2. Influence of titanium and titanium-zirconium alloy as implant materials on implant stability of maxillary implant retained overdenture: a randomized clinical trial.

Background: Long-term success of implant restoration depends on many factors one of them is the sufficient implant stability which is lowered in compromised bone density sites such as the maxilla as it is categorized as type III & IV bone, so searching for a new innovation and updates in implant material and features is very mandatory. So, the aim of this study was to compare between two implant materials (roxolid and traditional titanium) on the primary and secondary stability of implant retained maxillary overdenture. Methods: Eighteen completely edentulous patients were selected. All patients received maxillary implant-retained overdentures and lower complete dentures; patients were divided equally into two groups according to the type of implant materials. Group A received a total number of 36 implants made of roxolid material and Group B received a total number of 36 implants made of traditional titanium alloys. Implant stability was assessed using ostell device, the primary implant stability was measured at the day of implant installation however, secondary implant stability was measured after six weeks of implant placement. Paired t-test was used to compare between primary and secondary stability in the same group and an independent t-test was used to compare between the two groups with a significant level < 0.05. Results: Independent t-test revealed a significant difference between the two groups with p -value = 0.0141 regarding primary stability and p-value < 0.001 regarding secondary stability, as roxolid implant group was statistically higher stability than titanium group in both. Paired t- test showed a statistically significant difference in roxolid implant group with p-value = 0.0122 however, there was non-statistically significant difference in titanium group with p-value = 0.636. Mann Whitney test showed a significant difference between the two groups regarding amount of change in stability with p value = 0.191. roxolid implant group showed a higher amount of change in stability than the titanium implant group. Conclusion: Within the limitation of this study, it could be concluded that: Roxolid implants showed promising results regarding primary and secondary stability compared to conventional Titanium implants and can be a better alternative in implant retained maxillary overdentures. 3. The relationship between serum transglutaminase-2 levels and the severity of chronic spontaneous urticaria.

Chronic spontaneous urticaria (CSU) is an immunological disease that is depicted by high prevalence and eminent burden for patients and society that is attributable to the arbitrary nature of symptoms and inconsistent tools for assessment of activity and severity. Transglutaminase-2 (TG2) is a posttranslational enzyme that is pervasively expressed in many cells and tissue types including mast cells. It has various biological functions, and its role in allergic disorders has been highlighted and delineated through several postulated mechanisms. This case–control study aimed at determining the relationship between serum levels TG2 and severity of CSU. To the best of our knowledge, this is the first study in Egypt to determine the relationship between serum TG2 and severity of CSU. We enrolled 60 adult patients with confirmed diagnosis of CSU. According to urticaria activity score (UAS), patients were categorized into three groups [20 with mild disease; UAS = 0, 20 with moderate disease; UAS = 1–3, 20 with severe disease; UAS = 4–6]. Another 20 healthy individuals (age and gender matched) served as a control group. All patients were subjected to detailed medical history, clinical examination, complete blood count with differential, serum total IgE, CRP, ESR, TSH, ANA, liver and renal function tests. Serum level of TG2 was done by quantitative ELISA for all enrolled patients and controls. Serum TG2 is significantly higher in patients group compared to control group (P value < 0.001). Serum TG2 levels were significantly higher in patients with severe disease compared to patients with moderate or mild disease. This is illustrated by the significant positive correlation between serum TG2 and UAS (r 0.814 and P value 0.000). Moreover, serum TG2 accurately classified CSU patients into mild, moderate and severe subgroups: as regards differentiation between mild and moderate cases (sensitivity 70%, specificity 80%, PPV 77.8, NPV 72.7) and as for the differentiation between moderate and severe cases (sensitivity 95%, specificity 90%, PPV 90.5, NPV 94.7). 4. Teaching Writing within the Common European Framework of Reference (CEFR): A Supplement Asynchronous Blended Learning Approach in an EFL Undergraduate Course in Egypt

- Abstract
Based on the Common European Framework of Reference (CEFR) and following a blended learning approach (a supplement model), this article reports on a quasi-experiment where writing was taught evenly with other language skills in everyday language contexts and where asynchronous online activities were required from students to extend learning beyond classroom hours. The experiment was carried out with freshmen in one of the Egyptian private universities. Twenty one pre-intermediate level students represented the experimental group that was taught the new CEFR course, and twenty six other students of the same level represented the control group that was taught the traditional face to face academically contextualized course. A pre and post writing tests were used to reveal students' writing proficiency before and after the experiment, and a "t" test was also used to measure the development of each group to find out whether their writing has developed after tutoring or not. Results indicated that the experimental group transcended the control group in 70% of the rubrics used to grade students' writing, and when measuring the results of the experimental group in the pretest and the posttest, there was a significant development in their writing proficiency level. This experiment is considered a step towards developing students' learning techniques in the institution; henceforth in the country. The experiment is one of the leading initiatives to teach English as a Foreign Language according to CEFR following a blended learning approach to undergraduate students in Egypt.

Leontodon hispidulus Boiss is a wild annual plant growing in Egypt. The present study aims for the first time, to evaluate the phytochemical profile of the main secondary metabolites of the optimized ethanolic extract of the plant using Quadrupole Time-of-Flight Liquid chromatography-mass spectrometry and Gas chromatography-mass spectrometry. It also aims to assess the anticancer activity of its different fractions against the prostate carcinoma cell line. Moreover, an in-silico docking study was performed using the Hexokinase-two enzyme. LC-qToF-MS analysis revealed the tentative identification of 36 phenolic compounds including the glycosides of (luteolin, quercetin, kaempferol, apigenin, isorhamnetin, and daidzein), coumarines (esculin, esculetin, and daphnetin), and phenolic acids (chlorogenic, caffeic, quinic, P-coumaric, and rosmarinic). GC–MS/MS analysis revealed the presence of 18 compounds where palmitic acid, myristic acid, alpha-amyrin, and beta-amyrin were the major ones. The cytotoxic activity results revealed that methylene chloride and ethyl acetate fractions showed the highest cytotoxic activity against the PC3 cell line, with IC
50 values of 19, and 19.6 μg/ml, respectively. Interestingly, the docking study demonstrated that apigenin-7-O-glucoside, luteolin-7-O-glucoside, kaempferol-3-O-glucuronide, quercetin-4′-O-glucoside, esculin, rosmarinic acid, chlorogenic acid, and α-amyrin exhibited high affinity to the selected target, HEK-2 enzyme. Background: Achilles' tendon chronic rupture is a common entity that is usually misdiagnosed or mistreated. Hence, she was presented to us later or with complications affecting her gait. Surgical resection is needed to either bridge the gap or reinforce the strength of the tendon repair. Objectives: Our study's goal was to assess the clinical results of repairing chronic Achilles' tendon lesions employing the middle segment of the proximal portion of the tendon (gastro-soleus), as a turn-down flap. Methods: Our prospective interventional single arm study included 18 patients with chronic Achilles' tendon rupture attending at Al-Azhar university hospitals in Cairo, Egypt from May 2020 to April 2023. Diagnosis of the patients was confirmed by radiographic and clinical investigations. They were all treated with the same open reconstruction procedure using a modified GSF. The average follow-up was 12 months. The results of this study were assessed by the Achilles tendon rupture score (ATRS), American Orthopedic Foot and Ankle Society (AOFAS) score, and capacity to perform repeated heel raises on the affected side. Results: The mean operative time was 72.77 min. The median (IQR) time of reconstruction was 10 (8–12) after the injury. The median (IQR) length of flab was 4.5 (4.3–5) 9 (Table 2). No intraoperative complications occurred. The typical follow-up period was 12 months (6–18 months). In terms of the ATRS, we found a significant reduction from 82.8 ± 3 preoperatively to 20.8 ± 6.7 at 12 months postoperatively (P value = 0.001). As regards the AOFAS score, it was increased from 49.5 ± 10 preoperatively to 83.8 ± 8.5 12 months postoperatively (P = 0.001). In terms of the post operative complications, there was no re-rupture. Two patients experienced superficial wound infection which improved with daily dressing and antibiotics. Additionally, two patients had slight ankle stiffness four months after the operation, which improved after programmed rehabilitation at the sixth month. Conclusion: The modified GSTF is a simple, safe, well-tolerated and effective method of treatment with excellent functional results and greater patient content. [ABSTRACT FROM AUTHOR]

