The visibility of populations, policies, and the state matters greatly for questions of power, inequality, and democratic life. This article builds on existing scholarship by examining how visibility operates as a lever and effect of social control in a racially and economically stratified society. By doing so, the article identifies a paradox. Race- and class-empowered groups often pressure state actors to implement punitive policies or otherwise visibly contain and control disadvantaged populations. But they also tend to decry and disavow the necessary public costs of these disciplinary interventions. This creates a conundrum for authorities: how to satisfy popular demands for social control while concealing resource commitments. We use the term disciplinary tensions to describe the contradictory political desires that state actors must navigate to maintain legitimacy with privileged constituents. We examine two state projects that, in different ways, crystallize this dilemma: the expansion of low-income housing development in New York in the 1960s and 1970s and state prison construction in California in the 1980s and 1990s. In both episodes, officials responded to disciplinary tensions by turning to covert public finance options: specifically, revenue bonds, which seemingly detach policy from conventional tax-and-spend public finance. We argue that these cases shed light on the shifting nature of power as finance has come to pervade all aspects of government and covert governing tactics supplement and supplant society's more direct practices of social control. Revenue bonds, in particular, allow governing actors to appease and placate the populace by reconfiguring the state's disciplinary power so that social control appears to pay for itself. This research turns the spotlight to the deregulation of once publicly funded affordable housing. Through a microsimulation that follows the conversion from affordable to market-rate units on Roosevelt Island New York, we estimate the expected demographic changes each year between 1976 and 2070. The simulation combines information from the American Community Survey, the island's masterplan, the privatization agreements, and interviews with residents, to produce interactive graphs at three urban scales: the neighborhood, the project, and the building. We found that while the households of market-rate units are gradually becoming younger and more affluent, the households of affordable units are becoming older and more impoverished. Despite an individual agreement for each building, the demographic changes are similar, and that, those changes will affect low-income buildings first. Moreover, upon expiration, 30 percent of the existing protected tenants will be over 65 and at risk of being displaced. The simulation is available at http://ridigitaltwin.pythonanywhere.com/. Background: Care transitions are commonplace for ill older adults, but no studies to our knowledge have examined the occurrence of iatrogenic harm from medication changes during patient transfer. Objectives: To identify medication changes during transfer between hospital and nursing home and adverse drug events (ADEs) caused by these changes. Methods: Participants were residents of 4 nursing homes in the New York City metropolitan area admitted to 2 academic hospitals. Nursing home and hospital medical records were reviewed to identify changes in medication regimens between sites. Medications were matched and compared regarding dosage, route, and frequency of administration. Two physician investigators used structured implicit review to identify ADEs attributable to transfer-related medication changes. Results: During a total of 122 admissions, the mean numbers of medications altered during transfer from nursing home to hospital and hospital to nursing home were 3.1 and 1.4, respectively (P<.001 for comparison). Most changes in drug use were discontinuations, followed by dose changes and class substitutions. Of 71 bidirectional transfers that were reviewed by 2 physician investigators, ADEs attributable to medication changes occurred during 14 (20%). The overall risk of ADE per drug alteration (n = 320) was 4.4% (95% confidence interval, 2.5%-7.4%). Although most medication changes (8/14) implicated in causing ADEs occurred in the hospital, most ADEs (12/14) occurred in the nursing home after nursing home readmission. Conclusions: Medication changes are common during transfer between hospital and nursing home and are a cause of ADEs. On June 7th, 2019, Layleen Xtravaganza Cubilette-Polanco, a transgender woman from New York, died at Rikers Island while in solitary confinement. According to officials from the Department of