Introduction: The WHO European Mental Health Action Plan (2013–2030) emphasises the need to generate services that are more inclusive and attentive to the co‐construction of care practices. This exploratory research investigates the needs of young substance abusers shown during their stay in residential communities; in particular, it explores the idea that treatment may include a new phase focused on how to manage moderate or controlled alcohol intake during residential care. Interviews with young ex‐users open up critical reflections on complete abstinence programmes from all substances, including alcohol, as a prerequisite for discharge and also provide examples of how to co‐design a plan for mindful drinking. Methods: Fourteen young adults, aged 19–32 years, non‐alcoholists, treated at rehab in Fermo, in central Italy, were interviewed during a programme between 6 and 18 months of period. They were asked about exploring needs in preparation for the conclusion of the rehabilitation pathway. From this exploration emerged the need to introduce controlled alcohol intake during the rehabilitation stay. This request became the focus of the semi‐structured interviews. Results: Three main themes emerged, which are as follows: (1) difficulties in integrating the new identity with the past of consumption, (2) resistance to the idea of total abstinence in social relations and (3) uncertainties about post‐community behaviour regarding alcohol intake. At the same time, three unexpected needs were expressed: (1) test the personal knowledge and skills on how to manage the alcohol intake, (2) receive support during the residential path to build up self‐control competence given the post‐discharge period and (3) build a personalised therapeutic path together with the supervisor and the operators while still at the rehab, according to the realistic lifestyle and routine outside the rehab. Conclusions: This research highlights the importance of personalising treatment based on each user's needs, going far beyond the standardised treatments for users previously considered unable of self‐control and self‐determination. For that purpose, the relationship between the users and the operators might be privileged, as it is able to cover the specific needs aimed for the new identity. Involving the Participants: The research sparked a discussion within the community, involving and initiating an open dialogue between the operators and the users, allowing them to focus on certain innovative strategies offered by the service, putting the users' needs at the very centre of the attention. Objective: The high incidence of foodborne disease among children suggests the value of health promotion. Children are a high-risk group so far as foodborne disease is concerned, although they may be hard to reach with training programmes. This research investigated the use of drawings, compared with questionnaires, to evaluate the impact of a health promotion programme to improve knowledge and habits in the context of food handling and personal hygiene. Setting: Children (184) attending primary school and living in the north of Italy were enrolled in the programme. Methods: Qualitative and quantitative tools: pre- and post-intervention questionnaires were administered, and children were asked to produce pre- and post-intervention drawings about microorganisms and their effects on humans. An observation grid was built to code key features in the drawings. Results: Results clearly showed that some drawing features correlated with and predicted high scores in the questionnaire on knowledge of microorganisms. These were the use of captions, the representation of a natural context and the presence of a causal link between depicted elements. Conclusion: Study findings highlight the potential of the use of drawing as an effective evaluation tool. The use of drawing can support the design of strategies for the validation of health campaigns aimed at the amelioration of children's food contamination-related (and perhaps other) risks.

Introduction: Many studies have investigated patients' understandings of how to optimise mental health services. However, only a few studies in the Italian context have involved experts by experience (EbEs), who can be ex‐users, family members of ex‐users or current service collaborators. Their role is crucial in implementing collaborative service quality assessment projects. Method: The study investigated the experience of 35 EbEs, users, and family members who carried out a 9‐month fortnightly project aimed at imagining an 'ideal service'. The facilitators of the discussion groups (two EbEs) were interviewed; written reports of each meeting were produced with relevant comments, notes and specific suggestions; and content analysis was applied. Results: The most important result concerns the effectiveness of the project management method and group leadership carried out by the two EbEs. This approach allowed for complete autonomy of the work, without professional gaze or power imbalance. Also, the ideas and specific contents focused on by the two groups offer strategies to facilitate users' entry and reception in health care centres, to reduce the stigma of mental illness, to improve the centres' physical environment, to improve organisational aspects, to keep family members actively involved and to network mental health services with other territorial services. Conclusions: EbEs have proven to be key figures in ensuring equity of role in the service co‐design process. This also concerns a context, the Italian one, where their role has not yet been recognised and legalised. Their contribution and ideas to improve services could be fundamental not only in mental health centres, but also in other health facilities, and could concern the entire service delivery process rather than being limited to quality assurance, according to a virtuous circle based on active participation and transformation of the role of users. Introduction: People with poststroke acquired brain injury (ABI) face a complex and often troubled identity reconstruction journey. The literature is rich with studies related to the psychological and neuropsychological components involved in rehabilitation, but it is lacking with respect to the investigation of the existential dimensions and the challenges associated with finding new senses and meanings for one's identity and future perspectives, body and interpersonal relationships. Methods: The