Phosphorus applied to soils in excess of crop requirement could create situations favorable to P enrichment in subsurface flow that contributes to eutrophication of surface water. This pathway of P loss can be more severe in muck (i.e., organic) soils where agricultural production is intensive. This study evaluated the suitability of various environmental and agronomic soil P tests initially designed for mineral soils to predict dissolved reactive P (DRP) in subsurface flow from organic soils. Intact soil columns were collected from 44 muck soils in Ontario to provide a wide range of soil test P levels. A lysimeter leaching study was conducted by evenly adding water in an amount equivalent to 5 mm of rainfall. The leachate DRP concentration was linearly related to soil water-extractable P and CaCl2-extractable P with r2 values of 0.90 and 0.93, respectively, and to Bray-1 P and FeO-impregnated filter paper extractable P in a split-line model with a change point. Mehlich-3 P and Olsen P, a method recommended for agronomic P calibration in Ontario, were not related to leachate DRP concentration. All P sorption index (PSI) based degree of P saturation (DPS) values were closely related to leachate DRP in split-line models, with the DPS indices expressed as Bray-1 P/PSI and FeO-P/PSI having the highest correlation with leachate DRP concentration. Because it is desirable from practical and economic standpoints that the environmental risk assessment shares the same soil test with agronomic P calibration, the two PSI-based DPS indices as presented can be considered as environmental risk indicators of DRP subsurface loss from organic soils. Phosphorus movement in subsurface flow from agricultural soils can be a significant pathway contributing to eutrophication of surface waters. Our study aimed to evaluate several environmental and agronomic soil P tests as indicators of dissolved reactive P (DRP) concentrations in soil-column leachate from Ontario soils. Undisturbed soil columns were collected from six major soil series, with 10 sites of each to quantitatively cover a wide range of soil test P (STP) or degree of P saturation (DPS). Split-line models described the relationships (P < 0.001) between leachate DRP concentrations and the values of In(STP) and In(DPS), with a greater slope observed above the change points than below them. Among the tested soil P measures, water-extractable P (WEP), Mehlich-3 P/(Mehlich-3 Al + Fe) (DPSM3-1) and Mehlich-3 P/Mehlich-3 Al (DPSM3-2) had the strongest overall relationships with leachate DRP concentration. Ontario soils were grouped into no-risk, low-risk, medium-risk, and high-risk categories based on the conditional probability of yielding leachate DRP > 0.1 mg P L-1 at a given STP as measured by WEP and Olsen P or a given DPS as measured by DPSM3-1 and DPSM3-2. While the Olsen P test is most commonly used for agronomic calibration in Ontario, DPSM3-2 provided the best indicator of leachate DRP concentration from Ontario soils. Background: The rising incidence of infective endocarditis (IE) among people who inject drugs (PWID) has been a major concern across North America. The coincident rise in IE and change of drug preference to hydromorphone controlled-release (CR) among our PWID population in London, Ontario intrigued us to study the details of injection practices leading to IE, which have not been well characterized in literature., Methods: A case-control study, using one-on-one interviews to understand risk factors and injection practices associated with IE among PWID was conducted. Eligible participants included those who had injected drugs within the last 3 months, were > 18  years old and either never had or were currently admitted for an IE episode. Cases were recruited from the tertiary care centers and controls without IE were recruited from outpatient clinics and addiction clinics in London, Ontario., Results: Thirty three cases (PWID IE+) and 102 controls (PWID but IE-) were interviewed. Multivariable logistic regressions showed that the odds of having IE were 4.65 times higher among females (95% CI 1.85, 12.28; p = 0.001) and 5.76 times higher among PWID who did not use clean injection equipment from the provincial distribution networks (95% CI 2.37, 14.91; p < 0.001). Injecting into multiple sites and heating hydromorphone-CR prior to injection were not found to be significantly associated with IE. Hydromorphone-CR was the most commonly injected drug in both groups (90.9% cases; 81.4% controls; p = 0.197)., Discussion: Our study highlights the importance of distributing clean injection materials for IE prevention. Furthermore, our study showcases that females are at higher risk of IE, which is contrary to the reported literature. Gender differences in injection techniques, which may place women at higher risk of IE, require further study. We suspect that the very high prevalence of hydromorphone-CR use made our sample size too small to identify a significant association between its use and IE, which has been established in the literature.

Objectives: Antibiotic use varies widely between hospitals, but the influence of antimicrobial stewardship programs (ASPs) on this variability is not known. We aimed to determine the key structural and strategic aspects of ASPs associated with differences in risk-adjusted antibiotic utilization across facilities., Design: Observational study of acute-care hospitals in Ontario, Canada METHODS: A survey was sent to hospitals asking about both structural (8 elements) and strategic (32 elements) components of their ASP. Antibiotic use from hospital purchasing data was acquired for January 1 to December 31, 2014. Crude and adjusted defined daily doses per 1,000 patient days, accounting for hospital and aggregate patient characteristics, were calculated across facilities. Rate ratios (RR) of defined daily doses per 1,000 patient days were compared for hospitals with and without each antimicrobial stewardship element of interest., Results: Of 127 eligible hospitals, 73 (57%) participated in the study. There was a 7-fold range in antibiotic use across these facilities (min, 253 defined daily doses per 1,000 patient days; max, 1,872 defined daily doses per 1,000 patient days). The presence of designated funding or resources for the ASP (RRadjusted, 0·87; 95% CI, 0·75-0·99), prospective audit and feedback (RRadjusted, 0·80; 95% CI, 0·67-0·96), and intravenous-to-oral conversion policies (RRadjusted, 0·79; 95% CI, 0·64-0·99) were associated with lower risk-adjusted antibiotic use., Conclusions: Wide variability in antibiotic use across hospitals may be partially explained by both structural and strategic ASP elements. The presence of funding and resources, prospective audit and feedback, and intravenous-to-oral conversion should be considered priority elements of a robust ASP.