Corrections, Layleen's death was a result of a series of interrelated health problems.. Trans, queer, and gender non-conforming people, especially youth of colour and Black trans women, face heightened rates of homelessness, violent victimisation, barriers to housing, employment, and healthcare. These social conditions are tied to heightened engagement in survival sex work, over-policing, and surveillance of their communities. As a result, Trans and queer people are disproportionately impacted by mass incarceration, and must contend with the nexus of a system that functions on racist and colonial legal construction of gender, making carceral spaces sites of (in)visibility and death. The death of Trans people within these carceral spaces are followed by multiple discursive deaths, including the use of dead names in media, misgendering, and transphobic political debate. This paper aims to highlight and explore carceral settings as sites of death and (in)visbility for Trans and queer people and how reframing allows us to identify how (neo)liberal reforms meant to protect Trans people only entrench mass incarceration in the United States. Background: Hospitalization due to influenza has been stable in recent years. In March 2020, New York was an epicenter for coronavirus disease 2019 (COVID‐19). Because influenza and COVID‐19 present similarly, there were serious concerns that coinfection of these viruses would burden the healthcare system. We compared incidence and outcomes of patients hospitalized with influenza before and during COVID‐19 (seasons 2017–2021). Methods: We conducted a retrospective study evaluating hospitalized patients with influenza. Four influenza seasons were evaluated, 2017–2021, pre‐ and during COVID‐19 pandemic. We compared incidence of influenza and clinical outcomes across the seasons. Results: We found 412 patients hospitalized due to influenza in the study period; 394 had influenza, and 18 had both influenza and COVID‐19 infections. Demographics across the four influenza seasons were comparable; the cohort was predominantly female (61%) and had an average age of 60 years old. Comorbid conditions were common. No outcome differences were found for patients with influenza when comparing influenza seasons prior to and during the COVID‐19 pandemic. The mortality for the entire cohort was 6.5%. During the COVID‐19 pandemic, there were 18 (4.4%) influenza patients coinfected with COVID‐19 and 32 (7.8%) patients with bacterial super infection. Predictors of mortality in patients with influenza included presence of shock, heart failure, bacterial pneumonia, and use of mechanical ventilation. Coinfection with COVID‐19 did not increase mortality. Conclusion: We observed a significant decrease in the incidence of hospitalization due to influenza during the COVID‐19 pandemic. Clinical presentations and outcomes for patients with influenza remain stable. Being aware of possible increased mortality for patients with both influenza and bacterial pneumonia is important. Although coinfection with COVID‐19 did not increase mortality in influenza patients, identifying the specific virus responsible for infections has major therapeutic implications. Background: Diet is a modifier of metabolic syndrome which in turn is associated with World Trade Center obstructive airways disease (WTC-OAD). We have designed this study to (1) assess the dietary phenotype (food types, physical activity, and dietary habits) of the Fire Department of New York (FDNY) WTC-Health Program (WTC-HP) cohort and (2) quantify the association of dietary quality and its advanced glycation end product (AGE) content with the development of WTC-OAD.Methods: WTC-OAD, defined as developing WTC-Lung Injury (WTC-LI; FEV1 < LLN) and/or airway hyperreactivity (AHR; positive methacholine and/or positive bronchodilator response). Rapid Eating and Activity Assessment for Participants-Short Version (REAP-S) deployed on 3/1/2018 in the WTC-HP annual monitoring assessment. Clinical and REAP-S data of consented subjects was extracted (7/17/2019). Diet quality [low-(15-19), moderate-(20-29), and high-(30-39)] and AGE content per REAP-S questionnaire were assessed for association with WTC-OAD. Regression models adjusted for smoking, hyperglycemia, hypertension, age on 9/11, WTC-exposure, BMI, and job description.Results: N = 9508 completed the annual questionnaire, while N = 4015 completed REAP-S and had spirometry. WTC-OAD developed in N = 921, while N = 3094 never developed WTC-OAD. Low- and moderate-dietary quality, eating more (processed meats, fried foods, sugary drinks), fewer (vegetables, whole-grains),and having a diet abundant in AGEs were significantly associated with WTC-OAD. Smoking