This study evaluated the association between dietary patterns, gas-related symptoms (GRS) and their impact on quality of life (QoL) in a representative sample (n 936) of the French adult population. During the 2018–2019 'Comportements et Consommations Alimentaires en France' survey (Behaviors and Food Consumption in France), online evaluation of GRS in adult participants was performed using the validated Intestinal Gas Questionnaire (IGQ), which captures the perception of GRS and their impact on QoL via six symptom dimensions scores (range 0–100; 100 = worse) and a global score (mean of the sum of the six symptom dimensions scores). Socio-demographics, lifestyle parameters and dietary habits (7-d e-food diary) were also collected online. Quality of diet was determined using the Nutrient-Rich Food 9.3 (NRF9.3) score (range 0–900; 900 = best). Univariate and multivariate linear regression models were applied to identify factors associated with IGQ global score. K-means was used to identify clusters of subjects based on their dietary records. Data from 936 adults who completed both the IGQ and the food diary showed a mean IGQ global score of 11·9 (sd 11·2). Younger age and female sex were associated with a higher IGQ global score. Only 7 % of subjects reported no symptom at all and nearly 30 % of study participants reported a high impact of GRS on their QoL. Two dietary clusters were identified: cluster 1, characterised by a higher consumption of fruits and vegetables, lower sugars intake and higher NRF9.3 score and cluster 2, characterised by higher intake of sugars, lower intake in dietary fibres and lower NRF9.3 score. The IGQ global score was lower in cluster 1 and higher in cluster 2 v. the total sample average (P < 0·001). The prevalence of GRS in the French adult population is high and is associated with impaired QoL and dietary patterns. Alpine glaciers are shrinking and rapidly loosing mass in a warming climate. Glacier modeling is required to assess the future consequences of these retreats on water resources, the hydropower industry and risk management. However, the performance of such ice flow modeling is generally difficult to evaluate because of the lack of long-term glaciological observations. Here, we assess the performance of the Elmer/Ice full Stokes ice flow model using the long dataset of mass balance, thickness change, ice flow velocity and snout fluctuation measurements obtained between 1979 and 2015 on the Mer de Glace glacier, France. Ice flow modeling results are compared in detail to comprehensive glaciological observations over 4 decades including both a period of glacier expansion preceding a long period of decay. To our knowledge, a comparison to data at this detail is unprecedented. We found that the model accurately reconstructs the velocity, elevation and length variations of this glacier despite some discrepancies that remain unexplained. The calibrated and validated model was then applied to simulate the future evolution of Mer de Glace from 2015 to 2050 using 26 different climate scenarios. Depending on the climate scenarios, the largest glacier in France, with a length of 20 km , could retreat by 2 to 6 km over the next 3 decades. [ABSTRACT FROM AUTHOR]

Salmonella enterica subsp. enterica serovar Derby is one of the most frequent causes of gastroenteritis in humans. In Europe, this pathogen is one of the top five most commonly reported serovars in human cases. In France, S. Derby has been among the ten most frequently isolated serovars in humans since the year 2000. The main animal hosts of this serovar are pigs and poultry, and white meat is the main source of human contamination. We have previously shown that this serovar is polyphyletic and that three distinct genetic lineages of S. Derby cohabit in France. Two of them are associated with pork and one with poultry. In this study, we conducted a source attribution study based on single nucleotide polymorphism analysis of a large collection of 440 S. Derby human and non-human isolates collected in 2014–2015, to determine the contribution of each lineage to human contamination. In France, the two lineages associated with pork strains, and corresponding to the multilocus sequence typing (MLST) profiles ST39-ST40 and ST682 were responsible for 94% of human contaminations. Interestingly, the ST40 profile is responsible for the majority of human cases (71%). An analysis of epidemiologic data and the structure of the pork sector in France allowed us to explain the spread and the sporadic pattern of human cases that occurred in the studied period. The investigation of foodborne outbreaks (FBOs) from genomic data typically relies on inspecting the relatedness of samples through a phylogenomic tree computed on either SNPs, genes, kmers, or alleles (i.e., cgMLST and wgMLST). The phylogenomic reconstruction is often time-consuming, computation-intensive and depends on hidden assumptions, pipelines implementation and their parameterization. In the context of FBO investigations, robust links between isolates are required in a timely manner to trigger appropriate management actions. Here, we propose