Due to its high short-term global warming potential (GWP) compared to carbon dioxide, methane (CH4) is a considerable agent of climate change. This research is aimed at analyzing data on methane gathered at the GAW (Global Atmosphere Watch) station of Lamezia Terme (Calabria, Southern Italy) spanning seven years of continuous measurements (2016–2022) and integrating the results with key meteorological data. Compared to previous studies on detected methane mole fractions at the same station, daily-to-yearly patterns have become more prominent thanks to the analysis of a much larger dataset. Overall, the yearly increase of methane at the Lamezia Terme station is in general agreement with global measurements by NOAA, though local peaks are present, and an increase linked to COVID-19 is identified. Seasonal changes and trends have proved to be fully cyclic, with the daily cycles being largely driven by local wind circulation patterns and synoptic features. Outbreak events have been statistically evaluated depending on their weekday of occurrence to test possible correlations with anthropogenic activities. A cross analysis between methane peaks and specific wind directions has also proved that local sources may be deemed responsible for the highest mole fractions. [ABSTRACT FROM AUTHOR]

In Southern Mediterranean regions, the issue of summer fires related to agriculture practices is a periodic recurrence. It implies a significant increase in carbon dioxide (CO2) emissions and other combustion-related gaseous and particles compounds emitted into the atmosphere with potential impacts on air quality and global climate. In this work, we performed an analysis of summer fire events that occurred on August 2021. Measurements were carried out at the permanent World Meteorological Organization (WMO)/Global Atmosphere Watch (GAW) station of Lamezia Terme (Code: LMT) in Calabria, Southern Italy. The observatory is equipped with greenhouse gases and black carbon analyzers, an atmospheric particulate impactor system, and a meteo-station for atmospheric parameters to characterize atmospheric mechanisms and transport for land and sea breezes occurrences. High mole fractions of carbon monoxide (CO) and carbon dioxide (CO2) coming from quadrants of inland areas were correlated with fire counts detected via the MODIS satellite (GFED-Global Fire Emissions Database) at 1 km of spatial resolution. In comparison with the typical summer values, higher CO and CO2 were observed in August 2021. Furthermore, the growth in CO concentration values in the tropospheric column was also highlighted by the analyses of the L2 products of the Copernicus SP5 satellite. Wind fields were reconstructed via a Weather Research and Forecasting (WRF) output, the latter suggesting a possible contribution from open fire events observed at the inland region near the observatory. So far, there have been no documented estimates of the effect of prescribed burning on carbon emissions in this region. In recent years, there has been a more prudent use of drugs on livestock farms and alternative products have been considered, with a view of reducing the risk of the onset of antibiotic resistance. Salmonella enterica subsp. enterica serovar Enteritidis (S. Enteritidis) may cause disease in poultry, and it is also responsible for human food poisoning. The aim of this study was to evaluate the efficacy of garlic against S. Enteritidis and to define its Minimal Inhibitory Concentration (MIC)90 and MIC50 values. The study was carried out in vitro, testing 26 S. Enteritidis strains identified in laying hens from various farms in Southern Italy. A preliminary efficacy trial was carried out on two strains, S. Enteritidis and Escherichia coli, using a garlic concentration of 10 mg/mL (1%). Later, 26 strains of S. Enteritidis at 106 Colony Forming Unit (CFU) and 104 CFU were tested with different concentrations of garlic, ranging from 10 mg/mL to 1 mg/mL. Based on the results, intermediate concentrations of garlic, from 5 mg/mL to 4 mg/mL and 4 mg/mL to 3 mg/mL, were used to test 106 CFU and 104 CFU, respectively. The data were statistically analyzed. The MIC90 was 4.75 mg/mL for strains tested at 106 CFU and 4 mg/mL for strains tested at 104 CFU. The results highlight garlic's potential to inhibit the growth of Salmonella enterica ser. Enteritidis in vitro. Efficacy was dependent on the microbial concentration used. In vivo efficacy trials will be crucial to confirm the efficacy of garlic against S. Enteritidis and to assess whether garlic can be used in poultry flocks to prevent the spread of the bacterium in the field. [ABSTRACT FROM AUTHOR]

