Introduction: A popular intervention for paediatric clients, Ayres Sensory Integration® (ASI) must adhere to sensory integration (SI) fidelity. This study describes fidelity adherence in ASI intervention by occupational therapy practitioners in Malaysia. Methods: A questionnaire on ASI fidelity was developed before being tested for its validity by seven experts and for its internal consistency and test-retest reliability by 30 occupational therapists. The questionnaire was then used nationally to collect data on ASI practitioners. Data were collected from 161 occupational therapists working in various settings. Results: The mean of the sub-scales I-CVI was excellent, ranging between 0.97 and 1.00. The total S-CVI/Ave of the form was also reported as excellent, at 0.98, with subscales S-CVI ranging between 0.75 and 1.00. Cronbach's alpha was 0.80 for the questionnaire's internal consistency and the ICC for test-retest reliability ranged from 0.80 to 0.95. The survey received 161 responses, indicating that most respondents perceived themselves as having 'moderate competence' and showing 'high interest' in ASI implementation. Most fidelity aspects were addressed in their practices. Majority of the respondents adhered to the process elements. It was indicated that three aspects of physical space under the structural elements could not be provided by most occupational therapists in the study. Conclusion: To implement evidence-based practice, adherence to fidelity when providing ASI is important to ascertain its effectiveness. Improvements to ensure optimal space, ASI certification, and more related training are the first steps that can be taken by the related agencies to ensure effective ASI intervention could be delivered. Low levels of education and low income are often associated with communities located in rural areas. This is because rural communities have many constraints in obtaining education, employment opportunities, and earning a high income. This study aims to examine the factors that cause poverty in rural communities and investigate the socio-economic position and level of educational achievement of parents and children in rural communities in Bera District, Pahang, Malaysia. This study used a quantitative method for data collection. Questionnaires were administered to 100 respondents from the community living in the Bera district. Data obtained from respondents were analysed using Microsoft Excel and SPSS software through the Chi-square method. The study found that almost 51 percent of respondents agreed that the factors that drive poverty in rural families are the low education of household heads and the income earned by household heads based on their education level. In addition, a total of 59 respondents agreed with the issue of low education standards affecting the low level of income; 74 respondents agreed with the lack of employment opportunities in rural areas; and 59 respondents agreed that the education of the household heads makes it difficult for the children to get a good and higher education. As a result, the emphasis and opportunities on education, income, and employment opportunities, particularly in rural areas, must be increased more precisely. [ABSTRACT FROM AUTHOR]

Environmental degradation in estuaries may lead to a serious threat to marine life and may trigger human health risk with the help of the food web. This study was conducted to evaluate the concentration, distribution, sources, pollution status, and potential risk of contamination of five trace metals (Cd, Cu, Pb, Zn, and Fe) in estuarine surface sediment of ten different estuaries with their physicochemical parameters in the Kelantan State of Malaysia. The heavy metal concentrations in the estuarine surface sediment differ from one estuary to another following the order of: Fe (39.08%) > Zn (37.02 μg/g) > Pb (20.78 μg/g) > Cu (6.68 μg/g) > Cd (3.69 μg/g). The cluster analysis and source identification study indicated that almost all the estuaries are more or less polluted by anthropogenic activities. The Geo-accumulation index (Igeo) showed that all estuaries of the Kelantan Coastline were moderately polluted by Fe and strongly polluted by Cd which was also confirmed by enrichment factor (EF) and integrated potential ecological risk value (RI) assessment. The higher Pollution load index (PLI) values (PLI>1) revealed that all estuaries were polluted by studied trace metal contamination at a different level of magnitude. [ABSTRACT FROM AUTHOR]

Background: Dengue has emerged as a rapidly escalating health issue in low- and middle-income countries, with its burden and geographic spread increasing over the years. Malaysia, in particular, has witnessed a significant rise in dengue cases, accompanied by a spike in mortality rates. Several studies have identified various factors, primarily focusing on the 27 clinical aspects of severe dengue infection and the development of dengue-related fatalities. Expanding on this focus, this study aims to identify the demographic, clinical, and environmental factors contributing to dengue mortality, providing a more comprehensive understanding of the variables influencing dengue-related fatalities., Methods: This study utilized a 1:2 case-control design, analyzing data from the E-dengue system database and medical records from January 2015 to December 2022, involving 219 participants (73 dengue fatalities as cases and 146 recovered patients as controls). Dengue deaths were confirmed by the Penang State Mortality Review Committee, and controls were randomly selected from laboratory-confirmed dengue cases. Statistical analyses were performed using SPSS software, including descriptive statistics, chi-square tests, and multivariable logistic regression to identify predictors of dengue mortality, with variables included in the multivariable model if p < 0.05., Results: Several significant predictors of dengue-related mortality, including clinical and environmental factors were identified. Key predictors were a platelet count below 50,000/µL (OR 15.70; 95% CI: 5.65-43.53), presence of one comorbid condition (OR 2.90; 95% CI: 1.22-6.90), more than two comorbid conditions (OR 10.15; 95% CI: 3.53-29.23), bronchial asthma (OR 12.00; 95% CI: 1.08-132.13), and outbreak locality status (OR 2.3; 95% CI: 1.11-4.79). An interaction was also found between locality status and platelet levels., Conclusion: The study emphasizes the need for developing risk profiles for dengue patients by integrating factors such as platelet levels, comorbidities, and locality status to improve clinical care. Nuanced protocols are needed to address the specific challenges of single-case and outbreak areas. In single-case localities, patients with low platelet counts (below 100,000/µL) should be prioritized for rapid intervention to mitigate severe outcomes. In outbreak areas, healthcare systems should bolster resources and apply comprehensive triage approaches considering platelet levels and other risk factors. Implementing predictive models that account for geographical factors can enhance resource allocation and preparedness for dengue outbreaks.
