Background Examine the evolution of stunting in <5 years of age in Peru and its relationship with birth interval (BI), and associated factors from 1996 to 2014. Methods Data were extracted from the National Demographic and Health Survey of Peru (DHS) for the years 1996, 2000, 2005, 2010 and 2014 on children <5 years of age for stunting (height / age ≤ 2DS). The main independent variable of interest was BI dichotomized by interval between date of last and penultimate birth (≤24 months vs. >24 months) and others sociodemographic variables. Crude and adjusted odds ratios with 95% confidence intervals were estimated in logistic regression for the effect of BI and other variables on stunting by each survey year. Results Between 1996 and 2014, stunting declined in both BI groups: ≤24 months: 35.8% to 21.8%; >24 months: 29.5% to 14.3%. BI was associated with stunting after adjustment for other variables in each survey except 2005. Of the other factors, sex, birth order (fourth child or more), maternal education and poverty were independently associated with stunting in all survey years. Residence in rural areas and in Amazon forest and highland regions was associated with stunting 1996 and 2000. Maternal age was not independently associated with stunting. Conclusions While stunting showed a decreasing trend from 1996 to 2014, birth interval exceeding 24 months exerted a protective effect on stunting across the years surveyed. With the objective of evaluating the concordance between the self-sampling of vaginal samples and the standard collection of endocervical samples for the identification of Chlamydia trachomatis, Neisseria gonorrhoeae, Trichomonas vaginalis and Candida spp. carried out by health personnel in women from an urban-rural area of Peru, a prospective and cross-sectional study was carried out in 206 women of childbearing age, we identified some sexually transmitted infections such as Chlamydia trachomatis or Trichomonas vaginalis in 9/206 (4.4%). We obtained a high degree of agreement in the identification of Candida spp. (k = 0.97), Chlamydia trachomatis (k=0.92) and Trichomonas vaginalis by microscopy (k=1.00), and a considerable agreement for the identification of Trichomonas vaginalis by culture (k=0.66). The self-sampling technique can be used to identify some sexually transmitted infections in urban-rural populations.

Objectives: To describe the epidemiological profile of people living with disabilities due to traffic accidents (TA) in Peru., Materials and Methods: Secondary analysis of the National Survey Specialized on Disability (ENEDIS) of 2012 and an ecological analysis of TA records of the Ministry of Transportation and Communications was done., Results: Disability by traffic accidents (DAT) was reported by 49,036 persons; 81.3% of whom live in urban areas. The most frequent disability was limited locomotion and skill (77.4%), followed by visual impairment (22.9%). Dependence for activities was reported in 44.7% of persons with disabilities. The regions with the highest prevalence of TA have a higher prevalence of disability by traffic accidents (Spearman coefficient: 0.426, p=0.034)., Conclusions: Most of disability due to TA is found in urban areas, correspond to males and consist of persons in economically productive age. The most common form of disability is in locomotion. Most of individuals do not receive any form of rehabilitation, which accentuates health inequity related to traffic accidents.

Objective: Analyze malnutrition and anemia trends in Peruvian children under 5 years of age and their association with determinants in the 2000-2011 period., Methods: Nutritional indicators for children under 5 years of age from the 2011 Demographic and Family Health Survey (ENDES), and their evolution based on data from the 2000, 2005, and 2008 ENDES, were analyzed. Chronic malnutrition (CM) (height/age ≤ 2 SD), acute malnutrition (AM), (height/weight ≤ 2 SD), and anemia trends were estimated. Associations were found with factors such as sex, age, area of residence (urban or rural), region of residence, mother's education, wealth quintile, availability of public water system, sewer availability, altitude, presence of other children in household, birth order, presence of diarrhea in previous 15 days, and presence of cough in previous 15 days., Results: AM, CM, and anemia in Peruvian children under 5 years of age decreased from 2000-2011. This reduction was not uniform for the three conditions, with decreases of 1.1% to 0.4% recorded for AM, 31.6% to 19.6% for CM, and 50.4% to 30.7% for anemia. Although the factors analyzed were related to the prevalence of these three illnesses, calculation of the adjusted odds ratios showed significant differences for CM (mother's education, Sierra region, altitude greater than 2 500 m above sea level, presence of two or more children in household, and being the third or successive child) and anemia (child sex [higher in males], children under 2 years of age, Resto de costa region and Selva region, altitude greater than 2 500 m above sea level, availability of public water system, sewer availability, presence of two or more children in household, and presence of diarrhea within 15 days prior to the survey). For AM, differences were observed according to some factors but they were not significant in the adjusted model., Conclusions: In the 2000-2011 period, Peru achieved reduction of its CM, AM, and anemia rates. AM rates decreased to almost one-third, with overall figures remaining fairly low, and affecting to a greater extent highly localized areas and groups of the country. However, the current rates of chronic malnutrition and anemia continue to be very high, which represents a true challenge for public policy, as occurs in other countries of the region. In order to overcome this challenge it will be necessary to change the approach, no longer conceiving of child malnutrition exclusively as a nutritional problem and instead placing greater emphasis on the related determinants. Finally, comprehensive and integrated initiatives should be encouraged and strengthened in the poorest communities.

