School-based mental health support services allow children and adolescents easy access to services without requirement of traveling to clinics and hospitals. We describe a School Mental Health Support Scheme (SMHSS) piloted in Hong Kong and discuss the challenges and learnings from the experience. This conceptual paper argues that accessibility is not the only advantage of such services. Teachers are significant others in child development, alongside with families. They play a central role in impacting the children's/adolescents' needs for competence and adult attachment, while schools provide an expanded social network of peers for one's social relationship. The fulfillment of these needs has powerful implications in themental health of the children/adolescents. Teachers can help students to develop a sense of competence with self-worth and self-identity via providing guidance and feedback, whether they be on one's strengths or weaknesses, with acceptance, tolerance and unconditional positive regard. Particularly, the latter define a form of teacher-student relationship or adult attachment that offers the children/adolescents emotional security and nourishment, protecting them from failings and adversities. Teachers can also supervise and guide their students' social development with peers at schools. A recent meta-analysis has found preliminary evidence that those school-based mental health services integrated into the teachers' routine teaching activities aremore effective. Teachers, who are overworked and stressed by the schools' overemphasis on academics and grades, have yet to fully grasp their unique roles in supporting students with mental health needs. This paper ends by advocating a paradigm shift in which both the healthcare professionals and educators should forge a mutually beneficial collaboration in jointly enhancing the mental health of children/adolescents at schools. Objective: Medication combined with psychosocial intervention is the recommended treatment for ADHD. What is not clear is which of the parents participate in psychosocial interventions, and whether the impacts are different for fathers and mothers. This is important because of the different roles fathers and mothers have in shaping their child's development. This article examines the impact of multifamily therapy on a sample of fathers and mothers from Hong Kong whose children suffer from ADHD. Method: Fathers and mothers ratings on ADHD severity, parent-child relationship, parenthood stress, and parental self-esteem were compared between those who attended multifamily therapy and those who attended psychoeducation talks. Results: Fathers and mothers of the multifamily group, but not the psychoeducational group, reported reduction of child ADHD symptom severity. When the reduction was clinically significant, mothers reported an increased sense of competence while fathers reported improved father-child relationship. Conclusion: Fathers and mothers experienced different gains from attending MFT. Fathers' involvement in their child's treatment should be encouraged and facilitated. [ABSTRACT FROM AUTHOR]

Little information is available on the effect of parent-child relationships on self-perception among Chinese children with Attention Deficit Hyperactivity Disorder (ADHD) in Hong Kong. This study examined the impact of children’s age and symptom severity on the linkage of quality parent-child relationships to perceived competence/hope among children with ADHD. A clinical sample of 81 school-age children (ages 5–11, mean age = 8.38) who were diagnosed with ADHD were recruited in this study. Separate questionnaires were employed to collect data among children and their parents in the pre-treatment phase. Parent-child relationships were positively correlated with self-perception of children with ADHD, with physical competence to be father-child specific. Children’s age had moderated the correlation between mother-child relationship and children’s hope. Maternal role on children’s hope was significant for the mild symptom group; paternal role on children’s hope was significant for the severe symptom group. No moderation effect was found for symptom severity on the correlation between parent-child relationship and children’s self-perception. The study’s findings helped mental health professionals in a Chinese context to develop psychosocial interventions to foster quality parent-child relationships, which in turn will maintain children’s self-perception. This study assessed and compared oral health and oral-health behaviours among children with and without attention deficit hyperactivity disorder ( ADHD). The study included 31 children, 12-18 yr of age, with ADHD and 31 age- and gender-matched children without ADHD. Clinical data were recorded by a trained and calibrated examiner for caries, traumatic dental injuries, periodontal health, tooth wear, and salivary function. A questionnaire was also given to parents or caregivers about the oral health habits and behaviours of these children. Data were compared using Mann-Whitney U-tests and chi-square tests. No significant differences were found between children, with or without ADHD, in caries extent or prevalence, dental trauma prevalence, prevalence of periodontal disease or plaque, tooth wear, or unstimulated salivary flow. Children with ADHD had a significantly higher percentage of sites with gingival bleeding, as well as a higher frequency of parent-reported dislike of dentists, bruxism, history of assisted toothbrushing, and toothbrushing duration <1 min. They also had higher attendance at government dental clinics. The findings indicate that children with ADHD have poorer oral hygiene and more adverse oral-health attitudes and behaviours than do children without ADHD. [ABSTRACT FROM AUTHOR]