a non-parametric statistical method to assert the relatedness of samples (i.e., outbreak cases) or whether to reject them (i.e., non-outbreak cases). With typical computation running within minutes on a desktop computer, we benchmarked the ability of three non-parametric statistical tests (i.e., Wilcoxon rank-sum, Kolmogorov–Smirnov and Kruskal–Wallis) on six different genomic features (i.e., SNPs, SNPs excluding recombination events, genes, kmers, cgMLST alleles, and wgMLST alleles) to discriminate outbreak cases (i.e., positive control: C+) from non-outbreak cases (i.e., negative control: C−). We leveraged four well-characterized and retrospectively investigated FBOs of Salmonella Typhimurium and its monophasic variant S. 1,4,[5],12:i:- from France, setting positive and negative controls in all the assays. We show that the approaches relying on pairwise SNP differences distinguished all four considered outbreaks in contrast to the other tested genomic features (i.e., genes, kmers, cgMLST alleles, and wgMLST alleles). The freely available non-parametric method written in R has been designed to be independent of both the phylogenomic reconstruction and the detection methods of genomic features (i.e., SNPs, genes, kmers, or alleles), making it widely and easily usable to anybody working on genomic data from suspected samples. This study focuses on simulations of the seasonal and annual surface mass balance (SMB) of Saint-Sorlin Glacier (French Alps) for the period 1996-2015 using the detailed SURFEX/ISBA-Crocus snowpack model. The model is forced by SAFRAN meteorological reanalysis data, adjusted with automatic weather station (AWS) measurements to ensure that simulations of all the energy balance components, in particular turbulent fluxes, are accurately represented with respect to the measured energy balance. Results indicate good model performance for the simulation of summer SMB when using meteorological forcing adjusted with in situ measurements. Model performance however strongly decreases without in situ meteorological measurements. The sensitivity of the model to meteorological forcing indicates a strong sensitivity to wind speed, higher than the sensitivity to ice albedo. Compared to an empirical approach, the model exhibited better performance for simulations of snow and firn melting in the accumulation area and similar performance in the ablation area when forced with meteorological data adjusted with nearby AWS measurements. When such measurements were not available close to the glacier, the empirical model performed better. Our results suggest that simulations of the evolution of future mass balance using an energy balance model require very accurate meteorological data. Given the uncertainties in the temporal evolution of the relevant meteorological variables and glacier surface properties in the future, empirical approaches based on temperature and precipitation could be more appropriate for simulations of glaciers in the future. Salmonella enterica subspecies enterica serovar Dublin (S. Dublin) figures among the most frequently isolated Salmonella strains in humans in France. This serovar may affect production and animal health mainly in cattle herds with corresponding high economic losses. Given that the current gold standard method, pulsed-field gel electrophoresis (PFGE), provides insufficient discrimination for epidemiological investigations, we propose a standard operating procedure in this study for multiple-locus variable number tandem repeat analysis (MLVA) of S. Dublin, suitable for inter-laboratory surveillance. An in silico analysis on the genome of S. Dublin strains CT_02021853 was performed to identify appropriate microsatellite regions. Of 21 VNTR loci screened, six were selected and 401 epidemiologically unrelated and related strains, isolated from humans, food and animals were analyzed to assess performance criteria such as typeability, discriminatory power and epidemiological concordance. The MLVA scheme developed was applied to an outbreak involving Saint-Nectaire cheese for which investigations were conducted in France in 2012, making it possible to discriminate between epidemiologically related strains and sporadic case strains, while PFGE assigned only a single profile. The six loci selected were sequenced on a large set of strains to determine the sequence of the repeated units and flanking regions, and their stability was evaluated in vivo through the analysis of the strains investigated from humans, food and the farm environment during the outbreak. The six VNTR selected were found to be stable and the discriminatory power of the MLVA method developed was calculated to be 0.954 compared with that for PFGE, which was only 0.625. Twenty-four reference strains were selected from the 401 examined strains in order to represent most of the allele diversity observed for each locus. This reference set can be used to harmonize MLVA results and allow data exchange between laboratories. Background: During 2008-12, France and Europe experienced large measles outbreaks, involving also healthcare workers (HCW). We aimed to estimate the