Obstructive sleep apnoea (OSA) is a common disorder characterized by recurrent episodes of apnoea or hypopnea due to total or partial pharyngeal collapse and temporary upper airway obstruction during sleep. The prevalence of OSA is increasing and currently affects about 30% of men and 13% of women in Europe. Intermittent hypoxia, oxidative stress, systemic inflammation, and sleep fragmentation resulting from OSA can provoke subsequent cardiometabolic disorders. The relationships between endocrine disorders and OSA are complex and bidirectional. Indeed, several endocrine disorders are risk factors for OSA. Compared with the general population, the prevalence of OSA is increased in patients with obesity, hypothyroidism, acromegaly, Cushing syndrome, and type 1 and 2 diabetes. In some cases, treatment of the underlying endocrine disorder can improve, and occasionally cure, OSA. On the other hand, OSA can also induce endocrine disorders, particularly glucose metabolism abnormalities. Whether continuous positive airway pressure (CPAP) treatment for OSA can improve these endocrine disturbances remains unclear due to the presence of several confounding factors. In this review, we discuss the current state‐of‐the‐art based on the review of the current medical literature for key articles focusing on the bidirectional relationship between endocrine disorders and OSA and the effects of treatment. The Morphological Quality Index (MQI) and the Morphological Quality Index for monitoring (MQIm) have been applied to eight case studies across Europe with the objective of analyzing the hydromorphological response to various restoration measures and of comparing the results of the MQI and MQIm as a morphological assessment applied at the reach scale, with a conventional site scale physical-habitat assessment method. For each restored reach, the two indices were applied to the pre-restoration and post-restoration conditions. The restored reach was also compared to an adjacent, degraded reach. Results show that in all cases the restoration measures improved the morphological quality of the reach, but that the degree of improvement depends on many factors, including the initial morphological conditions, the length of the restored portion in relation to the reach length, and on the type of intervention. The comparison with a conventional site scale physical-habitat assessment method shows that the MQI and MQIm are best suited for the evaluation of restoration effects on river hydromorphology at the geomorphologically-relevant scale of the river reach. The effects of river restoration on hydromorphological conditions and variability are often documented immediately following the restoration, but rarely properly monitored in the long term. This study assesses outcomes of 20 restoration projects undertaken across central and northern Europe for a comprehensive set of hydromorphological parameters, quantified at both larger and smaller spatial scales. For each project, we compared a restored river section to an upstream degraded section. Ten pairs of large projects were contrasted to ten similar but less extensive projects, to address the importance of restoration extent for the success of each project. Overall, river restoration increased habitat diversity through changes in channel morphology. Our results indicated that restoration particularly improved macro- and mesohabitat diversity, but had a limited effect on microhabitat conditions, including the diversity of substrates. We found no significant difference in effects between large and small restoration projects. Our results reveal the need to assess hydromorphological parameters which reflect processes occurring at different spatial scales, including indicators of larger-scale hydromorphological processes such as bank erosion, to monitor restoration effects effectively and accurately. This introductory paper presents 20 river restoration cases throughout Europe that were investigated in the EU-funded research project REFORM. In the following, this special issue provides seven specific papers that highlight and discuss the effects of restoration on the investigated river-floodplain systems. Additionally, restoration success was estimated from a socio-economic perspective. The first part of this paper presents the overall study concept and the general sampling design of the field investigations. Each study site was examined with the same array of methods, covering habitat composition in the river and its floodplain, three aquatic and two floodplain-related organism groups, as well as food web composition and 'aquatic terrestrial' interactions as reflected by stable isotopes. An overview of the rivers and the study sites summarizes main attributes of all investigated sites, with emphasis on the large-scale restoration projects. Some of the projects represent the 'state of the art' restoration approaches for two major European river types: gravel-bed mountain rivers and sand-bed lowland rivers. Concluding, restoration efforts had positive effects even in the small restoration projects investigated but did not increase with project size. No 'single best' measure could be identified, but river widening generally had a larger effect compared to other restoration measures. [ABSTRACT FROM AUTHOR]

European rivers are highly degraded and restoration efforts are becoming more frequent. However, only few restoration projects have been rigorously evaluated so far. We investigated the response of fish assemblages to hydromorphological restoration measures including river widening, creation of instream structures, flow enhancement, remeandering and side-channel reconnection. We sampled 15 rivers with pairs of degraded and restored sites and calculated the effect sizes (i.e., restored-degraded) for species richness, species diversity, fish density and habitat traits. We analysed the following factors potentially affecting restoration success: (1) length of the restored river stretch, (2) time after restoration and (3) hydromorphological quality of restoration. While species diversity and density did not respond to restoration, proportion of small rheophilic fish increased and eurytopic decreased. Short-term (<3 years) and long-term effects (>12 years) of restoration measures have a stronger effect on fish assemblages than mid-term effects. Furthermore, the hydromorphological quality and the length of the restored section are relevant for the restoration effects on the fish community. Future restoration projects should focus on