Summary: A successful therapy needs high level of adherence consisting in right drug intake in terms of persistence and compliance. The aim of this study was to evaluate anti-osteoporotic therapies recorded in general practitioner databases in the area of Rome, which used the same computerized medical record management. The study focused on evaluating therapy adherence, any adherence changes excluding spot therapies (less than 30 days), and any cause of early therapy discontinuation in a subgroup of patients randomly selected. Methods: Thirty-one databases were evaluated, including a total of 6,390 anti-osteoporotic therapies: 5,853 were prescribed to women and 537 to men. The prescribed drugs were: vitamin D (13 %), calcium (8.7 %), vitamin D + calcium (40.1 %), raloxifene (3.3 %), alendronate (16.4 %), risedronate (7.7 %), clodronate (10.4 %), or other drugs (0.4 %). Spot therapies represented 53.7 % of the total prescriptions. The difference between adherence in the total group (24.64 %) and the group excluding spot therapies (43.38 %) is significant. The main factors influencing low adherence were side effects (27 %), misinformation given by the physician (17 %), insufficient motivation (9 %), difficult intake (9 %), and no perceived benefits (9 %). Results: Our study suggests adherence is high and similar to other chronic diseases if spot therapies are excluded. The analysis of spot therapy causes suggests that an important role is played by the physician and the interpersonal aspects of medical practice, especially at the first prescriptions. Conclusions: The physician should collaborate with patients in choosing a personalized medical treatment. During the period 2001–2002 a total of 236 inspections were performed on 27 catering establishments in the province of Ferrara (Emilia-Romagna region, Italy), after a HACCP system was introduced and educational programs for food staff was undertaken for approximately 10 years. A total of 370 food samples and 140 surface swabs were taken and examined for microbiological quality. The surveillance system has brought to light various shortcomings regarding the equipment (36 corrective actions) and incorrect procedures (47 corrective actions). The tool and work surfaces showed an unacceptable contamination in 10% of samples. The data also highlight a certain percentage of unacceptable samples of foods, especially with regard to Escherichia coli, ranging from 5.4% for the “first and second courses” to 10.8% for the “raw meats and meat preparations”. Nevertheless, the hygienic quality of services and foods has improved in comparison with previous surveys, showing that the staff educational programs and the application of HACCP principles have increased the level of awareness regarding food hygiene in those working in catering services. Twelve public swimming pools in Bologna (Emilia-Romagna region, Italy) were investigated for the microbiological quality of water and surfaces of the pool edges, showers and changing rooms. At the same time a cross-sectional study was carried out on the health of 238 users (bathers) compared with 238 controls who practised sports other than swimming. Data regarding duration and frequency of exposure, behaviour and recent medical history were gathered by means of a questionnaire. All participants also underwent a medical examination. Compliance of the pool water to the microbiological standards set by Italian regulations was generally good; compliance was total when free chlorine residual was within the recommended limits. However, when analysis was extended to the various surfaces, potentially pathogenic bacteria such as P. aeruginosa and C. albicans were isolated. Eye burning and diarrhoea were the only declared symptoms and verrucas the only diagnosed disease showing statistically significant differences between bathers and controls. Verrucas tended to increase in proportion to exposure. Athlete's foot had a very high prevalence among both bathers (34%) and controls (27.3%) and controls (27%). The environmental and epidemiological investigations both confirmed the risk of infection, mainly associated with the contamination of surfaces. [ABSTRACT FROM AUTHOR]

