Rationale: Surgical lung biopsies are often required for the definitive diagnosis of nonmalignant pediatric diffuse lung diseases; however, the literature on mortality after surgical lung biopsy in pediatric patients is sparse. Objectives: To determine the 30-day postoperative mortality rate after surgical lung biopsies for nonmalignant lung disease in pediatric patients in Ontario, Canada, and to identify risk factors associated with mortality. Methods: We performed an observational cohort study using population-based health administrative data available from ICES in Ontario, Canada, from 2000 to 2019. Cases were identified using the Canadian Classification of Health Interventions. Inclusion criteria were first surgical lung biopsies between 2000 and 2019 and age <18 years. Individuals with lung cancer, lung transplant, or missing data were excluded. A multivariable logistic regression model with generalized estimating equation was used to estimate the 30-day odds of mortality after surgical lung biopsy and to identify patient characteristics associated with increased mortality while accounting for clustering by hospital. Results: We identified 1,474 pediatric patients who underwent surgical lung biopsy in Ontario between 2000 and 2019. The overall mortality rates decreased over the study duration from 6.6% (2000–2004) to 3.0% (2015–2019). The study cohort for multivariate analyses consisted of 1,342 patients who had complete data. The pediatric mortality 30 days after surgical lung biopsy was 5.1% but was <1% in elective cases. Risk factors for increased mortality included open surgical lung biopsy (vs. video-assisted) (odds ratio [OR], 13.13; 95% confidence interval [CI], 3.76, 45.87; P < 0.001), nonelective procedure (OR, 11.74; 95% CI, 3.51, 39.27; P < 0.001), younger age (<3 mo) (OR, 6.04; 95% CI, 2.40, 15.22; P < 0.001), and higher comorbidity score (OR, 1.15; 95% CI, 1.05, 1.26; P = 0.003). Conclusions: Pediatric mortality postsurgical lung biopsy is not insignificant, particularly in nonelective procedures. Other important risk factors to consider when pursuing pathologic diagnosis include surgical approach, younger age, and higher comorbidity. [ABSTRACT FROM AUTHOR]

