AIM:: Screening and extended assessment of the nutritional status of patients on admission and on discharge from hospital were carried out. DESCRIPTION:: The studies were carried out in four teaching hospitals, four provincial hospitals and four county hospitals in Poland. SUBJECTS:: Screening examinations were carried out for 3310 randomly selected patients (every 10th patient admitted to hospital, including 1916 female cases aged from 16 to 92?y and 1394 male patients aged from 16 to 100?y). Extended examinations were carried out on 210 patients aged from 16 to 87?y (including 122 female and 88 male). MAIN ASSESSMENT PARAMETERS:: Anthropometric (height, weight, body mass index (BMI), waist-to-hip ratio (WHR), arm circumference) and biochemical indices (erythrocyte count, haemoglobin concentration, white blood cell count, blood lymphocyte count and serum albumin serum concentration). The extended examinations included determination of antioxidant vitamins (A, C, E), vitamin B12 and folic acid. RESULTS:: On admission to hospital, 10.43%of the patients surveyed had a BMI below 20?kg/m2, in 20.74%of patients serum albumin concentration was below 3.5?g/dl, indicating possible protein energy malnutrition. In addition, 21.02%had lymphocyte count below 1.5×103/mm3. During hospitalisation, deterioration in the nutritional status of the patient population occurred. On discharge from hospital, the percentage of patients with BMI<20?kg/m2 increased to 11.21%and the percentage with low blood albumin (<3.5?g/dl) increased to 28.57%. On admission, vitamin C deficiency was present in 51.8%of patients, folic acid deficiency in 32%, vitamin E deficiency in 10%, vitamin B12 deficiency in 6.8%and vitamin A deficiency in 1.4%. Vitamin deficiencies were present equally in malnourished, overweight and obese patients. CONCLUSIONS:: In patients admitted to hospitals in Poland, malnutrition risk demonstrated by BMI was observed in 10.43%of patients. On the basis of biochemical indices, increased nutritional risk was demonstrated in 21%of patients. Vitamin malnutrition was seen in the majority of patients. A significant correlation between weight, BMI, arm circumference, blood lymphocyte count and the number of days spent in hospital was observed. Increasing resistance to drugs represents a serious problem in treatment of infections with Helicobacter pylori, providing cause of frequent therapeutic failures. Present study aimed at analysis of changes in resistance of H. pylori to antibiotics in West Poland within the recent 15 years. 108 strains of H. pylori were analysed, isolated from gastric mucosa of adult patients. Group 1 involved 66 strains isolated in years of 1998/1999. Group 2 comprised 42 isolates obtained in years of 2013/2014. Susceptibility to amoxicillin (AMX), clarithromycin (CL), tetracycline (TC) and metronidazole (MTZ) was determined by E-test (AB Biodisc). All strains on both studied groups were susceptible to AMX. In group 1 all strains proved to be susceptible to TC, while 9% and 36% of tested strains were resistant to CL and MTZ, respectively. By contrast, in group 2, 31% and 83% of strains were resistant to CL and MTZ, respectively. In parallel, 14% strains were found to be resistant to TC (according to EUCAST interpretations). In West Poland, within recent 15 years a dramatic increase was noted in H. pylori strains resistant to metronidazole. In parallel, a significant increase was noted in proportion of strains resistant to clarithromycin.

Background and Objective: Irritable Bowel Syndrome (IBS) has a substantial impact on health-related quality of life (HR-QoL) but high-quality data pre- and post-treatment using the IBS-Quality of Life (IBS-QOL) measure are limited. The objective of this study was to evaluate the changes from baseline of the IBS-QOL scores, symptom scores and health economic data in IBS patients, after 4 and 8 weeks of treatment with mebeverine hydrochloride or pinaverium bromide., Methods: This was a prospective observational cohort study in patients with IBS, diagnosed using the Rome III criteria in four countries (Poland, Egypt, Mexico and China)., Results: A total of 607 patients were enrolled. At baseline, the IBS-QOL total scores were 52.0 in Poland, 48.9 in Egypt, 51.9 in Mexico, 76.4 in China and 56.4 overall. Increases in IBS-QOL total score were statistically significant at Weeks 4 and 8 overall and in each country (overall: 11.8 at Week 4, 24.3 at Week 8; p < 0.001). Improvements were shown in all IBS-QOL subscales and scores. Symptoms and health economic outcomes were improved. Furthermore, the favourable safety profile of these treatments was confirmed in this study., Conclusions: This study demonstrated that IBS patients have a substantially reduced HR-QoL and that treatment with mebeverine hydrochloride or pinaverium bromide improved HR-QoL.

The aim of the study was a screening assessment of the nutritional status of patients admitted to hospitals and discharged from hospitals. The study was carried out in 4 university hospitals, 4 woivodeship hospitals and 4 district hospitals. In randomly selected 3310 patients (every 10th patient admitted to hospital) anthropometric parameters were assessed: body height, body mass, body mass index (BMI), waist-hip ratio (WHR), arm circumference, blood morphological and biochemical parameters were determined (erythrocyte, white blood cell and lymphocyte count in blood, albumin and haemoglobin concentration in serum). The mean values of the assessed parameters in the whole studied population of patients admitted to various types of hospitals were not different from the normative values for adults, however, a gradual decrease of the values of certain parameters in the over 70 years age group was observed. Although, the mean values of the studied parameters of the nutritional status were within the accepted normal range, 10.43% of the studied patients had BMI below 20 kg/m2, and 20.7% of the patients the serum albumin level was below 3.5 g/dl on admission, which could suggest protein-energy malnutrition. In a yet higher proportion of patients (21%) lymphocyte count was below 1500/mm3. During hospital stay tendency became even more pronounced. On discharge from hospital the proportion of patients with BMI below 20 kg/m2 rose to 11.21%, and with serum albumin level below 3.5 g/dl rose to 28.6%. Only the proportion of patients with low lymphocyte count remained unchanged during hospital stay and was 21.1% on discharge. In the studied population 42.29% of the patients reported receiving of additional food beyond hospital diet.

Gastric cancerogenesis is a multistage process. Dietary studies may contribute to elucidation of that complex problem.

Socio‐economic development has shaped fisheries social‐ecological systems (SES) worldwide across different scales. No work has yet undertaken how this development led to novel, not experienced before, systems structure in marine SES. Here, we quantify socio‐economic novelty as the degree of dissimilarity relative to a specific spatiotemporal baseline in the Baltic Sea fisheries SES between 1975 and 2015. We used catch by "gears," catch by "commercial groups" and trade ("import" and "export") as respective indicators of novelty at national, regional and international governance levels. We found that socio‐economic novelty increased over time nonlinearly in relation to the 1975–1979 baseline. The contribution to total novelty shifted from the dominance of "gears" and "commercial groups" in the late 1990s and early 2000s to "import" and "export" after the mid‐2000s, i.e. from national and regional levels to the international level. The fastest increase in novelty occurred with the trade dominance shift, primarily related to monetary value rather than quantity. Spatially, novelty emerged with a large difference across countries, and a major contribution by Sweden, Denmark and Poland. We identified the influence of different management interventions and governance actions on the emergence of novelty in the Baltic SES. The decreasing socio‐economic novelty at national and regional levels could indicate reduced variability due to management intervention in recent years which might decrease SES resilience to shocks. Calculating socio‐economic novelty and studying its drivers at different scales could provide a better understanding of SES complexity and inform urgently needed adaptation and transformation towards sustainable future pathways.
