Emerging research suggests that students from equity-deserving groups (EDGs) may experience barriers within work-integrated learning (WIL) that other students may not face, and such barriers may negatively impact students' participation in WIL. Guided by a social justice lens, this study used interviews of co-operative education (co-op) students (n = 30) from EDGs to explore barriers that such students experienced in one Canadian co-op program. Analyses of qualitative data showed that these students experienced non-structural barriers (those that are less explicit, such as internalized discrimination) and structural barriers (those related to policy and practice, both within their co-op program and their host organizations). While some barriers were specific to a given EDG, others were common across EDGs. These findings provide a fuller picture of the kinds of barriers experienced by WIL students within and across EDGs.

The mental health of undergraduate students is a concern across many North American institutions. To support students' mental well-being and help them manage stress, many Canadian post-secondary institutions have introduced a mid-semester break during the fall term. However, there has been limited work evaluating whether this fall break is associated with a decrease in stress (a well-established correlate of mental health). Further, it is not known which demographic variables and lifestyle factors (e.g., sleep patterns, regular exercise) may contribute to a change in stress surrounding the fall break. We assessed the effects of the fall break on perceived stress levels and investigated whether these effects varied according to gender and the quality of students' social network. Our results indicate that on its own, the fall break did not reduce students' level of perceived stress. Further, students who reported feeling socially disconnected reported greater stress (both pre- and post-break) than those who reported being more content with their social network. Men in our sample reported less perceived stress than women. We recommend that mental health initiatives on campuses integrate strategies to build social connections between students and emphasize outreach programs to students who report feeling socially disconnected. We acknowledge the increased necessity and challenge of doing this in the context of education in the time of COVID-19.

New immigrants' arrival in any country indeed brings new challenges to settle in the country. Learning the language of a new country is one of the major hassles in settling and starting a new life. In Canada, new immigrants must learn English as a second language because English is an official language and is also used day to day in almost all provinces except for a few states where the French language is more acceptable compared to English. Learning English requires motivation and there are some barriers in learning. Thus, this study addresses these barriers and assesses what motivational factors are there for new immigrants to learn English. It was a cross-sectional quantitative study conducted at a poly-cultural center, and Canada from June-2018 to December-2018. A total of 325 participants who registered and gave consent were included in this study. The results showed that factor 1 (Desire for career and economic enhancement) is significantly associated with age groups (P=0.001), gender (0.001), educational status (P=0.012), and time in Canada (P=0.027). Factor 2 (Desire to become a global citizen) is not significantly associated with all demographic data except for gender (P=0.027). Factor 3 (Desire to communicate and affiliate with foreigners) shows an association with all other demographic characteristics, except for gender (P=0.63), nationality (P=0.568), and educational status (P=0.091). In factor 4 (Desire for self-satisfaction), only educational status (P=0.046) has a significant association. Factor 5 (self-efficacy) and factor 6 (Desire to be integrated with other cultures) do not show any significant association with any demographic characteristics. In conclusion, the main motivating factors for new immigrants learning English as a second language acquisition is the desire for career and economic enhancement and the desire to communicate and affiliate with foreigners.

The development and evaluation of valid assessments of scientific reasoning are an integral part of research in science education. In the present study, we used the linear logistic test model (LLTM) to analyze how item features related to text complexity and the presence of visual representations influence the overall item difficulty of an established, multiple-choice, scientific reasoning competencies assessment instrument. This study used data from n = 243 pre-service science teachers from Australia, Canada, and the UK. The findings revealed that text complexity and the presence of visual representations increased item difficulty and, in total, contributed to 32% of the variance in item difficulty. These findings suggest that the multiple-choice items contain the following cognitive demands: encoding, processing, and combining of textually presented information from different parts of the items and encoding, processing, and combining information that is presented in both the text and images. The present study adds to our knowledge of which cognitive demands are imposed upon by multiple-choice assessment instruments and whether these demands are relevant for the construct under investigation--in this case, scientific reasoning competencies. The findings are discussed and related to the relevant science education literature.

