Secondary school students' early choices related to staying in the science track define their future decisions to choose chemistry at college. This investigation aims at analyzing the role of gender in students' causal attributions to choose or abandon chemistry when it first becomes optional in the Spanish educational system. Our analyses uncovered a relevant effect of gender in the students' decision, boys being more likely to choose physics & chemistry when they face, for the first time, the possibility of continuing or opting out the subject. Besides, students' causal attributions to the subject relationship with mathematics and to friends are affected by gender regardless of the students' level of motivation. In turn, there is a gender effect in attributions to friends and media only in the case of highly-motivated students. A multinomial logistic regression model revealed that gender is a strong predictor of the students' decision. The regression model also uncovered a significant interaction effect between gender and attributions to the subject relationship with mathematics, girls becoming less likely to choose physics & chemistry when the latter increase. Our results highlight the need of working on the students' and families' stereotypes and propose gender-balanced teaching models to close the gap between girls' and boys' attitudes, motivation, and anxiety towards mathematics in the context of physics & chemistry teaching and learning.

Difficulties in implementing effective instruction for at-risk students arise from two challenges: the transfer of evidence-based knowledge and the lack of economic resources. Computer-assisted programmes offer a suitable solution, providing quality instruction using low-cost resources. Thirty-two first-grade students with early learning difficulties were identified and paired based on at least three of the pre-intervention reading measures (reading efficiency of monosyllabic and disyllabic items, words, pseudowords and text reading speed). Each pair was assigned to one of two different intervention programmes: a computer-assisted intervention programme (CAIP) focused on syllables or the programme provided by the Spanish State School Assistance Services (SSAS). Every week, the CAIP participants received in pairs four 15-min training sessions on syllable decoding plus one 30-min group comprehension session. The CAIP was delivered by trainee students. The SSAS programme typically consisted of a 1-hr individual or in small groups sessions per week delivered by trained practitioners. Both programmes were administered for 11 weeks. The CAIP intervention showed better results than the SSAS intervention for both decoding and comprehension, with moderate to large effect sizes.

The present investigation aims to analyze the effect of motivation on students' causal attributions to choose or abandon chemistry when it first becomes optional in the secondary education curriculum in Spain. Attributions to the effect of the family and to the teacher and classroom methodology were found to be common predictors of the choice to all the students in the sample. However, our analyses point to a significant effect of the students' motivation in other types of attributions. In the case of at-risk of abandonment students, specific causal attributions to the effect of friends and to the subject's relationship with mathematics were found. On the other hand, the effect of media was a significant predictor only in the case of highly-motivated students. Our study provides several suggestions for teachers, schools, and administrations to design counseling strategies to help students make the right choices.

Traditional learning and assessment systems are overwhelmed when it comes to addressing the complex and multi-dimensional problems of clinical communication and professional practice. This paper shows results of a training program in clinical communication under Problem Based Learning (PBL) methodology and correlation between student self-assessment and teachers' assessment. This involves a qualitative-quantitative cross-sectional study in usual practice in the 2nd year of the degree in Medicine. Teaching methodology is PBL, including 15 associate professors and 90 students. Educational tools for learning: PBL cases and seminars (video recorded, theoretical-practical lectures). Assessment tools: Tutorials on those cases worked on PBL (40%), knowledge test (30%), assessment of a case with PBL methodology (20%) and video recording report (10%). Communication skills are evidenced by CICCA-D scale (Connect-Understand-Identify-Agree-Help-Decision). Variables: academic performance, score on CICCA-D and academic methodological assessment. The analysis is carried out using descriptive statistics, calculating the intra-class correlation coefficients and weighted Kappa index with quadratic weights. 92.2% of students passed the course on the first round. In a range between 0 and 34 points students' self-assessment scored 13 (SD ± 5) points and teachers' 16 (SD ±7). A weak (21%-41%) or poor (< 20%) correlation was obtained between teachers and students for all questions on CICCA-D. The authors suggest a summative assessment using different instruments and techniques to assess clinical communication skills from the first year onwards, and highlight the key role of self-assessment, peer assessment and the use of video recording techniques along with feedback in formative assessment.

Objectives: The aim of this study was to estimate the prevalence of dental attrition in a young adult population and to explore associated factors. Method and materials: Dental students aged 18 to 40 years old with a minimum of 24 natural teeth were invited to take part in this cross-sectional study. Their demographic information, medical history, perceived stress, frequency of dietary acid intake, and frequency of oral habits were obtained through several questionnaires. Assessment of each type of tooth wear (erosion, attrition, and abrasion) was performed according to the tooth wear evaluation system (TWES 1.0). All participants underwent salivary tests. Statistical analysis was performed by chi-squared or Mann-Whitney tests and logistic regression models. Results: In total, 178 participants (88 women and 90 men) with a median age of 22 years were included and of those 74.7% (95% confidence interval [CI], 68% to 81%) had dental attrition. Univariate ana lysis revealed positive associations for male sex, age, body mass index, awake bruxism frequency, and erosive tooth wear with the presence of dental attrition. Multivariable logistic regression (Nagelkerke r2 = 0.31) indicated that dental attrition was associated with erosive tooth wear (adjusted odds ratio [aOR], 6.3; 95% CI, 2.8 to 14.2), body mass index (aOR, 1.3; 95% CI, 1.1-1.5), and age (aOR, 1.2; 95% CI 1.0 to 1.3). Conclusion: Dental attrition is present in most young adults of Catalonia. The associated factors were erosive tooth wear, body mass index, and age. In the framework of European educational convergence, university teaching must respond to the new challenges posed by the European System of Transfer Credits (ECTS) while encouraging improvement and innovation in didactical aspects of formation. In Spain, the field of the History of Education has assumed an active role in the advancement of teaching quality by promoting projects for innovation and reform through different institutions and organisms. In this article we will describe a project for teaching innovation, supported by the Universidad Complutense of Madrid's Vice-rectory of quality, which is based on the pedagogy of Freinet. The aim of the project is to apply the Freinet methodology to the teaching of the History of Education to undergraduate students of primary education, pre-school education and pedagogy. The reasons for introducing the Freinet techniques in the university classroom have to do above all with the versatility of its methodology. The method's didactic potential in forming teachers -- owing principally to its way of integrating and unifying diverse innovative methodologies from the Progressive Education movement -- has been amply demonstrated. On the fiftieth anniversary of his death, the pedagogy of the French educator has been shown to be as relevant as ever.

