Background: Some sensitive pollen-allergic patients may experience perennial symptoms, and for these patients it is vitally important to know the concentration of grass-pollen allergens in their homes. The main objective of this study was to detect predictors associated with high grass allergen concentration in settled house dust during and outside the grass pollination season. Methods: We detected group 5 grass-pollen allergens in dust samples from randomly selected homes of three counties in the region of Sachsen-Anhalt: Hettstedt (n=183), Zerbst (n=152), and Bitterfeld (n=119). Dust samples were collected from the floors of living rooms (LR) and children's rooms (CR) from April 1996 to March 1998. Results: Phl p 5 was detected in settled house dust in 91% of the homes. The range was between undetectable (<30 ng/m2) and 4300 ng Phl p 5/m2 sampling area during grass pollination, and 1300 ng Phl p 5/m2 outside the pollination season, respectively. We observed a partially higher concentration of indoor allergenic activity several weeks after the grass-pollen peak. A positive association was found between Phl p 5 levels and total dust amounts throughout the year. Thus, we assume that grass allergens accumulate in settled house dust. During pollination, the Phl p 5 levels are 1.8 times higher in homes where vacuuming of carpets takes place only once a week or less than in those vacuumed daily. Furthermore, higher concentrations of Phl p 5 in settled floor dust can be expected in homes located within suburban areas (mean ratio [MR] 1.74) and prefabricated slab buildings (MR=1.54). Conclusions: Quite high levels of Phl p 5 accumulate in settled house dust even during periods when no grass pollen is present in the atmosphere. The removal of dust by frequent cleaning reduces the indoor exposure to grass allergens. Such information is essential to train patients in allergen avoidance. [ABSTRACT FROM AUTHOR]

Objective: Gynecological sarcomas account for 3% of all gynecological malignancies and are associated with a poor prognosis. Due to the rarity and heterogeneity of gynecological sarcomas there is still no consensus on optimal therapeutic strategies. This study's objective was to describe the treatment strategies used in patients with gynecological sarcomas in the primary course of disease., Methods: The German prospective registry for gynecological sarcoma (REGSA) is the largest registry for gynecological sarcomas in Germany, Austria and Switzerland. Primary inclusion criteria for REGSA are histological diagnosis of sarcoma of the female genital tract, sarcoma of the breast or uterine smooth muscle tumors of uncertain malignant potential (STUMP). We evaluated data of the REGSA registry on therapeutic strategies used for primary treatment from August 2015 to February 2021., Results: A total of 723 patients from 120 centers were included. Data on therapeutic strategies for primary treatment were available in 605 cases. Overall, 580 (95.9%) patients underwent primary surgery, 472 (81.4%) of whom underwent only hysterectomy. Morcellation was reported in 11.4% (n=54) of all hysterectomies. A total of 42.8% (n=202) had no further surgical interventions, whereas an additional salpingo-ophorectomy was performed in 54% (n=255) of patients. An additional lymphadenectomy was performed in 12.7% (n=60), an omentectomy in 9.5% (n=45) and intestinal resection in 6.1% (n=29) of all patients. Among 448 patients with available information, 21.4% (n=96) received chemo- or targeted therapies, more commonly as single-agent treatment than as drug combinations. Information about anti-hormonal treatment was available for 423 patients, among which 42 (9.9%) received anti-hormonal treatment, 23 (54.8%) of whom with low-grade endometrial stroma sarcomas. For radiotherapy, data of 437 patients were available, among which 29 (6.6%) patients underwent radiotherapy., Conclusion: Our study showed that treatment of patients with gynecologic sarcomas is heterogeneous. Objectives: The aim of the current study was to evaluate the prevalence and the impact of sexual dysfunction, sexual distress and interpersonal relationships in patients with endometriosis., Study Design: A questionnaire-based multicentre cohort study was conducted in eight tertiary referral centres in Austria and Germany. One hundred and twenty-five patients with histologically proven endometriosis and dyspareunia were included. The Female Sexual Function Index and the Female Sexual Distress Scale were used to screen women's sexuality. Additionally, we evaluated psychological parameters and pain intensity during/after sexual intercourse via a self-administered questionnaire., Results: Female sexual distress and sexual dysfunction were observed in 97/125 and 40/125 patients. Statistically significant correlations were found between sexual dysfunction and pain intensity during/after sexual intercourse (p<0.01/p<0.01), a lower number of episodes of sexual intercourse per month (p<0.01), greater feelings of guilt towards the partner (p<0.01) and fewer feelings of femininity (p<0.01). Thirty-eight out of 125 women agreed that the primary motivation for sexual intercourse was to conceive and nearly half of women (46%) included stated that satisfying the partner acted as primary motivation for sexual contact., Conclusion: Overall, our findings demonstrate that dyspareunia as a common complaint in patients with endometriosis causes a severe impairment of sexual function, relationship and psychological wellbeing., (Copyright © 2013 Elsevier Ireland Ltd. Sustainable data management in biomedical research requires documentation of metadata for all experiments and results. Scientists usually document research data and metadata in laboratory paper notebooks. An electronic laboratory notebook (ELN) can keep metadata linked to research data resulting in a better understanding of the research results, meaning a scientific benefit [1]. Besides other challenges [2], the biggest hurdles for introducing an ELN seem to be usability, file formats, and data entry mechanisms [3] and that many ELNs are assigned to specific research fields such as biology, chemistry, or physics [4]. We aimed to identify requirements for the introduction of ELN software in a biomedical collaborative research center [5] consisting of different scientific fields and to find software fulfilling most of these requirements.

