Background: Passively acquired maternal antibodies are necessary to protect infants against circulating measles virus until they reach the eligible age of vaccination. Likewise, high levels of population immunity must be achieved and maintained to reduce measles virus transmission. This study was undertaken to (1) assess the presence of maternally acquired measles-specific IgG antibodies among infants less than 9 months of age in Bangui, Central African Republic and (2) determine the immune status of vaccination-age children and the concordance with reported vaccination status. A secondary objective was to describe the presence of rubella-specific IgG antibody in the study population.Methods: Vaccination history and blood samples were collected from 395 children using blotting paper. Samples were analyzed for the presence of measles-specific IgG antibodies using commercial ELISA kits.Results: Measles-specific IgG antibodies were detected in 51.3% of vaccinated children and 27.6% of non-vaccinated children. Maternally derived measles IgG antibodies were present in only 14.8% of infants aged 0-3 months and were absent in all infants aged 4-8 months. The presence of IgG-specific measles antibodies varied among children of vaccination age, from 57.3% for children aged 9 months to 5 years, to 50.6% for children aged 6-9 years and 45.6% for chidren aged 10 years and above. The overall prevalence of rubella-specific IgG was 55.4%, with a high prevalence (87.4%) among children over 10 years of age.Conclusion: The findings suggest that despite efforts to accelerate measles control by giving a second dose of measles vaccine, a large number of children remain susceptible to measles virus. Further research is required to determine the geographic extent of immunity gaps and the factors that influence immunity to measles virus in the Central African Republic. [ABSTRACT FROM AUTHOR]

A vast silvicultural experiment was set up in 1982 nearby the town of M'Baïki in the Central African Republic to monitor the recovery of tropical forests after disturbance. The M'Baïki experiment consists of ten 4-ha Permanent Sample Plots (PSPs) that were assigned to three silvicultural treatments in 1986 according to a random block design. In each plot, all trees with a girth at breast height greater than 30 cm were spatially located, numbered, measured, and determined botanically. Girth, mortality and newly recruited trees, were monitored almost annually over the 1982-2022 period with inventory campaigns for 35 years. The data were earlier used to fit growth and population models, to study the species composition dynamics, and the effect of silvicultural treatments on tree diversity and aboveground biomass. Here, we present new information on the forest stand structure dynamics and tree demography. Objective: The objective of this survey was to describe epidemiological characteristics and evaluate vaccine effectiveness against pertussis in the sub-prefecture of Baoro (Central African Republic)., Method: We conducted a case control study from June to July 2022 in the sub-prefecture of Baoro in the district of Baour-Baoro. Community-based definitions of surveillance were applied to include cases and controls., Results: All in all, 143 confirmed and suspected cases and 166 controls were enrolled in this investigation. The epidemic had affected 12 neighborhoods and 9 villages. The village of Bugbassem alone had 32 patients, and 50% of the patients lived in rural areas. The age groups of 12 to 59 months accounted for 58% of cases. Patients were female in 54% of cases (p = 0.8). The attack rate among unvaccinated patients (65%) was higher than that of vaccinated patients (40%), while 15% (N = 21) of children were hospitalized with no recorded deaths. Pentavalent 3rd dose vaccination coverage was 49% in controls versus 33% in cases. Vaccination provided a significant protective effect (Odds Ratio = 0.3; 95% CI: 0.2-0.6). All told, vaccine efficacy against pertussis was 38%., Conclusion: Pertussis is a re-emerging disease currently occurring as a result of low pertussis vaccination coverage. It strongly affects rural areas with low vaccination rates. This paper is a contribution to the distribution, taxonomy and phenology of Orthoptera Tettigoniidae of Central African Republic. Thanks to scientific expeditions and entomological missions, 2155 Orthoptera belonging to 118 species of five subfamilies of Tettigoniidae have currently been studied. Examined material is here listed with its known distribution. New genera and species have been found and here described: Eurycoplangiodes sanghaensis Massa, n. gen. and n. sp. Paraeulioptera emitflesti Massa, n. gen. and n. sp., Paraeurycorypha Massa, n. gen. ocellata Massa et Annoyer, n. sp., Arantia (Arantia) gretae Massa, n. sp., Arantia (Euarantia) syssamagalei Massa et Annoyer, n. sp. and Dapanera brevistylata Massa, n. sp. Overall, from 2012 to 2020, the study of the orthopteran material collected in the scientific expeditions to Central Africa carried out since 1984, enabled the description of 6 new genera and 27 new species. They represent 22.8% of the total amount of species currently recorded in the protected areas where this scientific activity has been carried out. This demonstrates that these areas still hold a high number of new species, representing a biodiversity hotspot.