Abstract: In the three-year period 2010–2012, 80 public swimming facilities in the metropolitan area of Bologna (Emilia Romagna Region, Italy), including 144 pools (69 indoor, 75 outdoor), were monitored to assess the microbiological and chemical water quality, after about ten years of implementing the new Italian guidance which introduced the principles of internal safety plans in the surveillance of swimming pools. According to the Italian guidance, water samples were collected from supply water (370 samples), pool water (645), and recirculating water entering the pool (307). The samples of supply water always conformed to the microbiological limits for drinking water. The pool water did not conform to the Italian legal requirements in around 16% of indoor pools and 25% of outdoor pools. In 65% of non-compliant samples, only one parameter exceeded the required standards. The microorganisms of faecal origin were isolated very rarely (Enterococci in less than 2% of samples) and pH and residual chlorine showed good compliance in pool water, implying an efficient management of the internal control. The inlet water exceeded the required standards in about 36% and 50% of samples, respectively in indoor and outdoor pools. However, 83.6% of the corresponding samples of pool water met the required limits. The microbiological incongruities were prevalently due to high levels of total heterotrophic counts (THCs) and Pseudomonas aeruginosa, and were indicative of bacterial colonization of the filters. The sampling of inlet water can thus be indicated as a critical control point for checking the correct functioning of the filters. The non-conformity of samples led to pool closure only in 1.5% of cases. In the other cases, the operators were officially invited to perform the corrective measures previously established in the plan of risk assessment. On the whole, the approach based on internal safety plans produced satisfactory results in terms of pool water quality, demonstrating the effective working of the internal system of analysis and management of risks. Background: In Italy, leprosy diagnosis is reported in immigrants from endemic countries or Italians who have stayed in endemic areas. We report the first leprosy case to be observed in a migrant from Nigeria in the Rimini district (Emilia-Romagna, Northern Italy)., Methods: After describing the tasks of the various health Institutions in the Italian integrated system for diagnosis, treatment, and surveillance of leprosy, we describe the management and outcomes of the leprosy case and of the patient's contacts., Results: In April 2017, Multibacillary Lepromatose Leprosy was diagnosed in a 29-year-old Nigerian man who arrived in Rimini in July 2014 after a 2-year stay in Libya. The local Public Health Service implemented the epidemiological investigation and identified the patient's close contacts. The management of the case and the surveillance of the 13 identified contacts, 7 Italians and 6 migrants, highlighted some critical issues. The late diagnosis of the case, due to the lack of knowledge of exotic diseases by general practitioners and other health and social professionals, and the loss at follow up of the close contacts (5 out of 6 migrants), represented important obstacles to the full success of surveillance measures., Conclusions: Although in Italy there is a well codified system of notification and surveillance of leprosy, the recognising of cases and the tracing and follow up of contacts are made difficult by the particular conditions of the involved people. This represents a new challenge for the Italian Public Health Authorities which, in the current context of immigration, often uncontrolled, must know how to respond to the new needs, in close collaboration with the State Institutions responsible for registering migrants and those health and social professionals who could facilitate the access of foreign people to health services.

Lynch syndrome is caused by germline mutations of genes affecting the mismatch repair proteins MLH1, MSH2, MSH6 or PMS2. Identification of Lynch syndrome patients using germline molecular testing in colorectal cancer (CRC) affected patients and in their healthy relatives is a cost-effective model of cancer prevention. Several studies demonstrate that universal tumor testing using immunohistochemical (IHC) analysis of CRC samples is the most efficient approach to identifying patients affected by Lynch syndrome. We studied a cohort of 352 consecutive CRCs for MSH2, MLH1, MSH6 and PMS2 protein expression using universal IHC screening. IHC mismatch repair (MMR) defects were identified in 70 out of 352 cases (19.8%) including six CRCs MSH2/MSH6 defective, two CRCs, respectively, MSH6 and PMS2 defective, 58 CRCs MLH1/PMS2 defective and four CRCs showing atypical MMR pattern. MLH1 promoter methylation and V600E BRAF mutation analysis were investigated on 61 CRCs. Cancer genetic counseling was offered to all 68 patients affected by MMR defective CRCs and 25 patients opted in to this service (36.8% compliance). Pathogenetic variants of MSH2 genes were identified in two cases (55 and 79 years old). Universal screening based on an IHC approach showed a Lynch syndrome incidence of 1/173. The protocol recommended by regional law improved patient compliance. This study demonstrates that the IHC approach for both MMR deficiency and V600E BRAF mutation detections is the most efficient approach for Lynch syndrome screening in the Italian population.

Background: The school gives access to children, regardless of age, ethnicity, gender and socio-economic class and can be identified as the key environment in which to promote children's physical activity (PA). The guidelines of the European Union recommend accumulating at least 10-min bouts of PA to reach the daily 60 min. Active breaks (ABs) led by teachers inside the classroom represent a good strategy to promote PA. The aim of this pilot and feasibility study was to evaluate the feasibility and effectiveness in terms of PA level of an AB programme in children aged 8-9 years attending primary school. Methods: A pre-post quasi-experimental pilot and feasibility study was performed in two primary school classes, one of which was assigned to a 14-week AB intervention (AB group) and the other to the control group (CG). At baseline and at follow-up, children were monitored for sedentary and motor activity during an entire week using ActiGraph Accelerometer (ActiLife6 wGT3X-BT). The satisfaction of children and teachers was assessed by self-administered questionnaires. Results: In the pre-post comparison, AB group ( n = 16) showed a reduction in the minutes spent in weekly sedentary activity (-168.7 min, p > 0.05), an increase in the number of step counts (+14,026.9, p < 0.05) and in time spent in moderate to vigorous PA (MVPA): weekly MVPA: +64.4 min, daily MVPA: +8.05 min, percentage of MVPA: +0.70%. On the contrary, CG showed a worsening in all variables. ANCOVA analysis, after adjusting for baseline values, showed significant differences between the AB group and CG for time spent in MVPA, percentage of MVPA and step counts. The satisfaction of children and teachers was good. Teachers were able to adapt the AB protocol to the needs of the school curriculum, thus confirming the feasibility of the AB programme. Conclusions: This pilot and feasibility study showed the feasibility and effectiveness of the AB protocol and represented the basis for a future controlled trial.