The subjective experiences of Chinese children in Hong Kong with attention-deficit/hyper-activity disorder (ADHD) are underexplored. This article reports the results of a qualitative study that aims to understand the subjective experiences of children with ADHD in Hong Kong, taken from among a clinical sample of Chinese families with children struggling with ADHD who have participated in cross-disciplinary research of multiple family groups (MFG). The participating children revealed the subjective experiences of their struggle with the disorder in response to their parents' concern at a "press conference," one of the MFG activities, which underscored the importance of developing a new social work model to meet the multiple psychosocial service needs of these families. This paper reports the results of an exploratory study that (i) identified parental stress and competence, parents' perception of their children's attention deficit hyperactivity disorder ( ADHD) symptoms, and the parent-child relationship in caring for children with ADHD; (ii) compared paternal and maternal experiences in these areas; and (iii) examined the effect of children's ADHD behavior on paternal and maternal experiences in Hong Kong Chinese parents. Seventy-two (59.5%) mothers and 49 (40.5%) fathers participated in the study, in which data were collected using a structured questionnaire. The results showed that: (i) mothers' level of parental stress was higher than fathers', but paternal and maternal competence in child-rearing did not significantly vary; (ii) mothers perceived the child's ADHD behavior more seriously than fathers; (iii) both mothers and fathers had positive perceptions of their parent-child relationship; and (iv) gender, employment, ADHD symptoms, and parental satisfaction explained the significant variance in parental stress but did not explain the significant variance in parental competence. Exploratory and qualitative in nature, this study aimed to identify the subjective experiences of Chinese parents with a child suspected of or diagnosed with attention deficit hyperactivity disorder (ADHD), in problem identification and management of the child, and to explore if the educational and mental health professionals have actively involved the parents in the decision-making process. A total of 24 parents/caregivers participated in this study. Data were collected through focus-group interviews and in-depth family interviews. Emerging from the data were seven major themes capturing the Chinese parents/caregivers’ experience, with three themes in problem identification and four in helping the child. Background: The strengths and difficulties questionnaire (SDQ) is now one of the most commonly used instruments for screening child psychiatric morbidities. Psychometric studies in the West affirm its reliability and validity, but similar studies are scarce among non-Western populations. This is an important gap because cultural differences can influence how children's behaviours are perceived and rated. This study explores the psychometric properties of the Chinese version of the SDQ among children in Hong Kong. Method: The SDQ was translated into Chinese. A community sample of 3,722 students between 6 and 12 years were recruited by stratified random sampling from across the whole of Hong Kong. Comparison group consisted of 494 consecutive children attending a general child psychiatric clinic. SDQ and basic socio-demographic data were collected from parents and teachers. Reliability was determined by internal consistency and test-retest stability. Validity was assessed by the questionnaire's ability to discriminate between community and clinic samples, and ROC curves. Cutoff scores and their sensitivity, specificity, positive predictive value and negative predictive value were calculated. Results: Our results confirm the questionnaire's reliability and validity. The total difficulties scale and hyperactivity subscale are potentially the most useful in discriminating between community and clinic children. The emotional subscale was relatively weaker, especially with respect to teachers' ratings. Of note also is that our normative scores are significantly higher than those reported in the West, highlighting once again the importance of examining a questionnaire's cultural applicability. Conclusion: Our data support the use of the Chinese version of the SDQ, especially the total difficulties scale, as a screening instrument for psychiatric morbidities among children in Hong Kong. Objective: There is no study on the quality of life (QOL) of parents of children with attention-deficit-hyperactivity disorder (ADHD), although QOL is increasingly gaining more attention and the interactions between children and their parents are bi-directional. The aim of the present study was therefore to survey the QOL of parents of children with ADHD in Hong Kong, and explore the relationships of sociodemographic and clinical variables with QOL. Method: Seventy-seven parents of children with ADHD were consecutively selected and the sociodemographic and clinical characteristics of the parents and their children were assessed. Results: Compared with the general population in Hong Kong, significantly lower scores in physical, psychological, social and environmental QOL domains were found in the parents of children with ADHD. On multivariate analysis, for the children with ADHD, the severity of emotional and hyperactivity/inattention symptoms, and having a comorbid pervasive developmental disorder were significantly correlated with one or more domains of QOL; while for the parents, educational level, household monthly income and having major medical conditions were significantly correlated with one or more domains of QOL. Objective: To understand the associations between adolescent family physical maltreatment and psychiatric morbidity or psychological problems.Methods: questionnaire survey on 489 secondary school students in Form 2 from 10 schools in Hong Kong. Questions regarding three forms of family physical maltreatment were measured: corporal punishment within the past 6 months, beaten without any reason within the past 6 months, or ever been beaten to injury.Main Result: Corporal punishment was not associated with the psychological variables after Bonferroni adjustment was made. Those who experienced the two other forms of physical maltreatment had significant and strong associations with positive Achenbach CBCL outcome (OR from 3.26 to 3.27), drug abuse problems (OR from 2.60 to 20.38), self-injurious behaviors (OR from 3.34 to 8.47) and poor perceived parental support. In addition, those who had ever been beaten to injury scored significantly lower in the “physical appearance” and “behavioral conduct” subscales of the Harter’s Self-Perception Profile for Adolescents.Conclusions: In Hong Kong, some forms of family physical maltreatment (beaten for no reason and beaten to injury) were associated with a number of psychiatric/psychological problems.