African forest elephants (Loxodonta cyclotis) occupy large ranges in dense tropical forests and often use far-reaching vocal signals to coordinate social behavior. Elephant populations in Central Africa are in crisis, having declined by more than 60% in the last decade. Methods currently used to monitor these populations are expensive and time-intensive, though acoustic monitoring technology may offer an effective alternative if signals of interest can be efficiently extracted from the sound stream. This paper proposes an automated elephant call detection algorithm that was tested on nearly 4000 h of field recordings collected from five forest clearings in Central Africa, including sites both inside protected areas and in logging concessions. Recordings were obtained in different seasons, years, and under diverse weather conditions. The detector achieved an 83.2% true positive rate when the false positive rate is 5.5% (approximately 20 false positives per hour). These results suggest that this algorithm can enable analysis of long-term recording datasets or facilitate near-real-time monitoring of elephants in a wide range of settings and conditions.

Yellow fever still causes high burden in several areas of sub-Saharan Africa and Latin America. There are few well-designed epidemiological studies and limited data about yellow fever in Africa. Staples et al., in a recently published paper in Transactions of the Royal Society of Tropical Medicine & Hygiene, performed a nationwide study in the Central African Republic (CAR) assessing infection risk and the operational impact of preventive measures. The rapid assessment of human, non-human and mosquito data call attention to the potential risk of future yellow fever outbreaks in the CAR and elsewhere. The study reinforces the need for intensified applied and operational research to address problems and human capacity needs in the realm of neglected tropical diseases in the post-2015 agenda., (© The Author 2015. In this paper, we carry out a combined analysis of autosomal (ten microsatellites and an Alu insertion), mitochondrial (HVR-1 sequence, 360 nucleotides) and Y-chromosomal (seven microsatellites) variation in the Mbenzele Pygmies from the Central African Republic. This study focuses on two important questions concerning the admixture and origin of African Pygmies. Ethnographic observations suggest a sex-biased gene flow between the Bantus and Pygmies, an issue which could be clarified through genetic analyses may shed light. A study of intrapopulational variation of mtDNA and Y-chromosome produces results in accordance with the hypothesized matrimonial behaviour. In fact, while shared mitochondrial haplotypes belonging to the L1c5 (or L1c1a1 clade) sub-haplogroup provides evidence of a Pygmy-to-Bantu female biased gene flow, a male biased gene flow from Bantu to Pygmies is supported by the distribution of the Y-chromosomes bearing M2 mutation. The second part of our study regards the question of the genetic relationships between Western and Eastern Pygmies. Our results favour the pre-Bantu hypothesis which suggests that the two Pygmy groups separated in ancient times (at least 18,000 years ago), whereas they do not support the recent divergence and differential admixture hypothesis which posits their separation as a consequence of the Bantu expansion (2,000-3,000 years ago).