This rapid systematic review aims to examine emerging evidence on the effects of COVID-19 on educational institutions and assess the prevalence of e-learning changes in the sector. This paper reviews literature on learning, teaching, and assessment approaches adopted since the COVID-19 outbreak, and assesses the impact on the sector, staff, and students, summarizing findings from peer-reviewed articles. It categorizes these into five key themes: (1) digital learning; (2) e-learning challenges; (3) digital transition to emergency virtual assessment (EVA); (4) psychological impact of COVID-19; and (5) creating collaborative cultures. This represents the first systematic review of COVID-19's impact on education, clarifying current themes being investigated. The author suggests that the term 'emergency virtual assessment' (EVA) is now added for future research discussion. Finally, the paper identifies research gaps, including researching the impact on lesser developed countries, the psychological impact of transition, and the important role of leadership and leadership styles during the transition and handling of the pandemic.

Scientific reasoning competencies (SRC) are one part of science teachers' professional competencies. This study examines the contribution of three factors to the development of pre-service science teachers' SRC: the "amount of science education classes," the "amount of science classes" and the pre-service science teachers' "age." The factors "amount of science education classes" and "amount of science classes" have been operationalised in terms of ECTS credit points. N = 438 pre-service science teachers from six universities in Germany, Chile and Canada voluntarily and anonymously responded to an established multiple-choice instrument for assessing SRC, which has been developed by the authors and is available in German, Spanish and English. Multiple linear regression analyses show that the included factors explain a proportion of about 9% of the pre-service science teachers' SRC. The factor "amount of science classes" is the only significant predictor and can be seen as an indicator of learning science content knowledge. These findings support the assumption of science content knowledge being a prerequisite for developing pre-service science teachers' SRC.

The COVID-19 pandemic has imposed unprecedented service interruptions in many sectors including services for children and youth with neuro-developmental disabilities (NDD). We examined the experiences of service providers as they supported this population during the pandemic. Five focus groups were convened with 24 service providers offering support to children/youth with NDD and their families. Results highlight substantial service changes and challenges, as observed by service providers. Service closures and program delivery modification resulted in the rapid adoption of virtual services and reduced program delivery. Service providers have faced heightened workloads, personal weariness and 'burn out', and new levels of conflict at work, yet with little opportunity and support for self-care. Beyond challenges, new learning and growth have emerged, with heightened collaboration amongst organizations. Strains in service delivery during the pandemic have exposed programming and systems gaps, for which proactive capacity building is warranted and recommended.

This essay uses an enactive approach to map out the ways Ontario teachers, students, and parents have reimagined online mathematics education at the K-12 level during the COVID-19 pandemic. The essay highlights the importance of education researchers using an appropriate framework in understanding the emerging mathematics education realities. It encourages education researchers to pay attention to this call to action while recognizing that such an action is not without challenges. To address the challenges, education researchers must engage with the evolving mathematics education environment and community by innovating and reimagining their research tools and techniques.

Community service learning (CSL) activities in undergraduate programs are associated with improvements in self-efficacy (confidence related to performing a specific task) and academic achievement. This study aimed to understand the impact of a CSL assignment on self-efficacy related to teaching community members about evidence-based nutrition and on the overall learning experience. Students were invited to participate in this mixed-methods study (surveys and focus groups), and the results indicate that the CSL activity not only increased students' self-efficacy related to nutrition science communication, but also gave students a greater feeling of connection to their community and an opportunity to practice skills needed for future careers.

There are some factors, which are motivational, and on the other hand, there are many challenges that may cause hindrance for learning English as a second language. Therefore, the main objective is to assess perception regarding factors for learning English as a second language, especially among new immigrants. This was a cross-sectional study conducted at a Polycultural center, Canada during the period of June-2017 to July-2017. The results of the study reinforced that new immigrants face challenges while learning English. Almost more than half the participants said that they find difficulty in understanding majority words. Regarding learning culture, two-thirds mentioned that the learning about culture gives them different points of view about the world and develops their critical thoughts. However, the socio-demographic characteristics like age, native language, play an important role in learning English.

