Introduction: In endodontic retreatment, gutta-percha removal must be done correctly in order to assure a successful outcome following failed procedures. The goal of this study was to compare the effectiveness of stainless-steel hand files with three different nickel-titanium rotary instrument systems for removing guttapercha using ultra-conservative access opening in curved canals. Aim: To compare the efficacy of different rotary instruments for removing obturating material from root canals with Ninja access in mandibular mesiobuccal moderately curved canals measured radiographically using Cone-beam Computed Tomography (CBCT). Materials and Methods: An in-vitro study was conducted in the Conservative and Endodontics department, Karnavati School of Dentistry, Karnavati University, Gandhinagar, Gujarat, India in the time period of June 2022 to July 2022. Forty extracted human mandibular molar teeth were accessed through Ultraconservative opening (Ninja Access). After preparing the root canals, they were sealed using gutta-percha and AH Plus sealer through lateral compaction. The teeth were then stored for one week. The samples were divided into four groups according to the rotary file used: Group I- Hedstrom Files; Group II- Pro taper Universal Retreatment (PTUR) Files; Group III- Neoendo Retreatment Files; Group IV- R-Endo Retreatment. The amount of remaining filling material after the retreatment procedure was checked with CBCT. The statistical analysis was performed using R statistical analysis software version 4.1.0 for windows. Results: In sagittal section, there was significant difference of residual filling material between the four file systems in middle third only (p=0.048) while significant difference was seen only in apical third in the coronal section (p=0.011). However, the three rotary retreatment files left significantly less remnants than Hedström files during removal of the gutta-percha. The time taken for retreatment was significantly higher for Hedström files (398.90±20.717) sec followed by R-endo retreatment files (274.30±14.407). Conclusion: H file was the least effective in removing guttapercha from the canals when compared to the other three file systems. The Neo-Endo rotary retreatment system was faster. Introduction: Root Canal Treatment (RCT) is the most common and effective treatment for saving natural tooth. Despite of modern technology and progress in the materials, failures in endodontic treatment still remains. Microbial infection majorly bacteria in the root canal is the prime cause of reinfection in root canal treatment. Endodontic instruments, solvents, lasers are the different techniques that can be used for removing gutta percha from the root canal. Aim: To compare the efficacy of orange oil, lemon oil, and xylene as solvents in removing gutta percha from curved root canals. Materials and Methods: This in-vitro study conducted in Sharad Pawar Dental College and Hospital, Sawangi Meghe, Wardha, Maharashtra, India, for three months and 24 freshly extracted teeth including incisors, premolars and molars having curved root will be taken. Then, RCT would be performed in all the extracted teeth by using cold lateral compression obturation method. Then all teeth would be divided in three groups (n=8). After one month, the Gutta Percha (GP) would removed by ProTaper Universal system. First group will use orange oil as solvent, second group will use lemon oil as solvent and the third group will use xylene as a solvent. The micro-Computed Tomography (CT) images prior to the application of solvent and after application of solvent will be taken and would be evaluated to check the percentage of filling material remaining. Results: The expected outcome is orange oil and lemon oil to act as a better root canal solvent, as compared to xylene. conclusion: The determination and comparison of the efficacy of orange oil, lemon oil and xylene as GP solvents from the present planned study will help the clinicians during re-RCT treatment. Introduction: During a pulpectomy, the infected or inflamed pulp tissues are removed, and the root canal is thoroughly cleaned with mechanical instrumentation and copious irrigation. Various endodontic irrigants are available, such as Sodium Hypochlorite (NaOCl), Chlorhexidine (CHX) gluconate, Ethylene Diamine Tetra Acetic Acid (EDTA), Mixture of doxycycline, citric acid, and a detergent (MTAD), etc. Among these, CHX gluconate is widely used as an endodontic irrigant and medicament due to its antibacterial effect and substantivity. Enterococcus faecalis is the most common bacterial species found in necrotic teeth, with high viability and antibiotic resistance. A 3.8% Silver Diamine Fluoride (SDF) has antimicrobial properties and has been shown to eliminate E. faecalis from the root canals of permanent teeth. However, there are no studies in the literature that have evaluated its efficacy as a root canal irrigant in primary teeth. Aim: To evaluate the antimicrobial efficacy of 3.8% SDF against E. faecalis in primary teeth. Materials and Methods: This was a in-vivo double-blinded randomised clinical trial conducted at Department of Paediatric and Preventive Dentistry, GSL Dental College and Hospital, Rajahmundry, Andhra Pradesh, India in children aged 3-8 years old. The study included 60 primary teeth that required