Rationale: Primary ciliary dyskinesia (PCD), a genetically heterogeneous, recessive disorder of motile cilia, is associated with distinct clinical features. Diagnostic tests, including ultrastructural analysis of cilia, nasal nitric oxide measurements, and molecular testing for mutations in PCD genes, have inherent limitations.Objectives: To define a statistically valid combination of systematically defined clinical features that strongly associates with PCD in children and adolescents.Methods: Investigators at seven North American sites in the Genetic Disorders of Mucociliary Clearance Consortium prospectively and systematically assessed individuals (aged 0-18 yr) referred due to high suspicion for PCD. The investigators defined specific clinical questions for the clinical report form based on expert opinion. Diagnostic testing was performed using standardized protocols and included nasal nitric oxide measurement, ciliary biopsy for ultrastructural analysis of cilia, and molecular genetic testing for PCD-associated genes. Final diagnoses were assigned as "definite PCD" (hallmark ultrastructural defects and/or two mutations in a PCD-associated gene), "probable/possible PCD" (no ultrastructural defect or genetic diagnosis, but compatible clinical features and nasal nitric oxide level in PCD range), and "other diagnosis or undefined." Criteria were developed to define early childhood clinical features on the basis of responses to multiple specific queries. Each defined feature was tested by logistic regression. Sensitivity and specificity analyses were conducted to define the most robust set of clinical features associated with PCD.Measurements and Main Results: From 534 participants 18 years of age and younger, 205 were identified as having "definite PCD" (including 164 with two mutations in a PCD-associated gene), 187 were categorized as "other diagnosis or undefined," and 142 were defined as having "probable/possible PCD." Participants with "definite PCD" were compared with the "other diagnosis or undefined" group. Four criteria-defined clinical features were statistically predictive of PCD: laterality defect; unexplained neonatal respiratory distress; early-onset, year-round nasal congestion; and early-onset, year-round wet cough (adjusted odds ratios of 7.7, 6.6, 3.4, and 3.1, respectively). The sensitivity and specificity based on the number of criteria-defined clinical features were four features, 0.21 and 0.99, respectively; three features, 0.50 and 0.96, respectively; and two features, 0.80 and 0.72, respectively.Conclusions: Systematically defined early clinical features could help identify children, including infants, likely to have PCD. The objective of this study was to identify the determinants of short hospital stay (< 24 h) among children admitted because of an acute asthma exacerbation. Computerized health records were used to identify children with a discharge diagnosis of asthma (ICD code 493.0) at the Hospital for Sick Children, Toronto, during the period October 1994 to October 1995. Cases were children with a length of hospital stay of < 24 h (short-stay group) and controls were children with a length of stay of > 24 h (long-stay group). Clinical and demographic data were extracted from the medical record. Over the 12-month period, 485 children were hospitalized because of asthma. Of these, 121 (25%) had short-stay admissions (< 24 h), whereas 364 (75%) had long-stay admissions (> 24 h). Simple random sampling was used to select 85 children from each of the two groups. There were no differences between the two groups regarding language, primary care physician, asthma history, management prior to emergency department (ED) presentation, respiratory rate on presentation, use of the observation unit, and time in the ED. Logistic regression analyses identified three variables associated with short hospital stay: milder asthma (adjusted odds ratio [OR] 4.9), male gender (adjusted OR 2.4), and availability of a delivery device at home (adjusted OR 2.0). In conclusion, many children admitted to hospital because of an asthma exacerbation have short, yet expensive, hospital stays. Abstract: The first population-based survey to determine predictors of influenza vaccination among children in a major metropolitan city with access to publicly funded health care and a universal influenza immunization program (UIIP). Previously collected demographic and health related data from 5619 school children aged 5–9 in Toronto, in 2006 were used to predict influenza vaccination. Vaccination was more likely in children with current asthma (OR 1.44, 95%CL 1.19–1.75), a high volume of contacts with a health service provider (OR 1.37, 95%CL 1.14–1.65), foreign born children (OR 1.20, 95%CL 1.01–1.42) and those with the lowest income adequacy (OR 1.37, 95%CL 1.12–1.68). Data from this study will be helpful in designing future influenza vaccination strategies to improve vaccination rates in the entire population. [Copyright &y& Elsevier]

Rationale: Material deprivation has been proposed as a more comprehensive measure of socioeconomic status than parental income. Stronger associations between childhood emergency department visits for asthma and air pollution have been demonstrated among children living in neighborhoods with high levels of deprivation, but the associations with asthma development and ongoing asthma are not known., Objectives: We determined the associations between neighborhood material deprivation and the development of new and ongoing childhood asthma., Methods: Prospectively collected administrative data housed at the Institute for Clinical Evaluative Sciences were examined for Toronto children born from 1997 to 2003. Neighborhood material deprivation, comprising no high school graduation, lone parent families, government transfers, unemployment, low income, and homes needing major repairs, was reported in the Ontario Marginalization Index. Incident asthma was defined by the time of entry into the Ontario Asthma Surveillance Information System (OASIS) database. We measured the risk of incident asthma using Cox proportional hazards models and the associations between ongoing asthma visits and deprivation by year of life with generalized linear mixed models., Results: OASIS asthma criteria were met for 21% of the 326,383 children. After adjustment for characteristics strongly associated with asthma, including male sex, prematurity, obesity, and atopic conditions other than asthma, children with high birth neighborhood deprivation were at increased risk of incident asthma (HR 1.11; 95% CI, 1.09-1.13). High deprivation in a given year of life was associated with increased odds of ongoing asthma during that year of life (OR 1.03; 95% CI, 1.02-1.05)., Conclusions: Children living in high-deprivation neighborhoods are at increased risk of incident and ongoing asthma. This study suggests that neighborhood material deprivation may represent a helpful tool for evaluating the effects of disparities in health and social advantages on the likelihood of developing and continuing to need healthcare visits for ongoing childhood asthma.

