Objective To investigate the imaging diagnosis and differential diagnosis methods for common tumors in the posterior fossa of children, and to propose a flow chart for the diagnosis of common tumors in the posterior fossa of children based on the limited diffusion of DWI according to the idea localization diagnosis followed by qualitative diagnosis. Methods A total of 118 pediatric patients with posterior fossa tumors who underwent surgical resection in Huashan Hospital, Fudan University from January 2021 to January 2024 were enrolled, and all of them underwent head CT and MRI examinations, and preoperative localization diagnosis and qualitative diagnosis were carried out accordingly. Then, based on whether DWI was diffusion limited, a flow chart for the diagnosis of common tumors in the posterior fossa of children was proposed. Results A total of 118 children with posterior fossa tumors were localization and qualitatively diagnosed, including 41 cases of fourth ventricular tumors, including medulloblastoma (27 cases), pilocytic astrocytoma (7 cases), ependymoma (5 cases), choroid plexus papilloma (one case), and rosette-forming glioneuronal tumor (RGNT, one case). There were 38 cases of brainstem tumors, including 24 cases of diffuse midline glioma, H3 K27 - altered, 5 cases of pilocytic astrocytoma, 3 cases of cavernous hemangioma, 2 cases of pediatric-type diffuse high-grade glioma (prone to diffuse midline glioma, H3 wild type), 2 cases of ganglio glioma, one case of atypical teratoid/rhabdoid tumor (AT/RT), and one case of pediatric - type diffuse low - grade glioma. There were 9 cases of cerebellopontine angle (CPA) tumors, including 3 cases of pilocytic astrocytoma, 2 cases of cholesteatoma, one case of medulloblastoma, one case of diffuse midline glioma, H3 K27-altered, one case of pilomyxoid astrocytoma, and one case of Ewing sarcoma. There were 30 cases of cerebellar tumors, including 15 cases of pilocytic astrocytoma, 7 cases of medulloblastoma, 2 cases of cavernous hemangioma, one case of ependymoma, one case of pediatric-type diffuse low-grade glioma, one case of pediatric-type diffuse high- grade glioma (prone to diffuse midline glioma, H3 wild type), one case of dysembryoplastic neuroepithelial tumor (DNT), one case of hamartoma, and one case of extrarenal rhabdomyomatoid tumor cerebellar metastases. Localization diagnosis, the accuracy of preoperative CT was 93.22% (110/118), while the accuracy of preoperative MRI was 100% (118/118). Qualitative diagnosis, 74 cases (62.71%) had accurate qualitative diagnosis, 23 cases (19.49%) had general diagnosis, and 21 cases (17.80%) had wrong diagnosis. The imaging differential diagnosis of posterior fossa tumors in children focuses on medulloblastoma, pilocytic astrocytoma, ependymoma, and diffuse midline glioma, H3 K27-altered. The incidence of limited diffusion of DWI in medulloblastoma, pilocytic astrocytoma and ependymoma outside the brainstem was 100% (35/35), 4% (1/25) and 5/6, and the difference among the three tumors was statistically significant (Z = - 5.601, P = 0.000). The incidence of limited diffusion of DWI in diffuse midline glioma, H3 K27- altered and pilocytic astrocytoma in the brainstem was 79.17% (19/24) and 1/5, and the difference between the two tumors was also statistically significant (Fisher's exact possibility: P = 0.038). Prepubertal testicular teratomas are rare tumors with limited practical guidance for their management. This study aimed to analyze a large multicenter database to establish the optimal management of testicular teratomas. We retrospectively collected data on testicular teratomas in children younger than 12 years who underwent surgery without postoperative chemotherapy in three large professional children's institutions in China between 2007 and 2021. The biological behavior and long-term outcomes of testicular teratomas were analyzed. In total, 487 children (with 393 mature teratomas and 94 immature teratomas) were included. Among mature teratomas, 375 cases were testis-sparing, 18 were orchiectomies, 346 were operated through the scrotal approach, and 47 underwent the inguinal approach. The median follow-up period was 70 months, and no recurrence or testicular atrophy was observed. Among the children with immature teratomas, 54 underwent testis-sparing surgery, 40 underwent orchiectomy, 43 were operated through the scrotal approach, and 51 were operated through the inguinal approach. Two cases of immature teratomas with cryptorchidism had local recurrence or metastasis within 1 year of the operation. The median follow-up duration was 76 months. No other patients had recurrence, metastasis, or testicular atrophy. Conclusion: Testicular-sparing surgery is the