Objectives: The aim of the study was to investigate the work competence of general practitioners (GPs) in the community health services (CHSs) of Shanghai, China. Methods: A questionnaire was designed based on a previous capacity evaluation indicator system. We used a stratified and proportional cluster sampling method in this self-assessment and cross-sectional study. We collected data with the questionnaire on GPs' demographic variables and work competence including patient care ability, teaching ability, communication skill and coordination ability. Univariate analyses were performed by Mann-Whitney U test and Kolmogorov-Smirnov test. Multivariate analyses were done with generalized liner model with significant univariate factors. Results: A total of 2954 GPs were sampled from 116 CHSs in Shanghai. The response rate was 99.9%. The median scores of patient care ability, teaching ability, communication skill and coordination ability were 80[70–88.75], 76[60–80] and 80[70–85] on a scale of 100, respectively. GPs who were 30–39 years old, or worked in urban CHSs, or took GP trainer's training or had teaching experience got higher scores in patient care ability. GPs who worked for 5–19 years in CHSs, or worked in CHSs with GP training program or took GP trainer's training had higher scores in teaching ability. For communication skill and coordination ability, GPs who worked in CHSs with GP standardized training program, or took GP trainer's training or had teaching experience in CHSs got higher scores. Conclusions: The work competence of GPs in CHSs of Shanghai could mainly cover daily work, but still needed more improvement in teaching ability. [ABSTRACT FROM AUTHOR]

Super high-rise building is a typical architectural form of urban modernization, and it is also the trend of urban architectural development. This paper reviews the development history of super high-rise buildings in China combined with the engineering practice of representative super high-rise buildings since the 1980s, such as the Shanghai Center, Shanghai Oriental Pearl Tower, Jinmao Tower, Shanghai World Financial Center, Canton Tower, and Shanghai Tower. The development process of the key technologies of super high-rise projects from the aspects of several technologies, such as soft soil pile foundation and foundation pit technology, concrete structure construction technology, integral formwork equipment technology, and steel structure comprehensive installation technology, is also elaborated. At the same time, the Shanghai Tower is selected as a typical case to share the innovative technology of China's tallest building. Introduction: The clinical performance of [–2]proPSA (p2PSA) and its derivatives in predicting the presence and aggressiveness of prostate cancer (PCa) has been well evaluated in prostate biopsy patients. However, no study has been performed to evaluate the common genetic determinants that affect serum level of p2PSA. Materials and Methods: Here, we performed a two-stage genome-wide association study (GWAS) on the p2PSA level in Chinese men who underwent a transperineal ultrasound-guided prostate biopsy at Huashan Hospital, Shanghai Cancer Center, and Ruijin Hospital in Shanghai, China. Germline variants significantly associated with the p2PSA level in the first stage (n = 886) were replicated in the second stage (n = 1,128). Multivariate linear regression was used to assess the independent contribution of confirmed single nucleotide polymorphisms (SNPs) and known covariates, such as age, to the level of p2PSA. Results: A novel non-synonymous SNP, rs72725879, in region 8q24.21 of the PRNCR1 gene was significantly associated with the serum level of p2PSA in this two-stage GWAS (p = 2.28 × 10−9). Participants with homozygous "T" alleles at rs72725879 had higher p2PSA levels compared to allele "C" carriers. This variant was also nominally associated with PCa risk (p -combined = 3.44 × 10−18). The association with serum level of p2PSA was still significant after adjusting for PCa risk and age (p = 0.017). Conclusions: Our study shows that the genetic variants in the 8q24.21 region are associated with the serum level of p2PSA in a large-scale Chinese population. By taking inherited variations between individuals into account, the findings of these genetic variants may help improve the performance of p2PSA in predicting prostate cancer. Background/purpose: The association between adverse mental health and unhealthy lifestyle behaviors in migrant workers remains poorly defined in Chinese rural-to-urban migrants.Methods: A cross-sectional study was conducted regarding health-related behaviors in 5484 migrants (51.3% males) employed in Shanghai for at least 6 months. The Chinese version of the Symptom Checklist-90-Revised (SCL-90-R) was used to assess migrant mental health status. Logistic regression was applied to determine the contribution of adverse mental health to lifestyle behaviors.Results: Of the 5484 migrants, 21.1% had potential mental health problems and 63.1% had an unhealthy lifestyle. The three most prevalent mental disorders were obsessions-compulsions (O-C; 13.7%; 751/5484), interpersonal sensitivity (I-S; 11.0%; 603/5484), and hostility (HOS; 10.8%; 590/5484). Compared with the male participants, the female participants exhibited significantly increased mean scores for phobic anxiety (PHOB) and anxiety (ANX) (p < 0.001). Logistic regression indicated that after adjustment for potential confounding factors in both genders, an unhealthy lifestyle score was significantly associated with all nine subscales of the SCL-90-R. The male participants with psychoticism [PSY; odds ratio (OR) = 4.908, 95% confidence interval (CI) 2.474-9.735], ANX (OR = 4.022, 95% CI 2.151-7.518), or depression (DEP; OR = 3.378, 95% CI 2.079-5.487) were the most likely to have an unhealthy lifestyle. In the female participants, an unhealthy lifestyle was most associated with HOS (OR = 2.868, 95% CI 2.155-3.819), PSY (OR = 2.783, 95% CI 1.870-4.141), or DEP (OR = 2.650, 95% CI 1.960-3.582).Conclusion: Lifestyle behaviors were significantly associated with mental health in rural-to-urban migrant workers, and these findings indicate the need to develop targeted psychological interventions to foster healthy lifestyles in migrants. Background: Rapid urbanization of China has resulted in significant domestic migration. The purpose of the present study was to survey the sexual behavior of migrant workers in Shanghai and determine the risk factors for unprotected sex.Methods: A cross-sectional study of the sexual behavior of 5996 migrant workers was