Rationale: Childhood asthma has shown variable associations with children's physical activity. Neighborhood walkability captures community features that promote walking and is protective against some chronic conditions, such as obesity and diabetes., Objectives: We evaluated associations between home neighborhood walkability and incident and ongoing childhood asthma., Methods: In this population-based cohort study, we used prospectively collected administrative healthcare data for the Province of Ontario housed at the Institute for Clinical Evaluative Sciences. We followed an administrative data cohort of 326,383 Toronto children born between 1997 and 2003, inclusive, until ages 8-15 years. Home neighborhood walkability quintile was measured using a validated walkability index with four dimensions: population density, dwelling density, access to retail and services, and street connectivity. Incident asthma was defined by time of entry into the validated Ontario Asthma Surveillance Information System database, which requires two outpatient visits for asthma within two consecutive years or any hospitalization for asthma and follows children with asthma longitudinally starting at any age. Associations between walkability and incident asthma were examined using Cox proportional hazards models. Associations between ongoing asthma and walkability in each year of life were examined using generalized linear mixed models., Results: Twenty-one percent of children (n = 69,628) developed incident asthma and were followed longitudinally in the Ontario Asthma Surveillance Information System database. Low birth home neighborhood walkability was associated with an increased incidence of asthma (hazard ratio, 1.11; 95% confidence interval, 1.08-1.14). Among children with asthma, low walkability in a given year of a child`s life was associated with greater odds of ongoing asthma in the same year (odds ratio, 1.12; 95% confidence interval, 1.09-1.14)., Conclusions: Children living in neighborhoods with low walkability were at increased risk of incident and ongoing asthma. Neighborhood walkability improvement, such as by adding pedestrian paths to improve street connectivity, offers potential strategies to contribute to primary asthma prevention.

Background: The cause of rising asthma incidence over time remains unexplained. Examining trends in the age of diagnosis across successive birth cohorts may offer insights into asthma etiology., Objective: To examine trends in the age at asthma diagnosis and the age and proportion of children hospitalized at first asthma diagnosis in Ontario, Canada., Methods: Eight consecutive birth cohorts of children (1993-2000) were observed using administrative data from a universal health insurance plan in Ontario, Canada (population 13 million). Trends in the need for hospitalization and age at asthma diagnosis were examined with descriptive and survival analyses., Results: The records of 1,059,511 children were examined, of whom 201,958 developed asthma in the first 8 years of life, with an average cumulative incidence of 19.1%. Mean age at asthma diagnosis decreased from 4.7 ± 1.5 years in birth year 1993 to 2.6 ± 2.0 years in birth year 2000 (P < .0001), with a higher adjusted risk of asthma diagnosis (hazard ratio, 6.7; 95% CI, 6.5-6.9) in the first 3 years of life for children born after 1996 versus children born in the period 1993 to 1995 (hazard ratio, 1.4; 95% CI, 1.3-1.4). The proportion of children hospitalized at asthma diagnosis stayed stable while the age at first asthma hospitalization decreased over time (P < .0001)., Conclusions: This study demonstrates a significant increase in asthma incidence and a decrease in the age of asthma diagnosis across multiple birth cohorts. Rationale: Individuals with asthma are more likely to die from chronic conditions than the general population. Measuring only mortality with asthma listed as the primary cause of death may lead to an underestimation of total asthma mortality., Objectives: To examine mortality patterns in the asthma population over 10 years, including asthma as the primary cause of death (asthma-specific mortality) and asthma as a secondary, contributing cause of death (asthma-contributing mortality)., Methods: Health administrative data from Ontario, Canada were used to identify mortality rates and cause of death in subjects 0 to 99 years of age. Mortality rates were calculated in the asthma and general population from 1999 to 2008. Total asthma mortality was estimated by adding rates of asthma-specific and asthma-contributing mortality for years 2003 to 2008., Measurements and Main Results: Asthma-specific mortality rates per 100,000 asthma population decreased by 54.4% from 13.6 in 1999 to 6.2 in 2008. In 2008, the asthma population had higher all-cause mortality compared with the general population (rate ratio, 1.3), asthma-specific mortality rates were 60% higher among those in the lowest compared with highest socioeconomic status, and total asthma mortality was fourfold higher than asthma-specific mortality alone (21.6 vs. 5.4 per 100,000)., Conclusions: All-cause mortality rates have decreased substantially over the past decade. Compared with the general population, the asthma population has higher all-cause mortality and is more likely to die from comorbid conditions. Total asthma mortality was fourfold higher than asthma-specific mortality, highlighting the importance of comprehensive measurement approaches that include asthma-specific and asthma-contributing mortality.

