Electronic learning (e-learning) has become inevitable for higher education institutions during the Corona Virus pandemic. This paper presents a systematic literature review on e-learning in Malawi's higher education institutions with comparable analysis from findings in other developing countries across the globe. The aim was to highlight some benefits and challenges of implementing a sustainable e-learning programme in higher education institutions. Methods for the review included a qualitative analysis of literature and employed the Bandara et al. (2011) framework and NVIVO 10.0 for windows for data analysis and management and incorporated the Preferred Reporting Items for Systematic Reviews and Meta-Analyses to help define the question, the inclusion and exclusion criteria, and the proposed methods, including a comprehensive search strategy. Sources for the review included research articles from databases such as Sage, Emerald, Science Direct and Elsevier. The paper has unearthed several benefits of online learning especially during the coronavirus pandemic. Despite the few publications and sustainable e-learning research in Malawi, the review has found that technological, individual, financial and managerial challenges, impede the development of best practice standards for e-learning implementation. The review has suggested recommendations of a sustainable e-learning programme to provide quality learning through technology. The paper provides further ideas for developing and implementing a sustainable e-learning program to provide quality learning through technology.

Climatic variability and soil fertility decline present a fundamental challenge for smallholder farmers to determine the optimum management practices in the production of maize. Optimizing genotype (G) and management (M) of maize under different environmental conditions (E) and their interactions are essential for enhancing maize productivity in the smallholder sector of Malawi where maize is the main staple food. Here, we evaluated over seven seasons, the performance of four commercial maize genotypes [including hybrids and one open pollinated variety (OPV)] managed under different Conservation Agriculture (CA) and conventional practices (CP) across on-farm communities of central and southern Malawi. Our results revealed significant G×E and E×M interactions and showed that hybrids such as DKC 80–53 and PAN 53 outyielded the other hybrid and the OPV in most of the environments while the OPV ZM523 had greater yields in environments with above-average rainfall and shorter in-season dry spells. These environments received a maximum of 1250 mm to 1500 mm of rainfall and yet the long-term averages were 855 mm and 1248 mm, respectively. Despite yielding lower, the OPV ZM523 also exhibited higher yield stability across environments compared to the hybrid MH 30, possibly due to its resilience to drought, heat stress, and low soil fertility conditions which are often prevalent in the target communities. Conservation Agriculture-based practices outyielded CP across the genotypes and environments. However, amongst the CA-based systems, intercropping of maize with pigeonpea [Cajanus cajan (L.) Millsp] and cowpea (Vigna unguiculata Walp.) performed less than monocropping maize and then rotating it with a legume probably due to competition for moisture between the main and the companion crops in the intercrop. The key findings of this study suggest the need to optimize varietal and management options for particular environments to maximize maize productivity in Malawi. This means that smallholder farmers in Malawi should adopt hybrids and CA-based systems for enhanced yields but could also consider OPVs where the climate is highly variable. Clinical immunity against Plasmodium falciparum infection develops in residents of malaria endemic regions, manifesting in reduced clinical symptoms during infection and in protection against severe disease but the mechanisms are not fully understood. Here, we compare the cellular and humoral immune response of clinically immune (0-1 episode over 18 months) and susceptible (at least 3 episodes) during a mild episode of Pf malaria infection in a malaria endemic region of Malawi, by analysing peripheral blood samples using high dimensional mass cytometry (CyTOF), spectral flow cytometry and single-cell transcriptomic analyses. In the clinically immune, we find increased proportions of circulating follicular helper T cells and classical monocytes, while the humoral immune response shows characteristic age-related differences in the protected. Presence of memory CD4+ T cell clones with a strong cytolytic ZEB2+ T helper 1 effector signature, sharing identical T cell receptor clonotypes and recognizing the Pf-derived circumsporozoite protein (CSP) antigen are found in the blood of the Pf-infected participants gaining protection. Moreover, in clinically protected participants, ZEB2+ memory CD4+ T cells express lower level of inhibitory and chemotactic