Background: Although the necessity and benefits of having the open and public discussion about suicide have been recognized, youths' opinions regarding such discussion remain unknown., Aims: To explore youths' attitudes toward open suicide discussion, particularly concerning their preference of discussion contexts and impacts of Internet use., Method: This exploratory sequential mixed-methods study targeted Hong Kong youths aged 15 to 19. A total of six focus groups and 12 individual interviews were held ( N  = 40). Topics included adolescents' views of open suicide discussion, concerns, perceived benefits, and preferred contexts. The questionnaire survey ( N  = 1,676) was conducted subsequently to investigate the prevalence of youths' perspectives on public discussion of suicide and relationships with discussion contexts, use of social media platforms, and motivations of online expression., Results: Qualitative findings revealed three types of attitudes toward open suicide discussion: reluctance, support, and indifference. Major barriers included cultural norms, topic sensitivity, privacy concerns, contagion effect, fear of embarrassment, and unpleasant experiences in school programs. Results of quantitative analyses showed that reluctance was the dominant attitude among adolescents, and taboo was the top concern. Variations in youths' attitudes were related to gender, school academic banding, and suicide-related experiences. Notably, adolescents who had been exposed to suicide messages in contexts of peer networks and online platforms were more likely to endorse open suicide discussion. In addition, an increased likelihood of engaging in public suicide discussion was associated with the use of Instagram, WhatsApp, and Snapchat, and the motive of 'expressing emotions and opinions' online., Conclusions: Our results indicated a prevalent rejection among adolescents toward open suicide discussion, suggesting the issue of stigma, the need for tailored programs, the value of appropriate contexts, and the impact of Internet use. These findings may facilitate the development of school-based suicide prevention initiatives and the efficacy of online services for suicide-related communication.

Background: Social networking sites (SNSs) have gained popularity in recent years for help seeking and self-distress expression among adolescents. Although online suicidal expression is believed to have major benefits, various concerns have also been raised, particularly around privacy issues. Understanding youths' help-seeking behavior on SNSs is critical for effective suicide prevention; however, most research neglects the impacts of the private SNS context., Objective: This study aims to examine youths' private SNS use via the new Instagram feature, Close Friends, and its association with both online and offline help-seeking willingness as well as youths' suicidality., Methods: This study employed an exploratory sequential mixed methods approach with a combination of explorative qualitative interviews and a systematic quantitative survey, targeting youth aged 15-19 years in Hong Kong. The motivations for utilizing Close Friends and concerns regarding online expression were addressed in the focus group and individual interviews (n=40). A cross-sectional survey (n=1676) was conducted subsequently with eligible secondary school students to examine the prevalence of Close Friends usage, their online and offline help-seeking willingness, and suicide-related experiences., Results: A total of 3 primary motives for using Close Friends were identified during interviews, including (1) interaction and help seeking, (2) release of negative emotions, and (3) ventilation and self-expression. Most participants also highlighted the privacy concerns associated with public online communication and the importance of contacting close friends for emotional support. Survey results showed that use of Close Friends was quite prevalent among adolescents (1163/1646, 70.66%), with around 46% (754/1646, 45.81%) of respondents being frequent users. Differences by gender and school academic banding were also revealed. Regarding help-seeking intentions, youths were generally positive about seeking help from peers and friends offline (1010/1266, 79.78%) yet negative about seeking assistance from online friends or professionals with whom they had not yet developed a real-world connection (173/1266, 13.67%). Most notably, frequencies of Close Friends usage were differentially associated with online and offline help-seeking willingness and youths' suicidality. Compared with nonusers, those who had ever used the feature were more likely to seek offline support (adjusted odds ratios [AORs] 1.82-2.36), whereas heavy use of Close Friends was associated with increased odds of online help-seeking willingness (AOR 1.76, 95% CI 1.06-2.93) and a higher risk of suicidality (AOR 1.53, 95% CI 1.01-2.31)., Conclusions: The popularity of Close Friends reflects the increasing need for private online expression among youth. This study demonstrates the importance of Close Friends for self-expression and private conversation and inadequacy of peer support for suicidal adolescents. Further research is needed to identify the causal relationship between Close Friends usage and help-seeking willingness to guide the advancement of suicide prevention strategies. Despite widespread interest in social media use, little is known about the heterogeneity of underlying motivations for online engagement among at-risk adolescents. This study adopted a mixed-methods approach to evaluate the motives of online expression among Hong Kong youths aged 15-19, as well as their relationships to online help-seeking willingness and suicide risks. In total, 6 focus groups and 12 individual interviews were conducted ( N  = 40) to elicit information regarding online expression purposes and to construct questionnaire items. The cross-sectional survey ( N  = 1,676) was undertaken subsequently to determine the prevalence of motives, and their correlations with online help-seeking willingness and suicide risks. Qualitative interviews revealed two key themes: major motivations for online expression, and features of online expression and help-seeking among suicidal youths. Quantitative analyses demonstrated the prevalence of each online expression purpose, with most reporting on "emotional expression" and "life sharing and documentation," but variations identified by gender, school academic banding, and suicide-related experience. Notably, an ordinal logistic regression model revealed that motives of "emotional expression" and "image building" were associated with increased suicide risks, while the purpose of "life sharing" was associated with decreased suicide risks. Those who expressed online for "self-expression" and "friend making" showed a stronger willingness for online help-seeking. These findings provide a more holistic overview of the purposes of online expression, which suggested the likelihood of mental health issues and the need for support. Such insight may facilitate the development of more effective suicide prevention strategies and online intervention services.