vaccination coverage (VC) of measles among medical residents of the University of Aix/Marseille, in South-Eastern France. Methods: In March 2013, we conducted a cross-sectional study among all medical residents of the Medical Faculty of Aix/Marseille. We used a self-administered questionnaire to collect information on self-reported VC and reasons for vaccination and non-vaccination. We compared proportions, using the chi-squared test and prevalence ratios (PRs) with 95% confidence intervals (95% CIs). Results: Of 1152 eligible residents, 703 (61%) participated in the study and 95 (14%; 95% CI: 12-17%) reported having had measles in the past. Of all participants, 613 (93%; 95% CI: 91-95%) reported having been vaccinated against measles and 389 (76%; 95% CI: 73-80%) received two doses. Only 268 (38%) reported having visited an occupational health physician. Vaccinated individuals were more likely to report easy access to vaccination as the main motivation for measles vaccination, compared with unvaccinated residents (435; 71% and 21; 45%; P < 0.001, respectively). Conclusions: VC among the medical residents of the University of Aix/Marseille was well below the recommended 95% coverage for two doses of measles vaccination. The majority of the study participants had not visited an occupational health doctor. Lack of easy access seems to represent major barriers to measles vaccination. A large set of ice ablation data from a glacier in the French Alps is used to investigate the sensitivity of ice melting to solar radiation and temperature. The data come from 7 years of observations on a small network of 16 stakes set up on Glacier de Saint-Sorlin. The high spatial variability of ice ablation is shown to be strongly dependent on potential solar radiation. On the other hand, temporal variations are highly correlated with air temperatures measured both at a nearby and at a remote meteorological station, but poorly correlated with incoming shortwave radiation measured at an automatic weather station located close to the glacier terminus. Spatial variations of ice ablation therefore appear to be mainly driven by potential solar radiation, while temporal variations are driven by temperature. This result suggests that minimizing the influence of temperature in an enhanced temperature-index melt model may compromise the model's ability to simulate interannual changes in melt. These new results may help to improve the calibration, and thus the performance, of these empirical melt models used for long-term simulations of glacier mass balances. [ABSTRACT FROM AUTHOR]

Objective Giant cell arteritis (GCA) is a large-vessel vasculitis of unknown origin. Recent findings indicate that at least 2 separate lineages of CD4+ T cells, Th1 and Th17 cells, participate in vascular inflammation. The pathways driving these T cell differentiations are incompletely understood, but may provide novel therapeutic targets. This study was undertaken to identify cytokines involved in the pathogenesis of GCA. Methods Thirty GCA patients fulfilling the American College of Rheumatology criteria, with active disease or disease in remission, and 30 age-matched controls were included. Levels of 27 cytokines were determined in culture supernatants, and flow cytometric analysis of peripheral blood mononuclear cells (PBMCs) and immunohistochemical analysis of temporal artery samples were performed. Results Multiparametric analysis of cytokines produced by PBMCs associated with GCA disease activity identified a signature involving interleukin-2 receptor (IL-2R), IL-12, interferon-γ (IFNγ), IL-17A, IL-21, and granulocyte-macrophage colony-stimulating factor (GM-CSF). An expansion of Th1 and Th17 cells and a decrease in Treg cells were observed in the peripheral blood of patients with active GCA. An expansion of IL-21-producing CD4+ T cells was also observed in patients with active GCA and correlated positively with Th17 and Th1 cell expansion. Immunohistochemical analysis revealed IFNγ, IL-17A, and IL-21 expression within inflammatory infiltrates. Stimulation of purified CD4+ T cells with IL-21 increased Th1 and Th17 cell frequencies and decreased FoxP3 expression. In contrast, blockade of IL-21 using IL-21R-Fc markedly decreased the production of IL-17A and IFNγ and increased FoxP3 expression. Conclusion Our findings indicate that IL-21 plays a critical role in modulating Th1 and Th17 responses and Treg cells in GCA, and might represent a potential target for novel therapy. The article presents a study involving the variational assimilation of glacial albedo in a snowpack model and the simulation of the surface-energy balance (SEB) of an alpine glacier. It mentions that data assimilation is starting to be considered for use in glaciology and adds that the assimilation glacial albedo in a snowpack model can lead to an improved simulation of glacier mass balances. Sites used in the study are the Col de Porte test site in France and Glacier de Saint-Sorlin in the French Alps. The assimilation approach is recommended for use on a large set of glaciers when seeking the estimated spatial distribution of temperate glacial mass.