receptors. We thus propose that clonally expanded ZEB2+ CSP-specific cytolytic memory CD4+ Th1 cells may contribute to clinical immunity against the sporozoite and liver-stage Pf malaria. It is important to understand why some individuals in endemic regions acquire natural immunity against malaria while others remain susceptible. Here authors show that during episodes of Plasmodium falciparum malaria, circumsporozoite-specific cytolytic memory CD4+ T cells are clonally expanded in patients, and those with clinical immunity demonstrate reduction in the chemotactic and inhibitory receptor expression in ZEB2+ memory CD4+ T cells. This study reports onthe effect of various production steps on levels of aflatoxins during preparation of thobwa, a traditional maize‐based fermented non‐alcoholic beverage, brewed across Malawi. The effect of boiling, fermentation and their interaction on the level of aflatoxins, the trends of aflatoxin reduction during brewing, and the distribution of aflatoxins between the solid and liquid phases of the beverage were studied using VICAM AflaTest immunoaffinity fluorometric assay. Fermenting and boiling thobwa pre‐mix‐, with initial aflatoxin content of 45–183 μg/kg, resulted in aflatoxin reduction of 47% (13–61 μg/kg) on average. Fermentation and boiling contributed about 20 and 33% aflatoxin reduction, respectively, but without interactive effect between the two factors. Fermenting the thobwa for 24 h led to further reduction of aflatoxins to about 37% of the initial content, and remainedconstant for up to 8 days. Thobwa is a popular beverage in Malawi which is consumed in large quantities by all gender categories including infants, therefore the presence of aflatoxins may constitute a significant health risk factor. This study highlights the need to use raw materials with low levels of aflatoxins for production of maize‐based non‐alcoholic beverages to ensure consumer safety. [ABSTRACT FROM AUTHOR]

This study assessed the acceptability of porridge from a corn‐common bean flour blend to increase the diversity of complementary foods in Malawi. Porridges prepared using commercial corn–soybean flour (C‐CSB), homemade orange corn–soybean flour (H‐CSB), and orange corn‐common bean flour (CCBB) were evaluated by 101 pairs of mothers and their respective children aged from 6 to 24 months. A home use test (HUT) setup was used in this study, and the flours were given sequentially to participating households following a randomized complete block design. Each sample type was evaluated for 3 days in a row followed by a 1‐day break (washout period) between sample types. Based on aggregate mean scores, all the samples were liked by both the children and their mothers. However, clustering results revealed two distinct consumer segments for mothers as well as for children. Most of the mothers (59.4% in cluster 1) liked all the samples, while the minority (cluster 2) were neutral (neither like nor dislike) regarding the H‐CSB porridge. Likewise, most children (66.3% in cluster 2) liked all the samples, while the rest in cluster 1 did not like CCBB porridge. Infants (≤12 months) and those from food‐insecure households, respectively, were 5.42 and 6.75 times more likely to like the CCBB porridge than their counterparts. The study has demonstrated the potential of introducing CCBB complementary porridge in Malawi and possibly in other countries with similar food preferences and socioeconomic stature. Practical Application: The study provides a solution to the limited diversity of complementary foods in sub‐Saharan Africa and Malawi in particular. The findings can help food scientists, nutritionists, marketers, and policymakers develop strategies for promoting the consumption of orange corn‐common bean porridge. This study was aimed at examining the impacts of the Sharing economy on the individual and community Quality of Life (QOL) and wellbeing by looking at their associated influencing factors using Village Savings and Loans Associations as a model of sharing economy in Malawi. An online community-based cross-sectional study design was conducted from November 2020 through January 2021. In the survey, 402 Village Savings and Loans Associations (VSLAs) members from the Mzuzu City area participated, recruited using snowball and respondent-driven sampling techniques. The sample size was computed using a single population proportion using the Yamane formula. Descriptive statistics and ordinal logistic regression model techniques were also employed. Additionally, we used the Chi-Square test, two-way ANOVA, and Ordinal regression model to determine statistical associations between socioeconomic data and QOL and wellbeing variables with a 5% level of significance. On the aspect of community wellbeing, the findings of our study indicated that income (levels and disposal) provided members with options to live a better QOL and wellbeing within the community by either facilitating payment for better education, eating healthier