Background: Disability is frequently associated with contextual or lifestyle factors. Some health conditions may affect the prevalence of disability differently, especially for some minority groups. This study aims to assess the impact and contribution of different health conditions to disability burden in Spain in Roma and immigrant populations, compared to the general population. Methods: This is a cross-sectional study. We have used data from the Spanish National Survey of 2017 and the National Health Survey of the Roma Population 2014. We have calculated frequencies of demographic variables and prevalence of health conditions grouped by body function. We also have fitted binomial additive hazard models, using the attribution method, to assess disabling impact and contribution of health conditions to disability burden. The software R was used for the computations. Results: Roma and immigrant populations had worse socioeconomic status than the general population, although the gap was more heavily marked among Roma. Roma population showed a higher prevalence in all health conditions, with a disability prevalence of 57.90%, contrary to the immigrant population, that showed a lower prevalence in all health conditions, including disability (30.79%), than the general population (40.00%). However, all health conditions were more disabling in the immigrant population. Neurological and cardiovascular diseases, and accidents among Roma, were the most disabling conditions. Nevertheless, musculoskeletal, chronic pain, and sensory diseases among Roma, had a greater contribution to disability burden, mainly due to a combination of a great prevalence and a great impact in functions of those health conditions. Conclusion: Both ethnicity and migrant status have shown differences in the burden of disability. While in the general population, musculoskeletal problems have the greatest contribution to the disability burden, in immigrants it was chronic pain and in the Roma population it was sensory problems. The use of reclaimed wastewater to irrigate crops is a valuable option due to water scarcity. However, the presence of antibiotics residues that are not removed in the tertiary treatments reaches crop soils when irrigated and it poses a serious concern for human health. Crops rhizosphere is considered a hotspot of antibiotic resistant genes (ARG) being in addition a link to plant phyllosphere and human microbiome. Understanding the structure of the soil microbiota is crucial before applying any bioremediation or biostimulation strategy. The aim of this work was firstly to confirm the presence of antibiotics residues in soil and fruits in two greenhouses at the south of Spain irrigated with reclaimed water. Secondly, to characterize the rhizosphere microbiome of three crops (Capsicum annuum, Cucumis melo and Solanum melongena) cultured in those greenhouses. Finally, a predictive functional analysis was done using PICRUSt2 to figure out the rhizosphere resistome. The presence of residues of antibiotics was confirmed both soil and fruits. Antibiotics absorbed by plants correlated with those on soil. The most abundant resistance gene was the multidrug in all the three crops tested. Compatibility of basidiomycete fungi (i.e., Pleurotus) with soil bacteria could be indicative of their possible use for restoration of agricultural soils polluted with antibiotics. Genome-wide prenatal cell-free DNA (cfDNA) screening can be used to screen for a wide range of fetal chromosomal anomalies in pregnant patients. In this study, we describe our clinical experience with a genome-wide cfDNA assay in screening for common trisomies, sex chromosomal aneuploidies (SCAs), rare autosomal aneuploidies (RAAs), and copy-number variations (CNVs) in about 6000 patients over a three-year period at our hospital's Prenatal Diagnostic Unit in Spain. Overall, 204 (3.3%) patients had a high-risk call, which included 76 trisomy 21, 21 trisomy 18, 7 trisomy 13, 29 SCAs, 31 RAAs, 31 CNVs, and 9 cases with multiple anomalies. The diagnostic outcomes were obtained for the high-risk cases when available, allowing for the calculation of positive predictive values (PPVs). Calculated PPVs were 95.9% for trisomy 21, 77.8% for trisomy 18, 66.7% for trisomy 13, 10.7% for RAAs, and 10.7% for CNVs. Pregnancy and birth outcomes were also collected for the majority of RAA and CNV cases. Adverse perinatal outcomes for some of these cases included preeclampsia, fetal growth restriction, preterm birth, reduced birth weight, and major congenital structural abnormalities. Several factors in the pathophysiology of obstructive sleep apnea (OSA) may increase the likelihood of excessive occlusal loads. Increased occlusal loads may compromise a tooth's mechanical resistance. This prospective controlled study assessed the frequency of OSA in patients presenting with tooth fracture. Patients with and without tooth fractures were selected in a consecutive manner from a private dental clinic between February 2018 and January 2020. Demographic and dental variables were registered. A parafunctional habits questionnaire, anthropometric measurements, and validated respiratory polygraphy were performed. Descriptive, comparative, and correlation statistical analyses were performed. Multiple regression analysis was also performed. Fifty-nine patients were included, and 29 presented tooth fracture. Patients with a tooth fracture formed the case group and patients without tooth fracture formed the control group. Patient age was significantly higher in the case group (59 ± 13 years) compared to the control group (44 ± 11 years). Most of the fractured teeth were molars, had a natural tooth as an antagonist, and were not endodontically treated. Twenty-one patients had OSA (apnea-hypopnea index: = 5 events per hour) in the case group compared with 12 patients in the control group (P = .027). Moreover, the patients in the case group had a significantly higher apnea-hypopnea index (P = .000). Multiple regression analysis showed that only age had a significant effect on tooth fracture (P = .002). However, there was a statistically significant relationship between OSA and the presence of tooth fracture in patients aged = 40 years (P = .041). The authors' aim was to determine the extent to which error detection contributes to the explanation of a cognitive and motivational model of student performance in an assessment test. A total of 151 science students of secondary education participated in the investigation. Two causal models were developed using a structural equation analysis. This allowed the authors to estimate the effects and relationships between the different variables involved. In addition, they conducted correlational and descriptive analyses of the study variables to further explore the data obtained for the sample. The results indicate that error detection is an important factor that influences student performance to a greater extent than other strategic and motivational variables that have traditionally been considered strong predictors of performance. The findings show that students' detection of their own errors is a mediatory variable that could act as an engagement in the self-regulation cycle.

