Simple Summary: Pazopanib is an oral drug for metastatic pretreated uterine leiomyosarcoma that received approval in 2012, but poor data have been reported on its activity in real life since the disease is very rare. Uterine leiomyosarcoma has a poor objective response rate to other agents. We assessed the effectiveness and safety of pazopanib in everyday clinical practice, showing its activity and tolerability in patients. Background: Uterine leiomyosarcoma (uLMS) is characterized by aggressive behavior associated with a high risk of relapse and mortality. Several therapeutic agents have been employed in the treatment of metastatic disease, with a poor objective response rate. Pazopanib, approved in 2012, is a multi-targeted, orally active small molecule that exerts its effects by inhibiting several tyrosine kinases. To date, poor research on real-life data has been conducted. We aimed to assess the effectiveness and safety of the drug in everyday clinical practice. Methods: We present results of multicenter retrospective data on 38 patients with heavily pretreated metastatic uLMS who underwent oral pazopanib during their therapeutic journey. Results: At a median follow-up of 8.6 months, the disease control rate was 55.2%, with 17% partial responses and 15 patients (39.5%) with stable disease. At a median follow-up of 8.6 months, median progression-free survival was 4 months, and median overall survival was 19.8 months. The most common grade 3 adverse events (AEs) drug-related were hepatic toxicities, diarrhea, hypertension, nausea, and vomiting (all of them with an incidence of 5% considering the whole study cohort). No grade 4 AEs occurred. Conclusions: Pazopanib in everyday clinical practice is safe and shows a good disease control rate with prolonged survival. [ABSTRACT FROM AUTHOR]

The intercalibration exercise is an important step in the building process of the surface water ecological quality assessment, which is required by the Water Framework Directive (WFD). Its aim is to apply the water quality classification in a uniform manner to all the Member States belonging to the same eco-region. Cyprus, France, Greece, Italy, Slovenia and Spain participated in the soft-bottom benthic invertebrate subgroup for the Mediterranean coastal region. The methodologies proposed by Member States were applied and tested; the results were compared and harmonized to establish agreed and comparable boundaries for the benthic invertebrate ecological status classes. The national methods used in the intercalibration process were: for Cyprus and Greece, the Bentix Index; for Slovenia, a combination of AZTI Marine Biotic Index (AMBI), richness and diversity with the use of factor and discriminant analysis (Multimetric AMBI); for Spain, a new index, named MEDOCC, which is an adaptation of the AMBI index to the Western Mediterranean area. Italy and France tested different methods, none of which have been officially adopted. Final class boundary values for the different official classification systems were obtained and compared. Besides describing methods and results obtained by the different countries, the Italian situation is examined in more detail. All the above methods have been applied to Italian data, but the results were not conclusive. Major causes for concern are related to insufficient sites and data, to the lack of real non-impacted reference sites, and to the difficulties in validating the ecological status classification in sites not showing a pollution gradient. New legislation for the protection of inland surface waters, transitional waters, coastal waters and ground waters has recently been established in Italy. This law presents a new integrated approach, where all water bodies are considered as complex ecosystems to be studied in each of their components. The new concept of environmental quality of the water body, based on the ecological and chemical status, is also introduced. At the same time (i.e. the end of 2000), the European Community approved the European Water Framework Directive based on the same basic environmental concepts and criteria of the new Italian law. This paper analyses the important points and innovations required by the new Italian legislation for monitoring and classification of marine coastal waters. Details of definitions, parameters, analysis and monitoring programs are discussed. A comparison with the European Water Framework Directive is eventually given, underlining the specific characteristics of the Mediterranean sea, which have to be taken into consideration when applying the European Directive to this particular ecoregion. The accumulation of Posidonia oceanica dead leaves on the beaches of the Mediterranean shores is a natural phenomenon. They are either temporary or permanent structures (banquettes) and represent a valuable resource, with important ecosystem functions including coastal protection against erosion. Nevertheless, the perception of these plant accumulations by the different stakeholders (beach managers, local administrations and tourists) is often negative; they consider these deposits a malevolent waste to be removed, rather than a natural and valuable component of the coastline. We propose an integrated/beneficial management model for posidonia deposits, called ECOLOGICAL BEACH, firstly proposed in France, and recently implemented and applied in Italy. The model promotes the preservation of posidonia beach casts on site, with a balanced coexistence of natural and anthropic elements. The model fosters the several important ecosystem services of the beach casts and contributes to coastal preservation. To successfully spread the model, several activities must be implemented: a regulatory framework, the collection of data about the occurrence of beach casts, management protocols and educational programs. The most important activity is the educational one, based on the dissemination of the ecological and economic value of the beach casts, aimed at switching the perception of this phenomenon towards positive appraisal. Sanitaryware products are one of the most diffused ceramic materials. In the present study, an environmental assessment of sanitaryware production was carried out. The Life Cycle Assessment was performed with a cradle-to-gate approach, considering the product life cycle from resource extraction to the factory gate of an Italian company and it aims to evaluate the environmental weight of sanitaryware production, identifying hotspots and comparing different energetic scenarios. The study was performed according to the current international standards and choosing, as the functional unit, 1000 kg of heterogeneous sanitaryware products. The system boundary includes raw materials extraction, transportation of raw materials and all the manufacturing activities within the factory, including electric and thermal energy supply. The results show how the study factory have implemented a mature green-based manufacturing and represents a reference point to estimate how the environmental impact can be improved by energy saving technologies and recycling water. Such technologies were compared to conventional ones and with results of other recent