Background: Adjuvant chemotherapy following curative resection is the standard treatment for pancreatic adenocarcinoma (PC). Randomized clinical trials using gemcitabine have shown a median overall survival (mOS) of 2 years and a 5-year survival rate of 15-20%. However, the effect of gemcitabine outside these trials is less clear. We examined the effect of postoperative gemcitabine on survival in an unselected cohort of patients receiving curative resection for PC in Denmark during a five-year period. Material and methods: From 1 May 2011 to 30 April 2016, 731 patients treated with curative resection were identified in the Danish Pancreatic Cancer Database (DPCD). Thirty patients died within 10 weeks postoperatively; 78 received other regimens or preoperative chemotherapy and were excluded. Of the remaining 623 patients, the chemotherapy (CT) group ( n  = 409, 66%) received gemcitabine within 10 weeks after resection, whereas the non-chemotherapy (NCT) group ( n  = 214, 34%) did not receive CT within 10 weeks. Results: CT patients were slightly younger than NCT patients but did not otherwise differ in baseline characteristics. The CT group showed a mOS of 24 months (95% CI; 21-27) and a 5-year survival rate of 22% (95% CI; 17-27); the NCT group had a mOS of 22 months (95% CI; 16-26, p  = .27) and a 5-year survival rate of 26% (95% CI; 19-34, p  = .66). Most patients (415/623) had lymph node metastases. Of these patients, those in the CT group ( n  = 280) had significantly longer mOS [20 months (95% CI; 18-24)] than those in the NCT group ( n  = 135) [14 months (95% CI; 11-17)]. Conclusions: In this national Danish cohort of PC patients undergoing resection between 2011 and 2016, the survival after postoperative gemcitabine was similar to that reported in previous clinical trials. However, the survival advantage of postoperative gemcitabine was limited to patients with lymph node metastases.

Study Design: Cohort study., Objective: To report outcome after primary surgery for adult spinal deformity (ASD), and assess risk factors for extended length of stay (extLOS) and readmission within 90 days., Summary of Background Data: Complications after ASD surgery have been reported in 20% to 50% of patients. A few studies have focused on detailed information concerning postoperative morbidity., Methods: Patients >18 years undergoing primary instrumented surgery for ASD in Denmark in the period 2006 to 2016 were included. Patients were identified in the Danish National Patient Registry (DNPR) using procedure codes for instrumented spine surgery combined with diagnosis of spinal deformity. Information on length of stay (LOS), readmissions, and mortality were retrieved from the DNPR. Medical records were reviewed for causes of extLOS (LOS >10 days) and readmission within 90 days after surgery. Charlson Comorbidity Index (CCI) was calculated for all patients based on DNPR., Results: We included 892 patients. Median LOS was 8 days (interquartile range, IQR, 7-11). A total of 175 (28.0%) patients had extLOS; 81% because of "medically" related complications. Most common medically related reason (>50%) for extLOS was pain/mobilization difficulties. The 90-days readmission risk was 13.8%; 74.5% of readmissions were "medically" related, primarily opioid related side effects (18.2%), and pain/mobilization issues (15.2%); 25.5% of readmissions were "surgically" related and 16.7% of patients required revision surgery. Ninety-days mortality was 0.9%. Age groups 61 to 70 years, >70 years, CCI score 1 to 2 and CCI score ≥3 were associated with increased risk of extLOS (odds ratio, OR = 1.79, 2.01, 1.81, and 2.49, respectively). Age group >70 years and CCI score 1 to 2 were associated with increased risk of readmission (OR = 2.21 and 1.83)., Conclusion: Increasing age and comorbidity were associated with increased risk extLOS and readmission. Pain/mobilization difficulties were the most common postoperative complications. A future focus on early mobilization and pain management may improve outcome of ASD patients., Level of Evidence: 3.

