Purpose: The aim of this multicenter single mandibular implant study was to compare the clinical outcome of an immediately loaded implant placed in the midline of an edentulous mandible with the clinical outcome when closed healing and delayed loading is chosen. Here, the patient recruitment period and the main reasons for exclusion were analyzed.Materials and Methods: Patient recruitment was performed at nine German universities. Rather conservative inclusion and exclusion criteria, including denture satisfaction, denture status, a psychologic symptom checklist, and a defined bone height in the posterior mandible, had to be fulfilled. It was initially calculated that 230 persons would have to be screened within 13 months to include 180 persons in the trial.Results: Within 13 months (December 2012 to December 2013), 201 patients were screened for possible inclusion in the trial and 148 met the inclusion criteria. Finally, after the recruitment period was extended by 2.5 months, 224 patients were screened and 169 were included. Of those screened, 55 (24.6%) did not meet the inclusion criteria and were excluded. Another 4 patients (1.8%) were excluded based on their psychologic symptom checklist score, while 5 others (2.8%) showed signs of noncompliance. A further 8 patients (4.4%) decided not to participate and withdrew their informed consent, and another 3 (1.3%) were no longer available after screening. Another 2 patients were excluded due to medical contraindications and 1 due to an insufficient mandibular denture. In 34 cases (15.2%), the residual bone height did not comply with McGarry type II or III.Conclusion: Within the limitations of this patient recruitment period, it can be concluded that residual bone height is the most important factor when considering elderly edentulous patients for implant therapy. Aim: To evaluate the efficacy of different types of rehabilitation with fixed or removable full-arch implant-supported prosthesis designs in terms of implant loss and success in patients with at least one edentulous jaw, with tooth loss mainly due to periodontitis. Materials and methods: Clinical studies with at least 12 months reporting on implant loss and implant success were searched. Meta-analysis was conducted to estimate cumulative implant loss considering different prostheses designs. Results: A total of 11 studies with unclear to low risk of bias were included in the analysis. Estimated cumulative implant loss for fixed prostheses within 1 year and 5 years was 0.64% (95% confidence interval [CI]: 0.31%-1.31%) and 1.85% (95% CI: 0.85%-3.95%), respectively. The corresponding values for removable prostheses amounted to 0.71% (95% CI: 0.22%-2.28%) and 4.45% (95% CI: 2.48%-7.85%). Peri-implantitis affected 10%-50% of the patients restored with implant-supported fixed prostheses. Conclusions: Based on the limited low-quality data, the present analysis points to a low and similar cumulative implant loss within 1 year for patients with tooth loss mainly due to stage IV periodontitis restored with either removable or fixed implantsupported full-arch prosthesis. Purpose: To evaluate changes in marginal bone levels around maxillary and mandibular mini-implants stabilizing complete dentures and to explore possible risk factors associated with bone loss. Materials and Methods: All eligible patients from nine private dental practices were invited to participate. Panoramic radiographs were obtained postoperatively and at the follow-up examination. The changes in bone level were estimated with linear mixed models that included the factors sex, age, jaw, region (anterior versus posterior), smoking habits, and loading concept. Results: Of the 180 invited patients, 133 participated in the follow-up (response rate: 73.9%). Of 336 mini-implants in 54 maxillas and 402 mini-implants in 95 mandibles, 15 maxillary Implants and 11 mandibular implants were lost after insertion, and 4 mandibular implants fractured. Radiographic evaluations in 11 participants were not possible. The mean marginal bone loss at the remaining 634 mini-implants in 122 patients was 0.8 mm in the maxilla and 0.5 mm in the mandible over a mean observation time of 2.2 ± 1.0 years. This difference in univariate estimation lost significance after adjusting for the other variables. Implants in former smokers showed more bone loss than implants in participants who had never smoked. Implants that were loaded 3 to 4 months after placement (delayed loading) showed more marginal bone loss than immediately loaded implants. No statistically significant differences were found between men and women, different age groups, or anterior and posterior implants. Conclusion: Mean marginal bone loss around mini-implants used to stabilize complete dentures was insignificantly higher in the maxilla than in the mandible after a mean observation period of 2.3 years. These values are comparable with marginal bone loss around standard-diameter implants. Purpose: Few studies have compared the