more dynamic, self-sustaining habitat improvements extending over several kilometres and should be coupled with other measures such as restoring the river continuity and species reintroductions. ABSTRACT This study examines adolescents' evaluation of the fairness of three forms of democratic decision-making procedures (direct democracy, representative democracy and group representation) and one non-democratic procedure (oligarchy). Social dominance orientation-Egalitarianism (SDO-E), religious group identification and the countries' level of democracy are examined as predictors. The 2008 Europroject dataset was used, which contained 4441 native majority adolescents (mean age = 16.1 years) in 18 European countries. Adolescents evaluated direct democracy as most fair, followed by group representation, representative democracy and oligarchy. This rank order was found independent of the issue under consideration (moral or social), and of SDO-E and religious identification, and across the countries. In addition, adolescents scoring higher on SDO-E and on religious identification found group representation and non-democratic oligarchy fairer. Copyright © 2014 John Wiley & Sons, Ltd. Background: Maternal hypothyroidism in pregnancy is associated with several adverse outcomes. The Endocrine Society Guidelines for the management of thyroid diseases in pregnancy were published in 2007; however, impact of the guidelines in routine clinical practice is unknown. Therefore, we have carried out a survey of members of the European Thyroid Association (ETA) to study current practices relating to the management of hypothyroidism in pregnancy. Subjects and methods: In December 2010, we emailed an electronic questionnaire survey based on clinical case scenarios to 605 members of the ETA. Responses from 190 clinician members (from 28 European countries) were analyzed. Results: For a pregnant woman with newly diagnosed overt hypothyroidism, most responders initiated a full dose of L-thyroxine (L-T4). For a woman with hypothyroidism planning pregnancy, 50% recommended increasing the dose of L-T4as soon as pregnancy is confirmed, whilst 43% favored testing thyroid function before adjusting the dose. Responders used diverse combinations of tests to monitor the dose of L-T4. The target of thyroid function tests that responders aimed to achieve with L-T4 was also inconsistent. Forty-two percent responders or their institutions screened all pregnant women for thyroid dysfunction, 43% performed targeted screening of only the high-risk group, whilst 17% did not carry out systemic screening. Timing of the screening, tests used, and criteria for starting treatment and monitoring were variable. Conclusions: There is wide variation in the clinical practice relating to the treatment and screening of hypothyroidism during pregnancy in Europe. [ABSTRACT FROM AUTHOR]

The objective of this article is to assess the competitiveness of the European food industry from an economic and legal perspective. Knowledge of its present competitiveness and improvement opportunities are lacking. To close this knowledge gap, we measured the competitiveness of eight subsectors, benchmarked them with four leading world economies, assessed the effect of differences in legal requirements, and predicted future developments using scenario analysis. International economics indicators supplemented with data on legal issues from a survey conducted by leading experts were used to measure competitiveness. The results show that the EU food industry's competitiveness is weak. The legal system was positively evaluated compared to the U.S. system, but major improvements are possible. The recommendations are to improve economies of scale, economies of scope, ICT-based supply chain management, and exploit cultural differences through innovation, within a more flexible and streamlined legal framework. In 1981, a "research-practice" breakthrough was realized through the foundation of the European Federation of Therapeutic Communities (EFTC). Since its foundation EFTC regularly organized conferences and symposia. A few years later (in 1983), the European Workshop On Drug policy Oriented Research (EWODOR) was established at the Erasmus University in Rotterdam, including a special section devoted to the therapeutic community (TC). The authors examined the proceedings of all meetings organized by EFTC and EWODOR. The major topics presented throughout the last 20 years are highlighted in a chronological selection and these presentations largely focus on the challenges of the TC and the state of the art regarding scientific knowledge. In this overview only those authors who published articles in scientific journals indexed in the "Web of Science" (Institute for Scientific Information—ISI) were retained. Although research has never been the primary consideration in TCs, it played a far from negligible role in its development. Research findings prompted innovations and thus often provoked resistance, but on the other hand they functioned as a catalyst between the necessity of a belief system and the reality of society. The role of "grey" literature and the availability of information through the Internet will become more and more important in the communication between researchers and practitioners. To investigate the role of competence and morality in stereotypes, a cross-national research was set up in six eastern-European countries. Study 1 measured the perceived desirability of stereotype attributes in ingroup versus outgroup members. Across countries, biopolar Competence and Morality components emerged. It was found that the perceived desirability of ingroup attributes was primarily competence-based, while desirability perceptions of outgroup attributes were mostly morality-based. In Study 2, participants in the six countries rated the occurrence of competence- and morality-related stereotype attributes among 10 national and ethnic target groups. Study 2 also assessed general evaluative attitudes and perceptions of power and conflict in inter-nation relations. Competence and morality dimensions fully explained the evaluative structure of national and ethnic stereotypes, generating a four-fold typology of sinful–loser , sinful–winner , virtuous–loser and virtuous–winner stereotypes. It presents the development of farmers as well as the agricultural industry which can be a key factor in fighting poverty. It emphasizes on letting readers participate in microeconomic research and designing of public policies for the future.

