This article focuses on the role of teachers' attitudes towards cultural diversity in teaching refugee students in Germany. We examine which patterns of attitudes towards cultural diversity are common among teachers, how these depend on their professional experience and how they correlate with the perceptions of problems in teaching refugee students. Using data from the project 'Changing schools in a post-migrant society: School culture(s) in the current context of forced migration', the results of cluster analysis show that there is a dividing line between teachers who support pluralistic approaches (i.e. multiculturalism, egalitarianism and anti-racism) and those who reject them. Overall, we identified four clusters: 'pluralism sceptics', 'pluralism opponents', 'pluralism supporters', and 'pragmatists' (who approved of all concepts). The 'pluralism rejecters' (clusters 1 and 2 combined) were the least experienced in teaching refugee children. The 'pluralism supporters' saw the fewest problems in teaching refugee students, while the 'pragmatists' had an above-average perception of problems. The 'pluralism rejecters' occupied a middle position in this regard, with one notable exception: they were particularly likely to perceive cultural differences as a problem in teaching refugee students.

In an era of globalization and global migration multilingualism represents a central educational goal for international political institutions and educational systems. However, except for bilingual schooling programs, institutionalized learning usually takes place monolingually, especially in Europe. Peer-Learning seems to be a promising method for integrating bilingualism in submersion. Pupils sharing the same languages could interact bilingually while working together without the necessity of a bilingual teacher. This paper investigates the prevalence of bilingual interaction of peer-tandems in submersion. The analysis is based upon a quasi-experimental setting with n = 114 Turkish-German bilingual pupils from German elementary schools and makes use of dyadic data analysis and non-parametric methods. The results show that (1) the prevalence of bilingual interaction is nonindependent of the dyad, (2) the learning contents do not affect the prevalence of bilingual interaction, and (3) an explicit offer is needed for the activation of bilingual interaction. Practical implications for the implementation of bilingual peer-learning in submersion will be discussed.

The article "Health Data Utilization Act: The Conflict between Data Protection and Data Usage" deals with the conflict between data protection and the use of health data. The law on improved use of health data in Germany aims to improve digital healthcare and research and create legal certainty. However, there are still challenges due to different nationwide regulations on health data usage in hospitals. Collaboration with data protection authorities and strengthening health data protection are also important factors. The article discusses the Health Data Utilization Act (GDNG) and the associated challenges in dealing with hospital data. It argues that the regulations for hospitals should be clearer, as the legal uncertainty is multiplied by the responsibility of around 100 institutions for data protection. It also addresses the differences between pseudonymization and anonymization of data and emphasizes the importance of a clear definition of legitimate interests for anonymization. The article also mentions the criminal prosecution of re-identification attempts and the need for further criminal procedural regulations to protect health data. It argues that the new regulation of § 25b SGB V exceeds medical competencies and could lead to discrimination. The possibility of objecting to data processing is also mentioned. It is suggested to conduct a pilot project with medical self-administration to verify the reliability of the data. The need for clear regulations for particularly sensitive data is emphasized. Overall, it is argued that data protection has been insufficiently considered in the implementation of the law. The article also addresses the risks to the protection goal of confidentiality in connection with criminal energy and artificial intelligence. It emphasizes that transparent and open collaboration with all stakeholders, as well as careful consideration of these risks, are necessary to achieve the desired health policy goal. The opt-out solution is proposed as a possible compromise to involve patients. Purpose: To survey the materials favored by dentists for intraoral repair of cohesive chipping. Materials and Methods: From August 2019 to February 2020, dentists were surveyed to determine the frequency of cohesive chipping experienced within the last 3 months and to identify their preferred approaches for intraoral repair. Results: Of the participants, 42.5% observed chipping (n = 506). Participants favored the application of roughening devices, silane, and composite resins. Self-etching glass-ceramic primers or hydrofluoric acid were used for ceramic etching. Purpose: The number of homeless people in Germany is steadily increasing. Due to their often precarious living conditions, this specific population may be increasingly exposed to ectoparasites that can transmit various pathogens. To assess the prevalence and thus the risk of such infections, we analyzed the seropositivity of rickettsiosis, Q fever, tularemia and bartonellosis in homeless individuals. Methods: A total of 147 homeless adults from nine shelters in Hamburg, Germany, were included. The individuals underwent questionnaire-based interviewing, physical examination, and venous blood was drawn between May and June 2020. Blood samples were analyzed for antibodies against rickettsiae (Rickettsia typhi and R. conorii), Coxiella burnetii, Francisella tularensis and bartonellae. Results and conclusion: A very low seroprevalence of R. typhi and F. tularensis infection was found (0–1%), while antibodies against R. conorii and C. burnetii were more common (7% each), followed by a relatively high seroprevalence of 14% for bartonellosis. Q fever seroprevalence was associated with the country of origin, whereas bartonellosis seroprevalence was associated with the duration of homelessness. Purpose: Sociodemographic and structural conditions have consequences for the labor market participation of young persons with disabilities (YPWD) in vocational rehabilitation (VR). As the type of program determines the labor market chances, we analyze the processes of selecting active labor market programs (ALMP) in VR. Which factors determine the allocation to (1) programs in general and (2) moreover, the allocation to specific programs? Materials and methods: We conduct logistic (1) and multinomial regression (2) using register data of the German Federal Employment Agency. Besides variables on the micro level, we control for a wide range of structural and organizational influences. The sample comprises VR and employment biographies of 255,009 YPWD accepted to VR between 2010 and 2015. Program participation is restricted to start 180 days after VR acceptance. Results: Sociodemographic factors, like age and the status before entering VR as well as the local apprenticeship market as a structural condition, highly influence the general allocation to ALMP. For the allocation to specific ALMP, sociodemographics (age, education, type of disability, status before entering VR) are highly relevant. Furthermore, structural conditions (regional structure of subsidized vocational training and of the apprenticeship market as well as local work possibilities on a special labor market for PWD) and – to a lesser extent - re-organization processes at the FEA (NEO, VR cohort) are important determinants. Conclusion: (Automatic) paths into VR programs for especially persons with mental disabilities in sheltered workshop are clearly shown. Furthermore, it is somewhat questionable that YPWD participate more often in sheltered workshops in regions where sheltered work possibilities are more common, as well as where NEO was implemented locally; and participate more often in company-external vocational training where VR service providers are commissioned to a greater extent. Objective: Thus far, there is very limited knowledge regarding homeless individuals during the COVID-19 pandemic, particularly related to the health-related quality of life (HRQoL). Thus, our aim was to evaluate HRQoL and to clarify the determinants of HRQoL among homeless individuals during the COVID-19 pandemic in Germany. Methods: Data were taken from the national survey on psychiatric and somatic