Open educational resources (OER) have become increasingly popular in recent times. Here, the aim was to synthesise the lessons learned through the development of OER materials for a university-level course called "environmental modelling". Consequently, the topics of discussion included: (i) how to choose an appropriate creative commons license; (ii) ways of incorporating materials from other sources, such as publicly available sources, other open access materials, and an author's own published materials if not published under a creative commons license; (iii) the impact of the developed OER in the field of environmental modelling; and (iv) the challenges in developing OER material. Upon developing the materials, we observed the following: (i) students enrolled in the course did not purchase textbooks; (ii) our OER materials ranked as one of the most accessed (i.e., number 7) materials according to the usage data that summed the number of file downloads and item views from PRISM (i.e., the hosting platform maintained by the University of Calgary); (iii) the students learned relatively better as per the data acquired by the University of Calgary's universal student ratings of instruction (USRI) instrument; and (iv) other universities expressed interest in adopting the materials.

We give an unconventional, mythopoetic response to the question of Why teach mathematics to all learners in school? In our work with pre-service teachers, we attempt to teach how to value the vulnerability of the multispecies world in a relational, anti-colonial way through passionate immersion, and to use mathematics education towards the ends of promoting its and our own (human) flourishing through taking on relationships of curatorial care. The study provides insight into counsellors' experiences of counselling clients online. The foci include (a) counsellors' experience of negotiating the therapeutic relationship online, (b) their experiences of utilising and adapting their clinical skills to assess clients in an online capacity, and (c) ethical issues associated with practicing online. Open-ended interviews were conducted with three counsellors located in Canada and one in the United States. Narrative analysis revealed eight major themes: convenience, therapeutic alliance, online counselling skills, assessing client suitability, reaching diverse clients, assessing client satisfaction, legal and ethical concerns, and personal and professional goals.

Canadian post-secondary institutions are increasingly introducing a fall break into their term calendars, with the stated goal of reducing student stress and improving academic success. We conducted a pilot study around the time of this fall break during which we collected saliva samples to measure the ratio of two metabolic hormones (cortisol and dehydroepiandrosterone (DHEA)) from first-year male engineering students in order to document possible changes in their stress levels before and after the break. Participants self-identified a particular day in the week prior to the break that they considered to be most stressful, followed by a day in the week after the break that was perceived to be equally stress-inducing. A control sample of student engineers was recruited from another university with equivalent academic rigour but without a fall break. Students who experienced the fall break exhibited a marginally lower ratio of cortisol to DHEA after the break than did those who did not experience the break indicating a difference in psychological stress. Since fall breaks are now increasing in popularity, we make the recommendation that it is imperative to empirically investigate their impact on student mental health.

Universities across Canada are responding to increasing levels of student stress and mental illness by introducing a fall break. However, scant research has investigated the effectiveness of this intervention. Our team assessed perceived stress and the number of stressors experienced by students at McMaster University using established self-report stress questionnaires, comparing stress before and after the break. We found that despite the widespread expectation that a fall break will decrease student stress, the effects of this intervention are not straightforward. Students experienced fewer stressors after the break than before it, but experienced higher levels of overall stress. Additionally, stress varied according to several demographic variables, revealing some groups to be at higher risk for stress-related problems. Given the wide-scale adoption of fall breaks, we hope that this investigation can initiate dialogue about the importance of evidence-based decisions in the development of stress-reduction interventions for Canadian university students.

Developing a new assessment requires the expertise of both content experts and assessment specialists. Using the example of an assessment developed for Ontario's Colleges Mathematics Assessment Program (CMAP), this article (1) describes the decisions that must be made in developing a new assessment, (2) explores the complementary contributions of content experts and assessment specialists, and (3) illustrates how the use of a validity argument framework can support collaboration in assessment development. The authors conclude that the validity argument framework facilitated effective collaboration between content experts and assessment specialists, and suggest that this approach may help other collaborators pursue transparent and effective assessment development.

More and more Canadian post-secondary institutions are introducing a fall break into their term calendars. In 2015, a full week fall break was introduced at our university in order to enhance academic performance and improve mental health amongst students. Our interdisciplinary team surveyed undergraduate students at our university about their experience of the fall break, collected standardized measures of experienced stressors and perceptions of stress before and after the break, and hosted several focus groups to develop a detailed narrative of students' experience. Stress can also be assessed through non-invasive hormone measures. We collected saliva samples to profile metabolic hormones, cortisol, and dehydroepiandrosterone (DHEA), from first-year male engineering students in order to document possible changes in their stress levels before and after the week-long break. This group was compared to male engineering students at a similar university that does not hold a fall break. Students exhibited a lower ratio of cortisol to DHEA after a fall break than those that did not experience a break. Our survey results indicate that the majority of students thought the fall break was a positive experience. However, self-reports of stress show a more complex picture, with many students reporting increased perceived stress after the break. Additionally, a portion of students reported that the fall break was a negative experience. To the best of our knowledge, our study is the first of its kind to use a mixed-methods approach to examine the impacts of a fall break.