Background: In developed countries, scabies is observed sporadically or as institutional outbreaks in hospitals and other health facilities. In the family context, outbreaks generally involve a limited number of cases., Methods: A local health authority in Emilia-Romagna (Northern Italy) carried out an epidemiologic investigation on a family outbreak of scabies that included an unusually high number of cases. Its possible connection with a nosocomial case in a long-term care facility (LTCF) and outbreak management are discussed., Results: Among the household members, 8 confirmed cases occurred (attack rate, 87.5%). Another case was reported in a patient of an LTCF where one of the family cases worked as a sociosanitary operator. In total, 244 contacts were placed under surveillance. The control strategy focused on a mass information campaign addressed to all contacts and the training of health care personnel. In addition, specific prophylaxis (permethrin 5%) was performed in 108 high-risk contacts and LTCF patients and staff., Conclusions: The control measures were successful in preventing the spread of the outbreak. Clostridium difficile is an emerging cause of healthcare associated infections. In nine hospitals of an Italian Local Health Authority the episodes of C. difficile infection (CDI) were identified using the data registered by the centralized Laboratory Information System, from 2010 to 2015. CDI incidence (positive patients for A and/or B toxins per patients-days) was analysed per year, hospital, and ward. A number of cases approximately equivalent to the mean of identified cases per year were studied retrospectively to highlight the risk factors associated to CDI and their severity. Nine hundred and forty-two patients affected by CDI were identified. The overall incidence was 3.7/10,000 patients-days, with a stable trend across the six years and the highest rates observed in smaller and outlying hospitals (up to 17.8/10,000), where the admitted patients were older and the wards with the highest incidences (long-term-care: 7.6/10,000, general medicine: 5.7/10,000) were more represented. The mean age of patients in each hospital was correlated with CDI rates. Of the 101 cases selected for the retrospective study, 86.1% were healthcare associated, 10.9% community acquired; 9.1% met the criteria for recurrent case and 23.8% for severe case of CDI. The overall mortality rate was 28.7%. Comorbidity conditions occurred in 91.1%, previous exposure to antibiotics in 76.2%, and proton pump inhibitors in 77.2%. Recurrent and severe cases were significantly associated with renal insufficiency and creatinine levels ≥2 mg/dL. The survey based on the centralized laboratory data was useful to study CDI epidemiology in the different centres in order to identify possible weaknesses and plan control strategies, in particular the reinforcement of staff training, mainly targeted at compliance with contact precautions and hand hygiene., Competing Interests: The authors declare no conflict of interest.

The study aimed to assess the prevalence of Legionella spp. in dental unit waterlines of a dental clinic and to verify whether the microbiological parameters used as indicators of water quality were correlated with Legionella contamination. A risk management plan was subsequently implemented in the dental health care setting, in order to verify whether the adopted disinfection protocols were effective in preventing Legionella colonization. The water delivered from syringes and turbines of 63 dental units operating in a dental clinic, was monitored for counts of the heterotrophic bacteria P. aeruginosa and Legionella spp. (22 °C and 37 °C). At baseline, output water from dental units continuously treated with disinfection products was more compliant with the recommended standards than untreated and periodically treated water. However, continuous disinfection was still not able to prevent contamination by Legionella and P. aeruginosa. Legionella was isolated from 36.4%, 24.3% and 53.3% of samples from untreated, periodically and continuously treated waterlines, respectively. The standard microbiological parameters used as indicators of water quality proved to be unreliable as predictors of the presence of Legionella, whose source was identified as the tap water used to supply the dental units. The adoption of control measures, including the use of deionized water in supplying the dental unit waterlines and the application of a combined protocol of continuous and periodic disinfection, with different active products for the different devices, resulted in good control of Legionella contamination. The efficacy of the measures adopted was mainly linked to the strict adherence to the planned protocols, which placed particular stress on staff training and ongoing environmental monitoring.

Introduction and Objective: Many devices have been marketed in order to improve the organoleptic characteristics of tap water resulting from disinfection with chlorine derivates. The aim of the presented study was to assess the degree of contamination by non-fermenting Gram-negative bacteria (NF-GNB) of drinking water dispensed from microfiltration devices at point-of-use., Methods: Water samples were collected from 94 point-of-use water devices fitted with a filter (0.5 μm pore size) containing powdered activated carbon. The microbiological contamination of water entering and leaving the microfiltered water dispensers was compared. The NF-GNB loads were correlated to Total Heterotrophic Counts (HPCs) at 37 and 22 °C, residua chlorine, and some structural and functional features of the devices., Results: NF-GNB were detected from 23% of supply water samples, 33% of still unchilled water, 33% of still chilled water and 18% of carbonated chilled water. The most frequent isolates were Pseudomonadaceae: Steno.maltophilia 30.2% of isolates, Pseudomonas 20.5%, Delftia acidovorans 13.4%, while the species more largely distributed was Ps. aeruginosa recovered from 13% of samples. The distribution of the various NF-GNB was different in the water entering and in that leaving the devices. Ps.aeruginosa and Steno.maltophilia were the predominant species in water leaving the microfiltration dispensers, probably due to their capacity to colonize the circuits and to prevail over the others. Recovery of NF-GNB was favoured by the reduction in residual chlorine of the supply water, occasional use, the absence of a bacteriostatic element in the filter and inadequate disinfection of the water lines., Conclusions: The presence of high concentrations of potentially pathogenic species of NF-GNB (Ps.aeruginosa, Steno. maltophilia, Burkhol.cepacia) in the water dispensed from microfiltration devices represents a risk of waterborne infections for vulnerable individuals. When these devices are used in environments such as hospitals, nursing homes for the elderly, etc., microbiological monitoring for the detection of NF-GNB is advisable.

