest caries, while for 18–19-year-olds; this proportion was one third. Girls (18–19-year-olds) had a significantly lower risk of caries compared to boys of the same age, RR for the DT index = 0.83 (95% CI = 0.81–0.85). This pattern was reversed in 7–12-year-old children. Children and adolescents in metropolitan and urban areas had significantly more caries than subjects in rural areas, for instance the RR for the DT index in metropolitan 7–9-year-olds was 2.26 (95% CI = 2.11–2.42) compared to their rural counterparts.Conclusions.In the permanent dentition, the overall pattern revealed that girls ≤ 12 years had a higher risk of caries, while adolescent girls had a lower risk of caries, both compared with boys of corresponding ages. Living in an urban or metropolitan area entailed a higher risk of caries than living in a rural area. A greater occurrence of dental caries in adolescents than in children was confirmed. AIM: This was to analyse the tooth types extracted and the reasons for extractions in 3-8 year-old Swedish children. Another aim was to determine what treatments and attendance patterns preceded the caries-related extractions. STUDY DESIGN AND METHODS: A retrospective study of all 5,045 dental records of the 3-8 year-olds at six public dental clinics in a Swedish county was performed to find all extractions performed during one year and the reason for each extraction. The dental records, that included a cariesrelated extraction of a primary molar, were further analysed to find possible non-attended dental visits and treatments preceding the extraction. STATISTICS: The data were analysed using Pearson correlation test, Mann-Whitney U test, and Chi-2 test. RESULTS: 309 (4 permanent and 305 primary) extractions were performed in 206 (119M, 87F) (4.1%) of the patients. The primary molar was the tooth type most frequently extracted, while the central primary incisor was the tooth most frequently extracted. Reasons for extractions were; caries: 60.5%, orthodontics: 11.3%, trauma: 10.4%, other reasons: 17.8 %. Dental records from 100 children with caries-related extractions of primary molars were analysed. A previous treatment had been performed in 51.0% of the caries-related extractions of primary molars, but only 24.0% had any long term restoration material with glass ionomer cement, composite resin or compomer. Non-attendance frequency prior to caries-related extraction of primary molar was higher (P=0.004) among the non-treated children, but 40.1% of the non-treated children had no missed appointments. Caries-related extractions were more common among boys (p=0.0315). CONCLUSIONS: Every 25th child in the agegroup 3-8 years had extractions performed during a one-year period. Only every second primary molar had received any treatment before caries-related extraction, and only 24% had received long-term restorations. A considerable number (40%) of the non-treated children had no missed appointments before the caries related extractions. To compare the dental health of a group of children with complex congenital heart disease with that of age and gender matched healthy controls. Case-control study. Faculty of Medicine and Odontology/Pediatric cardiology and Pedodontics, Umeå University, Sweden. All the cases and their controls lived in the county of Västerbotten in northern Sweden. Each group comprised 41 children with a mean age of 6·5 years. Data were collected from medical and dental records while all bitewing radiographs were read separately by one of the authors. Children with congenital heart disease had significantly more caries in their primary teeth than the control group. The mean dmfs-value was 5·2 ± 7·0 in the cardiac group compared to 2·2 ± 3·5 in the control group ( P<0·05). Twenty-six of the children had all four 6-year-molars, and their mean DMFS-values were 0·9 ± 1·9 in the cardiac group compared to 0·3 ± 0·6 in the control group ( P > 0·05). The children with congenital heart disease had received more caries prevention based on the use of fluorides than the control group. There was a significant correlation between the number of fluoride varnish treatments and the dmfs value of the child ( r = 0·411, P<0·01). Fifty-two per cent of the children in the cardiac group had been prescribed fluoride tablets on one or more occasions compared to 17% in the control group ( P<0·01). Number of months on digoxin medication and the dmfs-value had a significant correlation ( r = 0·368, P<0·05). Ten of the children had been on digoxin medication between 6 and 87 months; this subgroup had a mean dmfs-value of 10·1 ± 8·5. Swedish children with complex congenital heart disease have poorer dental health than healthy age and gender matched controls in spite of intensive preventive efforts. In many cases, intervention had been given when caries were present. A closer cooperation between paediatric cardiology and paediatric dentistry is needed. [ABSTRACT FROM AUTHOR]