Background: The aim of this work is to set up the standards for performing musculoskeletal ultrasound scans and reporting as an additional procedure in the rheumatology setting. We used two rounds of the Delphi approach to get the consensus on a musculoskeletal ultrasound reporting. Results: Fifteen expert panels had completed the two rounds of surveys. After the end of round two, eighteen recommendations distributed upon eight domains were released. The percentage of the agreement on the recommendations was 93.3 to 100 %. All eighteen key questions were answered at the end of the second round with agreement. Conclusion: A musculoskeletal ultrasound report template has been developed by this study, based on outcomes of a Delphi process, by an international participants' panel. All domains met the 80% voting threshold set in this work. The reporting template can be used for both clinical research as well as standard practice to provide guidance and standardize the musculoskeletal ultrasound reporting. [ABSTRACT FROM AUTHOR]

Introduction: Interest has recently frown in the use of exercise in managing hypothyroidism associated impairments. The objective of this research was to assess and contrast the effects of aerobic and resistance exercise regimens, and a combination of both, on depressive symptoms and sleep quality in women with managed hypothyroidism. Material and methods: A total of sixty women, aged 35 to 45 years, with controlled hypothyroidism associated with depression, and sleep problems were randomly assigned to four equally-sized groups: aerobic exercise (AE), resistance exercise (RE), combined AE/RE, and a control group. They engaged in low to moderate intensity workouts, three times a week, during a span of 12 weeks, while also undergoing levothyroxine therapy. Outcome measures included depression assessed with the Beck Depression Inventory II (BDI-II) and sleep quality, evaluated using the Pittsburgh Sleep Quality Index (PSQI). Results: The BDI-II and PSQI showed significant improvements in all exercise groups in comparison to the first measurements and the non-exercising group (p < 0.05). Most significantly, the AE/RE group demonstrated greater improvements in both outcomes then the AE and RE groups alone (p < 0.05). Conclusions: In well-managed hypothyroid women, AE, RE, and combined AE/RE can improve depression and sleep quality. However, combined AE/RE appears to induce the greatest improvements in both measures. [ABSTRACT FROM AUTHOR]

After the January 2011 revolution, Egyptian artists had the opportunity to express their ideas more openly. One of the major productions that started in 2011 in Egypt was TokTok, a comic magazine for adults. During the period of 2011–2015, TokTok issued fifteen issues that incorporated a collection of comics and comic strips created by different Egyptian artists. The purpose of this paper is to explore whether cringe humor has been an integral part of the drawings and stories presented in TokTok and how this type of humor reflected the socio-cultural and political scene in Egypt during that period. For this paper, Kress and Van Leeuwen's (2006. Reading images: The grammar of visual design. Routledge) model of multimodal analysis was applied to selected comics from the fifteen issues of TokTok to investigate possible elements of cringe humor as depicted in this Egyptian graphic magazine. Background: Low back pain (LBP) with clinical lumbar instability (CLI) is considered a subgroup of back pain. Poor core stability function and/or lack of motor controls are thought to play a role in inappropriate inter‐segmental movements and pain. There is no study investigating the changes in the lumbar multifidus muscle (LMM) morphology and motor control in this subgroup of patients. Objective: To assess motor control components and morphological changes of LMM in the patients suffering from chronic nonspecific low back pain (CNSLBP) with CLI. Design: Observational case‐control study. Methods: Thirty‐two patients suffering from (CNSLBP) with CLI and 32 healthy individuals were included. The muscle force element of lumbar motor control was assessed by using (the active straight‐leg raise test, leg lowering test, and Trendelenburg test). Ultrasonography was used to assess changes in the LMM morphology. Results: There was a significant decrease in motor control (p = 0.0001), an increase in LMM fatty infiltration (p = 0.002), and a decrease in the thickness of LMM in patients suffering from CNSLBP during contraction (p = 0.006), during rest (p = 0.018). The cross‐section area of the LMM showed no statistically significant differences during rest on the right and left sides (p = 0.827, 0.220 respectively) and contraction (p = 0.160, 0.278 respectively) between patients and healthy subjects. Conclusion: Motor control and the morphology of LMM in patients with CNSLBP with CLI may provide insight into the mechanisms of underlying pain and their effect on muscle function and structure. Purpose: Chronic granulomatous disease (CGD) is an inherited primary immunodeficiency disorder of phagocytes, characterized by recurrent fungal and bacterial infections. Our aim is to describe the different clinical presentations, non-infectious auto-inflammatory features, types and sites of infections, and to estimate the mortality among our large cohort. Methods: This is a retrospective study conducted at the Pediatric Department of Cairo University Children's Hospital in Egypt, including cases with a confirmed CGD diagnosis. Results: One hundred seventy-three confirmed CGD patients were included. AR-CGD was diagnosed in 132 patients (76.3%) including 83 patients (48%) with p47
phox defect, 44 patients (25.4%) with p22phox defect, and 5 patients (2.9%) with p67phox defect. XL-CGD was diagnosed in 25 patients (14.4%). The most common recorded clinical manifestations were deep-seated abscesses and pneumonia. Gram-negative bacteria and Aspergillus were the most frequently isolated species. Regarding the outcome, 36 patients (20.8%) were lost from follow-up. Among patients with known outcome, 94/137 patients (68.6%) are living, while 43/137 patients (31.4%) died. Conclusion: AR-CGD is predominant in Egypt; CGD must always be ruled out in any patient presenting with typical or atypical mycobacterial or BCG-disease. [ABSTRACT FROM AUTHOR]- Published