Aims: There is increasing evidence showing the importance of long-acting injectable antipsychotics in the management of schizophrenia, especially in terms of improving patient medication compliance. A panel of experienced clinicians in Hong Kong mapped out a set of consensus statements with an aim to facilitate the understanding and use of long-acting injectable antipsychotics among local physicians., Methods: Eight discussion areas regarding long-acting injectable antipsychotics were selected by the chairman of the consensus group. A series of meetings were held for the panelists to discuss the published literature and their clinical experience, followed by the drafting of consensus statements. At the final meeting, each consensus statement was voted on anonymously by all members based on its practicability of recommendation in Hong Kong., Results: A total of 12 consensus statements on the rational use of long-acting injectable antipsychotics were established and accepted by the consensus group., Conclusion: The consensus statements aim to provide practical guidance for Hong Kong physicians on the use of long-acting injectable antipsychotics in schizophrenia patients. Aripiprazole, a dopamine partial agonist, is a second-generation anti-psychotic that is widely used for the treatment of schizophrenia and other psychotic disorders. A group of psychiatric experts in Hong Kong developed a set of consensus statements, aiming to facilitate the understanding of clinical properties and usages of aripiprazole among local physicians. Of note, because aripiprazole long-acting injectable has been available locally not long before the establishment of the consensus panel, which limited the discussion on its use in the local context, the consensus statements were focused primarily on oral aripiprazole. To draft the consensus statements, the panellists discussed the published evidence and their clinical experience regarding aripiprazole in a series of meetings based on several areas. At the final meeting, each drafted statement was voted on anonymously by all panellists based on its practicability of recommendation in Hong Kong. A set of consensus statements on the characteristics and clinical use of aripiprazole was established and accepted by the panel. These statements serve to provide a practical reference for physicians in Hong Kong, and possibly other parts of the Asia-Pacific region, on the use of aripiprazole in people with schizophrenia spectrum disorders and other psychotic problems., (© 2020 Royal Australasian College of Physicians.)

To facilitate the understanding of pregabalin and optimize its clinical usage in Hong Kong, an expert panel (11 psychiatrists, one family physician and one anesthesiologist) experienced in treating anxiety and somatic symptoms was invited to establish a set of consensus statements based on several discussion areas. A non-systematic literature search for relevant articles was conducted. The panelists addressed the discussion areas by sharing their clinical experience and available literature in a couple of meetings. At the last meeting, consensus statements derived from the proceedings were discussed and finalized. A total of 11 statements were ultimately accepted by panel voting based on their practicability of recommendation in Hong Kong. Objective: Our aim is to develop a local consensus to guide medical practitioners and psychiatrists on the use of milnacipran in different psychiatric conditions., Methods: By utilizing the modified Delphi technique, 12 statements were electronically voted on anonymously for their practicability of recommendation., Results: There was a very high degree of agreement among the consensus group on 10 finalized consensus statements, but 2 statements were voted down due to a poor degree of agreement., Conclusions: The present consensus statements were developed as general recommendations for medical practitioners and psychiatrists to be practically referred to in clinical settings., (Copyright © 2018 John Wiley & Sons, Ltd.)

