Paenibacillus larvae (P. larvae) is responsible for American foulbrood (AFB), the most severe bacterial disease of honeybees. The enumeration of P. larvae spores in substrates taken from hives allows for the identification of the contamination levels of the colonies, mostly in those with atypical symptoms or with asymptomatic infections; in these cases, it is essential for the effective control of American foulbrood (AFB). In this work we described a new quantitative TaqMan® probe-based real-time PCR (qPCR) assay, targeting the 16S rRNA gene of P. larvae, used for the quantification of P. larvae spores in powdered sugar samples collected from hives, in comparison to the culture. A total of 105 colonies were selected, belonging to 10 apiaries with different levels of infection, located in northern Italy. The proportions of positive colonies was 54% (57/105) with the culture and 66% (69/105) with qPCR. A significant difference between the two methods was found with McNemar's test (p = 0.02). Out of the 51 positive samples by both methods, 45 showed higher infection by qPCR compared to the culture. A close concordance with the clinical–epidemiological status of the hives was observed by both methods, with higher infection levels found by qPCR. [ABSTRACT FROM AUTHOR]

Background & Aims: In Caucasian patients with compensated cirrhosis caused by hepatitis B virus (HBV), the risk of hepatocellular carcinoma (HCC) developing persist despite long-term nucleos(t)ide analogs (NUC) treatment. In the surveillance of this population with persistently normal transaminases because of NUCs, the added value of serum alpha-foetoprotein (AFP) monitoring is poorly defined., Methods: Two hundred and fifty-eight Caucasian HCC-free patients with HBV-compensated cirrhosis who started tenofovir or entecavir while having normal serum AFP levels (≤7 ng/mL) at baseline or within the first year of treatment underwent HCC surveillance by semiannual ultrasound evaluation and serum AFP determination., Results: During 96 (18-120) months of antiviral therapy, 3947 AFP values were collected, median AFP level was 2 ng/mL. Thirty-five patients developed an HCC at an overall 8-year crude cumulative incidence of 14% (annual incidence of 2%). HCC incidence increased in parallel with increasing AFP thresholds: 24%, 36%, 64% and 92% for AFP levels after exceeding 2, 4, 6 and 7 ng/mL for the first-time. Of the 12 patients who experienced an AFP rise > 7 ng/mL, 11 developed an HCC and one had liver metastases of lung cancer. Overall, an AFP > 7 ng/mL had 99.6% specificity, 31.4% sensitivity, 91.7% PPV, 90.2% NPV, LR+ 70.1 and LR- 0.69 for HCC; this excellent specificity was maintained up to 18 months before HCC detection., Conclusions: In Caucasian patients with HBV-compensated cirrhosis on long-term NUC, an increase in AFP over 7 ng/mL shows excellent specificity, heralding HCC development within 1 year., (© 2019 John Wiley & Sons A/S. Background and Aims: Tenofovir disoproxil fumarate (TDF) is recommended for chronic hepatitis B (CHB) treatment, but it may induce kidney dysfunction whose management is not yet known. This Italian, multicentre, retrospective study aimed to assess the efficacy and safety of switching to entecavir (ETV) patients who developed TDF-associated glomerular and/or tubular dysfunction., Methods: A total of 103 TDF-treated patients were included as follows: age 64 years, 83% male, 49% cirrhotics, 98% with undetectable HBV DNA, 47% with previous lamivudine resistance (LMV-R) and 71% previously treated with adefovir. Twenty-nine (28%) were switched to ETV because estimated glomerular filtration rate (eGFR MDRD ) was <60 mL/min, 37 (36%) because blood phosphate (P) levels were <2.5 mg/dL and 37 (36%) for both reasons. Kidney, liver and virological parameters were recorded every 4 months thereafter., Results: During 46 (4-115) months of ETV treatment, all patients' renal parameters significantly improved as follows: creatinine from 1.30 to 1.10 mg/dL (P < 0.0001), eGFR MDRD from 54 to 65 mL/min (P = 0.002), P from 2.2 to 2.6 mg/dL (P < 0.0001) and maximal tubule phosphate reabsorption (TmPO4/eGFR) from 0.47 to 0.62 mmol/L (P < 0.0001). Thirteen patients (52%) improved their eGFR MDRD class, P levels were normalised in 13 (35%), and eight (22%) showed improvements in both parameters. Viral suppression was maintained in all but five patients (5%), all of whom had been LMV-R. The 5-year cumulative probability of ETV-R was 0% in LMV-naïve patients, and 11% in LMV-R patients (P = 0.018)., Conclusions: Entecavir is an effective and safe rescue strategy for CHB patients who develop renal dysfunction during long-term TDF treatment., (© 2018 John Wiley & Sons A/S. Backgound: A significant proportion of hepatitis B surface antigen (HBsAg) negative/anti-hepatitis B core antigen (anti-HBc) positive patients with non-Hodgkin lymphoma (NHL) undergoing rituximab-based chemotherapy (R-CT) may suffer hepatitis B virus (HBV) reactivation., Aims: We wanted to assess efficacy and safety of lamivudine (LMV) prophylaxis to prevent this complication., Methods: Eighty-five consecutive HBsAg negative/anti-HBc positive NHL patients (71 years, 100% serum HBV DNA undetectable, 74% anti-HBs positive) received LMV coadministered with R-CT and for 18 months after the end of R-CT. Serum ALT, HBsAg, anti-HBs and HBV DNA were assessed every 4 months during and after end of LMV., Results: During 39 (2-108) months of study period, including 21 months of LMV and 27 additional months after LMV discontinuation, one patient (2%) had HBV reactivation, 31 months after stopping LMV and during administration of new immunosuppressive regimens, without LMV prophylaxis, owing to incomplete oncological response. A 50% decline of anti-HBs titers occurred in 22/63 (35%) patients, including 12 who became anti-HBs seronegative. Five (6%) patients had ALT increase during R-CT but none required R-CT discontinuation. Simple Summary: Hepatocellular carcinoma (HCC) is a leading cause of cancer deaths globally. Current gold-standard screening methods include imaging every six months and measuring alpha-fetoprotein (AFP) levels. This study evaluates Protein Induced by Vitamin K Absence-II (PIVKA-II) as a diagnostic biomarker for HCC and its correlation with disease stage. PIVKA-II levels progressively increase from chronic hepatitis C to liver cirrhosis and are highest in HCC patients, with levels correlating to advanced HCC stages based on the Barcelona Clinic Liver Cancer system. The optimal PIVKA-II threshold (>37 mAU/mL) achieved 80% sensitivity and 76% specificity, outperforming AFP, which had 53% specificity and 78% sensitivity at a cut-off of 5.2 ng/mL. Combining PIVKA-II and AFP improved diagnostic accuracy, achieving a positive predictive value of 73.9% and a negative predictive value of 94.2%. The findings suggest that PIVKA-II performs better than AFP alone and that their combined use enhances diagnostic accuracy, especially for advanced disease stages. Six-month imaging along with alpha-fetoprotein (AFP) serum levels detection are the current gold standard to exclude HCC. Protein induced by vitamin K absence (PIVKA-II) has been proposed as a potential screening biomarker for HCC. This study was designed to evaluate the role of PIVKA-II as diagnostic HCC marker, and the correlation between PIVKA-II levels and HCC stage. Methods: PIVKA-II levels were assessed on serum samples of Italian patients. The study population included 80 patients with HCC, 111 with liver cirrhosis (LC), and 111 with chronic hepatitis C (CHC). Results: PIVKA-II serum levels progressively increase from patients with CHC to patients with HCC. In the HCC group, PIVKA-II values are higher in the more advanced stages of the disease, assessed by the Barcelona Clinic Liver Cancer (BCLC) staging system (BCLC-B vs. BCLC-A vs. BCLC-0). Youden's index analysis identified a value >37 mAU/mL as the optimal threshold for the best combination of sensitivity and specificity (80% and 76%, respectively) and, at the best cut-off of 5.2 ng/mL, AFP yielded 53% specificity and 78% sensitivity. The combination of PIVKA-II and AFP reached positive and negative predictive values of 73.9% and 94.2%, respectively. Conclusions: PIVKA-II levels are increased in the HCC patients, compared to control groups. The increase is more evident in patients with advanced HCC. The diagnostic performance of PIVKA-II seems more sensitive than AFP while the combination of PIVKA-II and AFP resulted in the best diagnostic accuracy, reaching 73.9% positive predictive value and 94.2% negative predictive value, thus improving the diagnostic capability of the single marker. Several classes of organic compounds were analyzed in aqueous aerosols collected in June, November, and December 1996 from above the sewage aeration tanks of a treatment plant (Prato, Italy). Particle size distribution of organic compounds and their enrichment ratio (E[sub r]) with respect to the magnesium ion were determined to infer the extent to which various species were aerosolized. Organic components were found to be predominantly enriched in fine and large particles of the aerosol and their transfer may be attributed to the 1) adsorption of surfactant organic matter at the air/water interface (such phenomenon is particularly evident for the fine and ultra-fine fractions) and 2) flotation of colloidal matter from wastewater to the largest particles with consequent transport of the adsorbed organic compounds. Background: The epidemiology of hepatocellular carcinoma (HCC) is characterized by a dynamical temporal trend of well-established and emerging risk factors. Methods: We evaluated the temporal trend of aetiological factors of HCC over the last two decades in Italy. HCC cases