Summary. A total of 65 equine group A rotaviruses (GAR) isolated from diarrheal foals at 48 farms in Hokkaido, Japan, between 1996 (29 isolates) and 1997 (36 isolates) were characterized for their VP7 and VP4 serotypes by PCR, nucleotide sequencing, and virus neutralization (VN) tests. By PCR VP7 typing, all isolates were classified as G3 or G14, and the predominant serotype in each year was G3 (86%) in 1996 and G14 (53%) in 1997. VN tests with these 20 isolates randomly selected confirmed the specificity of PCR on the bases of complete agreement of the results in these methods (9 G3 and 11 G14), and revealed that all 9 G3 isolates were subtype G3B. There were five differing amino acid residues in three VP7 antigenic regions between subtypes G3A and G3B. Antiserum to a baculovirus recombinant that expressed P[12] VP4 neutralized all isolates and P[12] reference strains. These results suggest that genotype P[12] GAR belong to a single VP4 serotype, and that one VP4 and two VP7 serotypes (G3B and G14) of GAR were predominant in the equine population in Japan. We revisited the anatomo-functional characteristics of the basal temporal language area (BTLA), first described by Lüders et al. (1986), using electrical cortical stimulation (ECS) in the context of Japanese language and semantic networks. We recruited 11 patients with focal epilepsy who underwent chronic subdural electrode implantation and ECS mapping with multiple language tasks for presurgical evaluation. A semiquantitative language function density map delineated the anatomo-functional characteristics of the BTLA (66 electrodes, mean 3.8 cm from the temporal tip). The ECS-induced impairment probability was higher in the following tasks, listed in a descending order: spoken-word picture matching, picture naming, Kanji word reading, paragraph reading, spoken-verbal command, and Kana word reading. The anterior fusiform gyrus (FG), adjacent anterior inferior temporal gyrus (ITG), and the anterior end where FG and ITG fuse, were characterized by stimulation-induced impairment during visual and auditory tasks requiring verbal output or not, whereas the middle FG was characterized mainly by visual input. The parahippocampal gyrus was the least impaired of the three gyri in the basal temporal area. We propose that the BTLA has a functional gradient, with the anterior part involved in amodal semantic processing and the posterior part, especially the middle FG in unimodal semantic processing., (© The Author(s) 2024. Background: Helicopter emergency medical services' (HEMS) effectiveness for pediatric trauma patients remains unclear. We aimed to examine the relation between HEMS and reduced mortality in pediatric trauma patients., Methods: This retrospective cohort study utilized data from the Japan Trauma Data Bank, a national multicenter clinical trauma database. Participants were aged <18 years, admitted between 2004 and 2015, and transported from the scene to the hospital by HEMS or ground emergency medical services (GEMS). We used a standardized mortality ratio (SMR) weight method, and fitted a marginal structural model to adjust for measured confounders. The SMR weight was calculated using the estimation of the propensity scores. A logistic regression model was used with the baseline independent variables to estimate the propensity score., Results: Overall, 5,947 patients were identified in our study: 453 were transported by HEMS and 5,494 by GEMS. The mean injury severity score in the HEMS group was significantly higher than that in the GEMS group17.0 (Standard deviation = 11.0) vs 12.2 (Standard deviation = 9.2), p < .001. In-hospital mortality was higher in the HEMS group than that in the GEMS group in the unadjusted analysis (3.8% vs 1.3%, respectively; p < .001). After adjusting for covariates, HEMS transport was not associated with reduced hospital mortality. (odds ratio = 0.82, 95% confidence interval = 0.42-1.58)., Conclusions: HEMS was not associated with reduced mortality among pediatric trauma patients compared with GEMS in this nationwide study. We conducted a survey of 111 healthy middle-aged and older women, aged 40 to 80 years, to elucidate the relationships between blood leptin and adiponectin levels and bone mineral density (BMD) in September 2008. We examined the associations between the blood levels of these adipocytokines and the lumbar spine (L2-L4), left femoral, and distal forearm BMD after adjustment for the age and body fat percentage. No correlation was observed between leptin and the BMD. This is presumably due to the fact that the influence of menopause, age, duration of postmenopausal period, and body fat percentage on the BMD is greater than that of leptin levels in healthy women. The adiponectin levels were significantly correlated negatively with the lumbar spine and femoral BMD in premenopausal women, and with forearm BMD in postmenopausal women, regardless of adjustment for age and body fat percentage. It seems likely that adiponectin influences the BMD at different skeletal sites in pre- and postmenopausal women.

Objective: To study the incidence and clinical features of acute juvenile female non-herpetic encephalitis (AJFNHE) in Japan., Methods: A nationwide questionnaire on patients with severe non-herpetic encephalitis of unknown etiology with a prolonged clinical course or death was sent to the departments of Internal Medicine, Neurology, Pediatrics, and Emergency and Critical Care at all hospitals with 200 beds or more in Japan., Results: The recovery rate was 25% (1,279 out of 5,030 departments) and 90 patients were enrolled in this study. The annual incidence was 0.33/10(6) population. 85% of patients were female. The means and standard deviations of age at onset and hospital stay were 26+/-10 years and 180+/-228 days. As first symptoms, fever and psychosis were presented in 90%. Among the neurological symptoms, disturbance of consciousness was presented in 92%, convulsions in 65%, and involuntary movements in 55%. Respiratory failure during hospitalization was observed in 71% and required care with mechanical ventilation. The detection rate of anti-GluR epsilon2 and/or delta1 antibodies was 67% of patients. Anti-N-methyl-D-aspartate receptor NR1/NR2 antibody was detected in all four examined patients with anti-GluR epsilon2 antibody, and also detected in both of the two examined patients without anti-GluR epsilon2 antibody. As for outcome, 46% returned to work and 37% returned home, but 7% died. Associated tumors were demonstrated in 39%. All reported patients had ovarian tumors, among which teratoma was the most frequent., Conclusion: A nationwide survey provided data for the annual incidence and clinical features of AJFNHE in Japan.

