The distribution of heavy metals in plants (Castanea sativa, Sambucus nigra, Verbascum thapsus, Popolus spp., Salix spp., Acer pseudoplatanus, Robinia pseudoacacia) growing in soils from active and abandoned mining areas is of scientific significance as it allows to recognize their ability to survive in a hostile environment and provide useful indications for phytoremediation operations. In this work, soils from the former Hg-mining area of Abbadia San Salvatore (Tuscany, Central Italy) were analyzed for total, leached Hg, % of organic and inorganic-related Hg. The dehydrogenase enzyme activity (DHA) was also measured with the aim to evaluate the status of the soil, being characterized by high Hg contents (up to 1068 mg kg−1). Eventually, the concentration of Hg in the different parts of the plants growing on these soils was also determined. Most studied soils were dominated by inorganic Hg (up to 92%) while the DHA concentrations were < 151 µg TPF g−1 day−1, suggesting that the presence of Hg is not significantly affecting the enzymatic soil activity. This is also supported by the bioaccumulation factor (BF), being predominantly characterized by values < 1. Sambucus nigra and Verbascum thapsus had the highest Hg contents (39.42 and 54.54 mg kg−1, respectively). The plant leaves appear to be the main pathways of Hg uptake, as also observed in other mining areas, e.g., Almadèn (Spain), indicating that particulate-Hg and Hg0 are the main forms entering the plant system, the latter derived by the GEM emitted by both the edifices hosting the roasting furnaces and the soils themselves. High‐temperature geothermal areas are often characterized by widespread surficial manifestations, whose location is strictly controlled by sets of faults of regional relevance. The geochemical and isotopic signature of the discharged fluids can reveal key information on the geothermal fluids pathway, shedding light on the sources and fluid‐rock interaction within the geothermal reservoirs. In this paper, a geochemical and structural data set from the Larderello geothermal area and surroundings is presented and discussed. We constrain the role of transfer and normal faults in controlling the geothermal circulation enhanced by a cooling magmatic intrusion underneath the Lago area (SW of Larderello). The structural control on the fluids circulation is highlighted by both the location of the CO2 emissions along the fault segments, where permeability is enhanced, and their degassing rates, which increase moving away from the core of the Larderello geothermal system. The main results unravel the presence of deep regional pathways along which endogenous fluids circulate before being discharged in the investigated areas. The peripheral zone emissions are affected by interaction with shallow aquifers and condensation processes whereas the CO2 emitted from the central areas, located near the core of the geothermal system, was accompanied by high amounts of steam, and suffers intense shallow fractionation processes. The latter areas emit medium‐to‐low normalized‐CO2‐degassing rates (<270 t d−1 km−2) when compared to the extremely high values occurring in the peripheral sectors (up to 1,300 t d−1 km−2) of the Larderello geothermal systems, possibly suggesting an incipient propagation of such a system, likely wider than previously thought. Plain Language Summary: High‐temperature geothermal areas are specific zones on Earth where the geothermal gradient is greater than the average global value (∼30°C km−1). This is due to the existence of cooling magma source(s) at depth, providing heat that is transmitted to fluid reservoir(s) located at intermediate levels continuously and naturally fed by recharging fluids, and sealing rock(s) at shallower levels that maintain reservoir temperature and pressure. These geothermal areas commonly show steam‐dominated manifestations at the surface, accompanied by relevant degassing of carbon dioxide likely originated from different feeding systems (biogenic, thermometamorphic, and mantellic). Defining sources, processes, and transport mechanisms governing the CO2 emissions from soils is challenging but pivotal to understand the geothermal fluid origin, dynamics, and relation with the geological structures. To unravel these processes, we combined geochemical and structural measurements performed at the Larderello‐Travale‐Radicondoli (LTR) geothermal field (Italy). Distinct geothermal sectors were investigated according to their geochemical characteristics, CO2 degassing rates, and geological‐structural features, to understand how CO2 is transported and modified during its journey from the deep geothermal reservoir(s) to the surface, exploiting the permeability of fault zones and/or fractured