Aim: To determine the factors that influence nurses' fear of COVID‐19 and professional quality of life as well as their attitudes towards COVID‐19 in four different countries. Background: The emergence of COVID‐19 has affected the psychological and professional quality of life of the frontline health care workers, especially nurses. Design An online cross‐sectional multicultural study. Methods: A total of 1071 nurses from Turkey, Brazil, Spain and Italy were selected by convenience sampling. All participants were invited to complete the Fear of COVID‐19 Scale and Professional Quality of Life Scale through online form from October 2020 to January 2021. Results: It has been seen that nurses' fear of COVID‐19 has been above the average and their professional quality of life has been affected negatively during the pandemic. Almost one out of three nurses (28.6%) thought of quitting their job during the pandemic. The majority of nurses (91.0%) thought their professional quality of life changed during the pandemic. The mean score of the Fear of COVID‐19 Scale is higher in nurses who are working in Brazil, are female, have a chronic illness and are working in an outpatient clinic. Professional quality of life is even lower in nurses who are younger than 40, have a professional experience of less than 15 years, are working in Brazil and have concerns about themselves and their relatives because of COVID‐19. Conclusions: It was determined that there was a relationship between fear of COVID‐19 and professional quality of life subscales of nurses. Implications for Nursing Management: Nurse leaders have an important role in supporting nurses actively during and after the COVID‐19 pandemic and providing them with good working conditions, sufficient resources and rewards. In order for nurses to be able to cope with the challenges brought about by the pandemic, particularly the fear of COVID‐19, and to avoid thoughts of quitting the profession, it would be beneficial to take certain administrative measures on institutional and territorial basis. It is recommended that employees with a chronic disease work in low‐risk clinics, the number of nurses be increased in busier clinics, working hours be reduced and nurses be provided with psychosocial support by experts to cope with stress. On the other hand, effective infection control, personal protective measures and implementing institutional policies and protocols can help to reduce the fear of COVID‐19 and increase their professional quality of life. Objectives: Tropheryma whipplei has been detected in 3.5% of the blood culture-negative cases of endocarditis in Spain. Experience in the management of T. whipplei endocarditis is limited. Here we report the long-term outcome of the treatment of previously reported patients who were diagnosed with infective endocarditis (IE) caused by T. whipplei from the Spanish Collaboration on Endocarditis-Grupo de Apoyo al Manejo de la Endocarditis Infecciosa en España (GAMES) and discuss potential options for antimicrobial therapy for IE caused by T. whipplei.Patients and Methods: Seventeen patients with T. whipplei endocarditis were recruited between 2008 and 2014 in 25 Spanish hospitals. Patients were classified according to the therapeutic regimen: ceftriaxone and trimethoprim/sulfamethoxazole, doxycycline + hydroxychloroquine and other treatment options.Results: Follow-up data were obtained from 14 patients. The median follow-up was 46.5 months. All patients completed the antibiotic treatment prescribed, with a median duration of 13 months. Six patients were treated with ceftriaxone and trimethoprim/sulfamethoxazole (median duration 13 months), four with doxycycline + hydroxychloroquine (median duration 13.8 months) and four with other treatment options (median duration 22.3 months). The follow-up after the end of the treatments was between 5 and 84 months (median 24 months).Conclusions: All treatment lines were effective and well tolerated. Therapeutic failures were not detected during the treatment. None of the patients died or experienced a relapse during the follow-up. Only six patients received antibiotic treatment in accordance with guidelines. These data suggest that shorter antimicrobial treatments could be effective. [ABSTRACT FROM AUTHOR]