was not a significant risk factor of WTC-OAD.Conclusions: REAP-S was successfully implemented in the FDNY WTC-HP monitoring questionnaire and produced valuable dietary phenotyping. Our observational study has identified low dietary quality and AGE abundant dietary habits as risk factors for pulmonary disease in the context of WTC-exposure. Dietary phenotyping, not only focuses our metabolomic/biomarker profiling but also further informs future dietary interventions that may positively impact particulate matter associated lung disease. From the end of the First World War and until the 1950s, New York's Second Avenue was the world capital of the Yiddish stage. At the heart of this Jewish entertainment area that stretched from Houston to 14th Streets stood four flagship Yiddish playhouses built specifically for a Jewish clientele. Only one of the four has survived to this date: the Yiddish Art Theatre, also known as the Jaffe Theater, located on Second Avenue and 12th Street. Protected by its official interior landmark status, the building now functions a movie theatre. This essay focuses on its ornate neo- Moorish style, linking it to the style that is typical of numerous synagogues. Purpose: Taurine (2-aminoethanesulfonic acid), a molecule obtained from diet, is involved in bile acid conjugation, blood pressure regulation, anti-oxidation and anti-inflammation. We performed the first prospective study of taurine and CHD risk. Methods: We conducted a case-control study nested in the New York University Women's Health Study to evaluate the association between circulating taurine levels and risk of coronary heart disease (CHD). Taurine was measured in two yearly pre-diagnostic serum samples of 223 CHD cases and 223 matched controls and averaged for a more reliable measurement of long-term taurine levels. Results: Mean serum taurine was positively related to age and dietary intake of poultry, niacin, vitamin B1, fiber and iron, and negatively related to dietary intake of saturated fat (all p values ≤0.05). There was no statistically significant association between serum taurine levels and the risk of CHD in the overall study population. The adjusted ORs for CHD in increasing taurine tertiles were 1.0 (reference), 0.85 (95% CI, 0.51-1.40) and 0.66 (0.39-1.13; p for trend = 0.14). There was a significant inverse association between serum taurine and CHD risk among women with high total serum cholesterol (>250 mg/dL) (adjusted OR = 0.39 (0.19-0.83) for the third versus first tertile; p for trend = 0.02) but not among those with low total serum cholesterol ( p for interaction = 0.01). The data suggest a possible inverse association of serum taurine with diabetes and hypertension risk. Conclusions: The findings suggest that high levels of taurine may be protective against CHD among individuals with high serum cholesterol levels. [ABSTRACT FROM AUTHOR]

Latino subgroups in the US often shoulder a greater burden of some common diseases, including cancer. The cultural norm of patriarchy in health-related decision-making has been found to be common among Latinos, and thus male members may be important in the adoption of health practices amongst family members. Demographic information was collected from 488 male attendees (20% of the total sample) as part of a larger randomized trial focused on promoting breast and cervical cancer screening among Latinas in which attendance was open to the entire community. The majority of male attendees were over the age of 40 (62%) and originally from Mexico (39%) or Puerto Rico (25%). Approximately half of attendees reported having no health coverage and living less than 5 years in the United States. Male attendees demonstrated significant increases in knowledge relevant to their group attendance. There were few significant differences between the characteristics of male attendees at a female focused cancer program (intervention) versus more general diabetes program (control). The current study describes characteristics of a group that may be influential in guiding health behaviors and decisions. This information extends our understanding of community participation in health interventions and will assist in the development of effective interventions in the Latino community. Objective To characterise the patterns of occurrence of gestational diabetes among a wide range of ethnic groups that reside in New York City. Design Birth records and hospital discharge data were linked to more accurately assess the risk of gestational diabetes by ethnicity, compare risk in US-born to foreign-born women, and assess time trends. Setting New York City. Population All singleton live births occurring between 1995 and 2003. Methods