rocks. Although terrestrial hydrothermal systems are considered among the most fascinating environments, how their unique and extreme conditions can affect microorganisms selection and the role in biogeochemical cycles has not yet been well elucidated. A combined geochemical and microbiological exploration in waters and sediments from 10 sampling points along a sharp temperature gradient (15–90°C) within an extremely acidic hydrothermal system (Pisciarelli Spring, Campi Flegrei area, southern Italy) displayed how hydrothermal fluids influence the microbial dynamics. This area was characterized by high levels of reduced gaseous species (e.g. H2S, H2, CH4, CO) and very low pH values (<2.3). Thermodynamic calculations revealed a high microbial catabolic potential in oxidation/reduction reactions of N-, S- and Fe-bearing species. Overall, an increase of the archaeal/bacterial abundance ratio was observed by decreasing temperature and pH values. In particular, Archaea and Bacteria were present in almost equal cell abundance (up to 1.1 × 109 and 9.3 × 108 cell/g, respectively) in the <70°C sampling points (average pH = 2.09); on the contrary, the highest temperature waters (85–90°C; average pH = 2.26) were characterized by a low abundance of archaeal cells. The high-throughput sequencing of the 16S rRNA genes indicated strong differences in archaeal and bacterial communities composition along the temperature gradient. However, the microbiome in this extreme environment was mainly constituted by chemoautotrophic microorganisms that were likely involved in N-, S- and Fe-bearing species transformations (e.g. Acidianus infernus , Ferroplasma acidarmanus , Acidithiobacillus , Sulfobacillus , Thaumarchaeota), in agreement with thermodynamic calculations. This study is focused on resistance to carbapenems and third-generation cephalosporins in Gram-negative microorganisms isolated from swine, whose transmission to humans via pork consumption cannot be excluded. In addition, the common carriage of carbapenem-resistant (CR) bacteria between humans and pigs was evaluated. Sampling involved 300 faecal samples collected from slaughtered pigs and 300 urine samples collected from 187 hospitalised patients in Parma Province (Italy). In swine, MIC testing confirmed resistance to meropenem for isolates of Pseudomonas aeruginosa and Pseudomonas oryzihabitans and resistance to cefotaxime and ceftazidime for Escherichia coli, Ewingella americana, Enterobacter agglomerans, and Citrobacter freundii. For Acinetobacter lwoffii, Aeromonas hydrofila, Burkolderia cepacia, Corynebacterium indologenes, Flavobacterium odoratum, and Stenotrophomonas maltophilia, no EUCAST MIC breakpoints were available. However, ESBL genes (blaCTXM-1, blaCTX-M-2, blaTEM-1, and blaSHV) and AmpC genes (blaCIT, blaACC, and blaEBC) were found in 38 and 16 isolates, respectively. P. aeruginosa was the only CR species shared by pigs (4/300 pigs; 1.3%) and patients (2/187; 1.1%). P. aeruginosa ST938 carrying blaPAO and blaOXA396 was detected in one pig as well as an 83-year-old patient. Although no direct epidemiological link was demonstrable, SNP calling and cgMLST showed a genetic relationship of the isolates (86 SNPs and 661 allele difference), thus suggesting possible circulation of CR bacteria between swine and humans. Wider pea (Pisum sativum L.) cultivation has great interest for European agriculture, owing to its favorable environmental impact and provision of high-protein feedstuff. This work aimed to investigate the extent of genotype × environment interaction (GEI), genetically based trade-offs and polygenic control for crude protein content and grain yield of pea targeted to Italian environments, and to assess the efficiency of genomic selection (GS) as an alternative to phenotypic selection (PS) to increase protein yield per unit area. Some 306 genotypes belonging to three connected recombinant inbred line (RIL) populations derived from paired crosses between elite cultivars were genotyped through genotyping-by-sequencing and phenotyped for grain yield and protein content on a dry matter basis in three autumn-sown environments of northern or central Italy. Line variation for mean protein content ranged from 21.7 to 26.6%. Purely genetic effects, compared with GEI effects, were over two-fold larger for protein content, and over 2-fold smaller for grain and protein yield per unit area. Grain yield and protein content exhibited no inverse genetic correlation. A genome-wide association study revealed a definite polygenic control not only for grain yield but also for protein content, with small amounts of trait variation accounted for by individual loci. On average, the GS predictive ability for individual RIL populations based on the rrBLUP model (which was selected out of four tested models) using by turns two environments for selection and one for validation was moderately high for protein content (0.53) and moderate for grain yield (0.40) and protein yield (0.41). These values were about halved for inter-environment, inter-population predictions using one RIL population for model construction to predict data of the other populations. The comparison between GS and PS for protein yield based on predicted gains per unit time and similar evaluation costs indicated an advantage of GS for model construction including the target RIL population and, in case of multi-year PS, even for model training based on data of a non-target population. In conclusion, protein content is less challenging than grain yield for phenotypic or genome-enabled improvement, and GS is promising for the simultaneous improvement of both traits. Although in Europe the quality of swimming pools (SPs) is dictated by regulations, microbiological and chemical hazards are described in the literature. Environmental bacteria or toxic disinfection byproduct (DBP) compounds may indeed be recovered in waters even after disinfection. We evaluated the water quality from 26 outdoor seasonal SPs of the Versilia district, according to requirements of Regional Decree 54R/2015. In spring 2017, supply and reinstatement waters were collected after shock hyperchlorination (10 mg/L) while in summertime, a second sampling of waters before entering the pools, as well as in the pools, was performed after SPs were open to the public. In all samples, microbiological and chemical parameters were determined as defined by Directive 98/83/EC and the Italian Health Ministry. Microbiological data were within suggested limits. The first chemical analyses showed that in 35% of the feeding-pool seawater samples, the halogenated organic compounds were higher than the maximum permissible concentrations (30 µg/L). Pool waters were then dechlorinated and re-treated with hydrogen peroxide (10 mg/L) to ensure the abatement of DBPs (from 164±107 to 0.9 ±0.8 µg/L; p = 0.002). Results highlighted the need of self-controlled procedures for the SPs waters to prevent waterborne diseases and suggested hydrogen peroxide as the most appropriate disinfection method. Klebsiella pneumoniae is the most common Klebsiella species infecting animals and is one of the causing agents of mastitis in cows. The rise of antimicrobial resistance in K. pneumoniae , particularly in strains producing extended-spectrum β-lactamases (ESBLs) and/or carbapenemases, is of concern worldwide. Recently (Regulation UE No 2022/1255), carbapenems and cephalosporins in combination with β-lactamase inhibitors have been reserved only to human treatments in the European Union. The aim of this study was to investigate the role of cattle as carrier of human pathogenic carbapenem-resistant (CR) and ESBL-producing K. pneumoniae. On this purpose, a study involving 150 dairy farms in Parma province (Northern Italy) and 14 non replicate K. pneumoniae isolates from patients admitted at Parma University-Hospital was planned. Four multidrug resistant (MDR) K. pneumoniae strains were detected from 258 milk filters collected between 2019 and 2021. One carbapenemase KPC-3-positive K. pneumoniae ST307 (0.4 %; 95 % CI – 0.07 - 2.2) was detected in milk filters. The isolate also harboured OXA-9, CTX-M-15 and SHV-106 determinants, together with genes conferring resistance to aminoglycosides (aac(3′)-IIa, aph (3″)-Ib, aph (6)-Id), fluoroquinolones (oqxA, oqxB, qnrB1), phosphonic acids (fosA6), sulphonamides (sul2), tetracyclines (tet(A)6) and trimethoprim (dfrA14). One KPC-3-producing K. pneumoniae ST307 was identified also among the human isolates, thus suggesting a possible circulation of pathogens out of the clinical settings. The remaining three bovine isolates were MDR ESBL-producing K. pneumoniae characterized by different genomic profiles: CTX-M-15, TEM-1B and SHV-187 genes (ST513); CTX-M-15 and SHV-145 (ST307); SHV-187 and DHA-1 (ST307). Occurrence of ESBL-producing K. pneumoniae in milk filters was 1.2 % (95 % CI 0.4–3.4). All the isolates showed resistance to aminoglycosides, 3rd-generation cephalosporins, and fluoroquinolones. Among the human isolates, two multidrug resistant ESBL-producing K. pneumoniae ST307 were found, thus confirming the circulation of this high-risk lineage between humans and cattle. Our findings suggest that food-producing animals can carry human pathogenic microorganisms harboring resistance genes against carbapenems and 3rd-generation cephalosporins, even if not treated with such antimicrobials. This paper focuses on the chemical and isotopic features of dissolved gases (CH and CO) from four meromictic lakes hosted in volcanic systems