In late-stage metastatic colorectal cancer, difficult treatment decisions should incorporate a thorough evaluation of the patient's general condition and subject for shared decision making. Assessment of the individual patients' prognosis is valuable in this setting. The aim was to analyze the prognostic value of plasma levels of total cell-free DNA, carcinoembryonic antigen and C-reactive protein in 97 heavily pretreated patients with metastatic colorectal cancer. Patients received irinotecan, cetuximab, and everolimus in a phase-2 clinical trial ( clinicaltrials.gov NCT01387880). Plasma samples were used for DNA purification and quantification of total cell-free DNA by droplet digital polymerase chain reaction. Serum carcinoembryonic antigen and C-reactive protein were analyzed by routine methods. Clinical endpoints were overall survival and progression-free survival. A total of 82 patients had blood samples available for quantification of total cell-free DNA. Patients with pre-treatment cell-free DNA levels higher than the median total cell-free DNA (9800 alleles per milliliter plasma) had a significantly shorter overall survival of 4.3 months (95% confidence interval: 3.6-5.8) compared to patients with cell-free DNA levels below the median with an overall survival of 11.3 months (95% confidence interval: 8.0-14.8, p < 0.0001). When using the upper normal limit from a previously analyzed normal control group, the median overall survival was 11.3 (95% confidence interval: 7.3-14.8) and 4.3 (95% confidence interval: 3.7-6.1) months, respectively (p < 0.0001). Serum carcinoembryonic antigen and C-reactive protein had similar prognostic value with short overall survival and progression-free survival in patients with elevated levels compared to those within normal range. A high-risk profile of elevated cell-free DNA, carcinoembryonic antigen, and C-reactive protein was described, but in combined Cox regression multivariate analysis, only total cell-free DNA preserved a strong prognostic value. In conclusion, total cell-free DNA in plasma, carcinoembryonic antigen, and C-reactive protein could all contribute to assessment of patients' prognosis and potentially aid in clinical decision making in patients with metastatic colorectal cancer.