health of homeless individuals during the COVID-19 pandemic—NAPSHI (n = 616). The established EQ-5D-5L was used to quantify problems in five health dimensions, and its visual analogue scale (EQ-VAS) was used to record self-rated health status. Sociodemographic factors were included in regression analysis. Results: Pain/discomfort was the most frequently reported problem (45.3%), thereafter anxiety/depression (35.9%), mobility (25.4%), usual activities (18.5%) and self-care (11.4%). Average EQ-VAS score was 68.97 (SD: 23.83), and the mean EQ-5D-5L index was 0.85 (SD: 0.24). Regressions showed that higher age and having a health insurance were associated with several problem dimensions. Being married was associated with higher EQ-VAS scores. Conclusions: Overall, our study findings showed a quite high HRQoL among homeless individuals during the COVID-19 pandemic in Germany. (1) Background: Adolescents admitted as acute inpatients belong to a particularly psychosocially vulnerable population. This study aimed to examine the clinical characteristics of an affected population in Germany using a theory-based approach. (2) Methods: We assessed the mental health problems, levels of personality functioning, and the severity of social withdrawal and loneliness in n = 62 adolescents admitted to an acute psychiatric inpatient unit. Cases were investigated cross-sectionally utilizing standardized psychometric questionnaires from the perspective of the patients and clinical experts. (3) Results: Mental health, level of impaired personality functioning, social withdrawal, and loneliness were all positively associated with the need for acute admission. Further analyses revealed that the level of personality functioning fully mediated the positive association between social withdrawal and mental health problems. In contrast, level of personality functioning only partially mediated the positive association between loneliness and mental health problems. (4) Conclusions: Our results suggest that more impairment in personality functioning might lead to poorer mental health when adolescents socially withdraw in the aftermath of the COVID-19 pandemic. Loneliness, social withdrawal, and the level of personality functioning may help identifying essential characteristics of adolescents admitted to acute psychiatric inpatient units and guide the development of specific interventions. Introduction/objectives: The ASCORE study on treatment for rheumatoid arthritis (RA) showed better retention and clinical response rates for abatacept as first-line versus later-line therapy. This post hoc analysis of ASCORE assessed 2-year retention, efficacy, and safety of subcutaneous (SC) abatacept in Germany, Austria, and Switzerland. Methods: Adults with RA who initiated SC abatacept 125 mg once weekly were assessed. Primary endpoint was abatacept retention rate at 2 years. Secondary endpoints: proportions of patients with low disease activity (LDA)/remission per Disease Activity Score in 28 joints based on erythrocyte sedimentation rate (≤ 3.2), Simplified Disease Activity Index (≤ 11), and Clinical Disease Activity Index (≤ 10). Outcomes were analyzed by treatment line and serostatus. Results: For the pooled cohort, the 2-year abatacept retention rate was 47.6%; retention was highest in biologic-naïve patients (50.5% [95% confidence interval 44.9, 55.9]). Patients seropositive for both anti-citrullinated protein antibody (ACPA) and rheumatoid factor (RF; + / +) at baseline had a higher 2-year abatacept retention rate than patients with single seropositivity for either APCA or RF or double-seronegativity (− / −), irrespective of treatment line. At 2 years, a higher proportion of patients who were biologic-naïve were in LDA/remission than patients with one or ≥ two prior biologics. Conclusion: A higher proportion of patients with + / + RA (compared with − / − RA) had abatacept retention after 2 years. Early identification of patients with seropositive RA may facilitate a precision-medicine approach to RA treatment, leading to a higher proportion of patients in LDA/remission. Background: Information on the prevalence of temporomandibular disorders (TMD) or possible/probable bruxism in seniors is heterogeneous and sparse. Objectives: To elucidate the prevalence of TMD and possible/probable bruxism in German adults aged 60 years and older. Methods: Participants of the Interdisciplinary Longitudinal Study of Adult Development and Aging (ILSE) born between 1950–1952 (C1) and 1930–1932 (C2) were examined in 2014–2016 (fourth wave). The participants were surveyed and clinically examined by one calibrated examiner. Two questions of the Patient Health Questionnaire (PHQ) were utilised to evaluate self‐reported bruxism. The clinical examination included signs of probable bruxism and the RDC/TMD examination protocol. Results: Data from 191 participants were available. No RDC/TMD diagnosis was made in 83.2%. Of the participants, 15.2% received a single diagnosis and 1.6% multiple diagnoses that included disc displacements (9.4%) and degenerative joint diseases (8.9%). A total of 24.7% reported bruxism that included self‐reported awake bruxism in 11.9% and sleep bruxism in 16.2%. Wear was clinically identified in 27.2% of the participants. No sex‐related differences were observed. Significant differences were detected for probable bruxism between C1 (14.1%) and C2 (54.3%). Conclusion: In the German population aged 60 years and older, the prevalence of TMD is 16.8%. TMD is characterised by temporomandibular joint disorders, including disc displacements and degenerative joint disorders. Bruxism was observed in a quarter of the old population. Purpose: To survey dentists in Germany regarding their preferred cementation regimen for fixed dental prostheses (FDPs) fabricated from various restorative materials. Materials and Methods: An online survey was developed for completion by dentists in Germany between 08/2019 and 02/2020. The questionnaire gathered information about the cementation regimen (conventional, including the acid-base cements zinc-oxide phosphate or any glass-ionomer cement; self-adhesive; adhesive; don't know) that the participant would preferably select for the insertion of single- or multi-unit FDPs fabricated from various restorative materials. Data were also collected on demographic and personal characteristics. Results: A total of 721 dentists completed the survey, and the data from 688 questionnaires were included in the analysis. Conventional cementation was the option of choice for restorations fabricated from alloy/porcelain-fused- to-metal among 91.0% of the participants and for restorations fabricated from polycrystalline ceramics among 42.9% of the participants. Adhesive cementation was preferred for restorations fabricated from feldspathic/leucite-reinforced glass ceramics (77.9%), lithium-disilicate glass ceramics (67.5%), zirconia-reinforced lithium-silicate glass ceramics (42.0%), and CAD/CAM resin composites (63.8%). Uncertainties regarding the appropriate cementation protocol for restorations fabricated from CAD/CAM resin composites were reported by 15% of the participants. More than 20% of the participants selected a technique that is less or not at all recommended for the cementation of restorations fabricated from zirconia-reinforced lithium-silicate glass ceramic and CAD/CAM resin composites. Conclusion: The results of the survey suggest that dentists in Germany select the cementation regime depending on the restorative material. Some of the participating dentists selected cementation regimens that are less or not at all recommended for restorations fabricated from zirconia-reinforced lithium-silicate glass ceramic and CAD/CAM resin composites, or were not sure about the appropriate cementation protocol. The years between 2005 and 2014 have been declared as a worldwide Decade of Education for Sustainable Development (DESD) by the United Nations. DESD's intended purpose is to promote and more thoroughly focus education as a crucial tool preparing young people to be responsible future citizens, so that our future generations can shape society in a sustainable manner. All educational levels and domains are to be involved in contributing to ESD, including chemistry. This paper reflects upon the meaning of the UN's challenge and on what ESD pedagogy will mean for chemistry education. Additionally, it provides an overview of different models suggesting how such integration of sustainability issues can be compatible with chemistry education. Various consequences and implications arising from this approach will also be discussed. (Contains 1 table and 2 figures.)