first treatment choice for prepubertal testicular teratomas, with the scrotal approach being a safe and well-tolerated strategy for these diseases. Additionally, patients with immature teratomas and cryptorchidism may have tumor recurrence or metastasis after surgery. Therefore, these patients should be closely followed up in the first year after surgery. Objective. To study the clinical characteristics and surgical outcomes of anti-NMDAR encephalitis and the immunopathology of associated teratomas. Methods. Twenty-one patients were enrolled in this retrospective study, who were diagnosed with anti-NMDAR encephalitis with ovarian teratoma and admitted to two tertiary hospitals in South China from July 2014 to December 2019. The clinical data of patients were reviewed. Comparisons were made between the patients with different outcomes after surgery. Immunohistochemical analyses of associated ovarian teratomas were performed. Results. The mean age of the patients was 24.33 ± 5.12 years. The peak seasons of disease onset were autumn and winter (30.61% and 32.65%). The symptoms could be divided into 8 categories, including psychiatric abnormalities, seizures, movement dysfunction, consciousness disorders, autonomic dysregulation, speech disturbance, central hypoventilation, and memory deficits. All patients developed four or more categories of symptoms within the first four weeks. Twelve patients (57.1%) had a maximum mRS of 5, and 11 patients (52.4%) were admitted to ICU. Twenty patients received surgery, and only 3 patients were diagnosed pathologically with immature ovarian teratomas, while the other 17 patients had mature ovarian teratomas. After surgery, 17 patients (85.0%) got clinical improvement. The central hypoventilation symptom and mature ovarian teratomas were associated with surgical outcome. Immunohistochemical analysis revealed that there were NMDAR-positive neural tissues in all 8 teratomas and in which 3 cases also contained large numbers of NMDAR-positive sebaceous glands and squamous epithelial tissues. Conclusion. The disease is of high prevalence in autumn and winter. The central hypoventilation symptom and mature ovarian teratomas were associated with surgical outcome. NMDAR-positive neural tissue is not the only etiological factor of encephalitis. Objective: To report the first case series of robotic single-site (RSS) surgery via the da Vinci Si Surgical System for mature cyst teratoma cystectomy in China.Materials and Methods: The study was devised as a retrospective study in a single medical center. Five patients with mature cyst teratomas requested a minimally invasive surgical treatment. These patients were treated with RSS surgery for mature cyst teratoma between January 2014 and January 2015. RSS mature cyst teratoma cystectomies were performed with the da Vinci single-site platform in the Hainan branch of PLA General Hospital. Data regarding patient characteristics, surgical approach, and perioperative clinical outcomes were collected and analyzed in a retrospective study.Results: All RSS procedures were completed successfully in the five patients. No instrument failure was noted during the procedures. The median operating time was 65 minutes (range 45-100 minutes). The median docking time was 20 minutes (range 18-28 minutes). No instrument failure was noted during any surgical procedures. The median blood loss was 30 mL (range 10-70 mL). No patient had massive intraoperative bleeding nor required a transfusion. No extra trocar was placed during the surgery. None of the patients had bladder or rectal injury. The median length of stay in hospital was 2.8 days. All patients were followed up until 6 months postoperatively, and no surgical complication occurred.Conclusion: RSS mature cyst teratoma cystectomy using the wristed semirigid instrumentation is feasible. Randomized controlled trials with a larger number of patients and longer postoperative follow-up should be conducted to further evaluate the effect of this therapeutic strategy. Background: Testis-sparing surgery is not popular in South China. This study aimed to investigate this procedure for pediatric testicular tumors.Methods: Children with testicular benign tumors were retrospectively analyzed from January 2001 to June 2015 in the Sun Yat-sen University Cancer Center (SYSUCC) and the First Affiliated Hospital (SYSU-1st). Follow-up was performed until June 2016, and the proportions of TSS in the two hospitals during the different periods were compared.Results: Forty-seven children with testicular benign tumors were enrolled, and 16 cases underwent testis-sparing surgery. All patients were cured and discharged, which included mature teratoma (n = 37), testicular adrenal