pulpectomy. The teeth were divided into two groups and irrigated: 30 teeth with 3.8% SDF (Group I) and 30 teeth with 2% CHX (Group II). In all cases, two microbiological samples were taken using sterile absorbent paper points: the first after access opening and the second after the final irrigation. All samples were assessed using the agar plate method. The results were analysed statistically using a Student's paired t-test. Results: After analysing the pre- and postirrigation samples, there was a statistically significant reduction in Colony Forming Units (CFU)mL (p<0.05) in both groups. When comparing the two groups, no statistical difference was observed in the percentage reduction of bacterial colonies (p>0.05). Conclusion: The reduction in CFU/mL of E. faecalis was comparable in both groups. Thus, 3.8% SDF can be used as an alternative root canal irrigant to 2% CHX. [ABSTRACT FROM AUTHOR]

Introduction: Extrusion of periapical debris is one of the common problem encountered during root canal treatment. As the endodontic instruments differ in terms of design and use, apical extrusion of debris may vary. Aim: To determine the quantity of debris extruded apically during various hand and rotary assisted instrumentation in extracted primary single rooted teeth. Materials And Methods: This in-vitro study was conducted in Department of Paediatric and Preventive Dentistry at Kannur Dental College, Anjarakandy, Kannur, Kerala, India, from December 2020 to May 2021. Four file systems were compared i.e, two hand files (Kedo SH and Protaper hand) and two rotary files (Kedo SG blue and Pro AF baby gold) among 60 therapeutically extracted single rooted primary teeth. The tooth was inserted into Eppendorf tube till cementoenamel junction, access opening was done, pulp extirpated, working length determined with 10 size k-file. After instrumentation, the tooth was washed with 10 mL of distilled water for debris collection. Tube was incubated at 70°c for drying and weighed to measure the collected debris. Data collected was analysed statistically using Independent t-test, One-way Analysis of Variance (ANOVA) and Least Significant Difference (LSD) Posthoc test for multiple comparison to compare between the groups were done. Level of statistical significance was set at p-value<0.05. Results: There was significant difference in the amount of debris collected between the hand file and rotary files (p-value<0.001), between Kedo SG blue and Pro AF baby gold (p-value<0.001), and Kedo SH hand file and Protaper hand file (p-value<0.05). Conclusion: Rotary files shows lesser amount of debris extrusion than the hand files. Comparing the four file system Kedo SG Blue file shows least periapical debris extrusion, the second least is Pro AF Baby Gold (rotary), then Kedo SH hand file and last Protaper hand file. Introduction: When endodontic treatment has failed, non surgical retreatment is often the first choice. This procedure includes removal of previous obturating material followed by chemomechanical preparation, disinfection and refilling of the root canals. However, residual sealer during retreatment remains a concern when such bioceramic sealers are utilised for obturation and this can affect the prognosis. Aim: To evaluate and compare the amount of residual filling material after retreatment in straight and oval root canals filled with iRoot SP and AH Plus sealer using V-Blue file system using Micro-Computed Tomography (micro-CT). Materials and Methods: This in-vitro study was conducted at Bharati Vidyapeeth (deemed to be) Dental College and Hospital, Navi Mumbai, Maharashtra, India, between November 2020 to December 2021. Sixty freshly extracted human mandibular premolar with straight and oval canals standardised with Cone Beam Computed Tomography (CBCT) were prepared with ProTaper next files and were assigned to two groups for obturation with gutta-percha using AH Plus and iRoot SP sealer respectively. Quality of obturation was confirmed with CBCT. After one month, retreatment was carried with V-Blue files and percentage of remaining obturating material was were evaluated before and after retreatment through micro-CT imaging. Two sample means and Kolmogorov Smirnov test were used to analyse data. Results: Data of all the samples from the micro-CT evaluation indicated that the mean volume of the initial filling material did not differ significantly among AH Plus sealer and iRoot SP sealer group (p>0.05). Percentage of residual volume of AH Plus sealer (0.02%) was lower than that of iRoot SP sealer group (0.06%) and this difference was statistically significant (p<0.05). Background: The comprehensive healthcare approach including prophylactic guidance and motivation by the primary healthcare professionals towards oral and maxillofacial diseases such as post-treatment endodontic disease (PTED) plays a significant role in diagnosing and managing the condition. Especially in the developing countries like India where the hygiene practices are severely compromised, the primary healthcare professional plays an upfront role. Objectives: The present study was conducted to assess the clinical and radiographic characteristics of PTED by primary healthcare professional. Materials and Methods: The cross-sectional study was conducted in a dental hospital in Kutch, Gujarat, India. In the present study, out