• Incidence of CH in same-sex twins is much higher than that in general population. • Distinguishing same-sex twins and different-sex twins is more feasible in screening. • In cases with a positive result in a same-sex twin, the other one should be recall. • Specific screening guidelines should be evaluated for all same-sex twins. We estimated the incidence of CH in twins, analyse the clinical features of CH cases in twins and further evaluate the CH screening strategy and recall procedures for twins. A retrospective investigation of the screening results and confirmed cases in 724,791 newborns was conducted from 2015 to 2017 in Guangzhou. Clinical features were compared between twins with CH and singletons with CH. In addition, the twins were further divided into same-sex twins and different-sex twins to analyse the characteristics and incidence of CH and to compare differences in the confirmed cases in the 2 groups. The incidence of CH in same-sex twins was 1/593, which was much higher than the incidence of CH in singletons (1/1323) and different-sex twins (1/3060). Of the 20 twins diagnosed with CH, 17 were same-sex twins and 3 were different-sex twins. Among the six pairs of same-sex twins with CH, four had TSH inconsistency, which reached 67%. Eight of the 17 cases of same-sex twins diagnosed with CH had negative results at the first screening. Distinguishing same-sex twins from different-sex twins during newborn screening is more feasible. The incidence of CH in same-sex twins is much higher than that in the general population and the risk of transient CH is relatively high. In positive cases in same-sex twins, the simultaneous recall of the twin can effectively avoid a missed diagnosis. The Guangzhou Twin Registry is a population-based registry of twins residing in Guangzhou City. The initial registry database included 9,700 pairs of young twins recruited from the Official Household Registry of Guangzhou City. The registry is designed to provide a resource to identify the genetic and environmental causes of common diseases with an initial focus on eye diseases. From 2006 onward, phenotype and DNA collection have been completed for more than 1,200 twin pairs and their parents or siblings. Most of the young twins have come back for an annual examination of the progressive traits, such as refraction, ocular biometry, weight, and height. Genome-wide association scans have been completed recently. This article gives an update of the study design, cohort profile, previous findings, and future directions. The Guangzhou Twin Registry, initiated in 2005, is a population-based registry of twins born between 1987 and 2000. To date, over 9700 pairs of twins, regardless of their health and medical history, were enrolled in the database using the Official Household Registry of Guangzhou City. The twins were subsequently verified by door-to-door visits based on the registry address. The primary goal of this registry is to develop a resource for genetic epidemiological studies on common diseases in the southern Chinese population. The initial focus is to distinguish the genetic and environmental determinants of eye diseases, in particular myopia and glaucoma. About 1000 pairs of twins living close to the Zhongshan Ophthalmic Center were invited for the first phenotyping examination, questionnaire administration and DNA collection in July and August 2006. An annual eye examination and other phenotype data collection have been scheduled for up to 5 years in order to investigate changes in phenotypes including the myopia progression, physical development and the changes of other eye-related phenotypes.