Severe feather pecking (SFP) is a behavioral disorder, for which there are multifactorial reasons. Various aspects of pullet and laying-hen husbandry—including housing conditions, management, feeding, and genetics—must be considered, to prevent negative outcomes, such as severe plumage damage, skin injuries, and high mortality rates due to SFP. The aim of this study was to identify housing and management factors in the occurrence of feather pecking, so as to reduce the risk of this behavioral disorder in non-beak-trimmed laying hens on commercial farms with aviaries. Beak trimming may reduce the extent of plumage damage, but it does not prevent feather pecking and cannibalism. Thirty non-beak-trimmed flocks (16 in the first, and 14 in the second laying period) were investigated on 16 commercial farms in Germany. Each flock was visited twice during rearing, and three times during the laying period. During each visit, individual plumage and integument scoring were performed, and data were collected, regarding the housing and management conditions of the flocks. To analyze the influence of management and housing on the plumage condition of the rearing and laying flocks, models were calculated, using univariate multifactorial analysis. In the rearing period, high stocking density and poor litter quality were significant risk factors in plumage damage due to SFP. In the laying period, a lack of free range, poor litter quality, insufficient enrichment, and plumage damage during rearing were significant risk factors for the development of SFP. An individual risk analysis of pullet and layer farms is therefore strongly recommended, to prevent outbreaks of SFP and cannibalism, especially in non-beak-trimmed birds. [ABSTRACT FROM AUTHOR]

We analyzed symptoms and comorbidities as predictors of hospitalization in 710 outpatients in North-East Germany with PCR-confirmed SARS-CoV-2 infection. During the first 3 days of infection, commonly reported symptoms were fatigue (71.8%), arthralgia/myalgia (56.8%), headache (55.1%), and dry cough (51.8%). Loss of smell (anosmia), loss of taste (ageusia), dyspnea, and productive cough were reported with an onset of 4 days. Anosmia or ageusia were reported by only 18% of the participants at day one, but up to 49% between days 7 and 9. Not all participants who reported ageusia also reported anosmia. Individuals suffering from ageusia without anosmia were at highest risk of hospitalization (OR 6.8, 95% CI 2.5–18.1). They also experienced more commonly dyspnea and nausea (OR of 3.0, 2.9, respectively) suggesting pathophysiological connections between these symptoms. Other symptoms significantly associated with increased risk of hospitalization were dyspnea, vomiting, and fever. Among basic parameters and comorbidities, age > 60 years, COPD, prior stroke, diabetes, kidney and cardiac diseases were also associated with increased risk of hospitalization. In conclusion, due to the delayed onset, ageusia and anosmia may be of limited use in differential diagnosis of SARS-CoV-2. Background: Communication is an indispensable skill in the everyday working life of a veterinary team. In German higher educational institutions, communication skills training is explicitly mentioned in the curricula of veterinary assistants, including veterinary nurses and veterinary technicians, and of animal keepers, but not for undergraduate veterinary medicine. Little is known about interprofessional education in veterinary medicine and related professions. Therefore, the purpose of this study is to describe and explore the current interprofessional state of knowledge regarding communication skills of the aforementioned groups in Germany. Methods: To explore the perception, assess the knowledge and identify the attitude regarding communication skills and interprofessional training, an online survey was distributed. The survey was sent to all five veterinary higher educational institutions, 38 schools for veterinary assistants and 15 schools for animal keepers throughout Germany. Results: In total, 294 veterinary students, 111 veterinary assistant trainees and 62 animal keeper trainees participated. The majority of participants (98.07%, n = 458) perceived communication skills as highly important for their everyday work. In total, 413 participants (88.44%) felt that their communication skills needed improvement and more than half admitted having difficulties in effective communication (59.31%; n = 277). In addition, 62.74% of respondents (n = 293) were not sufficiently informed about the training content of their future colleagues. Most were convinced that training could positively influence on their communication with clients (95.72%; n = 447) and the team (92.29%; n = 431), and 76.45% of respondents (n = 357) wished to participate in an interprofessional training. Conclusions: Results of this study confirm that communication skills are perceived as highly important for professional life. Students and trainees show a great interest in communication skills and interprofessional training. The findings indicate that appropriate adjustments to existing curricula are necessary in Germany. Background: Breast cancer (BC), which is most common in elderly women, requires a multidisciplinary and continuous approach to care. With demographic changes, the number of patients with chronic diseases such as BC will increase. This trend will especially hit rural areas, where the majority of the elderly live, in terms of comprehensive health care.Methods: Accessibility to several cancer facilities in Bavaria, Germany, was analyzed with a geographic information system. Facilities were identified from the national BC guideline and from 31 participants in a proof-of-concept study from the Breast Cancer Care for Patients With Metastatic Disease registry. The timeframe for accessibility was defined as 30 or 60 minutes for all population points. The collection of address information was performed with different sources (eg, a physician registry). Routine data from the German Census 2011 and the population-based Cancer Registry of Bavaria were linked at the district level.Results: Females from urban areas (n = 2,938,991 [ie, total of females living in urban areas]) had a higher chance for predefined accessibility to the majority of analyzed facilities in comparison with females from rural areas (n = 3,385,813 [ie, total number of females living in rural areas]) with an odds ratio (OR) of 9.0 for cancer information counselling, an OR of 17.2 for a university hospital, and an OR of 7.2 for a psycho-oncologist. For (inpatient) rehabilitation centers (OR, 0.2) and genetic counselling (OR, 0.3), women from urban areas had lower odds of accessibility within 30 or 60 minutes.Conclusions: Disparities in accessibility between rural and urban areas exist in Bavaria. The identification of underserved areas can help to inform policymakers about disparities in comprehensive health care. Objectives: This study aimed to survey dentists in Germany to identify their favored materials for the fabrication of tooth-supported single crowns (SCs) depending on the location of the abutment teeth and the preparation margin. Materials and methods: The survey included questions regarding demographic characteristics of dentists/their dental practice and preferred restorative materials for the fabrication of SCs for abutment teeth 16, 11, 34, and 36 with either supra- or subgingival preparation margins. Results: Between August 2019 and February 2020, 721 dentists participated in the survey; responses from 33 dentists were excluded from data analysis because the dentists did not work in Germany or provided less than one fixed dental prosthesis/month. Dentists favored ceramic materials independent of the location of the abutment tooth and preparation margin (56.6–92.2%). CAD/CAM