The authors' objective was to study the role of error detection and retroactive self-regulation as determinants of performance in secondary education students. A total of 198 students participated in the quasiexperimental study, which involved a control group and two experimental groups. This enabled the authors to analyze the effects of both error detection and the subsequent self-regulation by means of several analyses of variance. In addition, the authors analyzed the effect of an assessment script on student's self-assessment. Nevertheless, the divergence in both their self-assessment and error detection was most pronounced when not using the assessment instrument. Furthermore, the correlation analysis revealed that error detection was significantly and positively correlated with students' performance. Students who conducted error detection and subsequently formulated and completed self-regulation activities achieved better performance. The study results suggest that the use of error detection as a tool has considerable potential in the teaching and learning process.

The time series analysis and prediction techniques are highly valued in many application fields, such as economy, medicine and biology, environmental sciences or meteorology, among others. In the last years, there is a growing interest in the sustainable and optimal management of a resource as scarce as essential: the water. Forecasting techniques for water management can be used for different time horizons from the planning of constructions that can respond to long-term needs, to the detection of anomalies in the operation of facilities or the optimization of the operation in the short and medium term. In this paper, a deep neural network is specifically designed to predict water consumption in the short-term. Results are reported using the time series of water consumption for a year and a half measured with 10-min frequency in the city of Murcia, the seventh largest city in Spain by number of inhabitants. The results are compared with K Nearest Neighbors, Random Forest, Extreme Gradient Boosting, Seasonal Autoregressive Integrated Moving Average and two persistence models as naive methods, showing the proposed deep learning model the most accurate results. High uptake of HIV and hepatitis C (HCV) testing in Gay, bisexual, and other men who have sex with men (GBMSM) is needed to interrupt transmission. The objective was to identify subgroups with increased probability of lack of testing among HIV-negative GBMSM in Spain. Cross-sectional study including 3486 HIV-negative GBMSM attending prevention facilities in Madrid and Barcelona, 2018–2020. Data came from self-administered online sociodemographic, health, and risk behaviors questionnaires. Outcomes were lack of HCV (lifetime) and HIV (lifetime, last year) testing. Crude and adjusted prevalences and prevalence ratios were assessed for each outcome using negative binomial regression models. Lifetime lack of HIV and HCV testing prevalence was 6.3% and 35.8%, respectively, while lack of HIV testing in the last year was 22.4%. Prevalences were also substantial in GBMSM with high-risk behaviors. After sociodemographic adjustment, the highest probability of lack of HCV testing (lifetime) and HIV (last year) was among GBMSM with insufficient viral hepatitis knowledge, no history of STI, or HCV (or HIV) testing, aged < 25, non-outness about sex life with men, and less high-risk behaviors. Children from families whose members have reading impairments are found to be poorer performers, take less advantage of instruction, and require more time to reach the reading level of children whose relatives are good readers. As a family's reading history may not be available, a self-report of reading abilities is used to identify children's background. In this paper, we explored the contribution of phonological, literacy, and linguistic abilities and reported parental reading abilities to predict reading achievement at the end of the school year in a Spanish sample. Children who were starting to read were assessed in a variety of oral language, phonological, and literacy tasks at the beginning and end of the school year. Parents filled out a self-report questionnaire about their reading abilities. Their answers were used to assign children to good or poor reader parent groups (GRP vs PRP). A logistic and ROC analysis were used to assess the variables' discriminative capability, considering literacy scores at the end of the year as a measure of reading achievement. GRP children obtained higher scores than PRP children did. Performance on tasks of rapid naming assessment (RAN) letters (78.6%), Word Reading (75.7%), and Deletion (75.6%) were the most accurate predictors of children's reading achievement. IPRA showed slightly lower accuracy (73.8) than did the behavioral measures and as high specificity as RAN letters (96.2%), similarly to the percentages found in previous studies. Although behavioral measures were shown as the best predictors, parents' self-reports could also provide a quick estimation of family risk of difficulties in literacy acquisition.

The ease with which 3D CAD models may be modified and reused are two key aspects that improve the design-intent variable and that can significantly shorten the development timelines of a product. A set of rules are gathered from various authors that take different 3D modelling strategies into account. These rules are then applied to CAD strategic-knowledge learning methodology and included in 3D CAD modelling exercises for students following the degree in mechanical engineering at the University of Burgos (Spain). The experiment was conducted in two groups with a total of 75 students. The design-intent rules were introduced in the different exercises that the teacher explained in both the theoretical and the practical classes. In addition, a summary of the different design rules in each of the practical exercises was explained in the practical classes in only one of the groups. The experimental results, reported in this paper, tested the influence of these summaries on overall improvements in 3D modelling and on the design-intent variable, which is subdivided into four sections: skeleton, structures, alterations and constraints. The use of the summaries of the design intent rules led to statistically significant improvements in 3D modelling in the experimental group, in comparison with the group of students to whom those summaries were not explained.

Purpose: The aim of this study was to investigate the association between Spanish physical education teachers' perceived self-efficacy toward including overweight and obese students and their attitudes toward overweight and obese students. Method: Four hundred and seventy-one physical education teachers filled out questionnaires to assess their self-efficacy expectations to meet overweight and obese students' educational needs, and to identify physical education teachers' beliefs and attitudes toward these students. Results: The physical education teachers with a higher perceived self-efficacy in fostering participation and in assessing and promoting overweight and obese students' learning were more sensitive toward these students' fitness and healthcare and showed less pejorative attitudes toward overweight and obesity. Conclusions: These results highlight the need for teaching education programs capable of increasing the physical education teachers' perceived self-efficacy in this area. The results also show that there is a need for training for teachers and students in strategies aimed at reducing the cases of victimization of overweight and obese students.

Introduction: The debate over the education of immigrant pupils relative to native Spanish students is currently a hot topic, but very little research has been undertaken in this area in Spain. The objective of this study was to detect certain possible differences in motivation and strategies between immigrant and Spanish pupils, and also between boys and girls. Method: A sample of 436 secondary school pupils was used. The Motivated Strategies for Learning Questionnaire (MSLQ) was used as a basis, but was adapted in the shape of the elimination of the goal sub-scales (intrinsic and extrinsic), which were replaced by the Goal Orientation Scale of Skaalvik. Various Student's t-tests were carried out to determine whether or not there were statistically significant differences between the independent samples studied, concretely in respect of nationality of origin and of gender. In interpreting results, account was also taken of the effect of the size of sample. Moreover, MANOVA analyses were also used to check whether there were any interactions between the nationality of origin and gender in respect of the variables studied. Results: In general, it was noteworthy that the main differences found were linked to the gender of pupils, much more than their nationality of origin. Hence, statistically significant differences were found in a total of twelve variables by gender and only three by nationality. Moreover, differences encountered lay mostly in the area of learning strategies rather than in academic motivation and self-motivation. Discussion and conclusion: As a general conclusion for this study, it may be emphasized that the main differences recorded were in respect of gender, much more than with regard to the nationality of origin. More specific studies would be desirable so as to develop further this line of investigation, which is novel in a Spanish context.