Introduction: As the primary treatment of patients with severe trauma continues to improve, increasing interest has been directed towards long-term survival and Health Related Quality of Life (HRQoL). In trauma patients, there are few studies describing long-term outcome using tools specifically directed at HRQoL., Hypothesis: HRQoL measured with EQ-5D is significantly reduced compared to the Danish norm score 15 years after severe injury., Materials and Methods: All patients more than 18 years of age, admitted to a level 1 trauma center from March 1996 to September 1997 were prospectively included and scored with Injury Severity Score (ISS). Survival status was recorded in May 2012 and EQ-5D questionnaires were sent out., Results: 95 of the original 154 trauma patients were eligible for participation. The response rate was 66%. The average EQ-5D index score in the trauma population was significantly reduced compared to the index score in the Danish norm population (P=0.00, one-sample t-test). In addition, ISS is associated with HRQoL and ISS≥16 predicts poorer HRQoL., Conclusion: EQ-5D is significantly reduced 15years after severe trauma High ISS was associated with low HRQoL. Background: The novel BD Onclarity TM HPV assay (Onclarity) on the BD Viper™ LT system (BD Diagnostics, Sparks, MD), detects E6/E7 DNA from 13 high-risk HPV genotypes and HPV66. We compared the analytical and clinical performance of the Onclarity Assay to that of Hybrid Capture 2 and LINEAR ARRAY using adjudicated histological outcomes from Danish women referred for colposcopy., Methods: 276 women from Copenhagen, Denmark were referred for colposcopy with abnormal cytology and/or a positive HPV test. Two samples for HPV analysis were taken in BD SurePath™ and in the BD cervical brush diluent (CBD) media. Study Design: Cross-sectional analyses on a consecutive, prospective cohort., Objective: To evaluate the ability of the Scoliosis Research Society (SRS)-Schwab Adult Spinal Deformity Classification to group patients by widely used health-related quality-of-life (HRQOL) scores and examine possible confounding variables., Summary of Background Data: The SRS-Schwab Adult Spinal Deformity Classification includes sagittal modifiers considered important for HRQOL and the clinical impact of the classification has been validated in patients from the International Spine Study Group database; however, equivocal results were reported for the Pelvic Tilt modifier and potential confounding variables were not evaluated., Methods: Between March 2013 and May 2014, all adult spinal deformity patients from our outpatient clinic with sufficient radiographs were prospectively enrolled. Analyses of HRQOL variance and post hoc analyses were performed for each SRS-Schwab modifier. Age, history of spine surgery, and aetiology of spinal deformity were considered potential confounders and their influence on the association between SRS-Schwab modifiers and aggregated Oswestry Disability Index (ODI) scores was evaluated with multivariate proportional odds regressions. P values were adjusted for multiple testing., Results: Two hundred ninety-two of 460 eligible patients were included for analyses. The SRS-Schwab Classification significantly discriminated HRQOL scores between normal and abnormal sagittal modifier classifications. Individual grade comparisons showed equivocal results; however, Pelvic Tilt grade + versus +  + did not discriminate patients according to any HRQOL score. All modifiers showed significant proportional odds for worse aggregated ODI scores with increasing grade levels and the effects were robust to confounding. However, age group and aetiology had individual significant effects., Conclusion: The SRS-Schwab sagittal modifiers reliably grouped patients graded 0 versus + / +  + according to the most widely used HRQOL scores and the effects of increasing grade level on odds for worse ODI scores remained significant after adjusting for potential confounders. Background: Data on long-term prognosis after spondylodiscitis are scarce. The purpose of this study was to determine long-term mortality and the causes of death after spondylodiscitis., Methods: A nationwide, population-based cohort study using national registries of patients diagnosed with non-post-operative pyogenic spondylodiscitis from 1994-2009, alive 1 year after diagnosis (n = 1505). A comparison cohort from the background population individually matched for sex and age was identified (n = 7525). Kaplan-Meier survival curves were constructed and Poisson regression analyses used to estimate mortality rate ratios (MRR)., Results: Three hundred and sixty-five patients (24%) and 1115 individuals from the comparison cohort (15%) died. Unadjusted MRR for spondylodiscitis patients was 1.76 (95% CI = 1.57-1.98) and 1.47 (95% CI = 1.30-1.66) after adjustment for comorbidity. No deaths were observed in 128 patients under the age of 16 years. Siblings of patients did not have increased long-term mortality compared with siblings of the individuals from the comparison cohort. This study observed increased mortality due to infections (MRR = 2.57), neoplasms (MRR = 1.40), endocrine (MRR = 3.72), cardiovascular (MRR = 1.62), respiratory (MRR = 1.71), gastrointestinal (MRR = 3.35), musculoskeletal (MRR = 5.39) and genitourinary diseases (MRR = 3.37), but also due to trauma, poisoning and external causes (MRR = 2.78), alcohol abuse-related diseases (MRR = 5.59) and drug abuse-related diseases (6 vs 0 deaths, MRR not calculable)., Conclusions: Patients diagnosed with spondylodiscitis have increased long-term mortality, mainly due to comorbidities, particularly substance abuse.

Background: We compared the sensitivity and specificity of liquid-based cytology (LBC) and computer-assisted reading for SurePath/FocalPoint and ThinPrep with those of manually read conventional cytology in routine cervical screening in four Danish laboratories., Methods: Using data from five nationwide registers, technological phases were identified by slide preparation, reading technique, and triage of borderline cytology. Trends in the detection of cervical intraepithelial neoplasia (CIN) were an indicator of the technology's relative sensitivity, and trends in false-positive tests an indicator of relative specificity., Results: At 23-29 years, SurePath/FocalPoint statistically significantly increased the detection of ⩾CIN3 by 85% compared with manually read conventional cytology. The 11% increase with ThinPrep was not significant. At 30-44 years, the increase with SurePath/FocalPoint was 58%; the 16% increase with ThinPrep was not significant. At 45-59 years, both technologies led to nonsignificant decreases in the detection. SurePath/FocalPoint doubled the frequency of false-positive tests at any age. With ThinPrep, these proportions remained the same at 23-29 years, but decreased by two-thirds at 45-59 years. In a fourth laboratory with continuous use of manually read conventional cytology, no such trends were seen., Conclusions: The sensitivity and specificity of modern LBC and computer-assisted reading technologies may be brand- and age-dependent.