The article presents a study which examines the temporal and spatial variations of surface albedo on Saint Sorlin Glacier in French Alps, France. The study develops a method of retrieving the albedo of frozen surfaces using terrestrial photography during the summers 2008 and 2009. The results of the study reveal the factors contributing to the variability of the albedo of glaciers including size and shape of the ice grains, surface roughness and content of impurities.

The article presents a re-analysis of the cause of the outburst flood from Glacier Tête Rousse that devastated the village of Saint-Gervais-Le Fayet, French Alps in 1892. It offers historical observations and investigations about the disaster with detailed descriptions of the glacier surface and the volume of its upper cavity. Results of the new analysis revealed that the meltwater reservoir in the upper cavity could not come from an enlarging crevasse and that its origin was more likely a supgraglacial lake.

Repeated dog deaths occurred in 2002, 2003, and 2005 after the animals drank water from the shoreline of the Tarn River in southern France. Signs of intoxication indicated acute poisoning due to a neurotoxin. Floating scum and biofilms covering pebbles were collected in the summers of 2005 and 2006 from six different sites along 30km from the border of this river. The cyanobacterial neurotoxic alkaloid anatoxin-a and/or its methyl homolog, homoanatoxin-a, was detected in the extracts of most samples examined by gas chromatography-mass spectrometry. Fifteen filamentous cyanobacteria of the order Oscillaloriales were isolated and displayed four distinct phenotypes based on morphological characteristics and pigmentation. Three of the phenotypes can be assigned to the genus Oscillatoria or Phormidium, depending on the taxonomic treatises (bacteriological/botanical) employed for identification. The fourth phenotype is typical of the genus Geitlerinema Anagnostidis 1989. Eight strains rendered axenic were analyzed for production of anatoxin-a and homoanatoxin-a, and all strains of Oscillatoria/Phormidium proved to be neurotoxic. The genetic relatedness of the new isolates was evaluated by comparison of the intergenic transcribed spacer sequences with those of six oscillatorian strains from the Pasteur Culture Collection of Cyanobacteria. These analyses showed that the neurotoxic representatives are composed of five different genotypes, three of which correspond to phenotypes isolated in this study. Our findings prove that neurotoxic oscillatorian cyanobacteria exist in the Tarn River and thus were most likely implicated in the reported dog poisonings. Furthermore, they reemphasize the importance of monitoring benthic cyanobacteria in aquatic environments to fully assess the health risks associated with these organisms. [ABSTRACT FROM AUTHOR]

This work presents an extensive evaluation of the Crocus snowpack model over a rugged and highly glacierized mountain catchment (Arve valley, Western Alps, France) from 1989 to 2015. The simulations were compared and evaluated using in-situ point snow depth measurements, in-situ seasonal and annual glacier surface mass balance, snow covered area evolution based on optical satellite imagery at 250 m resolution (MODIS sensor), and the annual equilibrium-line altitude of glaciers, derived from satellite images (Landsat, SPOT, and ASTER). The snowpack simulations were obtained using the Crocus snowpack model driven by the same, originally semi-distributed, meteorological forcing (SAFRAN) reanalysis using the native semi-distributed configuration, but also a fully distributed configuration. The semi-distributed approach addresses land surface simulations for discrete topographic classes characterized by elevation range, aspect, and slope. The distributed approach operates on a 250-m grid, enabling inclusion of terrain shadowing effects, based on the same original meteorological dataset. Despite the fact that the two simulations use the same snowpack model, being potentially subjected to same potential deviation from the parametrization of certain physical processes, the results showed that both approaches accurately reproduced the snowpack distribution over the study period. Slightly (although