Purpose: Children are assumed to acquire orthographic representations during autonomous reading by decoding new written words. The present study investigates how deaf and hard of hearing (DHH) children build new orthographic representations compared to typically hearing (TH) children. Method: Twenty-nine DHH children, from 7.8 to 13.5 years old, with moderate-to-profound hearing loss, matched for reading level and chronological age to TH controls, were exposed to 10 pseudowords (novel words) in written stories. Then, they performed a spelling task and an orthographic recognition task on these new words. Results: In the spelling task, we found no difference in accuracy, but a difference in errors emerged between the two groups: Phonologically plausible errors were less common in DHH children than in TH children. In the recognition task, DHH children were better than TH children at recognizing target pseudowords. Phonological strategies seemed to be used less by DHH than by TH children who very often chose phonological distractors. Conclusions: Both groups created sufficiently detailed orthographic representations to complete the tasks, which support the self-teaching hypothesis. DHH children used phonological information in both tasks but could use more orthographic cues than TH children to build up orthographic representations. Using the combination of a spelling task and a recognition task, as well as analyzing the nature of errors, in this study, provides a methodological implication for further understanding of underlying cognitive processes.

Purpose: To determine changes in the prevalence of zolpidem consumption since the change in the regulations of prescription. Formulations containing zolpidem were subject to the regulations of narcotics by the French decree of April 7, 2017. Methods: Longitudinal cohort study using data from the representative French healthcare database. The main outcome was the prevalence of oral hypnotic drug reimbursement before and after April 2017. The secondary outcome was the change in prescription habits for zolpidem since the decree in long-term users and excessive users. Results: A total of 81,174 individuals had at least one hypnotic drug reimbursement; among, whom 2143 had at least one reimbursement of zolpidem. Before the decree, 26% had at least one reimbursement of zolpidem, whereas it dropped to 18.4% after the decree. Among the 545 long-term users, the reimbursement of zolpidem was discontinued after the decree for 60.4% and 24.2% retained zolpidem as a treatment. The main replacement drug was zopiclone for 6.4% of them. Among the 1598 excessive users, the reimbursement of zolpidem was stopped after the decree for 16.5% and 56.3% retained zolpidem as a treatment. The main replacement drug was zopiclone for 12.1% of them. Conclusions: The French decree had a major impact on the reimbursement of oral zolpidem. Indeed, prescription of the hypnotic was discontinued for half of the long-term users of zolpidem, and just over one-sixth of the excessive users discontinued the prescription of zolpidem after the decree. Abstract: Understanding long-term variability in the frequency of intense storm activity is important for assessing whether changes are controlled by climate evolution. Understanding this variability is also important for predicting present and future community vulnerability and economic loss. Our ability to make these assessments has been limited by the short (less than 50years) instrument record of storm activity. Storm-induced deposits preserved in the sediments of coastal lagoons offer the opportunity to study the links between climatic conditions and storm activity on longer timescales. In this study, we present a record of these extreme climatic events that have occurred in the French Mediterrannean coast over the past 1500years. The identification of these extreme events is based on the analysis of sediment cores from Gulf of Aigues-Mortes lagoons that contain a specific sedimentary and geochemical signature associated with intense storms. Overwash deposits do not show any evidence of intense storm landfalls in the region for several hundred years prior to the late 17th century A.D. The apparent increase in intense storms around 250years ago occurs during the latter half of the Little Ice Age, a time of lower continental surface temperatures. Comparison of the sediment record with palaeoclimate records indicates that this variability was probably modulated by atmospheric dynamics. The apparent increase of the superstorm activity during the latter half of the Little Ice Age was probably due to the thermal gradient increase leading to enhanced lower tropospheric baroclinicity over a large Central Atlantic/European domain and leading to a modification of the occurrence of extreme wind events along the French Mediterranean coast. A complete understanding of the relationship between climate fluctuations, storm activity, and the coastal response will be crucial to predicting the impacts of future climate change. Abstract: Research with immigrant youth has shown that they have a variety of orientations to their acculturation process. These different paths (“acculturation strategies”) have been described in terms of assimilation, integration, marginalisation and separation (AIMS). This research has also shown that there are variable adaptations or outcomes to acculturation. Of greatest importance is the finding that these variations in adaptation are usually related to the acculturation strategies that youth use. Generally, those who involve themselves in both their heritage culture and that of the national society (by way of integration) have the most positive psychological well-being, and are most adjusted in school and in the community; in contrast, those who are minimally involved with either culture (the marginalisation course), are least well-adapted; and those who are primarily oriented towards one or the other culture (assimilation or separation) generally fall in between these two adaptation poles. The present study examines the generality of this pattern by comparing samples from two societies that have very different policies and practices with respect immigration and acculturation: Canada and France. We measured acculturation attitudes, identity and behaviours along the two basic dimensions of cultural maintenance and social contact, and then cross-tabulated them, creating the four acculturation orientations. In both societies the usual pattern is replicated: youth who prefer integration have higher self-esteem scores than those who are marginalised; youth preferring assimilation and separation fall in between. This pattern was stronger in Canada than in France, a finding that is interpreted in terms of the differential social context faced by youth in the two societies, particularly the differing national policies and youths’ experience of discrimination. The article presents information about the book workshop held on January 26, 2009 in Paris, France, sponsored by the Bibliothèque nationale de France. The theme was called "Crise de la lecture?" and focused on determining whether there is a reading crisis. The workshop featured several speakers including president of the Association "Lire et faire lire," Gérard David, sociologist Olivier Donnat, and historian Jean-Yves Mollier.