resin composites or full metals were preferred by only a few participants. A significantly higher proportion of dentists recommended porcelain fused to metal for subgingival preparation margins than for supragingival margins (p < 0.001). Characteristics of dentists/dental practices influenced a single scenario (11 subgingival) that was dependent on the dentist's time since graduation. When asked to specify the ceramic materials, numerous participants wrote a free response (5.7–7.8%) or did not answer (0.7–4.8%). Conclusions: Dentists in Germany selected restorative materials for SCs depending on the clinical scenario. Since numerous dentists did not specify the ceramic materials, postgraduate information and education might help to extend expertise. Objective: The occurrence of pneumonia separates severe cases of COVID-19 from the majority of cases with mild disease. However, the factors determining whether or not pneumonia develops remain to be fully uncovered. We therefore explored the associations of several lifestyle factors with signs of pneumonia in COVID-19. Methods: Between May and July 2020, we conducted an online survey of 201 adults in Germany who had recently gone through COVID-19, predominantly as outpatients. Of these, 165 had a PCR-based diagnosis and 36 had a retrospective diagnosis by antibody testing. The survey covered demographic information, eight lifestyle factors, comorbidities and medication use. We defined the main outcome as the presence vs. the absence of signs of pneumonia, represented by dyspnea, the requirement for oxygen therapy or intubation. Results: Signs of pneumonia occurred in 39 of the 165 individuals with a PCR-based diagnosis of COVID-19 (23.6%). Among the lifestyle factors examined, only overweight/obesity was associated with signs of pneumonia (odds ratio 2.68 (1.29–5.59) p = 0.008). The observed association remained significant after multivariate adjustment, with BMI as a metric variable, and also after including the antibody-positive individuals into the analysis. Conclusions: This exploratory study finds an association of overweight/obesity with signs of pneumonia in COVID-19. This finding suggests that a signal proportional to body fat mass, such as the hormone leptin, impairs the body's ability to clear SARS-CoV-2 before pneumonia develops. The objective of this study was to evaluate different analytical methods of assessing failure of passive transfer (FPT) in neonatal calves. We hypothesized that 3 different media (i.e., centrifuged serum, centrifuged plasma, filtered plasma) and different analytical methods [i.e., ELISA, capillary electrophoresis (CE), Brix refractometer, and handheld optical refractometer] would be highly correlated with the gold standard radial immunodiffusion (RID) and would generate comparable results. Serum and plasma blood samples were collected from Holstein Friesian calves (n = 216) aged 1 to 7 d, from 2 commercial dairy herds in northeast Germany. The RID analysis showed that 59 of 216 calves (27%) had serum IgG concentrations of <10 mg/mL and 157 calves (73%) had serum concentrations of ≥10 mg/mL. The mean IgG concentration (± standard deviation) was 17.1 ± 9.8 mg/mL, and the range was 0.8 to 47.8 mg/mL. In serum, the correlation between RID and CE was r = 0.97, and between RID and ELISA was r = 0.90; CE and ELISA were also highly correlated (r = 0.89). Both refractometry methods were highly correlated with RID using centrifuged serum, centrifuged plasma, or filtered plasma (Brix refractometer: r = 0.84, 0.80, and 0.78, respectively; handheld optical refractometer: r = 0.83, 0.81, and 0.80, respectively). We determined test characteristics (optimum thresholds, sensitivity, specificity, positive predictive value, negative predictive value, and area under the curve) for CE, ELISA, and the handheld optical and digital refractometers using receiver operating characteristic curve analyses with RID as the reference value. Optimal thresholds for assessing FPT using plasma were higher than for serum, regardless of the method of plasma harvesting. Variegated squirrel bornavirus 1 (VSBV-1) is a zoonotic virus that causes fatal encephalitis in humans who are infected after contact with exotic squirrels. We analyzed the brain lesions and the immune responses in all 4 known human cases that showed panencephalitis. Inflammatory infiltrates in areas positive for VSBV-1 RNA and antigen consisted of CD4+ and CD8+ T cells, with perivascular B-cell accumulation. Strong microglial response and bizarre astroglial expansion were present. Areas of malacia contained neutrophils and foamy microglia and macrophages. Immunopathologic examination during infection showed cleavage of caspase 3 in brain cells adjacent to CD8+ cells and widespread p53 expression, hallmarks of apoptosis. Cerebrospinal fluid analyses over time demonstrated increasing protein concentrations and cell counts, paralleled by pathologic lactate elevations in all patients. The most severe cerebrospinal fluid and histologic changes occurred in the patient with the highest viral load, shortest duration of disease, and most medical preconditions. [ABSTRACT FROM AUTHOR]

Background: Studies from multiple countries have shown that acute kidney injury (AKI) in hospitalized patients is associated with mortality and morbidity. There are no reliable data at present on the incidence and mortality of AKI episodes among hospitalized patients in Germany. The utility of administrative codings of AKI for the identification of AKI episodes is also unclear. Methods: In an exploratory approach, we retrospectively analyzed all episodes of AKI over a period of 3.5 years (2014--2017) on the basis of routinely obtained serum creatinine measurements in 103 161 patients whose creatinine had been measured at least twice and who had been in the hospital for at least two days. We used the "Kidney Disease: Improving Global Outcomes" (KDIGO) criteria for AKI. In parallel, we assessed the administrative coding of discharge diagnoses of the same patients with codes from the International Classification of Diseases (ICD-10-GM). Results: Among 185 760 hospitalizations, stage 1 AKI occurred in 25 417 cases (13.7%), stage 2 in 8503 cases (4.6%), and stage 3 in 5881 cases (3.1%). AKI cases were associated with length of hospital stay, renal morbidity, and overall mortality, and this association was stage-dependent. The in-hospital mortality was 5.1% for patients with stage 1 AKI, 13.7% for patients with stage 2 AKI, and 24.8% for patients with stage 3 AKI. An administrative coding for acute kidney injury (N17) was present in only 28.8% (11 481) of the AKI cases that were identified by creatinine criteria. Like the AKI cases overall, those that were identified by creatinine criteria but were not coded as AKI had significantly higher mortality, and this association was stage-dependent. Conclusion: AKI episodes are common among hospitalized patients and are associated with considerable morbidity and mortality, yet they are inadequately documented and probably often escape the attention of the treating physicians. To compare the in-vitro performance and wear behavior of additively or subtractively fabricated resin-based composite molar crowns for temporary and permanent application. Identical molar crowns (n = 8 per group) were manufactured from materials for temporary or permanent application (3x temporary additive fabrication, 3x additive permanent fabrication, 1x temporary subtractive fabrication, 1x permanent subtractive fabrication). All crowns were adhesively bonded (Calibra Universal, Dentsply Sirona, USA) on standardized resin-based composite molars (FDI 46, P Pro temporary Crown & Bridge). Thermal cycling and mechanical loading (2 × 3000 × 5°C/55 °C, 2min, H 2 0 dist., 1.2 × 106 force 50N) were performed and fracture force