statistically significantly) better results were obtained by using the distributed approach. The evaluation of the snow cover area with MODIS sensor has shown, on average, a reduction of the Root Mean Squared Error (RMSE) from 15.2% with the semi-distributed approach to 12.6% with the distributed one. Similarly, surface glacier mass balance RMSE decreased from 1.475 m of water equivalent (W.E.) for the semi-distributed simulation to 1.375 m W.E. for the distribution. The improvement, observed with a much higher computational time, does not justify the recommendation of this approach for all applications; however, for simulations that require a precise representation of snowpack distribution, the distributed approach is suggested. Abstract: Lake Bourget (France) provides drinking water and is a place for professional and recreational fishing. Since the mid 1990s, the lake has been exhibiting blooms of the filamentous cyanobacterium Planktothrix rubescens. This species is able to produce microcystin-LR and RR, toxins that contaminate different fish tissues and, if concentrated in the liver, can induce fish mortality. However, data on fish exposure to these toxins in a natural environment are scare and comparisons of spatial distribution between P. rubescens and exploited fish are needed to determine whether these fish avoid or converge in zones affected by the cyanobacteria. From June to November 2009, diurnal data on P. rubescens and whitefish (Coregonus lavaretus) spatial distributions have been monitored by hydroacoustic and BBE probe sampling. For all water samples, intracellular microcystin concentration of P. rubescens was quantified by HPLC/PDA. Furthermore, an arbitrary sample of eight whitefish captured at the moment of highest P. rubescens concentration in the lake were analyzed to investigate both the presence of this cyanobacterium in the gastrointestinal tract by optical microscopy and the possible bioaccumulation of the microcystin in their tissues by liquid chromatography–tandem mass spectrometry. Results show that P. rubescens abundance was at a maximum between the end of July and the beginning of September. During this period, P. rubescens abundance was vertically stratified with a maximum around 14–22m, depending on the sampling station. The horizontal distributions of P. rubescens were heterogeneous at the scale of the lake. Results indicate that the presence of P. rubescens, for the observed cyanobacterial abundance, does not exert a significant pressure on the distribution pattern of the whitefish. Whitefish were present in the same areas as P. rubescens maxima during daytime, and they do not avoid or seek out the zone of high P. rubescens abundance. Filaments of P. rubescens have been observed in intestinal tracts of whitefish and the presence of microcystin-LR has been detected in their intestine and liver. Consequently, because of a direct contact between these organisms, toxins can be incorporated into whitefish by ingestion of P. rubescens filaments, leading to potential adverse effects on the health of this species. • In 2013, the French national agency for public health (Santé publique France) and its regional units performed a prospective study to compare the effectiveness and protection duration of whole-cell versus acellular pertussis vaccines. • Seven investigations of epidemic clusters in a cohort of 305 school children from the 2nd grade to the 6th grade resulted in the identification of 102 clinical cases of pertussis, including 10 cases biologically confirmed by Bordetella pertussis specific PCR. • The risk of pertussis when vaccinated with an acellular vaccine alone was 1.6 (RR = 1.6; 95% CI = 1.1–2.5) times higher than when vaccinated with a whole-cell vaccine or using a combined schedule. • The limited duration of protection conferred by the acellular vaccine is in favor of the introduction of the pertussis booster for 6-year-old children in 2013. We conducted a prospective study in 2013 to compare the whole-cell versus acellular pertussis vaccines effectiveness and duration of protection, following the occurrence of pertussis clusters. During seven school outbreaks, we identified 102 clinical pertussis cases, including 10 cases biologically confirmed by Bordetella