Effective use of natural materials and industrial by products has a direct impact on economy, sustainable environment, and waste management. Poorly graded desert sand (SP) is widely observed in sand dunes and sand seas. Studies were performed to get beneficial use of cement kiln dust (CKD) in soil improvement. But there is a lack of information concerning using of CKD as an improving material for poorly graded sand with the aid of salt water. Salt water is widely available in Egypt from seas and some deep wells, using it will help in saving drink water which suffers from shortage in many countries. The aim of this research is to study the possibility of improving SP using CKD and salt water to be used in backfilling work and subbase layers of roads and pavements. Mixtures of poorly graded sand and various CKD ratios (10%, 20% and 30%) were classified and tested using grain size distribution, compaction, shear strength and California bearing tests. The results were compared with potable water. The results show that adding CKD improves the characteristic properties of SP even with salt water. The study enhances the using of 20% CKD with optimum water content of 9.12% of salt water to have noticeable improvement for SP and to be used efficiently in backfilling and subbase layers. The objective of the study was to demonstrate a relationship between CRP level and the severity of depressive symptoms and see if C-reactive protein (CRP) levels can be used as a biomarker to distinguish between unipolar and bipolar depression. A convenience sample of 90 individuals was consecutively recruited from the psychiatry outpatient clinic at Beni-Suef University Hospital, Egypt. They were divided into three equal groups: patients with major depressive disorder (MDD) (group 1), bipolar depression (group 2), and a healthy volunteer group (group 3). Patient groups were assessed using semi-structured interviews, and CRP levels were measured. Results: Patients with bipolar depression and MDD showed a significantly higher score on the Beck Depression Inventory scale than the control group (32.97 ± 2.4 vs. 31.93 ± 2.3 vs. 8.00 ± 2.3 in all groups respectively) (P-value < 0.001). Patients with bipolar depression and MDD showed a significantly higher serum CRP level than the control group (134.96 ± 16.45 vs. 133.86 ± 17.59 vs.56.04 ± 26.71 vs. in all groups respectively) (P-value < 0.001). In all studied groups, elevated plasma CRP levels have a significant linear correlation with the severity of depression as measured by the Beck Depression Inventory (BDI) (r = 0.887, P-value < 0.001). Conclusions: Serum CRP levels are significantly higher in MDD and BD patients when compared to controls. Individuals with higher CRP levels had more severe depression, and this finding was significantly higher in women than in men. [ABSTRACT FROM AUTHOR]

17. Contributions of Investment and Employment to the Agricultural GDP Growth in Egypt: An ARDL Approach.

This paper explores the impact of investment and employment on Egypt's agricultural growth during the period 1991 to 2021 using annual time series data. We use the ARDL approach to examine the long-run and short-run relationships among agricultural investment, agricultural employment and agricultural GDP. The results reveal that the variables of interest are bound together in the long run. The long-run relationship and the error correction model are estimated. The accompanying equilibrium correction proves that long-run linkages exist in a meaningful way. Results show that agricultural investment and agricultural employment are major short- and long-run determinants of the agricultural GDP. In the long run, every 1% increase in agricultural employment (AEMP) results in an increase in the agricultural GDP (AGDP) of 3.73%, while every 1% increase in agricultural investment (AINV) improves the AGDP by 0.43%. In the short run, 26% of all disequilibrium-causing motions are adjusted for in a single session. Therefore, it takes 3.85 years for the Egyptian agricultural GDP to achieve the transition from a short-term disequilibrium situation to a long-term equilibrium. Thus, decision makers should increase the rates of investment in the agricultural sector, in parallel to the development of the agricultural labor force in Egypt. Moreover, the increased allocation of public investments and the injection of private investments are highly recommended. In addition, the Egyptian agricultural sector needs improvements regarding human capital development and agricultural training. Finally, the government must initiate comprehensive farmer support services, bolstered farm/non-farm links and the promotion of rural SMEs to serve as the foundation for agricultural and rural development. Due to the rapidly changing knowledge, teachers are supposed to teach their students ways of thinking and gathering information, not certain contents that would change shortly. In this sense, sustainability, which, in part, means the preparation of an individual who has the ability to practise critical thinking and to find creative solutions to the problems they face, is considered a must. This paper focuses on the integration of sustainability in curricula, specifically ways of introducing it to students in higher education institutions that provide highly specific and specialised knowledge and skills. So, this study presents a framework for reorienting a university course in the field of physiological psychology to address sustainability. The results of the quantitative analysis showed significant differences at the level of 0.001 between the pre- and post-testing of students' knowledge of and attitudes towards sustainability in favour of the post-test. While results of qualitative analysis showed positive transformation of students' practices health wise.

Background: Endometriosis is regarded as a benign condition, despite the potential for recurrence and metastasis,. It might lead to secondary ovarian cancer from endometrioma. Objective: The aim of this current study was to determine the relationship between risk factors for endometriosis and disease stages. Methods: This current cross-sectional study was performed at the endoscopic unit at Zagazig University Hospitals, Egypt. The participants included 85 women who were candidates for laparoscopy and diagnosed with endometriosis in the period from November 2019 to November 2020. The tools used for data collection were a structured interviewing form and the American Society of Reproductive Medicine scoring tool (ASRM) for determining stages of endometriosis. Results: The high percentages of studied women were nulliparous (74.1%) and (51.8%) have secondary infertility. Endometriosis was staged as 20.0% minimal (ASRM stage I), 34.1% mild (ASRM stage II), 29.4% moderate (ASRM stage III), and 16.5% severe (ASRM stage IV). Congestive dysmenorrhea (78.8%), dyspareunia (77.6%), and secondary infertility (51.8%) were more reported symptoms in all stages of endometriosis. Conclusion: This study concluded that women within reproductive age, nulliparity, lower body weight, urban residence, and past surgery of the pelvic were considered as risk factors in all disease stages. The application of Environmental Impact Assessment (EIA) studies faces many challenges, especially in developing countries. This problem was investigated in the existing literature and via a designed questionnaire among local practitioners. The reasons were mainly attributed to the seclusion of the EIA process across the project's phases, as well as the complexity of the EIA procedures. Hence, the research argues that Building Information Modelling (BIM) applications can be the key to resolve several associated problems to EIA either directly, or indirectly by streamlining Environmental Management System (EMS). Thus, a keyword-based scientometric analysis was used to define parameters and interrelations and establish network analysis for a proposed BIM-based EIA framework. The proposed framework presents scoring schemes to support three types of assessments: the efficiency of applying BIM techniques for an EIA process (Score A), proper integration of BIM techniques for different EIA steps (Score I) and proper integration of BIM techniques across project phases (Score B). It also shows the interplay of points between the three schemes via a Sankey diagram. The proposed framework was applied to compare three major construction projects in a developing country, the case of Egypt to pinpoint areas of drawbacks. The results pinpointed poor application of advanced BIM models to support sustainability analysis for EIA studies. This is in addition to imbalance of applying BIM applications across different project phases, it was notably less pronounced for the operation and maintenance stage as well as end of life scenarios. Also, poor integration of BIM tools could not support decisions related to the mitigation step. Hence, applying 6-dimensional BIM model shall enable developing mitigation measures and shall support design optimization during building operation and end of life phase. This novel approach presents the required level of verification and quality control procedure needed by decision-makers and environmental engineers to perform EIA studies and pave the way for further related research. Background: Kawasaki disease (KD) is an acute, self-limited febrile disease of unidentified cause that mostly affects children less than 5 years of age. This work aimed to provide an appropriate Egyptian evidence-based consensus on clinical practice recommendations for the management of Kawasaki disease. The main objective of this study, which employed the Delphi method, was to reach a consensus among experts on a treat-to-target management approach for KD. Results: The expert panel was confined to an online survey (n=26), and all the expert completed the three rounds. At the conclusion of round 3, a total of 17 recommendation items were gathered, which were divided into two sections. The range of respondents (ranks 7–9) who agreed with the recommendations was 92.3 to 100%. All 17 clinical standards identified by the scientific committee were written in the same way. There have been algorithms proposed for managing various KD conditions. Conclusion: The developed evidence-based consensus recommendations for the diagnosis and management of KD represent an up-to-date document that focuses on clinical management questions which are generally posed to health care professionals involved in the management of KD. Background: Children with chronic kidney disease (CKD), particularly those who require hemodialysis (HD), are at high risk of hepatitis B virus (HBV) infection. The HBV vaccine non-/hypo-response rate among HD children remains high, and it is critical to investigate the influencing factors and their linkages. The aim of this study was to identify the pattern of HB vaccination response in HD children and to analyze the interference of various clinical and biomedical factors with the immunological response to HB vaccination. Methods: This cross-sectional study included 74 children on maintenance hemodialysis, aged between 3 and 18 years. These children were subjected to complete clinical examination and laboratory investigations. Results: Out of a total of 74 children with HD, 25 (33.8%) were positive for the HCV antibody. Regarding the immunological response to hepatitis B vaccine, 70% were non-/hypo-responders (≤100 IU/mL) and only 30% mounted a high-level response (more than 100 IU/mL). There was a significant relation between non-/hypo-response and sex, dialysis duration, and HCV infection. Being on dialysis for more than 5 years and being HCV Ab-positive were independent variables for non-/hypo-response to HB vaccine. Conclusions: Children with CKD on regular HD have poor seroconversion rates in response to the HBV vaccine, which were influenced by dialysis duration and HCV infection. [ABSTRACT FROM AUTHOR]