Migrant health workers fill care gaps in their destination countries, but they also actively engage in improving living conditions for people of their countries of origin through expatriate professional networks. This paper aims to explore the professional links that migrant health workers from sub-Saharan African countries living in five African and European destinations (Botswana, South Africa, Belgium, Austria, and the United Kingdom) have to their countries of origin. Qualitative interviews were conducted with migrant doctors, nurses, and midwives from sub-Saharan Africa (N=66). A qualitative content analysis of the material was performed using the software ATLAS.ti. Almost all migrant health workers have professional ties with their countries of origin supporting health, education, and social structures. They work with non-governmental organizations, universities, or hospitals and travel back and forth between their destination country and country of origin. For a few respondents, professional engagement or even maintaining private contacts in their country of origin is difficult due to the political situation at home. The results show that African migrant health workers are actively engaged in improving living conditions not only for their family members but also for the population in general in their countries of origin. Our respondents are mediators and active networkers in a globalized and transnationally connected world. The research suggests that the governments of these countries of origin could strategically use their migrant health workforce for improving education and population health in sub-Saharan Africa. It states that when Mansholt took office in 1958, many contented that the problem of European agriculture was one of the structure which is the farm that were too small. It infers that Mansholt convinced Germany to give for an agricultural policy required to make a common market for industrial products.

Background Recent initiatives in Europe have encouraged the formalisation of research infrastructure to unify fragmented facilities, resources and services; and to facilitate world-class research of complex public health challenges, such as those related to non-communicable disease. How this can be achieved in the area of food and health has, to date, been unclear. Scope and approach This commentary paper presents examples of the types of food and health research facilities, resources and services available in Europe. Insights are provided on the challenge of identifying and classifying research infrastructure. In addition, suggestions are made for the future direction of food and health research infrastructure in Europe. These views are informed by the EuroDISH project, which mapped research infrastructure in four areas of food and health research: Determinants of dietary behaviour; Intake of foods/nutrients; Status and functional markers of nutritional health; Health and disease risk of foods/nutrients. Key findings and conclusion There is no objective measure to identify or classify research infrastructure. It is therefore, difficult to operationalise this term. EuroDISH demonstrated specific challenges with identifying the degree an organisation, project, network or national infrastructure could be considered a research infrastructure; and establishing the boundary of a research infrastructure (integral hard or soft facilities/resources/services). Nevertheless, there are opportunities to create dedicated food and health research infrastructures in Europe. These would need to be flexible and adaptable to keep pace with an ever-changing research environment and bring together the multi-disciplinary needs of the food and health research community. Digitalisation affects the agri-food sector and its governance. However, what digitalisation of the sector will imply for future agricultural policymaking remains unclear. The objective of the study is to develop and evaluate explorative scenarios of digitalisation in the agri-food sector of Europe that are explicitly relevant to agricultural policy. The study aims to provide guidance for strategic development of agricultural policy to address the potentials, uncertainties and unknowns arising with digitalisation of the sector. We combine a Delphi study and a participatory scenario workshop to develop and evaluate plausible explorative scenarios of digitalisation of Europe's agri-food sector. For all scenarios we identify gaps in achieving a range of important European agricultural policy goals, drawing on the Delphi study and desk-based analysis. Subsequently we deduce strategies to address these agricultural policy gaps. Four scenarios of digitalisation of the agri-food sector were developed for Europe in 2030. They comprise of 1) digitalisation of the sector following current directions at current rates as a baseline scenario, 2) strong digitalisation of a regulatory government, 3) use of autonomous farming technology and 4) digitalised food business. These explorative scenarios entail various gaps in achieving European agricultural policy goals. Our findings suggest that the baseline scenario needs strategies to ramp up technological and institutional infrastructure for digitalisation. The other scenarios need strategies to prevent risks, e.g., of technological failures or undesired social impacts. They also need strategies to cater for special cases and diversity, e.g., of ecosystems and farming practices. Across the scenarios, it seems useful to increase digital competencies of the stakeholders. The study is the first that derives implications for policy strategies from explorative scenarios of future digitalisation of agricultural systems that target gaps in achieving agricultural policy goals. The combination of developing and analysing scenarios generated findings that are of significance to policymaking stakeholders and researchers alike, who all need to address the uncertainties arising with future digitalisation of the agri-food sector.