Women's education is one of the areas that has received most attention from historical-educational research at a global level and, particularly, in Spain. Its relevance has been constant since its establishment --in the sixties of the last century-- being a priority and particularly fruitful area of study at present. This study aims to vindicate the trajectory of this discipline and its projection in higher education. To this end, a series of arguments are set out which demonstrate its necessity in our context; the educational historiography with a gender focus carried out in recent years in the Spanish context is reviewed; and, finally, its validity in the current study plans in the state's public universities is analyzed. The methodology used was based, on the one hand, on a bibliographical review of gender studies applied to the History of Education, as well as an analysis of the teaching guides of Spanish public universities. The results reveal that the Herstory of Education has had a great impact on the field of research in Spain, judging by the historiographical production, although there is no evidence of a direct correspondence in university teaching. In conclusion, we are still far from fulfilling one of the objectives demanded by international agendas, which necessarily invites us to rethink this discipline in the future curricula of educators required by the democratic societies of the twenty-first century. The advent of the Enlightenment and the cultural shift from the France of the Bourbons resulted in significant changes for Spanish women. The changes in their ways of life turned Enlightenment women into subjects who were conscious of the new ideas and of their own capacities, which they asserted in various spaces. This study centres on Josefa Amar y Borbón, a highly educated woman and a pioneer in joining cultural societies, who created a body of work related to the education and worth of women. Based on the historical-educational method, we analyse its impact and reception in the Spanish press from the end of the 18th century to the first third of the 20th century. We point to Josefa Amar y Borbón's qualities as a translator and to the vindication of her figure as a feminist in Spain, starting in the mid-19th century. [ABSTRACT FROM AUTHOR]

To evaluate molecular assays for Mpox diagnosis available in various clinical microbiology services in Spain through a quality control (QC) approach. A total of 14 centers from across Spain participated in the study. The Reference Laboratory dispatched eight serum samples and eight nucleic acid extracts to each participating center. Some samples were spiked with Mpox or Vaccinia virus to mimic positive samples for Mpox or other orthopox viruses. Participating centers provided information on the results obtained, as well as the laboratory methods used. Among the 14 participating centers seven different commercial assays were employed, with the most commonly used kit being LightMix Modular Orthopox/Monkeypox (Mpox) Virus (Roche®). Of the 12 centers conducting Mpox determinations, concordance ranged from 62.5% (n = 1) to 100% (n = 11) for eluates and from 75.0% (n = 1) to 100% (n = 10) for serum. Among the 10 centers performing Orthopoxvirus determinations, a 100% concordance was observed for eluates, while for serum, concordance ranged from 87.5% (n = 6) to 100% (n = 4). Repeatedly, 6 different centers reported a false negative in serum samples for Orthopoxvirus diagnosis, particularly in a sample with borderline Ct = 39. Conversely, one center, using the TaqMan™ Mpox Virus Microbe Detection Assay (Thermo Fisher), reported false positives in Mpox diagnosis for samples spiked with vaccinia virus due to cross‐reactions. We observed a positive correlation of various diagnostic assays for Mpox used by the participating centers with the reference values. Our results highlight the significance of standardization, validation, and ongoing QC in the microbiological diagnosis of infectious diseases, which might be particularly relevant for emerging viruses. Given the growing interest in drought impacts on crops, this work studied the impact of agricultural drought on wheat and barley during the period 2001–2020. The study was carried out in the Spanish regions of Castilla y León and Castilla–La Mancha, with approximate areas of 94,000 km2 and 79,000 km2, respectively, and in the German regions of Nordrhein-Westfalen, Niedersachsen and Bayern, with approximate areas of 34,000 km2, 48,000 km2 and 71,000 km2, respectively. These are the main cereal-growing regions of Spain and Germany. Soil moisture (SM) in the root zone was extracted from the LISFLOOD model database, and SM anomalies were used as the agricultural drought index. Gross primary productivity (GPP) and leaf area index (LAI) variables were obtained from the Moderate Resolution Imaging Spectroradiometer (MODIS), and the month in which SM is most influential on these crop state variables was identified. Crop yields in Spain and Germany were obtained from the Spanish Ministry of Agriculture, Fisheries and Food and the German Federal Statistical Office, respectively. Agricultural drought years and their impact on cereal yields were determined on a regional scale using three approaches based on the critical month with different time periods. These approaches were the use of the critical month and the two (before or after) and the three months (before and after) around the critical month. Two different analyses were used to identify the critical month, depending on the different environmental conditions in each country. These two approaches consisted of a monthly correlation analysis between SM anomalies and cereal yield in Spain and a monthly trend analysis of SM anomalies in Germany. The results showed a dependence of crop variables on SM in spring months in both countries and in summer months in Germany. Differences were found depending on the environmental conditions. A considerable reduction in cereal yields was obtained in Spain which exceeded 30%. Similarly, a worrying sign was observed in Germany, with a positive agricultural drought trend and a yield reduction of almost 5% in cereal crops. In view of future forecasts of the negative impact of climate change on global food production, this study provides valuable information for water and agricultural management under climate change scenarios. Both in regions that are already threatened and in those that until recently were not affected, it is necessary to study adaptation measures to avoid aggravating the impact of agricultural drought on crops, which could improve water productivity and future food security. Whether the use of more active teaching-learning methods has a positive impact on academic performance remains unanswered. This article seeks to contribute to the issue by conducting a study of an Operations Management course with almost 1000 students per year over three consecutive academic years. The study compares three scenarios with differing degrees of student involvement and Information Technology usage. The findings show no differences in terms of exam grades, but there is an increase in the percentage of passes in the most active scenario analysed, in which continuous assessment tests are used. Notwithstanding, student involvement in the optional assessable activities was found to be a significant factor for predicting the likelihood of passing the subject, especially when continuous assessment tests are used, since their grades show a higher correlation with exam grades.

