Urinary tract infections are among the most prevalent types of healthcare-associated infections (HAIs) in hospitals and nursing homes, and they are primarily a result of unnecessary catheter usage and inadequate care. In Portugal, epidemiological data indicate that catheter-associated urinary tract infections (CAUTIs) remain widespread in clinical settings, resulting in increased morbidity and mortality rates among vulnerable populations. This study aimed to assess urinary catheter use in an oncology ward in Portugal and to evaluate nurses' adherence to the government-endorsed standards for preventing CAUTIs. An observational study was conducted over a four-month period with daily assessments of nurses' practices during urinary catheter insertion and maintenance using a government-endorsed auditing tool. Data were collected through on-site observations and nurses' feedback. The findings revealed a urinary catheter utilization rate of 17.99%. However, there was a lack of complete adherence to government-endorsed standards among oncology nurses (0%). These results indicate that current practices lack evidence-based standardization. Predawn leaf water potential (Ψpd) is the main parameter to determine plant water status, and it has been broadly used to support irrigation management. However, the Scholander pressure chamber methodology is laborious, time-consuming and invasive. This study examined a low-cost hyperspectral proximal sensor to estimate the Ψpd in grapevine (Vitis vinifera L.). For this, both the Ψpd and spectral reflectance (340–850 nm) were accessed in grapevines in a commercial vineyard located in the Douro Wine Region, northeast Portugal. A machine-learning algorithm was tested and validated to assess grapevine's water status. The experiment was performed in a randomized design with 12 grapevines (cv. Touriga Nacional) per irrigation treatment: non-irrigated, 30% crop evapotranspiration (Etc), and 60% Etc. The dataset was analyzed using Principal Component Analysis (PCA), and the machine-learning regression algorithm applied was Extreme Gradient Boosting (Xgboost). Results from the validation dataset (n = 108) for the Xgboost tested exhibited a root mean square error (RMSE) of 0.23 MPa, a mean absolute error (MAPE) of 16.57% and an R² value of 0.95. These results demonstrate that the hyperspectral sensor and Xgboost algorithm show potential for predicting the Ψpd in vineyards, regardless of a plant's water status. [ABSTRACT FROM AUTHOR]

Sustainable and efficient agricultural production is a growing priority in modern society. Viticulture, an important agricultural and food sector, also faces this challenge. Precision Viticulture (PV) has gained prominence as it aims to foster high-quality, efficient, and environmentally sustainable practices. The Soluble Solids Content (SSC) is essential for assessing grape ripeness and quality in the winemaking process. Conventional methods for determining SSC values (expressed in °Brix) are invasive, expensive, and labour-intensive, necessitating sample preparation, making large-scale analysis impractical. In response to these limitations, this study presents an innovative approach within the field of Precision Viticulture. It focuses on the non-invasive prediction of SSC using low-cost proximal hyperspectral optical sensors. These sensors rely on spectral reflectance measurements in the range of 340–850 nm. This study was conducted in a commercial vineyard in the Demarcated Douro Region, Cima-Corgo sub-region, Portugal, over six weeks during ripening. In total, 169 grape berries from Touriga Nacional vines were analysed under three irrigation regimes (no irrigation, 30% ETc, and 60% ETc). After organising and preprocessing the data, machine learning algorithms, namely Partial Least Squares Regression (PLS), Random Forest (RF), and the Generalised Linear Model (GLM), were applied to predict SSC values. These models' performance was thoroughly evaluated using cross-validation techniques. The performance of different models was evaluated, showing significant differences according to the metrics used (R2, RMSE, and MAPE). The RF model demonstrated effectiveness and precision. A high R² value of 0.9312, coupled with low RMSE (0.9199 °Brix) and MAPE (3.88%), signifies a strong fit to the data and accurate predictive capabilities. The results of this benchmarking study on predictive models of SSC provide valuable insights into the performance of various models, aiding winegrowers and winemakers in decision making. In order to achieve a more efficient allocation of healthcare resources in the near future, it is crucial to understand the patterns and causes of excess mortality and hospitalizations. Neonatal mortality still poses a significant challenge, particularly in developed nations where the mortality rates are already low and healthcare resources are generally available to most of the population. Furthermore, the low mortality rates mean that the data available for modeling are often