rest tumors (n = 4), epidermal cysts (n = 3), granulomatous inflammation (n = 2) and adenomatoid tumors (n = 1). Inguinal testis-sparing surgery was performed in 16 children, and no recurrence was detected during follow-up. It was performed more frequently in SYSUCC than in SYSU-1st (P = 0.031), and the tumor size of these patients was smaller than those of patients who underwent radical orchiectomy (P = 0.044). Moreover, testis-sparing surgery has become more common in the past 5 years, although differences over time have not reached significance (P = 0.051).Conclusions: Testis-sparing surgery is reliable, and tumor size and special hospitals affect its success. Additionally, its use has become more popular in recent years. However, advocacy is still needed for the use of this technique in pediatric testicular benign tumors that are small sized. Background and Purpose: Anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis is a synaptic autoimmune disorder in which the auto-antibodies target NMDARs were first reported in 2007. Now, anti-NMDAR encephalitis is a widely recognized disease in the world. Our purpose was to analyze the clinical characteristics of anti-NMDAR encephalitis patients in West China Hospital in China and report the prognosis of the patients after accepting immunotherapy. Methods: Patients admitted to the West China Hospital who were diagnosed with anti-NMDAR encephalitis from 2013 to 2014 were retrospectively collected and their clinical features were analyzed. Results: We ultimately included 11 anti-NMDAR encephalitis patients. The median age of the 11 patients was 25 years, 46% patients were females, 27% patients were 15-18 years, and 73% patients were over 18 years. One of them was diagnosed with teratoma after discharging from hospital. The application of immunotherapy produced favorable outcomes in 67% patients included in our study. Conclusions: Anti-NMDAR encephalitis should be suspected in patients who developed a rapid change in behavior or psychosis, seizures, dyskinesia, and hypoventilation. Myelin oligodendrocyte glycoprotein (MOG) antibody disease is a rare inflammatory disease of the central nervous system. There are a variety of clinical and imaging manifestations of MOG antibody disease (MOG-AD). At present, there is no report on related cases of MOG antibody positivity complicated with teratoma in China, and one case admitted to our hospital is reported. The aim of this study is to document the clinicopathologic features of teratoid cyst of the floor of the mouth in 20 Chinese patients, who were treated in Wuhan University. The 20 teratoid cysts were analyzed. All cases were confirmed by pathology. English literature was also reviewed. Our investigations showed that both sexes were equal to have these cysts. The age at diagnosis ranged from 25 days to 34 years, and the greatest frequency was within the first decade. It was found that the congenital cysts were most commonly diagnosed in the midline. All cases were treated by cystectomy, using 1 of the 2 approaches: extraoral or intraoral. Whenever, combined approaches were used in 2 cases. In all, 16 patients were followed-up, and 2 patients had recurrences. The intraoral approach was also effective for the treatment of large lesions and led to very good cosmetic and functional results. [ABSTRACT FROM AUTHOR]

Objective : To describe the clinical features, laboratory data, treatment, and outcomes of anti- N -methyl-D-aspartate (NMDAR) encephalitis in Chinese patients. Methods : This retrospective study included hospitalized patients definitively diagnosed with anti-NMDAR encephalitis and positive for anti-NMDAR antibodies in the cerebrospinal fluid (CSF) in Shenzhen People's hospital, between November 2015 and February 2020. The clinical manifestation, laboratory data, treatments and outcomes were collected retrospectively. Patients were followed up for more than 1 year. Results : The study included 31 patients (15 men, 48.4%) with a median age of 31 years (interquartile range 21-48). The most common clinical presentations were psychosis ( n = 23, 74.2%), seizures ( n = 20, 64.5%), and memory impairment ( n = 20, 64.5%). Total magnetic resonance imaging abnormalities were found in 11 patients (35.5%), with the medial temporal and frontal lobes as the most commonly involved. Abnormal electroencephalogram was observed in 16 patients (51.6%). Five out of 31 patients (19.5%) were diagnosed as neoplasm, including five females with ovarian teratoma and one male with a central nervous system tumor. Multiple immune antibodies, including anti-SSA antibody in four patients (15.4%), anti-Ro52 antibody in four (15.4%), antinuclear antibody (ANT) in four (15.4%), anti-thyroglobulin