of Central-Southern Italy: Lake Albano (Alban Hills), Lake Averno (Phlegrean Fields), and Monticchio Grande and Piccolo lakes (Mt. Vulture). Deep waters in these lakes are characterized by the presence of a significant reservoir of extra-atmospheric dissolved gases mainly consisting of CH and CO. The δC-CH and δD-CH values of dissolved gas samples from the maximum depths of the investigated lakes (from −66.8 to −55.6 ‰ V-PDB and from −279 to −195 ‰ V-SMOW, respectively) suggest that CH is mainly produced by microbial activity. The δC-CO values of Lake Grande, Lake Piccolo, and Lake Albano (ranging from −5.8 to −0.4 ‰ V-PDB) indicate a significant CO contribution from sublacustrine vents originating from (1) mantle degassing and (2) thermometamorphic reactions involving limestone, i.e., the same CO source feeding the regional thermal and cold CO-rich fluid emissions. In contrast, the relatively low δC-CO values (from −13.4 to −8.2 ‰ V-PDB) of Lake Averno indicate a prevalent organic CO. Chemical and isotopic compositions of dissolved CO and CH at different depths are mainly depending on (1) CO inputs from external sources (hydrothermal and/or anthropogenic); (2) CO-CH isotopic exchange; and (3) methanogenic and methanotrophic activity. In the epilimnion, vertical water mixing, free oxygen availability, and photosynthesis cause the dramatic decrease of both CO and CH concentrations. In the hypolimnion, where the δC-CO values progressively increase with depth and the δC-CH values show an opposite trend, biogenic CO production from CH using different electron donor species, such as sulfate, tend to counteract the methanogenesis process whose efficiency achieves its climax at the water-bottom sediment interface. Theoretical values, calculated on the basis of δC-CO values, and measured δC values are not consistent, indicating that CO and the main carbon-bearing ion species (HCO) are not in isotopic equilibrium, likely due to the fast kinetics of biochemical processes involving both CO and CH. This study demonstrates that the vertical patterns of the CO/CH ratio and of δC-CO and δC-CH are to be regarded as promising tools to detect perturbations, related to different causes, such as changes in the CO input from sublacustrine springs, that may affect aerobic and anaerobic layers of meromictic volcanic lakes. We present the first real attempt to directly and continuously measure GEM through a Lumex RA-915 M, designed for real-time detection of mercury vapor, mounted on an UAV (Unmanned Aerial Vehicle, namely a heavy-lift octocopter), inside and outside the former Hg-mining area of Abbadia San Salvatore (Mt. Amiata, Italy), known as a GEM source. We tested the effectiveness of the UAV-Lumex combination at different heights in selected sites pertaining to both mining facilities and surrounding urban zones, shedding light on the GEM spatial distribution and concentration variability. The Lumex great sensitivity and the octocopter optimal versatility and maneuverability, both horizontally and vertically, allowed to depict the GEM distribution in the atmosphere up to 60 m above the ground. The acquisition system was further optimized by: i) synchronizing Lumex and UAV GPS data by means of a stand-alone GPS that was previously synchronized with Lumex; ii) using a vertical sampling tube (1.20 m high) connected to the Lumex inlet to overcome the rotors strong airflows and turbulence that would have affected GEM measurements; iii) supplying the octocopter with batteries for power supply to avoid the release of exhaust gases; iv) taking the advantage of the UAV ability to land in small spaces and stop at selected altitudes. The resulting dot-map graphical representations, providing a realistic 3D picture of GEM vertical profiling during the flights in near real-time, were useful to verify whether the guideline concentrations indicated by competent authorities were exceeded. The results showed that the GEM concentrations in the urban area, located a few hundred meters from the mining structures, and close to already reclaimed areas remained at relatively low values. Contrarily, GEM contents showed significant variations and the highest concentrations above the facilities containing the old furnaces, where increasing GEM concentrations were recorded at decreasing heights or downwind. Diffuse soil fluxes of gaseous elemental mercury (GEM) were measured in a hydrothermal area (20,000 m2) located inside " Le Biancane " natural park, north of Monterotondo Marittimo village (Larderello-Travale geothermal field, Central Italy), using a static closed chamber (SCC) coupled with a Lumex® RA-915 M portable analyzer. At 89 sites, GEM and CO 2 fluxes (ϕGEM and ϕCO 2 , respectively) and temperature at the soil-air interface