Objective: To analyze the expression of estrogen receptors α and β as well as their target genes implicated in proliferation, c-myc, cyclin D1, and GREB1, in the endometrium of women with or without endometriosis., Design: Expression analysis in human tissue., Setting: University hospitals and a clinic., Patient(s): Ninety-one premenopausal women (59 patients with endometriosis and 32 controls) undergoing laparoscopic surgery., Intervention(s): Biopsies were obtained at time of surgery, performed during the proliferative phase of the cycle., Main Outcome Measure(s): Estrogen receptors α and β as well as c-myc, cyclin D1, and GREB1 mRNA expression levels were determined by quantitative reverse transcriptase-polymerase chain reaction. Tissue localization of these estrogen-regulated genes was analyzed by immunohistochemistry., Result(s): Estrogen receptors α and β as well as c-myc, cyclin D1, and GREB1 mRNA expression levels were increased in ectopic tissue in comparison with both normal and eutopic endometrium. Estrogen receptor mRNA levels also were upregulated in the eutopic peritoneal tissue of patients with endometriosis. Cyclin D1 and GREB1 expression was augmented in eutopic endometrium. c-myc, cyclin D1, and GREB1 proteins exhibited a nuclear localization in ectopic endometrial tissue., Conclusion(s): This constitutes the first report of increased expression of GREB1, as well as cyclin D1 and c-myc, in peritoneal endometriotic lesions, implicating these proteins in estrogen-dependent growth in this context., (Copyright © 2012 American Society for Reproductive Medicine. Purpose: To investigate the efficacy and safety of dienogest as a long-term treatment in endometriosis, with follow-up after treatment discontinuation. The study included women with endometriosis, who had previously completed a 12-week, placebo-controlled study of dienogest, who participated in an open-label extension study for up to 53 weeks. Thereafter, a patient subgroup was evaluated in a 24-week follow-up after treatment discontinuation., Methods: A multicenter study performed in Germany, Italy and Ukraine. Women with endometriosis were enrolled at completion of the placebo-controlled study (n = 168). All women received dienogest (2 mg once daily, orally) and changes in pelvic pain (on a visual analog scale), bleeding pattern, adverse events and laboratory parameters were evaluated during and after treatment., Results: The completion rate among women who entered the open-label extension study was 90.5% (n = 152). A significant decrease in pelvic pain was shown during continued dienogest treatment (P < 0.001). The mean frequency and intensity of bleeding progressively decreased. Adverse events, rated generally mild or moderate, led to withdrawal in four patients (2.4%). No clinically relevant changes in laboratory parameters were observed. During treatment-free follow-up (n = 34), the reduction in pelvic pain persisted, while bleeding frequency and intensity returned to normal patterns., Conclusions: Long-term dienogest showed a favorable efficacy and safety profile, with progressive decreases in pain and bleeding irregularities during continued treatment; the decrease of pelvic pain persisted for at least 24 weeks after treatment cessation.

Background: Is it really true, that prematures have an accelerated maturation, are very intelligent and--with or without ectopic macula--get as myopic as Annette von Droste-Hülshoff was? To find a time schedule in prevention or early detection of retinopathy of prematurity (ROP) and also to detect risk factors, we looked for critical phases in the development of premature babies in analogy to those found in the development of other mammals., Patients and Methods: We examined the data of 294 children (1981-84) retrospectively and then prospectively those of 100 children from 1984 - 93 and of further 1999 babies since then. All findings were noted in the International Committee for Retinopathy of Prematures scheme (ICROP 1984). We elaborated a time table which shows in parallel the actual dated and the age of the baby in gestational months, duration of oxygen application, birth weight, safety index s of the ancient Körner-Bossi and known risk factors, recently the blood serum glucose levels. In this so-called prospective card we noted the time table of developing ROP in 31 children. Furthermore, we chose matched pairs for these ROP-babies out of the data of 1200 healthy babies and evaluated risk factors such as days of oxygen supply and blood glucose levels. Finally, we transformed our data into an excel data base and calculated the economic advantages of examinations by our method., Results: After an adaptation to recent data we showed the epidemiology, therapeutic attempts and a critical review of so-called risk factors. Risk babies had a Körner-Bossi index of less than 1.0; this corresponds to a birth weight of less than 1000 g, a gestational age of less than 29 weeks, and oxygen application for more than 3 days. During the last 15 years, age and weight of the prematures decreased steadily. The onset of ROP occurred always between the 35th and the 40th gestational week. In spontaneous regression, cicatricional stages mostly started in the 41st week., Conclusion: In order to win time for the examinator and to avoid stress for the baby, the prospective card proved very useful. It was helpful also in medico-legal discussions. The prospective card made it evident that the babies pass critical maturation stages during all their development, before as well as after birth. In screening premature babies, this critical phase requires to be respected.