Aim: In view of the clinical importance of the adherence issues in schizophrenia management, a consensus group of experienced local psychiatrists and nurse specialists gathered to outline a number of consensus statements for clinicians to consider enhancing adherence in their patients., Process: Prior to the consensus group meeting, three core members drafted eight statements on the issue of adherence in schizophrenia. Using a modified Delphi method, published literature and published guidelines regarding the management of schizophrenia were reviewed by the full panel during the group meeting. After discussion and reflection from each individual member of the consensus group, the eight statements were reworded and electronically voted on anonymously in two steps: acceptance on quality of evidence and practicability in implementation., Results: After modifications of the original statements, there was very high overall level of agreement and acceptance (reaching international standard) on all the five areas of adherence within the eight statements of the finalised statement., Conclusions: The present consensus statements are the first in Hong Kong to address systematically adherence issues in schizophrenia management. They include areas on adherence assessment and definition, treatment strategies in enhancing adherence, and treatment considerations at specific phases of schizophrenia. Background: In Hong Kong, neonatal bacillus Calmette-Guerin vaccination coverage has been around 99% since 1970. Children younger than 14 years of age appear to have a relatively low risk of tuberculosis (TB), but the risk of TB increases rapidly after 15 years of age to a secondary peak in young adulthood., Methods: We followed prospectively 19,383 students who were 6 to 10 years of age participating in the 1999/2000 bacillus Calmette-Guerin revaccination program by cross-matching with the territory-wide TB registry until December 31, 2010, using the identity card number as a unique identifier., Results: After 214,753 person-years of follow-up, 44 active TB cases (22 culture-confirmed) were detected for an overall incidence of 20.5/100,000 person-years. The incidence differed significantly by baseline tuberculin reaction sizes (13.0, 18.8, 22.5, 280.4 per 100,000 person-years for reaction size of 0-4, 5-9, 10-14, and ≥15 mm, respectively, P < 0.001). Consistent results were observed for culture-confirmed cases and after adjustment for gender and baseline age. For those with tuberculin reaction size ≥15 mm, the incidence of TB was significantly higher beyond the age of 15 years than for those less than 15 years (608.1 vs. 37.5 per 100,000 person-years, P < 0.001). Although older baseline age was associated with larger tuberculin reaction sizes, it did not independently predict subsequent development of disease., Conclusion: Strong tuberculin reactions in primary school children predicted TB in adolescents after an initial quiescent period. Endogenous reactivation, possibly related to changes in host immunity, might account for the upsurge of TB in adolescence.

Objective: There are increasing expectations on primary care doctors to care for patients with common mental health problems. This study examines the outcomes of a postgraduate training course in psychological medicine for primary care doctors., Methods: A questionnaire developed by the research team was sent to the Course graduates (year 2003-2007). A retrospective design was adopted to compare their clinical practice characteristics before and after the Course. Differences in the ratings by the respondents before and after the Course were analyzed using the nonparametric Wilcoxon signed rank test., Results: Sixty-nine graduates replied with a response rate of 58.5% (69/118). Most respondents were confident of diagnosing (96.9%) and managing (97.0%) common mental health problems after the Course, compared to 50.0% and 50.7%, respectively, before the Course. Most graduates had modified their approach, increased their attention and empathy to patients with mental health problems. The percentage of respondents having enough time to treat these patients had increased from 55.8% to 72.1%. The median number of patients with mental health problems seen per week was in the range of 3-6 before, and had increased to the range of 7-10 after the Course. The proportion of respondents being confident of making appropriate referrals had increased from 72.8% to 97.0%, while the number of referrals to psychiatrists had dropped significantly., Conclusions: The Course is effective in improving graduates' confidence, attitude, and skills in treating patients with common mental health problems. There are significant increases in the number of mental health patients handled, increased confidence in making referrals to psychiatrists, and decreased percentage of patients being referred.