A sediment core encompassing 3500 years of continuous sedimentation has been collected from a coastal lagoon located on the southwestern French Mediterranean coast. Lead concentrations and stable isotopes show that the sediments have recorded the three major periods of Pb pollution: the Etruscan-Greek-Roman period (650 BC to AD 50), the medieval period (AD 650 to AD 1450), and the modern period (from around AD 1850 to the present). These periods were separated by low pollution periods during the Dark Ages (between AD 50 and 650) and during the 16th century. From the end of the 19th century to the 1960s, Pb pollution increased exponentially. Coal combustion was the major source of Pb in the lagoon in the second half of the 20th century. Both the decrease in coal consumption and the ban on leaded gasoline resulted in a decrease in Pb pollution by a factor of 1.5 between 1973 and 1995. From 1991, sewage treatment plants and incinerators could be the major source of Pb. The average baseline Hg concentration from 1525 BC to AD 900 was 0.017 ± 0.003 μg g-1 (n = 54). The Hg concentrations profile shows three major peaks: in AD 1150, AD 1660, and AD 1969, with the concentrations being respectively 8, 5, and 34 times higher than the baseline levels. The medieval peak (AD 1150) is attributed the medical use of Hg in the town of Montpellier and/or the burning of soil and vegetation. Noticeable Hg pollution was also detected during the 17th century in relation to gold and silver amalgamation in Europe. From the end of the 19th century, Hg concentrations increased exponentially until 1969. This modern pollution is attributed to the burning of coal. [ABSTRACT FROM AUTHOR]