The article presents the results of an audit on the dispensing of antibiotics in community pharmacies in Spain. It was found that there is variability in the dispensing of antibiotics among different Spanish community pharmacies. In 15% of the dispensations, the three necessary safety checks were performed, while in 24.6% personalized information about the medication was not provided. It is concluded that this audit can serve as a starting point to improve clinical practice in the dispensing of antibiotics and reduce resistance to them. [Extracted from the article]

Objectives: In the context of COVID-19, patients with a severe or critical illness may be more susceptible to developing secondary bacterial infections. This study aims to investigate the relationship between the use of prophylactic antibiotic therapy and the occurrence of bacterial or fungal isolates following the administration of tocilizumab in hospitalized COVID-19 patients who had previously received steroids during the first and second waves of the pandemic in Spain. Methods: This retrospective observational study included 70 patients hospitalized with COVID-19 who received tocilizumab and steroids between January and December 2020. Data on demographics, comorbidities, laboratory tests, microbiologic results, treatment, and outcomes were collected from electronic health records. The patients were divided into two groups based on the use of antibiotic prophylaxis, and the incidence of bacterial and fungal colonizations/infections was analyzed. Results: Among the included patients, 45 patients received antibiotic prophylaxis. No significant clinical differences were observed between the patients based on prophylaxis use regarding the number of clinically diagnosed infections, ICU admissions, or mortality rates. However, the patients who received antibiotic prophylaxis showed a higher incidence of colonization by multidrug-resistant bacteria compared to that of the subgroup that did not receive prophylaxis. The most commonly isolated microorganisms were Candida albicans, Enterococcus faecalis, Staphylococcus aureus, and Staphylococcus epidermidis. Conclusions: In this cohort of hospitalized COVID-19 patients treated with tocilizumab and steroids, the use of antibiotic prophylaxis did not reduce the incidence of secondary bacterial infections. This study explores whether activation and inhibition word processes contribute to the characteristic speed deficits found in transparent orthographies (Wimmer, "Appl Psycholinguist" 14:1-33, 1993). A second and fourth grade sample of normal school readers and dyslexic school readers participated in a lexical decision task. Words were manipulated according to two factors: word frequency (high vs. low) and syllable frequency (high vs. low). It has been repeatedly found that words with high-frequency syllables require extra time for deactivating the lexical syllabic neighbors: the so-called inhibitory effect of positional frequency syllable (Carreiras et al., "J Mem Lang" 32:766-780, 1993). We hypothesized that dyslexic readers would show a stronger inhibitory effect than normal readers because they are slower decoders and they may also be slower at the activation and inhibition of word representations that are competing (i.e., syllabic candidates). Results indicated an interaction between word and syllable frequency (i.e., a strong inhibitory effect was found in the low-frequency word condition). According to our hypothesis, the inhibitory effect size was almost three times bigger in dyslexics than in the normal readers. This difference shows an alteration, not a developmental lag. Interestingly, the inhibitory effect size did not interact with school grade. Thus, reading experience did not impact the lexical processes involved on the inhibitory effect. Our outcomes showed how activation and/or inhibition of lexical processes can contribute to the lack of speed beyond decoding deficit.

Final degree dissertations in cooperation and development (FDDCD) can be a suitable tool for raising the awareness of the university community. In this paper the paradigmatic actions made in this frame in the University of Extremadura for the last five years have been analysed with the aim of elucidating the possible ways to improve the teaching-learning process. For this target, FDDCDs have to be included in a learning project that is designed according to the needs and circumstances of each student. Background: Currently, in the scientific literature there is a great interest on the study of strategies to implement patient-centered care. One of the main tools for this is the therapeutic relationship. Some studies suggest that the perception of the environment in which the treatment takes place can influence the perception of its quality, but this is not explored in physical therapy. For all these reasons, the aim of this study was to understand the influence of the environment in which physical therapy treatment takes place on the patients' perception of the quality of the patient-centered therapeutic relationship in public health centers in Spain. Methods: A qualitative study analysed thematically using a modified grounded theory approach. Data collection used semistructured interviewing during focus groups. Results: We conducted four focus groups. The size of the focus groups ranged from six to nine participants. In total, 31 patients participated in these focus groups. Participants described a series of specific experiences and perceptions relating to the environment, which they felt were influential in the establishment of therapeutic patient-centered relationships, including six physical factors (Architectural barriers, Furniture, Use of the computer, Physical space, Ambiet conditions, and Privacy) and six organizational factors (Patient-physical therapist ratio, Treatment interruptions, Social factors, Continuity with the professional, Lack of professional autonomy, and Coordination or communication among team members). Conclusion: The results of this study highlight environmental factors that affect the quality of the therapeutic patient-centered relationship in physical therapy from the patient's point of view, and emphasize the need for physical therapists and administrators to underline the need to review these factors and take them into account in their service delivery. Eubrachyurans, or 'higher' true crabs, are the most speciose group of decapod crustaceans and have a rich fossil record extending into the Early Cretaceous. However, most extant families are first found in the fossil record in the Palaeogene, and particularly in the Eocene. Unfortunately, fossils of many early eubrachyuran groups are often fragmentary, and only a few studies have combined extinct and extant taxa in a phylogenetic context using different optimality criteria. Here, we report the dairoidid crab Phrynolambrus sagittalis sp. nov., an enigmatic eubrachyuran from the upper Eocene of Huesca (northern Spain), whose completeness and exquisite preservation permit examination of its anatomy in a phylogenetic context. Dairoidids have previously been considered among the oldest stone crabs (Eriphioidea) or elbow crabs (Parthenopoidea), two disparate and distantly related groups of true crabs living today. Mechanical preparation and computed tomography of the fossil material revealed several diagnostic features that allow a detailed comparison with families across the crab tree of life, and test hypotheses about its phylogenetic affinities. Phrynolambrus sagittalis is the first record of the genus in the Iberian Peninsula, and represents one of the oldest crown parthenopoidean crabs worldwide, expanding our knowledge of the biogeographical distribution of elbow crabs during the Palaeogene, as well as their early origins, anatomical diversity and systematic affinities. Understanding the disparity of Eocene eubrachyurans is pivotal to disentangling the systematic relationships among crown families, and interpreting the spatio‐temporal patterns leading to the evolution of modern faunas. This work, situated within the Spanish legal context, presents a reflection upon the need to apply and interpret the law bearing in mind the mandate of the gender perspective contained in national and international legislation. In spite of the significant advances, in questions of gender equality, evidenced by the Spanish legal system in recent decades, these have not led to real, effective equality. In the spheres of both private and public law, this task should be prioritised and undertaken in transversal fashion by all legal operators, judges in particular. Without this commitment, the guarantee of women's human rights will continue to be hindered by the shortcomings of an androcentric legal culture that perpetuates structural discrimination against women. This study estimates the effects of food hypersensitivity on individuals' perceived welfare and well-being compared to non-food hypersensitive individuals. Study respondents were recruited in the Netherlands, Poland, Spain and UK. The difference in welfare between food hypersensitive respondents and those asymptomatic to foods was estimated using a subjective welfare approach, including income evaluation. Well-being was measured using the Cantril Ladder-of-Life Scale, and health status using the Self-Perceived Health Scale. The difference in well-being, welfare and health status between participant groups was explained further using a number of background variables. No significant within-country differences in welfare between food hypersensitive respondents and respondents asymptomatic to foods were found. In terms of well-being, adult food hypersensitive respondents and their spouses reported significantly less happiness than respondents and their spouses asymptomatic to foods in the Netherlands and Poland. In Spain, the spouses of the food hypersensitive respondents were significantly less happy than respondents asymptomatic to foods. The well-being of children did not significantly differ between groups. The degree of severity of food hypersensitivity was negatively related to overall health status. In Poland, food hypersensitive respondents reported worse health status compared to asymptomatic respondents. In Spain, the converse was true. Food hypersensitive respondents were generally less happy with their life as a whole than respondents asymptomatic to foods, presumably because they experienced more negative effects, which were not related to perceived health status.