Objective: To explore the differential outcome of schizophrenia in developed and developing countries. The 15-year outcome of patients with schizophrenia in Hong Kong is reported., Method: In a 15-year retrospective outcome study, 100 patients with first-onset schizophrenia in 1977-1978 were randomly selected for outcome assessment from a pool of 797 patient files. The patterns of outcome were assessed by the same set of instruments and methodology as in other centres, since Hong Kong is 1 of the World Health Organization (WHO) centres for the International Study of Schizophrenia (ISoS)., Results: Nineteen subjects were untraceable, and 10 subjects committed suicide. More than one-half of the subjects had a good symptomatic outcome, while about 20% of the subjects were rated as having a good psychosocial adjustment. Over the entire 15-year period, 96% of the subjects were maintained on medications, and 79% were living with family members., Conclusion: Despite Hong Kong's developed economy, its schizophrenia outcome is similar to that in developing countries. The existence of good family support and active follow-up treatment may be important contributory factors to better outcome.

The purpose of this survey was to understand how the people of Hong Kong perceive mental health, mental illness, mental patients and mental health facilities. In March 1994, a total of 1,043 successful telephone interviews were conducted out of 2,202 valid telephone numbers. The sample was found to be representative of the Hong Kong adult population in terms of sex ratio, age structure, educational achievement and economic status. It was found that the majority of people of Hong Kong were very concerned about their mental health. Their knowledge on mental health was fairly good. Their attitudes towards mental patients and mental health facilities were, however, quite negative. Unlike previous western findings, these views towards mental patients and their rehabilitation facilities were found to be weakly associated with socio-economic factors including age, educational level and household income.

Background: The aim was to assess the clinical usefulness and economic viability of an aftercare worker for psychotic patients discharged from half-way houses in Hong Kong., Method: A sample of 32 chronic psychotic patients was provided with a full-time aftercare worker. A matched control group received no such service., Results: The experimental group was found to have greater and better employment prospects, better mental status with less hospitalisation and less law-breaking behaviour than the control group. Some of these benefits were converted into economic terms and the tangible costs incurred in the project were calculated., Conclusions: The provision of an aftercare service is clinically useful and economically viable.

Advances in modern technology have undoubtedly been of benefit to dentistry in its preventive and curative activities for populations worldwide. Studies, mainly from Western societies, show nonetheless that utilisation of services or adoption of preventive methods are not taken up at satisfactory levels. The focus of this paper is on the special problems in a Chinese society where major differences exist between the traditional lay health concepts and the modernised professional ones. Traditional Chinese health concepts comprise the balance between and interaction of 'Yin' and 'Yang', the vital forces in both the universe and in man's body, as the basis for maintaining health. To restore a disturbed equilibrium of vital forces, which will manifest itself as disease, a range of dietary products are available in Hong Kong, for example cooling herb tea to reduce fire from the stomach producing gum disease. Seeking dental treatment from dentists could be delayed or made 'unnecessary' due to reliance on home remedies. This could reduce the effectiveness of any planned health education activities and dental services available for improving the oral health status of the community according to prevailing Western concepts. An understanding of the cultural meaning of dental disease is fundamental to changing beliefs and attitudes of people in order to improve the oral health of the community.

This paper reports on a study of the living environment of 64 psychiatric patients discharged from half-way houses in Hong Kong. The majority were living in a deprived situation compared to that of their neighbours. They were leading a very monotonous life throughout the year; the walls of their homes were dirty and very plain; and some even ate scraps from a neighbouring bakery. Many did not have any social life at all, and their hobbies were smoking, watching television and gambling. Their neighbours, by comparison, had hobbies such as floriculture, birds and goldfish; decorated their walls with pictures and had newspapers and periodicals in their houses; more often had telephone facilities, wardrobes and sofas; used town gas rather than kerosene stoves; and were more likely to have electrical appliances, especially an iron, refrigerator and shaver. Judged by their income from employment or from public assistance, the 'quality of life' (QOL) of these patients should not have been in such a desperate state. They lacked guidance and instruction in their management of money, and the care of their homes. It is proposed that an aftercare worker might improve the QOL of these discharged psychiatric patients.