Background: Childhood asthma prevalence is widely measured by parental proxy report of physician-diagnosed asthma in questionnaires. Our objective was to validate this measure in a North American population., Methods: The 2884 study participants were a subsample of 5619 school children aged 5 to 9 years from 231 schools participating in the Toronto Child Health Evaluation Questionnaire study in 2006. We compared agreement between "questionnaire diagnosis" and a previously validated "health claims data diagnosis". Sensitivity, specificity and kappa were calculated for the questionnaire diagnosis using the health claims diagnosis as the reference standard., Results: Prevalence of asthma was 15.7% by questionnaire and 21.4% by health claims data. Questionnaire diagnosis was insensitive (59.0%) but specific (95.9%) for asthma. When children with asthma-related symptoms were excluded, the sensitivity increased (83.6%), and specificity remained high (93.6%)., Conclusions: Our results show that parental report of asthma by questionnaire has low sensitivity but high specificity as an asthma prevalence measure. In addition, children with "asthma-related symptoms" may represent a large fraction of under-diagnosed asthma and they should be excluded from the inception cohort for risk factor studies.

Objective: Asthma is the most common chronic disease in children. Previous studies described significant variations in acute asthma management in children. This study was conducted to examine whether asthma management in the pediatric emergency department (ED) was improved through the use of an evidence-based acute asthma care guideline reminder card., Methods: The Pediatric Acute Asthma Management Guideline (PAMG) was introduced to the ED of a pediatric tertiary care hospital in Ontario, Canada. Medical charts of 278 retrospective ED visits (January-December 2002) and 154 prospective visits (July 2003-June 2004) were reviewed to assess changes in acute asthma management such as medication treatment, asthma education, and discharge planning. Logistic and linear regressions were used to determine the effect of PAMG on asthma management in the ED. The propensity score method was used to adjust for confounding., Results: During the implementation of PAMG, patients who visited the ED were more likely to receive oral corticosteroids (Adjusted Odds Ratio [AOR] = 2.26, 95% CI: 1.63-3.14, p < 0.0001) and oxygen saturation reassessment before ED discharge (AOR = 2.02, 95% CI: 1.45-2.82, p < 0.0001). They also received 0.23 (95% CI: 0.03-0.44, p = 0.0283) more doses of bronchodilator in the first hour of ED stay. Improvements in asthma education and discharge planning were noted, but the changes were not statistically significant., Conclusions: After the implementation of an evidence-based guideline reminder card, medication treatment for acute asthma in the ED was significantly improved; however, asthma education and discharge planning remained unchanged. Future efforts on promoting guideline-based practice in the ED should focus on these components.