Background: Tacrolimus is a widely prescribed immunosuppressant agent for kidney transplantation. However, optimal dosing is challenging due to its narrow therapeutic index, potentially serious adverse effects, and wide inter‐individual variability in pharmacokinetics. Cytochrome P450 3A (CPY3A) enzymes metabolize tacrolimus, so allelic variants such as CYP3A4*22 and CYP3A5*3 may contribute to individual differences in pharmacokinetics and therapeutic efficacy of tacrolimus. This study assessed the frequency and influences of CYP3A4*22 and CYP3A5*3 genotypes, alone and combined, on tacrolimus pharmacokinetics and dose requirements in Egyptian kidney transplant patients. Methods: This is a prospective multicenter observational cohort study. Patients were genotyped for the CYP3A4*22 (rs35599367), and CYP3A5*3 (rs776746). Tacrolimus dose (mg), through blood level (ng/ml), and dose‐adjusted trough concentration (C0/D) (ng/ml per mg/kg) were recorded during the first and third months post‐transplantation and compared among genotype groups. Results: The CYP3A4*22 allele was rare (3.2% of subjects) while the CYP3A5*3 allele was widespread (90.38%) in this cohort. At the third month post‐transplantation, median C0/D was significantly higher among CYP3A4*22 carriers than CYP3A4*1/*1 (146.25 [100–380] versus 85.57 [27–370] ng/ml per mg/kg, p = 0.028). Patients harbouring the one copy of the CYP3A4*22 allele and the CYP3A5*3/*3 genotype (n = 5) were classified as poor tacrolimus metabolizers, the CYP3A5*3/*3 plus CYP3A4*1/*1 genotype as intermediate metabolizers (n = 60), and the CYP3A4*1/*1 plus CYP3A5*1/*1 genotype as normal metabolizers (n = 13). During the first month post‐transplantation, C0/D was significantly greater in poor metabolizers (113.07 ng/ml per mg/kg) than intermediate and normal metabolizers (90.380 and 49.09 ng/ml per mg/kg) (p < 0.0005). This rank order was also observed during the third month. Acute rejection rate and renal function at discharge did not differ among genotypes. Conclusion: Pharmacogenetics testing for CYP3A4*22 and CYP3A5*3 before renal transplantation may help in the adjustment of tacrolimus starting dose and identify patients at risk of tacrolimus overexposure or underexposure. 25. Upper Cretaceous-Paleocene Unconventional Oil Shales in Egypt and Jordan: Insights into Organic Geochemistry.