foods, acquiring assets, etc. Further, the absence of discrimination provided a platform for voice, inclusion, and social trust, enhancing freedom of expression. We also found that education facilitated better earnings and knowledge of public health-related issues. As for the contribution to the sharing economy, our study has emphasized the role played by trust in enhancing sharing economy. We recommend and encourage people to join these VSLAs so as to improve their QOL and wellbeing. However, there is a need to replicate the study on a larger scale to validate our study findings for effective policy formulation and implementation geared to improving the overall quality of people's lives. Based on these findings, we further recommend that authorities reinstate programs like National Strategy for Financial Inclusion 2016–2020 and Savings and Loan Groups Best Practice Guidelines (SLG BPGs) 2016–2017 that could further enhance the future of VSLAs, which are vital for QOL and community wellbeing of the people in developing countries like Malawi. The study examined the information behaviour of Mzimba prison inmates focusing on the research themes which are information needs, sources of information, and the barriers to seeking and using information of Mzimba prison inmates. A qualitative approach was used coupled with a case study design. Data were collected through focus group discussions and interviews from 12 inmates and two prison teaching staff respectively. The study found that health information, education information and spiritual information are the major information needs of Mzimba prison inmates. The study also found that the majority of respondents agreed that the six popular sources of information are their friends, teachers, radio, television, books and newspapers. However, the study concludes that Mzimba prison (library) fails to fulfil its role as a source of information and in meeting the information needs of inmates due to challenges of lack of information resources, limited time available for inmates to search for information, poor services and lack of funding for the school and library. The study recommends that the Malawi Prison Service should engage some stakeholders such as the Malawi National Library Services, National Initiative for Civic Education and Mzuzu University Library and Learning Resources Centre to support prison libraries with the provision of information resources to meet the information needs of inmates. The study further recommends that the Malawi Prison Service Command should lobby for financial support in order to hire qualified and capable librarians and teachers to manage prison libraries and schools. BACKGROUND Undernutrition remains a challenge in countries like Malawi. The search for effective nutrition interventions to address this challenge is ongoing. In this study, the sensory profiles of six peanut‐based beverage prototypes were determined. Three of the prototypes had barley malt in their formulations, and the other three had sorghum malt in their formulations. Nutrient profiles were determined using official standard analytical methods, and the results were compared with daily nutrient requirements to estimate their nutritional value. RESULTS: All the six prototypes were acceptable, with mean overall liking scores ranging from 7.0 to 7.6. Prototypes containing sorghum malt were perceived as tasty, creamy, thick, and whitish, whereas those containing barley malt were perceived as sweet, watery, and brown. These terms, except whitish, had a significant impact (P < 0.05) on the overall liking mean score. The formulation incorporating sorghum malt (F‐B) had a better nutrient profile than that containing barley malt (F‐A) in terms of protein, total dietary fiber, potassium, phosphorus, calcium, magnesium, iron, and zinc. A 237 mL (8 oz.) serving of F‐B was an excellent source (% DV ≥20) of high‐quality protein, total dietary fiber, phosphorus, calcium, molybdenum, and manganese, and a good source (10 ≤ % DV < 20) of potassium and magnesium. CONCLUSION: Based on consumer acceptability, both formulations can potentially increase peanut consumption in Malawi. Given its nutritional quality, F‐B may help in the management of undernutrition. Although sensory appeal influences peanut consumption, peanut varieties are mostly selected based on agronomic traits. As a result, the sensory properties of peanut varieties, especially in southern Africa, are not known. Therefore, the primary objective of the study was to determine the sensory properties of the Malawi peanut varieties and the volatile compounds associated with roasted peanut flavor. Six dominant Malawi peanut varieties (Chalimbana, CG7, Nsinjiro, Kakoma, Baka, and Chitala) were evaluated in this study. All peanut samples were shelled and then, roasted in a convection oven to reach medium doneness as indicated by the surface color lightness (L) value of approximately 50. A hybrid descriptive analysis (DA) was done to determine the sensory profile