Few studies have reported comparisons of out-of-hospital clinical outcomes after transcatheter aortic valve replacement (TAVR) and surgical aortic valve replacement (SAVR) in patients with severe aortic stenosis (AS) in the era of current-generation valves that reflect the real-world situation. Data on patients with severe AS aged 65 years or older who underwent TAVR or SAVR between 2015 and 2018 were obtained from the National Health Insurance Service in Korea and clinical event rate was analyzed. The primary endpoint was all-cause death at 1 year. The cohort included a total of 4623 patients over 65 years of age, of whom 1269 (27.4%) were treated with TAVR. After 1:1 propensity score matching, 2120 patients were included in the study. TAVR was associated with reduced 1-year mortality (hazard ratio (HR): 0.55; 95% confidence interval (CI): 0.42–0.70; p < 0.001). There was no difference between the groups in the incidence of ischemic stroke (HR: 0.72, 95% CI: 0.43–1.20; p = 0.21) and intracranial hemorrhage (HR: 1.10; p = 0.74). Permanent pacemaker insertion was observed more frequently in the TAVR cohort (9.4% vs. 2.5%, HR: 3.95, 95% CI: 2.57–6.09; p < 0.001), whereas repeat procedures were rare in both treatments (0.5% vs. 0.3%, p = 0.499). In the nation-wide real-world data analysis, TAVR with current-generation devices showed significantly lower 1-year mortality compared to SAVR in severe AS patients. [ABSTRACT FROM AUTHOR]

This article explores South Korea's journey to protect athletes from physical abuse and sexual harassment in response to the International Olympic Committee's Olympic movement initiative to safeguard athletes' human rights. The paper critically reviews a series of campaigns to protect athletes, providing an academic opportunity to understand how Olympic movement initiatives have been accepted and developed by the National Olympic Committees through a case study of South Korea. The analysis of these cases indicates that violations of human rights against South Korean athletes were mainly due to unequal balances of power within the structural hierarchy of the Korean sports apparatuses. Our conclusion of structural inequality within elite sports and a lack of individual human rights awareness proves consistent with previous discussions demonstrating an absence of ethical understanding fostered by a win at all costs mentality. Although the Korean government and the Korean Sports and Olympic Committee have failed to promote a safe sports environment for athletes over the past 15 years, they have made institutional reforms to safeguard athletes through numerous trials and errors. Adverse drug reactions (ADRs) pose a global public health threat, substantially contributing to death. Due to the relative paucity of clinical evidence regarding fatal ADRs, this study was performed to characterize the epidemiology of fatal ADRs in Korea. This was a retrospective, cross-sectional analysis of ADR cases reported to the Korea Adverse Event Reporting System from 2010 to 2019. All ADRs were coded using the World Health Organization-Adverse Reaction Terminology system and classified as either fatal or non-fatal events. Logistic regression was performed to identify factors associated with fatal events. Among 289,756 ADR records, 629 fatal events (0.2%) occurred. The most common causative agent of fatal ADRs was antibacterials (20.3%), followed by antimycobacterials (5.4%), analgesics (4.0%), and contrast media (1.9%). Among antimicrobials, vancomycin was most frequently implicated without significantly increasing the risk of fatal events. The risk for fatal ADRs was significantly increased with male sex; advanced age; polypharmacy; piperacillin/β-lactamase inhibitor; cefotetan; ceftriaxone; combination antimycobacterial therapy consisting of rifampicin, isoniazid, pyrazinamide, and ethambutol; morphine; and iopromide (reporting odds ratio > 1, p < 0.05 for all). Background: In patients with acute myocardial infarction receiving potent antiplatelet therapy, the bleeding risk remains high during the maintenance phase. We sought data on a uniform unguided de-escalation strategy of dual antiplatelet therapy (DAPT) from ticagrelor to clopidogrel after acute myocardial infarction.Methods: In this open-label, assessor-masked, multicentre, non-inferiority, randomised trial (TALOS-AMI), patients at 32 institutes in South Korea with acute myocardial infarction receiving aspirin and ticagrelor without major ischaemic or bleeding events during the first month after index percutaneous coronary intervention (PCI) were randomly assigned in a 1:1 ratio to a de-escalation (clopidogrel plus aspirin) or active control (ticagrelor plus aspirin) group. Unguided de-escalation without a loading dose of clopidogrel was adopted when switching from ticagrelor to clopidogrel. The primary endpoint was a composite of cardiovascular death, myocardial infarction, stroke, or bleeding type 2, 3, or 5 according to Bleeding Academic Research Consortium (BARC) criteria from 1 to 12 months. A non-inferiority test was done to assess the safety and efficacy of de-escalation DAPT compared with standard treatment. The hazard ratio (HR) for de-escalation versus active control group in a stratified Cox proportional hazards model was assessed for non-inferiority by means of an HR margin of 1·34, which equates to an absolute difference of 3·0% in the intention-to-treat population and, if significant, a superiority test was done subsequently. To ensure statistical robustness, additional analyses were also done in the per-protocol population. This trial is registered at ClinicalTrials.gov, NCT02018055.Findings: From Feb 26, 2014, to Dec 31, 2018, from 2901 patients screened, 2697 patients were randomly assigned: 1349 patients to de-escalation and 1348 to active control groups. At 12 months, the primary endpoints occurred in 59 (4·6%) in the de-escalation group and 104 (8·2%) patients in the active control group (pnon-inferiority<0·001; HR 0·55 [95% CI 0·40-0·76], psuperiority=0·0001). There was no significant difference in composite of cardiovascular death, myocardial infarction, or stroke between de-escalation (2·1%) and the active control group (3·1%; HR 0·69; 95% CI 0·42-1·14, p=0·15). Composite of BARC 2, 3, or 5 bleeding occurred less frequently in the de-escalation group (3·0% vs 5·6%, HR 0·52; 95% CI 0·35-0·77, p=0·0012).Interpretation: In stabilised patients with acute myocardial infarction after index PCI, a uniform unguided de-escalation strategy significantly reduced the risk of net clinical events up to 12 months, mainly by reducing the bleeding events.Funding: ChongKunDang Pharm, Medtronic, Abbott, and Boston Scientific. A chain of paired mound and depression structures are presented in the southwestern continental shelf of the Ulleung Basin, East Sea, Korea. We integrate multibeam bathymetry to identify topographic features, sub-bottom profile and multi-channel seismic reflection data to confirm geological features, and piston cores to understand the sediments of the mounds and depressions. Paired structures occur with a northwest–southeast orientation at water depths between 130 and 150 m and have similar scales of 10–20 m in height and depth. The mounds consist of cold-water corals and authigenic carbonate with shelly sand, whereas, the surficial lithology of depressions are only composed of siliciclastic deposits. The mounds appeared to have grown from the depression based on the observation for morphological features in the cross-section profiles of the paired mound and depression structures. Seismic profiles show acoustic blanking anomalies and seismic chimneys, indicating that the topographic relief formed under the influence of gas seepage. The petrological and geochemical signature of carbonate chunks also displayed gas leak effects different from CWC. This study suggests a developmental history model with paired mound and depression structures, in which gas/fluid escape causes depressions and the selective coral growth creates mounds growing on one side of these depressions. The mounds had sufficiently grown, depressions would have been further developed in interaction with the bottom current. • The seafloor mounds consist of authigenic carbonate and cold-water coral that is L. pertusa. • Cold-water coral may have settled on authigenic carbonates that act as a hard bottom. • Cold-water coral colonized the mounds at least during the late Holocene. • The depressions initially are formed by gas and developed by the bottom current.