The present paper investigates the unconventional hydrocarbon potential and characteristics of Campanian-Paleocene oil shales at five outcrop sections in central Egypt and Jordan. Onehundred and seven outcrop samples were analyzed for their content of total organic carbon and total sulfur. Twenty-four samples were selected for further geochemical treatment, including Rock-Eval pyrolysis, and six samples were subjected to molecular analyses using gas Chromatography (GC) and Gas Chromatography-Mass Spectrometry (GC-MS). The distribution of aliphatic hydrocarbon biomarkers indicated that most of the organic matter in the studied samples had a marine origin under anoxic to suboxic conditions. The results indicated that the upper Campanian Duwi Formation in the Quseir area of central Egypt and the Maastrichtian-Paleocene Muwaqqar Chalk Marl (MCM) Formation in the Lajjun area of central Jordan had a relatively high hydrocarbon generating potential with a TOC content of up to 20%wt of immature Type I/II kerogen. Biomarker studies of the investigated formations revealed that the origin of the organic matter is marine algae with a minor contribution of bacterial biomass. The terrigenous/aquatic ratio (TAR) indicated the marine origin of the Jordanian organic matter (MCM and Al-Hisa Phosphorite Formation formations). By contrast, the Duwi Formation in Egypt and the Amman silicified limestone Formation in the Umm Qais section of Jordan showed that these localities were affected by a more terrigenous influx to the principal marine organic matter compared with other rock units. In addition, in the Abu Tartur section the Duwi Formation showed a significant microbial reworking of organic matter reducing the actual amount of organic matter and its hydrocarbon generating potentiality. Clear correlations between nature and the preservation of organic matter is noticed, where the high organic matter mainly occurs in regional paleo-upwelling areas, strongly related to an increase in marine phytoplanktonic/algal and bacterial productivity. Objective: To assess the pattern of internet usage among preparatory school students in Beni-Suef Governorate, Egypt, and the Al Qassim Region, Saudi Arabia, and identify its associated factors. Methods: A cross-sectional study using a multistage proportionate sampling technique was conducted from October to December 2019 for 1537 preparatory school students from both countries; 772 from Beni-Suef Governorate, Egypt, and 765 students from the Al Qassim Region, Saudi Arabia. The Young's Internet Addiction Test (YIAT) was used. Results: Mean age of Egyptian and Saudi participants were 12.82 ± 0.81 and 13.8 ± 1.16 years, respectively. Mild to moderate IA was reported in 42.5% & 42.9%, among Egyptian students compared to 51.6% & 24.3% for Saudis, while severe IA was reported in 4.0% & 3.7% among Egyptian and Saudis, respectively. Internet searches were conducted by 43.5% among Egyptian students compared to online communication among Saudis (52.9%). Facebook was used by 42.0% of Egyptian students, while 52.4% Saudi students used Snap chat. In both samples, there was a statistically significant difference between internet addiction and gender, academic year, level of academic achievement, and purpose of internet use (p ≤ 0.05). Background: Inborn errors of immunity (IEI) are a group of heterogeneous disorders with geographic and ethnic diversities. Although IEI are common in Egypt, genetic diagnosis is limited due to financial restrictions. This study aims to characterize the genetic spectrum of IEI patients in Egypt and highlights the adaptation of the molecular diagnostic methods to a resource-limited setting. Methods: Genetic material from 504 patients was studied, and proper diagnosis was achieved in 282 patients from 246 families. Mutational analysis was done by Sanger sequencing, next-generation sequencing (NGS) targeting customized genes panels, and whole-exome sequencing (WES) according to the patients' phenotypes and availability of genetic testing. Results: A total of 194 variants involving 72 different genes were detected with RAG1/2 genes being the most encountered followed by DOCK8, CYBA, LRBA, NCF1, and JAK3. Autosomal recessive (AR) inheritance was detected in 233/282 patients (82.6%), X-linked (XL) recessive inheritance in 32/282 patients (11.3%), and autosomal dominant (AD) inheritance in 18/282 patients (6.4%), reflecting the impact of consanguineous marriages on the prevalence of different modes of inheritance and the distribution of the various IEI disorders. Conclusion: The study showed that a combination of Sanger sequencing in selected patients associated with targeted NGS or WES in other patients is an effective diagnostic strategy for IEI diagnosis in countries with limited diagnostic resources. Molecular testing can be used to validate other nonexpensive laboratory techniques that help to reach definitive diagnosis and help in genetic counseling and taking proper therapeutic decisions including stem cell transplantation or gene therapy. Objectives: To report on the live birth rates (LBRs) following percutaneous epididymal sperm aspiration (PESA) in men with obstructive azoospermia (OA) and factors affecting treatment outcome which is under reported in the literature. Methods: This is a multicenter study that was conducted in Egypt including all couples undergoing intra cytoplasmic sperm injection (ICSI) for OA using PESA-derived sperms. Men were subdivided according to aetiology into congenital, iatrogenic and idiopathic groups. Fertilization, pregnancy and LBRs were determined and compared in each group. The longitudinal LBR, crude and expected cumulative delivery rates (CCDR, ECDR) were calculated. Multiple logistic regression analysis was used to determine significant associations between maternal, paternal and ICSI factors with successful live births. Results: Ninety couples were included in the study. Viable sperm for ICSI was retrieved in 89 men (98.9%). A total of 155 ICSI cycles with 17 frozen embryo transfers resulted in 81 pregnancies and 55 live births. After 5 cycles, the longitudinal LBR, CCDR and ECDR were 30%, 57.3% and 88.6% respectively. Maternal age and number of fertilized eggs were the only factors significantly affecting LBRs. Conclusions: PESA is a minimally invasive procedure for securing viable sperm for ICSI in OA men, with high cumulative delivery rates. Maternal age and number of fertilized eggs are the only factors that significantly affecting LBR. Background: Human inborn errors of immunity (IEI) are a group of inherited genetic disorders of the immune system. IEI Patients suffer from severe repeated infections, autoimmunity, lymphadenopathy and/or increased susceptibility to malignancies. IEI are due to absence, disproportion, or loss of function of immune cells; mostly inherited in autosomal recessive manner, hence are more common in countries with high rate of consanguinity. Definite diagnosis of IEI is achieved by genetic analysis, however it is not always available. Aim: to report on different IEI categories and impact of expanding the use of flow cytometry (FCM) in diagnosis, categorization and follow up of IEI patients in a highly consanguineous population. Methods: Retrospective chart review on different IEI categories diagnosed at the primary immunodeficiency center in Cairo University Specialized Pediatric hospital from 2011 to 2021 based on expanding the use of FCM. Results: 1510 IEI patients were diagnosed; 480 were diagnosed genetically with FMF, 11 with cystic fibrosis and 1019 patients were diagnosed with other IEI disorders. Phagocytic defects were the commonest (30%) followed by severe combined immunodeficiency (22%) and combined immunodeficiency (18.3%). FCM testing properly diagnosed and categorized 73% of the cases. Conclusion: Using multi-color FCM to evaluate immune cells populations, subpopulations, functions, and intracellular proteins expression is proved a useful cost-effective method for screening, categorization and follow up of IEI patients. FCM can improve the diagnosis of IEI significantly when tests are properly targeted and well designed. This study presents a 10-year experience in diagnosis of IEI using FCM at a tertiary referral center in a setting of limited resources and yet high prevalence of IEI. [ABSTRACT FROM AUTHOR]

Background: Although cervical cancer is potentially preventable, lack of knowledge and poor attitude among healthcare professionals toward cervical cancer screening and Human Papilloma Virus (HPV) vaccination can result in underutilization of these preventive strategies. Then, the objective of this study was to assess the knowledge, attitude, and practice of cervical cancer and its prevention through Pap test screening and HPV vaccination among obstetricians and gynecologists (Ob-Gyns). Methods: A cross-sectional study was conducted on 250 Egyptian attendees of a national Ob-Gyns professional conference. Data collection was performed using a pre-designed self-administered questionnaire, which tested participants' knowledge, attitude, and practices related to cervical cancer, Pap test screening, and HPV vaccination. Results: The study included 41.2% Ob-Gyns specialists and 37.6% of consultants from secondary and tertiary care hospitals or centers. About 45% of participants had poor-to-fair knowledge, 57% had negative-to-fair positive attitudes toward cervical cancer screening and HPV vaccination, and 44% had ever-performed Pap test, while 45% of participants had ever-prescribed the HPV vaccine to their patients. Physicians' knowledge and attitude were significantly associated with their age, professional level, work experience, and place of work. Although performing cervical cancer screening was significantly more common among older, more experienced, and highly professional participants, HPV vaccine prescription was associated with young, less experienced participants at lower educational and professional levels. Background: The realization of the profession's role in both of global and personal concept affected the self-identification among 2020 medical students. COVID-19 pandemic can hinder, accelerate, or change medical students' professional identity formation. Objective: To assess the level of satisfaction about blended learning among undergraduate medical students at Zagazig University and to determine challenges, which face the medical students during blended learning education in Faculty of Medicine. Patients and methods: This cross-sectional study was performed during the period from September 2020 to July 2021 at Zagazig University, Egypt. This research included 277 undergraduate medical students. Results: The current study reported that interaction satisfaction score ranged from 18 to 53 with median 37 and 53.1% of students reported high satisfaction with interaction domain. There was statistically significant correlation between total satisfaction score and both age and technology barrier. There was statistically non-significant correlation between total satisfaction score and communication barrier. Conclusion: The results show that the majority of our sample students were highly satisfied with blended learning and reported low barriers to e-learning. Students still prefer face-to-face courses despite their satisfaction with their grades and performance in blended learning courses. [ABSTRACT FROM AUTHOR]