literature research. In addition, a potential improvement, based on cogeneration, was evaluated to consider if the economic improvement goes together with a minor environmental impact. Results show how economic benefit due to the implementation of cogeneration agrees with environmental impact, presenting only an increase in Global Warming Potential and Abiotic Depletion of fossil fuels. This is the inspiration for a further research where Life Cycle Assessment can be coupled to a Life Cycle Cost. • Environmental impacts of sanitaryware production in Italy are evaluated. • A cogeneration solution reduces environmental impacts and leads to costs reduction. • Firing and drying cause the greatest Thermal Energy consumption in the process. • Raw materials mining and transport provide the highest environmental impacts. • The order of magnitude of CO2 emission is consistent with other ceramic productions. The loggerhead sea turtle Caretta caretta has been chosen as bioindicator to monitor the amount of litter ingested by marine animals within the European Marine Strategy Framework Directive and the Barcelona Regional Sea Convention. European Member States and Contracting Parties are committed to achieve the Good Environmental Status (GES), which is reached when the quantity of ingested litter does not adversely affect the health of the species concerned. Although the monitoring strategy has been outlined for more than a decade, to date no threshold values have been adopted to verify GES achievement. A comprehensive understanding of the concentration of microplastics (MPs) in seawaters is essential to implement monitoring programs and understand the impacts on ecosystems, as required by the European legislation to protect the marine environment. In this context, the purpose of this study is to investigate the composition, quantity, and spatial distribution of microplastics from coastal to offshore areas in three Italian seawaters. In addition, the distribution of microplastics between surface and subsurface water layers was analyzed in order to better understand the dynamics of MPs in the upper layers of the water column. A total number of 6069 MPs (mean total concentration of 0.029 microplastics · m -2 ) were found to be heterogeneous in type, shape, and color. In general, MPs concentrations decrease with coastal distance, except when environmental forcings are predominant (such as sea currents). Moreover, the amount of surface MPs was almost four times that of subsurface microplastics, which consisted mostly of fibers. This study assesses microplastic ingestion in Boops boops at different geographical areas in the Mediterranean Sea. A total of 884 fish were caught at 20 coastal sites in Spain, France, Italy and Greece and analyzed using a common methodological protocol. Microplastics were found in 46.8% of the sampled fish, with an average number of items per individual of 1.17 ± 0.07. Filaments were the predominant shape type, while polyethylene and polypropylene were indicated by FTIR as the most common polymer types of ingested microplastics. The frequency of occurrence, as well as the abundance and proportion of types (size, shape, color and polymer) of ingested microplastics, varied among geographical areas. The spatial heterogeneity of the abundance of ingested microplastics was mainly related to the degree of coastal anthropogenic pressure at the sampling sites. Recently, many studies focus on the ingestion of microplastics by marine biota. Fish exploit almost every kind of marine environment, occupy many ecological niches and are an important food source for human populations worldwide. For these reasons, they seem to represent very appropriate biological indicators of microplastic ingestion. UNEP/MAP SPA/RAC (2018) identified the bogue, Boops boops (Linnaeus, 1758), as a possible target species for monitoring microplastic ingestion in fish populations. This study provides the first report of microplastic ingestion by B. boops from the Tyrrhenian and the Ligurian Seas (Western Mediterranean Sea). Generalized Linear Mixed Models were used to analyse the relationship among biological parameters and environmental factors. A total of 379 bogues were collected in three Italian regions, subject to different anthropogenic pressures (river input, human population, shipping lanes and distance from the coast). Microplastics were detected in the gastrointestinal tract of most individuals (56%) with a mean of 1.8 (±0.2) microplastics per individual. Our study further confirms that this species is able to highlight differences in the ingestion of microplastics according to local anthropization, resulting Latium region to be the most polluted. Fish with lower physical condition are more likely to ingest microplastics, suggesting a relationship with the level of local environmental contamination. Finally, the ingestion of microplastics might be influenced by behavioural differences between sexes. According to our results, males ingest significantly more microplastics than females (p < 0.05). This study analyzes microplastic ingestion by three deep-water elasmobranch species (Galeus melastomus, Scyliorhinus canicula and Etmopterus spinax) from the Tyrrhenian Sea, discriminating between stomach and intestine contents. The absence of significant differences in frequency and abundance of plastic items into stomachs seems to suggest that ecological diversity among the three sharks does not strongly influence the probability of plastic ingestion in the study area. On the other hand, the detected differences in the microplastic content into the intestine might be due to a different retention time of microplastics, suggesting how feeding habits could influence metabolic features, and therefore affect the recovery of ingested plastic items. This information would improve the future development of marine micro-litter monitoring systems, following the MSFD requirements. The assessment of human-induced pressures on the coastal area is essential to target management plans effectively, and moreover is required by the EU Water Framework Directive. A simple and cost-effective assessment of human-induced pressures on the coastal zone is applied using two methodologies: a qualitative visual assessment which uses satellite images; and a quantitative assessment based on governmental census data. These methods are applied to defined areas (23 areas) of four Italian regions: Liguria, Tuscany, Latium and Sardinia. The results show a high agreement (83%) between these two methods, in which only four of the 23 areas are classified differently. These differences may mainly be ascribed to the qualitative or quantitative properties of the methods, and to the different geographical units adopted. These characteristics however provide complementary information, which suggests that the application of both proposed methods confirms reliability and allows fine-tuning of the assessment. The pressure assessment proposed is simple, time and cost-effective, and repeatable over time and space. It therefore can be applied in different contexts to respond to legislative requirements or to target management plans and remedial actions effectively.
