Breast milk acts as an intermediary for the transfer of functionally important commensal bacteria from mother to infant, especially for Bifidobacterium that can colonize the infant gut. However, the vast majority of rRNA amplicon-based studies reported the conspicuous intercohort and interindividual variation for the prevalence of Bifidobacterium in breast milk. In order to elucidate whether Bifidobacterium phylotypes persistently co-occured at the species or strain level in mother–breast milk–infant triads, we analyzed collectively the next-generation sequencing (NGS) datasets of bacterial 16S rRNA gene and the Bifidobacterium -specific gro EL gene from maternal feces, breast milk, and infant feces in a small yet very homogeneous cohort of 25 healthy Uyghur mother–infant pairs (lactation for 7–720 days) in Kashgar, Xinjiang, China. Overall, 16S rRNA gene analysis showed that microbiome in the newborn gut was closer to that of breast milk in the first 4 months of lactation, and subsequently showed an obvious trend of adulthood at 6–12 months. Based on the BLAST accurate taxonomic result of the representative sequences of all ASVs (amplicon sequencing variants), only three sets of ASVs could be clearly assigned into Bifidobacterium species, whereas the remaining eight sets of ASVs corresponded to four indefinite Bifidobacterium species group. By contrast, the groEL gene dataset was partitioned into 376 ASVs, at least belonging to 13 well-known Bifidobacterium species or subspecies, of which 15 ASVs, annotated to seven well-known Bifidobacterium species or subspecies, showed triadic synchronism in most 23 mother–infant pairs tested. However, several other rare bifidobacterial phylotypes, which were frequently encountered in animals, were found to display no correspondence of the presence between the three ecosystems of mother–infant pairs. Our test results were obviously to support the hypothesis that breast milk acts as an intermediary for the transfer of probiotic commensal bacteria from mother to infant, especially for endosymbiotic Bifidobacterium that can colonize the infant gut. Some oxygen-insensitive exogenous Bifidobacterium phylotypes with a cosmopolitan lifestyle may be indirectly transferred to breast milk and the infant's intestinal tract through environmental contamination. Exposure to heavy metals may affect a vast array of biochemical and nutritional processes in the human body. The toxic manifestations of these metals are caused primarily due to the imbalance between pro-oxidant and antioxidant homeostasis which is termed as oxidative stress. A high-performance liquid chromatography–chemiluminescence (HPLC-CL) method was developed for the simultaneous determination of two antioxidants, glutathione and cysteine, in human breast milk. The HPLC system included a C18 analytical column with a mobile phase composed of 1.0% methanol. The aminothiols were eluted from the C18 column, mixed with formaldehyde and potassium permanganate solutions, and subsequently monitored by a chemiluminescence detector. The new method was validated by monitoring glutathione and cysteine in human breast milk of volunteers from the Gusu and Industrial Districts of Suzhou, China. Four heavy metals in atmospheric particulate matter collected from the two districts were also determined by inductively coupled plasma–mass spectrometry (ICP-MS). The results demonstrated that the concentrations of cysteine and glutathione were significantly (p < 0.05) lower in human breast milk from the subjects living in the Gusu region of Suzhou, Jiangsu Province, China compared to an industrial district. These results were inversely correlated with the concentration levels of the toxic metals in collected atmospheric particulate matter. This study quantified the fatty acid profile with emphasis on the stereo-specifically numbered (sn) 2 positional distribution in TAG and the composition of main phospholipids at different lactation stages. Colostrum milk (n 70), transitional milk (n 96) and mature milk (n 82) were obtained longitudinally from healthy lactating women in Shanghai. During lactation, total fatty acid content increased, with SFA dominating in fatty acid profile. A high ratio of n -6: n -3 PUFA was observed as 11:1 over lactation