Background: Anterior cruciate ligament (ACL) reconstruction can effectively return athletes to the playing field, but they are still at risk of developing posttraumatic osteoarthritis (PTOA). No studies have used multivariable analysis to evaluate the predictors of radiographic PTOA in the lateral compartment of the knee at short-term follow-up after ACL reconstruction. Purpose: To determine the predictors of radiographic joint space narrowing in the lateral compartment 2 to 3 years after ACL reconstruction in a young, active cohort. Study Design: Cohort study; Level of evidence, 2. Methods: A nested cohort of 358 patients from the Multicenter Orthopaedic Outcomes Network (MOON) prospective cohort who were aged ≤33 years, were injured playing a sport, and had never undergone surgery on the contralateral knee were followed up 2 years after ACL reconstruction with questionnaires and with weightbearing knee radiographs using the metatarsophalangeal (MTP) joint technique. The joint space width in the lateral compartment was measured using a semiautomatic computerized method, and multivariable predictive modeling was used to evaluate the relationship between meniscus treatment, cartilage injury, graft type, and joint space while adjusting for age, sex, body mass index, and Marx activity score. Results: The mean lateral joint space width was 0.11 mm narrower on the ACL-reconstructed knee compared with the contralateral healthy knee (7.69 mm vs 7.80 mm, respectively; P < .01). Statistically significant predictors of a narrower joint space width on the ACL-reconstructed knee included lateral meniscectomy (P < .001) and a Marx activity score less than 16 points (P < .001). Conclusion: This study identifies lateral meniscectomy and a lower baseline Marx activity score to be predictors of radiographic joint space narrowing in the lateral compartment 2 to 3 years after ACL reconstruction in young, active patients without a prior knee injury. Background Cerebral angiography remains the gold standard for the detection of mycotic aneurysms, and it has been estimated that ruptured mycotic aneurysms result in 5% of the neurological complications of patients with infective endocarditis (IE). Objective To determine the diagnostic yield of cerebral angiography in the above patient population and to assess patient factors that might suggest greater or lesser utility. Methods We retrospectively reviewed 168 patients who underwent cerebral angiography with a diagnosis of IE or infected left ventricular assist device at the Cleveland Clinic between January 2003 and March 2010 in accordance with institutional review board guidelines. Chart and imaging review was performed. Results 15/168 patients (8.9%) had mycotic aneurysms; 93.3% (14/15) of the patients with mycotic aneurysms presented with CNS hemorrhage and 66.7% (10/15) had acute ischemic findings. Of the 15 patients with mycotic aneurysms on angiography, seven underwent CT angiography and six underwent MR angiography, which showed mycotic aneurysms in three (42.9%) and two cases (one of which was questionable; 33.3%), respectively. Conclusions Patients with IE or similar sources of central bacterial emboli are prone to neurovascular complications. Approximately 9% of patients with IE at our institution who undergo cerebral angiography have mycotic aneurysms. Presentation with hemorrhage appears to be more predictive of aneurysm, as approximately 22% of patients with IE and hemorrhage were found to have an aneurysm compared with only 1% when hemorrhage was absent. Background Stable access to target lesions is foundational to endovascular therapy, be it in hemorrhagic or ischemic disease. Continued evolution in access technology has resulted in next generation catheters that afford improved trackability and proximal support. Objective Assess safety and patterns of use at two high volume centers, and conceptualize usage patterns. Materials and methods A retrospective review of 608 cases in which a ‘ next generation ' catheter was used during 2008 - 2010 at Cleveland Clinic (Cleveland, Ohio, USA) and throughout 2009 - 2010 at Emory University Hospital (Atlanta, Georgia, USA) was conducted, and the cases classified by indication. Catheter placement, distal most location, and related complications were recorded and experience summarized. We also reviewed the differences in the catheters and the rationale for catheter selection, as well as relative costs for each approach. Results 311 Neuron 053, 166 Neuron 070, 36 distal access catheter (DAC) 3.9 F, 61 DAC 4.3 F, and 34 DAC 5.2 F catheters were deployed. Of these, 459 placements were in the anterior circulation, 130 in the posterior circulation, 11 in the external carotid artery, and eight were