effects of immediate loading on implant prognoses in smokers and nonsmokers. The aim of this prospective study was to evaluate the long-term success of immediately loaded platform-switched implants placed in smokers and nonsmokers with edentulous jaws. Materials and Methods: Platform-switched implants were placed using the same treatment protocol in the healed edentulous jaws of two groups of patients: group A patients had smoked at least 20 cigarettes a day for more than 10 years, and group B consisted of nonsmokers. All implants were loaded immediately with provisional fixed prostheses, and definitive cement-retained restorations were delivered 4 to 6 weeks later without removing the abutments. In a standardized follow-up protocol, the implants were evaluated clinically and radiographically at the start of loading, 4 to 6 weeks later, and annually thereafter. Results: Sixty-six implants (36 in the maxilla and 30 in the mandible) were placed in eight smokers. Twelve nonsmokers received 97 implants (55 in the maxilla and 42 in the mandible). During an average loading period of 62.53 (± 44.13) months for the smokers and 98.20 (± 19.53) months for the nonsmokers, three implants failed. Two failed in group A (one because of overloading and one because of peri-implanitis) and one failed in group B (overloading), resulting in implant survival rates of 97% and 99%, respectively. No significant differences were found between groups regarding plaque, bleeding, probing pocket depths, or crestal bone loss, either mesially or distally, resulting in success rates of 92.42% for group A and 98.00% for group B. Conclusions: Long-term clinical outcomes for immediately loaded platform-shifted implants placed in heavy smokers appear to be comparable to those for nonsmokers if the abutments are placed on the day of surgery and never removed. Background: It is not well-known which pre-implantological procedures are preferred by maxillofacial (MFS) and oral surgeons (OS) for the narrow atrophic alveolar ridge under practice based conditions and, if different training paths in surgery lead to other pre-implantological techniques being preferred. This study aims to identify which procedures are preferred by the respective specialists in which indication. Methods: A questionnaire was sent to a total of 300 MFS and OS in southern Germany. The questionnaire examined pre-implantological procedures (bone block, bone grafting material and/or particulate autogenous bone, titanium mesh, bone split, resection) in the edentulous severely atrophic mandible and in the severely atrophic single-tooth gap. Kendall's Tau-b test was used for statistical analyses. Results: One hundred seventeen participants returned the questionnaire. 68 (58%) were OS and 49 (42%) were MFS. In the edentulous mandible, bone substitute material and resection were most preferred by both specialists. Bone blocks were statistically significantly more frequently associated with MFS and bone substitute materials with OS. Bone split was more frequently used in the atrophic single tooth gap than in the edentulous mandible. OS preferred bone blocks in the single tooth gap more often than in the edentulous mandible. MFS and OS preferred resection in the edentulous mandible significantly more frequently than in the single tooth gap. Conclusions: MFS in general prefer more invasive pre-implantological therapies with the same initial diagnosis than OS, which seems to be attributed to different training paths. Purpose: The use of short implants can reduce the need for augmentative procedures prior to implant placement and, thus, morbidity and treatment time for patients with severely atrophied alveolar ridges. However, the inevitably less favorable crown-to-implant ratio is often associated with higher implant failure rates and greater marginal bone loss. The aim of this study was to evaluate the long-term survival and success rates of short implants in severely atrophic alveolar ridges retaining restorations on these short implants only. Materials and Methods: In this study, 8-mm and 9-mm implants were inserted in atrophic alveolar ridges according to the manufacturer's protocol for the respective bone quality and loaded after 3 months of healing. Prosthetic restorations were supported only by short implants (not in combination with longer implants). After a mean observation period of 10.1 years (± 1.9 years), all patients were re-examined clinically and radiographically. Results: In this study, fifty-two 8-mm and 9-mm implants were placed in 14 patients. After 10.1 years, no implants and suprastructures had been lost. A mean marginal bone loss of 0.3 mm (± 0.4 mm) was recorded. According to the Albrektsson criteria, all implants were successful; with respect to the more rigorous Karoussis et al criteria, four implants failed. Conclusions: The results of this long-term study suggest that the use of short implants results in marginal bone resorption and failure rates similar to those for longer