• More than half of the subjects had either been institutionalized or had contact with CAP services almost one in three subjects had a history of both. • Subjects with CAP contacts and institutionalization demonstrated more externalizing disorders. • Subjects with CAP contacts and institutionalization had earlier onset of criminal behaviors. • Subjects with CAP contacts and institutionalization had higher numbers of criminal behaviors. Forensic psychiatric patients constitute a heterogeneous patient group, with common comorbidity within the externalizing spectrum. Increased knowledge on early antecedents in the pathway to severe mental illness and criminality is needed. In this study, we investigated early onset externalizing behaviors in three groups of forensic psychiatric patients 1) patients without contact with child and adolescent psychiatry (CAP), 2) patients with CAP contact, and 3) patients with both CAP contact and institutional placement. Participants (N = 98) were consecutively recruited from a cohort of forensic psychiatric patients in Sweden between 2016 and 2020. Data were collected through file information and semi-structured interviews and analyzed with a Bayesian approach. A history of CAP together with an institutional placement during childhood or adolescence was associated with more externalizing disorders, a higher number of convictions over the lifetime, a lower age at first conviction, and a lower age at first self-reported crime. Our findings provide further insight into the importance of early-onset adverse behaviors in the development of later externalizing behaviors, and may be of particular use for practitioners within social services and CAP services. Aim: To examine the prevalence of infant dental enucleation (IDE) of primary canines, an East African traditional remedial procedure, in a multiethnic population of children in Sweden., Design: A cross-sectional study was conducted of dental records of 1133 patients (mean age 4.6 years, SD ± 1.4) attending one public dental service clinic in Sweden. The clinic was located in an area with a large multiethnic community. All were born within the years 2002-2006 and had received a check-up in one of the years 2007-2009. A registry was made of missing primary canines where no reason could be found. In documented cases, information about ethnic origin was extracted. Statistical grouping was made according to known East African ethnicities., Results: At least 36 ethnicities were recorded. Twenty-four (2.1%) patients were missing one or more canines according to the criteria for IDE. Significant difference was seen when comparison was made between patients of known East African ethnicities, of whom 20.8% (21/101) manifested findings consistent with the criteria, and the rest of the population (3/1032; P < 0.001)., Conclusions: Prevalence of cases suggestive of IDE among patients of East African origin points to a need for increased awareness within dental and healthcare communities., (© 2013 BSPD, IAPD and John Wiley & Sons A/S. Efficiently navigating through an interface and conducting work tasks in flow is what GUI designers strive for. Dental professionals, who alternate between examination and treatment of a patient and insertion of data into the Computerized Patient Record system, particularly need an interface that would facilitate the workflow. In this paper we present an inspection evaluation of an existing and widely used Computerized Patient Record system. The Semiotic Inspection Method was applied with the expectation that the method could provide evidence that task flow, navigation and wayfinding were major usability issues of the interface. Also expected was that the Semiotic Inspection would reveal the means and strategies used in the interface in order to communicate the flow. The analysis conducted using the Semiotic Inspection Method showed inconsistencies in the communication of the way forward through the interface. In addition, the profile of the users, regarding digital skills, appears to be ambiguous. Finally, the strategies used in the interface for conveying the workflow could be identified as well.

