In Romania, systematic information on the occurrence of oral diseases in children is scarce. The purpose of the present study was to describe the prevalence and the pattern of dental caries in schoolchildren, and to USC the data to provide a baseline for planning and evaluation of oral health care. A national sample of children at grade 1 (n = 729) and grade 6 (n = 660) was chosen consistent with the WHO pathfinder principle. Clinical examinations were carried out according to the recording system for the Danish Municipal Dental Service and the following results were obtained. In children of grade 1 (7-yr-olds) the prevalence proportion of caries in primary teeth was 86% and in permanent teeth 39%. The mean caries indices were 11.4 defs and 1.3 DMFS. At grade 6 (12-yr-olds) the prevalence proportion of caries in primary teeth was 17% and in permanent teeth 90%. The mean caries experience was 0.8 defs and 6.5 DMFS, and a mean of 4.1 DMFT was observed. In both groups, the D-component of the caries index was dominant. The children were also classified by caries severity zone. At grade 1, 61% had a very severe pattern of caries in the primary teeth, i.e. caries in pits/fissures, proximal surfaces, smooth surfaces, and incisors. Forty-three percent of the children at grade 6 showed this pattern in permanent teeth. In Romanian schoolchildren the present level and pattern of dental caries are most severe and the implementation of oral health promotion and prevention at the community level is urgently needed. Secular trends in anthropometric parameters have been documented in most European countries, but no data is available regarding Romanian. The aim of the study was to calculate secular trend in height, body mass and mean menarche age for Romanian children and adolescents., Methods: A secondary data analysis was performed using ten data sets for urban and eight data sets for rural boys and girls, age 5-15 years, covering 80 years (1936 to 2016). Secular trend in height (cm/decade), body mass(kg/decade) and mean menarche age (years) were calculated., Results: Overall, there was a positive secular trend for height in both genders, which parallels the gross domestic product (GDP)/capita difference, more pronounced in boys, across all age-groups, with a maximum for 15 years-old boys (~3 cm/decade) and 13 years-old girls (~2 cm/decade). Body mass trend was also positive, more accentuated in the rural population. Mean age at menarche was higher in rural compared to urban girls, had a negative trend with the disappearance of the difference in the latest available data set (2013)., Conclusion: In summary, an overall positive and ongoing secular trend in height and body mass was documented in Romanian children and adolescents, especially for the pubertal age-range, in concordance to other western countries, but out of phase by approximately 20 years.

Purpose: Adiponectin may be beneficial in incipient chronic kidney disease by antagonizing oxidative stress. We evaluated adiponectin, malondialdehyde (MDA), and superoxide dismutase (SOD), in type 2 diabetes mellitus patients (T2DP) with and without incipient nephropathy., Methods: T2DP with glomerular filtration rate (GFR) >30 ml/min were compared with 20 healthy controls. Clinical and laboratory evaluations, levels of MDA (fluorimetric thiobarbituric test), SOD (cytochrome reduction method) and adiponectin (ELISA) were obtained., Results: Sixty-four patients (GFR 91.44 ± 38.50 ml/min, urinary albumin-to-creatinine ratio [UACR] 20.81 [4.64-72.88 mg/g]) were included. MDA was higher in T2DP than in controls: 3.97 (2.43-4.59) versus 1.35 (1.16-1.81) nmol/ml, p < 0.0001. MDA correlated with glycated hemoglobin (r = 0.40, p = 0.001), adiponectin (r = -0.28, p = 0.03), systolic blood pressure (r = -0.28, p = 0.03) and SOD (r = -0.35, p = 0.005); adiponectin (p = 0.01) and glycated hemoglobin (p = 0.02) remained significant predictors of MDA in multiple regression analysis. SOD was negatively correlated with glycemia (r = -0.71, p < 0.0001) and glycated hemoglobin (r = -0.5, p < 0.0001). When patients were divided according to a ROC-derived adiponectin cutoff of 8.9 µg/ml, patients with higher adiponectin had lower MDA, [2.55 (2.35-3.60) vs. 4.10 (2.89-5.31) nmol/ml, p = 0.005] but similar SOD levels. In T2DP with nephropathy (GFR < 60 ml/min or UACR > 30 mg/g), the correlation of adiponectin with MDA was stronger, (r = -0.51, p = 0.004) confirmed in multiple regression analysis (p = 0.03). Adiponectin was not correlated with MDA, and SOD was inversely related to MDA in patients without nephropathy., Conclusion: Adiponectin is a significant predictor of MDA in early diabetic nephropathy, whereas SOD strongly depends only on glycemic control and is not directly related to adiponectin.

Glaucoma is a degenerative optic neuropathy progressive, multifactorial, which can lead to blindness. Blindness in patients with glaucoma is defined as visual field reduction below 10 degrees. Artificial drainage systems are a solution for refractory to medication, laser treatment or conventional surgery. Used by over 100 years, improved with good surgical technique and careful patient follow-up surgery, postoperative results are satisfactory.