Background: This study aimed to assess whether a school-based physical education intervention was effective in improving physical abilities and influencing daily physical activity habits in primary school children. The possible effect on body mass index (BMI) was also considered., Methods: Twenty-six 3rd-grade classes were randomly selected stratifying by geographic location (city, plain, hills) and were assigned either to an intervention (127 boys; 120 girls) or to a traditional (129 boys; 121 girls) physical education program. At baseline (age: 8-9 years) and after a 2-year follow-up (age: 10-11 years), information was collected about sport participation and daily activity habits using a self-administered questionnaire. Height, weight, and BMI were measured and physical performance was assessed by means of standardized tests., Results: The enhanced program of physical education was effective in improving physical abilities of children and determining a decrease (boys: 10%; girls: 12%) in daily sedentary activities (preintervention versus postintervention, p < .05; intervention versus control group, p < .01). The percentages of overweight and obese children did not vary significantly, but the experimental group showed a significantly lower rise in BMI compared to the control group (p < .001)., Conclusion: The school proved to be an ideal setting for promoting physical activity and achieving the required daily activity levels., (© 2013, American School Health Association.)

In accordance with the international and national guidelines, the Emilia-Romagna Region (Italy) has established regional guidelines for the surveillance and prevention of legionellosis based on the concept of risk assessment, with particular attention to environmental monitoring. The aim of this study was to verify how environmental surveillance in the context of risk assessment plans could help to guide decisions about preventive strategies against Legionella infections in Long Term Care Facilities (LTCF). In six LTCFs in the city of Bologna (Emilia-Romagna Region) a self-control plan was implemented that included the environmental monitoring of Legionella spp. and the surveillance of hospital-acquired Legionnaires' Disease. At baseline, four hot water systems were colonized by Legionella pneumophila (3 LCTFs) and Legionella londiniensis (1 LCTF). In each establishment specific control measures were adopted based on the characteristics of the system, the virulence of the strain and the level of the contamination. The monitoring, carried out for around two years, was also extended to the ways in which the system and the distal water distribution points were used and maintained with respect to the good practices in operation and management. The adopted actions (shock and/or continuous disinfection treatments) and the implementation of the good practice measures reduced the contamination to acceptable and stable levels. No cases of hospital-acquired legionellosis occurred during the period of study. The Working Group "Movement Sciences for Health" of the Italian Society of Hygiene, Preventive Medicine and Public Health has promoted the Italian translation of the WHO Guidelines for Safe Recreational Water Environments, relating to coastal and freshwater environments. In this article the authors briefly summarize the main areas covered in the guidelines, namely drowning and injury, exposure to cold, heat and sunlight, water quality, contamination of beach sand, exposure to algae and their products, aesthetic issues, exposure to chemical and physical agents, dangerous aquatic organisms, monitoring and assessment hazards and risks.

A prospective observational study was conducted during a two-year period to evaluate the prevalence of hospitalized pneumonia in six hospital units of the Bologna S.Orsola-Malpighi hospital (Italy). The selected units were: general surgery, general medicine, internal medicine, geriatrics, respiratory physiopathology and pneumology, with a total of 205 beds and around 4,800 admissions per year. Data were collected from the clinical cards and cases of pneumonia were distinguished by origin (community-acquired or hospital-acquired according to CDC definition), individual and clinical characteristics, and aetiology. The study involved 486 cases of pneumonia: 75 hospital-acquired and 411 community-acquired (84.6%). Patients affected by hospital-acquired pneumonia were older (average age 77 years) compared to community-acquired cases (74 years) and show a more homogeneous gender distribution (males: 48.0% vs 59.4%). Hospital stays (42 vs 21 days) and mortality rates (24.0% vs 11.7%) were significantly higher in hospital-acquired pneumonia. The incidence rate of nosocomial pneumonia was 7.4 per 1000 hospitalized patients and increased to 17-23 per 1000 in the pneumology and respiratory physiopathology units. Only 16.9% of cases had an aetiological diagnosis (14.1% community-acquired; 31.8% hospital-acquired); the most common isolates were S. aureus and P. aeruginosa. The hospital-acquired cases were caused by Gram-negative bacilli more often than the community-acquired cases, and infections were more frequently polymicrobial (37.5% vs 3.4%). In order to reduce morbidity and mortality due to pneumonia it is important to implement prevention measures in the community (i.e. specific vaccination campaigns), improve clinical protocols for aetiological diagnosis in hospitalised patients and increase epidemiological surveillance of hospital-acquired infections.