32. Analysis of a two-year independent screening effort for retinopathy of prematurity in rural Egypt.

Background: The third epidemic of retinopathy of prematurity (ROP) has majorly involved middle income countries in which tailored screening and local guidelines require development. The data regarding ROP prevalence and cutoff numbers for screening in Egypt are lacking.Methods: Retrospective analysis of an independent screening effort spanning 2 years (February 2019 to February 2021) and involving 32 neonatal care units within Sharkia governorate, Egypt. Infants of gestational age (GA) ≤ 34 weeks and/or birth weight (BW) ≤ 2000 g were included, as well as those with unstable clinical course. Two eyecare centers located in Sharkia and Cairo governorates served as referral centers for any required interventions.Results: Of the 276 screened infants, 133 (48.2%) had some form of ROP that was bilateral in 127 (95.5%) of them. Aggressive posterior ROP (AP-ROP) was detected in both eyes of 24 infants (8.7%). The median (IQR) GA of infants with ROP was 32 (30-34) weeks, and the median (IQR) BW was 1600 (1350-2000) g. Sixty-three infants (47.4%) required treatment. Of the total 84 eyes that primarily were treated, 73 (86.9%) received intravitreal ranibizumab, 8 (9.5%) underwent laser ablation therapy, and 3 eyes (3.6%) underwent surgery. Recurrence rate was 16.7% (14 eyes). Final outcome was favorable in 83 eyes (98.8%). Applying the American Academy criteria would have led to the missing of 36.8% of infants with ROP and 28.6% of those requiring treatment in our sample.Conclusion: The incidence of both ROP and AP-ROP in the Egyptian rural setting appears to be in the high end of global reported rates. One of the unique comedic expressions that evolved during the first wave of the Covid-19 pandemic in Egypt is a Facebook page called "Coronavirus". What is exceptional about the memes published on this page is that the author adopts the persona of the coronavirus to explore issues of the pandemic in the Egyptian context. Moreover, followers of the page fully participate in this make-believe scenario that the author has established. In addition, most of the posts mock the illogical actions undertaken by both the public and the state. Memes have been created, and shared, by Facebook users, not only as comic relief but also as a means of documenting events during the pandemic. This article aims to analyse different memes created by the "Coronavirus" Facebook page in the Egyptian context. The discussion is based on content and visual analysis. Memes were contextualised in relation to real incidents or decisions criticised by the author of the page, or the meme creator. This paper discusses how much food is wasted in Egypt, and how can we prevent these losses considering wheat crop case study. The main objective is to define and reduce the amount of food losses in Egypt. 3 from total wheat available for consumption in Egypt. The amount of water losses is considerable. Also, the relationship of the demographic variables of the sample of the study with the presence or absence of loss of wheat bread indicate that there is a statistically significant relation between the demographic variables studied, and the wheat wastage in Egypt. Objective: Previous reports show conflicting results regarding hepatitis B virus (HBV) vaccine efficacy in Hepatitis C virus (HCV) infected individuals and in those with isolated hepatitis B core antibodies (HBcAb). We aimed to evaluate the effectiveness of HBV vaccine and identify possible factors that may contribute to hyporesponsivness in HCV-treated patients, including those with isolated HBcAb.Methods: We conducted a prospective study with 118 enrolled chronic HCV patients who followed a 12-week regimen of direct acting antivirals (DAAs) and were evaluated for HBV serological markers. Eventually, 98 received appropriate HBV vaccination and were assessed for response.Results: A total of 57.1% were vaccine responders although only 5.1% achieved a seroprotective level of HBsAb titre. The response rate was significantly lower among treated HCV patients with isolated HBcAb [2 (5.6%) vs. 40 (64.5%) respectively]. On multivariate analysis, advanced age [OR (95% CI) = 1.09 (1.02-1.17)] and presence of isolated HbcAb [OR (95% CI) = 39.59 (7.98-196.63)] were predictors of vaccine non-response. In our cost-effectiveness models, the cost of HBV serological screening was less than the nationally adopted non-screening approach. A model ratifying reinforced vaccination in non-responder HBcAb seropositive HCV patients would incur extra cost.Conclusion: Hyporesponsiveness to the HBV vaccination is frequent in chronic HCV patients even after achieving SVR following DAAs. Although there is no consensus on the clinical management of patients with isolated HBcAb, our cost-effectiveness options may support decision-making for better clinical benefit and proper health investments. Background: Endometriosis is considered the most common cause of pelvic adhesions in women. Endometriosisassociated adhesions could result in the formation of fibrous bands, which contain endometriotic glands, stroma and scarring. The aim of this study was to identify the impact of endometriosis-related adhesions on quality of life among infertile women. Materials and Methods: This descriptive study was conducted at Endoscopic Unit, in Zagazig University Hospitals, Egypt. Oral consent for participation in this study was taken from 109 women who were candidates for laparoscopy as infertile cases and were diagnosed with endometriosis. They were classified into two groups namely, group I (n=41) who had endometriosis with adhesions and group II (n=68) who had endometriosis without adhesions. A structured interviewing form, adhesion scoring method of the American Fertility Society, and Global Quality of Life Scale were used to collect required information. Results: The prevalence of adhesions resulted from endometriosis was 37.6%. Demographic characteristics of the women with endometriosis-related adhesions were not significantly different from those of women without endometriosis- related adhesions. The most common location for endometriotic adhesions was adnexal adhesion (51.2%) followed by adhesion of anterior abdominal wall (24.4%). Quality of life was significantly impacted by endometriosisrelated adhesions (P=0.002). Conclusion: A high percentage of studied patients had a moderate degree of adhesions. Adhesions caused by endometriosis had an impact on quality of life of the studied women. [ABSTRACT FROM AUTHOR]