Smoking prevalence is decreasing in developed countries in the last decades. Nevertheless, there is a constant increase of morbidity and mortality of smoking-related diseases (COPD, asthma, respiratory infections, cardiac diseases, cancer). Then why do doctors smoke? Because they experimented in adolescence and college and the addiction developed fast, while information about smoking risks arrived too late. Five hundred questionnaires were distributed to 100 doctors and 400 students between March-April 2009; 50% were returned from the doctors and 90.5% from the students. We analyzed comparative the smoking status (60% non-smoking doctors and 56.9% non-smoking students. Mean age was 25.05 for doctors and 24.2 for smoking students. Mean starting age was 17.13 years, 17.78 respectively. They were asked about major components of cigarettes, main diseases induced by smoking, the effects of nicotine addiction, known laws and projects, the need for introducing in the university curricula of anti-smoking modules. 20% of the doctors and 14.5% of the students had a smoking-related disease. 52% of doctors and 57% of students supported forbidding smokingin public spaces, 13.7% of doctors and 88.9% of students supported the increase of cigarettes price, and only 48% of doctors and 47% of students suggested the need of help for smoking cessation. We noticed there are flaws in knowledge of extra pulmonary diseases in students as well as doctors. Most respondents had lacking, non-systematic information and 96% of doctors and only 69.6% of students consider useful tabaccology lectures. For stopping epidemic morbidity and mortality due to smoking related diseases, there is need for involving all those working in health programmes. For this, medical schools should do better in preparing specialists. We also consider it is time that all medical specialties should consider smoking cessation a priority, while the pneumologists should sustain a systematic and routine activity against smoking.

Infectious diarrhoea is a syndrome caused by a variety of bacterial, viral and parasitic organisms which represents a major cause of morbidity and mortality all over the world. The wide diversity of etiological agents impairs the surveillance and the diagnosis and affects the correct treatment applied to reduce the long-term complications. Besides well known enteric pathogens such as Salmonella, Shigella and Yersinia, a high number of emergent and re-emergent aetiologies are now recognised to be at the origin of diarrhoea. The lack of a correct diagnostic algorithm and adequate methods of analyses leads to under-evaluation and incertitude in an important number of clinical cases. Our study was designed as a complex analysis of the stool specimens collected from the patients, in the purpose to improve the laboratory diagnostic and to enhance the number of confirmed cases of infectious diarrhoea. A number of 756 samples from inpatients with diarrhoea were tested targeting pathogenic and opportunistic bacteria, viruses and parasites by classical and molecular methods. We documented that, in case of non-Salmonella, non-Shigella, non-Yersinia diarrhoea, the quality of diagnostic was improved by increasing the percentage of positive specimens to 22.49% compared to 11.12% when only bacteria, 5.56% when only viruses and 4.10% when only parasites were investigated. The laboratory data are of great value in evaluating the diarrhoea syndrome offering the documentation for an accurate epidemiological response and an adequate treatment.

Purpose: The main objective of this study was to identify the prevalence of ocular manifestations in rheumatic patients admitted in a specialized clinic., Methods: Information regarding rheumatic and ocular diseases was extracted from medical records system available in "Dr. I. Cantacuzino" Clinical Hospital from Bucharest. The prevalence of ocular involvement reported passively by rheumatologists (retrospective descriptive study of 375 different cases of rheumatic patients) was compared with the literature data., Results: There were 45 cases of ocular manifestations. Keratoconjunctivitis sicca was noted in 16 patients with rheumatoid arthritis, two patients with systemic lupus erythematosus and one patient with scleroderma. Anterior uveitis was found in seven patients with ankylosing spondylitis, one patient with reactive arthritis, two patients with psoriatic arthritis and one patient with LES. Conjunctivitis was present in two patients with reactive arthritis. In LES ocular involvement also included four cases of retinal vasculitis. Complications clearly related to steroid therapy were nine cases of cataracts. One case with typical "bull's eye" maculopathy due to Hydroxychloroquine treatment was detected., Conclusions: The main conclusion of our study is that the rheumatic patients need to be referred to an ophthalmologist for the diagnosis and the optimal treatment of ocular involvement.

Osteosarcomas are tumors with a high aggressivity which gives early and frequent pulmonary metastasis. These patients have a poor prognosis. The reveal of prognosis factors and early detection of pulmonary relapse would influence the outcome of these patients. We performed a retrospective study at Oncology Institute from Cluj, observing 35 patients with osteosarcomas and pulmonary metastasis at the onset or in evolution. We tried to quantify some prognosis factors. We observed the predominance of male sex (20), from urban area (25), with age between 11-20 years (16 cases), with primary tumors localized more frequently at the limbs. Pulmonary metastasis were shown without free interval from the onset (12 patients), or in the first years (17 cases), more frequently with multiples locations (26 cases). 28 patients were submitted to surgical treatment for cure of primary tumor, 18 chemotherapy, and pulmonary metastasis resection in 5 cases only. The outcome under treatment was favorable in 4 cases only, compared with other studies. Early detection of pulmonary metastasis and aggressive therapy with augmentation of resection criteria for pulmonary metastasis would improve the prognosis of this patients.