Goal: To investigate the in vitro antimicrobial susceptibility of Neisseria gonorrhoeae strains isolated in 2004 and 2005 in Bangui, Central African Republic; Yaoundé, Cameroon; Antananarivo, Madagascar; and Ho Chi Minh Ville and Nha Trang, Vietnam., Study Design: Antimicrobial susceptibility testing was performed by both disk diffusion and agar dilution methods according to Clinical and Laboratory Standards Institute (CLSI) recommendations. Minimum inhibitory concentrations (MICs) to 5 antimicrobials (penicillin G, ceftriaxone, ciprofloxacin, spectinomycin, and tetracycline) were determined when feasible. Penicillinase-producing N. gonorrhoeae (PPNG) was analyzed by the paper acidometric method (nitrocefin test)., Results: Thirty N. gonorrhoeae isolates from Bangui could be studied, 79 from Yaoundé, 126 from Antananarivo, 56 from Nha Trang, and 126 from Ho Chi Minh Ville in 2004 and 2005. Unfortunately, because of problems of electricity supply, no strains could be recovered for the determination of MICs in Yaoundé, and only 68 strains could be tested in Antananarivo and 121 in Ho Chi Minh Ville. Patterns of resistance were similar in Antananarivo, Bangui, and Yaoundé but different from those observed in Vietnam. Ciprofloxacin was highly effective in Africa, but nearly all strains in Vietnam were resistant to this drug. Overall, ceftriaxon and spectinomycin were the best antibiotics, with one strain resistant to spectinomycin in Antananrivo and one strain resistant to ceftriaxon in Ho Chi Minh Ville., Conclusions: Ciprofloxacin remains highly efficient in Madagascar and Central Africa, ceftriaxone and spectinomycin should be used as the first-line antimicrobial agents in treating gonorrhea in Vietnam.

Objective: The purpose of this paper is to examine the relationship between childhood undernutrition and poverty in urban and rural areas., Design: Anthropometric and socio-economic data from Multiple Indicator Cluster Surveys in Angola-Secured Territory (Angola ST), Central African Republic and Senegal were used in this analysis. The population considered in this study is children 0-59 months, whose records include complete anthropometric data on height, weight, age, gender, socio-economic level and urban or rural area of residence. In addition to simple urban/rural comparisons, the population was stratified using a wealth index based on living conditions and asset ownership to compare the prevalence, mean Z-score and odds ratios for stunting and wasting., Results: In all cases, when using a simple urban/rural comparison, the prevalence of stunting was significantly higher in rural areas. However, when the urban and rural populations were stratified using a measure of wealth, the differences in prevalence of stunting and underweight in urban and rural areas of Angola ST, Central African Republic and Senegal disappeared. Poor children in these urban areas were just as likely to be stunted or underweight as poor children living in rural areas. The odds ratio of stunting in the poorest compared with the richest quintile was 3.4, 3.2 and 1.5 in Angola ST, Senegal and Central African Republic, respectively., Conclusions: This paper demonstrates that simple urban/rural comparisons mask wide disparities in subgroups according to wealth. There is a strong relationship between poverty and chronic undernutrition in both urban and rural areas; this relationship does not change simply by living in an urban environment. However, urban and rural living conditions and lifestyles differ, and it is important to consider these differences when designing programmes and policies to address undernutrition.

This paper reports a two-phase study in Bangui, Central African Republic (CAR): first, we assessed the clinical efficacy to chloroquine (CQ), sulfadoxine-pyrimethamine (SP), and amodiaquine (AQ), then we tested the efficacy of two combinations: CQ + SP and AQ + SP. We used the standard 14-day WHO 2001 protocol to compare therapeutic responses in children under 5 years of age with acute uncomplicated Plasmodium falciparum malaria in Bangui between February 2002 and March 2004. The overall treatment failure rates with CQ, AQ, SP, CQ + SP, and AQ + SP were 40.9%, 20.0%, 22.8%, 7.2%, and 0%. These findings suggest that the Ministry of Health should recommend an interim policy with AQ + SP combination as the first-line antimalarial drug in Bangui until best alternative treatments like artemisinin-based combination therapies (ACTs) become available at low prices in the CAR.

