In managing major accident hazards in industry risk, both the United Kingdom and the Netherlands use quantitative risk analysis and quantified risk criteria. These should be understood in the historical, legal, and political context in which they were drawn up. Even when criteria are numerically the same they do not necessarily mean the same thing. The legal and historical context differs widely between the two countries. Nevertheless, it is surprising that the final results in terms of land use and levels of risk are very similar. Occupational risk rates per hour of exposure have been quantified for 63 occupational accident types for the Dutch working population. Data were obtained from the analysis of more than 9,000 accidents that occurred over a period of six years in the Netherlands and resulted in three types of reportable consequences under Dutch law: (a) fatal injury, (b) permanent injury, and (c) serious recoverable injury requiring at least one day of hospitalization. A Bayesian uncertainty assessment on the value of the risk rates has been performed. Annual risks for each of the 63 occupational accident types have been calculated, including the variability in the annual exposure of the working population to the corresponding hazards. A wide variety of methods exist in the field of accident investigation. The challenge to find the cause for each and every accident has perpetuated a complicated and fundamental debate. In spite of the different paradigms, the many branch specific investigation methods, decades of accumulated and documented accident investigation experience, the diversity in taxonomy of causal factors, and an increasing depth of general systematic analysis tools, there are still causal factors missing out. A recent study identifies language issues as an underestimated danger. Dutch Labour Inspectorate records identify procedures and communication as an important causal factor area for major accidents. Accident investigation method taxonomies that are frequently used in The Netherlands, were investigated on the content related to language issues. Language issues are found to be either present less than proportional with observed accident rates or not present at all in frequently applied accident investigation classification systems.