was determined (v = 1 mm/min, Z010, Zwick, Germany). Mean wear, maximum wear, and roughness were investigated on polished (P1200) specimens (n = 8 per group, d = 8 mm) in a pin-on-block test (50N; 120000 cycles; 1.6Hz; H 2 O). Statistics were performed by using one-way ANOVA, Bonferroni post-hoc-tests, and Pearson-correlation (α = 0.05). All crowns survived TCML without failures. Fracture forces ranged from 1362.4 ± 182.4N to 2354.1 ± 373.3N for the additive temporary crowns, from 1680.4.4 ± 525.1N to 2601.6 ± 403.7N for the additive permanent crowns, and reached values of 2988.5 ± 604.7N for subtractive temporary crowns and 3092.0 ± 307.6 N for subtractive permanent crowns. Significant (p < 0.001) differences were identified between the various additively manufactured systems, but not for the subtractively fabricated systems (p = 0.673). Mean wear of the additive temporary crowns ranged between 114.5 ± 25.8 μm and 163.8 ± 21.4 μm without significant differences (p = 0.061). Mean wear of the additive permanent crowns ranged between 120.0 ± 27.5 μm and 171.3 ± 31.8 μm with significant differences (p = 0.004). No statistically significant differences were identified between temporary and permanent subtractively manufactured specimens, with mean wear ranging between 140.5 ± 51.1 μm and 176.6 ± 26.8 μm (p = 0.673). Maximum wear of additive temporary specimens ranged between 221.4.5 ± 53.3 μm and 322.1 ± 50.6 μm; significant differences were identified between the groups (p = 0.016). Maximum wear of additive permanent specimens ranged between 246.3 ± 47.3 μm and 337.4 ± 61.4 μm, and significant differences were identified between the groups (p = 0.006). Mean wear of the subtractive group (permanent and temporary) showed no differences in maximum wear from 277.9 ± 79.7.1 μm to 316.4 ± 58.1 μm (p = 0.288). Ra roughness ranged from 0.7 ± 0.2 μm to 3.6 ± 1.3 μm with significant differences (p < 0.001) and Rz reference between 65.9 ± 26.2 μm and 16.8 ± 6.3 μm. Temporary and permanent molar crowns provided at least acceptable in-vitro performance and fracture force for clinical mid-term application. Aims: The Study on Mental Health at Work (S-MGA) generates the first nationwide representative survey enabling the exploration of the relationship between working conditions, mental health and functioning. This paper describes the study design, sampling procedures and data collection, and presents a summary of the sample characteristics. Methods: S-MGA is a representative study of German employees aged 31–60 years subject to social security contributions. The sample was drawn from the employment register based on a two-stage cluster sampling procedure. Firstly, 206 municipalities were randomly selected from a pool of 12,227 municipalities in Germany. Secondly, 13,590 addresses were drawn from the selected municipalities for the purpose of conducting 4500 face-to-face interviews. The questionnaire covers psychosocial working and employment conditions, measures of mental health, work ability and functioning. Data from personal interviews were combined with employment histories from register data. Descriptive statistics of socio-demographic characteristics and logistic regressions analyses were used for comparing population, gross sample and respondents. Results: In total, 4511 face-to-face interviews were conducted. A test for sampling bias revealed that individuals in older cohorts participated more often, while individuals with an unknown educational level, residing in major cities or with a non-German ethnic background were slightly underrepresented. Conclusions: There is no indication of major deviations in characteristics between the basic population and the sample of respondents. Hence, S-MGA provides representative data for research on work and health, designed as a cohort study with plans to rerun the survey 5 years after the first assessment. [ABSTRACT FROM AUTHOR]

Purpose: The present study aims to analyze a cohort of advanced breast cancer patients in Germany to assess their interest in complementary and alternative medicine (CAM) and patient's use of most frequent CAM methods.Patients and Methods: Based on the PREGNANT real-time breast cancer registry which is a multicenter study in Germany, questionnaires of 580 patients with advanced breast cancer were evaluated. The implemented questionnaire for CAM asked for general interest in CAM and for patient's use of different CAM methods at present and in the past. The interest and application of CAM were analyzed for association with patients' characteristics such as tumor, patient, and therapy characteristics.Results: In total, 436 out of 580 (75%) patients claimed to be interested in CAM. Further, interest in CAM is significantly correlated with younger age and absence of metastasis at the time of diagnosis. Multivariate analysis confirmed the patient's age and distant disease status at the time of diagnosis as related to interest in CAM. A total of 56.4% of patients applied any CAM method in the past. Moreover, with increasing lines of therapies, the more frequent use of CAM was observed. Hereby, praying, vitamin supplements, and other food supplements were most frequently applied.Conclusion: Our data demonstrate high overall interest and frequent use of CAM in advanced breast cancer patients supporting a strong demand of breast cancer patients for complementary counseling and treatments additional to the established cancer therapies. It is indispensable to implement counseling and evidence-based complementary treatments into clinical routine of cancer centers and to adapt postgraduate medical education, respectively. Informal learning activities are increasingly acknowledged as significant for learning and development in modern workplaces. Yet, systematic research on effects of informal learning on work‐related outcomes remains scarce. The present research focuses on deliberate practice—a construct from cognitive‐psychological expertise research that describes effortful practice activities specifically designed to improve one's performance. We propose that deliberate practice can be applied informally at work and, in the context of entrepreneurship, may contribute to entrepreneurial success. In a longitudinal study with 132 small business owners in Germany, we found partial support for the notion that success is increased in entrepreneurs who engage in self‐regulated and informal deliberate practice. In addition, deliberate practice interacted with environmental dynamism, indicating that deliberate practice pays off particularly in dynamic environments and may be detrimental in stable environments. This research not only informs entrepreneurial research as it sheds light on how entrepreneurs learn and develop their capabilities outside systematic training. It may also have broader implications for work and organisational psychology as self‐regulated deliberate practice may be a useful informal learning activity for a wider range of occupations and across work tasks, particularly those with rapidly changing work requirements. We analyse strategic groups in the German insurance market. We use cluster analysis to subdivide insurance groups into strategic groups. Furthermore, we analyse whether strategic group affiliation can affect the performance of the insurance groups' property-liability subsidiaries. In addition, we examine the consequences of the financial crisis of 2008 on the competitive situation in the German insurance sector and examine whether changes in strategic group affiliation can be considered as a consequence of the financial crisis. Using a data set of 829 firm-year observations for the years 2004-2012, our results indicate the existence of three strategic group in the German insurance sector. In addition, we find that performance differences at the