aim of this study is to investigate the narrative processes of identity reconstruction after brain damage. Through a qualitative approach, 30 autobiographical narratives about self, body and the relationships with others were collected and analyzed. Semistructured interviews were used for the data collection. Narrative and positioning analysis were applied. Results: Four main positionings emerged: sanctioning a radical break with one's previous life; assuming a sense of salvation and compulsory as well as irreversible adaptation to the limitations associated with one's condition; feeling different and disabled; and considering new possibilities and active constructions of self‐being in relationship with others. These results underline the narrative processes of construction of the injury and the identity and delineate possible resources and instruments to improve the clinical practice for health practitioners. They are also valuable for other professionals who deal with neurological services and rehabilitation, such as psychological counselling and support for persons who have experienced ABI and their families. Three associations of people with stroke and their families living in the same area contributed to designing the research on the basis of the needs expressed by their members with the aim to identify strategies and devices to be implemented in the public service to improve the care pathway. In many European Countries, a diagnosis is needed to access the gender transition process, which has sparked debate about whether gender variance should be equated with a psychodiagnosis. This study explores mental health practitioners' perspectives about the implications of using the diagnosis for gatekeeping purposes. Semi‐structured interviews were conducted with 11 Italian mental health specialists. The personal positions and interpretative repertoires emerging from the interviews centred on three thematic areas: the diagnosis, the practitioner's role, and the clinical relationship. In relation to the development of health promotion policies, findings underscore the importance of exercising reflexivity, adhering to theory and national and/or international guidelines, and analysing people's needs to ensure that the clinical setting is an affirmative space, especially for non‐binary people. Objective: Little is known about the process of becoming a mother in women who experienced a breast cancer diagnosis (BC). In this qualitative study, we investigated maternal representations in pregnant women with experience of BC and those with no oncological history. Methods: A total of 38 women were recruited, 19 women who experienced a BC diagnosis and 19 who had not. To explore maternal representations, semi‐structured interviews were conducted and analysed through thematic analysis. Results: Four main themes were identified: fears and worries, meaning of motherhood, mother–foetus relationship and partner support. Across themes, differences between primiparous and multiparous are reported. Women with gestational breast cancer (GBC) described fear for their own and their child's survival. Women with previous BC recall contrasting emotions. All women with experience of BC perceived breastfeeding as fundamental and inability to do so provoked worry. Relationship with the partner was considered central, while healthy women were projected towards the future triadic relationship. Conclusions: Finding a mental space during pregnancy for the representation of the future child could be hard for women with GBC. Dissimilarities in the experience of motherhood in cancer patients provide insight into psychological aspects that should be taken into account in clinical practice. [ABSTRACT FROM AUTHOR]

Transgender people face a range of personal and social conflicts that strongly influence their well-being. In many cases, the Internet can become the main resource in terms of finding support. The aim of this study was to understand how transgender people give and receive help online. Between 2013 and 2015, 122 online community conversations were collected on Italian forums and Facebook groups involving transgender people, and online interviews were conducted with 16 users of these communities. A qualitative content analysis was conducted by using the software package, NVivo10. The main categories that emerged were: motivations to join an online community, online help, differences between online and offline interactions, status, conflicts and professional help. Results indicate that participation in online communities often derives from the users' need for help. This help can be given by peers who have had similar experiences, and by professionals who participate in the discussions as moderator. The need to test one's own identity, to compare oneself with others and to share one's personal experiences made online communities at risk of exposing users to invalidation and transphobic messages. Administrators and moderators try to ensure the safety of users, and suggest that they ask for professional help offline and/or online when over-specific medical advice was sought. This study confirms that transgender people might find benefit from an online platform of help and support and might minimise distance problems, increase financial convenience and foster disinhibition. On 20th and 29th of May 2012, two powerful earthquakes struck northern Italy. The epicentres were recorded respectively in Finale Emilia (magnitude 5.9 Ml) and Medolla (magnitude 5.8 Ml) in the province of Modena, though the earthquake was formed by a series of seismic shakes located in the district of the Emilian Po Valley, mainly in the provinces of Modena, Ferrara, Mantova, Reggio Emilia, Bologna and Rovigo. Many monuments in the city of Mantova were hit by the earthquake and, among these, Palazzo Ducale with the well-known Castello di San Giorgio which host the noteworthy "Camera degli Sposi". This building, the most famous of the city, was so damaged that it was closed for more than one year after the earthquake. The emblem of the Palace and Mantova itself, the previously cited "Camera degli Sposi" realized by Andrea Mantegna, was damaged and all the economic and social life of the city was deeply affected. Immediately after the earthquake, the Soprintendenza per i Beni Architettonici e Paesaggistici of Brescia, Cremona and Mantova establish an agreement with the University Iuav of Venice, requiring an analysis and assessment of the damage in