A randomized stratified sample of 522 children attending the third class of primary schools within the area of Bologna Local Health Unit was analysed for physical activity and sports practice. Information about the children's habits and availability of facilities for physical and sports activities were collected by means of structured questionnaires completed by children (507 respondents), parents (491), reference teachers for physical education (26) and class teachers (46) during the school year 2006-07. At the same time, the children's heights and weights were measured in order to calculate BMI values. Regular sports activity is practised by 80.1% of children (males: 82.4%, females: 77.6%), with significant diferences between genders only in children with at least one non-Italian parent (M>F, p < 0.05); the practice of sports is influenced by the area of residence (metropolitan > plain and hills, p < 0.05) and nationality (Italians > non-Italians, p < 0.01). Children with at least one actively sports practising parent are involved more frequently in sports activities (p < 0.001). In free time, sedentary activities are prevalent for both sports-practising children and not. However children not involved in regular sports activities tend to practise outdoor physical activities with a frequency significantly higher than children involved in sports (17.3% vs 10.4% of respondents). The percentage of completely sedentary children, who stated that they practise neither sports nor physical activity in their free time, is 7.3% (metropolitan area: 4.5%, hills: 8.7%, plain: 10.6%). The prevalence of overweight is 24.4%, of obesity 9.7%, with a better distribution of BMI values in the metropolitan area where there is the highest occurrence of positive conditions and behaviours: availability of sports facilities, the highest prevalence of sports practice, and the lowest prevalence of completely sedentary children.

A prospective surveillance study of legionnaires disease and an environmental survey of Legionella species were performed simultaneously in a general hospital. During a period of 3 years, 705 patients with pneumonia were screened with a Legionella urinary antigen test, and pneumonia was confirmed by culture and serological tests. Twelve cases of legionnaires disease were identified, none of which were hospital acquired, despite the fact that 60% of hospital water samples were contaminated with Legionella pneumophila at a concentration of more than 10(3) colony-forming units/L. The probable source of infection was identified for only 2 community-acquired cases. The results show that environmental contamination alone is not able to predict the risk of legionnaires disease. If no cases are present, monitoring of hospital water systems is of little significance; clinical surveillance is much more important.

The hygienic characteristics of the freshwater from Garda Lake used for the water supply of Sirmione were evaluated before and during the various phases of treatment. The samples were taken at the moment of entry, after ozonization, after filtration and after the addition of chlorine dioxide. The coliforms and Escherichia coli, occasionally detected in the incoming water, were reduced by ozone and eliminated by filtration. Enterococci were always absent. The overall reduction in the total bacteria count was satifactory, considering the low initial levels (96.0% and 83.0% respectively for the Heterotrophic plate count at 22 degrees and the Heterotrophic plate count at 36 degrees). The chemical and physical indicators that underwent the greatest reductions were the sulphates, iron, manganese, ammonia and the turbidity. Several species of unicellular algae belonging to Chlorophyta, Cyanophyta and Bacillariophyta were identified in the water at entry and after ozonization, but were completely eliminated during the subsequent phase of filtration. deliver of high quality services.

A cross-sectional multicenter survey of Italian hotels was conducted to investigate Legionella spp. contamination of hot water. Chemical parameters (hardness, free chlorine concentration, and trace element concentrations), water systems, and building characteristics were evaluated to study risk factors for colonization. The hot water systems of Italian hotels were strongly colonized by Legionella; 75% of the buildings examined and 60% of the water samples were contaminated, mainly at levels of > or =10(3) CFU liter(-1), and Legionella pneumophila was the most frequently isolated species (87%). L. pneumophila serogroup 1 was isolated from 45.8% of the contaminated sites and from 32.5% of the hotels examined. When a multivariate logistic model was used, only hotel age was associated with contamination, but the risk factors differed depending on the contaminating species and serogroup. Soft water with higher chlorine levels and higher temperatures were associated with L. pneumophila serogroup 1 colonization, whereas the opposite was observed for serogroups 2 to 14. In conclusion, Italian hotels, particularly those located in old buildings, represent a major source of risk for Legionnaires' disease due to the high frequency of Legionella contamination, high germ concentration, and major L. pneumophila serogroup 1 colonization. The possible role of chlorine in favoring the survival of Legionella species is discussed.