very limited, restricting the modeling methods that can be used. It is also important that the chosen methods allow for explainable, non-black-box models that can be interpreted by healthcare professionals. Considering these challenges, the work hereby presented thoroughly analyzed the time series of the neonatal mortality rates in Portugal between 2014 and 2019 in terms of trend and seasonal patterns. The applicability and performance of different data-based methods were also analyzed. Furthermore, the mortality rates were studied in terms of their relation to environmental variables, such as temperature and air pollution indicators, with the goal of establishing causal relations between such variables and excess mortality. The preliminary results show that ARMA, neural and fuzzzy models are able to forecast the studied mortality rates with good performance. In particular, neural models have the best predictive performance, while fuzzy models are well suited to obtain interpretable models with acceptable performance. [ABSTRACT FROM AUTHOR]

The article presents a study which identified clusters of anti-Ku-positive patients according to their spectrum of antinuclear antibodies (ANA) specificities and analyzed their clinical and analytical features. Contrary to many international settings, there are no clinical guidelines for peripheral intravenous catheter (PIVC) insertion and maintenance in Portugal. We sought to derive an international consensus on a PIVC bundle that could guide Portuguese nurses' clinical decision-making in this scope. Methods: Two international vascular access specialist groups participated in an online Delphi panel. During the first round, specialists (n = 7) were sent a summary report from a previous observational study conducted in a surgical ward in Portugal. Based on the report findings, specialists were asked to provide five to eight PIVC insertion and maintenance interventions. Then, another set of specialists (n = 7) scored and revised the recommendations until a consensus was reached (≥70% agreement). The PIVC bundle was made available and discussed with the surgical ward's nurses. Results: After three rounds, a consensus was achieved for five evidence-informed interventions: (i) involve the person and assess the peripheral venous network; (ii) maintain an aseptic no-touch technique; (iii) ensure proper catheter dressing and fixation; (iv) perform catheter flush & lock; (v) test the peripheral venous catheter's functionality and performance at each shift. Conclusion: The final version of the PIVC bundle achieved consensus among international experts. Despite the positive feedback provided by the ward nurses, future studies are warranted to assess its effectiveness in standardizing PIVC care delivery and its potential implications for care outcomes in Portuguese clinical settings. Pseudomonas syringae pv. actinidiae (Psa) has been responsible for numerous epidemics of bacterial canker of kiwi (BCK), resulting in high losses in kiwi production worldwide. Current diagnostic approaches for this disease usually depend on visible signs of the infection (disease symptoms) to be present. Since these symptoms frequently manifest themselves in the middle to late stages of the infection process, the effectiveness of phytosanitary measures can be compromised. Hyperspectral spectroscopy has the potential to be an effective, non-invasive, rapid, cost-effective, high-throughput approach for improving BCK diagnostics. This study aimed to investigate the potential of hyperspectral UV–VIS reflectance for in-situ, non-destructive discrimination of bacterial canker on kiwi leaves. Spectral reflectance (325–1075 nm) of twenty plants were obtained with a handheld spectroradiometer in two commercial kiwi orchards located in Portugal, for 15 weeks, totaling 504 spectral measurements. Several modeling approaches based on continuous hyperspectral data or specific wavelengths, chosen by different feature selection algorithms, were tested to discriminate BCK on leaves. Spectral separability of asymptomatic and symptomatic leaves was observed in all multi-variate and machine learning models, including the FDA, GLM, PLS, and SVM methods. The combination of a stepwise forward variable selection approach using a support vector machine algorithm with a radial kernel and class weights was selected as the final model. Its overall accuracy was 85%, with a 0.70 kappa score and 0.84 F-measure. These results were coherent with leaves classified as asymptomatic or symptomatic by visual inspection. Irrigation is essential for a large part of Mediterranean agricultural systems, but scarce resources may cause conflicts between agricultural and domestic uses. These conflicts might be exacerbated by climate change, which could bring a drier climate and thus increase irrigation water demands while lowering supplies. These issues were addressed when designing a climate change adaptation plan for water resources in the Algarve region (southern