pertussis specific PCR, among a cohort of 305 children in 2nd to 6th grade. The risk of pertussis when vaccinated with an acellular vaccine alone was 1.6 (RR = 1.6; 95% CI = 1.1–2.5) times higher than when vaccinated with a whole-cell vaccine or using a combined schedule. The family Kareniaceae is mostly known in France for recurrent blooms of Karenia mikimotoi in the Atlantic, English Channel, and Mediterranean Sea and for the unusual green discoloration in the saltwater lagoon of Diana (Corsica) caused by Karlodinium corsicum in April 1994. In terms of diversity, this taxonomic group was long overlooked owing to the difficult identification of these small unarmored dinoflagellates. In this study, thanks to the molecular characterization performed on single cells from field samples and cultures, twelve taxonomic units were assigned to the known genera Karenia , Karlodinium and Takayama , whereas one could not be affiliated to any described genus. The molecular phylogeny inferred from the D1–D2 region of the LSU rDNA showed that five of them formed a sister taxon of a known species, and could not be identified at species-level, on the basis of molecular analysis only. Among these latter taxa, one Karlodinium which was successfully cultured was investigated by studying the external morphological features (using two procedures for cells fixation), ultrastructure, pigment composition, and haemolytic activity. The results of our analyses corroborate the genetic results in favour of the erection of Karlodinium gentienii sp. nov., which possesses an internal complex system of trichocysts connected to external micro-processes particularly abundant in the epicone, and a peculiar pigment composition. Abstract: Objective: A prospective observation study was undertaken in seven medical centers, in the French region Alpes-Maritime, to assess nail hygiene of healthcare professionals and how often they wear hand jewellery. Method: Seven hundred and six healthcare workers in seven medical centers were interviewed from March to April 2008. Results: Among the 706 professionals, 306 (43%) were wearing one or several pieces of jewellery. The nails of 81 (11.5%) were non-standard according to guidelines. Three hundred and forty-four health care professionals (49%) were wearing one or several pieces of jewellery and/or presented with non-standard nails. In univariate analysis, the wearing of jewellery was linked to the medical centre (P <0.001), to the professional category (p <0.001), to the number of times people washed their hands or used hand gel per day (ABHR) by categories (<10 times per day or ≥10 times per day) (P <0.017). In the multivariate analysis, the risk factors linked to the wearing of jewellery were the medical centers, the professional category, and the age. Conclusion: There are still too many healthcare professionals who do not comply to French recommendations on hand hygiene concerning the wearing of jewellery and nail hygiene standards. Cette étude évalue pour la première fois les associations entre les habitudes alimentaires et les symptômes liés à la production de gaz intestinaux dans un échantillon représentatif de la population française adulte. Habitudes alimentaires : échantillon représentatif de données de consommation d'adultes de 18 ans et plus (n = 936 ; carnet alimentaire électronique de 7 jours) de l'enquête « Comportements et consommation alimentaire en France » CCAF (2019), après exclusion des sous-déclarants. Symptômes digestifs : questionnaire validé de 17 items (IGQ : Intestinal Gas Questionnaire) sur la perception des symptômes liés aux gaz intestinaux et leur impact sur la qualité de vie (score total 0–100 où 100 = impact maximal ; 6 dimensions : ballonnements, flatulences, éructations, mauvaise haleine, borborygmes, évacuation difficile des gaz). Qualité de la diète : score PANDiet (score 0–100 ; 100 = meilleure adhésion aux recommandations nutritionnelles). Statistiques : la méthode K-means appliquée aux quantités consommées de 38 groupes alimentaires a permis de constituer des classes d'individus selon leurs profils alimentaires de manière non supervisée. Les liens entre les profils alimentaires et les scores IGQ dans chaque classe ont été évalués avec la statistique V-test. Le