The main purpose of this study was to investigate the influence of three different rabbit rearing systems using animal-based measures. Therefore, in 2021, cortisol and IL-6 were assessed during a Biosecurity System pilot program to evaluate rabbit welfare, based on EU Regulation 2016/429 in the Apulia region of Southern Italy. The study was carried out on a total of 45 meat-type rabbits, divided into 3 groups of 15 subjects, aged approximately 60–65 days. Groups came from either: (i) a niche open-air system; (ii) a niche system in a shed with conventional cages, or (iii) an industrial system. Based on our findings, the rearing system had a significant effect (p < 0.0001) on the animal-based measures. Cortisol levels were higher in rabbits raised in the niche open-air system compared to those raised in both the niche system located in a shed with conventional cages and the industrial system (11.91 vs. 2.86 and 2.72 ng/mL, respectively). Likewise, IL-6 values were higher in rabbits from the niche open-air system (45.80 ng/mL) and lower in rabbits from the niche system in a shed with conventional cages and those from the industrial system (23.30 and 16.80 ng/mL, respectively). According to the results of the stress indicators measured, cortisol and IL-6, rabbit welfare and meat quality may be affected by rabbit rearing systems. [ABSTRACT FROM AUTHOR]

The aim of the paper is to disentangle the roles that patients, physicians and technology can have on patient health outcomes. The analysis focuses on patients suffering from hypercholesterolemia. Using a large and detailed dataset of patients collected by the Italian College of General Practitioners (SIMG) over the period 2001-2006, we observe the existence of heterogeneity in the time needed to reach an optimal level of health stock. We firstly explore whether patients recovering faster exhibit lower hospitalization rates. Secondly, we study the determinants of the speed of recovery to a good health status. Results suggest that a 10% increase in the speed of recovery reduces hospitalization rates by 1% in the general sample and by 1.25% in patients in primary prevention. Furthermore, we show that recovering to a good health status is a multifaceted phenomenon, with technology explaining from 54 to 68% of the total effect. [ABSTRACT FROM AUTHOR]

Simple Summary: The XXL trial has recently shown that biological downstaging is an effective strategy to also allow liver transplantation into patients with more advanced hepatocellular carcinoma without alternative curative options. Some potential limits of the XXL downstaging protocol are (a) the rather high downstaging failure rate (i.e., 32%), and (b) the additional prioritization of transplantation for patients with a potential good prognosis without transplant, i.e., those obtaining a complete response to downstaging. In this study, we showed that, using aggressive surgical downstaging, it is possible to considerably decrease the downstaging failure rate. Moreover, we showed that it is possible to avoid an immediate prioritization of transplantation for patients with a sustained complete response to downstaging by applying a "wait and see" policy. This policy seems to spare a relevant number of organs without worsening patient outcome. The XXL trial represents the first prospective validation of "biological downstaging" in liver transplantation (LT) for hepatocellular carcinoma. The aim of this study was to compare the Padua downstaging protocol to the XXL protocol in terms of downstaging failure rates and patient outcome. A total of 191 patients undergoing aggressive surgical downstaging and potentially eligible for LT from 2012 to 2018 at our center were retrospectively selected according to XXL trial criteria. Unlike the XXL trial, patients with a complete response to downstaging did not receive any prioritization for LT. Downstaging failure was defined as stable progressive disease or post-treatment mortality. The statistical method of "matching-adjusted indirect comparison" was used to match the study group to the XXL population. Downstaging failure rate was considerably lower in the study group than in the XXL trial (12% vs. 32%, d value = |0.683|). The survival curves of our LT group (n = 68) overlapped with those of the LT-XXL group (p = 0.846). Survival curves of non-LT candidates with a sustained complete response (n = 64) were similar to those of transplanted patients (p = 0.281). Our study represents a validation of the current Padua and Italian policies of denying rapid prioritization to patients with complete response to downstaging. This set-up, which exploits the UV synchrotron radiation source available at Elettra (Trieste, Italy), results in an innovative spectroscopic facility to be used for addressing a large array of open problems, ranging from the electronic properties of nanostructures and strongly correlated materials to biochemistry. The southeast