According to public opinion the increasing number of foreign population has became one of the most problematic issues in the last decade. Since 2005 immigration is the second biggest concern for Spaniards after unemployment and followed by terrorism. It is has became a state issue and a central government worry of utmost importance. The media coverage given to immigration is very important, specifically the way in which it is approached and presented to the readers. This paper shows the different perspectives or frames used by two key nationwide Spanish newspapers, El País y El Mundo, to cover one of the migration crises that took place in the Canary Islands in spring 2006. Following Framing theory, media content has been analyzed using variables to show the different ways in which these media depict the phenomenon of migration and the approach which each newspaper follows. We screened index cases from 410 Spanish breast/ovarian cancer families and 214 patients (19 of them males) with breast cancer for germ-line mutations in the BRCA1 and BRCA2 genes, using SSCP, PTT, CSGE, DGGE, and direct sequencing. We identified 60 mutations in BRCA1 and 53 in BRCA2. Of the 53 distinct mutations observed, 11 are novel and 12 have been reported only in Spanish families (41.5%). The prevalence of mutations in this set of families was 26.3%, but the percentage was higher in the families with breast and ovarian cancer (52.1%). The lowest proportion of mutations was found in the site-specific female breast cancer families (15.4%). Of the families with male breast cancer cases, 59.1% presented mutations in the BRCA2 gene. We found a higher frequency of ovarian cancer associated with mutations localized in the 5' end of the BRCA1 gene, but there was no association between the prevalence of this type of cancer and mutations situated in the ovarian cancer cluster region (OCCR) region of exon 11 of the BRCA2 gene. The mutations 187underscore;188delAG, 330A>G, 5236G>A, 5242C>A, and 589underscore;590del (numbered after GenBank U14680) account for 46.6% of BRCA1 detected mutations whereas 3036underscore;3039del, 6857underscore;6858del, 9254underscore;9258del, and 9538underscore;9539del (numbered after GenBank U43746) account for 56.6% of the BRCA2 mutations. The BRCA1 330A>G has a Galician origin (northwest Spain), and BRCA2 6857underscore;6858del and 9254underscore;9258del probably originated in Catalonia (northeast Spain). Knowledge of the spectrum of mutations and their geographical distribution in Spain will allow a more effective detection strategy in countries with large Spanish populations. Resistance and vulnerability to trauma: The moderator effect of personality variables on secondary traumatic stress. The purpose of this study was to examine the role of several personality variables (empathy, comprehensibility, challenge and sense of humour) as moderators of the relationship of job demands (traumatic task and overload) with secondary traumatic stress. 175 emergency professionals of the Community of Madrid completed the Secondary Traumatic Stress Measure (STSM). The results of the hierarchical multiple regression analysis provide evidence for the moderator role of personality variables in the secondary traumatic stress process. Lastly, the discussion emphasises the need to focus on the interaction between personality and job demand variables in order to advance our understanding of the process of trauma in emergency professionals. The Coronavirus Disease of 2019 (COVID-19) pandemic caused by SARS-CoV-2 led the Spanish government to impose a national lockdown in an attempt to control the spread of the infection. Mobility restrictions and the requirement of a medical prescription for serological testing for COVID-19 were included among the control measures. Under this scenario, between April 15th and June 15th, 2020, we performed an observational study including 449 individuals allowed to be tested according to the governmental restrictions, i.e. fulfilling the following prescription requirements: manifestation of COVID-19-compatible symptoms, contact with a confirmed COVID-19 patient, or employment as an essential worker, including health care workers, firefighters and public safety personnel such as police. Importantly, a relevant feature of the studied cohort was that none of the participants had been hospitalized. We analyzed SARS-CoV-2 IgG seropositivity in this specific cohort, uncovering intrinsic features of great demographic interest. The overall rate of IgG seropositivity was 33.69% (95% CI: 29.27-38.21). This frequency was comparable among the different participant occupations. A RT-PCR positive test, contact with a household member previously tested positive and the presence of COVID-19-compatible symptoms were positively associated with IgG + results. Among these symptoms, ageusia/anosmia was positively and independently associated with SARS-CoV-2 IgG seropositivity, while odynophagia was inversely associated. However, fever, ageusia/anosmia and asthenia were the most frequent symptoms described by IgG + subjects. Therefore, our data illustrate how specific cohorts display particular characteristics that should be taken into account when studying population-wide SARS-CoV-2 seroprevalence and key defining symptoms of COVID-19., (© 2021.