order to proceed with the development of an intervention project. This activity turned out to be very important not only from the point of view of the recovery of the architectural and artistic heritage but also because the city's economy is based primarily on tourism. Aims and objectives This exploratory research investigates obese patients' beliefs and expectations before and one year after bariatric surgery. Changes and resistance to change in the identity system, in the perception of the body, in relationships and in the quality of life were of particular interest. Background Although bariatric surgery represents a promising treatment for obesity, nevertheless, it is still under utilised. This may be because, either the intervention is not always easily accessible or because very little is known about its psychological implications. Design Open answers provided by participants were analysed through discourse analysis. Methods A semi-structured interview about the exchanges in self-representations through time was administered to 30 Italian women, 15 of whom were obese before surgery and 15 were ex-obese, one year after surgery. Results The research shows that those participants who have already been operated on are generally satisfied with the results obtained; however, they faced considerable difficulties in adapting their identity to their new body. Participants reported that even one year after surgery they still thought, behaved and related to others as though they were still obese, and only after a change in awareness did they become able to realise they are now ex-obese. Conclusions This paper contributes to the understanding of the unrealistic expectations of patients before surgery compared with the real changes in their lives, as well the difficulties they still faced one year later. More studies in evaluating how bariatric patients live with this long-term condition are recommended to foster patient-centered care. Relevance to clinical practice Health professionals should also help people to face the difficulties of the postsurgery period, which are linked to the acceptance of the new body and of the new emerging self-image in relationships with others. The article discusses an study on assistance services for sexual activity in Italy and the opinions of people with disabilities and the potential assistants. Topics include the views of clients and assistants on sexual assistance (SA) services and their related needs, concerns, and experiences, the use of qualitative methods, semi-structured interviews, and a Grounded Theory approach, and results showing that clients still prefer to have their sexual needs met in a romantic relationship.

Today, Italian prisons are reporting the highest level of overcrowding ever recorded. In reference to this situation, this article proposes an ethnographic study carried out inside the prison of Padua as a voluntary 2 years’ experience. It documents the values and conventions that convicts share in prison and details the ways in which prisoners constantly construct and adapt to an informal conduct’s rule system, the inmate code. It also illustrates the interactions among fellow prisoners as scenes or plays enacted by various teams. The prisoners’ words reveal a complex universe based upon three basic conditions: loyalty, discipline and circumspection. Finally, we argue about the fading of the distinction between the prison front stage and the back stage and we analyse the possible consequences. [ABSTRACT FROM AUTHOR]

Objective: This study explores the time experience of Italian couples undergoing medically assisted reproductive procedures.Background: Research has mainly focused on the motivation for childbearing and the impact of infertility on women, with little attention given to the couple’s experience during assisted reproduction procedures.Methods: We conducted semi-structured interviews with nine women and seven men undergoing a medically assisted reproductive programme at a fertility clinic in Italy. An interpretative phenomenological analysis was used to analyse the transcripts.Results: Four main themes were identified: present moment, waiting, hope, and death. Each theme describes the experiences of the study participants and was linked to their personal identities and body perceptions.Conclusion: Time is a significant dimension in understanding participants’ experience because it represents the contradictory emotions and expectations that characterise a medically assisted reproductive journey. Health professionals might take into account the indications derived from these results to forge their approach to patients’ needs, and policymakers might adjust the organisation of the medically assisted reproductive system to patients’ time experience. Purpose – Flexible automated assembly is an emerging need in several industries. The purpose of this paper is to address the introduction of an innovative concept in flexible assembly: the fully flexible assembly system (F-FAS). Design/methodology/approach – After an analysis of the state of the art, the authors describe the proposed F-FAS, from a layout, constitutional elements, functioning principles and working cycle point of view. Second, the authors compare the traditional FAS and the manual assembly system versus the proposed F-FAS according to their throughput and unit production costs, deriving a convenience map as a function of the number of components used in assembly and of the efficiency of the F-FAS. Finally, using a prototype work cell developed at the Robotics Laboratory of University of Padua, the authors validate the F-FAS concept. Findings – Results of the research indicate that the concept of full-flexibility can be exploited to bring automation to a domain where traditional FAS are not competitive versus manual assembly. In fact, the F-FAS outperforms both traditional FAS and manual assembly, in terms of unit direct production costs, when the size of the batch is small, the number of components used in assembly is large and the efficiency of the F-FAS is reasonably high. The F-FAS prototype demonstrated the possibility of working, for certain conditions (models/components/production mix), in the F-FAS convenience area, highlighting the achievable cost reduction versus traditional assembly systems. Originality/value – The novelty of the study lies in the F-FAS concept, its performances in terms of flexibility, compactness, throughput and unit direct production costs.