of Italy is characterized by the presence of some highly phytogeographically relevant taxa with a Balkan-Aegean distribution. Three of these taxa, Cainpanula versicolor, Carum multiflorum and Scrophularia lucida, characterize the chasmophytic vegetation in central-southern Apulia (Serre Salentine and Bassa Murgia). Chasmophytic vegetation has already been referred to the Onosmetaliafrutescentis order and to its only alliance Campanulion versicoloris, which are syntaxa with mainly a south-western Balkan distribution. This paper reports a phytosociological study of chasmophytic vegetation throughout the entire distributional range of these three species in south-eastern Italy and describes two new associations: Piptathero holciformis-Campanuletum versicoloris and Iberido carnosae-Athamantetum siculi. We analyse their floristic relationships with other communities described for southern Italy, the Adriatic area and the southern Balkans. As distinct from reports in the literature, the results show that south-eastern Italian rupicolous communities, now grouped under the new alliance Caro multiflori-Aurinion megalocarpae, have higher floristic similarity with the order Asplenietalia glandulosi, where they have been placed. In other words, the presence of different species gravitating in the East discriminates the chasmophytic vegetation of south-eastern Italy within a central Mediterranean order, rather than justifying its assignment to Onosmetaliafrutescentis, whose distributional area is limited to the Balkan Peninsula. Olive groves represent an important economic, agro-ecological, and cultural resource in the Mediterranean Basin. Weed management plays a fundamental role in their sustainable management. The aim of this work was to characterize and assess the plant diversity associated with different weed control practices, in a homogeneous olive-dominated landscape in the South-East of Italy. Sixty-five vegetation plots were sampled in orchards treated with different weed control practices: mowing, tillage, and use of chemical herbicides. The multi-response permutation procedure was used to test the hypothesis of no difference among the treatments. The relationships between plots were visualized by means of non-metric multidimensional scaling ordination. A generalized linear mixed model was used to analyze the relationships between weed control practices and life forms, chorotypes, and diversity indexes. The results showed that the three weed control practices determined slightly different plant communities. Chemically weeded orchards showed an impoverished floristic composition and the lowest diversity, whereas mowing and tillage yielded similar values. These latter two treatments differed for the percentages of hemicryptophytes and therophytes. Moreover, different from other studies, we did not find plant species of particular concern for biodiversity conservation. We hypothesize that this result is due to the monotonous structure of the agro-landscape we investigated, where natural elements are almost lacking. From this point of view, a correct management of agro-districts should consider both the agronomic practices at the level of the individual olive groves and the structure of the agro-landscape. Split liver transplant(ation) (SLT) is still considered a challenging procedure that is by no means widely accepted. We aimed to present data on 25-year trends in SLT in Italy, and to investigate if, and to what extent, outcomes have improved nationwide during this time. The study included all consecutive SLTs performed from May 1993 to December 2019, divided into three consecutive periods: 1993–2005, 2006–2014, and 2015–2019, which match changes in national allocation policies. Primary outcomes were patient and graft survival, and the relative impact of each study period. SLT accounted for 8.9% of all liver transplants performed in Italy. A total of 1,715 in situ split liver grafts were included in the analysis: 868 left lateral segments (LLSs) and 847 extended right grafts (ERGs). A significant improvement in patient and graft survival (p <0.001) was observed with ERGs over the three periods. Predictors of graft survival were cold ischaemia time (CIT) <6 h (p = 0.009), UNOS status 2b (p < 0.001), UNOS status 3 (p = 0.009), and transplant centre volumes: 25–50 cases vs. <25 cases (p = 0.003). Patient survival was significantly higher with LLS grafts in period 2 vs. period 1 (p = 0.008). No significant improvement in graft survival was seen over the three periods, where predictors of graft survival were CIT <6 h (p = 0.007), CIT <6 h vs. ≥10 h (p = 0.019), UNOS status 2b (p = 0.038), and UNOS status 3 (p = 0.009). Retransplantation was a risk factor in split liver graft recipients, with significantly worse graft and patient