of the roasted peanuts. Volatile compounds were extracted from equilibrated ground peanut sample using headspace‐solid phase microextraction technique and analyzed by GC‐MS. Analysis of Variance (ANOVA) of the DA data showed significant differences (P < 0.05) in the sensory profiles of the peanut varieties. Nsinjiro and Baka had a significantly higher intensity of roasted peanutty aroma and flavor (P < 0.05). The GC‐MS results showed that pyrazines and furans were the dominant volatile compounds but, their respective concentrations, in the evaluated peanut varieties, were significantly different (P < 0.05). Among the pyrazines, 2‐ethyl‐3,5‐dimethyl pyrazine was strongly correlated with roasted peanutty flavor (r = 0.927) just like 2,5 dimethyl pyrazine (r = 0.916). Therefore, 2‐ethyl‐3,5‐dimethyl pyrazine and 2,5‐dimethyl pyrazine production pathways could provide more insights into the origins of roasted peanut flavor. Practical Application: The findings of this study can help food product developers, who have no access to sensory and analytical analyses, to identify Malawi peanut varieties that are suitable for various food applications. The challenges of climate change, food insecurity and land degradation have all led to a push for 'scaling' innovation for sustainable agriculture. For this purpose, international agricultural development projects often use farm trials or farmer field schools as a way for farmers to engage with technically-constructed knowledge and empirical evidence. However, the role of such trials in the socio-political construction of knowledge is often overlooked. This study conceptualises agricultural development interventions as taking place within an interaction space between researchers and farmers. Unpacking the processes and dynamics of the interaction space from four case studies across Malawi and mainland Tanzania, we present findings which evaluate: 1) how agricultural innovation takes place in the context of funded agricultural development projects, and 2) how space for technical and social knowledge construction can be opened up or closed down in these contexts. Results show that farm trials provide a basis for interaction, but that knowledge exchange in these contexts also require knowledge brokers for successful implementation and scaling. Both knowledge brokers, and the trials themselves shape social dynamics, often simultaneously facilitating social learning for some, but contributing to social exclusions for others. A strong connection was identified between the design of the interaction space and social dynamics evident within it, indicative of the close interconnection between the processes of socio-political and technical construction of knowledge. Key factors open or close the interaction space , such as the continuity of knowledge brokers and the complexity of technologies. Background: In developing countries like Malawi, further investigation is rare after patients with chronic cough test negative for tuberculosis. Chronic airways disease has presentations that overlap with tuberculosis. However, chronic airways disease is often unrecognised due to a lack of diagnostic services. Within developing countries, referral systems at primary health care level are weak and patients turn to unskilled informal health providers to seek health care. Delayed diagnosis and treatment of these diseases facilitates increased severity and tuberculosis transmission. The World Health Organisation developed the Practical Approach to Lung Health strategy which has been shown to improve the management of both tuberculosis and chronic airways disease. The guidelines address the need for integrated guidelines for tuberculosis and chronic airways disease. Engaging with informal health providers has been shown to be effective in improving health services uptake. However, it is not known whether engaging community informal health providers would have a positive impact in the implementation of the Practical Approach to Lung Health strategy. We will use a cluster randomised controlled trial to determine the effect of using the two interventions to improve case detection and treatment of patients with tuberculosis and chronic airways disease.Methods: A three-arm cluster randomised trial design will be used. A primary health centre catchment population will form a cluster, which will be randomly allocated to one of the arms. The first arm personnel will receive the Practical Approach to Lung Health strategy intervention. In addition to this strategy, the second arm personnel will receive training of informal health providers. The third arm is the control. The effect of interventions will be evaluated by community surveys. Data regarding the diagnosis and management of chronic cough will be gathered from primary health centres.Discussion: This trial seeks to determine the effect of Informal Health Provider and Practical Approach to Lung Health interventions on the detection and management of chronic airways disease