Objectives: To determine the long-term mortality and the causes of death after Staphylococcus aureus spondylodiscitis., Methods: Nationwide, population-based cohort study using national registries of adults diagnosed with non postoperative S. aureus spondylodiscitis from 1994-2009 and alive 1 year after diagnosis (n Z 313). A comparison cohort from the background population individually matched on sex and age was identified (n Z 1565). Kaplan-Meier survival curves were constructed and Poisson regression analyses used to estimate mortality rate ratios (MRR) adjusted for comorbidity., Results: 88 patients (28.1%) and 267 individuals from the population-based comparison cohort (17.1%) died. Un-adjusted MRR for S. aureus spondylodiscitis patients was 1.77 (95% CI, 1.39-2.25) and 1.32 (95% CI, 1.02-1.71) after adjustment for comorbidity. We observed increased mortality due to infectious (MRR 8.57; 95% CI, 2.80-26.20), endocrine (MRR 3.57; 95%CI, 1.01-12.66), cardiovascular (MRR 1.59; 95% CI, 1.02-2.49), gastrointestinal (MRR 3.21; 95% CI, 1.178.84) and alcohol and drug abuse-related (MRR 10.71; 95% CI, 3.23-35.58) diseases., Conclusions: Patients diagnosed with S. aureus spondylodiscitis have substantially increased long-term mortality, mainly due to comorbidity. To improve survival after S. aureus spondylodiscitis these patients should be screened for comorbidity and substance abuse predisposing to the disease [corrected]., (Copyright © 2014 The British Infection Association. Study Design: We conducted a retrospective cohort study of 151 patients with breast cancer spinal metastases., Objective: To investigate the influence of breast cancer subtypes on survival duration of patients with breast cancer spinal metastases, and to aid spine surgeons in selecting treatments on a more precise basis., Summary of Background Data: There is lack of knowledge about specific prognosis of patients with spinal metastases in various breast cancer subtypes. Estrogen receptor (ER), progesterone receptor (PgR), and human epidermal growth factor receptor 2 (Her-2) status are the key factors in determining breast cancer subtypes and predicting patients' response to adjuvant treatments., Methods: Until August 2013, we retrieved 151 surgically treated patients with breast cancer spinal metastases and followed up all the patients for at least 2 years. Survival duration analysis and Cox proportional hazards regression model unadjusted and adjusted by age were used., Results: Patients with ER-negative (-) breast cancer had 11 months shorter median survival duration (10.6 vs. 21.5 mo) and 48% higher mortality risk (P=0.03) than those with ER-positive (+) breast cancer. Patients with PgR (-) status had 59% higher mortality risk than those with PgR (+) status (P=0.02). Hormone receptor (HR) status is a combination of ER and PgR status. Patients with HR (-) status had an 11-month shorter median survival duration and 52% higher mortality risk (P=0.01) than patients with HR (+) status. Human epidermal growth factor receptor 2 subtypes had similar median survival duration and mortality risk. Patients with triple-negative breast cancer had a median survival duration of only 9.9 months., Conclusion: Patients with spinal metastases with ER/HR (-) status and triple-negative breast cancer could be downgraded from score "5" to "3" in Tokuhashi scoring system and from "slow growth" to "moderate growth" in Tomita scoring system. Spine surgeons should be critical before performing high-risk extensive surgery in patients with ER/HR (-) status, and especially, in those with triple-negative status., Level of Evidence: 3.