Purpose: In the general context of medical judicialization, spine surgeons are impacted by the part that medical responsibility and the risk of malpractice play in their actions and decisions. Our aim was to evaluate possible shifts in practices among private neurosurgeons who are highly exposed to this judicial risk and detect alterations in their pleasure in exercising their profession. We present the first national survey on French physicians' perception of surgical judicialization and consequences on their practice., Methods: An online survey was submitted to the 121 members of the French Society of Private Neurosurgery, who represent 29.1% of the total number of spine surgeons and perform 36.0% of the national total spine surgery activity. The French law (no-fault out-of-court scheme) significantly impacts these surgeons in the event of litigation., Results: A total of 78 surveys were completed (64.5% response rate): 89.7% of respondents experienced alteration of doctor-patient relationship related to judicialization and 60.2% had already refused to perform risky surgeries. Fear of being sued added negative pressure during surgery for 55.1% of respondents and 37.2% of them had already considered stopping their practice because of this litigation context., Conclusion: The increasing impact of medical liability is prompting practitioners to change their practice and perceptions. The doctor-patient relationship appears to be altered, negative pressure is placed on physicians and defensively, some neurosurgeons may refuse high-risk patients and procedures. Aims of the Study: This study assesses clinical interventions by pharmacists prospectively collected from medical and surgical wards, notably the acceptance of interventions, computerised physician order entry (CPOE)related problems, the potential impact of interventions on patient safety evaluated by a multidisciplinary committee, and their evolution over the 10 years since a first assessment., Methods: A prospective observational study covering 13 months was conducted in a French teaching hospital with a patient information system that integrates an electronic health record (EHR) with a CPOE. Interventions by pharmacists were prospectively recorded using CPOE. All interventions were reviewed by two pharmacists. We assessed the interventions, the possible implications of the CPOE in prescribing errors, and the acceptance of interventions by physicians. A committee reviewed the potential clinical impact for patients. The results were compared with the same outcomes collected 10 years ago in the same hospital., Results: A total of 2141 interventions by pharmacists were reviewed. Among them, 1589 (74.1%) were accepted by physicians. Regarding the potential clinical impact, a total of 1136 (53%) interventions concerned prescriptions that were potentially significant or serious for patients and 42 (2%) of them were potentially life-threatening. Ten years earlier, the acceptance rate was 23%. Moreover, 14.7% of errors were attributed to the use of the software, whereas 10 years earlier the rate of errors was 49%., Conclusions: The acceptance rate and frequency of CPOE-related errors were better than 10 years before, which is encouraging and shows the importance of regular training and collaboration with healthcare givers to reduce errors. The routine analysis of interventions by pharmacists with medical staff feedback should continue to improve their relevance and effectiveness.

While considerable insights on the ecological consequences of climate change have been gained from studies conducted on remote lakes, little has been done on lakes under direct human exposure. Ecosystem vulnerability and responses to climate warming might yet largely depend on the ecological state and thus on local anthropogenic pressures. We tested this hypothesis through a paleolimnological approach on three temperate large lakes submitted to rather similar climate warming but varying intensities of analogous local forcings (changes in nutrient inputs and fisheries management practices). Changes in the structure of the cladoceran community were considered as revealing for alterations, over the time, of the pelagic food web. Trajectories of the cladoceran communities were compared among the three study lakes (Lakes Geneva, Bourget, and Annecy) over the last 70-150 years. Generalized additive models were used to develop a hierarchical understanding of the respective roles of local stressors and climate warming in structuring cladoceran communities. The cladoceran communities were not equally affected by climate warming between lakes. In Lake Annecy, which is the most nutrient-limited, the cladoceran community was essentially controlled by local stressors, with very limited impact of climate. In contrast, the more nutrient-loaded Lakes Geneva and Bourget were more sensitive to climate warming, although the magnitude of their responses and the pathways under which climate warming affected the communities varied between the two lakes. Finally, our results demonstrated that lake vulnerability and responses to climate warming are modulated by lake trophic status but can also be altered by fisheries management practices through changes in fish predation pressure.

Most of the Mediterranean coastal porous aquifers are intensively exploited. Because of climatic and anthropogenic effects, understanding the physical and geological controls on groundwater distribution and flow dynamics in such aquifers is crucial. This study presents the results of a structural investigation of a system located along the coastline of the Gulf of Lions (NW Mediterranean). A key aspect of this study relies on an onshore-offshore integrated approach combining outcrops, seismic profiles, and borehole data analysis. This multidisciplinary approach provides constraints on pore-fluid salinity distribution and stratigraphic organization, which are crucial in assessing the modes of groundwater/seawater exchanges. Onshore, Lower Pliocene deposits dip gently seaward. They are unconformably overlain by Holocene clays in the lagoons. Offshore the Pliocene deposits either outcrop at the seabed or are buried below nonconsolidated sands infilling paleo-valleys. Beneath the lido, the groundwater salinity distribution consists of salty pore water, overlying fresher pore water. Active circulation of groundwater masses is inferred from the geophysical results. Host-feeding patterns play a key role in the transmission of vector-borne diseases such as West Nile fever, which involves two kinds of vertebrates, birds and mammals. In this study, we propose a theoretical formulation for mosquito host-feeding patterns using three quantities, as follows: the apparent attractiveness/contact probabilities, the conditional host(-feeding) preferences, and the enzootic versus bridge probabilities. Using results from host-baited trap collections, the quantities defined above were assessed for the most abundant mosquito species in the main West Nile virus focus of southern France. We found that host availability is important in determining the efficiency of bridge vectors, and that even ornithophilic mosquitoes like Culex species, classically classified as enzootic vectors, may turn out to be efficient bridge vectors in certain contexts of host abundance. Our developed theoretical framework can easily be adapted and applied to other experimental data and other vector-borne diseases.