due to the abundance of linoleic acid in Chinese human milk. As the main SFA, palmitic acid showed absolute sn -2 selectivity, while oleic acid, linoleic acid and α -linolenic acid, the main unsaturated fatty acids, were primarily esterified at the sn -1 and sn -3 positions. Nervonic acid and C22 PUFA including DHA were more enriched in colostrum with an sn -2 positional preference. A total of three dominant phospholipids (phosphatidylethanolamine (PE), phosphatidylcholine (PC) and sphingomyelin (SM)) were analysed in the collected samples, and each showed a decline in amount over lactation. PC was the dominant compound followed by SM and PE. With prolonged breast-feeding time, percentage of PE in total phospholipids remained constant, but PC decreased, and SM increased. Background and Objectives: As breast milk is considered nature's best food for infant growth and development, understanding its nutritional composition is crucial for optimising the components of infant formula milk. In this study, we aimed to summarise the available evidence on the nutritional composition of breast milk from Chinese women, in particular, the macronutrients, fatty acids and minerals.Methods and Study Design: We searched PubMed, Embase, and Chinese databases for articles about human breast milk from Chinese women published in English or Chinese between 1999 and 2015. We classified the data in 35 articles from the literature search into three lactation stages: colostral, transitional and mature milk.Results: The content of each component varied greatly during the three lactation stages. Protein content decreased from colostral milk to mature milk (mean±SD, 1.64±0.32 g/dL vs 1.22±0.12 g/dL). In contrast, lipid content increased from colostral milk (2.36±1.17 g/dL) to mature milk (3.39±1.24 g/dL). Colostrum contained more linoleic acid (LA) than transitional and mature milk, while colostrum contained less α-linolenic acid (ALA) than transitional and mature milk. As lactation progressed, the ratios of docosahexaenoic acid (DHA) and arachidonic acid (AA) to total fatty acids decreased while the potassium (K), zinc (Zn), and copper (Cu) concentrations decreased significantly, but their standard deviations were large. Magnesium (Mg) and manganese (Mn) concentrations showed significant differences across the three lactation stages.Conclusions: The stage of lactation was an important factor affecting the nutritional composition of breast milk from Chinese women. Chlorinated paraffins (CPs) are complex mixtures of synthetic chemicals found widely in environmental matrices. Short-chain CPs (SCCPs) are candidate persistent organic pollutants under the Stockholm Convention. There should be great concern about human exposure to SCCPs. Data on CP concentrations in human breast milk is scarce. This is the first study in which background SCCP and medium-chain CP (MCCP) body burdens in the general rural population of China have been estimated and health risks posed to nursing infants by CPs in breast milk assessed. The concentrations of 48 SCCP and MCCP formula congeners were determined in 24 pooled human milk samples produced from 1412 individual samples from eight provinces in 2007 and 16 provinces in 2011. The samples were analyzed by comprehensive two-dimensional gas chromatography electron capture negative ionization high-resolution time-of-flight mass spectrometry. The median SCCP and MCCP concentrations were 303 and 35.7 ng g − 1 lipid weight, respectively, for the 2007 samples and 360 and 45.4 ng g − 1 lipid weight, respectively, for the 2011 samples. The C 10 and C 14 homologs were the dominant CP carbon-chain-length groups, contributing 51% and 82% of the total SCCP and MCCP concentrations, respectively. There are probably multiple CP sources to the general Chinese population and numerous exposure pathways. The median estimated daily SCCP and MCCP intakes for nursing infants were 1310 and 152 ng kg − 1 d − 1 , respectively, in 2007 and 1520 and 212 ng kg − 1 d − 1 , respectively, in 2011. SCCPs do not currently pose significant risks to infants in China. Human breast milk monitoring programs were recommended to be carried out to assess human body burden to persistent organic pollutants and their temporal trends. Thus, we conducted a