The aim of the present study was to determine the prevalence and reasons for endodontic treatment in a sample of 19-year-old adolescents living in the city of Malmö, Sweden. Dental records and available radiographs of all 19-year-olds (1,971) registered at the 20 Public Dental Service (PDS) clinics in Malmö were reviewed and data were collected on the following: endodontic treatment, number and type of teeth involved, and diagnosis leading to treatment. Endodontic treatment had been performed in 180 (9.1%) patients. The percentage of 19-year-olds at a PDS clinic who had undergone such treatment varied between 1.2% and 25.2%. One tooth had been treated endodontically in 138 (76.7%) of these patients. In three patients four teeth and in two patients five teeth had needed treatment. The molars were the teeth most frequently treated (46.5%), followed by the anteriors (39.5%), and the premolars (14.0%). The most commonly treated tooth was the maxillary central incisor and the most common diagnosis was caries (56.4%) followed by trauma (22.2%) and tooth developmental disturbances (1.6%). In conclusion, endodontic treatment seems to be fairly common among 19-year-olds in the city of Malmö. The great variation seen between the different PDS clinics most likely reflected variations in the socio-economic structure of the catchment areas of the clinics.

In several large epidemiological studies chronic periodontitis has been implicated as an additional risk factor, independent of other risk factors, for the development of ischaemic heart disease. The underlying mechanism is thought to be a localised infection giving rise to an inflammatory host response, and some experimental data agree with this hypothesis. Recently, however, some studies have questioned the post dated relationship between the two diseases. The current case-record study was undertaken to evaluate the prevalence of chronic periodontitis and the severity of such periodontal disease in a heart transplant population, assuming the latter represented a relatively severely compromised cardiovascular patient population. The study demonstrated that 76% of the patients had various degrees of periodontal disease prior to undergoing a heart transplant. Thus, it is possible that a relationship between cardiovascular disease and periodontal disease exists, but further, large intervention studies will be needed to confirm such a conclusion.

With 852 victims from 17 different countries, the sinking of the Estonia was Europe's most severe passenger ferry disaster. The Finnish Disaster Victim Identification (DVI) team identified all 93 victims recovered from the sea within 33 days of the accident as well as victim number 94 found 18 months later. Dental identification was established in 57 cases (60%).

The etiologies of dental fear and dental behavior management problems in children were investigated in a database of information on 2,257 Swedish children 4-6 and 9-11 years old. The analyses were performed using computerized inductive techniques within the field of artificial intelligence. The database held information regarding dental fear levels and behavior management problems, which were defined as outcomes, i.e. dependent variables. The attributes, i.e. independent variables, included data on dental health and dental treatments, information about parental dental fear, general anxiety, socioeconomic variables, etc. The data contained both numerical and discrete variables. The analyses were performed using an inductive analysis program (XpertRule Analyser, Attar Software Ltd, Lancashire, UK) that presents the results in a hierarchic diagram called a knowledge tree. The importance of the different attributes is represented by their position in this diagram. The results show that inductive methods are well suited for analyzing multifactorial and complex relationships in large data sets, and are thus a useful complement to multivariate statistical techniques. The knowledge trees for the two outcomes, dental fear and behavior management problems, were very different from each other, suggesting that the two phenomena are not equivalent. Dental fear was found to be more related to non-dental variables, whereas dental behavior management problems seemed connected to dental variables.

In forensic odontology, registration of dental characteristics is crucial in the identification procedure. It has been found that the most common errors made are incorrect registration of restorations and confusion about premolars and molars in both jaws. In an earlier study, dental students were observers and the charting was made without radiographs. However, in practical forensic work dentists make the registrations and radiographs are usually available. In this investigation eight dental students and eight dentists made registrations on ten excised macerated jaws with the aid of radiographs. The mean number of errors for each jaw for the students and the dentist was 4 and 3 respectively. The most common error among the dentists was incorrect registration of restorations, while errors on registrations of missing teeth were most common among the students. Even though the material in this study was limited, the results indicate the importance of re-examining of postmortem findings before the comparison with the antemortem data is done. Additionally, the forensic work should be performed by specialists.

A case is described where the identity of an unknown body was established several years after death and despite the fact that the body was found in a foreign country. This was made possible due to the established Interpol routines and the good cooperation between the authorities involved in the Scandinavian countries. A significant role was also played by a computerized register with antemortem dental findings of missing Swedish citizens.