implants. Aims: Since occlusal variables such as overbite and overjet have been thought to be associated with tern porornandibular disorders (TMD), and joint sounds are sorne of the rnost prevalent signs of TMD, the airn of this study was to deterrnine whether overbite and overjet are risk factors for tern porornandibular joint (TMJ) sounds. Methods: A population-based cross-sectional study of 3,033 subjects (age range, 10 to 75 years; 53% fernale) was con- ducted in Gerrnany. Overbite/overjet, reproducible reciprocal clicking (RRC) during open-close jaw rnovernents that did not occur in the protrusive jaw position, and joint crepitus were assessed according to the Research Diagnostic Criteria for Tern porornandibular Disorders (RDC/TMD). Results: When age and gender were controlled for, high or low values of overbite and overjet were not associated with a greater risk of RRC and crepi- tus as corn pared to a reference category of a norrnal overbite and overjet of 2 to 3 rnrn (rnultiple logistic regression; odds ratios 0.7 to 1.3; P > .05 for all). Conclusion: This study showed that higher or lower overbite or overjet jaw relationships, even extrerne val- ues, are not risk factors for TMJ sounds as assessed by clinical exarnination. It was the aim of this 24-mo randomized controlled clinical trial to investigate whether the survival of a single median implant placed in the edentulous mandible to retain a complete denture is not compromised by immediate loading. Secondary outcomes were differences in prosthetic complications between the loading principles. Each of the 158 patients who received an implant was randomly assigned to the immediate loading group ( n = 81) or the delayed loading group ( n = 77). Recall visits were performed 1 mo after implant placement (for only the delayed loading group) and 1, 4, 12, and 24 mo after implant loading. Nine implants failed in the immediate loading group, all within the first 3 mo of implant loading, and 1 implant failed in the delayed loading group prior to loading. Noninferiority of implant survival of the immediate loading group, as compared with the delayed loading group, could not be shown ( P = 0.81). Consistent with this result, a secondary analysis with Fisher exact test revealed that the observed difference in implant survival between the treatment groups was indeed statistically significant ( P = 0.019). The most frequent prosthetic complications and maintenance interventions in the mandible were retention adjustments, denture fractures, pressure sores, and matrix exchanges. There was only 1 statistically significant difference between the groups regarding the parameter "fracture of the denture base in the ball attachment area" ( P = 0.007). The results indicate that immediate loading of a single implant in the edentulous mandible reveals inferior survival than that of delayed loading and therefore should be considered only in exceptional cases (German Clinical Trials Register: DRKS00003730). Introduction: To identify potential risk factors for the development of jaw osteoradionecrosis (ORN) after 3D-conformal radiotherapy (3D-CRT) and intensity-modulated radiotherapy (IMRT) among patients with newly diagnosed head and neck cancer.Material and Methods: This study included 776 patients who underwent 3D-CRT or IMRT for head and neck cancer at the Department of Radiotherapy at the University Hospital Halle-Wittenberg between 2003 and 2013. Sex, dental status prior to radiotherapy, tumor site, bone surgery during tumor resection, concomitant chemotherapy, and the development of advanced ORN were documented for each patient. ORN was classified as grade 3, 4, or 5 according to the Radiation Therapy Oncology Group/European Organization for Research and Treatment of Cancer classification or grade 3 or 4 according to the late effects in normal tissues/subjective, objective, management, and analytic scale. The cumulative incidence of ORN was estimated. Cox regression analysis was used to identify prognostic risk factors for the development of ORN.Results: Fifty-one patients developed advanced ORN (relative frequency 6.6 %, cumulative incidence 12.4 %). The highest risk was found in patients who had undergone primary bone surgery during tumor resection (hazard ratio [HR] = 5.87; 95 % confidence interval [CI]: 3.09-11.19) and in patients with tumors located in the oral cavity (HR = 4.69; 95 % CI: 1.33-16.52). Sex, dentition (dentulous vs. edentulous), and chemotherapy had no clinically relevant influence.Discussion and Conclusion: In contrast to most previous studies, we noted a low cumulative incidence of advanced ORN. Patients with tumors located in the oral cavity and those who undergo bone surgery during tumor resection prior to RT may be considered a high-risk group for the development of ORN. Background: Acceptable short-term survival rates (>90 %) of mini-implants (diameter < 3.0 mm) are only documented for mandibular overdentures. Sound data for mini-implants as strategic abutments