Portugal), which was co-created between hydrologists and local stakeholders and policy-makers, by using the Dynamic Adaptive Policy Pathways (DAPP) approach to synthetize and communicate the results from hydrological modelling of future scenarios. The evolution of water availability and irrigation demands for key water assets in Algarve (southern Portugal) were simulated until 2100 for climate scenarios RCP4.5 and RCP8.5, using a modified version of Thornthwaite-Mather. The results show an increase in water stress, mainly in the RCP8.5 scenario. The results and need for adaptation were discussed with local and regional decision-makers and other stakeholders, and a set of adaptation measures was agreed upon. The discussed adaptation measures were then modelled and integrated the design of tailor-made DAPP. Finally, decision-makers and stakeholders were presented with DAPP and selected the most suitable and political reliable adaptation pathway that tackles projected climate change impacts in water resources until the end of the 21 st century. Stakeholders showed a strong preference for incremental and distributed small-scale measures, including the promotion of water use efficiency and landscape water retention, to large-scale measures such as wastewater recycling or new dams. A decrease in irrigation water use for agriculture was not considered socially desirable. Desalination was considered too costly for irrigation in the short term but kept in reserve in case other measures fail to keep water supplies at an acceptable level. [ABSTRACT FROM AUTHOR]

The paper addresses the seismic vulnerability assessment of a multi-span footbridge, prone to span unseating due to shorter seat lengths. The structure is representative of a series of pedestrian crossings located in the Southern part of Portugal, a region with a relevant seismicity. A probabilistic approach allows considering the variability of the seismic action and uncertainties in the definition of the material properties and/or structural behavior. Based on incremental dynamic analyses and corresponding fragility curves, it is shown that, for code compliance design acceleration, there is a significant probability that the structure will only suffer minor damage. Coastline retreat is an evident sign of the vulnerability of low-lying sandy coasts to erosion and sea level rise (SLR). Sand nourishments are commonly used to maintain foreshores, beaches, and dunes to preserve their role in flood protection, beach recreation and biodiversity. The feasibility of sand nourishments for long-term maintenance of large-scale coastal systems is hard to determine with detailed numerical morphological models. This study presents a method using a sand nourishment impact model (Ntool) that accounts for repeated sand nourishments over time as well as erosion and SLR impacts on the shoreline position. In Ntool the redistribution of sediment alongshore is based on detailed simulations of the current situation of waves (Delft3D-WAVE) and shoreline development (UNIBEST-CL+). Two sand nourishment strategies for the period 2010–2100 were evaluated and compared for the ca. 100 km long sandy shores of the Dutch coast (the Netherlands) and the Aveiro coast (Portugal): i) the hold-the-line strategy, in which coastline retreat is directly restored by means of local sand nourishments and ii) the sand balance strategy, in which sand nourishments are applied each five years at high-value locations with volumes equal to the sand deficiencies due to sea level rise plus other known structural sediment losses in a particular sediment cell. Regionalised SLR projections of up to 0.5, 0.7 and 1.9 m by 2100 were taken into account. Results show that sand nourishments remain a cost-effective method of flood protection for the Dutch coast and are feasible for the centuries to come, even under the high-end SLR scenario, because large sand resources are available in the North Sea. For the Aveiro coast, large-scale sand nourishments might be feasible in combination with a policy of managed retreat for low-value areas to avoid expensive locked-in situations. The use of DNA technologies for criminal investigation purposes illuminates an interplay of knowledge and expertise where meaning and relevance of biological traces are negotiated. Through the analysis of five criminal cases that took place in Portugal between 1995 and 2010, and where DNA technologies were used, this article will focus on the dialogues established between the police and the forensic laboratories. I will argue that, on the one hand, the police investigators' uses of DNA technologies seek to legitimate and provide an external source of neutrality and objectivity to the constructed narratives surrounding the commission of a crime. On the other hand, laboratories and forensic experts engage in the delimitation and preservation of their professional autonomy by developing boundary work around their scientific expertise through the translation and conversion of