To identify the mosquito species able to sustain the transmission of West Nile Virus (WNV) in the Camargue region (the main WNV focus of southern France), we assessed the vector competence of Culex modestus and Culex pipiens, the most abundant bird-feeders, and Aedes caspius, the most abundant mammophilic species occasionally found engorged with avian blood. Female mosquitoes were exposed to the infectious meal (10(10.3) plaque forming units (PFU)/mL) by membrane feeding, and hold at 26 degrees C. After the incubation period, disseminated infection was assessed by WNV detection using an indirect fluorescent antibody assay (IFA) on head squashes, and the transmission rate was assessed by the presence of WNV RNA in salivary secretions with a real-time reverse transcriptase-polymerase chain reaction (RT-PCR). After 14 incubation days, the disseminated infection and the transmission rates were 89.2% and 54.5% for Cx. modestus, 38.5% and 15.8% for Cx. pipiens, and 0.8% and 0 for Ae. caspius. Culex modestus was found to be an extremely efficient laboratory WNV vector and could thus be considered the main WNV vector in wetlands of the Camargue. Culex pipiens was a moderately efficient laboratory WNV vector, but in dry areas of the region it could play the main role in WNV transmission between birds and from birds to mammals. Aedes caspius was an inefficient vector of WNV in the laboratory, and despite its high densities, its role in WNV transmission may be minor in southern France.

The Camargue area of southern France experienced the re-emergence of West Nile Virus (WNV) in the late summer of 2000 and 2004. Immediately preceding the 2004 outbreak, samples were collected from 432 birds of 32 different species captured in mist nets and from 201 Cattle Egret (Bubulcus ibis) nestlings sampled in their nests between 1 April and 12 June 2004. West Nile virus neutralizing titers of >/=40 were detected in 4.8% (95% confidence limit, 2.9-7.5%) of the adult birds and in 1.6% (0.3-4.6%) of the egret nestlings. Migratory passerines had a higher prevalence of WNV neutralizing antibodies (7.0%) than did resident and short-distance migratory passerines (0.8%), suggesting exposure to WNV or a related flavivirus during overwintering in Africa.

European magpies (Pica pica) from southern France were tested for antibodies to West Nile virus (WNV) and viral shedding in feces during spring-autumn 2005. Results suggest that this peridomestic species may be a suitable sentinel species and a relevant target for additional investigations on WNV ecology in Europe.

An equine West Nile virus (WNV) outbreak occurred in 2004 in the Camargue, a wetland area in the south of France where the virus was first reported in 1962 and re-emerged in 2000. WNV neutralizing antibodies were detected in resident birds and two isolates from a House Sparrow (Passer domesticus) and a Common Magpie (Pica pica) were completely sequenced. Phylogenetic analyses revealed that these isolates are closely related to strains previously found in horses in southern Europe and North Africa. More extensive investigation is required to determine whether WNV has been re-introduced or has become endemic in the Camargue.

Recent outbreaks of West Nile virus-associated (WNV) diseases, both in the old World and Americas, underline the importance for early warning systems that rapidly identify emerging and re-emerging diseases and thus help in their control. Traditional approaches of disease monitoring become less reliable and increasingly costly when used for rare health-related events, such as WNV outbreaks in southern France. The objective of this work was to discuss methodological issues related to syndromic monitoring of WNV-associated disease in Camargue horses by veterinary practitioners. Tracking cases of equine encephalitis by veterinarians is an example of such syndromic monitoring of an emerging disease. Signs of illness, observed prior diagnostic confirmation, can be of interest because they may provide an early warning for WNV circulation in a given area and allow authorities to take appropriate preventive measures for public health.