Cultural and faith-based narratives live in our skins and personal lives. Each body is tied to the stories that are written on it, and thus the body and the soul within it are central to our coming to know of tears, wounds, pain, and suffering. Using autobiographical narrative inquiry and poetry, I share my lived experiences and encounters with the pain of silence and truth telling. As a Canadian, Muslim, woman of color, and mother of four children, I unapologetically question the "psychic violence" of strategic silencing, scripted conformity, and structured surrender enforced and performed in institutions and society toward people of color. I seek to embrace, dwell in, and engage with pain to pull out passion, potential, and possibility. I offer the truth of my knowing to construct pathways and possibilities for reimagining and deconstructing pain and tears as the cure for personal and systemic liberation.

This paper aims to introduce a novel family of probability distributions by the well-known method of the T–X family of distributions. The proposed family is called a "Novel Generalized Exponent Power X Family" of distributions. A three-parameters special sub-model of the proposed method is derived and named a "Novel Generalized Exponent Power Weibull" distribution (NGEP-Wei for short). For the proposed family, some statistical properties are derived including the hazard rate function, moments, moment generating function, order statistics, residual life, and reverse residual life. The well-known method of estimation, the maximum likelihood estimation method is used for estimating the model parameters. Besides, a comprehensive Monte Carlo simulation study is conducted to assess the efficacy of this estimation method. Finally, the model selection criterion such as Akaike information criterion (AINC), the correct information criterion (CINC), the Bayesian information criterion (BINC), the Hannan–Quinn information criterion (HQINC), the Cramer–von-Misses (CRMI), and the ANDA (Anderson–Darling) are used for comparison purpose. The comparison of the NGEP-Wei with other rival distributions is made by Two COVID-19 data sets. Introduction: Chronic cough (persisting for ≥ 8 weeks) is a common disorder that includes refractory chronic cough (RCC; cough that persists despite treatment of underlying disease) and unexplained chronic cough (UCC; cough with no identifiable cause). We evaluated self-reported health-related quality of life (HR-QoL) and work/activity impairment associated with RCC/UCC in Canada. Methods: Our exploratory study included Canadians in the Leger Opinion Panel with RCC or UCC. Key entry criteria were ≥ 18 years of age, cough for ≥ 8 weeks, not currently smoking/quit ≥ 1 year ago, no serious respiratory disease or lung cancer, and not taking angiotensin-converting enzyme inhibitors. Respondents completed a 30-min online survey with general and cough-specific HR-QoL questionnaires, including the EuroQol (EQ) visual analogue scale (VAS), EQ-5-dimension 5-level (EQ-5D-5L), cough severity VAS, Leicester Cough Questionnaire (LCQ), and Work Productivity and Activity Impairment-Specific Health Problem (WPAI-SPH). Results: Of 49,076 individuals who completed the chronic cough screening questionnaire (July 30-September 1, 2021), 1,620 (3.3%) met entry criteria for RCC/UCC and 1,046 (2.1%) completed the survey. The mean age of respondents was 45 years and 61% were female. Respondents reported impairments in global HR-QoL (EQ-VAS 73.8, 61% with anxiety/depression on the EQ-5D-5L) and cough-specific HR-QoL (mean cough severity VAS score 29.7, LCQ index 15.2). Work and non-work activities were reduced by 34% and 30%, respectively, on the WPAI-SPH. Conclusion: RCC/UCC is prevalent in Canada and associated with impaired HR-QoL, particularly in mental health domains. Lesbian, gay, bisexual, transgender, queer, intersex, and asexual (LGBTQIA+) Black, Indigenous, and People of Color (BIPOC) live at the intersection of marginalized identities, resulting in unique experiences of exclusion within their various communities. Despite experiences of community exclusion, LGBTQIA+–BIPOC find ways to resist such structures to thrive in the face of adversity and strive for transformative change. The present study uses an intersectional framework to explore LGBTQIA+–BIPOC's experiences with community exclusion and acts of resistance. Fourteen LGBTQIA+–BIPOC participated in semistructured interviews to explore their experiences of exclusion and acts of resistance. Findings supported two broad categories with subthemes: (a) "on the fringes" (i.e., invalidation of LGBTQIA+–BIPOC experiences; White supremacy, colonization, and Western imperialism; "out and proud" dynamics) and (b) resistance: "we're here, and I'm gonna let you know" (i.e., resisting intersectional oppression; intersectional identity cohesion; coalition building). Utilizing an intersectional framework, we critique larger oppressive structures that disenfranchise LGBTQIA+–BIPOC while offering implications for social justice interventions and transformational change to best serve LGBTQIA+–BIPOC. The knowledge content of university-level introductory sustainability courses elicits emotional reactions by students that are novel within the typical classroom context. Common negative reactions include 'sadness', 'worry', 'guilt' and 'disgust', while more positive responses include 'feeling angry', 'empowered', 'like trying to make a difference' or 'having raised awareness'. These emotions are indexical of a deeper social epistemic collision between historically established social identities, including behavioural scripts consistent with, and generative of, unsustainability on the one hand, and a growing collective awareness of the consequent unsustainability that threatens students' future well-being on the other. The authors argue that introductory sustainability courses set up the potential for not only a learned eco-anxiety, but also an ontological adjustment. That adjustment might bring student, historical inheritance and environment from a state of living in a suffering, but still separate, world to a practice of becoming with a world into which we extend and that also extends into us. Therefore, it is arguably important for instructors to be aware of the possibility of students getting into a negative state of eco-anxiety and for instructors to also have some tools for supporting a more positive ontological adjustment. We recommend that they become skilled in facilitating transformational learning by including some discussions about the ontology of self in any introductory sustainability instruction. Directing students' attention to their own emotional responses can also be useful for grounding such classroom discussions and transformational learning.