conducted in 7 administrative regions of Shanghai in 2012 from August to October. A self-administered questionnaire was used to collect data.Results: Five thousand seven hundred seventy two out of the 5996 migrants enrolled into the present study were primarily young adults aged 34.3 ± 10.6 years. Of them, 73.5 % were married, 51.1 % graduated from junior high school, 46.0 % earned 1500-2500 yuan (RMB) monthly. The majority (82.3 %) of the migrants engaged in sexual behavior, and 58.0 % did not use condoms in sexual intercourse. Some of the participants (15.2 %) had casual extramarital partners within the previous 12 months; among them, 76.2 % never or only occasionally used condoms. The results of the multivariate logistic regression analysis suggested that condom use was associated with age, occupation, monthly income, education, and housing conditions. Having temporary sexual partners was significantly associated with several factors such as unmarried (OR: 0.47, 95 % CI: 0.38-0.57), working at domestic (OR: 1.65,95 % CI: 1.17-2.34), working at wholesale/retail(OR: 1.65, 95 % CI: 1.13-2.13), and male migrants (OR: 2.37, 95 % CI: 1.96-2.85), but not with other factors such as age, monthly income, or education. Having casual extramarital partners was significantly associated with female migrants working at domestic (OR: 1.89, 95 % CI: 1.09-3.28), unmarried male migrants (OR: 0.51, 95 % CI: 0.36-0.74).Conclusion: Closer attention should be paid to sexual health education among migrant workers, especially women and those working in domestic and wholesale/retail occupations. Background: Lifestyle behaviors significantly impact health, yet remain poorly defined in Chinese rural-to-urban migrants. Methods: In a cross-sectional study of health-related behaviors of 5484 rural-to-urban migrants who had worked in Shanghai for at least six months, we assessed the contribution of demographics and physical and mental health to lifestyle behaviors in male and female participants by multiple stepwise cumulative odds logistic regression. Results: Respondents were 51.3% male. 9.9% exhibited abnormal blood pressure; 27.0% were overweight or obese; 11.2% reported abnormal mental health; 36.9% reported healthy lifestyle. Multiple stepwise cumulative odds logistic regression indicated that men working in manufacturing reported less unhealthy lifestyle than those in hospitality (cumulative odds ratio (COR) = 1.806, 95%CI 1.275–2.559) or recreation/leisure (COR = 3.248, 95%CI 2.379–4.435); and women working in manufacturing and construction reported less unhealthy lifestyle than those in all other sectors. Unhealthy lifestyle was associated with small workplaces for men (COR = 1.422, 95%CI 1.154–1.752), working more than 8 or 11 hours per day for women and men, respectively, and earning over 3500 RMB in women (COR = 1.618, 95%CI 1.137–2.303). Single women and women who had previously resided in three or more cities were more likely to report unhealthy lifestyle (COR = 2.023, 95%CI 1.664–2.461, and COR = 1.311, 95%CI 1.072–1.602, respectively). Abnormal mental status was also correlated with unhealthy lifestyle in men (COR = 3.105, 95%CI 2.454–3.930) and women (COR = 2.566, 95%CI 2.024–3.252). Conclusions: There were different risk factors of unhealthy lifestyle score in male and female rural-to-urban migrants, especially in number of cities experienced, salary, marital status, work place scale. Several demographic groups: employment sectors (e.g. hospitality and recreation/leisure), working conditions (e.g. long hours) and abnormal mental status were associated with unhealthy lifestyle behaviors in Chinese rural-to-urban migrants, and health interventions should be targeted to these groups. Vertical and differential deformations in super-high-rise buildings are extremely important during construction. In order to be able to adjust the construction process accurately, taking the deformations into account, such deformations must be calculated exactly. The analysis of the vertical deformations of the high-rise Shanghai World Financial Centre building is illustrated in this paper. The prediction models of CEB-FIP 90 are used for calculating shrinkage and creep. The fictitious degree of hydration method has been applied for calculating the creep behaviour of the early-age concrete. The influence of steel bars on the vertical deformations has been analysed based on the macroscopical modulus method. Calculation of the vertical deformations includes the pre-construction and the post-construction deformations. Finally, the differential deformation between the concrete and the mega-structure column is determined. Safety performance functions (SPFs) are indispensable analytical tools that usually play a crucial role in estimating crash frequencies, identifying hotspots, analyzing crash contributing factors, and assessing the effectiveness of safety countermeasures. Due to the limited availability of safety data, municipalities tended to adopt SPFs from Highway Safety Manual or other neighboring jurisdictions. Recently, boosting algorithms have been frequently exploited for data analysis and statistical regression modeling problems. This research, therefore, aims to examine the efficiency of boosting calibration techniques to transfer the SPF using the limited region data in an international context. To this end, AdaBoost.R2, an adaptive boosting algorithm, Two-stage TrAdaBoost.R2, an instance-based transfer learning algorithm, and Gradient Boosting algorithm were employed to investigate their efficiencies in acquiring knowledge from the available source domain data to predict crashes in the target domain. As a comparison, the calibration factor method was adopted to transfer the traditional negative binomial (NB) regression model. Two training dataset groups were developed to train the four calibration techniques. The first group was used to examine the adaptability of the employed calibration techniques to the limited target region data. While the second group was utilized to further investigate the influence of larger vital information on the performance of transferred models. This study was conducted between two U.S. states, Florida and New York, and two Chinese cities, Shanghai and Suzhou. According to the goodness-of-fit results, boosting calibration techniques showed better prediction accuracy than the calibrated NB-based model using the limited target region data. In addition, the amount and distribution of the training dataset were considered the two significant factors that influence the proficiency of the boosting calibration techniques.