This article examines the relationship between individual differences in speech perception and sublexical/phonological processing in reading. We used an auditory phoneme identification task in which a /ba/-/pa/ syllable continuum measured sensitivity to classify participants into three performance groups: poor, medium, and good categorizers. A lexical decision task manipulated syllable and word frequency. We found that the two tasks were associated. Poor categorizers did not present the typical syllable frequency effect; however, the other groups were sensitive to phonological information to differing degrees and showed the inhibitory syllable frequency effect only for low-frequency words. These results suggest that auditory phoneme identification efficiency may be related to the sublexical processes involved in reading words. (Contains 2 figures and 1 table.)

Solutans were among the most enigmatic pre-radial and asymmetric echinoderms. A new species Dehmicystis ariasi sp. nov. is described from the upper part of the Llagarinos Formation, lower Ludlow (Silurian) of Northwest Spain. This is the first solutan formally described from Iberia and the first from the Silurian worldwide. Dehmicystis was previously known based on a small number of poorly preserved specimens from the Emsian, Lower Devonian Hunsrück Slate of Germany. New observations based on the newly studied material suggest that Dehmicystis displayed a feeding appendage facing towards the substrate, and the periproct on the opposite face of the theca. Comparisons with other solutans and new data suggest that Dehmicystis was a detritus feeder that moved over the substrate capturing organic particles from the sediment with a single feeding arm. Background: Measurement of muscle mass and function, and thereafter, screening and diagnosis of sarcopenia, is a challenge and a need in hospitalized older adults. However, it is difficult in complex real-world old patients, because usually they are unable to collaborate with clinical, functional, and imaging testing. Ultrasound measurement of quadriceps rectus femoris (QRF) provides a non-invasive, real-time assessment of muscle quantity and quality, and is highly acceptable to participants with excellent inter-rater and intra-rater variability. However, normative data, protocol standardization, and association with longitudinal outcomes, needs further research and consensus. Methods: Prospective exploratory multicenter study in older adults admitted to Acute Geriatric Units (AGUs) for medical reasons. 157 subjects from 7 AGUs of Spain were recruited between May 2019 and January 2022. Muscle ultrasound measurements of the anterior vastus of the QRF were acquired on admission and on discharge, using a previously validated protocol, using a Chieson model ECO2 ultrasound system (Chieson Medical Technologies, Co. Ltd, Wimxu District Wuxi, Jiangsu, China). Measurements included the cross-sectional area, muscle thickness in longitudinal view, intramuscular central tendon thickness, echogenicity, and the presence or absence of edema and fasciculations. Functional, nutritional, and DXA measurements were provided. Clinical follow-up was completed at discharge, and 30 and 90 days after discharge. Variations between hospital admission and discharge ultrasound values, and the relationship with clinical variables, will be analyzed using paired t-tests, Wilcoxon tests, or Mc Nemar chi-square tests when necessary. Prevalence of sarcopenia will be calculated, as well as sensitivity and specificity of ultrasound measurements to determine sarcopenia. Kappa analysis will be used to analyze the concordance between measurements, and sensitivity analysis will be conducted for each participating center. Discussion: The results obtained will be of great interest to the scientific geriatric community to assess the utility and validity of ultrasound measurements for the detection and follow-up of sarcopenia in hospitalized older adults, and its association with adverse outcomes. Background: The COVID-19 pandemic modified hospital functioning, social mobility and assistance to the elderly. These factors may alter hip fracture evolution. Methods: Retrospective study of patients > 65 years suffering hip fracture from the 1st of March to the 31st of August of 2020, and compared with patients from the same period of 2019; 199 patients from 2019 (79.4%women) (mean age: 84.9 ± 7.72 years), and 203 patients from 2020 (69.5%women) (mean age: 85.37 ± 8.13 years). Extracapsular fractures were 57.3% in 2019, and 58.6% in 2020. Follow-up was made until death or 6 months after surgery. Results: In 2019, 44 patients (22.1%) died in the six postoperative months, and 61 patients (30%) in 2020 (OR 1.513; 95% CI 0.96-2.37; p = 0.199). There were no significant differences in mortality during postfracture hospitalization (11.1% in 2019; 11.82% in 2020) and in the first postoperative month (16.6% in 2019; 22.66% in 2020). Four patients died during hip fracture hospitalization over 8 patients presenting PCR+. In 30 patients SARS-CoV-2 infection was diagnosed during the six months after hip fracture in 13 patients this was the cause of death (21.3% of causes). Dementia was associated with high mortality in patients with a SARS-CoV-2 diagnosis (6 out of 13 patients died with SARS-CoV-2 infection and dementia). Conclusions: An increase in the 6 postoperative months mortality in 2020 compared with the same period in 2019 was observed. An important cause of death was SARS-CoV-2 infection. Background: Botulism is a low incidence but potentially fatal infectious disease caused by neurotoxins produced mainly by Clostridium botulinum. There are different routes of acquisition, food-borne and infant/intestinal being the most frequent presentation, and antitoxin is the treatment of choice in all cases. In Spain, botulism is under surveillance, and case reporting is mandatory. Methods: This retrospective study attempts to provide a more complete picture of the epidemiology of botulism in Spain from 1997 to 2019 and an assessment of the treatment, including the relationship between a delay in antitoxin administration and the length of hospitalization using the Cox proportional hazards test and Kruskal–Wallis test, and an approach to the frequency of adverse events, issues for which no previous national data have been published. Results: Eight of the 44 outbreaks were associated with contaminated commercial foods involving ≤7 cases/outbreak; preserved vegetables were the main source of infection, followed by fish products; early antitoxin administration