used intravenously. Complication rates were 9/131 (6.9%) for the DAC catheter group, 16/311 (5.1%) for the Neuron 053 group, and 14/166 (8.4%) for the Neuron 070 group (p=0.37, χ 2 test). Conclusions Next generation access catheters possess characteristics that blend qualities of traditional microcatheters and stiff guide catheters. There was no statistically significant difference in complication rates between the various catheter families in this small retrospective review, and the complication rates were similar to historical complication rates. Background: High levels of humeral torsion allow baseball pitchers to achieve maximum external rotation in the late cockingphase of pitching with lower twisting and shear forces on the long head of the biceps tendon and rotator cuff tendons.Hypothesis: Humeral torsion is inversely related to the incidence and severity of shoulder injuries and other upper extremityinjuries in professional baseball pitchers.Study Design: Case-control study; Level of evidence, 3.Methods: A total of 25 professional pitchers from a single Major League Baseball organization were prospectively recruited intothis study. Computed tomography (CT) was performed on dominant and nondominant humeri, and image data were processedwith a 3-dimensional volume-rendering postprocessing program. The software program was then modified to model a simplifiedthrowing motion and to measure potential internal impingement distances in a small subset of players. Players were followed for 2years after CT, and the number of days missed from pitching activities was recorded as a measure of injury severity andincidence.Results: The mean dominant humeral torsion was 38.5° ± 8.9°; the mean nondominant humeral torsion was 27.6° ± 8.0°. Thedifference between dominant and nondominant torsions was significant (P<.0001). Among the 11 pitchers (44%) injured duringfollow-up, 5 players had shoulder injuries, 7 had elbow injuries, and 2 had finger injuries. Dominant humeral torsion was a statisticallysignificant predictor of severe injuries (≥30 days; P = .048) but not of milder injuries. Among injured players, higher numbersof days missed because of injury were strongly correlated with lower degrees of dominant humeral torsion (r = 20.78; P = .005)and smaller differences between dominant and nondominant humeral torsions (r = 20.59; P = .055). There was no significantassociation between the incidence of shoulder injury and minimum glenoid-tuberosity distance in the dominant or nondominantshoulder or degree of dominant glenoid version.Conclusion: A strong relationship was found between lower degrees of dominant humeral torsion and more severe upperextremity injuries as well as a trend relating lower side-to-side differences in torsion with more severe dominant upper extremityinjuries. In addition, there was a higher incidence of severe injuries in players with lower degrees of dominant torsion. Background: Cardiac computed tomography (CT) is emerging as an adjunctive modality to echocardiography in the evaluation of infective endocarditis (IE) and surgical planning. CT studies in IE have, however, focused on its diagnostic rather than prognostic utility, the latter of which is important in high-risk diseases like IE. We evaluated the associations between cardiac CT and transesophageal echocardiography (TEE) findings and adverse outcomes after IE surgery., Methods: Of 833 consecutive patients with surgically proven IE during May 1, 2014 to May 1, 2019, at Cleveland Clinic, 155 underwent both preoperative ECG-gated contrast-enhanced CT and TEE. Multivariable analyses were performed to identify CT and TEE biomarkers that predict adverse outcomes after IE surgery, adjusting for EuroSCORE II (European System for Cardiac operative Risk Evaluation II)., Results: CT and TEE were positive for IE in 123 (75.0%) and 124 (75.6%) of patients, respectively. Thirty-day mortality occurred in 3 (1.9%) patients and composite mortality or morbidities in 72 (46.5%). Pseudoaneurysm or abscess detected on TEE was the only imaging biomarker to show independent association with composite mortality or morbidities in-hospital, with odds ratio (95% CI) of 3.66 (1.76-7.59), P =0.001. There were 17 late deaths, and both pseudoaneurysm or abscess detected on CT and fistula detected on CT were the only independent predictors of total mortality during follow-up, with hazards ratios (95% CI) of 3.82 (1.25-11.7), P <0.001 and 9.84 (1.89-51.0), P =0.007, respectively., Conclusions: We identified cardiac CT and TEE features that predicted separate adverse outcomes after IE surgery. Imaging biomarkers can play important roles incremental to conventional clinical factors for risk stratification in patients undergoing IE surgery.