Purpose: A retrospective study to assess the health-related quality of life in patients undergoing posterior fixation of thoracolumbar fractures, and to compare the outcome with norm scores and HRQL in patients undergoing surgical treatment for degenerative conditions of the spine., Methods: The study population consisted of 93 consecutive patients surgically treated for a thoracolumbar fracture from 2005 to 2009. All patients underwent posterior pedicle screw fixation and fusion. 83 patients were available for follow-up. Three questionnaires reflecting health-related quality of life (HRQL) were mailed to the patients: SF-36, ODI, and EQ-5D., Results: 52 men and 31 women were operated with an average age at the time of surgery of 46 years. All fractures were AO-type A3.1 and A3.2 with L1 being the primary fracture level. All patients were neurologically intact; Frankel E. The overall response rate on the questionnaires was 75% with no significant differences between responders and non-responder on basic demographic characteristics. The median follow up period was 3.8 years (range 2-6 years). The scores on all three questionnaires were significantly reduced compared to the national norm scores (p<0.05)., Conclusion: Health-related quality of life is affected several years after short segment posterior instrumentation of thoracolumbar fractures without neurological deficit., (Copyright © 2013 Elsevier Ltd. Objective: Human papillomavirus (HPV) genotype distribution in invasive cervical cancers may differ by geographic region. The primary objective of this study was to estimate HPV-genotype distribution in Danish women with a diagnosis of invasive cervical cancer., Design: Observational, cross-sectional., Population: Danish data from a multi-center study undertaken in 12 European countries., Methods: A total of 342 archived fixed tissue samples with diagnosis of invasive cervical cancer from the Departments of Pathology in the University Hospitals in Hvidovre and Odense, Denmark, were anonymized and shipped to a central laboratory for histopathology review and PCR testing for HPV DNA. A standardized HPV-test methodology was used to enable comparison of HPV-type distribution., Main Outcome Measures: Occurrence of HPV genotypes in Danish women with cervical cancer., Results: There were 261 samples evaluated as histologically adequate and 251 (96%) of these were HPV-positive (HPV+). The most frequent diagnosis was squamous cell carcinoma (78.9% of histological adequate and 79.3% of HPV+). Adenocarcinoma, adenosquamous carcinoma and other types were found in 14.9, 3.4 and 2.7% of the histologically adequate group and 14.7, 3.6 and 2.4% of the HPV+ group, respectively. In 92.8% of HPV+ women only a single HPV type was diagnosed. HPV-type distribution in the latter population was as follows: HPV-16: 62.2%; HPV-18: 14.6%; HPV-33: 6.9%; HPV-45: 6.4% and HPV-31: 3.4%. Of the HPV+ women, 6.4% were diagnosed with multiple HPV types and 0.8% had unknown HPV types., Conclusion: HPV-16 and -18 are detected in 74.3% of Danish women with diagnosis of invasive cervical cancer, while HPV-16, -18, -31, -33, -45 and 58 are detected in 90.0% of women with invasive cervical disease., (© 2013 Nordic Federation of Societies of Obstetrics and Gynecology.)

Objective: A study was undertaken to assess the distribution of high-risk HPV-genotypes in high-grade cervical intraepithelial neoplastic lesions in Danish women., Design: Observational, cross-sectional., Setting: Danish data from a multi-centre study undertaken in 13 European countries., Population: 290 archived fixed biopsies with high-grade cervical lesions from the Departments of Pathology at the University Hospitals in Hvidovre and Odense, Denmark., Methods: Relevant histological samples were anonymized and shipped to a central laboratory for histopathology review and PCR-testing for HPV-DNA. A standardised HPV-test methodology was utilised to enable comparison of HPV-genotype distribution., Results: Of 290 Danish cervical samples, 276 were evaluated as histologically adequate and all of these were HPV-positive (HPV⁺). Of the HPV⁺ samples 77.9% were diagnosed with a single HPV-type, with cervical intraepithelial neoplasia (CIN)3 diagnosed in 82.3% and CIN2, CIN2/3, adenocarcinoma in situ (AIS) and AIS⁺ other high-grade lesion diagnosed in the remaining 17.7%. The most prevalent HPV-types were: HPV16 (54.0%), HPV33 (13.5%), HPV31 (10.7%), HPV18 (7.9%) and HPV52 (4.7%). Of the HPV⁺ samples, 21.4% were diagnosed with multiple HPV-types, with CIN3 diagnosed in 79.6% and CIN2, CIN2/3, AIS and AIS⁺ other high-grade lesion diagnosed in the remaining 20.4%. The most prevalent HPV-types were: HPV16 (49.2%), HPV31 (30.5%), HPV52 (27.1%), HPV51 (20.3%), HPV18 (16.9%), HPV33 (13.6%), HPV45 (11.9%), with 0.7% unknown types., Conclusions: HPV16 and HPV18 were detected in approximately 75% of high-grade intraepithelial cervical lesions in a Danish population (single or multiple infections); these two genotypes are considered causative in at least 61.9% of the high-grade intraepithelial lesions (single infection)., (© 2013 Nordic Federation of Societies of Obstetrics and Gynecology.)