significantly reduces the hospital stay, and adverse reactions to the antitoxin affect around 3% of treated cases. We describe the first 25 persons with HIV diagnosed with human monkeypox virus (MPXV) in our hospital in an ongoing outbreak in Spain. Proctitis was the predominant finding in 52%, and MPXV DNA was detected in rectal swabs from 90%. Proctitis and demonstration of MPXV in rectal swabs support the sexual transmission of MPXV. Background: Only very few studies have investigated the effect of the COVID-19 pandemic on the pre-hospital stroke code protocol. During the first wave, Spain was one of the most affected countries by the SARS-CoV-2 coronavirus disease pandemic. This health catastrophe overshadowed other pathologies, such as acute stroke, the leading cause of death among women and the leading cause of disability among adults. Any interference in the stroke code protocol can delay the administration of reperfusion treatment for acute ischemic strokes, leading to a worse patient prognosis. We aimed to compare the performance of the stroke code during the first wave of the pandemic with the same period of the previous year. Methods: This was a multicentre interrupted time-series observational study of the cohort of stroke codes of SUMMA 112 and of the ten hospitals with a stroke unit in the Community of Madrid. We established two groups according to the date on which they were attended: the first during the dates with the highest daily cumulative incidence of the first wave of the COVID-19 (from February 27 to June 15, 2020), and the second, the same period of the previous year (from February 27 to June 15, 2019). To assess the performance of the stroke code, we compared each of the pre-hospital emergency service time periods, the diagnostic accuracy (proportion of stroke codes with a final diagnosis of acute stroke out of the total), the proportion of patients treated with reperfusion therapies, and the in-hospital mortality. Results: SUMMA 112 activated the stroke code in 966 patients (514 in the pre-pandemic group and 452 pandemic). The call management time increased by 9% (95% CI: -0.11; 0.91; p value = 0.02), and the time on scene increased by 12% (95% CI: 2.49; 5.93; p value = <0.01). Diagnostic accuracy, and the proportion of patients treated with reperfusion therapies remained stable. In-hospital mortality decreased by 4% (p = 0.05). Conclusions: During the first wave, a prolongation of the time "on the scene" of the management of the 112 calls, and of the hospital admission was observed. Prehospital diagnostic accuracy and the proportion of patients treated at the hospital level with intravenous thrombolysis or mechanical thrombectomy were not altered with respect to the previous year, showing the resilience of the stroke network and the emergency medical service. : Few studies have investigated the psychometric properties of the Scale for Contraversive Pushing (SCP) in depth, and none have evaluated its ability to establish differential diagnosis between pusher behavior (PB) and thalamic astasia (TA). : To study the ability of the SCP to establish differential diagnosis, its reliability, content, construct, and internal validity in the assessment of subacute stroke patients. : 120 individuals were evaluated using the SCP over a four-week period of treatment. Intra- and inter-observer reliability, floor and ceiling effects, minimum detectable change (MDC), internal validity and sensitivity to change were explored. In addition, the Barthel Index and the Trunk Control Test were used to study their correlations with the SCP. : Discriminant validity provides evidence that the correlation between SCP items was large or moderate. Convergent validity demonstrated that the correlation of each item with the total score of the scale was high (at around 0.8). Sensitivity to change was large (W = 0.274). Intra- and inter-observer reliability were excellent (Intraclass Correlation Coefficient > 0.9; k > 0.8), except for items B standing and C sitting (k > 0.7). The MDC was 1.39, and ceiling (8.333%) and floor (15.833%) effects were adequate. Cronbach's alpha (α) was equal to 0.901 (0.874–0.924) and McDonald's Omega (ω) was equal to 0.883 (0.856–0.973), showing excellent internal consistency. : The SCP is a reliable and valid tool which can successfully establish differential diagnosis between PB and TA and evaluate the changes generated by physiotherapy treatment. The almond crop in Spain has increased in importance in recent years and consequently there is a need to improve knowledge about pests, diseases, and weeds. The present study was conducted from 2017 to 2020, with the objective of determining the fauna of spider mites and their natural enemies, with a special emphasis on phytoseiids. The main spider mite species was Tetranychus urticae Koch, and secondary species were Bryobia rubrioculus (Scheuten) and Eutetranychus banksii (McGregor). Phytoseiidae were the most abundant group of natural enemies, with 59% of the individuals observed; Euseius stipulatus (Athias-Henriot) was the predominant species, accounting for 96% of adult females identified, Typhlodromus (Anthoseius) athenas Swirski & Ragusa accounted for the remaining 4%. Other (potential) natural enemies were Chrysopidae, Scolothrips longicornis Priesner, and Stethorus sp. with 36, 2, and 3%, respectively, of the natural enemy individuals. The seasonal pattern of T. urticae indicated population peaks from July to September, and its control was based on miticides in most seasons. Euseius stipulatus and T. athenas appeared mainly in May–June and did not show interaction with the spider mite population. Chrysopidae were present throughout the season, from May to October in the 4 years, but no direct relationship with the spider mite population was observed. In contrast, the seasonal pattern of both S. longicornis and Stethorus sp. coincided with the most important peaks of spider mites and these predators were seen in the spider mite colonies, although in very low numbers. The S:A222V point mutation, within the G clade, was characteristic of the 20E (EU1) SARS-CoV-2 variant identified in Spain in early summer 2020. This mutation has since reappeared in the Delta subvariant AY.4.2, raising questions about its specific effect on viral infection. We report combined serological, functional, structural and computational studies characterizing the impact of this mutation. Our results reveal that S:A222V promotes an increased RBD opening and slightly increases ACE2 binding as compared to the parent S:D614G clade. Finally, S:A222V does