Background: The Earias spp. are devastating pests which reduce cotton yield up to 40% as seed cotton. Efficacy of Metarhizium anisopliae and Beauveria bassiana as bio-insecticides were investigated against the spiny bollworm, Earias insulana Boisduval (Lepidoptera: Noctuidae), under field conditions in Egypt throughout two successive seasons 2018 and 2019. Results: Results showed E. insulana population reduction during both seasons after bio-insecticidal applications. M. anisopliae strains (S1, S2) treatments showed significant reductions in the mean numbers of infested bolls with E. insulana after 14 days from the 1st spray. Reduction percentages of E. insulana infestation reached to 77.74 and 76.51% respectively, after application of M. anisopliae strains (S1, S2) treatments; then, the infestation reduction percentages increased to 88.48 and 85.41% after the 2nd spray by the same fungal strains; then, the infestation reduction percentages increased to 90.16 and 90.84% after 3rd spray of the same fungal strains in season 2018. In 2019 season, the infestation reduction percentages of E. insulana was 85.48 and 80.75%, after the 1st spray of M. anisopliae strains (S1, S2), respectively, which increased to 92.40 and 89.87%, after 14 days from the 2nd spray by the two fungal strains respectively, and then increased to 94.12 and 93.73%, after 14 days from the 3rd spray of M. anisopliae (S1 and S2) respectively. In season 2018, the infestation reduction percentage of E. insulana by B. bassiana strains (S1, S2) recorded 73.09 and 71.89%, respectively and 81.04 and 82.89% respectively, in season 2019, after 14 days of the 1st spray. While after the 2nd spray of the two tested B. bassiana strains, the infestation reduction percentage of E. insulana was 85.41 and 85.41% respectively, in season 2018, whereas it was 89.16 and 89.16% respectively, in season 2019. Then after 14 days of the 3rd spray by the same fungus strains, the reduction percentage of E. insulana increased to 86.56 and 85.35%, respectively, in season 2018, and 90.83 and 90.83% in season 2019. Conclusions: Tested strains of B. bassiana and M. anisopliae fungi proved their potential for decreasing E. insulana infestation percentages through 2–3 spray treatments under field conditions. Rural electrification represents a great worry for developing nations. Isolated microgrids establishment is the brightest solution for this problem. Optimization strategies for mathematical simulation and modelling are accomplished in this research to size an isolated residential microgrid in Egypt consists of wind generators (WG), photovoltaic (PV), battery energy storage system (BESS) and Biomass generator. This paper's novelty relies on the techno-economic performance comparison of three sizing strategies of isolated microgrid based on renewable sources, considering the sources' uncertainty. Also, the environmental and economic effects of using rice straw in biomass gasification are introduced to handle the problem of dark clouds from its open-air burning. Three categories with nine combinations of renewable energy sources (RES) are introduced in this paper to select the ideal option. Multi-objective genetic algorithm (MOGA) optimized the systems under both deterministic and stochastic conditions, taking into consideration the uncertainties of solar radiation and wind speed using Monte-Carlo techniques. The obtained results are compared with the results of epsilon multiobjective genetic algorithm ("-MOGA). Simulation outcomes show that PV with BESS is the most economical option for microgrid sizing with a total system cost of $ 4.04 million. Background: Cholinestrase inhibitor insecticides considered one of the most common causes of morbidity and mortality due to poisoning especially in developing countries like Egypt. This work aims to study the possible role of Creatine Phosphokinase (CPK), Amylase, Lactate Dehydrogenase (LDH), C-Reactive Protein (CRP) and blood sugar status in predicting the severity of acute cholinesterase inhibitor insecticides poisoning at the time of admission. Subjects and Methods: This work was done at Poison Control Center of Ain Shams University Hospitals – Egypt, and was carried on 150 patients admitted with acute (CEI) insecticides poisoning. Clinical evaluation of the patients and poisoning severity was graded according to Peradynia organophosphorus poisoning (POP) scale, then venous blood sample was collected from patients and examined for CPK, Amylase, LDH, CRP and random blood sugar (RBS). There is significant increase of CPK, LDH, CRP RBS and amylase enzyme specially with increase the severity in those patients admitted to ICU rather than inpatient, and in mechanically ventilated patients rather than non mechanically ventilated and in died patients rather than survivors. Conclusion: CPK, Amylase, LDH, CRP and random blood sugar could be used to predict severity of cholinesterase inhibitor insecticides poisoning. They can also predict ICU admission, mechanical ventilation need and mortality. Amylase is a better predictor of respiratory failure and need of mechanical ventilation. [ABSTRACT FROM AUTHOR]

Summary: Severe combined immunodeficiency (SCID) is fatal if not treated with immune reconstitution. In Egypt, T‐B+ SCID accounts for 38·5% of SCID diagnoses. An accurate genetic diagnosis is essential for choosing appropriate treatment modalities and for offering genetic counseling to the patient's family. The objectives of this study were to describe the clinical, immunological and molecular characteristics of a cohort of twenty Egyptian patients with T‐B+ SCID. The initial diagnosis (based on clinical features and flow cytometry) was followed by molecular investigation (whole‐exome sequencing). All patients had the classic clinical picture for SCID, including failure to thrive (n = 20), oral candidiasis (n = 17), persistent diarrhea (n = 14), pneumonia (n = 13), napkin dermatitis (n = 10), skin rash (n = 7), otitis media (n = 3) and meningitis (n = 2). The onset of manifestations was at the age of 2·4 ± 1·6 months and diagnosis at the age of 6·7 ± ·5 months, giving a diagnostic delay of 4·3 months. JAK3 gene variants were most frequent (n = 12) with three novel variants identified, followed by IL2Rγ variants (n = 6) with two novel variants. IL7Rα and CD3ε variants were found once, with a novel variant each. T‐B+NK− SCID accounted for approximately 90% of the Egyptian patients with T‐B+SCID. Of these T‐B+NK− SCID cases, 60% were autosomal recessive syndromes caused by JAK3 mutations and 30% were X‐linked syndromes. It might be useful to sequence the JAK3 gene (i.e. targeted Sanger sequencing) in all T‐B+ SCID patients, especially after X‐linked SCID has been ruled out. Hence, no more than 10% of T‐B+ SCID patients might require next‐generation for a molecular diagnosis. [ABSTRACT FROM AUTHOR]

Sexual health and education are rarely investigated in the Middle East countries, including Egypt. We performed the current study to investigate the knowledge, attitudes and behaviors towards sexual health practices among Egyptian youth. A self-administered questionnaire was translated from English to Arabic. After pilot-testing, the eligible population was invited to fill it online. Nine-hundred and fifty-five participants filled the questionnaire (53% males and 88.5% single). Around 61% of the participants were of the opinion that sexual education is necessary and rejected the notion that sexual education is religiously prohibited. Only 26% discussed sex-related matters with a parent. Although over 80% think that masturbation is either prohibited, wrong or medically harmful, 62% have masturbated before. Living with parents was not associated with less masturbation (p = 0.23). Moreover, although 85% of the participants think that watching pornography is either wrong or prohibited, 74% have watched porn clips before (significantly more frequent among males: p < 0.0001). A quarter of the responders indicated that they have been touched by a stranger/relative against their will. In conclusion, the majority of surveyed Egyptian youth expressed unfavorable attitudes towards masturbation and watching pornography -despite being quite common- and supported the importance of sexual education. Background: one of the most nearly daily compromising symptoms in hemodialysis patients is skin itching. Blood lead level (BLLs) may have a risk factor that increase the intensity of hemodialysis pruritus. Objective: To evaluate the prevalence of pruritus and its relationship with blood lead level in renal patients maintained on hemodialysis. Patients and Method: Cross -sectional Study carried on 100 patients with End stage renal disease (ESRD) divided into Group I; low BLLs (<15 µg/dl) included 56 patients, Group II; High normal BLLs (15 ≤ BLL < 25 µg/dl) included 32 patients and Group III; High BLLs (≥ 25 µg/dl) included 12 patients. All subjects were evaluated for kidney and liver function, electrolytes, CBC, estimation of GFR by MDRD equation, and Lead level in serum and dialysis water. Results: we found in this study that prevalence of pruritus was (19%), BLLs have a high statistically significant positive relation to HD duration (P<0.001) & I-PTH(P<0.001) & pruritus (P< 0.001) & visual analogue scale (VAS) (P<0.044) and high significant negative relation to serum albumin (P< 0.015) & hemoglobin(P<0.001). Also, Serum Lead (µg/dL) is an independent predictor for incidence of uremic pruritus [odds ratio (OR)=1.207; 95% by Multiple logistic regression analysis Conclusions: pruritus prevalence was (19%), blood lead level is highly related to pruritus and independent risk factor with HD duration for early onset and progression of itching in chronic haemodialysis patients. So we recommend serum lead investigation in these patients for early detection and reduction of its dermatological and psychological morbidity. [ABSTRACT FROM AUTHOR]