of a total of 755, 96 patients were diagnosed with PTED, met the inclusion criteria, and were enrolled for the study. After performing intraoral and extraoral examination, intraoral periapical radiographs were taken of the concerned teeth. Under dark room conditions, radiographs were examined using dentsply light box and magnifying glass by healthcare professionals. Results: Out of 755 patients, 96 (12.71%) patients were enrolled in the study with 98 concerned teeth. The most common teeth diagnosed with PTED were maxillary molars with 25.51% (21) individuals. Well-defined radiolucent lesions were seen in 62.24% (61) individuals. Voids in both coronal and apical region were seen in majority (38.77%) of patients. The length of root-end fillings with respect to the radiographic apex was satisfactory in 44.89% (44) individuals. The present study showed strong correlation between sinus formation and presence of periapical lesion with P value of 0.0219*. Conclusion: The proper guidance and preventive care by primary healthcare professionals leads to the relatively less prevalence of post-treatment endodontic disease in Indian population. Elimination of bacteria from infected root canal systems is a challenging task. Various techniques have been described to reduce the number of bacteria within the root canal system, which include chemomechanical instrumentation, use of various irrigants to remove or dissolve organic and inorganic debris, and to destroy bacteria. The intracanal medicament plays a key role in the success of root canal treatment. With the rise in bacterial resistance to antibiotics, there is considerable interest in the development of other classes of antimicrobials for the control of infection. Natural products are known to play an important role in human life. The use of herbal products as mouthwash has been tried and tested in the literature. However, the use of herbal intracanal medicament has been shown promising results when used under in vitro conditions, but in vivo studies are very scarce. This may be due to the limited supporting literature available to use it as intracanal medicament in patients due to the ethical concern. Hence, the purpose of this review is to highlight the current guidelines (laid by the drugs and cosmetics act as per the Gazette of India) regarding the use of herbal medicaments for the clinical trials in endodontics. The present study was conducted with the aim to gather information on routine endodontic treatment performed by dental practitioners in Himachal Pradesh. The questionnaire was prepared in the form of multiple choice questions covering all the aspects ranging from the personal details like gender, nature of professional activity to details of use of instruments, methods of isolation, techniques of root canal preparation, root canal irrigants, intracanal medicaments, and methods of root canal obturation. Out of 8550 questionnaires mailed, 2066 replies were received and finally 2000 were evaluated as useable as ones thus giving a response rate of around 24% for the study. Amajority of respondents never used rubber dam (68.5%) with only 182 dentists reportedly using rubber dam always for endodontic treatment. 76% of dentists were found using Nickel Titanium Instruments. 1290 respondents aimed at achieving a working length between 0 and 1mm short of the radiographic apex. About 58% instrumented the canal With a Step Back technique, generally using a combination of different instruments (K File +Hedstrom file). 93% dentists were found using Zinc oxide eugenol as sealer with very less of them opting other ones like metapex or AH plus. Similarly Cavit was the temporary filling material of choice for majority of practitioners. The first-choice root-canal irrigant was Saline and approximately 45% used formocresol as intracanal medication. The most popular obturation technique was lateral condensation (57.5%). It was concluded that most of the dental practitioners in the state were not following the quality guidelines for endodontic treatment and so the professional bodies in endodontics should conduct training programs, seminars and workshops aimed at updating the knowledge and skills of the practicing dentists and develop methods to check the same periodically. Root canal system is complex to understand because of its unpredictable nature. It differs for different teeth and for the same teeth in different individuals. Successful endodontic therapy thus depends on the clinician's ability to anticipate and look for these variations. A mandibular first molar with six root canals represents a rare anatomical variant, particularly when four canals are found in distal root. This case report discusses successful nonsurgical endodontic management of two-root mandibular first molar with four distal canals and two mesial canals reported for the first time in Indian population. Information on the 24th Federation of Operative Dentistry of India (FODI) and the 17th Indian Endodontic Society (IES) National Conference held on November 13-15, 2009 at the Sree Gokulam Convention Center in Cochin, Kerala, India is presented. The event highlighted three pre-conference workshops on topics like microscopes in dentistry, posterior composites and rotary endodontics. It also discussed lectures from six international and national guest speakers.