not reduce sera neutralization capacity, suggesting it does not affect vaccine effectiveness. Author summary: Since early 2020, the trajectory of the COVID-19 pandemic has mostly been shaped by the appearance of novel variants of the SARS-CoV-2 virus. Accordingly, much of the scientific effort has been directed toward the question of explaining, understanding, and predicting the evolutionary fate of individual mutations in the viral genome. In this article, we focus on A222V, a particular mutation in the Spike protein that emerged in Spain in mid-2020 and reappeared independently in the AY.4.2 subvariant of Delta one year later. As reemerging mutations often indicate an evolutionary advantage, we explored potential mechanisms linking A222V to biologically relevant outcomes. Using serological, functional, structural, and computational approaches, we identified key molecular-level differences conferred by A222V that potentially explain its repeated emergence in different genetic backgrounds. Our results point to subtle changes in the dynamic behavior of the receptor-binding domain in the binding-competent "up" conformation, ones that affect receptor binding itself, but can also act synergistically with other mutations by changing the accessibility of key residues involved in molecular recognition. The relationship between modelled root zone soil moisture (SM) and black truffle production in the Iberian Peninsula was studied. Previous works have investigated the influence that precipitation exerts on truffle yield highlighting the importance of water for the growth of black truffle. However, SM had not been used until now due to the lack of suitable databases. The SM series from the LISFLOOD hydrological rainfall–runoff model was used in this study. Annual black truffle yield series from 175 locations in Spain was correlated with SM for the period 1991–2012. For this, different approaches were applied considering daily, weekly and monthly temporal scales. The same analysis was carried out using precipitation data to compare the behaviors of both variables related to truffle production variability. The results obtained show critical periods in terms of soil water content in summer (June–September) and during October–November months. Moreover, a clear delay between precipitation and SM influence on black truffle was observed. The results obtained in this study highlight the importance of SM for black truffle production, since this variable truly expresses the available water for this fungus, which completes its entire life cycle living below ground. The Roda Formation (early Eocene Ypresian, Huesca, Aragón, northern Spain) records successive levels including rich decapod faunas relevant for the understanding of Ypresian decapod assemblages. Asthenognathus fernandezi n. sp. is described from one of these levels, representing the oldest report of a fossil Asthenognathinae, and the first report from the Iberian Peninsula. Fossil members of Asthenognathinae Stimpson, 1858, are difficult to distinguish from other families, especially when they lack preserved pereiopods, sternum, and abdomen. The new taxon shows important features that include reduced pereiopods, small and sub-cylindrical carapace, and absence of protruding ornamentation. This combination of characters indicates a possible early adaptation of Asthenognathus Stimpson, 1858 to life in cylindrical cavities. Asthenognathus fernandezi n. sp. occurred on poorly consolidated siliciclastic substrates associated with a rich benthic fauna, including echinoderms, molluscs, and other crustaceans. BACKGROUND: Noninvasive respiratory support is commonly used in treatment of bronchiolitis. Determinants of failure are needed to prevent delayed intubation. METHODS: We conducted a prospective observational pilot study in infants admitted to a pediatric ICU. Diaphragmatic excursion (dExc), diaphragmatic inspiratory/expiratory time, and diaphragmatic thickening fraction (dTF) were recorded at admission, 24 h, and 48 h in both hemidiaphragms. RESULTS: Twenty-six subjects were included (14 on HFNC and 12 on NIV) with a total of 56 ultrasonographic evaluations. Three subjects required invasive ventilation. Sixty-four percent of the subjects on HFNC required NIV as rescue therapy and 2/14 invasive ventilation (14.2%). In the HFNC group there were no differences in dExc between those who required escalation to NIV or invasive ventilation and those who didn't. Left dTF was higher in subjects on HFNC requiring invasive ventilation versus those needing NIV (left dTF 47% vs 22% [13-30]; P = .046, r = 0.7). Diaphragmatic I:E ratios were higher in infants on HFNC requiring invasive ventilation and diaphragmatic expiratory time was shorter (left P = .038; right P = .02). In the NIV group there were no differences in dExc, I:E ratios, or dTF between subjects needing escalation to invasive ventilation and those who didn't. We found no correlation between a clinical work of breathing score and echographic dTF. CONCLUSIONS: In infants with moderate or severe bronchiolitis receiving HFNC, the use of ultrasonographic left dTF could help predict respiratory treatment failure and need for invasive ventilation. Cereals are keystone crops for achieving food security and socioeconomic equilibrium, but rainfed cropland is highly sensitive to environmental anomalies that impact yields. The impact of soil moisture on cereal yield is particularly overlooked. This study evaluates the impact of root-zone soil moisture on yield compared to nine common climatic variables: maximum and minimum temperature, diurnal temperature range, growing degree days, accumulated rainfall, radiation, photothermal quotient, relative humidity of the air, and vapor pressure deficit. This study used the climatic database E-OBSv23 and the soil moisture databases ERA5-Land and LISFLOOD, focused on wheat and barley over the main cereal areas of Spain. Correlation analysis between annual yield and daily soil moisture and climatic data provided indicated the prevalence and concurrence of the impact factors on phenological stages of the Zadoks scale. Critical periods of impact on wheat and barley yields primarily concentrate during the growth and reproductive phases of spring. Soil moisture exceeds all other factors in magnitude and duration of influence, and our results suggest a complex interplay of factors during the critical spring period. This study highlights the preeminent role of soil moisture over climatic factors on the variability of rainfed cereal yields in water-limited areas. [ABSTRACT FROM AUTHOR]