The aim of the study was to improve preparatory stage pupils' communicative grammar performance using task-based language instruction. The study problem was the low scores of the first year preparatory stage pupils' communicative grammar performance obtained from the pre-research. The participants of this study consisted of (N=60). Pupils enrolled at the first year preparatory stage at Kafr-Saqr Preparatory School for Girls, Sharkia Governorate, Egypt, in the academic year (2018-2019). They were randomly divided into two groups: Experimental group (N=30), and control group (N=30), during the treatment period, first-group participants received task-based language instruction, while second-group participants received instruction through traditional method. Firstly, pupils were administered a pre-test, then a post-test was administered to evaluate whether the progress between pre and post-test results were meaningful or not. The analyzed results clearly demonstrated the significance contribution of Task-Based Language Instruction to the EFL first year preparatory stage pupils' communicative grammar performance. The hydrocarbon potential is determined by the quantity and quality of organic matter encountered in the Jurassic sediments in two wells at the Northern Western Desert. It utilizes to define the zones of oil and gas using the well logging data for calculates the total organic carbon (TOC). The evaluation of source rock has been based on two steps; the first one depended on the geochemical parameters including TOC, S1, S2, Tmax, and Vitrinite reflectance (Ro %) of two wells JG-1 and JD-4. The second step was to calculate (TOC) from wireline logs. The well log types utilized in such kind of analysis are the density log, sonic log, resistivity log and gamma-ray log. The stratigraphic sequence, in the studied wells ranges in age from Paleozoic to Recent. The present work focuses on the Jurassic rocks represented by Khatatba Formation as they include the main source horizon. Based on the obtained results, Jurassic sediments called as fair to excellent source rock potential. The genetic type of organic matter can be identified through the study of pyrolysis data, which indicate that is rich in mixed oil and gas-prone kerogen except few samples reflect type I organic facies. LPS-responsive beige-like anchor (LRBA) deficiency is an autosomal recessive primary immunodeficiency disorder, OMIM (#614700). LRBA deficiency patients suffer from variable manifestations including recurrent infections, immune dysregulation, autoimmunity, cytopenias, and enteropathy. This study describes different clinical phenotypes and immunological characteristics of 18 LRBA deficiency patients diagnosed from Egypt. T and B lymphocyte subpopulations, LRBA, and cytotoxic T lymphocyte-associated protein 4 (CTLA4) expression were evaluated in resting and stimulated T cells using flow cytometry. Next-generation sequencing was used to identify mutations in the LRBA gene. LRBA deficiency patients had significantly lower B cells and increased percentage of memory T cells. CTLA4 levels were lower in LRBA-deficient T regulatory cells in comparison to healthy donors at resting conditions and significantly increased upon stimulation of T cells. We identified 11 novel mutations in LRBA gene ranging from large deletions to point mutations. Finally, we were able to differentiate LRBA-deficient patients from healthy control and common variable immunodeficiency patients using a simple flow cytometry test performed on whole blood and without need to prior stimulation. LRBA deficiency has heterogeneous phenotypes with poor phenotype-genotype correlation since the same mutation may manifest differently even within the same family. Low LRBA expression, low numbers of B cells, increased numbers of memory T cells, and defective CTLA4 expression (which increase to normal level upon T cell stimulation) are useful laboratory tests to establish the diagnosis of LRBA deficiency. Screening of the siblings of affected patients is very important as patients may be asymptomatic at the beginning of the disease course. [ABSTRACT FROM AUTHOR]

Purpose: Civil registration and vital statistics (CRVS) systems should be the primary source of routine mortality data. However, there is lack of information about the completeness of death registration at the sub-national level of Egypt. The current study was conducted to estimate the completeness of death registration at the national and sub-national levels of Egypt, to investigate the spatial patterns of the completeness, and to examine the factors that influence it. Methods: Data from the Central Agency for Public Mobilization and Statistics (CAPMAS, 2018) and Egypt Demographic and Health Survey (EDHS 2008, 2014) were used to estimate the completeness of death registration using an empirical method (random-effects models); hot spot analysis was conducted using Moran's I and Getis-Ord Gi*; and the geographically weighted regression (GWR) model has been also carried out. Results: The study estimates show that Egypt has 96% completeness of death registration, and all governorates have completeness of more than 90% except for Beni-Suef, Menia, Aswan, Suhag, Luxor, ELWadi ELGidid, and South Sinai. According to sex, the death registration of females is slightly better than that of males (96.8% compared to 95.4%). Concerning residence, urban area has almost complete death registration compared to rural area (99.5% and 85.4%, respectively). Hot spot analysis shows that all hot spots are centered on the north of Egypt, while all cold spots are focused on the south. However, according to the geographically weighted regression (GWR) model, poverty, illiteracy, and health office density are considered major factors for the completeness of death registration. Conclusion: Although the completeness in Egypt is almost 100%, this analysis suggests that it may not be, and that it could be somewhat lower in some rural areas. However, there is uncertainty in the sub-national estimates because deaths are only reported by place of occurrence and not place of usual residence. Thus, efforts should focus on improving the quality of data of the vital registration system in some rural areas and in lower Egyptian governorates. [ABSTRACT FROM AUTHOR]

Purpose: To describe the clinical experience with intermediate uveitis at six Egyptian tertiary eye centers.Methods: A multicenter retrospective chart review of all patients with intermediate uveitis seen at six ocular inflammation referral clinics in Egypt between January 2010 and January 2017.Results: The study included a total of 781 patients with intermediate uveitis. The study cohort comprised of 282 male and 499 female patients. In over half of our cohort (58.77%), no specific cause could be confirmed. The remaining patients had sarcoidosis (16.26%), tuberculosis (14.85%), multiple sclerosis (9.09%), and TINU (1.02%). By the end of our study, 62% of the affected eyes had a BCVA better than 20/40Conclusion: More than 40% of our patients with intermediate uveitis had sarcoidosis, tuberculosis, multiple sclerosis, or TINU as the underlying etiology. Owing to their potential morbidity, these diseases need to be considered in Egyptian patients presenting with intermediate uveitis. [ABSTRACT FROM AUTHOR]

Purpose: To report the prevalence of subclinical keratoconus (KC), and KC suspects, in pediatric Egyptian population with astigmatic error ≥ 2 diopters (D), with the use of CSO Sirius tomographer. Methods: A cross-sectional study that included all the children aged 6– 18 years with an astigmatic error of ≥ 2 D who attended the pediatric ophthalmology clinic for routine checkups during the period between March 2017 and December 2017 was done. All subjects underwent comprehensive ophthalmic examination, and corneal imaging using CSO Sirius instrument, a pentacam with Scheimpflug technology. Results: There were 547 subjects enrolled in our study. Mean age was 11.34 ± 3.03 (range 6– 18) years. There were 278 subjects (50.8%) between 6 and 12 years, and 269 subjects (49.2%) between 12 and 18 years. Males represented 48.4% and females represented 51.6% of patients. Among all the subjects, there were 26 (4.8%) KC patients, 24 (4.4%) KC suspects, and 497 (90.9%) were normal. Conclusion: Prevalence of subclinical KC among pediatric age group with astigmatism is relatively high, hence the importance of corneal tomography screening of children with astigmatic error ≥ 2 D.