Multivariable binomial regression analysis of ethnicity and gestational diabetes, yielding adjusted risk ratios with non-Hispanic white women as the referent. Main outcome measure Diagnosis of gestational diabetes on birth certificate or in hospital discharge. Results Adjusted relative risks (aRRs) were modestly elevated for African-Americans and sub-Saharan Africans and somewhat higher (<2.0) for non-Hispanic Caribbeans, Hispanic Caribbeans, Central Americans, and South Americans. The aRR was 4.7 (95% CI = 4.6–4.9) for South Central Asians (with an absolute gestational diabetes risk of 14.3%), 2.8 (95% CI = 2.7–3.0) among South-East Asian and Pacific Islanders, and 2.3 (95% CI = 2.2–2.4) among East Asians. Among South Central Asians, the greatest risks were found for women from Bangladesh (aRR = 7.1, 95% CI = 6.8–7.3). Foreign-born women consistently had higher risk than US-born women. Risk for gestational diabetes increased over time among South Central Asians, some Hispanic groups, and African-Americans. Conclusions Risk of gestational diabetes appears to vary markedly among ethnic groups, subject to potential artefacts associated with screening and diagnosis. Drug dependence mortality appears to be highly concentrated in certain disadvantaged populations and in certain disadvantaged areas. Using a relatively large sample of census tract data for New York City, 1991–1995 (N = 2,037), the present study examines the structural covariates of drug dependence mortality rates. Spatially lagged negative binomial regression analyses indicated considerable support for previous findings regarding the importance of poverty as a predictor of drug mortality. Furthermore, two variables especially relevant for the social disorganization and deviant opportunity perspectives in criminology exhibited significant independent effects: the neighborhood homeownership rate and the prevalence of boarded-up housing. ians have an increased susceptibility to type 2 diabetes, despite relatively low prevalence of obesity in this population. Asian American is a diverse population and there are yet limited data on the prevalence of diabetes among different Asian subgroups and existing studies are limited by small sample size. Hence, we conducted a cross-sectional survey to estimate the prevalence of diabetes and impaired fasting glucose (IFG) in this population among Chinese Americans, the largest Asian subgroup in the US. Our study population consisted 2,071 individuals (52.8% women; mean age: 52.7 ± 13.8 years and mean body mass index (BMI): 23.9 ± 3.2 kg/m) living in New York City. Data on sociodemographic factors, anthropometric measurements and medical history is obtained during a 1 day clinic visit. In addition, a fasting blood sample was collected to perform measurements on plasma glucose and lipids. Diabetes was defined as self-reported treatment or a fasting glucose ≥126 mg/dl) and IFG was defined as fasting glucose of 100-125 mg/dl. The age-adjusted prevalence of diabetes in this population was 8.6% and that of IFG was 34.6%. The prevalence of IFG/diabetes was high (38.3%) even among those with low BMI by Asian standards (<23.0 kg/m) and showed a linear increasing trend with increasing waist circumference. These data suggest a high prevalence of impaired glucose regulation in Chinese immigrants even among individuals with normal BMI. Future studies should focus on evaluating the mechanisms of increased susceptibility of IFG and diabetes in this population. [ABSTRACT FROM AUTHOR]

Background. There are apparently no published data on cancer incidence in the Puerto Rican-born populations of the northeastern United States. methods. Standardized incidence ratios (SIRs) were calculated for 1980 through 1986 for the Puerto Rican-born population in Long Island (New York). Results. Significantly reduced SIRs were found for males (SIR = 0.77) but not for females (SIR = 0.91), using expected numbers derived from incidence rates for all areas in the Surveillance, Epidemiology and End Results (SEER) Program (excluding Puerto Rico). Using incidence rates for Puerto Rico to obtain expected numbers, there was evidence for the retention of elevated SIRs for stomach cancer (both sexes) and for significantly elevated SIRs for lung cancer (both sexes), colon-rectum cancer (females), prostate cancer, and breast and uterine corpus cancer. Using rates for SEER areas, the SIRs for lung cancer approached 1.00, in contrast to other US Puerto Rican-born populations. Conclusion. The data indicate the need for surveys on smoking and other health-related behaviors in the population studied and provide further evidence for heterogeneity in cancer patterns in US Puerto Rican-born populations.