Selection of the priority tuberculosis (TB) research topics in Peru occurred in three stages: a literature review of TB research findings and information in Peru; proposal of the most important topics by 31 key investigators identified through 233 articles published between 1981 and 2011,16 (48.5%) of the 33 regional coordinators of the National Tuberculosis Prevention and Control Strategy, and influential 16 academics and officials in the area of TB; and a participatory workshop attended by 103 health professionals and officials grouped under 10 thematic discussion groups. Of the 49 research topics identified in the second stage of the process, the 30 most important ones for Peru were selected and prioritized during the workshop. These topics, selected through this inclusive, transparent, and participatory methodology, became the National Tuberculosis Research Agenda in Peru for 2011 - 2014. These results should help to improve TB control strategies in the country and optimize the use of financial and human resources.

Objectives: To estimate HIV incidence in the adult population of Peru, 2010, and analyze its distribution based on risk behavior., Materials and Methods: The UNAIDS model was applied based on the modes of transmission (MoT). The information was obtained from the review detailed in 59 documents (1984 - 2008). National databases were also analyzed to obtain specific data. Final selection of data was validated by the technical group and a group of experts. After consultation with experts, values were corrected and an uncertainty analysis was conducted. The model was adjusted to 2009 national prevalence (0.45%)., Results: Incidence estimated for 2010 was 0.03%, (4346 new infections). 84% of new infections concentrate on higher risk groups: men who have sex with men (55%) and people who have casual sex with heterosexuals (6.2%). The remaining 16% corresponds to low-risk heterosexual population. Heterosexual transmission accounts for 43% of new cases, 18% of which corresponds to female partners of high risk individuals. Only 2.2% of cases is related to female sexual work and 1.0% to injection drug users., Conclusions: The model provides an estimation of the incidence and its distribution among risk groups according to the mode of transmission, consistent with the HIV case reporting. The model creates scenarios to help decision making and policy formulation, as well as surveillance and planning of prevention and control.

Objectives: To develop and validate an instrument to assess Internet Addiction (IA) phenomenon in adolescents of Metropolitan Lima., Materials and Methods: We performed an observational analytical study, including a sample of 248 high school adolescent students. In order to evaluate the IA, we constructed the questionnaire: "Scale for Internet Addiction of Lima" (SIAL), which assesses symptoms and dysfunctional characteristics. The resulting items were submitted to experts' judgment, finally obtaining a 11-item scale., Results: The mean age was 14 years old. The psychometric analysis of the instrument showed a Cronbach' Alpha Coefficient of 0.84, with values of item-total correlation ranging from 0.45 to 0.59. The dimensional analysis yielded a two-dimensional structure that explained up to 50.7% of the total variance. The bi-dimensional data analysis revealed a significant association (p<0,001) between Dimension I (symptoms of IA) and the weekly time spent on the Internet, male sex, past history of bad behavior in school and plans for the future. Dimension II (dysfunction due to IA) had a significant association to past history of bad behavior, plans for the future (p<0,001) and missing school without valid reasons., Conclusions: The SIAL showed a good internal consistency, with moderate and significant inter-item correlations. The findings show that addiction has a dynamic role, which evidences a problem generated in family patterns and inadequate social networks.