survival for both types of graft (p <0.001). Our analysis showed Italian SLT outcomes to have improved over the last 25 years. These results could help to dispel reservations regarding the use of this procedure. With more than 1,700 cases, it is one of the largest series, examining long-term national trends in in situ SLT since its introduction. The data presented indicate that the outcomes of SLT improved during this 25-year period. Improvements are probably due to better recipient selection, refinements in surgical technique, conservative graft-to-recipient matching, and the continuous, yet carefully managed, expansion of donor selection criteria under a strict mandatory split liver allocation policy. In light of the adoption of recent global policies and strategies for a more sustainable food waste management system and a greater control of environmental impacts, this study describes a circular bioeconomy plant model, currently operating in South Italy, for treatment and enhancement of organic fraction of municipal solid waste. The key basis for plant activity is dry anaerobic digestion of separately collected organic fraction of municipal solid waste (SC-OFMSW) followed by digestate composting process. Biomethane for use in the transport sector is obtained by biogas cleaning and upgrading, while high-quality compost for organic farming is produced by digestate composting. Plant activities are completed by the transformation of part of the produced waste into refuse derived fuel (RDF) to be allocated to waste-to-energy plants and the treatment of odour emissions which allows the recovery of ammonium sulphate as a fertilizer. A rooftop photovoltaic system supplies most of electric energy needed by the plant. For plant activities relative to 2020, carbon footprint was equal to −112 kg CO 2 eq. for Mg of organic waste, while depletion of fossil resources was estimated at −89.6 kg oil eq. Mg−1 of waste. Primary energy demand of food waste treatment system was −2.66 GJ Mg−1 of input waste. Background/Aims: Prognosis assessment in patients with hepatocellular carcinoma (HCC) remains controversial. The most widely used HCC prognostic tool is the Okuda classification, but new staging systems (Cancer of the Liver Italian Program score, Chinese University Prognostic Index, French classification and Barcelona Clinic Liver Cancer, BCLC, staging) have been recently described. We investigated the value of known prognostic systems in the particular setting of a surgically oriented Liver Unit where 187 HCC Italian patients were mainly treated with radical therapies (resection and percutaneous ablation).Methods: A retrospective analysis of 187 HCCs observed at a single Institution from 1990 and 1999 was performed. By using survival time as the only outcome measure (Kaplan–Meier method and Cox regression), the performance of any prognostic system was assessed according the criteria of discriminatory and stratification abilities between different stages, homogeneity of survival within each stage and additional explanatory power respect to the other classifications.Results: In the particular cohort studied, BCLC proved the best HCC prognostic system. This was true for the whole study group and for the 2 subgroups of surgical and non-surgical patients.Conclusions: BCLC staging showed the best interpretation of the survival distribution in an HCC population comprising a large proportion of tumors treated with potentially radical therapies. Aims: Drug-eluting stent (DES) implantation is an effective treatment of in-stent restenosis (ISR). However, literature data indicate that drug-coated balloons (DCBs) may be a valid alternative, particularly for recurrent ISR. We sought to evaluate clinical results on the long-term efficacy of a new DCB for ISR treatment., Methods: One hundred and ninety-nine patients were treated with paclitaxel drug-coated balloons (Pantera Lux, Biotronik, Switzerland) in the Italian REGistry of Paclitaxel Eluting Balloon in ISR (REGPEB study). Clinical follow-up was scheduled at 1 and 12 months. A subgroup of patients received adjunctive 5-year follow-up. Primary end point was Major Adverse Cardiac Events (MACE) at 1 year., Results: A total of 214 ISR coronary lesions were treated (75.4% DES-ISR). Mean time between stent implantation and DCB treatment is 41 months. DCBs were successfully delivered in 99% of the cases; crossover to a DES occurred in 3% of cases. Procedural success rate was 98.5%. Clinical success rate was 98.5%. First-month follow-up compliance was 98% and freedom from MACE was 96.9%. Twelve-month follow-up compliance was 89.3% with a freedom from MACE rate of 87.3% (CI: 81.3-91.5%). Five-year long-term follow-up showed 65.2% of freedom from MACE., Conclusion: Our study confirms that Pantera Lux treatment