criminal traces into scientific artifacts. This article offers a review of research and policy on climate change in Portugal and is organized into three main themes: scientific knowledge and assessment of climate change; policy analysis and evaluation; and public engagement. Modern scientific research on meteorology and climatology started in Portugal in the 1950s and a strong community of researchers in climate science, vulnerabilities, impacts, and adaptation has since developed, particularly in the last decade. Nevertheless, there are still many gaps in research, especially regarding the economic costs of climate change in Portugal and costs and benefits of adaptation. Governmental policies with a strong emphasis on mitigation were introduced at the end of the 1990s. As greenhouse gas emissions continued to rise beyond its Kyoto target for 2012, the country had to resort to the Kyoto Flexibility Mechanisms in order to comply. Climate change adaptation policies were introduced in 2010 but are far from being fully implemented. Regarding public engagement with climate change, high levels of concern contrast with limited understanding and rather weak behavioral dispositions to address climate change. Citizens display a heavy reliance on the media as sources of information, which are dominated by a techno-managerial discourse mainly focused on the global level. The final part of the article identifies research gaps and outlines a research agenda. Connections between policy and research are also discussed. A significant number of adult oncology patients require at least one peripheral intravenous catheter to fulfill their therapeutic plan. Recent evidence indicates that catheter failure rates are high in this cohort, impacting care outcomes and patient experience during cancer treatment. This reality represents a challenge to nurses worldwide since in most international settings they are responsible for delivering quality care during the insertion and maintenance of such devices. This study aims to explore current nursing practices regarding the insertion, maintenance, and surveillance of peripheral intravenous catheters in oncology patients. A two-phase mix-method study was conducted with the nursing team from the surgical ward of a large oncology hospital in Portugal. In phase one (observational prospective study), nurses' practices during catheter insertion and maintenance were observed by the research team and recorded using standardized instruments and validated scales. In phase two, three online focus groups were conducted with the nursing team to present the results observed in phase one and explore their perceptions of current practices. All ethical principles were assured throughout the study. Significant divergent practices were observed and identified by the nurses, especially concerning patient involvement, nurses' adherence to the aseptic, non-touch technique, catheter stabilization and dressing, and catheter flushing and locking. Impact of sea-surface warming upon the fish assemblages of the Portuguese coast was assessed under two scenarios, the A2 and the B2 scenarios (Special Report on Emission Scenarios), which when coupled with a regional circulation model HadRM3 predict 1 and 2°C of sea-surface warming until the year 2100. Species richness increased in the Portuguese coast. In both scenarios, there was a latitudinal gradient in the amount of lost and gained species, increasing from north to south. An anomaly in the latitudinal gradient of species richness was detected in the southernmost area, which presented the lowest species richness of all areas, in the A2 scenario, and appears to be particularly vulnerable to climate change. Very few species were totally eliminated from Portuguese waters. For the Portuguese coast as a whole, there were more new than lost species, in both scenarios. Most of the new species were commercially important species, in the majority subtropical in the north and tropical in the south, mainly demersal and reef-associated. Reef-associated species also increased in relative importance. A commercial opportunity for fisheries may arise from climate warming, since most of the new species were commercial species and not many commercial species were lost. An increasing gradient from north to south was detected in the colonization of new species of herbivores, planktivores, and omnivores, as well as lower mean trophic level, which might have consequences for the future of trophic webs. Two cases of missing children in Portugal (Joana and Maddie) have recently highlighted the dilemmas and contingencies associated with the technology of “genetic fingerprinting” for forensic purposes in the context of criminal investigations. The purpose of this article is to analyze the popular press’s discourses and representations around forensic genetics in the context of those two highly mediatized criminal investigation cases. The symbolical construction and representation of forensic genetics by the media presents