Introduction: Benign positional paroxysmal vertigo (BPPV) stands as the commonest cause for vertigo. It accounts for 20% of all cases of vertigo, even with its high prevalence rate it often goes underdiagnosed and undertreated. Development of the consensus document by the Bárány society's International Classification of Vestibular Disorders (ICVD)significantly facilitates the diagnosis of BPPV and its variants. This study assesses the utilisation of ICVD criteria for managing BPPV., Methodology: This is a cross-sectional descriptive study conducted at a tertiary care hospital in Northern India spanning from November 1, 2022, to November 30, 2023. A total of 110 participants diagnosed with BPPV were enrolled consecutively. All participants underwent Dix-Hallpike and supine log roll positional maneuvers. Diagnosis was made based on the history and type of nystagmus seen, and classified as per the ICVD criteria., Results: Posterior semicircular canalolithiasis (pc-BPPV) accounted for 25.45% of cases and horizontal canal canalolithiasis (hc-BPPV) accounted for 20.91% of cases. Probable BPPV, spontaneously resolved (pBPPVsr) was diagnosed in 16.36% of participants and possible BPPV(pBPPV) was diagnosed in 18.18% of participants. Multiple canal BPPV (mc-BPPV) accounted for 17.27% of cases. One participant was diagnosed with horizontal canal cupulolithiasis and anterior canal canalolithiasis respectively. No participant was diagnosed with posterior canal cupulolithiasis., Conclusion: The most common type of BPPV was pc-BPPV followed by hc-BPPV. The affected canal in possible BPPV, can be identified, and appropriate repositioning maneuvers are effective in treating them as well as aids in confirming the diagnosis. The diagnostic clarity provided by ICVD, aids in effective management of BPPV. More studies with larger sample size are required to further validate its clinical utility., (© 2024. Inheritance of polymorphisms in the interleukin (IL)-10 promoter and IL-12B genes, which influence cytokine production and activities, may define the balance in T helper response in infection and autoimmune diseases. In the present study, we investigated the distribution of the IL-10 promoter and IL-12B gene polymorphisms in a multiethnic Malaysian population. Overall, our findings suggest that the IL-12B and IL-10 -592 genotypes were distributed homogenously across all major ethnic groups, including Malays, Chinese, and Indians, except for polymorphisms at IL-10 -1082. At this gene locus, the ethnic Chinese showed a significantly lower allele frequency of -1082G (2.1%) compared to the Malay (12.2%) and Indian (15.3%) populations. Results for the IL-12B and IL-10 gene polymorphisms were consistent with those reported for the Asian population, but markedly different from those of the African and Caucasian populations. Our findings suggest that there are specific genetic variations between different ethnic groups, which should be examined in all gene population-based association studies.

Purpose: Genetic risk to tobacco related cancers are associated with polymorphisms in CYP1A1 and GST, which are involved in the metabolic activation and detoxification of carcinogens. The genetic variations in these drug-metabolizing enzymes may alter the susceptibility to UADT cancers triggered by environmental exposures. The hospital-based case-control study evaluated the impact of combined CYP1A1 MspI and GST (M1 & T1) polymorphisms among the individuals exposed to environmental risk factors as modulators in the risk of UADT cancers in Tamilians, a population of south India., Methods: The unrelated histopathologically confirmed 408 cases and 220 population-based controls matched by age and gender were genotyped for CYP1A1 MspI, GSTM1 and GSTT1 polymorphisms using PCR based methods. To investigate the potential gene-environment interactions, analyses were carried out stratifying by smoking and tobacco chewing status using SPSS software., Results: The combination of genes and environment interactions by stratified analyses revealed significant interactions among the habitual tobacco smokers (CYP1A1 MspI & GSTM1 null: OR 14.06; 95% CI 3.90-50.68, CYP1A1 MspI & GSTT1 null: OR 33.28; 95% CI 4.24-261.19) and tobacco chewers (CYP1A1 MspI & GSTM1 null: OR 20.51; 95% CI 6.77-62.13, CYP1A1 MspI & GSTT1 null: OR 79.35; 95% CI 10.40-605.55) on the multiplicative scale., Conclusion: Our findings have indicated that the individuals polymorphic for CYP1A1 MspI either with GSTM1 null or with GSTT1 null genotypes revealed an increased risk for UADT cancers than that ascribed to a single susceptible gene among the tobacco users in the population [single gene risk among smokers and chewers, respectively, for CYP1A1 MspI (OR 6.43; 95% CI 3.69-11.21); (OR 10.24; 95% CI 5.95-17.60), GSTM1*0 (OR 3.77; 95% CI 1.94-7.37); (OR 7.97 95% CI 4.10-15.76) and GSTT1*0 (OR 6.95 95% CI 2.88-16.77); (OR 25.83 95% CI 7.78-85.76).

Background: The inter-individual differences in upper aerodigestive tract (UADT) cancer risk may be partly attributed to the polymorphic variability in the CYP1A1 gene that is involved in the metabolic activation of xenobiotics to carcinogenic reactive metabolites., Methods: The hospital-based case-control study evaluated CYP1A1*2A and CYP1A1*2C polymorphisms in 408 histopathologically confirmed cases and 220 controls using polymerase chain reaction-restriction fragment length polymorphism methods., Results: The multivariate logistic regression analyses demonstrated that CYP1A1 *1A/*2A (odds ratio [OR] 1.76; 95% Confidence interval [CI] 1.19-2.60) and *2A/*2A (OR 2.83; 95% CI 1.43-5.61) genotypes were significantly associated with increased risk for UADT cancers. The gene-environment interaction analyses revealed a significant interaction among tobacco smokers and chewers carrying CYP1A1*2A mutant genotypes on the multiplicative scale., Conclusion: CYP1A1*2A polymorphic genotypes are associated with an enhanced risk to UADT cancers, in particular, among the habitual tobacco smokers and chewers carrying mutant genotypes in the Tamilians of the Indian population., ((c) 2008 Wiley Periodicals, Inc.)

This study aimed to determine the prevalence of genetic polymorphism in the CYP2D6 gene, which codes for the polymorphically expressed CYP2D6 drug-metabolizing enzyme. The common variants CYP2D6 *2, *3, *4, *5, *10, *14, and *17 were studied in the populations (n=447) of the four South Indian states namely Tamilnadu (TN), Kerala (Ker), Karnataka (Kar) and Andhra Pradesh (AP). Genetic polymorphisms were identified using polymerase chain reaction (PCR) and PCR-Restriction Fragment Length Polymorphism (PCR-RFLP) based methods. Differences in frequencies of CYP2D6 polymorphism between each South Indian state were statistically compared, and also the frequency of South Indian population as a whole in relation to other major populations. The CYP2D6*2 allele was the most frequent variant (34.8%), followed by the *10 allele (10.2%). The *4 and *5 alleles occurred at 7.3% and 1.9% respectively. The *3, *14 and *17 alleles were not detected in the study. The *1/*2, *1/*1 and *2/*2 genotypes were the most common CYP2D6 genotypes, representing 32.7%, 19.4% and 11.8% of the South Indian population. Genotypes that predict poor metabolizer phenotype i.e. *4/*4 and *4/*5 were found at 0.6% in South Indian population. The genetic composition at the CYP2D6 locus in South Indians is distinct from Caucasian, African and even other Asian (Chinese and Japanese) populations.