for a better retention of partial removable dental prosthesis (PRDP) are not available. Methods/design: The purpose of this study is to test the hypothesis that immediately loaded mini-implants show more bone loss and less success than strategic mini-implants with delayed loading. In this four-center (one university hospital, three dental practices in Germany), parallel-group, controlled clinical trial, which is cluster randomized on patient level, a total of 80 partially edentulous patients with unfavourable number and distribution of remaining abutment teeth in at least one jaw will receive supplementary min-implants to stabilize their PRDP. The mini-implant are either immediately loaded after implant placement (test group) or delayed after four months (control group). Follow-up of the patients will be performed for 36 months. The primary outcome is the radiographic bone level changes at implants. The secondary outcome is the implant success as a composite variable. Tertiary outcomes include clinical, subjective (quality of life, satisfaction, chewing ability) and dental or technical complications. Discussion: Strategic implants under an existing PRDP are only documented for standard-diameter implants. Mini-implants could be a minimal invasive and low cost solution for this treatment modality. Objective Evaluation of differences in the clinical performance and crestal bone levels between implants restored with single crowns with platform-matched or platform-switched abutments after 3 years. Material and Methods The study enrolled adult patients missing two or more adjacent teeth in the posterior mandible with natural teeth mesial to the implant site. Randomization followed open-flap implant insertion and the corresponding matching or switching healing abutments placed at surgery. Conventional loading was made with cemented crowns. Clinical follow-up took place annually after loading up to 3 years. Bone level changes were measured in standardized radiographs as the variation in crestal bone from one evaluation to the next. Results Sixty-three patients with a total of 135 implants (66 platform matching, 69 platform switching) were analysed. From surgery to 36 months, mean bone loss was 0.28 ± 0.56 mm for the platform-switching group and 0.68 ± 0.64 mm for the platform-matching group. A statistically significant difference was found between groups ( p = 0.002) with an estimate of 0.39 mm (0.15-0.64, 95% CI) in favour of platform switching. Conclusions After 3 years, platform-switching restorations showed a significant effect in the preservation of marginal bone levels compared to platform-matching restorations. [ABSTRACT FROM AUTHOR]

Background: The aim of this prospective 10-year study of partially edentulous subjects treated for generalized aggressive periodontitis and periodontally healthy subjects was a clinical, microbiologic, and radiographic comparison of teeth and implants and assessment of the implants' success rate. Methods: Five subjects treated for generalized aggressive periodontitis (GAgP) and five periodontally healthy subjects who were orally rehabilitated with osseointegrated implants participated in the study. First, they were examined 2 to 4 weeks before extraction of the non-retainable teeth (baseline) and 3 weeks after insertion of the final abutments. All further examinations were performed during a 3-month recall schedule over a 10-year period. At every session, clinical parameters were recorded, and the composition of the subgingival microflora was determined. Radiographs were taken at baseline after insertion of the superstructure and 1, 3, 5, 8, and 10 years later. Results: Throughout the follow-up period, the peri-implant gingival index of GAgP subjects was significantly higher than in periodontally healthy subjects. There was no difference in plaque index between teeth and implants or between the two groups. The peri-implant probing depths were comparable in the two groups and remained < or =4 mm throughout the follow-up. The probing depth was significantly higher around the teeth of the GAgP subjects compared to periodontally healthy subjects. Implants of GAgP subjects showed a significantly higher attachment loss (Ø 2.4 mm). The attachment level at teeth and implants of the periodontally healthy subjects and at teeth of the GAgP subjects was almost unchanged. Microbiologically, GAgP subjects had fewer cocci and more motile rods and filaments at teeth and implants than periodontally healthy subjects. GAgP subjects showed significantly more peri-implant bone loss in the first year (Ø 2.07 mm) and in the subsequent 9 years (total 1.3 mm). Bone loss at teeth also was significantly higher at baseline (Ø 26.39%) and in the following years (total 9.3%). Implant survival rates were 100% in periodontally healthy subjects versus 83.33% in GAgP subjects. Conclusions: This 10-year study showed that partially edentulous subjects treated for GAgP can be rehabilitated successfully with osseointegrated implants.