Although individuals spend the majority of their time indoors, most epidemiological studies estimate personal air pollution exposures based on outdoor levels. This almost certainly results in exposure misclassification as pollutant infiltration varies between homes. However, it is often not possible to collect detailed measures of infiltration for individual homes in large-scale epidemiological studies and thus there is currently a need to develop models that can be used to predict these values. To address this need, we examined infiltration of fine particulate matter (PM(2.5)) and identified determinants of infiltration for 46 residential homes in Toronto, Canada. Infiltration was estimated using the indoor/outdoor sulphur ratio and information on hypothesized predictors of infiltration were collected using questionnaires and publicly available databases. Multiple linear regression was used to develop the models. Mean infiltration was 0.52 ± 0.21 with no significant difference across heating and non-heating seasons. Predictors of infiltration were air exchange, presence of central air conditioning, and forced air heating. These variables accounted for 38% of the variability in infiltration. Without air exchange, the model accounted for 26% of the variability. Effective modelling of infiltration in individual homes remains difficult, although key variables such as use of central air conditioning show potential as an easily attainable indicator of infiltration.

Background: Screening for non-tuberculous mycobacteria (NTM) is recommended for adults with cystic fibrosis (CF). The relevance of this organism in North American pediatric CF patients is unclear as there is limited NTM prevalence data for children. We aimed to determine the prevalence of NTM in children with CF from a single expectorated sputum and identify clinical predictors of NTM isolation. Additionally, we compared two different sputum decontamination methods before mycobacterial culture., Methods: From March to November 2004, all sputum-producing patients aged 6-18 years attending the CF clinic at the Hospital for Sick Children in Toronto, Canada, were screened for NTM. Sputum samples were processed by both a single (N-acetyl-l-cysteine + NaOH) and double (N-acetyl-l-cysteine + NaOH + oxalic Acid) decontamination method. Using our CF clinic database and patient charts we analyzed differences in FEV(1), age, sex, pancreatic sufficiency, body mass index, bacterial colonization, and antibiotic use between NTM positive and negative patients., Results: Of 98 study patients, 6 (6.1%) were positive for NTM, 2 with Mycobacterium abscessus, and 4 with Mycobacterium avium complex. One patient with M. abscessus had clinically significant lung disease requiring treatment. We found no predictors of NTM isolation. The double decontamination method allowed detection of only half (3/6) of the positive NTM cultures., Conclusions: As the NTM prevalence rate in children with CF is within the range previously reported in adults and there are no reliable clinical predictors for isolation, annual sputum screening is needed to identify NTM in children. Further research is needed to determine the best sputum decontamination method for NTM culture in pediatric patients.

Introduction: Asthma guidelines have suggested that treatment decisions should be guided by indices of asthma control and not only by disease severity. In adults, symptom-based asthma control parameters have been shown to predict exacerbations and health care services use (HSU). We hypothesize that defining asthma control using parent-reported symptoms alone is not adequate in children., Materials and Methods: Cross-sectional data from the population-based asthma in Canada study were reanalyzed. Random-digit dialing was used to produce the final sample, consisting of 1,001 asthmatics: 801 adults (aged 16+) and 200 children (aged 4-15) participating by parental proxy. Weighted frequencies of Canadian guideline defined asthma control parameters, perceived asthma control, HSU and medication use were calculated separately for adults and children. Stratified analyses compared HSU in controlled versus uncontrolled asthmatics., Results: Over 90% of parents of asthmatic children believed their child's asthma to be controlled. Only 45% were actually controlled as defined by guideline parameters. Among controlled asthmatics, children reported higher HSU (32% reported 2+ health care encounters versus 17% of adults, P < 0.001). Irrespective of control and despite similar use of controller therapy, children reported a higher number of health care encounters than adults (any emergency department visits 37% vs. 24%, P = 0.00003; unscheduled doctor visits 59% vs. 36%, P < 0.00001). While reporting higher HSU, asthmatic children had less frequent episodes of excessive daytime symptoms than adults (29% vs. 49%, respectively, P < 0.0001)., Discussion: Current symptom-based asthma control parameters reported by parental proxy are likely poor predictors of asthma HSU and may not provide adequate asthma control estimates in children.