subsidiary level can be attributed to strategic group affiliation. Purpose: We investigated whether (1) current employment status (regular full-time, regular part-time and marginal employment) is associated with depressive symptoms and (2) whether these associations are mediated by current working conditions and previous employment history. Methods: Two cohorts of German employees aged 46 and 52 years were selected from administrative data of the German Federal Employment Agency and answered questions about depressive symptoms (we use an applied version of BDI-V) and their current working conditions. In addition, the participants gave written consent to link register data regarding their employment histories ( n = 4,207). Multiple linear regression analyses were conducted. Results: Men experienced elevated depressive symptoms when working regular part-time; women experienced such symptoms when engaged in marginal employment. These associations decreased when we adjusted for job insecurity and rose slightly when we adjusted for leadership quality. Men and women who reported a low level of influence at work showed a higher risk of depressive symptoms. For women, the association between current employment position and depressive symptoms could be partly explained by low levels of influence at work. For men, the association between depressive symptoms and current regular part-time employment decreased when we adjusted for previous part-time employment. Conversely, for women, the association with depressive symptoms increased in current regular part-time and marginal employment when we adjusted for employment history. Conclusions: In both genders, the observed associations between depressive symptoms and current employment status were mediated by both current psychosocial conditions and employment history. Regional geochemical mapping projects have been carried out for mineral exploration in Germany since 1954. In the early days, geochemical prospecting methods were mainly used for exploration of ore and non-metal deposits in Germany. Mapping for the first geochemical atlas of Germany was conducted in the 1980s by the Federal Institute of Geosciences and Natural Resources (BGR) with a sample density of one sample per 3 km 2 . Parts of eastern Germany (Flechtingen Ridge, Harz Mountains, Thuringian Forest, Slate Mountains, Ore Mountains, Granulite Mountains, the Lausitz region, and along the Elbe River) were mapped by the former Central Geological Institute Berlin (ZGI) during the same period at a density of 1.3 samples per km 2 . Stream sediment and surface water were sampled in all of these high-density geochemical mapping projects. The first low sample density geochemical survey (one site per 380 km 2 ) was carried out for the latest geochemical atlas of Germany, which presents comprehensive information about the regional distribution of hazardous inorganic and organic substances. A density of one sample per 5000 km 2 was also used in Germany for the FOREGS Geochemical Mapping Project of Europe. Geochemical stream sediment and surface water data obtained at these three different scales in different geochemical mapping projects have been compared in this study for the elements Ba, Cu, Cr, Pb and U in stream sediment as well as pH, EC and U in stream water. Comparison of overlapping high and low sample density surveys conducted in the same study areas, demonstrates that the geochemical patterns produced from low sample density surveys are very nearly the same as those from the high sample density surveys, and that they can be related to natural processes. The low sample density geochemical surveys in Germany provide element background values very similar to those obtained by high sample density mapping of the same region. Background: The best therapy for patients with stage I non-small cell lung cancer (NSCLC) who are medically unfit for lobectomy or prefer not to undergo surgery has not yet been demonstrated. Objectives: We analyzed data from our prospective database to evaluate the recurrence and survival rates and assess the extent to which the type of treatment explains outcome differences. Methods: This study included 116 patients with histologically proven clinical stage I NSCLC who were treated with sublobar resection (SLR; n = 42), radiofrequency ablation (RFA; n = 25) or radiotherapy (RT; n = 49) between 2009 and 2013. The primary end point was the time to primary tumor recurrence (PR). Kaplan-Meier curves and Cox regression were used to compare the recurrence patterns and survivals after adjustments for potential confounders. Results: The SLR patients were younger and exhibited better performance status. The RT patients had larger tumors. After adjusting for age and tumor size, there were differences between the different treatments in terms of the PR rate, but no differences were observed in overall (OS) or disease-free survival. The hazard ratio for PR comparing SLR versus RT adjusted for age and tumor size was 2.73 (95% confidence interval, CI, 0.72-10.27) and that for SLR versus RFA was 7.57 (95% CI 1.94-29.47). Conclusions: Our study suggests that SLR was associated with a higher primary tumor control rate compared to RFA or RT, although the OSs were not different. The lidA Cohort Study (German Cohort Study on Work, Age, Health and Work Participation) was set up to investigate and follow the effects of work and work context on the physical and psychological health of the ageing workforce in Germany and subsequently on work participation. Cohort participants are initially employed people subject to social security contributions and born in either 1959 (n = 2909) or 1965 (n = 3676). They were personally interviewed in their homes in 2011 and will be visited every 3 years. Data collection comprises socio-demographic data, work and private exposures, work ability, work and work participation attitudes, health, health-related behaviour, personality and attitudinal indicators. Employment biographies are assessed using register data. Subjective health reports and physical strength measures are complemented by health insurance claims data, where permission was given. A conceptual framework has been developed for the lidA Cohort Study within which three confirmatory sub-models assess the interdependencies of work and health considering age, gender and socioeconomic status. Background: Chronic heart failure (CHF) patients suffer from multiple and agonizing symptoms like shortness of breath and reduced functional status, the latter of which is usually assessed using New York Heart Association (NYHA) functional class. Purpose: In order to identify potentially modifiable factors of reduced functional status in patients with CHF, we investigated somatic and psychosocial correlates of NYHA functional class. We subsequently compared the results to correlates of left ventricular ejection fraction (LVEF) as an objective parameter of disease severity. Methods: The cross-sectional study ( n = 314) was part of the German Heart Failure Network. Data were analysed using a logistic regression modelling process. Results: In the final regression model, NYHA functional class was only significantly associated with depression (odds ratio (OR) = 1.18; 95 % confidence interval (CI) = 1.09-1.27) and multimorbidity (OR = 1.17; 95 % CI = 1.04-1.32). LVEF was associated with amino-terminal pro-brain natriuretic peptide (NT-proBNP) (OR = 0.60; 95 % CI = 0.44-0.82), aetiology of CHF (OR = 2.08; 95 % CI = 1.11-3.90), and smoking (OR = 2.21; 95 % CI = 1.25-3.91). Conclusions: Whereas LVEF was not related to depression, our data revealed a close association between functional status (as assessed by NYHA functional class) and depression in patients with CHF, even after adjusting for cardiac and socio-demographic variables. Different forms of underlying