Background/Aim: Baricitinib (BAR) is the first oral selective Janus kinase inhibitor approved in Europe for rheumatoid arthritis (RA). Real-world data are still needed to clarify its long-term benefits/risk profile. This study aimed to evaluate the effectiveness, persistence, adherence, and safety of BAR in a real-world setting. Methods: An ambispective study was conducted between October 2017 and December 2021 in RA patients starting BAR. The effectiveness was evaluated, assessing changes from the baseline of the Disease Activity Score using 28-joint counts-C reactive protein (DAS28CRP), and the achievement of low disease activity/remission. Drug persistence was evaluated using Kaplan–Meier analysis. Adherence was estimated using the medication possession ratio (MPR) and the 5-item Compliance Questionnaire for Rheumatology. Safety was assessed determining global incidence proportion and adverse event adjusted incidence rates. Results: In total, 61/64 recruited patients were finally analyzed, 83.6% were female, 78.7% were seropositive, the mean age was 58.1 (15.4) years, and the disease duration was 13.9 (8.3) years. A total of 32.8% of patients were naïve to biologics and 16.4% received BAR as monotherapy. The median exposure to BAR was 12.4 (6.6–31.2) months (range 3.1–51.4). A significant change in DAS28CRP was observed after treatment (difference −1.2, p = 0.000). 70.5% and 60.7% of patients achieved low disease activity or remission, respectively, and 50.8% (31/61) remained on BAR throughout the follow-up, with a median persistence of 31.2 (9.3–53.1) months. The average MPR was 0.96 (0.08) and all patients exhibited "good adherence" according to the questionnaire. In total, 21.3% of patients discontinued baricitinib due to toxicity. Conclusions: In our real-world practice, BAR demonstrated effectiveness, large persistence, high adherence to treatment, and an acceptable safety profile. [ABSTRACT FROM AUTHOR]

This research combines Strontium (87Sr/86Sr) and Oxygen (δ18O) isotope analysis to challenge the prevailing interpretation of patrilocal exogamic practices among eastern European Early Neolithic Linearbandkeramik (LBK) communities. Patrilocality has been considered the key factor influencing the mobility patterns of central Europe's first farmers (c. 5500–4900 cal. BC), especially in the south-eastern Moravian region (Czech Republic). Focusing our attention on both male and female tooth enamel samples from cemeteries, settlement graves and small clusters of graves, this paper reassesses the correlation between mobility, biological sex, and funerary practices. This task is accomplished by establishing a new isotopic footprint using new 87Sr/86Sr data, as well as significantly increasing the number of sampled individuals for 87Sr/86Sr and δ18O. The outcome of this research contributes to a better understanding of the mobility patterns among early farmers in central Europe, challenging existing theories and providing new insights into their social and cultural dynamics. Background: Gastrointestinal nematode (GIN) control is traditionally achieved with the use of anthelmintic drugs, however due to regulations in organic farming and the rise in anthelmintic resistance, alternatives are sought after. A promising alternative is the use of bioactive plant feeding due to the presence of plant secondary metabolites (PSMs) such as proanthocyanidins (PAs). This study focussed on the perennial shrub heather (Ericaceae family), a plant rich in PAs, highly abundant across Europe and with previously demonstrated anthelmintic potential. Methods: In vitro assays were used to investigate heather's anthelmintic efficacy against egg hatching and larval motility. Heather samples were collected from five European countries across two seasons, and extracts were tested against two GIN species: Teladorsagia circumcincta and Trichostrongylus colubriformis. Polyphenol group-specific ultraperformance liquid chromatography-tandem mass spectrometry analysis was performed to identify relevant polyphenol subgroups present, including the PA concentration and size and ratio of the subunits. Partial least squares analysis was performed to associate efficacy with variation in PSM composition. Results: Heather extracts reduced egg hatching of both GIN species in a dose-dependent manner by up to 100%, while three extracts at the highest concentration (10 mg/ml) reduced larval motility to levels that were not significantly different from dead larvae controls. PAs, particularly the procyanidin type, and flavonol derivatives were associated with anthelmintic activity, and the particular subgroup of polyphenols associated with the efficacy was dependent on the GIN species and life stage. Conclusions: Our results provide in vitro evidence that heather, a widely available plant often managed as a weed in grazing systems, has anthelmintic properties attributed to various groups of PSMs and could contribute to sustainable GIN control in ruminant production systems across Europe. Cannabis use remains an important concern in modern societies, adolescence being the critical period when the adolescents start to use cannabis. This study is aimed at extending and validating the Cannabis Use Intention Questionnaire (CUIQ), which was previously developed in Spain and based on the Theory of Planned Behaviour, in Italy, Portugal, and Romania. There are existing questionnaires which assess cannabis use problems and several risk factors; however, there is a need for a questionnaire based on a proven theory to assess interventions. A total of 3447 adolescents aged between 14 and 18 years old from Italy (43.14%), Portugal (23.90%), and Romania (32.96%) participated in this study. The data were analyzed using structural equation modeling. The scores were validated against cannabis use and the risk of experiencing problems related to cannabis use. The cut-off points were calculated using Receiver Operating Characteristic (ROC) curve analysis. The reliability index, α ordinal, indicated good reliability on the scales of attitudes, perceived behavioral control and intention. The results confirmed the four dimension structure of CUIQ. The invariance tests indicated that the structure and measurement of the scales were equivalent in all countries. The cut-off points showed good classification capability, especially for intention and attitudes. The CUIQ allows comparative studies addressed to study underlying psychological processes in adolescents’ decisions to use cannabis as well as to assess interventions aimed to reduce the problem of cannabis use. Aims: The onset and maintenance of cannabis use have been explained with reference to the Theory of Planned Behaviour (TPB). At the same time, personal values play an important role in determining appropriate behaviour during social interactions, and cannabis use is no exception. Combining Schwartz’s Theory of Human Values and the TPB offers a framework for identifying the relationship between adolescents’ personal values structure and cannabis use. The two main aims of this study are to: (1) test a predictive model that combines these two theories to explore the interaction of cultural and personal variables that influence cannabis use during adolescence; and (2) analyse the moderation effect of four European countries on the proposed model.  Methods: A total of 3768 secondary school students from Spain, Italy, Romania and Portugal completed a questionnaire on personal values and the TPB applied to cannabis use intention. Direct and indirect effects were assessed using path analysis and moderation effects using multi-group analysis. Results: Collectivistic values (i.e. conformity and tradition) were associated with low cannabis intention and use, individualistic values (i.e. hedonism and power) with higher cannabis intention and use. Regarding the moderation effect of countries, the effects of attitudes on intention and cannabis use were higher among Italian participants, who also showed a high prevalence of cannabis use. Meanwhile the effect of abstinence self-efficacy was lower among Romanian and Portuguese participants, both of whom reported lower cannabis use. The book aims to clarify the dynamics between rural gender process and comparative gender regime perspective which focuses on the interaction between states regulations as the manifestation of local rural process and citizens rights in Europe. The chapters of the book include societies wide range and discussed the diverse aspects of citizenship rights offered by different national-level gender regimed for the rural gender regime development.