a form of public exposure to beliefs on forensic genetics’ characteristics and potential. These are blended with popular cultural contexts that are constructed with reference to images of a super-science which may carry consequences in the public understanding of forensic science. The media coverage of both cases and their actual disclosure resembles the patterns of a CSI effect, insofar as real science’s capabilities and limitations are placed against fictionalized representations of forensic science. We present the results concerning the archaeological work carried on on Monument 2, in Chão Redondo (Sever do Vouga, Aveiro, Portugal), by the responsibility of ARQUEOHOJE company. The foregoing mentioned dolmen, presents quite conventional (classical) characteristics, with chamber and hall way different in layout and hoist. Engravings can be seen on some of their orthostats. This recent intervention is quite important, firstly because it was possible to define, in a satisfactory way, the whole set of processes and techniques used for the erecting and subsequent sealing off of this tomb. Secondly, a frontier-limit of the monument was settled, with areas for the rituals and accesses to its interior, imbued with a symbolic meaning rather than a functional one. Abstract: Larvae of various coastal fish undergo immigration from spawning grounds towards estuarine nurseries. Several environmental factors can have an important impact on survival at this stage making it crucial for recruitment. Generalized linear models were applied in order to investigate the relation between river drainage, sea surface temperature (SST), NAO index and the North-South wind component intensity, over the two months prior to the estuarine colonization peak, and the densities of Platichthys flesus, Dicentrarchus labrax, Diplodus vulgaris and Diplodus bellottii in the nursery grounds based on a discontinuous historical dataset (from 1978 to 2006), for the Tagus estuary. The relation between SST over the 12 months prior to the estuarine colonization peak and fish densities in the nurseries was also investigated, as it integrates the periods of spawning stock maturation, spawning and larval immigration. While SST over the prior 12 months was negatively correlated with the abundance of P. flesus, it was positively correlated with the abundance of D. bellottii. Abundance of D. vulgaris was positively correlated with SST in the two months prior to the estuarine colonization peak, while the abundance of D. labrax was positively correlated with river drainage. The relations between SST and the abundance of P. flesus, a cold-water species with declining densities, and the subtropical species D. bellottii and D. vulgaris, which are increasing in abundance, are indicative of species abundance alterations related to climate warming. Dicentrarchus labrax will probably also be affected by climate change because of lowered precipitation and consequently river drainage. [Copyright &y& Elsevier]

Abstract: This paper analyses the impact of the 2003 European heatwave on excessive human mortality in Portugal, a country that presents a relatively high level of exposure to heatwave events. A total of 2399 excessive deaths are estimated in continental Portugal, which implies an increase of 58% over the expected deaths. When these values are split by gender, it is seen that women increase (79%), was considerably higher than that recorded for men (41%). The increment of mortality due to this heatwave was detected for all the 18 districts of the country, but its magnitude was significantly higher in the inner districts close to the Spanish border. When we split by gender all districts reveal significant mortality increments for women, while the impact in men''s excess deaths is not significant over 3 districts. Several temperature derived indices were used and evaluated in their capacity to explain, at the regional level, the excessive mortality (ratio between observed and expected deaths) by gender. It is shown that the best relationship is found for the total exceedance of extreme days, an index combining the length of the heatwave and its intensity. Both variables hold a linear relationship with r =0.79 for women and a poorer adjustment (r =0.50) for men. Additionally, availability of mortality data split by age also allowed obtaining detailed information on the structure of the population in risk, namely by showing that statistically significant increments are concentrated in the last three age classes (45–64, 65–74 and 75 or more). The use of air conditioning systems in some Portuguese hospitals had a major impact on the decrease (up to 40%) in excessive mortality values. A finer approach is relevant for prevention strategies, since it allows to identify better the target population of any preventive strategy regional and national authorities may be interested to implement. The extraordinary media coverage regarding the disappearance of the British 3-year-old Madeleine McCann