We reported a 62-year-old man of late-onset familial amyloid polyneuropathy type I(transthyretin Met 30-associated familial amyloid polyneuropathy) from Ehime Prefecture. There was no family history related to endemic Japanese foci (Nagano and Kumamoto foci). He demonstrated paraesthesia in the legs and mild autonomic symptoms at the age of 52. These symptoms gradually developed. Analysis of the transthyretin gene from his leucocytes demonstrated he had Met 30 transthyretin mutation. Therefore, he was diagnosed with late-onset familial amyloid polyneuropathy type I(FAP 1). In some families, asymptomatic carriers with the mutant transthyretin gene were diagnosed. In early stage, this patient's polyneuropathy and autonomic nervous system dysfunction were less serious than those of FAP 1 patients from endemic Japanese foci. These symptoms of this patient was slowly progressive. He hoped liver transplantation (brain death or living-related) treatment if possible. Now he became 68-year-old and bed-ridden.

Aims/methods: To investigate whether smoking affects the serum level of leptin, 708 male workers aged 25-65 years old were cross-sectionally surveyed., Results: Multiple regression analysis indicated that among the various parameters examined, the level of leptin was positively associated with the body mass index and the levels of insulin, total cholesterol and uric acid, and was inversely associated with physical activity and the level of creatinine. The partial correlation coefficient of leptin was highest against the body mass index (r = 0.40), followed by insulin (0.29) and physical activity (-0.14), after adjustment for other leptin-related variables. However, no association was observed between the level of leptin and smoking (0.05), alcohol consumption (0.09) or age (0.09)., Conclusions: The findings suggest that among life-style habits, physical activity, but not smoking or alcohol consumption, significantly affects the serum level of leptin in Japanese men., (Copyright 2001 S. The relationship of fat mass, lean body mass (LBM), and fat distribution to blood pressure was examined in Japanese adult men and women. Percent body fat was estimated using two skinfold thicknesses, and fat mass and LBM were then calculated. Correlation coefficients showed that fat mass, fat distribution, and age were weakly associated with blood pressure levels, and that the correlation coefficients of LBM to blood pressure levels were nearly zero. Stepwise regression analysis revealed that fat mass and age contributed significantly to the variations in blood pressure levels. In addition, only in the case of men, trunk-extremity skinfolds ratio was also entered into the model for diastolic and mean arterial blood pressure. It thus has been concluded that fat mass may be the more predictive determinant of blood pressure level than LBM in the Japanese population, and that the independent correlation of fat distribution to blood pressure is rather small.

Whole body counting of potassium-40 and anthropometric measurements were done on 88 urban and rural Japanese boys 12 to 14 years of age. The log-normal distribution provided a good fit to the observed data of potassium content in both groups. The urban and rural boys did not differ significantly in age, weight, body and sitting height, chest and abdomen circumferences, antero-posterior diameter of chest and abdomen, and grip strength. On the other hand, the mean potassium content, K/body weight, K/fat-free mass and estimated upper-arm muscle circumference were significantly greater in rural boys. Skinfold thickness and percentage fat were significantly greater in urban boys. In a regression analysis estimating whole body potassium from anthropometric variables, body weight and skinfold thickness were the most significant variables, accounting for 85.2% of the total variation. Estimation errors were smaller in the rural than in the urban group. Factor analysis was used to identify the factors which could explain the items measuring body composition and strength. Factor 1, in which potassium content and grip strength are the most important items, was designated as the 'Muscular Factor', and Factor 2, in which skinfold thickness and upper-arm circumference are the most significant items, was designated as the 'Fatness Factor'. The factor scores of rural subjects were scattered in a considerably narrower range than those of urban subjects.

Since 1971, stature and body weight in five siblings have been measured monthly. Time-series analysis of stature, body weight and their increments per month was made by the Program of Census Method IIX11 resulting in three components: a trend-and-cycle factor, a seasonal factor and an irregular factor. Significant seasonal variation was found in both stature and body weight. In two young subjects, trend curves of stature are very close to each other while those of body weight are different. Trend factor in increment indicates that growth rate of stature or body weight fluctuates, instead of being smooth, suggesting that from birth to maturity acceleration and deceleration occur alternately, like repeated retardations and subsequent catch-ups. A high peak of body weight increment precedes menarche by 1 1/2 years in two girls. In one case, no adolescent spurt in stature was observed while a high peak appeared in body weight in that period. Irregularity in stature is about one-tenth that of body weight when the two are compared in terms of the ratio of SD of irregular factor/mean of trend factor. The raw data are given in an appendix.