and tuberculosis at primary care level in Malawi.Trial Registration: The unique identification number for the registry is PACTR201411000910192--21 November 2014. We measured enteric parasitic infection prevalence and the effect of treatment on human immunodeficiency virus (HIV) RNA levels to assess their importance to HIV primary care in resource-limited settings. Adults in Lilongwe, Malawi, were evaluated, treated, and followed-up for parasitic and HIV infections. Of 389 patients, 266 (68%) were HIV infected. Helminth infections were more common in HIV-uninfected than in HIV-infected patients (39% vs. 17%). Among HIV-infected patients, helminth infections were associated with higher CD4 cell counts but not with higher HIV RNA levels. Successful treatment of parasitic infections had no effect on HIV RNA levels. Although common, parasitic infections did not impact HIV RNA levels. [ABSTRACT FROM AUTHOR]

Introduction: Chronic cough is a distressing symptom and a common reason for people to seek health care services. It is a symptom that can indicate underlying tuberculosis (TB) and/or chronic airways diseases (CAD) including asthma, chronic obstructive pulmonary disease (COPD) and bronchiectasis. In developing countries including Malawi, provision of diagnostic services and clinical management of CAD is rudimentary, so it is thought that patients make costly and unyielding repeated care-seeking visits. There is, however, a lack of information on cost of illness, both direct and indirect, to patients with chronic cough symptom. Such data are needed to inform policy-makers in making decisions on allocating resources for designing and developing the relevant health care services to address universal coverage programmes for CAD. This paper therefore explores health seeking costs associated with chronic cough and explores information on usage of the coping mechanisms which indicate financial hardship, such as borrowing and selling household assets. Methods: This economic study was nested within a community-based, population-proportional cross-sectional survey of 15,795 individuals aged 15 years and above, in Dowa and Ntchisi districts. The study sought to identify individuals with symptoms of chronic airways disease whose health records documented at least one of the following diagnoses within the previous year: TB, Asthma, COPD, Bronchitis and Lower Respiratory Tract Infection (LRTI). We interviewed these chronic coughers to collect information on socioeconomic and socio-demographic characteristics, health care utilization, and associated costs of care in 2015. We also collected information on how they funded their health seeking costs. Results: We identified 608 chronic coughers who reported costs in relation to their latest confirmed diagnosis in their hand-held health record. The mean care-seeking cost per patient was US$ 3.9 (95% CI: 3.00–5.03); 2.3 times the average per capita expenditure on health of US$ 1.69. The largest costs were due to transport (US$ 1.4), followed by drugs (US$ 1.3). The costs of non-medical inputs (US$ 2.09) was considerable (52.3%). Nearly a quarter (24.4%) of all the patients reportedly borrowed or/and sold assets/property to finance their healthcare. CCs with COPD and LRTI had 85.6% and 62.0% lower chance of incurring any costs compared with the TB patients and any patients with comorbidity had 2.9 times higher chance to incur any costs than the patients with single disease. COPD, Bronchitis and LRTI patients had 123.9%, 211.4% and 87.9% lower costs than the patients with TB. The patients with comorbidity incurred 53.9% higher costs than those with single disease. Conclusions: The costs of healthcare per chronic cougher was mainly influenced by the transport and drugs costs. Types of diseases and comorbidity led to significantly different chances of incurring costs as well as difference in magnitude of costs. The costs appeared to be unaffordable for many patients. Maize-based stiff porridge, a starchy protein-deficient staple food, dominates among the populations in sub-Saharan Africa (SSA). Unfortunately, this is often consumed along with leafy vegetables since the majority of the population in this region lack resources for the purchase of high protein animal source foods, a situation that exacerbates protein-energy malnutrition. Considering this, the current study evaluated the effect of enriching maize-based stiff porridge with flour made from hydrothermally treated soybeans on consumer acceptability. A total of nine experimental flours were prepared from maize and maize-soybean mixtures following a 32 factorial design involving two factors, namely maize flour type (whole maize, non-soaked dehulled maize, and soaked dehulled maize) and soybean flour proportion (0%, 20%, and 30%). A total of 125 adult consumers from a rural setting in Malawi evaluated maize-based stiff porridges made thereof using a 7-point hedonic scale. Subsequently, the participants were asked to guess an ingredient that was added