national survey during the period of 2016-2019 to determine PCDD/Fs and dl-PCBs in human breast milk from China. The total TEQ amounts in the upper bound (UB) was in the range of 1.97 to 15.1 pg TEQ g -1 fat with a geometric mean (GM) of 4.50 pg TEQ g -1 fat. 2,3,4,7,8-PeCDF, 1,2,3,7,8-PeCDD, and PCB-126 were more predominant contributors with the percentage contribution of 34.2 %, 17.9 %, and 17.4 %, respectively. By comparison with our previous monitoring results, the total TEQ in breast milk sample of the present study is statistically lower than that in 2011 with a reduction of 16.9 % in the average (p < 0.05), and comparable to that in 2007. The estimated GM of dietary intake of total TEQ for the breastfed was 25.4 pg TEQ kg -1 body weight day -1 that was higher than that of the adult. Background The present study evaluated the relationship between dietary fatty acid ( FA) intakes and human milk FA levels. Methods Healthy lactating women ( n = 514) from Northern China participated in the study. Dietary intake was assessed with a 24-h dietary recall questionnaire and evaluated using golden key maternal nutrition software (Wincome, Shanghai, China) and China Food Composition 2009. Human milk FA composition was determined by gas chromatography. Results The maternal daily median intakes of linoleic acid ( LA), α-linolenic acid ( ALA) and arachidonic acid ( AA) were 19.93 g, 3.08 g and 16.33 mg, respectively. Eicosapentaenoic acid ( EPA) and docosahexaenoic acid ( DHA) intakes were below the recommended levels. FA levels in 100 g of human milk were 0.363 g LA, 0.038 g γ-linolenic acid ( GLA), 0.052 g dihomo γ-linolenic acid ( DGLA), 0.144 g ALA, 0.079 g AA, 0.007 g EPA, 0.018 g docosatetraenoic acid ( DTA) and 0.048 g DHA. Multiple linear regression analysis revealed that human milk DGLA levels were negatively correlated with dietary LA intake (β = −0.223, P = 0.030), and human milk GLA and DTA levels were negatively correlated with dietary ALA intake (β = −2.189, P = 0.031; β = −2.252, P = 0.027) after adjusting for possible confounding factors. Human milk provides infants with various immune molecules. The objective of the present study was to measure human β-defensin-1 (hBD-1) and human β-defensin-2 (hBD-2) levels in the colostrum and mature milk of healthy Han Chinese, to identify factors regulating milk hBD-1 and hBD-2 expression and to explore the potential protective effect of milk hBD-1 and hBD-2 on infants. A total of 100 mothers and their babies were recruited into the study. Sociodemographic characteristics and other factors were obtained by a questionnaire. Babies were followed up for a period of 6 months. Colostrum samples (n 100) and mature milk samples (n 82) were collected by hand expression. The hBD-1 and hBD-2 concentrations were measured by ELISA. The hBD-1 and hBD-2 levels differed in the colostrum and mature milk. In the colostrum, the concentration ranges of hBD-1 and hBD-2 were 1·04–12·81 μg/ml and 0·31–19·12 ng/ml, respectively. In mature milk, the hBD-1 and hBD-2 levels were 1·03–31·76 ng/ml and 52·65–182·29 pg/ml, respectively. Several independent factors influence their production. The multivariable analysis showed a strong association between pre-pregnancy BMI and hBD-1 levels in the colostrum (P= 0·001), mode of delivery was significantly associated with hBD-2 levels in the colostrum (P= 0·006) and gestational age was significantly associated with hBD-1 levels in mature milk (P= 0·010). During the first 6 months of life, the incidence rate of upper respiratory infection was found to be less in the high-colostrum hBD-1 group than in the low-colostrum hBD-1 group (χ2= 4·995, P= 0·025). The present study suggested that the abundance of hBD-1 in the colostrum may have a protective function against upper respiratory infection for infants younger than 6 months. Breast milk samples were collected from 51 mothers in a seaside city Dalian, where the residents usually have higher dietary exposure to polychlorinated dibenzo- p -dioxins and dibenzofurans (PCDD/Fs) and polychlorinated biphenyls (PCBs) due to the larger consumption of seafood. The lipid-basis concentrations of ∑Cl 2–8 DD/Fs, ∑Cl 2–10 Bs, and total toxicity equivalent (WHO-TEQ) were measured to be in the ranges of 35.7–2727.8 pg/g, 