score IGQ global (moyenne ± erreur standard) était de 11,2 ± 0,3 et seulement 7 % des individus ont déclaré être indemnes de tout symptôme gastro-intestinal. Deux classes de consommateurs ont été identifiés. Dans le premier profil alimentaire caractérisé par un apport élevé en glucides simples mais pauvre en fibres, une consommation faible d'aliments bruts et un score PANDiet plus faible (59,5 ± 0,3), le score IGQ global (13,6 ± 0,6) et les dimensions éructations (12,8 ± 0,8), mauvaise haleine (13,2 ± 0,8), borborygmes (14,4 ± 0,9) et ballonnement (14,6 ± 0,8) étaient significativement plus élevés que dans le reste de la cohorte (p < 0,001) avec une surreprésentation de femmes (58,6 %, p < 0,001). Dans le second profil, caractérisé par une consommation importante de fruits et légumes, un apport plus riche en fibres et moins riche en glucides simples, et un score PANDiet plus élevé (62,8 ± 0,4), le score IGQ global (10,2 ± 0,4) ainsi que les dimensions ballonnements (10,8 ± 0,7), borborygmes (9,8 ± 0,7), éructations (8,4 ± 0,6) et mauvaise haleine (8,5 ± 0,6) étaient significativement moins élevés que dans le reste de la cohorte (p < 0,001). Cette étude montre une association entre le profil alimentaire et les symptômes liés aux gaz. Les individus ayant un profil alimentaire pauvre en aliments bruts et en fibres mais plus riche en glucides simples rapportent une fréquence plus élevée des symptômes liés aux gaz et une qualité de vie plus impactée. Streptococcus agalactiae (group B streptococcus (GBS)) is the leading cause of invasive infections among newborns in industrialized countries, with two described syndromes: early-onset disease (EOD) and late-onset disease (LOD). Since the introduction in many countries of intrapartum antibioprophylaxis (IAP), the incidence of EOD has dramatically decreased, whereas that of LOD remains unchanged. We describe the clinical and bacteriological characteristics of 438 GBS neonatal invasive infections notified to the French National Reference Centre for Streptococci in France from 2007 to 2012. Clinical data were retrieved from hospitalization reports or questionnaires. Capsular type, assignment to the hypervirulent clonal complex (CC)17 and antibiotic susceptibility profiles were determined. One hundred and seventy-four (39.7%) and 264 (60.3%) isolates were responsible for EOD, including death in utero, and LOD, respectively. EOD was associated with bacteraemia (n = 103, 61%) and LOD with meningitis (n = 145, 55%). EOD was mainly due to capsular polysaccharide (CPS) III isolates (n = 99, 57%) and CPS Ia isolates (n = 40, 23%), and CPS III isolates were responsible for 80% (n = 211) of LOD cases. CC17 accounted for 80% (n = 121) of CPS III isolates responsible for meningitis (n = 151; total cases of meningitis, 188). Bad outcome risk factors were low gestational age and low birthweight. LOD represents almost 60% of cases of neonatal GBS disease in France and other countries in which IAP has been implemented. Background: The capsular polysaccharide (CPS) is an important virulence factor and a vaccine target of the major neonatal pathogen group B Streptococcus (GBS). Population studies revealed no strong correlation between CPS type and multilocus sequence typing (MLST) cluster, with the remarkable exception of the worldwide spread of hypervirulent GBS CC17, which were all until recently CPS type III., Methods: A total of 965 GBS strains from invasive infection isolated in France were CPS typed and the presence of the CC17-specific surface protein encoding gene hvgA gene was investigated. Three hvgA-positive GBS strains screened were surprisingly CPS type IV and thus further characterized by MLST typing, pulsed-field gel electrophoresis (PFGE), and whole genome sequencing., Results: MLST and PFGE demonstrated a capsular switching from CPS type III to IV within the highly homogeneous GBS CC17. Sequence analysis revealed that this capsular switch was due to the exchange of a 35.5-kb DNA fragment containing the entire cps operon., Conclusions: This work shows that GBS CC17 hypervirulent strains have switched one of their main vaccine targets. Thus, continued surveillance of GBS population remains of the utmost importance during clinical trials of conjugate GBS vaccines.