were measured. The ϕGEM values showed no correlation with those of ϕCO 2 and soil temperatures, suggesting that secondary chemical-physical processes affected these gases as they move toward the surface. Formation of Hg-bearing minerals from primary GEM and secondary Hg2+, as well as secondary GEM production likely occurred through processes mediated by microbial activity, particularly in those areas characterized by soils affected by strong air contamination (CO 2 /N 2 ratio < 0.1), where high ϕGEM and low ϕCO 2 values were measured. Mercury is a toxic and noxious element and is the only metal that naturally occurs as gas. One of the most challenging topics (included in the United Nations Minimata convention) is to understand the adsorption–release processes of manmade materials (e.g., concrete, bricks, tiles, painting). Adsorption of Hg by construction and demolition waste materials has recently been studied, but investigations on how much Hg0 can be released from these products are rather poor. The abandoned mining site of Abbadia San Salvatore (Siena, central Italy) where, for about one century, cinnabar was roasted to produce liquid mercury, is known for the high concentrations of (i) Hg0 in edifices and structures and (ii) total and leachate Hg in synthetic materials. In the present paper, a new, simple and low-cost method to measure the amount of GEM (Gaseous Elemental Mercury) released from anthropic materials (concrete, wall rocks, and tiles) located in the Hg0-rich environments of the former mining site, is proposed. The adoption of precision agriculture has the potential to increase the environmental sustainability of cropping systems as well as farmers' income. Farmers in transition to precision agriculture need low-input and effective protocols to delineate homogenous management zones to optimize their actions without past knowledge e.g., yield maps. Different approaches have been developed so far, based on the analysis of the within-field variability in crop and soil properties, but procedures were rarely suited for operational conditions. We identified here a low-inputs protocol to map management zones from soil electrical conductivity and/or crop vegetation indices, using a winter wheat field in northern Italy as a pilot case. The reliability of the alternative data sources was evaluated at three crop development stages using a yield map as reference. Red-edge and NIR (NDRE) bands were the most reliable data sources for management zones identification, with 62%, 68%, and 74% of correct classifications at early tillering, stem elongation, and late booting, respectively. Our work identifies a minimum dataset for accurate management zones' definition and highlights that in-season monitoring based on the red-edge band was able to reliably identify management zones already at early tillering, despite minor differences in crop growth. Abstract The Colli Albani volcanic complex (Lazio, Italy) hosts areas characterized by anomalously high emissions of CO 2 -rich gases (e.g. Tivoli, Cava dei Selci, Tor Caldara, Solforata). The source of these gases is a regional aquifer within the Mesozoic carbonate rock sequences. These degassing zones release significant concentrations of H 2 S and other toxic gases (e.g. GEM: Gaseous Elemental Mercury, and Rn) and represent a serious hazard for local inhabitants, especially for those living at Cava dei Selci (near Rome, Italy), where the emitting areas are nested inside residential neighborhoods. In April 2016, a comprehensive geochemical survey was carried out in an abandoned stone quarry nearby the urban settlement aimed to: (i) investigate the gas composition from both punctual discharges and anomalously high diffuse soil degassing sites, and (ii) evaluate their environmental impact on the local air quality. The spatial distribution of the soil CO 2 fluxes was mainly dependent on the local geostructural setting, whereas shallow secondary processes (e.g. oxidation and gas-water interaction) likely represent the main controlling factor on reactive and/or water-soluble gas species, such as CH 4 and H 2 S. The total output of CO 2 from the abandoned stone quarry accounted for 0.53% of total CO 2 discharged from the whole Colli Albani volcanic district. The naturally emitted toxic gases (e.g. CO 2 , H 2 S, CH 4 , GEM) largely affect the air quality and pose a serious threat for the health of the local residents. A mobile multi-instrumental station able to continuously and simultaneously acquire CO 2 , H 2 S, SO 2 , CH 4 , GEM and CO was deployed to verify the concentrations of both the main deep-originated gas compounds and potential secondary gaseous contaminants (i.e. SO 2) around and inside the urban settlement most exposed to the lethal gases. Hydrogen