Agricultural land abandonment is an increasing problem in Europe. The Comunitat Valenciana Region (Spain) is one of the most important citrus producers in Europe suffering this problem. This region characterizes by small sized citrus plots and high spatial fragmentation which makes necessary to use Very High-Resolution images to detect abandoned plots. In this paper spectral and Gray Level Co-Occurrence Matrix (GLCM)-based textural information derived from the Normalized Difference Vegetation Index (NDVI) are used to map abandoned citrus plots in Oliva municipality (eastern Spain). The proposed methodology is based on three general steps: (a) extraction of spectral and textural features from the image, (b) pixel-based classification of the image using the Random Forest algorithm, and (c) assignment of a single value per plot by majority voting. The best results were obtained when extracting the texture features with a 9 × 9 window size and the Random Forest model showed convergence around 100 decision trees. Cross-validation of the model showed an overall accuracy of the pixel-based classification of 87% and an overall accuracy of the plot-based classification of 95%. All the variables used are statistically significant for the classification, however the most important were contrast, dissimilarity, NIR band (720 nm), and blue band (620 nm). According to our results, 31% of the plots classified as citrus in Oliva by current methodology are abandoned. This is very important to avoid overestimating crop yield calculations by public administrations. The model was applied successfully outside the main study area (Oliva municipality); with a slightly lower accuracy (92%). Agricultural land abandonment is an important environmental issue in Europe. The proper management of agricultural areas has important implications for ecosystem services (food production, biodiversity, climate regulation and the landscape). In the coming years, an increase of abandoned areas is expected due to socio-economic changes. The identification and quantification of abandoned agricultural plots is key for monitoring this process and for applying management measures. The Valencian Region (Spain) is an important fruit and vegetable producing area in Europe, and it has the most important citrus industry. However, this agricultural sector is highly threatened by diverse factors, which have accelerated land abandonment. Landsat and MODIS satellite images have been used to map land abandonment. However, these images do not give good results in areas with high spatial fragmentation and small-sized agricultural plots. Sentinel-2 and airborne imagery shows unexplored potential to overcome this thanks to higher spatial resolutions. In this work, three models were compared for mapping abandoned plots using Sentinel-2 with 10 m bands, Sentinel-2 with 10 m and 20 m bands, and airborne imagery with 1 m visible and near-infrared bands. A pixel-based classification approach was used, applying the Random Forests algorithm. The algorithm was trained with 144 plots and 100 decision trees. The results were validated using the hold-out method with 96 independent plots. The most accurate map was obtained using airborne images, the Enhanced Vegetation Index (EVI) and Thiam's Transformed Vegetation Index (TTVI), with an overall accuracy of 88.5%. The map generated from Sentinel-2 images (10 m bands and the EVI and TTVI spectral indices) had an overall accuracy of 77.1%. Adding 20 m Sentinel-2 bands and the Normalized Difference Moisture Index (NDMI) did not improve the classification accuracy. According to the most accurate map, 4310 abandoned plots were detected in our study area, representing 32.5% of its agricultural surface. • Evaluation of a new analytical method for Phenylacetylglutamine in urine. • Biomarker of monitoring and adherence to the treatment in urea cycle disorders. • Clinical data are correlated to Phenylacetylglutamine for the first time in Europe. • The use of spot urines is easier to send for analysis due to its stability. Salts of phenylacetic acid (PAA) and phenylbutyric acid (PBA) have been used for nitrogen elimination as a treatment for hyperammonaemia caused by urea cycle disorders (UCD). A new analytical method for PBA measurement in urine which helps to evaluate the drug adherence has been implemented. Urine specimens from UCD patients receiving PBA were analysed by tandem mass spectrometry to measure urine phenylacetylglutamine (PAGln). Some clinical and biochemical data for each patient were collected. Our study included 87 samples from 40 UCD patients. The PAGln levels did not correlate with height, weight or age. However, the PAGln values showed correlation with PBA dose (r = 0.383, P = 0.015). Plasma glutamine and ammonia levels presented a positive correlation (r = 0.537, P < 0.001). The stability for PAGln in urine was determined at different storage temperatures. We have developed a simple method for the determination of PAGln in urine, which acts as useful biomarker of effective drug delivery. PAGln in urine is stable at room temperature at least for 15 days, and for several months when frozen at -20 °C.