This paper examines determinants of one aspect of sexual behaviour--coital frequency--among 2,188 married women in the Central African Republic using a secondary analysis of data from the Demographic and Health Survey of 1994-95. Female genital cutting (or circumcision) is practised in the Central African Republic and self-reported circumcision status was included in the questionnaire enabling it to be examined as a possible determinant of coital frequency. Multiple logistic regression was used to find a subset of factors independently associated with coital frequency. Decreased coital frequency was found in those who had longer duration of marriage, those who were not the most recent wife in a polygamous marriage and those who had more surviving children. Coital frequency was higher in more educated women and those not contracepting because they wanted to get pregnant. After adjusting for confounders no association between female genital cutting and coital frequency was found. The extent to which women can control coital frequency in this culture is not known and fertility desires may override any negative effects of circumcision on sexual pleasure. It was therefore not possible to draw conclusions about how female genital cutting affects a woman's desire for sexual intercourse and consequently there is a need to develop research methods further to investigate this question.

Objective: To evaluate the performance of serological tests using dried blood on filter-papers (micro-card agglutination test for trypanosomiasis (micro-CATT)) performed under field and laboratory conditions and using whole blood ((CATT/T.b. gambiense) (wb-CATT) and latex agglutination (LATEX/T.b. gambiense) (wb-LATEX)) for the serodiagnosis and surveillance of human African trypanosomiasis in West and Central Africa., Methods: We evaluated the micro-CATT, wb-CATT and wb-LATEX methods in Côte d'Ivoire and the Central African Republic by screening 940 people. Sensitivity and specificity were calculated for each serological test; only patients with the confirmed presence of trypanosomes in the blood or lymph aspirate were considered true positives. Positive and negative predictive values were also calculated., Findings: Each of the tests showed a lower sensitivity in the Central African Republic than in Côte d'Ivoire., Conclusion: The results confirmed the efficiency of the classic wb-CATT to detect sleeping sickness patients. The micro-CATT method can be used for human African trypanosomiasis surveillance if the test is performed on the same day as the blood collection, or if samples are stored at 4 degrees C. Otherwise, micro-CATT can be used when absolute sensitivity is not required. wb-LATEX should only be used for high-specificity screening.

This paper describes the design and implementation of a health worker training program in diarrhea case management and its effect on patient education in health facilities in the Central African Republic (C.A.R.). In 1989, a facility-based assessment of health worker practices in managing diarrheal disease in children under 5 years of age documented serious deficiencies in patient education as performed by health workers. Based on these results, the Ministry of Health (MOH) designed an inservice training program that promoted education as an integral component of curative care. The training program was implemented in all five health regions of the country. An evaluation of the training's impact on the delivery of patient education indicated dramatic increases in the number of messages health workers communicated to mothers. This experience demonstrated that the patient education practices of health workers can be improved through inservice training that integrates the teaching of clinical and communication skills. Additional study in C.A.R. is needed to (1) further improve the quality of patient education for diarrhea and other childhood communicable diseases, (2) determine the impact of patient education on the care provided by mothers in the home following a clinic visit, and (3) assess how operational research can be conducted within the limitations of inservice training programs and routine clinical operations.

The epidemiology of Trypanosoma vivax infections was studied at a riverside site in the Ouro-Djafoun livestock area situated in the Central African Republic during the period between July 1991 and July 1992. This paper examines the possibility that stable flies (Diptera: Stomoxyinae) were also vectors of this trypanosome species in a non-cyclic way. Previous studies have revealed that the usual cyclic transmission by the tsetse fly Glossina fuscipes fuscipes was probably not the only transmission route. At the study site, at least five species or subspecies of stable flies were encountered: Stomoxys nigra nigra (approximately 60% of the sample), S taeniata, S sitiens, S omega omega and Haematobia spp. The hypothesis that stable flies could be good vectors of T vivax in this country is supported by three main observations: i) stable flies were very abundant at the cattle resting site; ii) an estimation of the 'contact index' between the cattle and stable flies demonstrated close interactions between cattle and stable flies at this site, particularly during the rainy season, and iii) there was a good correlation (P < 0.05) between the apparent stable fly densities at the resting site and the frequency of T vivax in the cattle. The relevance of this phenomenon in terms of epidemiology and combatting T vivax-caused nagana is discussed.