Objective: To analyze the evolution of published scientific articles on HIV/AIDS in Peru., Methods: A bibliometric analysis of papers on HIV/AIDS published in journals indexed in MEDLINE, SciELO and LILACS until October 2010. We selected research papers fully developed in Peru, and multicenter studies with participating Peruvian sites., Results: We identified 257 publications on HIV/AIDS, showing an increase since 2003. The average publication delay was 2.8±1.8 years. Only 94 (36.6 %) articles were published in Spanish. The most studied areas were epidemiology (36.6 %) and clinical topics (35.8 %). The cross-sectional design was the most frequent (56.8 %) followed by case series. According to the WHO classification, studies to learn more about the disease and risk factors predominated (85.6 %) and according to the intervention areas, 46.7 % focused on diagnosis and treatment. Most studies were conducted in Lima (65.9 %). 48.2 % of studies focused on people living with HIV/AIDS. Finally, Revista Peruana de Medicina Experimental y Salud Pública was the journal that published most articles on HIV/AIDS (9.7 %)., Conclusions: We found a growth in scientific production on HIV/AIDS in Peru; however, we believe that the research undertaken was not based on an agreed national agenda or national research priorities, which might have limitted its dissemination and application.

Objectives: To evaluate the correlation and concordance between the 'Peruvian National Exam of Medicine' (ENAM) and the Mean Grade Point Average (GPA) in recently graduated medical students in the period 2007 to 2009., Materials and Methods: We carried out a secondary data analysis, using the records of the physicians applying to the Rural and Urban Marginal Service in Health of Peru (SERUMS) processes for the years 2008 to 2010. We extracted from these registers, the grades obtained in the ENAM and GPA. We performed a descriptive analysis using medians and 1st and 3rd quartiles (q1/q3); we calculated the correlation between both scores using the Spearman correlation coefficient, additionally, we conducted a lineal regression analysis, and the concordance was measured using the Bland and Altman coefficient., Results: A total of 6 117 physicians were included, the overall median for the GPA was 13.4 (12.7/14.2) and for the ENAM was 11.6 (10.2/13.0).Of the total assessed, 36.8% failed the TEST. We observed an increase in annual median of ENAM scores, with the consequent decrease in the difference between both grades. The correlation between ENAM and PPU is direct and moderate (0.582), independent from the year, type of university management (Public or Private) and location. However, the concordance between both ratings is regular, with a global coefficient of 0.272 (CI 95%: 0.260 to 0.284)., Conclusions: Independently of the year, location or type of university management, there is a moderate correlation between the ENAM and the PPU; however, there is only a regular concordance between both grades.

Objectives: To estimate the frequency of self-reporting of road traffic accidents in the previous year in the general population and to determine the associated factors., Materials and Methods: We conducted a secondary analysis of the data of the III National Survey of Drug Use in the General Population of Peru, 2006. We measured socio-demographical variables: age, gender, place of origin, educational level and marital status. We also evaluated the use of legal, illegal and medical drugs. The independent variable was the self-reporting of a road traffic accident. We performed the descriptive, bivariate and multivariate analysis of the socio-demographical variables and the drug use (legal and illegal), together with the self-reporting of the traffic accident., Results: The frequency of reporting of road traffic accidents in the last year according to the survey was 2.93% (95%CI: 2.92-2.94). The associated factors for self-reporting of a road traffic accident were: to live in the jungle areas (OR: 2.03; 95%CI:1.55-2.65), male gender (OR: 1.79; 95%CI: 1.46-2.22), legal drugs use in the last year (OR: 1.98, 95%CI: 1.53-2.55), alcohol consumption in the last year (OR: 1.82; 95%CI: 1.44-2.32) and medical drugs use in the last year (OR: 2.45, 95%CI 1.63-3.68)., Conclusions: The prevalence of self-reporting of road traffic accidents in the last year was very high compared to similar studies and other reporting sources. The variables associated with having had a traffic accident were: living in the jungle area, being male, legal drug use in the last month, especially alcohol and medical drug use in the last month. It is necessary to think carefully about the information system of the road traffic accidents in order to achieve a better picture of the problem putting emphasis in the legal drugs use.