In this study, we analysed the scientific reasoning competencies of preservice science teachers from a Canadian sample at the beginning and end of a science teacher education methods course. The course contained standard topics, such as the nature of science, assessment, and unit and lesson planning in science. The preservice science teachers were asked to reason about two types of problems in a validated pre- and post-questionnaire: investigatory-process problems and problems regarding modeling. Statistical analysis of the data revealed that the course significantly contributed to the development of preservice science teachers' competencies for those who had two previous degrees compared with those that did not. Furthermore, a greater proportion of teachers were deemed highly competent at planning investigations and testing models than the more generative dimensions of scientific reasoning, such as formulating questions and generating hypotheses. Implications for science teacher education internationally and the movement towards competency-based curricula are put forward.

Grade 12 students in my son's psychology class had been asked to share their deepest reactions and thoughts toward the victims of the "Charlie Hebdo" incident in Paris. "Charlie Hebdo" is a satirical magazine that had published controversial cartoons of the Prophet Muhammad. Students felt sympathetic toward the victims and solely held Islamic extremism responsible for it. During the heated conversations, a female Muslim student stood up and voiced her perspective. Given the teacher's discomfort with the potential reaction to the counter story, he shut down a crucial conversation that could have created a space for critically assessing polarizing debates.

Spring-breakup ice-jam floods are a major hazard for many rivers in cold regions. They can cause severe damage to infrastructure, property, and ecosystems along riverbanks. To reduce the risk and impact of these events, it is essential to develop reliable and timely forecasting systems that can provide early warning and guidance for mitigation actions. In this paper, we highlight the elements and processes required for the successful development of a spring-breakup ice-jam flood forecasting system, using the lower Churchill River in Labrador, Canada as a case study. We review the existing forecasting methodologies and systems for spring-breakup ice-jam floods and discuss their strengths and limitations. We then describe the case study of the lower Churchill River, where a large ice-jam flood occurred in May 2017, triggering an independent review and a series of recommendations for improving the flood preparedness and response. We present the main components and features of the forecasting system that was developed for the lower Churchill River, based on the recommendations from the independent review. We also discuss the improvements that were made to the forecasting system, such as parallelization, adaptation, and determination of ice-jam prone areas. Finally, we provide some conclusions and recommendations for future research and development of spring-breakup ice-jam flood forecasting systems, focusing on the requirements for a technical framework that incorporates community engagement and special considerations for regulated rivers. • Nearly half of female pwCF had an abnormal Pap smear. • 12% reported at least one HPV-associated complication. • Both male and female pwCF reported high psychosocial burden related to HPV disease. • 60% of adult pwCF are unvaccinated for HPV; over half were >35 years of age. • 80% of adult pwCF never discussed HPV vaccination with their CF care providers. In persons with cystic fibrosis (pwCF), little is known about the prevalence or impact of HPV on quality of life and attitudes towards vaccination. We conducted a national online survey of adult pwCF. We sought to determine the prevalence of self-reported HPV infection, disease-associated complications and effects on quality of life. Additionally, we investigated factors associated with vaccination