1. A Multi-Center, Epidemiologic Survey of the Current Medical Practice of General Practitioners Treating Subjects with Type 2 Diabetes Mellitus in Latin America.

The study was designed to evaluate how diabetes is treated in Latin America by general practitioners, and whether the prescribed treatment is successful in controlling the disease. A multicenter, cross-sectional, epidemiological survey was conducted in 9 Latin American countries. General practitioners from private settings were asked to recruit patients (18-75 years) with type 2 diabetes mellitus and to collect demographic data, medical history, antidiabetic medication, laboratory exams and information on the challenges of patient management. 3 592 patient questionnaires were returned by 377 physicians. The majority of patients had a family history of diabetes, were overweight, followed a poor diet and were sedentary. Poor glycemic control (fasting blood glucose ≥ 110 mg/dL) was observed in 78% of patients. The number of patients with HbA1c < 7.0% was 43.2%, while 30.0% had HbA1c < 6.5%. Glycemic control decreased significantly with increased duration of T2DM. Co-morbid conditions associated with type 2 diabetes were observed in 86% of patients. The incidence of co-morbid conditions increased significantly with increased duration of the disease. The incidence of conditions associated with microvascular complications increased significantly with the duration of the disease. Insulin use increased with duration of the disease. Ensuring compliance with diet and exercise were most often cited as patient management challenges. Blood glucose levels are not well controlled in type 2 diabetes mellitus patients in Latin America, particularly in those with longer duration of the disease.