emerges as an illustrative example of a 'public drama' and 'trial by media'. This article presents a comparative analysis of the perspectives and narrative devices employed by two Portuguese newspapers in establishing a dialogue with their respective audiences. High-profile mediatized criminal cases have the potential to linger in the public memory and become cultural references which may affect longterm public representations of crime and justice. Our analysis is limited to a sample of representative Portuguese newspapers. We found a basic distinction between 'quality' and 'popular' press which may be related to inherent differences of their market and implicit audiences. A distanced, neutral and reflexive style of the quality press contrasts with the construction of a sensationalistic narrative by the popular press. The latter provided the audience with a daily dose of vicarious participation in a criminal drama which developed into a trial by media, sustained by a rhetoric that encourages the audience to 'take sides'. Sensationalist media narratives can potentially undermine the principles of fair trial and the presumption of innocence. But they can also elicit relevant collective energies directed at starting processes of change. [ABSTRACT FROM AUTHOR]

In this study we investigated the potential impact of climate change in Portugal on heat-related mortality, air pollution-related health effects, and selected vectorborne diseases. The assessment used climate scenarios from two regional climate models for a range of future time periods. The annual heat-related death rates in Lisbon may increase from between 5.4 and 6 per 100,000 in 1980-1998 to between 8.5 and 12.1 by the 2020s and to a maximum of 29.5 by the 2050s, if no adaptations occur. The projected warmer and more variable weather may result in better dispersion of nitrogen dioxide levels in winter, whereas the higher temperatures may reduce air quality during the warmer months by increasing tropospheric ozone levels. We estimated the future risk of zoonoses using ecologic scenarios to describe future changes in vectors and parasites. Malaria and schistosomiasis, which are currently not endemic in Portugal, are more sensitive to the introduction of infected vectors than to temperature changes. Higher temperatures may increase the transmission risk of zoonoses that are currently endemic to Portugal, such as leishmaniasis, Lyme disease, and Mediterranean spotted fever. Adaptation Pathways is a decision support tool designed to create adaptation policies under different climate change scenarios. This tool has been used successfully in several sectors and contexts such as coastal and river adaptation, urban heat waves, floods and rural livelihoods but its use in natural resource management, has faced several challenges and limitations. In the sector of agroforestry its use has seldom been done or documented and one of the reasons for this may due to some of its specific challenges. In this study, these challenges were addressed when using the Adaptation Pathways for the adaptation planning of three case studies in the semi-arid Alentejo region, a Mediterranean dryland of southern Portugal. This tool was integrated in a participatory approach combined with the Scenario Workshop method, to plan the adaptation of the agriculture and forestry sector of one municipality (Mértola) and two agroforestry farms (221 ha and 1000 ha). The methodology included, for each case study, 20 interviews, two workshops, literature review, expert analysis and the use of indicators of efficacy of adaptation measures, to define tipping points. The adaptation process and the resulting adaptation plans were evaluated by questionnaire and expert review. This combination of methods has supported the choice of effective adaptation measures for the case studies and when combined with several adaptation pathways and a landscape approach it supported the creation of integrated climate change adaptation plans that are now in implementation. Smart and precision agriculture concepts require that the farmer measures all relevant variables in a continuous way and processes this information in order to build better prescription maps and to predict crop yield. These maps feed machinery with variable rate technology to apply the correct amount of products in the right time and place, to improve farm profitability. One of the most relevant information to estimate the farm yield is the Leaf Area Index. Traditionally, this index can be obtained from manual measurements or from aerial imagery: the former is time consuming and the latter requires the use of drones or aerial services. This work presents an optical sensing-based hardware module that can be attached to existing autonomous or guided terrestrial vehicles. During the normal operation, the module collects periodic geo-referenced monocular images and laser data. With that data a suggested processing pipeline, based on open-source software and composed by Structure