causal mechanisms could be suspected; it appears promising to further investigate this specific interaction. In any event, pending further investigation, our results underscore the need to examine CHF patients with respect to both somatic symptom burden and potential depressive disorders. Background Three-dimensional transesophageal echocardiography (3D TEE) has been demonstrated to provide more accurate information than two-dimensional transesophageal echocardiography (2D TEE) in the localization of mitral valve prolapse ( MVP). However, most studies have been single-rater studies. Few results were tested for inter-observer variability with a single second rater. This multicenter study aimed to determine reliability of 2D and 3D TEE mitral valve evaluations by calculating inter-observer agreement between various echocardiographers. Methods Fifteen observers from 4 institutions in Germany and Switzerland interpreted 2D and 3D TEE images from 6 patients selected to represent a large spectrum of MVP diversity. Surgical findings served as reference. Individual assessments of MVP and ruptured chordae tendineae (ChR) pathology were compared by calculating Randolph's free-marginal multirater kappa coefficient. Results Accuracy of MVP evaluation with 3D TEE was 83.9%, CI [81.0%; 86.8%] and 78.7% CI [76.6% 80.8%] with 2D TEE. Flail leaflets with chordal ruptures were described correctly in 91.1%, CI [85.8, 96.4] with 3D compared to 71.1%, CI [65.0, 77.2] with 2D TEE. The multirater kappa coefficient of inter-observer agreement among all 15 observers was κ = 0.65/0.58 for 3D/2D evaluation of MVP and κ = 0.70/0.54 for detection of ChR. Conclusion Three-dimensional assessment of MVP was superior to 2D TEE, although the accuracy of both 3D TEE and 2D TEE was found to be lower than previously published. 3D MVP assessment is less operator dependent than 2D TEE evaluation. Although validity has been demonstrated before, we provide evidence that 3D TEE is reproducible among 15 observers and is a reliable method for MVP evaluation. [ABSTRACT FROM AUTHOR]

Abstract: The results presented in this paper on uranium in bottled and tap water were determined within the scope of the project “European Groundwater Geochemistry: Bottled Water” of the Geochemistry Expert Group of EuroGeoSurveys. The analyses of bottled water provide an inexpensive approach to obtain information about European groundwater geochemistry. For this study, the uranium concentrations in 1785 European mineral water samples were analyzed by ICP–QMS in the BGR laboratories. The dataset is used to obtain a first impression about natural concentration levels and variation of uranium in groundwater (and bottled water) at the German and European scale. A range of <0.0005–16.0µg U/L (median 0.17µg/L) was obtained in 908 bottled water samples in Germany. A range of 0.00115–9.0µg U/L (median 0.073µg U/L) was obtained for tap water from 163 municipal tap water systems. The regional distribution of uranium in the analyzed bottled and tap water is influenced primarily by lithological (Bunter and Keuper strata, crystalline basement rocks, Permian strata, and loess) and anthropogenic (old mining districts, phosphate fertilizers, and industrial emissions) factors. Elevated uranium concentrations (>2µg U/L) in bottled water have geological sources and were found in Hesse, Thuringia, North Rhine–Westphalia, Saxony, southern part of Saxony–Anhalt and in Baden–Württemberg (Black Forest). Bunter and Keuper sandstone are the main source of uranium, followed by Permian and crystalline basement aquifers. The EU Guidelines for Drinking Water and the German Drinking Water Regulations give no upper limit for uranium; however, a provisional value of 15µg U/L exists in the Guidelines for Drinking Water Quality of the World Health Organization. The only value for German bottled water is 2µg U/L, above which the label may not say that the water may be used for the preparation of baby food. Nevertheless the uranium content of bottled and tap water needs to be monitored and controlled by governmental offices. In all, 14% of all the analyzed German bottled water samples contain >2µg U/L and 15% of all analyzed European bottled water. Germany is the country with the highest number of bottled mineral water brands (908 bottled water samples from 502 wells/brands were analyzed). The per capita consumption of mineral water in Germany in 2003 was 129L. A wide range of values of one to seven orders of magnitude was determined for 71 elements in the bottled water samples analyzed by ICP-QMS, ICP-AES, IC, titration, photometric, conductometric and potentiometric methods. A comparison of the element concentrations and the legal limits for both bottled and tap water (EU, Germany, US EPA, WHO) shows that only 70% of the 908 mineral water samples fulfill the German and EU drinking water (i.e., tap water) regulations for all parameters (not including pH) for which action levels are defined. Nearly 5% of the bottled water samples not fulfill the German and EU regulations for mineral and table water. Comparison of our results with the current German and European action levels for mineral and table water shows that only 42 of the bottled water samples exceed the limits for one or more of the following elements: arsenic, nitrate, nitrite, manganese, nickel and barium concentrations. Ten of the bottled water samples contain uranium concentrations above the 10μg/L recommended limit. A selection of existing water quality classification systems was used in order to characterize the different mineral water types. For example, the relationships among selected variables were examined using Durov, Piper and van Wirdum diagrams. Univariate and multivariate (component analysis) statistical methods were used to analyze the data to characterize the bottled water from the different well regions of Germany. Maps of the element distributions are presented. Most of the elements show a clear regional dependency. The bottled water shows conspicuous geochemical influences by the limestone aquifers of the Muschelkalk (Middle Triassic), the sandstone aquifers of the Bunter and Keuper (Lower and Upper Triassic), the gypsum of the Keuper, and the aquifers in gneiss basement rocks. A new reference calorimeter has been developed under a European research project and set-up by Physikalisch-Technische Bundesanstalt (PTB) in Germany. The objective of the project is to measure the superior calorific value (SCV) of methane and other pure gases with a measurement uncertainty of less than 0.05 %. This paper presents the measurement results obtained for methane. Nine repeatability measurements were made. The molar SCV obtained when the measurements were averaged is 890.578 kJ·mol−1. This value agrees very accurately with the value of 890.63 kJ·mol−1 specified by ISO 6976 [ Natural Gas—Calculation of Calorific Values, Density, Relative Density and Wobbe Index from Composition. International Standard ISO 6976, corrected and reprinted 1996-02-01]. Twice the standard deviation determined for the measurements is 0.023 % and is thus clearly lower than in previous experiments. The recent German welfare state reform with the introduction of Social Code II has created a complex situation for the labour market integration of long-term unemployed people with disabilities. A range of social laws with differing underlying principles is now applicable. In this article, we examine the effects that the implementation of this social code has on long-term unemployed people with disabilities. We show that their integration patterns changed. This is due to the building of new institutions responsible for labour market integration, followed by a temporary destabilisation of work routines at the operational level. Additionally, more persistent consequences occur because