4.91–52.64 ng/g, and 2.27–36.30 pg/g, respectively. The average proportion of ∑Cl 2–3 DD/Fs was higher than that of ∑Cl 4–7 DD/Fs in the collected human breast milk samples, suggesting that the health risk of Cl 2–3 DD/Fs should be especially concerned. The concentration data of PCDD/Fs and PCBs in human breast milk essentially followed a positive skew probability distribution. Women in high-level exposure scenarios exhibited a higher potential to accumulate homologues Cl 4 DFs, Cl 7 DFs, Cl 8 DF, and Cl 6 Bs in breast milk. Three PCDD/F congeners (1,2,3,6,7,8-Cl 6 DF, 1,2,3,4,7,8-Cl 6 DF, and 1,2,3,4,6,7,8-Cl 7 DD) and three PCB congeners (PCB 126, PCB 138, and PCB 169) were identified as good indicators for the accumulation of PCDD/Fs and PCBs in human breast milk, respectively. The food-to-milk accumulation factors (FMAF) were calculated to evaluate the accumulation potentials of different PCDD/F and PCB congeners in human breast milk via dietary exposure. The calculated FMAF value presented a non-monotonic variation with the logarithm of n -octanol–water partition coefficient (log K OW) with a peak at a log K OW value of about 7.3 and a valley at a log K OW value of about 8. The mean value of the estimated daily intake (EDI) of total WHO-TEQ for breast-fed infants in Dalian, predicted by Monte Carlo simulation, was 10 folds higher than the upper range of the tolerable daily intake (TDI) value (4 pg WHO-TEQ/kg bw/d), suggesting continued and enhanced efforts should be made to reduce the exposure risk of infants to PCDD/Fs and PCBs. Abstract: Exposure of mothers to organochlorine pesticides (OCPs) was assessed by measuring the levels of 20 OCPs in 70 human breast milk samples pooled from 210 individuals from China, Korea and Japan. The OCPs were analyzed using gas chromatography/mass spectrometry (GC/MS) in electron capture negative ionization (ECNI) monitoring. The results showed that β-hexachlorocyclohexane and hexachlorobenzene were one order of magnitude higher in China than in the other nations, whereas chlordanes and polychlorinated biphenyl levels were highest in Japan. Heptachlor epoxide, dieldrin, endrin, toxaphenes and mirex were detected in most samples, and levels of these chemicals were significantly higher in Japan (0.8–4.5ngg−1 lipid), followed by Korea (0.2–4.7ngg−1 lipid), and lowest in China (less than 1.0ngg−1 lipid). α- and β-endosulfans were detected at a range of 0.9–1.5ngg−1 lipid levels in all samples analyzed, and their levels were higher in Korean than in Chinese samples. Abstract: Sixty breast milk samples were collected in Shenzhen, China from July to November in 2007. The samples were analyzed of the concentrations of polychlorinated dibenzo-p-dioxins (PCDDs), polychlorinated dibenzofurans (PCDFs) and polychlorinated biphenyls (PCBs). The range of upper-bound for ∑TEQ-(PCDD/Fs+PCBs) in the samples was 4.10–35.3pgTEQg−1 lipid (median: 10.6pgTEQg−1 lipid; mean: 11.9pgTEQg−1 lipid). The levels of the measured contaminants in the breast milk had significant correlations with the length of inhabitation period in Shenzhen (r=0.487, p<0.05 for PCDD/Fs, r=0.431, p<0.05 for PCBs and r=0.478, p<0.05 for ∑TEQ-(PCDD/Fs+PCBs)), and the consumption rate of fish (r=0.366, p<0.05 for PCDD/Fs, r=0.486, p<0.05 for PCBs and r=0.416, p<0.05 for ∑TEQ-(PCDD/Fs+PCBs)), respectively. Moreover, significant positive correlations were also detected between the participant''s age (r=0.305, p<0.05 for ∑TEQ-PCBs and r=0.275, p<0.05 for ∑TEQ-(PCDD/Fs+PCBs)) and the body burdens of these contaminants respectively. It is estimated that the daily intake (EDI) of the sum of PCDD/Fs and DL-PCBs by the breast-fed infants was 5.60–161pgTEQkg−1 bw per day (mean: 48.2pgTEQkg−1 bw per day; median: 42.2pgTEQkg−1 bw per day). The result showed that both the body burdens of PCDD/Fs and PCBs of the recruit population and the calculated EDI of the breast-fed infants were higher than those in the non-exposed areas in mainland China. This suggests that continuous surveillance on PCDD/Fs and PCBs levels in human milk is critical to more precisely evaluate the human health risk posed by the negative environmental impact in Shenzhen in the future. Abstract: In this study, 90 human breast milk samples collected from Japan, Korea, and