from Motion, Multi-View Stereo and point cloud registration stages, can extract Leaf Area Index and other crop-related features. Additionally, in this work, a benchmark of software tools is made. The hardware module and pipeline were validated considering real data acquired in two vineyards—Portugal and Italy. A dataset with sensory data collected by the module was made publicly available. Results demonstrated that: the system provides reliable and precise data on the surrounding environment and the pipeline is capable of computing volume and occupancy area from the acquired data. Objective: To perform a systematic literature review (SLR) aimed at evaluating the efficacy and safety of pharmacological and non-pharmacological treatments for Raynaud's phenomenon (RP) and digital ulcers (DU) in patients with systemic sclerosis (SSc) and other connective tissue diseases (CTD), in order to inform the Portuguese recommendations for managing RP and DU in these patients., Methods: A SLR was conducted until May 2022 to identify studies assessing the efficacy and safety of pharmacological and non-pharmacological interventions for RP and DU in SSc and other CTD. Eligible study designs included randomized controlled trials (RCTs), controlled clinical trials, and their extensions for assessing efficacy and safety of interventions. Observational studies with a comparator were included for evaluating the efficacy and safety of non-pharmacological interventions and safety of pharmacological interventions. The risk of bias of each study was assessed using standard tools., Results: Out of 71 publications meeting the inclusion criteria, 59 evaluated pharmacological and 12 non-pharmacological interventions. We found moderate quality evidence supporting the efficacy of calcium channel blockers, phosphodiesterase-5 inhibitors, and intravenous prostacyclin analogues in reducing RP frequency, severity, and duration. Intravenous iloprost had a small to moderate effect size in improving DU healing. Phosphodiesterase-5 inhibitors were effective in reducing total DU count, new DU occurrence, and enhancing DU healing. Bosentan effectively prevented new DU in SSc patients. No new safety concerns were associated with these treatments. The studies on non-pharmacological interventions were, in general, of low quality, and had a small sample size. Warming measures decreased frequency and duration of RP attacks; laser therapy improved RP-related outcomes; local oxygen-ozone therapy improved RP outcomes as an add-on therapy; bone marrow mononuclear cell implantation improved DU-associated pain; periarterial sympathectomy and vascular bypass reduced DU number and finger amputation risk., Conclusion: The available evidence supports the efficacy and safety of pharmacological interventions, namely nifedipine, sildenafil, iloprost, and bosentan in treating RP and DU in patients with SSc and other CTD. Scarce and low-quality evidence does support the use of some non-pharmacological interventions but with only a modest effect size. This SLR underscores the limited availability of high-quality evidence for determining the optimal treatment.

Objective: To develop evidence-based recommendations for the non-pharmacological and pharmacological management of Raynaud's phenomenon (RP) and digital ulcers (DUs) in patients with systemic sclerosis and other immune-mediated connective tissue diseases (CTDs)., Methods: A task force comprising 21 rheumatologists, two surgeons (vascular and plastic), two nurses, and one patient representative was established. Following a systematic literature review performed to inform the recommendations, statements were formulated and discussed during two meetings (one online and one in-person). Levels of evidence, grades of recommendation (GoR), and level of agreement (LoA) were determined., Results: Five overarching principles and 13 recommendations were developed. GoR ranged from A to D. The mean ± standard difference (SD) LoA with the overarching principles and recommendations ranged from 7.8±2.1 to 9.8±0.4. Briefly, the management of RP and DUs in patients with CTDs should be coordinated by a multidisciplinary team and based on shared decisions with patients. Nifedipine should be used as first-line therapy for RP and/or DUs. Sildenafil, tadalafil, and/or iloprost IV are second-line options for severe and/or refractory patients with RP and/or DUs. Sildenafil, tadalafil and/or Iloprost IV, should be prescribed for healing and prevention (also including bosentan) of DUs. In patients with RP and/or DUs, non-pharmacological interventions might be considered as add-ons, but there is limited quality and quantity of scientific evidence supporting their use., Conclusions: These recommendations will inform rheumatologists, specialist nurses, other healthcare professionals, and patients about a comprehensive and personalized management of RP and DUs. A research agenda was developed to address unmet needs, particularly for non-pharmacologic interventions.