It is known that the rate of low birth weight in African developing countries is very high. A birth weight analysis based on their delivery charts is made in this paper for the babies born from 1988 to 1990 at two maternity hospitals, Castor and Boy-Rabe, in Bangui, the capital of the Republic of Central Africa. The total number of births amounted to 27,188 for Castor and 7,667 for Boy-Rabe, excluding multiple births. As a result, it was found that the youngest mother's age was twelve and the oldest fifty three, with the average age at the first delivery seventeen, followed by the another delivery every two years. Mean birth weight (MBW) values were significantly higher in Castor (males 3,134 +/- 527.5, females 3,018 +/- 511.6 grams) than in Boy-Rabe (males 3,017 +/- 542.6, females 2,909 +/- 507.1 grams). The total rate of low birth weight (LBW) under 2,500 grams was 10.9 percent. That of Castor maternity was 9.8 percent, while it was 14.7 percent for Boy-Rabe, which is significantly higher than the former. In addition, the rate of LBW is the highest in the first delivery for mothers younger than 16, while it is the lowest in the sixth delivery of the group of mothers group aged 25-29. Looking at the seasonal variation of LBW, we noticed that it became higher in July and August every year, the busiest period for farming, with harvesting followed by planting. These findings suggest that low birth weight infants are due to the age of delivery being younger than sixteen and also to the mother's heavy labor.

In Bangui (Central African Republic), where seroprevalence of HIV is 11% in the adult population, AIDS presents some clinical aspects different from the ones known in the west; the clinical experience reported in this paper is based on 504 cases infested by HIV group 4; diagnosis is very often made thanks to the clinical score recommended by World Health Organization (predictive value of 66%). Predominant manifestations (14%) are: asthenia (100%), emaciation (100%), fever (88%), diarrhea (42%), pulmonary attacks (37%), adenopathies, cutaneous manifestations (35%), neurological manifestations (14%). Some affections call for HIV infection with a significant predictive value: herpes zoster (96%), Kaposi's symptom (68%), mouth candidiasis (71%), pulmonary tuberculosis (56%: as far as some others are concerned, HIV has to be suspected: infant denutrition, acute infections, neurological disorders. Development is severe: 45% of the patients examined died in the 4 months coming after diagnosis. Epidemiology speaking, they are young patients (mean age 27.4 years), neither addicted nor "doped", heterosexual with multiple partners, with female prostitution occasionally; sex ratio is 0.95. Recognized transmission by transfusion is the exception (2/504). The transmission due to vaccination or injection is rare and difficult to evaluate. Only radical alteration of sexual behaviour will modify HIV dissemination.

In the course of a long-term research project, three groups of Pygmies and some non-Pygmy Central Africans have been examined for the following red cell enzyme markers: ACP, PGM1, PGM2, PEPA, PEPB, and PEPC, AK, ADA, and PHI. Several other red cell enzymes (ESD, CA1 and CA2, GPT, GLO, and DIA1) have been studied in only some of these groups. This paper reports all the information we obtained, including what we have already published. The following conclusions can be drawn from the whole body of data: (1) Gene patterns of Pygmies are those typical of other Africans (e.g.: lack of ADA2 and AK2 genes, low GPT2 gene frequency, polymorphism of the CA2 locus, and presence at polymorphic frequencies of PEPA2 allele. (2) Superimposed on this African genetic makeup, a number of Pygmy characters were identified, namely, a private polymorphism for the PGM26 Pygmy allele and possibly one for the PEPC2 allele, and particularly high ACPR and low PGM12 gene frequencies. (3) Some markers, especially PGM1 and ACP, turned out also to discriminate efficiently among different groups of Pygmies.
