Background & Objective: The her2 amplification plays an important role in breast cancer management. Therefore, there is a need for using supplementary molecular methods in IHC equivocal cases. Present study has been conducted to determine the effects of clinicopathological variables on her2 gene amplification by chromogenic in situ hybridization (CISH) in IHC Her2 (2+) breast cancer individuals. Methods: A cross-sectional study was conducted in Zaferanyeh Laboratory collaborated with Shahid Beheshti University of Medical Sciences (Tehran-Iran; 2015-2018). All pathological data related invasive breast cancer patients with equivocal IHC results were included. CISH method was performed as a supplementary technique. The associations between histopathologic variables, status of Ki-67 index, progesterone and estrogen receptors (PR & ER) with her2 amplification by CISH were investigated and analyzed. The level of significance was considered as P-value < 0.05. Results: Totally, 239 patients with mean age of 53.2 years were studied. CISH identified her2 gene amplification in 51 subjects (21.3%). The type of tumor (invasive ductal carcinoma), the tumor grade, and the value of Ki-67 index were directly correlated with her2 amplification. Significant negative associations were also observed between CISH results and ER and PR expression. Conclusion: As her2 gene amplification was identified in 21.3% of invasive breast cancer patients with equivocal IHC results, it is supposed that applying CISH method may consider as a potentially valuable supplementary method. Results have also shown that higher grades of tumor, invasive ductal carcinoma, absences of hormone receptors and high Ki-67 index significantly correlated with the her2 amplification. Background: There are conflicting reports regarding the association between coronary artery disease (CAD) and mild cognitive impairment (MCI). Volumetric Magnetic resonance imaging (MRI) investigations have been considered as an objective biomarker for MCI. In this study, we determined the relationship between the regional brain volumes and the extent of CAD in MCI patients and cognitively normal controls. Materials and Methods: In a case-control study a subset of MCI patients (n = 20) and cognitively normal controls (n = 20), aged 66.4 ± 4.6 and 65.3 ± 3.9 respectively, from subjects who were recently admitted to cardiac catheterization facilities in two general hospitals were selected. All subjects underwent a clinical interview, biochemical measures, neuropsychological testing and Neuropsychiatry Unit COGnitive assessment tool. Video records of coronary angiography were scored with the Gensini method. For volumetric evaluation of regions of interest, brain MRI scans was processed using the FreeSurfer software package the relationship between the regional brain volumes and the extent of CAD in MCI patients and cognitively normal controls were compared. Results: We have found that, there were significant differences between the two groups in volumes of left fusiform (P = 0.039), left pars triangularis (P = 0.003) and left superior temporal gyrus (P = 0.009), after controlling for intracranial volumes. Higher Gensini scores were associated with reduced volumes of total cortical volume (P = 0.047, R = −0.4), left precuneus (P = 0.022, R = −0.5), right inferior parietal lobule (P = 0.011, R = −0.5) and left supra marginal gyrus (P = 0.035, R = −0.04) in MCI. Conclusion: In MCI, a greater degree of coronary stenosis correlates with greater loss of gray matter in specific brain regions relevant to cognitive function. Background: The emergence of non-tuberculous mycobacteria as clinically relevant pathogens has necessitated us for the study of these organisms in the context of their environment. Differentiation of Mycobacterium tuberculosis complex and non-tuberculous mycobacteria is important especially when we have a positive smear slide test result. Aim: In this study, we planned to survey the prevalence of tuberculosis and non-tuberculous mycobacteria among archival acid-fast bacilli positive smear slides. Materials and Methods: A number of 200 acidfast bacilli positive smear slides were collected from different parts of Sistan and Baluchestan Province, the biggest province of Iran with the highest incidence of tuberculosis. The presence of mycobacterial IS6110 was evaluated in slides' scraped material by TaqMan real-time polymerase chain reaction assay. Results: The real-time polymerase chain reaction tests of archival acid-fast bacilli positive smear slides showed that 171 slides from 200 examined slides had M. tuberculosis DNA and in the remaining 29 examined slides, M. tuberculosis DNA was not found. Conclusion: Our findings showed that there was no M. tuberculosis DNA in 14.5% of archival AFB positive smear slides, and this finding necessitates us to reviewing our diagnostic and anti- tuberculous protocols. OBJECTIVE: Paraoxonase (PON) is associated with high-density lipoprotein and protects serum lipid from oxidation. The aim of this study was to determine serum PON, arylesterase (ARE) activities, and total antioxidant capacity (TAC) in metabolic syndrome (MES). METHODS: This case–control study was performed on 106 patients with MES and 231 healthy subjects. Serum PON and ARE activities were determined spectrophotometrically. TAC was determined using ferric reducing ability of plasma assay. RESULTS: The results showed that serum PON activity was significantly lower in patients with MES (69.62±59.86 IU/l) than healthy subjects (91.64±77.45 IU/l) (P<0.05). The serum ARE activity in MES and normal subjects were 45.23±23.24 and 65.69±31.10 kU/l respectively. The ARE activity was significantly lower in patients with MES than normal subjects (P<0.0001). No significant differences were observed between MES and normal subjects regarding TAC. CONCLUSION: The lower PON and ARE activities in MES may be considered an independent risk factor for cardiovascular disease, which remains to be cleared. [ABSTRACT FROM AUTHOR]

Aims: Cervical cancer (CC) is a common malignancy in women, predominantly caused by human papillomavirus. The most subtypes are adenocarcinomas (AC) and squamous cell carcinomas (SCC), which show various features and treatment responses. Programmed death-ligand 1 (PD-L1) and programmed cell death protein 1 (PD-1) as Immune checkpoint molecules, play a role in immune evasion. We investigated PD-L1 expression in AC and SCC of the cervix and explored its link to clinical characteristics., Methods and Results: The present cross-sectional research was done between 2016 and 2022 on samples in Shahid Beheshti University of Medical Sciences-affiliated hospitals in Iran. Histological tissue samples of CCs (16 AC and 48 SCC) were assessed, and clinical information was obtained by reviewing their medical documents. PD-L1 expression was evaluated by immunohistochemistry and we used the combined positive score. SCC cases showed a higher (not significant) PD-L1 expression. The PD-L1 expression and clinical characteristics were not significantly correlated in both subgroups., Conclusion: Although SCC cases exhibited higher PD-L1 expression, this difference was non-significant. Background: Although vaccination coverage against a disease is not exactly the same as community immunity against that disease, it is undoubtedly directly related to it and provides an estimate of the coherence and efficacy of community health infrastructure., Aims: To evaluate the vaccination coverage of children throughout the Islamic Republic of Iran in 2019., Methods: This was a cross-sectional study. A probability proportional to size cluster sampling method was used and the vaccination data of 8682 children aged 24-35 months were collected in the form of 1447 clusters, each comprising six participants. Only valid data, including vaccination card or electronic health files, were used. The results were reported in the form of descriptive tables., Results: Overall, 97.82% of Iranian participants (8068 of 8248) and 90.32% of non-Iranian participants (392 of 434) had received all essential childhood vaccination by the time of interview. In total, 93.02% of all participants had presentable vaccination cards, and the immunization history of 535 (6.16%) children was retrieved using their electronic health files. The dropout rate between receiving pentavalent vaccine 1 and pentavalent vaccine 3 was 0.01%. In 29 provinces, vaccination coverage was ≥ 95%. In the other two provinces, the figure was 93.30%., Conclusion: Immunization coverage of children aged 24-35 months fully complied with eradication/elimination goals of vaccine-preventable diseases. In 2019, measles and rubella elimination was certified in the Islamic Republic of Iran. Crimean-Congo hemorrhagic fever (CCHF) has repeatedly caused nosocomial outbreaks among hospital staff. In the summer of 2003, we studied the seroprevalence of anti-CCHF IgG among health care workers who had come in contact with Crimean-Congo hemorrhagic fever patients from three referral hospitals in endemic regions of Iran. A total of 223 eligible staff were examined. Whereas 5 of 129 (3.87%) exposed health care workers tested positive, none of the 94 in the unexposed group did (P=0.075). Seropositivity was more frequent among those whose intact skin had come in contact with nonsanguineous body fluids (9.52%) and those who had had percutaneous contacts (7.14%). Health care workers exposed to Crimean-Congo hemorrhagic fever patients, those who live in Systan-Baluchestan province, and older health care workers were more prone to seropositivity. Where introduction of high-risk modes of contact cannot be confined, we propose that health care workers take all the protective measures when handling Crimean-Congo hemorrhagic fever patients, particularly their blood and other body fluids.