the inconsistencies of the relevant laws are creating an area of conflict, which is increasing the risk of marginalising people with disabilities in terms of labour market integration. The concentration of uranium was determined in 944 samples from stream water by the inductively coupled plasma quadrupole mass spectrometry (ICP-QMS) method and represented on a color-shaded contour map. Uranium concentrations in surface water were determined to be between 0.007 μg/l and 43.7 μg/l with median of 0.33 μg/l. The regional distribution of uranium is influenced primarily by lithological and anthropogenic factors. In Mecklenburg, northern Brandenburg, and eastern Schleswig-Holstein, elevated uranium concentrations coincide with the extent of the last Weichselian ice sheet. The maximum concentrations are observed in the surface waters of the old mining districts in the western part of the Ore Mountains and in eastern Thuringia. Elevated concentrations are found in areas of agriculturally used loess soils. These concentrations correlate with the use of phosphate fertilizers. There is a zone of elevated concentrations up to 10.0 μg U/l in the Keuper Sandstone area south of the Thuringian Forest and from northwest of Stuttgart as far as Coburg. The distribution of elevated values in mineral water shows a clear correlation with the elevated values in surface water and the geology of those locations. Simple Summary: Severe feather pecking (SFP) is a major animal welfare problem in layers. It results in pain and injuries in the affected animal. SFP is a behavioral disorder and should not be confused with aggressive pecking. The aim of our study was to observe the pecking behavior of layers on farms with flock sizes common in practice and to identify possible influencing factors. We found that SFP occurred in all flocks, but the pecking rate varied widely between flocks. A low stocking density and the provision of a winter garden or free range (or both) had a positive effect and reduced SFP. Keeping mixed flocks of brown and white layers was a risk factor for SFP. SFP occurred mainly in the litter area and only rarely on perches. This finding emphasizes the importance of providing enough litter, litter areas and environmental enrichment. Aggressive pecking and SFP were correlated, which may indicate a higher stress level in the flock. Beak trimming reduced pecking rates but did not entirely prevent SFP. Instead of subjecting chicks to this potentially painful procedure, reasons for SFP should be addressed. SFP remains a multifactorial problem, but in recent years, many risk factors have been identified and included in best-practice recommendations, allowing the housing of non-beak-trimmed layers. Severe feather pecking (SFP) is a major animal welfare problem in layers. It results in pain and injuries in the affected animal. It was the aim of this study to gain insight into the actual pecking behavior of laying hens kept on commercial farms with flock sizes common in practice. We observed aggressive pecking and SFP in non-beak-trimmed and beak-trimmed flocks of laying hens and investigated possible influencing factors. The study took place on eight conventional farms in Germany with aviaries, including three farms with a free range and a winter garden, one with a free range and one with a winter garden. Pecking behavior was observed during three observational periods (OPs): OP 1, at the peak of the laying period between the 28th and 33rd week of life; OP 2, in the middle of the laying period between the 42nd and 48th week of life; and OP 3, at the end of the laying period between the 63rd and 68th week of life in one laying period. Videos were analyzed using behavior sampling and continuous recording. We found that SFP occurred in all flocks, but the pecking rate differed significantly between the flocks. SFP correlated positively with the number of hens per square meter of usable area, with statistical significance in the litter area (r = 0.564; p = 0.045). The multivariate analysis revealed that access to a winter garden or free range significantly reduced the SFP rate on perches (p = 0.001). The stocking density (number of birds per usable square meter) had a significant influence on the SPF rate in the nest-box area (p = 0.001). The hybrid line had a significant effect on the SFP rate on perches and in the nest-box area (p = 0.001 each). Lohmann Brown hens in mixed flocks had a higher SFP rate (significant in OP 2) than those in homogeneous flocks, indicating that mixed flocks may be a risk factor for SFP. Lohmann Brown hens pecked significantly less than Dekalb White hens in the litter area (p = 0.010) and in the nest-box area (p = 0.025) and less than Lohmann Selected Leghorn hens in the litter area (p = 0.010). Lohmann Brown and Lohmann Selected Leghorn hens showed increasing SFP rates during the laying period. All hybrid lines had significantly higher SFP rates in the litter area, followed by the nest-box area and perches. These findings emphasize the importance of providing enough litter, litter areas and environmental enrichment. We found a significant positive correlation between aggressive pecking and SFP—in OP 1: rho (Spearman) = 0.580, p < 0.001; OP 2: rho = 0.486, p = 0.002; and OP 3: rho = 0.482, p = 0.002 (n = 39) —indicating that SFP may lead to a higher stress level in the flock. Beak trimming reduced pecking rates but did not entirely prevent SFP. Instead of subjecting chicks to this potentially painful procedure, reasons for SFP should be addressed. In conclusion, our data suggest a positive influence of a lower stocking density and the provision of a winter garden or free range for additional space. The hybrid line had a significant influence on the feather-pecking rate on perches and the nest-box area. Aggressive pecking and severe feather pecking correlated positively. We assume that vigorous and painful AP were an additional stress factor, especially in non-beak-trimmed flocks, leading to more SFP in due course. Beak trimming had a reducing effect on SFP. Background: Despite the growing concern over its potentially severe side effects and considerable economic burden, stress ulcer prophylaxis (SUP) is still frequently prescribed to patients in medical non-intensive care units. Recent data indicate that the situation is similar in surgical departments. Currently, data on the concepts within and regulation of routine SUP practice in surgical departments are sparse. The present study was designed to examine the current practice of SUP in Mecklenburg West Pomerania, Germany, and to identify possible reasons for the dissociation of medical literature and clinical practice. Methods: A questionnaire-based survey was conducted to elucidate current SUP practices in surgical departments of acute care hospitals in Mecklenburg Western Pomerania, Germany. Results: In most surgical departments (68%), a standard operating procedure (SOP) for SUP had not been developed. In departments with an existing SOP, 47.6% of responding medical staff members (MSM) with prescribing authority did not know of its existence. Of the MSMs aware of the existence of an SUP-SOP, only 42.9% indicated that they were familiar with its content. Critical re-evaluation of SUP indications upon transfer from the intensive care unit (ICU) to the general hospital ward (GHW) and before hospital discharge was performed frequently or systematically by only about half of the responding MSMs. Discussion: In the face of continued massive over-prescription of SUP in the perioperative routine, the development of easy-to-use local guidelines and their strict implementation in the clinical routine, as well as intensified medial education on this subject, may be effective tools to reduce acid-suppressive medication (ASM) associated side effects and economic burden.