antibodies (TGAb) in five (17.2%), and thyroid peroxidase antibodies (TPOAb) in three (10.3%) were present. All patients received first-line immunization therapy (intravenous immunoglobulin, glucocorticoids, or plasmapheresis alone or combined), and only two patients (7.3%) received second-line immunization therapy (rituximab). Mechanical ventilation was more necessary in women (37.5%) than in men (6.7%) ( p = 0.04), and 29 (93.5%) had favorable clinical outcomes. At more than 12 months of follow-up, the median modified Rankin Scale score decreased from 4 to 0. Conclusions : Patients with anti-NMDAR encephalitis in China had high rates of psychosis and seizures, with low rates of underlying neoplasms. A higher proportion of female patients required mechanical ventilation. Complications with other positive autoimmune antibodies were a common clinical symptoms of anti-NMDAR encephalitis. PURPOSE: To analyze the clinicopathology of the teratoid cyst of the floor of the mouth and the time for surgical treatment. METHODS: The incidence, manifestations, histology, treatment methods and prognosis of 10 patients treated in our department between April 2000 and May 2008 were reviewed. RESULTS: The lesions occurred almost exclusively in neonates and children. It appeared as mobile, cystic solid and well-defined lesions. It frequently caused difficulty in feeding, breathing and even more affecting growth and development because of its special location. Histology indicated that squamous, respiratory and /or gastrointestinal epithelium constituted basic structure of teratoid cyst. Complete surgical extirpation was the only treatment of choice. It was not routinely necessary to do the operation immediately unless the cyst oppressed the airway of the patients. There had been no recurrence of the lesion during follow-up. CONCLUSION: Teratoid cysts have great impact on the patients due to its specific location, although it is a cystic disease. B-type ultrasonography, CT and MRI are helpful to the diagnosis. Introduction: Prepubertal testicular tumors are fundamentally distinct from their postpubertal and adult counterparts. Racial and ethnic differences contribute to the incidence, characteristics, and histological distribution of prepubertal testicular germ cell tumors (PTGCTs)., Objective: To elucidate the characteristics and treatment outcomes of PTGCTs in a Chinese cohort., Study Design: Data were retrospectively reviewed from consecutive PTGCT patients aged <12 years who received treatment at the current institution from January 2007 to December 2015. Exclusion criteria were: patients aged >12 years, non-primary testicular germ cell tumors, and para-testicular tumors., Results: A total of 167 patients qualified for the study (Table). The median age at diagnosis was 20 months (range 1-142). Pathology revealed 87 (50.9%) teratomas, 50 (29.2%) yolk sac tumors (YSTs), 33 (19.3%) epidermoid cysts, and one (0.6%) mixed germ cell tumor. Of the teratomas, 70 (80.5%) mature teratomas (MTs) and 17 (19.5%) immature teratomas (ITs) were detected. ITs manifested at a younger age and were larger in size compared with MTs (P < 0.001). Testis-sparing surgery (TSS) was performed for all MTs and 10 ITs. Radical orchiectomy (RO) was performed for seven ITs. No metastatic or local recurrence was detected in benign tumors during follow-up. Obviously elevated alpha fetoprotein (AFP) levels were detected in all the YSTs. Of these, 47 (94%) were diagnosed as Stage I, one (2%) as Stage II, and two (4%) as Stage IV. After RO, recurrence was detected in one Stage I patient with a median follow-up of 46 months. One Stage II and two Stage IV patients died due to tumor progression., Discussion: Racial and ethnic differences were detected in PTGCTs. It is believed that the present study is the largest study detailing the clinicopathologic characteristics and treatment outcomes of PTGCTs in a Chinese cohort., Conclusions: Teratoma was the most common subtype of PTGCTs, followed by YST. ITs manifested as benign behavior and were more likely to be present at a younger age and have a larger tumor size compared with MTs. TSS was reliable for benign lesions. For Stage I YST, RO alone was valid. Salvage chemotherapy was effective and preventative retroperitoneal lymph node dissection may not be necessary for YST. The article presents a case study on the diagnosis of spontaneous rupture of a large intracranial teratoma to a 16 year old female patient in China. The patient was recommended to undergo computed tomography (CT) to be able to determine the cause of headache and dizziness. The findings confirm the occurrence of teratoma due to the detection of a large posterior lesion in the posterior fossa.