Background: Protein S100B has proven to be a useful biomarker for cerebral damage. The predictive ability of S100B may, however, be affected by extracerebral injuries. The aim of this study was to investigate serum levels of S100B in patients with either isolated head injury (IHI), multi trauma with head injury (MTHI), or no head injury (NHI). The primary aim was to assess if a significant difference in serum levels of S100B could be found between IHI and MTHI patients., Methods: Patients (233) were primarily admitted to the trauma centre. Serum samples were drawn on admission and 6 h after trauma and then stored at -80°C until analysed. Variables included Abbreviated Injury Scale (AIS) for head trauma, Injury Severity Score (ISS) and 30-day survival., Results: Two patients could not be classified. IHI occurred in 28, MTHI in 102 and NHI was found in 101. The median S100B concentrations on arrival were 0.47, 1.68 and 0.49 μg/l, respectively (p<0.0001). The corresponding values at 6 h were 0.14, 0.31 and 0.15 μg/l, respectively (p<0.0001). S100B was significantly higher in patients with MTHI than in patients with IHI at both time points (p values 0.0005 and 0.01). There was no significant difference in S100B between patients having IHI and patients with NHI (p=0.81 and p=0.67)., Conclusions: High serum levels of S100B were found early after trauma. The highest concentrations of S100B were found in patients with multi trauma. This suggests that S100B serum concentrations are significantly affected by extracerebral injuries.

Aim: This paper is a report of a study, which examines the moderating effect of collective efficacy on the associations between physical workload, intention to leave and sickness absence., Background: The positive association between physical workload and both intention to leave and sickness absence in the healthcare sector is well-established. However, knowledge is limited with respect to how social contextual factors such as collective efficacy moderate these associations., Methods: A questionnaire was distributed over a 4-month period in 2006/2007 to all employees at elderly care centres in 35 Danish municipalities who were asked to rate their groups' collective efficacy. The final sample consisted of 6929 female employees from 290 work groups. Employees were predominantly healthcare helpers and assistants, but also nurses and other healthcare professionals participated in the study. Information on collective efficacy was aggregated to work group level., Results: Collective efficacy moderated the association between physical workload and intention to leave, i.e. employees with high levels of physical workload had lower intention to leave if they worked in a group with high levels of collective efficacy and higher intention to leave if they worked in a group with low levels of collective efficacy. No moderating effect of collective efficacy on the association between physical workload and sickness absence was found., Conclusion: Our findings suggest that enhancing the sense of collective efficacy might be a mean for organizations and managers to reduce intention to leave, but further research is needed., (© 2011 Blackwell Publishing Ltd.)

Objective: The aim of this study was to explore the screening histories of all cervical cancers in a Danish screening population. The intention was to decide suboptimal sides of the screening program and to evaluate the significance of routine screening in the development of cervical cancer., Methods: The study describes the results of a quality control audit, performed on all new cervical cancer cases diagnosed in the years 2008-2009 at two major Danish screening-centers. All relevant cytological and histological cervical samples were reviewed., Results: 202.534 cytological samples were evaluated in the study period, while 112 women were diagnosed with cervical cancer. The histological diagnoses comprised: 62 (55.4%) squamous cell carcinomas, 20 (17.9%) microinvasive squamous cell carcinomas, 25 (22.3%) adenocarcinomas and 5 cancers of different histology. The mean age of study subjects was 46.6 years. 51 (45.5%) women had deficient screening histories, while 45 (40.2%) women had followed the screening recommendations and had normal cervical samples in review. 11 (9.8%) women were diagnosed with false negative cytology, 2 women had false negative histological tests, while pathological review was not feasible for 3 subjects., Conclusions: More than 45% of the cervical cancer cases in our study were due to deficient cervical screening, stressing the importance of increasing the screening-uptake and coverage. 40% interval cancers emphasize the relevance of further cervical testing of women with relevant symptoms, despite of prior normal cervical samples. Background and Purpose: Integrated rehabilitation (IR) in patients with stroke with respect to death rate and feasibility, initiated as a reduced death rate, was observed in patients with angina pectoris receiving IR., Design: A case-control study included 73 consecutive patients with ischemic stroke. Death rates were compared with those of the general Danish population matched for age, gender, and observation period, as well as data from the community-based Copenhagen Stroke Study., Interventions: IR was conducted in an outpatient clinic, by professionals as well as by the patient: the former as a specific acupuncture treatment, the latter as a comprehensive biofeedback guided stress management program including diets, physical- and relaxation exercise, Chinese health philosophy, cognitive and mindfulness-related exercises, and specific biofeedback guided acupressure., Results: The 4(1/2)-year accumulated risk of death was 11.6% (95 confidence limits: 3.2%-20.0%) for the 73 patients with stroke treated with IR, compared to 18.4% for the general Danish population matched for sex, age, and time period. The corresponding figures for patients receiving conventional stroke treatment were 43.2% (95 confidence limits: 39.7%- 46.7%), and 20.0% for the general Danish population matched for sex, age, and time period., Conclusions: IR was found to be feasible for patients with stroke as a complementary treatment to conventional stroke treatment, and added no risk of dying when compared to Danish stroke patients receiving conventional medical treatment. The results invite further testing in a randomized trial.