China were analyzed for perfluorooctanoic acid (PFOA) (C8), perfluorononanoic acid (PFNA) (C9), perfluorodecanoic acid (PFDA) (C10), perfluoroundecanoic acid (PFUnDA) (C11), perfluorododecanoic acid (PFDoDA) (C12), and perfluorotridecanoic acid (PFTrDA) (C13). In addition, infant formulas (n =9) obtained from retail stores in China and Japan were analyzed. PFOA was the predominant compound and was detected in more than 60% of samples in all three countries. The PFOA, PFNA, PFDA, and PFUnDA levels in Japan were significantly higher than those in Korea and China (p <0.05). The PFTrDA level was highest in Korea (p <0.05). The median PFOA concentrations were 89pgmL−1 (48% of total perfluorinated carboxylic acids (PFCAs) (C8–C13)) in Japan, 62pgmL−1 (54%) in Korea, and 51pgmL−1 (61%) in China. The remaining ∑PFCAs (C9–C13) were 95pgmL−1 in Japan, 52pgmL−1 in Korea, and 33pgmL−1 in China. Among the long-chain PFCAs, odd-numbered PFCAs were more frequently detected than even-numbered PFCAs, except for PFDA in Japan. There were no evident correlations between the mother’s demographic factors and the PFCA concentrations. PFOA, PFNA, and PFDA were frequently detected in both Japan and China, but there were no significant differences between the two countries. The total PFCA concentrations in the infant formulas were lower than those in the breast milk samples in Japan (p <0.05), but not in China (p >0.05). Aim: The objective of this study was to measure and compare the macro- and micronutrient composition of breast milk in urban and suburban lactating mothers living in Shanghai, China. Methods: Primiparous women residing in three urban regions ( n= 30/region) and a suburban area of Shanghai ( n= 30) were recruited. Breast milk samples were collected 8–10 days postpartum and dietary intake was determined. Results: Women from the suburban area were of lower socio-economic status as indicated by levels of education, occupation and income. With the exception of carbohydrates, mothers in all urban areas had significantly higher macronutrient intake levels than suburban area mothers. Sodium, potassium, phosphorus and calcium intake levels were significantly lower in suburban compared to all urban women. Concentrations of protein, lipids, copper, sodium, potassium, chlorine, zinc, manganese, phosphorus and iron were all significantly lower in milk obtained from suburban compared to urban women. Concentrations of carbohydrates in milk from suburban women were significantly higher than concentrations in than two of the three urban groups. Offspring from women in Chongming county gained significantly less weight between birth and 6 months than did offspring born to women from the three other counties. Conclusions: In summary, there were significant differences in macro- and micronutrient intake levels and breast milk concentrations between women from urban and suburban Shanghai. The breast milk concentrations of polybrominated diphenyl ethers (PBDEs; sum of eight congeners: BDE-28, 47, 99, 100, 153, 154, 183 and 209) were determined (by high resolution gas chromatographic high resolution mass spectrometry) in samples from primiparous women collected in 2006 in Tianjin, China. Dietary and lifestyle habits of the participants were obtained by questionnaires. The median total PBDE concentration (including BDE-28, 47, 99, 100, 153, 154, 183) was 2500pgg−1 lipid weight, ranging from 1700 to 4500pgg−1 lipid weight. These levels were in the same range as those from Europe and Japan but much lower than those from North America. The congener profile in China was also different from other countries. The contribution of BDE-28 was around 15%, much higher than any other country, implying that a different Penta-BDE formulation might be used in the Chinese market. The lower ratio of the sum of BDE-47, 99, and 100 to the sum of BDE-153 and 154 suggested that Octa-products were in more demand in China. A significant correlation was found between a woman’s PBDE concentration and the time she used electronic appliances (h/d). This implies that electronics are a potential source of PBDEs to people. Inhalation or ingestion of particulate matter (such as dust) may also be an important exposure pathway. There was no significant correlation