Implementation of evidence-based practice (EBP) is essential for ensuring high-quality nursing care. In Portugal, nurses are responsible for care delivery to patients who require peripheral intravenous access. However, recent authors emphasized the predominance of a culture based on outdated professional vascular access practices in Portuguese clinical settings. Thus, the aim of this study was to map the studies conducted in Portugal on peripheral intravenous catheterization. A scoping review was conducted based on the Joanna Briggs Institute recommendations, with a strategy adapted to different scientific databases/registers. Independent reviewers selected, extracted, and synthesized the data. Of the 2128 studies found, 26 were included in this review, published between 2010 and 2022. Previous research shows that Portuguese nurses' implementation of EBP was found to be relatively low overall, while most studies did not attempt to embed EBP change into routine care. Although nurses are responsible for implementing EBP at an individual patient level, the studies conducted in Portugal report nonstandardized practices among professionals, with significant deviations from recent evidence. This reality, combined with Portugal's absence of government-endorsed evidence-based standards for peripheral intravenous catheter (PIVC) insertion and treatment and vascular access teams, may explain the country's unacceptably high incidence of PIVC-related complications reported over the last decade., (Copyright © 2023 Infusion Nurses Society.)

Introduction: Despite years of experience with biological disease modifying anti-rheumatic drugs (bDMARD) in rheumatoid arthritis (RA), little is known about differences in infectious risk among bDMARDs. The aim of this study was to assess the incidence and type of infections in RA patients on bDMARDs and to determine possible predictors., Methods: A retrospective multicenter cohort study that included patients registered in the Rheumatic Diseases Portuguese Registry (Reuma.pt) with RA, and exposed to at least one bDMARD until April 2021. RA patients under bDMARD and with at least one episode of severe infection (SI), defined as infection that requires hospitalization, use of parenteral antibiotics or that resulted in death, were compared to patients with no report of SI. Demographic and clinical data at baseline and at the time of each SI were collected to establish comparisons between different groups of bDMARDs. Comparisons between different bDMARDs were assessed and logistic regression was performed to identify predictors of SI., Results: We included 3394 patients, 2833 (83.5%) female, with a mean age at RA diagnosis of 45.5±13.7 years. SI was diagnosed in 142 of the 3394 patients evaluated (4.2%), totaling 151 episodes of SI. At baseline, patients with SI had a significantly higher proportion of prior orthopedic surgery, asthma, interstitial lung disease, chronic kidney disease and corticosteroid use, higher mean age and longer median disease duration at first bDMARD. Nine patients died (6.0%). Ninety-two SI (60.9%) occurred with the first bDMARD, the majority leading to discontinuation of the bDMARD within 6 months (n=75, 49.7%), while 65 (43.0%) restarted the same bDMARD and 11 (7.3%) switched to another bDMARD (6 of them to a different mechanism of action). In the multivariate analysis, we found that chronic kidney disease, asthma, infliximab, corticosteroid use, interstitial lung disease, previous orthopedic surgery, higher Health Assessment Questionnaire and DAS284V-ESR are independent predictors of SI., Conclusion: This study described the incidence and types of SI among Portuguese RA patients on biologics, identifying several predictors of SI, both globally and with different bDMARDs. Physicians should be aware of the real-word infectious risk in RA patients on bDMARDs when making treatment decisions.

In this paper we aim to discuss how Portuguese prisoners know and what they feel about surveillance mechanisms related to the inclusion and deletion of the DNA profiles of convicted criminals in the national forensic database. Through a set of interviews with individuals currently imprisoned we focus on the ways this group perceives forensic DNA technologies. While the institutional and political discourses maintain that the restricted use and application of DNA profiles within the national forensic database protects individuals' rights, the prisoners claim that police misuse of such technologies potentially makes it difficult to escape from surveillance and acts as a mean of reinforcing the stigma of delinquency. The prisoners also argue that additional intensive and extensive use of surveillance devices might be more protective of their own individual rights and might possibly increase potential for exoneration., (Crown Copyright © 2011.