A bicarbonate-sulphate-calcic water of a therapeutic spa was monitored for the presence of Legionella, Pseudomonas and Mycobacteria. The water was analysed by taking samples from the well, the feed tank and from the final aerosol generating devices of two different water lines, the former at 21-23 degrees, the second at 36-38 degrees. The bacteria in question were always absent from the well. Legionellae were found in the water of aerosol equipment: Legionella micdadei was isolated from 75% of samples, L. bozemanii from 75% and 50% (respectively 36-38 degrees and 21-22 degrees water lines) and other species of environmental Legionellae from 25% of samples. The water of aerosol equipment presented high total bacterial counts (10(3)-10(4) cfu/ml) and exspecially high concentrations of Pseudomonadaceae (10(2)-10(3) cfu/100 ml). These bacteria, unlike the Legionellae, were also isolated from the feed tank at mean concentrations of about 10(2) cfu/100 ml. Mycobacteria were found in 75 and 50% of samples collected from final devices, respectively from the heated and not heated water lines. The isolates were M. gordonae (85% of isolates) and M. fortuitum (15%), but at concentrations very low. Both treatments with sodium hypochlorite (20 ppm of residual chlorine) and peracetic acid (20 ppm) resulted in the reduction of total bacterial counts and elimination of Pseudomonas from the water in the tank, but not in elimination of Pseudomonas and Legionellae from the nebulizers. The disinfectants were evidently not able to efficiently reach all the points where Pseudomonas and Legionella had settled and grown. In order to obtain total abatement it was necessary to carry out a radical restructuring of the plant, involving the replacement of the old nebulizer benches with new aerosol equipment that could be subjected to a new system of programmed thermal shock.

Aims: An evaluation was made of the prevalence of Legionella species in hot water distribution systems in the city of Bologna (Italy) and their possible association with bacterial contamination (total counts and Pseudomonadaceae) and the chemical characteristics of the water (pH, Ca, Mg, Fe, Mn, Cu, Zn and Total Organic Carbon, TOC)., Methods and Results: A total of 137 hot water samples were analysed: 59 from the same number of private apartments, 46 from 11 hotels and 32 from five hospitals, all using the same water supply. Legionella species were detected in 40.0% of the distribution systems, L. pneumophila in 33.3%. The highest colonization was found in the hot water systems of hospitals (93.7% of samples positive for L. pneumophila, geometric mean: 2.4 x 10(3) CFU l(-1)), followed by the hotels (60.9%, geometric mean: 127.3 CFU l(-1)) and the apartments with centralized heating (41.9%, geometric mean: 30.5 CFU l(-1)). The apartments with independent heating systems showed a lower level of colonization (3.6% for Legionella species), with no evidence of L. pneumophila. Correlation analysis suggests that copper exerts an inhibiting action, while the TOC tends to favour the development of L. pneumophila. No statistically significant association was seen with Pseudomonadaceae, which were found at lower water temperatures than legionellae and in individual distribution points rather than in the whole network., Conclusions: The water recirculation system used by centralized boilers enhances the spreading of legionellae throughout the whole network, both in terms of the number of colonized sites and in terms of CFU count., Significance and Impact of the Study: Differences in Legionella colonization between types of buildings are not due to a variation in water supply but to other factors. Besides the importance of water recirculation, the study demonstrates the inhibiting action of copper and the favourable action of TOC on the development of L. pneumophila.

We investigated Legionella and Pseudomonas contamination of hot water in a cross-sectional multicentric survey in Italy. Chemical parameters (hardness, free chlorine, and trace elements) were determined. Legionella spp. were detected in 33 (22.6%) and Pseudomonas spp. in 56 (38.4%) of 146 samples. Some factors associated with Legionella contamination were heater type, tank distance and capacity, water plant age, and mineral content. Pseudomonas presence was influenced by water source, hardness, free chlorine, and temperature. Legionella contamination was associated with a centralized heater, distance from the heater point >10 m, and a water plant >10 years old. Furthermore, zinc levels of <100 microg/L and copper levels of >50 microg/L appeared to be protective against Legionella colonization [corrected]. Legionella species and serogroups were differently distributed according to heater type, water temperature, and free chlorine, suggesting that Legionella strains may have a different sensibility and resistance to environmental factors and different ecologic niches.

A multicentric Italian investigation on legionnaires' disease is in course to clarify host factors as well as pathogen associated characteristics involved in the infection/disease. The main goal of the research plan is to account for some critical aspects concerning identification and prevention of legionellosis. To improve knowledge on factors associated with Legionella spp colonisation in hot waters, to detect cases and to characterize risk factors in subjects which develop pneumonia are specific objectives of the research programme. Preliminary results show that hot waters of houses and hotels are frequently contaminated (22.6% and 54.6%, respectively), mainly by L. pneumophila. Microbial concentrations were low in domestic waters (<1.000 ufc/l), but higher in samples from the hotels (geom. mean 1.85 x 10(3) ufc/l). Warming system, age of the plant, type of building were risk factors significantly associated with Legionella spp positivity. The active surveillance on patients affected by pneumonia with search for Legionella urinary antigen allowed the identification of 34 cases, 3 of which of nosocomial origin, corresponding to 4.2% of the screened pneumonia. After informed consent, 26 subjects were recruited for a case-control-study to clarify risk factors for the disease.

