The article focuses on the disease leishmaniasis, whose case has been reported from southern parts of Italy. In Italy, leishmaniasis is endemic in the southern regions, with Leishmania infantum accounting for most of the clinical cases. An unusual case of visceral leishmaniasis that occurred in a nonendemic area after a protracted incubation period, has been discussed in the article. The complete chronology of the treatment carried out in the illustrated case has been included in the article. This case serves as a reminder that visceral leishmaniasis must be suspected even in nonendemic areas. It also demonstrates that experience with both bone marrow examination and specific antibody tests is essential for an accurate diagnosis.