sulfide was found to be the most impacting gas, occasionally exceeding the 24-h air quality guideline for ambient air and causing odor annoyance at a distance up to more than 250 m downwind from the emitting area. In poorly ventilated basements, toxic gas accumulations up to hazardous levels were measured, producing anomalous outdoor air concentrations at the street level in front of the descending vehicular access to private garages and relatively far from the main emitting area. The geochemical survey, carried out via mobile station and soil gas measurements, resulted to be particularly efficient for evaluating the potential effects caused by gas emissions in inhabited areas. The multi-measurement approach adopted in the present study is of paramount importance for managing future urban development plans. • A site-specific N recommendation system for maize was developed. • Soil electrical conductivity and vegetation index maps were merged. • Site-specific crop responses to N were developed for each zone of the merged map. • The N reccomendations made by the system could allow up to 12% of N savings. The Oklahoma State University algorithm (OSU) is the most widespread recommendation system to calculate the optimal nitrogen (N) rate in cereal crops. This system is based on the map of a crop vegetation index and N -rich strips in the field, serving as a reference for crop vigour. Based on these inputs, yield potential and in-season crop response to N fertilisation are predicted. The system does not consider other factors influencing the spatial variability of crop growth (e.g. , soil), even if the integration of additional data sources could provide more correct N recommendations by considering simultaneously the main drivers of crop growth and production variability. Therefore, the main aims of this study were to modify the OSU algorithm by merging maps of soil electrical conductivity and crop vegetation index (NDRE) to identify management zones instead of using vegetation index alone and to implement and apply the algorithm to define crop response to nitrogen, specific by management zones using a statistical approach instead of N -rich strips in the field. The proposed algorithm was calibrated for maize in northern Italy on four experimental fields in 2021. A comparison among the proposed, the original algorithm and one its previous modification (at Clemson University) was carried out in terms of spatial accuracy and levels of recommended N rates. The proposed algorithm, thanks to the integrated approach, distinguished more in detail different crop responses to N within each management zone and provided N recommendations that match the within-field variability better than the original algorithm and subsequent similar modifications. The new approach resulted in potential average N savings of about 12% compared to uniform management. We conclude that the potential of merging soil and vegetation indices and the definition of N rates with a statistical approach could improve performances and, at the same time, facilitate the adoption of the OSU algorithm. Background and geochemical baseline values of Potentially Toxic Elements (PTEs) in soils developed on natural or anthropogenically-affected areas are increasingly important parameters as they represent highly useful tools at local, regional, national and European level. While background values are mostly applied to uncontaminated areas to verify the presence of high to anomalous concentrations of PTEs due to geogenic sources, the geochemical baseline values are better representing those areas where the anthropogenic activities have altered more or less significantly the background values. This is the case of the Municipality of Abbadia San Salvatore, mostly lying above the volcanic products of Mt. Amiata (Tuscany, central Italy) and, subordinately, above sedimentary formations, where >100 years of exploitation of cinnabar (HgS) and production of liquid mercury have affected the environmental matrices, including the pedosphere compartment. Consequently, 51 top-soils and 51 sub-soils, developed on the volcanic and sedimentary domains were collected and analyzed for PTEs (Hg, As, Sb, Cr, Co, Ni, Cu and V). A CoDA (Compositional Data Analysis) approach (robust Principal Component Analysis) combined with geochemical mapping allowed to account for the compositional nature of geochemical data and define the spatial distribution of the investigated chalcophile and siderophile elements. The results obtained by CoDa are useful to understand data variability and relative variations of the elements compared to the barycenter of the total composition. Independently by the substratum (volcanic or sedimentary), the concentrations of Hg, As, Sb, Cr, Co, Ni, Cu