The hot water supplies of 11 private healthcare facilities in the city of Bologna, Italy, were monitored for the presence of Legionella spp. Four samplings were made in each establishment over a period of one year and in total 121 samples were collected from distribution points situated near the water-boiler and inside the wards (taps and showers). Legionellae were recovered from all the water supplies in question: Legionella spp. in 86.8% of samples, L. pneumophila in 82.6% of samples. L. pneumophila was found in all the water supplies at levels averaging above 10(4)cfu/L in five health facilities and reaching a maximum concentration of 10(7)cfu/L. The only parameter to have affected the presence of legionellae was the water temperature, which was seen to be inversely correlated to the concentration of Legionella spp. Despite the high levels of contamination from L. pneumophila, no cases of nosocomial Legionnaires' disease were reported during the period of the study., (Copyright 2002 The Hospital Infection Society.)

A 5 years survey (1996-2000) was performed on the microbiological quality of shellfish (mussels and clams) collected from authorized shellfish-growing area of the North Adriatic Sea (province of Rimini). 7.0% of mussel samples (33/474) and 21.9% of clam samples (218/996) exceeded the legal limits for faecal indicators (Dlgs 530/92). The faecal contamination of shellfish was related to the organic pollution arriving from inland surface waters. In fact shellfish harvested near the coast were more contaminated, as well as shellfish grown in the surface layers of the open sea, where the fresh waters of the streams tend to stratify. Faecal contamination was also directly correlated with rainfall, probably because the atmospheric precipitation, increasing the flow of the streams, favoured the transport of organic substances and micro-organisms deriving from the soil washing and the municipal sewage effluents. Furthermore, when rainfall was particularly plentiful, sewage inflow could also exceed the hydraulic capacity of the treatment plants; thus making necessary to discharge untreated waste directly into surface waters. Salmonellae were found in 0.7% of clam samples. Although this percentage is very low, it shows that pathogenic micro-organisms are present in this area of sea. This finding, together with the high variability of shellfish pollution due to occasional factors as rainfall, emphasise the importance of the systematic monitoring of the microbiological quality of shellfish.

In August 1997, an outbreak of gastroenteritis from Salmonella Hadar phage type 2 occurred among customers of a restaurant in Rimini (Emilia-Romagna region, Italy). Twenty-nine people who had eaten food prepared in the restaurant on 2 or 3 August had symptoms of acute gastroenteritis. The infection was culture-confirmed in 24 cases and the stool specimens of four healthy people were positive for Salmonella Hadar. Twelve people had to be hospitalized and a 3-year old girl died. The case-control study identified roast rabbit as the likely vehicle of infection (OR: 6.00; CI 95%: 1.65-22.83). The microbiological investigation carried out on food taken from the restaurant confirmed high levels of Salmonella Hadar in a sample of roast rabbit. Since the rabbit was well cooked, the food contamination likely occurred after cooking. Poor hygienic conditions found in the restaurant, together with inappropriate food-handling practices and inadequate storage temperatures may have contributed to spread to other foods and the severity of the outbreak.

A 5-year study was carried out on the growth curve of two strains of P. aeruginosa inoculated at a density of 10(2) cfu/ml into samples of natural mineral water with different levels of dissolved solids (TDS at 180 degrees C: 72.5 and 382 mg/l) and low organic content (TOC: 0.17 and 0.35 mg/l). The resulting growth curves were similar for both strains, with only slight differences depending on the different amounts of dissolved solids in the water. After 4-5 days counts were increased by 3 log units. This level was maintained until 70-100 days from inoculation, after which a slow decrease began, culminating in the death of one of the strains after 5 years. No difference in recovery was observed between the method using direct inoculation on Cetrimide Agar and the resuscitation technique (preincubation in Tryptone Soya Agar followed by inoculation on Cetrimide Agar) in the exponential phase of the growth curve. During the stationary and death phases, however, the enrichment technique gave statistically slightly higher counts than the selective technique, indicating the presence of damaged P. aeruginosa cells. The use of a resuscitation step when using Cetrimide Agar to assess bottled water quality is recommended.

A study was made of the occurrence of Aeromonas spp. in drinking water supplies in a mountain area in northeast Italy (the Dolomites). On account of its location, the water in question is exposed to a low level of pollution and systematic chemical disinfection is not necessary. Out of 7395 water samples analysed over a 3 year period, 1623 (21 x 95%) were found to be positive for Aeromonas, with levels ranging from 1 to 240 cfu 100 ml-1; 72 x 4% of the strains were identified as Aer. hydrophila, 14 x 7% as Aer. caviae and 12 x 9% as Aer. sobria. The percentage of recovery from surface water (approximately 40%) was found to be higher than that of ground water (springs: 24 x 9%; wells : 28x 6%). Aeromonas spp. were isolated from 21 x 7% of samples from the distribution network and showed no significant variations compared with water from reservoirs. There was no evidence, therefore, of after-growth in the distribution system. No correlation was found between the concentration of Aeromonas spp. and faecal indicator organisms. As the distribution of Aeromonas spp. was unrelated to anthropic pollution, it is believed that the search for these micro-organisms should be adopted as a further indicator of drinking water quality, especially in waters such as those in the present investigation not undergoing systematic purification treatment.