Health care workers have a high prevalence of sickness absence because of low back pain (LBP). This study examined whether fear-avoidance beliefs (FAB): 1) predicted sickness absence and 2) moderated the association between LBP and sickness absence among 1724 newly educated health care workers with LBP >or= day during the previous 12 months. High FAB was associated with sickness absence days 1 year later (relative risk, 1.45, 95% confidence interval = 1.24 to 1.70), controlled for LBP, previous sickness absence, and age. When controlling for work environmental factors, the association remained significant but decreased. Furthermore, the results showed that health care workers with high degree of LBP would have more sickness absence days if they also had high FAB. To reduce sickness absence, education about pain management must be presumed to be beneficial.

Objective: Iatrogenic ureteral injury during pelvic surgical procedures is a well-known complication and important cause of morbidity. The authors investigated the circumstances surrounding registered ureteral injuries in order to identify potential opportunities to prevent such injuries., Design: Evaluation of claims concerning ureteral injuries reported to the Danish Patient Insurance Association., Setting: Danish Patient Insurance Association., Sample: All registered claims for ureteral injuries from 1996 to 2006., Methods: Retrospective study of medical records and data from Danish Patient Insurance Association., Main Outcome Measures: Preventable ureteral injuries., Results: From 1996 to 2006, 136 submitted claims concerning ureteral injuries were registered. Among these, 73 claims were approved (54%), and compensation paid. In 44 of these, the injury was caused by negligence. Failure to dissect the ureter despite indications for this procedure was the most common type of negligence. Laparotomy procedures were associated with 107 injuries (79%) and 29 injuries (21%) were caused during laparoscopic procedures. Thirty-four patients suffered from chronic renal dysfunction on the affected side. Only 17 of the ureteral injuries were discovered during the procedure., Conclusions: Forty-four ureteral injuries could potentially have been avoided using established surgical practices, most importantly by exposing the ureter via dissection when indicated. Most of the ureteral injuries were discovered postoperatively.

Purpose: Maximal muscle contraction force and muscle mass are both reduced during the natural aging process. Long-term training may be used to attenuate this age-related loss in muscle function and muscle size., Methods: Maximum isometric quadriceps strength (MVC), rate of force development (RFD), and muscle fiber composition and size (CSA) were studied in elderly individuals (68-78 yr) chronically exposed (> 50 yr) to either endurance (E) or strength (S) training, and in age-matched, untrained (U) elderly group., Results: E and S showed greater MVC than did U. Contractile RFD was elevated in S compared with U, and S also demonstrated greater type II fiber CSA than did U and E. The proportion of type I fibers was greater in E compared with U and S., Conclusions: Muscle fiber size and mechanical muscle performance, particularly RFD, were consistently elevated in aged individuals exposed to chronic (i.e., lifelong) strength training. This relative preservation in muscle morphology and function may provide an important physical reserve capacity to retain muscle mass and function above the critical threshold for independent living at old age.