is effective and well tolerated in ISR, showing good acute and long/very long-term results in the treatment of complex lesions (DES and late ISR)., (Copyright © 2021 Italian Federation of Cardiology - I.F.C. Antioxidant agents have the potential to reduce ischemia/reperfusion damage to organs for liver transplantation (LT). In this prospective, randomized study, we tested the impact of an infusion of N-acetylcysteine (NAC) during liver procurement on post-LT outcomes. Between December 2006 and July 2009, 140 grafts were transplanted into adult candidates with chronic liver disease who were listed for first LT, and according to a sequential, closed-envelope, single-blinded procedure, these patients were randomly assigned in a 1/1 ratio to an NAC protocol (69 patients) or to the standard protocol without NAC [71 patients (the control group)]. The NAC protocol included a systemic NAC infusion (30 mg/kg) 1 hour before the beginning of liver procurement and a locoregional NAC infusion (300 mg through the portal vein) just before cross-clamping. The primary endpoint was graft survival. The graft survival rates at 3 and 12 months were 93% and 90%, respectively, in the NAC group and 82% and 70%, respectively, in the control group (P = 0.02). An adjusted Cox analysis showed a significant NAC effect on graft survival at both 3 months [hazard ratio = 1.65, 95% confidence interval (CI) = 1.01-2.93, P = 0.04] and 12 months (hazard ratio = 1.73, 95% CI = 1.14-2.76, P ≤ 0.01). The incidence of postoperative complications was lower in the NAC group (23%) versus the control group (51%, P < 0.01). In the subgroup of 61 patients (44%) receiving suboptimal grafts (donor risk index > 1.8), the incidence of primary dysfunction of the liver was lower (P = 0.09) for the NAC group (15%) versus the control group (32%). In conclusion, the NAC harvesting protocol significantly improves graft survival. Objective: We surveyed the datasheets of 29 laboratories concerning prenatal diagnosis of de novo apparently balanced chromosome rearrangements to assess the involvement of specific chromosomes, the breakpoints distribution and the impact on the pregnancy outcome., Method: By means of a questionnaire, data on 269.371 analyses performed from 1983 to 2006 on amniotic fluid, chorionic villus and fetal blood samples were collected., Results: A total of 246 balanced anomalies were detected at frequencies of 72% for reciprocal translocations, 18% for Robertsonian translocations, 7% for inversions and 3% for complex chromosome rearrangements. The total frequencies of balanced rearrangements were 0.09%, 0.08% and 0.05% on amniotic fluid, chorionic villus and fetal blood samples., Conclusion: A preferential involvement of chromosomes 22, 7, 21, 3, 9 and 11 and a less involvement of chromosomes X, 19, 12, 6 and 1 was observed. A nonrandom distribution of the breakpoints across chromosomes was noticed. Association in the location of recurrent breakpoints and fragile sites was observed for chromosomes 11, 7, 10 and 22, while it was not recorded for chromosome 3. The rate of pregnancy termination was about 20%, with frequencies decreasing from complex chromosomal rearrangements (33%), reciprocal translocations (24%) to inversions (11%) and Robertsonian translocations (3%).

Purpose: We evaluated the experiences of 19 Italian laboratories concerning 241 small supernumerary marker chromosomes (sSMCs) with the aim of answering questions arising from their origin from any chromosome, their variable size and genetic content, and their impact on the carrier's phenotype., Methods: Conventional protocols were used to set up the cultures and chromosome preparations. Both commercial and homemade probes were used for the fluorescent in situ hybridization analyses., Results: A total of 113 of the 241 sSMCs were detected antenatally, and 128 were detected postnatally. There were 52 inherited and 172 de novo cases. Abnormal phenotype was present in 137 cases (57%), 38 of which were antenatally diagnosed. A mosaic condition was observed in 87 cases (36%). In terms of morphology, monocentric and dicentric bisatellited marker chromosomes were the most common, followed by monocentric rings and short-arm isochromosomes. The chromosomes generating the sSMCs were acrocentric in 132 cases (69%) and non-acrocentric chromosomes in 60 cases (31%); a neocentromere was hypothesized in three cases involving chromosomes 6, 8, and 15., Conclusion: The presented and published data still do not allow any definite conclusions to be drawn concerning karyotype-phenotype correlations. Only concerted efforts to characterize molecularly the sSMCs associated or not with a clinical phenotype can yield results suitable for addressing karyotype-phenotype correlations in support of genetic counseling.