between the PBDE concentration and the consumption of meat, fish, and milk. Fourteen PBDE congeners from mono- to deca-BDE were determined in breast milk of primiparous mothers from two locations in East China, i.e. Nanjing (n =9), an urban area, and Zhoushan (n =10), a semi rural coastal area. PBDEs were detected in all the human breast milk samples of the present study, indicating that general population in these two locations are widely exposed to these pollutants. Relatively higher concentrations of PBDEs were found in the milk of mothers from Nanjing than Zhoushan, suggesting the existence of significant sources of PBDEs in urban areas. PBDE levels in the present study were similar to those in European countries, but one or two orders of magnitude lower than in North America. Except for BDE-3, all congeners from di- to deca-BDE were detected in the samples of the present study. BDE-209, a congener considered to have less bioavailability, was detected in about 50% of the samples at concentrations higher than that of other congeners. Other higher brominated congeners, such as BDE-153, -197 and -207, were also prominent in the present study, which is different from the pattern generally observed in previous studies on human milk as well as biota samples. These results may indicate that the inhabitants of Nanjing and Zhoushan are exposed to location specific sources of PBDEs. [Copyright &y& Elsevier]

The diet is the main route that polycyclic aromatic hydrocarbons (PAHs) enter the body and measuring breast milk is one of the best ways to understand the maternal body burden and can be passed on to infants. In this study, it was determinate the concentrations of 23 PAHs in 60 milk samples taken from 3 cities in Colombia and to determine the potential routes of exposure and risk to human health. On average, concentration for the ∑ PAHs across all locations was 186.6 ng g -1 , lipid mass (LM), with city means of 260.1, 175.7, and 123.9 ng g -1 LM for Cartagena, Bogota and Medellin, respectively. Monte Carlo simulations were used to estimate the hazard quotient (HQ) and incremental lifetime cancer risk (ILCR) for infant dietary exposure to PAHs. HQs were below the safe thresholds (HQ = 1) while ILCRs were greater than the reference value equal to 10 -6 (mg kg -1 day -1 ). Dietary source assessment indicated that fish is a significant source of PAHs, with mothers that consumed fish at least once per week having ∼2.5 times greater PAH milk concentrations than other groups. While a disparity was also observed among consumers of exclusively marine (∑ PAHs 198.5 ng g -1 LM) or freshwater fish (∑ PAHs 85.7 ng g -1 LM). Perfluoroalkyl carboxylates (PFCAs) and perfluoroalkyl sulfonates are widespread in human breast milk. However, the occurrence of chlorinated polyfluorinated ether sulfonates (Cl-PFESAs) and fluorotelomer alcohols (FTOHs) in breast milk and their effects on postnatal growth of infants through breast milk consumption are still not well known. This study characterized the occurrence of 16 poly- and perfluoroalkyl substances (PFASs) in breast milk from 174 women in Hangzhou, China and investigated the association between lactation exposure to these PFASs through breast milk consumption and the postnatal growth of infants. Our results showed that perfluorooctanoate (mean 87 pg/mL) was the predominant PFAS in breast milk, followed by perfluorohexanoate (41 pg/mL), 6:2 Cl-PFESA (28 pg/mL), and perfluorooctane sulfonate (25 pg/mL). The occurrence and levels of Cl-PFESAs in Chinese breast milk were firstly reported in the current study. The 8:2 and 10:2 FTOH were detected in half of breast milk samples, with the mean concentration of 9.0 pg/mL and 10 pg/mL, respectively. Breast milk concentrations of C 8 -C 10 PFCAs and 6:2 Cl-PFESA were negatively correlated with infant's length gain rate. Exposed to higher levels of 8:2 FTOH were correlated with decreased infant's weight gain rate. Daily intakes of PFASs via the consumption of breast milk were calculated for infants. Many studies show that bisphenol A (BPA) is widespread in human breast milk. However, the occurrence of other bisphenol analogues (BPs), including bisphenol S (BPS), bisphenol F (BPF), and bisphenol AF (BPAF), in breast milk is still not well known.