Objective: The purpose of this study was to compare the diagnostic performance of a 5-minute shoulder MRI protocol consisting of multiplanar 2D fast spin-echo (FSE) sequences with parallel imaging to that of a standard shoulder MRI protocol., Materials and Methods: A retrospective review of 151 3-T MRI examinations of shoulders of 147 patients (mean age, 46.95 years) and 50 1.5-T MRI examinations of shoulders of 50 patients (mean age, 53.74 years) with four fast and five standard sequences from two academic centers between January 2014 and April 2015 was performed by three musculoskeletal radiologists. Interchangeability of fast and standard MRI was tested by comparing interprotocol (fast vs standard) interreader agreement with standard MRI interreader agreement. Interreader agreement was also compared using kappa statistics. The frequency of major findings was compared using an adjusted McNemar test. Sensitivity and specificity of MRI were measured for 51 patients who underwent surgery., Results: Interprotocol reader agreement was essentially equal to reader agreement on standard MRI (mean difference ≤ 1%; 95% CI, -3.8% to 3.9%; 61-96% across structures). Interprotocol kappa values (0.373-0.645) were similar to standard MRI kappa values (0.320-0.726). Frequencies of major findings on fast and standard MRI were similar (0.7-19.6% across structures; p ≥ 0.08). Sensitivities of fast MRI for tendon and labral tears (33-92%) were equivalent or higher than those of standard MRI with similar specificities (77-98%)., Conclusion: Fast 5-minute shoulder MRI with multiplanar 2D FSE sequences using parallel imaging is interchangeable, with similar interreader agreement and accuracy, with standard shoulder MRI for evaluating shoulder injuries.

Background: Intraprocedural rupture is a dangerous complication of endovascular treatment. Small ruptured anterior communicating artery (ACoA) aneurysms and microaneurysms present a challenge for both surgical and endovascular therapies to achieve obliteration. An understanding of the complication rates of treating ruptured ACoA microaneurysms may help guide therapeutic options., Objective: To report the largest cohort of ACoA microaneurysms treated with endovascular therapy over the course of the past 10 years., Methods: We performed a retrospective review of 347 ACoA aneurysms treated in 347 patients at Cleveland Clinic and Emory University over a 10-year period. Patient demographics, aneurysmal rupture, size, use of balloon remodeling, patient outcomes, intraprocedural rupture, and rerupture were reviewed., Results: Rupture rates were examined by size for all patients and subgroups and dichotomized to evaluate for size ranges associated with increased rupture rates. The highest risk of rupture was noted in aneurysms less than 4 mm. Of 347 aneurysms, 74 (21%) were less than 4 mm. The intraprocedural rupture rate was 5% (18/347) for ACoA aneurysms of any size. There was an intraprocedural rupture rate of 2.9% (8/273) among ACoA aneurysms greater than 4 mm compared with 13.5% (10/74) in less than 4-mm aneurysms. Procedural rupture was a statistically significant predictor of modified Rankin score after adjusting for Hunt and Hess grades (HH)., Conclusion: ACoA aneurysms less than 4 mm have a 5-fold higher incidence of intraprocedural rerupture during coil embolization. Outcome is negatively affected by intraprocedural rerupture after adjusting for HH grade.

Purpose: To determine whether using estimated glomerular filtration rate (eGFR) values rather than serum creatinine levels to identify patients with renal insufficiency facilitates any substantial change in the number of outpatients scheduled for computed tomography (CT) who are considered at increased risk for contrast medium-induced nephropathy., Materials and Methods: The study was HIPAA compliant and institutional review board approved for medical chart review; the requirement for informed patient consent was waived. Patients (n = 5138; 2569 women, 2569 men, 753 African Americans, 4385 non-African Americans) examined during a 2-year period formed the final study group after exclusion of patients undergoing dialysis (n = 49), for whom no age data were recorded (n = 9), and younger than 18 years (n = 113). Patient age, sex, and race and the blood urea nitrogen, albumin, and serum creatinine levels most recently measured within 6 months before CT were obtained from the electronic medical records. The number of patients with creatinine levels higher than 1.4 mg/dL was directly compared with the number of patients with eGFR values (calculated with four- and six-variable Modification of Diet in Renal Disease [MDRD] equations) lower than 60 mL/min/1.73 m(2) by using the two-tailed McNemar test. For 2689 patients, data to calculate the eGFR by using the four-variable equation were available, and for 2005 patients, data to calculate the eGFR by using the six-variable equation were available., Results: Among the outpatients scheduled to undergo CT, the percentage of patients with an eGFR lower than 60 mL/min/1.73 m(2) was significantly greater than the percentage of patients with a creatinine level higher than 1.4 mg/dL for both the four-variable (412 [15.3%] vs 166 [6.2%] of 2689 patients) and the six-variable (346 [17.3%] vs 117 [5.8%] of 2005 patients) MDRD equation groups (P < .001)., Conclusion: A significantly higher number of outpatients scheduled for contrast medium-enhanced CT met the National Kidney Foundation criteria for renal insufficiency when the MDRD equations were used to estimate the glomerular filtration rate compared with the number of outpatients who met the criteria on the basis of elevated creatinine levels., ((c) RSNA, 2008.)