An outbreak of hepatitis A is described involving thirteen cases in a town in central-northern Italy. The consumption of infected clams, caught in a polluted coastal area of the Adriatic Sea, caused hepatitis A in all five members of a family consisting of mother, father and children, two of whom attended primary school. The epidemic subsequently spread to the primary school with a secondary attack rate of 7.9%, 0 for female pupils and 18.9% for males. The epidemiological and environmental investigations showed that the critical exposure took place in the boys' toilet in the primary school (OR: 32.79, 95% CI: 6.83-157.45). To prevent the spreading of contagion, extra disinfection was carried out in the whole school and health education campaigns were mounted. The good basic hygienic conditions of the homes, together with the cooperation of school staff in surveillance and providing information, allowed the epidemic to be controlled.

Wash water from self-service washing machines in three commercial launderettes of Bologna (Italy) were examined to verify which factors affect their bacterial contamination and to determine which procedures in the laundering process have the most significant effects on the removal of bacteria. Four washing formulas were compared: a delicates cycle (programmed temperature 25-30 degrees C; actual temperature: 28-31 degrees C); a whites cycle (programmed temperature: 80-90 degrees C; actual temperature: 50-57.5 degrees C); a delicates cycle with the addition of an oxygen-based bleach safe for delicate fabrics and a whites cycle with the addition of an oxygen-based bleach. Bacterial contamination of washing machines was higher in the launderette most heavely used, and, furthermore, it was in relation with the washing temperature and the use of bleaches. The low temperature laundering cycle (20-30 degrees C) did not guarantee elimination of bacterial content from either the inside of the washing machine or from the fabric being washed. Washing with water at a higher temperature, of about 55 degrees C, or adding an oxygen-based bleach to the low temperature cycle did ensure a significant reduction in bacterial recovery from water samples and fabrics, but did not prevent bacteria such as P. aeruginosa from surviving inside the washing machine. Only the addition of bleaches to the hot water program ensured the almost total elimination of bacteria and also guaranteed their elimination from protected parts of the drum.

In response to a request from the personnel of a credit bank that had been moved into a new building, an evaluation was made of employee health complaints. The prevalence of symptoms was determined using a self-administered questionnaire. Ventilation effectiveness, contaminant concentrations (which included chemicals and bioaerosols) and microclimate were studied to establish their relationship to environmental discomfort and to the health problems mentioned by the employees. Indoor air quality--which was assessed in different seasons--revealed relatively high levels of carbon dioxide and bioaerosols in areas occupied by employees as compared with other places. Temperature and humidity were considered to be satisfactory. Although a causative agent was not isolated, the authors conclude that an ineffective rate of room air exchange, possibly in interaction with chemical contaminants, may be responsible for the discomfort and, perhaps, for the health symptoms experienced by workers in fully enclosed rooms.

In Italy the mortality rate for gastric cancer is still very high compared to that of other European countries, with marched differences between the regions (age-adjusted rates were much higher in the North than in the South). On the basis of official certifications, gastric cancer ranks as the second cause of death from cancer in both sexes, only surpassed by tumors of respiratory tract in men and breast cancer in women. This note examines the data relative to the official death certifications for stomach cancer in Italy, between 1950-1982, to value quantitatively the trend of the mortality in the last years as regards sex, age and birth year. The absolute values relative to death certifications for gastric cancer between 1950 and 1982 were obtained from the Istituto Centrale di Statistica which gathers all the death certifications issued throughout the whole of Italy. The 1951-1961-1971-1981 census data were used to calculate the rates for 100,000 inhabitants. For the thirty years considered, the following rates were calculated separately for each sex: a)--the age-specific mortality rates for five-year age groups in the three years around the census; b)--the age-adjusted mortality rates (direct methods, using the Italian population of the October 1981 census as standard population) for the total population and for those aged between 35 and 74; c)--the age-specific mortality rates in five-year age group, by cohorts of birth from 1883-85 to 1933-35; d)--the male/female ratio for gastric cancer mortality. Age-standardized death certification rates for the population of all ages decreased from 51.7 to 32.9/100,000 males and from 40.3 to 21.5/100,000 females. Age-standardized death truncated 35-74 years mortality rates showed a more marked declining trend in both sexes: from 92.5 to 50.1/100,000 males and from 57.3 to 23.3/100,000 females. If differences among the age-specific mortality rates between 1951 and 1981 is examined, it can be seen that there is a constant more or less marked decline for all the age groups up to 74, whereas in the higher age groups the rates tend to increase. The analysis of the cohorts of birth reveals that the decline in mortality rates for stomach cancer was already evident in females born at 1894 and in males born at 1904 and this continued regularly in the following decades. Finally, the trend male/female ratio for gastric cancer mortality increased over time from 1.28 in 1951 to 1.53 in 1981.(ABSTRACT TRUNCATED AT 400 WORDS)