and V were highly variable and up to 258, 276, 110, 261, 32, 132, 80 and 173 mg/kg, respectively. Owing to the significant differences between the two geological domains, the two datasets were considered separately in order to define the geochemical baseline values of the analyzed PTEs computed using ProUCL 5.2.0 software. Accordingly, the geochemical baseline values of Hg, As, Cr, Co and V were much higher than the values imposed by the Italian Legislative Decree 152/06, suggesting the importance of exploring the geological features of the investigated territories and their anthropic history, particularly in Italy where poly-metallic sulfide ore deposits have been exploited in many regions since the Etruscan and Roman times. • The former Hg-mining area at Mt. Abstract: The Mt. Amiata volcano is the youngest and largest volcanic edifice in Tuscany (central-northern Italy) and is characterized by a geothermal field, exploited for the production of electrical energy. In the past Mt. Amiata was also known as a world-class Hg district whose mining activity was mainly distributed in the central-eastern part of this silicic volcanic complex, and particularly in the municipality of Abbadia San Salvatore. In the present work we report a geochemical survey on Hg0 measurements related to the former mercury mine facilities prior the reclamation project. The Hg0 measurements were carried out by car for long distance regional surveys, and on foot for local scale surveys by using two LUMEX (915+ and M) devices. This study presents the very first Hg0 data obtained with this analytical technique in the Mt. Amiata area. The facilities related to the mining areas and structures where cinnabar was converted to metallic Hg are characterized by high Hg values (>50,000ngm−3), although the urban center of Abbadia San Salvatore, few hundred meters away, does not appear to be receiving significant pollution from the calcine area and former industrial edifices, all the recorded values being below the values recommended by the issuing Tuscany Region authorities (300ngm−3) and in some cases approaching the Hg background levels (3–5ngm−3) for the Mt. Background: Hypotensive susceptibility in hypertensive patients could facilitate orthostatic hypotension, syncope and fall. The aim of this study was to identify incidence, clinical form, complications and risk factors for non-cardiac syncope in a cohort of hypertensive patients., Methods: This is an observational, case-controlled, retrospective study carried out on 168 patients, evaluated at the Hypertension Center of the University Hospital of Parma (Italy). Based on the presence of episodes of syncope during the six months prior to enrolment, we identified cases and controls and then we compared them to personal data, comorbidities, current drug regimens, presence of orthostatic hypotension, office and ambulatory blood pressure monitoring (ABPM) blood pressure (BP) values., Results: In patients with previous syncopal episodes (29.8% of total), we more frequently found female gender, comorbidities associated with autonomic dysfunction, diuretics and non-CV drugs potentially associated with hypotension in their current drug regimen, orthostatic hypotension and lower office and ABPM BP values., Conclusions: To identify hypertensive patients at higher risk for syncope and falls, physicians should focus on comorbidities and current drug regimens, systematically perform an active standing test to identify orthostatic hypotension, employ ABPM to compare BP values with the pre-established target and highlight systolic BP drops and abnormalities suggesting concomitant autonomic dysfunction. The modulation of antihypertensive therapy is an effective tool to counteract the risk of non-cardiac syncope, with possible trauma or other negative influences.

Background: Dyspnea is one of the most frequent causes of admission in Internal Medicine wards, leading to a sizeable utilization of medical resources., Study Design and Methods: The role of bedside lung ultrasound (LUS) was evaluated in 130 consecutive patients (age: 81±9years), in whom blindly collected LUS results were compared with data obtained by clinical examination, medical history, blood analysis, and chest X-ray. Dyspnea etiology was classified as "cardiac" (n=80), "respiratory" (n=36) or "mixed" (n=14), according to the discharge diagnosis (congestive heart failure either alone [n=80] or associated with pneumonia [n=14], pneumonia [n=24], and obstructive disventilatory syndrome [n=12]). An 8-window LUS protocol was applied to evaluate B-line distribution, "interstitial syndrome" pattern, pleural effusion and images of static or dynamic air bronchogram/focal parenchymal consolidation., Results: The presence of a generalized "interstitial syndrome" at the initial LUS evaluation allowed to discriminate "cardiac" from "pulmonary" Dyspnea with high sensitivity (93.75%; confidence intervals: 86.01%-97.94%) and specificity (86.11%; 70.50%-95.33%). Positive and negative predictive values were 93.76% (86.03%-97.94%) and 86.09% (70.47%-95.32%), respectively. Moreover, LUS diagnostic accuracy for the diagnosis of pneumonia was not inferior to that of chest X-ray., Conclusions: Bedside LUS evaluation contributes with high sensitivity and specificity to the differential diagnosis of Dyspnea. This holds true not only in the emergency setting, but also in the sub-acute Internal Medicine arena. Background: Complete surgical resection of gastric cancer is potentially curative, but long-term survival is poor., Methods: Patients with histologically proven adenocarcinoma of the stomach of stages IB, II, IIIA and B, or IV (T4N2M0) and treated with potentially curative surgery were randomly assigned to follow-up alone or to intravenous treatment with four cycles (repeated every 21 days) of PELF (cisplatin [40 mg/m(2), on days 1 and 5], epirubicin [30 mg/m(2), days 1 and 5], L-leucovorin [100 mg/m(2), days 1-4], and 5-fluorouracil [300 mg/m(2), days 1-4] in a hospital setting. Frequencies and severity of adverse events were determined. Overall survival (OS) and disease-free survival (DFS) were compared between the treatment arms using Kaplan-Meier analysis and a Cox proportional hazards regression model. All statistical tests were two-sided., Results: From January 1995 through September 2000, 258 patients were randomly assigned to chemotherapy (n = 130) or surgery alone (n = 128). Patient characteristics were well balanced between the two arms. Among those who received chemotherapy, grade 3 or 4 toxic effects including vomiting, mucositis, and diarrhea were experienced by 21.1%, 8.4%, and 11.8% of patients, respectively. Leucopenia, anemia, and thrombocytopenia of grade 3 or 4 were experienced by 20.3%, 3.3%, and 4.2% of patients, respectively. After a median follow-up of 72.8 months, 128 patients (49.6%) experienced recurrence and 139 (53.9%) deaths were observed, one toxicity-related. Relative to treatment with surgery alone, adjuvant chemotherapy did not increase disease-free survival (hazard ratio [HR] of recurrence = 0.92; 95% confidence interval [CI] = 0.66 to 1.27) or overall survival (HR of death = 0.90; 95% CI = 0.64 to 1.26)., Conclusions: Our results failed to provide proof of an effect of adjuvant chemotherapy with PELF on overall survival or disease-free survival. The estimated effect of chemotherapy (10% reduction in the hazard of death or relapse) is modest and consistent with the results of meta-analyses of adjuvant chemotherapy without platinum agents.

Aims: Doppler tissue echocardiography (DTE) was applied to extract the myocardial wall velocities along different planes and evaluate the left ventricular function in essential hypertension., Methods and Results: Fifty-four hypertensives (HT) were compared to a control group of 31 normotensive (NT) subjects. The short-axis shortening and lengthening was assessed through the parasternal projections, sampling from interventricular septum and posterior wall. Through the apical projections the mitral annulus excursion was observed at four sites (anterior, posteroseptal, lateral, inferior walls) to assess the longitudinal dynamic of the heart. In each myocardial segment, peak velocity and time-velocity integral for systolic (S) and diastolic waves (E and A) were measured and their means for the long- and short-axis directions were calculated. Significant changes in hypertensives involved mainly the longitudinal motion. In diastole, the E-wave relaxation velocity was significantly decreased and the late A-wave velocity was unchanged. The E/A velocity ratio was significantly reduced. Relaxation velocity was negatively correlated to age, left ventricular mass and diastolic blood pressure. In systole, the peak S-wave shortening velocity was reduced and no association with age, left ventricular mass and blood pressure could be demonstrated. The range of segmental data produced by DTE proved useful to manufacture sensitive indices for recognition of hypertensive damage. Single DTE variables also proved slightly more sensitive than those extracted from the mitral flow pattern for the discrimination of HT patients., Conclusion: The presence of impaired relaxation was confirmed by DTE in a large portion of patients with hypertension and left ventricular hypertrophy. A peculiar systolic disturbance is evidenced by this technique. DTE-derived information can be used to detect early and quantify target-organ damage and its progression or regression during antihypertensive treatment.