West Nile virus (WNV) is a mosquito-transmitted Flavivirus with a transmission cycle involving birds as amplifying hosts. Wild birds are also believed to carry WNV over large distances and are able to introduce it into new areas during migration and dispersal. In this paper, our objective is to provide lists of birds potentially involved in the introduction, the amplification and the spread of WNV in the Camargue, a Mediterranean wetland in the south of France where several WNV outbreaks have occurred since the 1960s. Bird species were classified according to the following ecological factors: migratory status and provenance area, used biotopes, abundance and period of presence in the Camargue. The obtained lists of bird species potentially involved in the introduction, amplification and spread of WNV should prove useful to determine target species on which further studies on WNV ecology in birds could be focused.

After 35 yr of disease absence, West Nile virus (family Flaviviridae, genus Flavivirus, WNV) circulation has been regularly detected in the Camargue region (southern France) since 2000. WNV was isolated from Culex modestus Ficalbi, which was considered the main vector in southern France after horse outbreaks in the 1960s. Recent WNV transmissions outside of the Cx. modestus distribution suggested the existence of other vectors. To study potential WNV vectors, horse- and bird-baited traps and human landing collections of mosquitoes were carried out weekly from May to October 2004 at two Camargue sites: one site in a wet area and the other site in a dry area, both chosen for their past history of WNV transmission. At the wet site, the most abundant species in bird-baited traps were Culex pipiens L. and Cx. modestus; both species also were found in lower proportions on horses and humans. The most abundant species in horse-baited traps and human landing collections were Aedes caspius (Pallas), Aedes vexans (Meigen), and Anopheles hyrcanus (Pallas) sensu lato; some of these species were occasionally collected with avian blood at the end of the summer. Anopheles maculipennis Meigen sensu lato was an abundant horse feeder, but it was rarely collected landing on human bait and never contained avian blood. At the dry site, Cx. pipiens was the most abundant species in bird- and horse-baited traps. The seasonal and circadian dynamics of these species are analyzed, and their potential in WNV transmission in Camargue discussed.

Anaplasmosis is caused by the bacterium Anaplasma phagocytophilum and transmitted by Ixodes spp. ticks. According to some reports the disease can be introduced into disease-free zones by migrating birds. The purpose of this study was to evaluate the seroprevalence of A. phagocytophilum in horses in the Camargue. Data concerning 424 horses were gathered and the sera were tested for A. phagocytophilum and for piroplasmoses using an enzyme-linked immunosorbent assay and a complement fixation test, respectively. The seroprevalence rates were 11.3 % for A. phagocytophilum, 64.4 % for Theileria equi and 19.7% for Babesia caballi. Stallions were less likely to produce antibodies against A. phagocytophilumthan were females or geldings (odds ratio [OR] = 0.3; p = 0.021). The presence of swallows increased the risk of infections in stables (OR = 5.18; p = 0.011). Spatial analysis showed the existence of groups of infected stables along canals and rivers (p = 0.008). These results suggest an emergence of A. phagocytophilum in the Camargue.

Twist-drill trephination associated with continuous closed drainage was first proposed forty years ago for treating chronic subdural hematomas. We describe the original percutaneous technique and report our experience in a retrospective study of 65 consecutive patients who have been operated using this method.

Epidemiological studies should allow comparisons to be made of the prevalence of disease in populations from different countries, but the population characteristics and health problems in French horses are not well established. We have conducted a retrospective evaluation of the causes of death and vital characteristics of insured horses in France for the year 1995, with a view to comparison with published data from other countries. Files on 448 deceased horses were provided by nine insurance companies. Most of the animals were used for breeding (60%), followed by leisure (20%), eventing and racing (10% each). Physical characteristics were associated significantly with occupational categories. The overall mortality rate was 2.47%, and was due, in decreasing order, to foaling (24%), colic (21%) or locomotor (21%), cardiovascular (9%), neurological (8%), respiratory (5%) or infectious (4%) disease. Infectious disease was more frequent in younger animals (p < 0.05) and locomotor disease in racehorses (p < 0.01). Horses aged over 15 years had a lower incidence of colic (p <0.05). The cause of death was not significantly linked to breed, insurance value or season. Despite some selection bias, the study provides useful information about mortality in the French equine population.