to some of the test samples. The 10% and 20% soybean-enriched maize-based stiff porridges scored 5/7 and above, with some being statistically similar to plain maize-based stiff porridges. No participant recognized that soybeans were incorporated into the maize-based stiff porridges. The study has clearly demonstrated the potential of enriching maize-based stiff porridge with hydrothermally treated soybeans without compromising consumer acceptability. Peanuts are a valuable source of nutrients, but peanut consumption patterns, consumption frequencies, and the factors influencing peanut consumption in Malawi are not known. This study surveyed consumers to fill this knowledge gap and to assess Malawian consumers’ readiness to try new food products. Out of the 489 respondents surveyed, all but three consumed peanuts (in any form). The majority (70.4%) consumed peanuts at least three times in a week. Chi-square test showed that demographic and socioeconomic variables had significant effects (p < 0.05) on peanut product preferences, the frequency of peanut consumption, and readiness to try new foods. For instance, women mostly preferred peanut flour compared to men, and peanut butter was the most preferred form for younger consumers. Logistic regression analysis showed that consumers with high school education or below were 2.35 times more likely to eat peanuts more often than consumers with post high school education. Among the participants that were ready to try new foods (54%), men and those with post high school education were 1.90 and 2.74 times more likely to try new foods than their respective counterparts. In general, the diversity of peanut products on the Malawian market is limited, and socioeconomic restrictions override consumer preferences. This study assessed the ethanol and methanol contents of homemade spirit (Kachasu) sold in Blantyre, Malawi. The likelihood of ethanol and methanol toxicity, respectively, was determined through Monte Carlo simulations using reported Kachasu intake volumes of 21 consumers and the determined methanol and ethanol contents. Ethanol concentration, in samples from 20 different distillers, ranged from 11 to 55% v/v. Methanol was detected in 10 of the 20 samples (0.01-0.28% v/v). The likely mean ethanol intake of drinkers in Blantyre was found to be 214 ± 93 mL per day (90% CI, 68.9-373.4 mL), and mean methanol intake was 0.44 ± 0.37 mL (90% CI, 0.03-1.17 mL). The intake values translated to mean blood ethanol and methanol concentrations of 38 ± 16 mg/mL and 0.05 ± 0.04 mg/mL, respectively. Therefore, the risk of methanol toxicity was considered as negligible. However, there was a high risk of ethanol toxicity. Since production and selling of Kachasu are already illegal in Malawi, enforcement of regulations should be strengthened to reverse the current situation where Kachasu is being distilled and sold openly even within cities. Consumers should also be sensitized about the likely risks associated with consumption of Kachasu in Malawi so that they can make informed choices. [ABSTRACT FROM AUTHOR]

Abstract: In Malawi and throughout much of Africa, maize yields have declined over the past several decades due to continuous cultivation, often in monocropping with little or no inputs. As a result, soil degradation has been aggravated by the loss of valuable top soil caused by rainwater runoff due to the absence of effective conservation practices. To combat this trend, Conservation Agriculture (CA) systems were introduced using a pointed stick or hand hoe to plant directly into untilled soil with crop residues as surface mulch. The objective of this study was to compare the effects of different cropping systems (CA and conventional) on soil physical and chemical parameters and long-term maize productivity in target communities of the southern and central regions of Malawi. This study analysed the effects of CA on soil parameters and maize yield over eight cropping seasons. The biophysical variability of the communities was explored through principal component analysis. Results showed that maize yields in CA systems were strongly affected by rainfall infiltration, which was 24–40% greater compared with the conventional ridge and furrow system. In some cases, maize yields in CA plots were double that of conventional tillage plots. The larger water infiltration observed in CA plots relative to conventional tillage indicated that CA systems may increase access to soil water by the crop and offset the negative effects of seasonal dry spells. Yield benefits of CA over conventional tillage systems were greater especially from the 5th season although, in some instances, greater yields on CA were recorded almost immediately. CA can be practiced in diverse environments from sandy to clay soils, nutrient rich to infertile soils and from low to high rainfall areas as long as adequate inputs (fertilizer, herbicides and labour) are available with good extension support to farmers, especially in the initial years.
