The SARS-CoV-2 epidemic is pressuring healthcare systems worldwide. Disease outcomes in certain subgroups of patients are still scarce, and data are needed. Therefore, we describe here the experience of four dialysis centers of the Brescia Renal COVID Task Force. During March 2020, within an overall population of 643 hemodialysis patients, SARS-CoV-2 RNA positivity was detected in 94 (15%). At disease diagnosis, 37 of the 94 (39%) patients (group 1) were managed on an outpatient basis, whereas the remaining 57 (61%) (group 2) required hospitalization. Choices regarding management strategy were made based on disease severity. In group 1, 41% received antivirals and 76% hydroxychloroquine. Eight percent died and 5% developed acute respiratory distress syndrome (ARDS). In group 2, 79% received antivirals and 77% hydroxychloroquine. Forty two percent died and 79% developed ARDS. Overall mortality rate for the entire cohort was 29%. History of ischemic cardiac disease, fever, older age (over age 70), and dyspnea at presentation were associated with the risk of developing ARDS, whereas fever, cough and a C-reactive protein higher than 50 mg/l at disease presentation were associated with the risk of death. Thus, in our population of hemodialysis patients with SARS-CoV-2 infection, we documented a wide range of disease severity. Digested sludge samples from domestic wastewater treatment plants located in Northern Italy were tested as far as the presence of viruses (enteric viruses and coliphages), bacteria (faecal coliforms, salmonella) and helminth eggs is concerned. Heavy metals were also analysed. Escherichia coli bacteriophages and faecal coliforms were isolated from all samples, while salmonellae and helminth eggs were isolated only from four and three out of 27 total samples, respectively. The 66% of sludge samples, 46 and 82% of aerobic and anaerobic digested sludges respectively, showed the presence of enteric viruses (enteroviruses and reoviruses). The virus concentrations ranged from 0.6 to 123 MPNCU/g. Results of this study suggest that significant concentrations of pathogens such as enteric viruses can be present in digested sludge, which showed compliance with Italian legislation. As suggested by other authors, there is the need for surveillance and reference indications both in EU (European Union) and Italian regulations concerning the use of sludge in agriculture. An Italian, 46-year-old female patient carrying the complex allele p.[R74W;V201M;D1270N] in trans with CFTR dele22_24 was diagnosed at the Cystic Fibrosis (CF) Center of Verona as being affected by CF-pancreatic sufficient (CF-PS) in 2021. The variant V201M has unknown significance, while both of the other variants of this complex allele have variable clinical consequences, according to the CFTR2 database, with reported clinical benefits for treatment with ivacaftor + tezacaftor and ivacaftor + tezacaftor + elexacaftor in patients carrying the R74W-D1270N complex allele, which are currently approved (in USA, not yet in Italy). She was previously followed up by pneumologists in northern Italy because of frequent bronchitis, hemoptysis, recurrent rhinitis, Pseudomonas aeruginosa lung colonization, bronchiectasis/atelectasis, bronchial arterial embolization and moderately compromised lung function (FEV1: 62%). Following a sweat test with borderline results, she was referred to the Verona CF Center where she presented abnormal values in both optical beta-adrenergic sweat tests and intestinal current measurement (ICM). These results were consistent with a diagnosis of CF. CFTR function analyses were also performed in vitro by forskolin-induced swelling (FIS) assay and short-circuit currents (Isc) in the monolayers of the rectal organoids. Both of these assays showed significantly increased CFTR activity following treatment with the CFTR modulators. Western-blot analysis revealed increased fully glycosylated CFTR protein after treatment with correctors, in line with the functional analysis. Interestingly, tezacaftor, together with elexacaftor, rescued the total organoid area under steady-state conditions, even in the absence of the CFTR agonist forskolin. In conclusion, in ex vivo and in vitro assays, we measured a residual function that was significantly enhanced by in vitro incubation with CFTR modulators, especially by ivacaftor + tezacaftor + elexacaftor, suggesting this combination as a potentially optimal treatment for this case. Purpose – In the EU, over the past three decades, poultry production has been growing around the 5 percent per year. This increase determined a higher volume of livestock and poultry farming waste, which represents a potential renewable fuel suitable for waste to energy conversion. One way is through biological process such as the dry anaerobic digestion (AD). Tests normally applied for the measurement of the anaerobic biogas potential (ABP) are reliable, but not feasible as a routine analysis. This kind of test is expensive and time consuming (21-100 days). The purpose of this paper is to propose a rapid method for assessing the potential of biogas in anaerobic poultry manure, with the goal of helping the choices of operators of AD. Design/methodology/approach – The Dynamic Respiration Index was calculated as “key parameter” in order to estimate the ABP, which provides important information about the biogas generation for the poultry manure. Furthermore, two dynamic respirometric approaches were considered developed in two Italian universities. The main difference between them is the control of temperature. Findings – Calculate ABP through IRD is a useful method to determine the value of the potential production of biogas of different biomasses in a short time and assess which one is more productive. Monitoring was carried out during summer 1997 along a selected area of the Tyrrhenian coast near the Tiber river mouth. Fifty-eight seawater samples, collected from 19 stations, were examined for coliforms, streptococci, Enteroviruses, Salmonellae, coliphages, Bacteroides fragilis phages, Pseudomonas, alophilic Vibrios, Aeromonas and yeasts. Salmonellae and coliphages were isolated in 3 and 12 out of 58 samples, respectively. Enteroviruses and Bacteroides fragilis phages were not isolated. Reoviruses were isolated only from 2 out of 58 samples. A limited number of samples of the northern stations located near the Tiber and other river mouths exceeded the guide values for bathing water by the EU Directive. All the southern stations, located near canals, were of very good microbiological quality. Pseudomonas, Vibrio, Aeromonas and yeasts were isolated from all stations and their values in 100 ml of seawater were 10-10[sup 6], 10-10[sup 6], 0-10[sup 6] and 1-10[sup 3], respectively. An extensive disinfection practice carried out on domestic wastes, which are discharged in rivers and canals, probably brought pollution levels of most stations to values within the bacterial standards. The spread of Pseudomonas, Aeromonas, etc. showed that all the coastal area studied was characterized by the presence of organic matter coming from land that can support the presence of opportunistic pathogens and other microbial flora.
