Purpose: Chronic rhinosinusitis (CRS) is a common condition associated with a significant reduction of the health-related quality of life. One of the most widely used assessment tools in CRS is the disease-specific, health-related questionnaire SNOT-22. The aim of this study was to translate and validate the SNOT-22 into the German language. Methods: The questionnaire was translated using the forward–backward translation technique. After the translation its reliability, validity, and sensitivity were evaluated. For this purpose, the questionnaire was completed by patients diagnosed with CRS before, 3 months and 1 year after endoscopic sinus surgery and by healthy individuals as controls at three university hospitals in Germany. The individual scores of the questionnaire before surgery was correlated with the Lund–Mackay score as well as a global disease-specific question. Results: A total of 139 CRS patients and 31 healthy individuals participated in the study. Internal consistency at all timepoints was very good, with Cronbach's alpha scores of 0.897, 0.941, and 0.945. The questionnaire was able to discriminate between CRS patients and control subjects (p < 0.0001) and scores improved significantly 3 month and 1 year after sinus surgery (p < 0.0001), indicating a good test–retest reliability, validity, and responsiveness. A significant correlation to the single global disease-specific question could be found (p < 0.0001), but no correlation with the Lund–Mackay score. Conclusion: The German Version of the SNOT-22 is a reliable, valid, and sensitive instrument for measuring health-related quality of life in patients with CRS. Introduction: Di(2-ethylhexyl)phthalate (DEHP) is suspected to be toxic for several reasons. During contact with a lipophilic medium, DEHP leaks from polyvinylchloride (PVC), but its influence on inflammatory reactions remains unknown. We examined specific DEHP leaching out of different tubing types, the possibly modulated liberation of proinflammatory cytokines and the induction of adhesion molecule expression in primary endothelial cells.Materials and Methods: Blood samples were circulated in traditional PVC, nodioctyl phthalate (DOP) PVC and heparin-coated PVC tubing within a Chandler loop model. The blood was tested for the concentration of DEHP and its active metabolites as well as the liberation of the proinflammatory cytokines TNFα and IL1ß. Furthermore, we exposed human endothelial cells to circulated blood and analysed them for the expression of the adhesion molecules ICAM-1, VCAM-1 and E-selectin.Results: In contrast to the other tubing, PVC tubing showed significantly elevated DEHP levels, but no alteration was observed concerning a potential up-regulation of the cytokines or activation of the endothelial adhesion molecule receptors.Conclusions: Our data conclude that there is no correlation between DEHP leaching and the inflammatory response after ECC support, but this study showed that even DEHP-free material is leaching DEHP and its toxic metabolites. Chronic obstructive pulmonary disease (COPD) is one of the most important causes of morbidity and mortality in the world. The disease is often aggravated by periods of increased symptoms requiring medical attention. Among the possible triggers for these exacerbations, meteorological factors are under consideration. The objective of this study was to assess the influence of various meteorological factors on the health status of patients with COPD. For this purpose, the daily number of ambulatory care visits due to COPD was analysed in Bavaria, Germany, for the years 2006 and 2007. The meteorological factors were provided by the model at the European Centre for Medium Range Weather Forecast (ECMWF). For the multivariate analysis, a generalised linear model was used. In Bavaria, an increase of 1% of daily consultations (about 103 visits per day) was found to be associated with a change of 0.72 K temperature, 209.55 of log air surface pressure in Pa, and a decrease of 1% of daily consultations with 1,453,763 Ws m of solar radiation. There also seem to be regional differences between north and south Bavaria; for instance, the effect of wind speed and specific humidity with a lag of 1 day were only significant in the north. This study could contribute to a tool for the prevention of exacerbations. It also serves as a model for the further evaluation of the impact of meteorological factors on health, and could easily be applied to other diseases or other regions. [ABSTRACT FROM AUTHOR]

This article studies farm families between tradition and modernity. The topic of tradition and modernity as a structural problem in the organization of society is still in its infancy where the theory construction is concerned. It is generally accepted that Western European agriculture is a social sphere in which traditional elements are particularly tenacious. The article intends to investigate this question by examining four farms in West Germany. It will be discussed within the theoretical framework and refers specifically to cases in which the objective problem of action, its origin and the actual situation are considered. That the agricultural action problem is in the strict sense a social problem is evident on at least four levels structured according to their own logic. First, the general Western rationalization process with its specific direction and the general structure of society and development of the economy. Second, the particular features of the region and the agricultural landscape with regard to that natural and economic area, the local social structure and traditional patterns. Third, the history of the farm, the actual farm organization, the family and its particular life history. And fourth, the subjective action orientation within the structural framework and against the background of individual life history and education. The research approach to family and farm naturally results in a concentration on levels 3 and 4.

Abstract: The FOPI detector at the heavy-ion synchrotron SIS-18 at GSI in Darmstadt has upgraded part of its time-of-flight (ToF) system by adding a new sub-detector shell (called the MMRPC Barrel) that is made out of Multi-strip Multi-gap Resistive-Plate Counters (MMRPCs). The MMRPC Barrel has an active area of 5m2 covered by 2400 individual anode strips which are read out at both ends by custom-designed electronics . With these multi-strip anodes we have obtained an enhanced detector granularity with 95% efficiency and a ToF resolution better than 90ps which increases the identification limit for charged kaons up to laboratory momenta of at least 1GeV/c. In this paper we report on the design, construction and operational characteristics of the MMRPC Barrel and describe its performance during the first experiments. [Copyright &y& Elsevier]