Physical fitness and muscular strength are important performance factors for Olympic class sailors, but the physical demands vary greatly between yacht classes, and limited information is available regarding the physical demands for the different crew positions. In the present paper, strength and aerobic capacity data from elite Olympic sailors are presented and compared with previous findings. Furthermore, a system for classification of Olympic class sailors is suggested. Peak aerobic capacity (peak oxygen uptake, VO(2peak)) and maximal isometric and isokinetic muscle strength of the knee extensors and flexors were assessed, together with the hamstring/quadriceps strength ratio (H/Q ratio). Peak aerobic capacity (ml O(2) . min(-1) . kg(-2/3)) was as follows: males - static hikers (n = 5) 215, s = 7; dynamic hikers (n = 8) 252, s = 17; trapezing helmsmen (n = 6) 234, s = 15; trapezing crew (n = 10) 239, s = 16; females - dynamic hikers (n = 6) 194, s = 16; trapezing crew (n = 2) 200, s = 13. Strength data for hikers, presented as peak moments (normalized to body weight) obtained during eccentric, isometric, and concentric contraction (Nm . kg(-1)) respectively were as follows: males - quadriceps: 3.66 (s = 0.68), 3.97 (s = 0.66), 1.82 (s = 0.34); hamstrings: 1.93 (s = 0.22), 1.38 (s = 0.41), 1.05 (s = 0.21); females - quadriceps: 3.84 (s = 0.71), 3.81 (s = 0.58), 1.60 (s = 0.28); hamstrings: 1.75 (s = 0.23), 1.10 (s = 0.16), 0.84 (s = 0.13). The peak moment based H/Q ratios for slow eccentric and concentric contractions were 0.42 (s = 0.11) and 0.39 (s = 0.04) for males and 0.43 (s = 0.06) and 0.39 (s = 0.04) for females respectively. Elite Olympic class sailors demonstrated high VO(2peak) values comparable to those observed in other non-endurance sports. The strength data revealed very high quadriceps strength for hikers, which is likely a result of the high muscle forces encountered during sailing, and a low H/Q ratio. To ensure optimal knee joint stabilization during sailing and other training activities, it is suggested that hikers should counter this strength imbalance by performing additional strength training for the hamstrings muscle group.

Objective: Good metabolic control, careful monitoring of late diabetic complications and early intervention against cardiovascular risk factors improve the prognosis in Type 1 and Type 2 diabetic patients. The aim of the present study was to analyse the treatment quality in a routine outpatient clinic., Design: A descriptive survey of unselected outpatient Type 1 diabetic patients followed from 1995 to 1997., Setting: The outpatient clinic at Steno Diabetes Center, Denmark., Subjects: 2011 unselected Type 1 diabetic patients, representatives for the County of Copenhagen, Denmark., Main Outcome and Measures: Levels of metabolic control, screening frequencies for late diabetic complications and cardiovascular risk factors., Results: The mean age (SD) and diabetes duration of the patients were 49 (16) (SD) and 20 (13) years, respectively, and 54% of the patients were males. Glycosylated haemoglobin A(1c) (HbA(1c)) was measured at least once yearly in >99.5% of the patients, and the annual number of measurements was 3 pt(-1). The mean HbA(1c) (normal 4.1-6.4%) was 8.8, 8.7 and 8.6% in 1995, 1996, 1997, respectively. The frequency of HbA(1c)<140% of the non-diabetic mean (HbA(1c)<7.5%) was 14.6, 15.4 and 17.7% in the 3 years (P<0.05). Urinary albumin excretion was measured in 87.0, 87.1 and 80.5%, and blood pressure in 71.4, 76.7 and 79.8% in the 3 years, respectively. In the 3-year period, foot examination and fundus photography were performed at least once in 84.7 and 88.6%, respectively., Conclusions: in unselected Type 1 diabetic patients followed at the outpatient clinic at Steno Diabetes Center, only some, but not all, important clinical variables are measured at high frequency despite standard operation procedures. Continuous collection of data may help to explain why there is such a difference.