were recruited from two previously published national studies in 1996 and in 2008 and HCC cases were also enlisted from two national surveys in 2001 and in 2014 enrolling consecutive subjects with chronic liver disease (CLD) referring to more than 80 liver units scattered all over the country for a 6-month period. Results: Out of the 9997 subjects with CLD recruited in 2001 and the 2408 recruited in 2014, 3.3% and 5.7% (P < 0.001), respectively, had HCC. The temporal trend of HBsAg −/HCV + HCC cases significantly linearly decreased from 71.1% in 1996 to 57.2% in 2014 (P < 0.001). Conversely, that of virus-negative cases significantly linearly increased from 12.1% to 28.3% (P < 0.001). The proportion of HBV-related HCC cases showed a steady low rate, reflecting the reduced endemicity of the infection in Italy. The proportion of HCC with compensated cirrhosis (i.e., Child–Pugh A) linearly increased over time from 55.6% in 1996 to 76.0% in 2014 (P < 0.001) reflecting the growing effectiveness of semi-annual ultrasound surveillance for early detection of HCC. Conclusion: In conclusion, with decreasing viral aetiology, an overall decrease in the incidence of HCC might be expected in the future. The proportion of metabolic diseases is conversely increasing being considered as an aetiology. The school archives have been left as a hidden memory until very recent times, in Italy. In fact, only recently this kind of documents has attracted scholars' specific researches and the attention of archival administration. This contribution synthetically describes how an interest in these archives has increasingly grown and it points out the different strategies for their management and custody, focusing on the Salentine experience. The aim of this article is to underline the importance of the contents of the schools archives also for historical demography studies, and to give conservation and cataloging to this archival material. Abstract The relationship between rate of water loss of Italian ryegrass (Lolium multiflorum ) and crop properties, weather conditions and mechanical treatments were studied on a field scale, as was the importance of the variables and their combined effects on influencing the wilting process. Eleven forage-drying experiments were performed between 1992 and 1994 on diploid and tetraploid cultivars cut at different growth stages, ranging from early elongation to flowering, under environmentally variable conditions. The rate of drying was measured until a moisture content of 1 g water g-1 dry matter (DM) (500 g kg-1 ) was reached, or for a maximum of 2 days by calculating the rate of water loss from two consecutive hourly weights of swath portions placed on trays. Multiple regression analysis was used to correlate the drying rate (k ) with crop properties and with meteorological and mechanical treatment variables. The interaction between potential evapotranspiration (ET0 ) and the moisture content of grass at cutting (M grass appeared to be the most important variable, from the twenty-one tested, in influencing K. This variable shows that the same quantity of energy available for evaportaion acts in different ways when the water content of the crop at cutting differs. The rate of water loss is also influenced by the M grass itself, surgacae density of the swath, tedding and the weather conditions on the first day of drying. The value of M grass at cutting is greatly greatly dependent on crop maturity and ploidy. The tetraploid cultivars, with a higher inital moisture content, lost water more slowly than diploid cultivars did. Because the date of cutting cannot be delayed, owing to the decline in nutrive value, it is helpful to select for cultivars with low moisture contents at cutting, as well as applying mechanical treatment (spreading and tedding), in order to keep the wilting period as short as... In this paper we demonstrate SAR system capability for detecting and characterizing marine surface slicks. During an aircraft measurement campaign over the Gulf of Genoa (Italy), a multi-frequency SAR system, operating in P-, L- and C-bands, explored a sea area heavily covered by slicks. At the same time in situ measurements were performed with an interferential microwave probe, installed on board a small boat, capable of measuring high resolution sea spectra up to frequencies of capillary waves. By plotting SAR pixel intensity versus sea wave Bragg frequency we obtained wide portions of the sea spectrum region affected by the surface film damping. Spectra derived from SAR imagery and from gauge data present comparable slopes and furthermore the ratio between clean to slicked water spectrum obtained with the two techniques were surprisingly similar. This demonstrates the multi-frequency SAR systems ability to detect and characterize sea surface films assuming the Bragg mechanism in the radar backscatter.
