OBJECTIVE: To describe and compare the use of antidepressants between Black or African descent and White nursing facility residents with moderate to severe cognitive impairment. DESIGN: This was a secondary data analysis using baseline data from the Function and Behavior Focused Care for Nursing Facility Residents with Dementia randomized control trial. SETTING: Participants were recruited from 10 urban and two rural nursing facilities from Maryland. METHODS: Participants had to be 55 years of age or older, English-speaking, reside in long-term care at time of recruitment, and score a 15 or less on the Mini Mental-State Examination. A total of 336 residents participated at baseline. Data were collected by a research evaluator through observation, proxy report from staff caring for the resident the day of testing, and patient charts. MAIN OUTCOMES: A significant difference of antidepressant use between Black or African descent and White nursing facility residents with moderate to severe cognitive impairment would be noted when controlling for depression, age, gender, functional status, agitation, and number of co-morbidities. RESULTS: In adjusted models, Black or African descent residents were less likely to be prescribed antidepressants compared with White residents. CONCLUSION: Racial differences were noted regarding antidepressant use among nursing facility residents with moderate to severe cognitive impairment, but it is unknown if race could impact prescribing practices when indications for use are known. Further research is needed to ascertain if knowing the specific indications for use might contribute to racial disparities with antidepressant prescribing in nursing facility residents with moderate to severe cognitive impairment. National organizations have developed guidelines and tools for antimicrobial stewardship (AMS) in post-acute and long-term care (PALTC), but there is a need to effectively translate these into actionable, measurable, and impactful programs. An electronic needs assessment survey was developed and distributed to health care providers and administrators involved with AMS activities in PALTC facilities in Maryland. The results of this survey were used to develop a statewide initiative to improve AMS in nursing facilities. The survey revealed that barriers to implementing AMS include limited access or poor utilization of experts in AMS and infectious disease, adverse event data collection tools, and locally developed protocols and guidelines. Strategies to improve AMS included the provision of free continuing education to a multidisciplinary audience and improved access to individuals with expertise in infectious disease and the development of an adverse drug event tool. Continuing to provide meaningful tools and resources that address the specific needs of nursing facilities should lead to improved compliance with regulations and ultimately improved resident outcomes. The aim of the current study is to describe proton pump inhibitor (PPI) prescribing trends in an older adult population and elucidate perspectives regarding PPI deprescribing. A retrospective chart review and a prospective cross-sectional analysis of provider and patient surveys were conducted. The retrospective chart review identified 107 patients age ≥65 who were prescribed PPI therapy. Nineteen patients on PPI therapy and 74 providers completed surveys regarding their perspectives on PPI deprescribing. PPI therapy was potentially inappropriate for 66% of patients based on dose, duration, and/or indication. Provider barriers to deprescribing included fear of outcomes, access to documentation, and uncertainty of current guidelines. This study illustrates the prevalence of long-term PPI use in geriatric patients without associated clinical indications, as well as perceived barriers to deprescribing. Long-term PPI use is associated with significant side effects; therefore, successful deprescribing must address these perceived barriers. Medication reconciliation has been an area with increased focus among transitions of care due to associations with error rates and risk of patient harm. Chart reviews were performed to evaluate the discrepancies between the initial physician order sheet (POS), hospital discharge summary, electronic health record (EHR), health information exchange (HIE), and the patient interview/home medication list. The objectives were to determine which medication information source provided the least number of discrepancies and describe the different types of discrepancies among sources. Of all orders, 30% contained a discrepancy. The average number of discrepancies per medication source per patient included: 5.6 for the hospital discharge summary, 7.6 for the EHR, and 9.6 for the home medication list/interview. The most frequent types of discrepancies included: omission of medication orders between lists (42.7%), additional medications not included on the initial POS (24.6%), and discrepancies in frequency (11.8%). The hospital discharge summary proved to be the medication source that provided the least number of discrepancies, compared to the initial POS. [ABSTRACT FROM AUTHOR]

Purpose: The purpose of this study was to describe the first 4 months of implementation of the University of Maryland Interdisciplinary Wellness Clinic (UM-IWC). The UM-IWC provided health care services that are difficult for older adults in low-income senior housing communities to access., Design: This was a descriptive study including four low-income communities., Methods: Measures included health information, participation, and services provided., Findings: A total of 165 residents were seen (44%). The residents were mostly female (80%), Black (63%), and non-Hispanic (83%), with an average age of 77 years. We provided 253 patient visits with 98 residents seen once, 38 twice, 19 three times, and 10 four times. Visits included 165 intake assessments, 61 immunizations, 193 blood pressure checks, 28 foot care treatments, 12 cerumen removals, 12 completions of advance directives, 14 Annual Wellness Visits, and 3 home visits for acute medical problems., Conclusions: There was strong engagement of the residents, and we were able to provide a large number of services that were not easily accessible to these individuals. Future work will focus on increasing and maintaining the engagement of residents and sustainability of the clinics.

The Chesapeake Regional Information System for our Patients (CRISP) is a health information exchange in Maryland that is designed to improve communication among members of the health care team including physicians, nurses, and other health professionals. Senior care pharmacists gained access to CRISP through successful collaborative efforts at both the state and national levels. This involved efforts to address medication-related problems that continue to be a concern during transitions of care, especially for older, vulnerable adults. Having access to recent clinical data assists the senior care pharmacist in providing medication therapy management services during transitions of care-the movement of a patient from one setting of care to another.This article highlights the importance of collaboration and advocacy between state and national leadership of the American Society of Consultant Pharmacists (ASCP) with key stakeholders to negotiate and gain access to such information for senior care pharmacists working in the post-acute and long-term care settings in Maryland.