status. A total of 235 adult pwCF across Canada (≥18 years, 68% female) completed the survey. Forty-eight percent of female pwCF had a history of abnormal Pap smear, with 62% self-reporting a 'no' or 'low' chance of risk of HPV-associated disease. Across participants, 12% reported at least one HPV-associated complication including anogenital warts (58%), HPV-associated malignancies (34%) and cervical dysplasia requiring colposcopy (69%). Only 19% reported discussions with their CF care provider around HPV complications. Across both sexes, pwCF experienced high psychosocial burden in the domains of 'worries and concerns', 'sexual impact' and 'self-image'. Sixty percent of adult pwCF were unvaccinated for HPV. Eighty-one percent reported never having discussed HPV vaccination with their CF care provider, with similar rates in vaccinated and unvaccinated groups. Barriers to vaccination included: lack of discussions with healthcare providers (31%), insured coverage (based on age) (19%) and perceived side effects/risk (10%). Across adult pwCF, we found high prevalence of HPV disease and associated HPV-psychosocial burden and low vaccination uptake. Given the limited medical discussions reported, incorporation of HPV prevention and management should be prioritized by CF care providers as part of comprehensive multimodal care. Background: Variations in primary care practices may explain some differences in health outcomes during the COVID-19 pandemic. We sought to evaluate the characteristics of primary care practices by the proportion of patients unvaccinated against SARS-CoV-2. Methods: We conducted a population-based, cross-sectional cohort study using linked administrative data sets in Ontario, Canada. We calculated the percentage of patients unvaccinated against SARS-CoV-2 enrolled with each comprehensive-care family physician, ranked physicians according to the proportion of patients unvaccinated, and identified physicians in the top 10% (v. the other 90%). We compared characteristics of family physicians and their patients in these 2 groups using standardized differences. Results: We analyzed 9060 family physicians with 10 837 909 enrolled patients. Family physicians with the largest proportion (top 10%) of unvaccinated patients (n = 906) were more likely to be male, to have trained outside of Canada, to be older, and to work in an enhanced fee-for-service model than those in the remaining 90%. Vaccine coverage (≥ 2 doses of SARS-CoV-2 vaccine) was 74% among patients of physicians with the largest proportion of unvaccinated patients, compared with 87% in the remaining patient population. Patients in the top 10% group tended to be younger and live in areas with higher levels of ethnic diversity and immigration and lower incomes. Interpretation: Primary care practices with the largest proportion of patients unvaccinated against SARS-CoV-2 served marginalized communities and were less likely to use team-based care models. This article is a collaborative initiative of the TESL profession in Manitoba. It reflects the authors' attempts to articulate the role of the ESL teaching profession and practice in Canadian multiculturalism. Recognizing ESL teachers' historical roles as agents of assimilation, the authors consider how the field can reorient itself to serve instead the equity and diversity needs of Canadians. They argue that this involves the promotion of more multilingual, intercultural approaches to ESL/SESD. The article begins by considering the model of multiculturalism promoted by Canadian constitutional and legislative policies over the last decades and the key role the TESL field can and does play in this mandate. The article continues on to recommend expanding the potential stakeholders included as ESL students and professionals. This is not done to expand the profession so much as to enhance the professional ethics and responsibilities of the field.

Simple Summary: The impact of breast cancer on individuals and in populations can be reduced if the cancer is detected in an early stage of disease as treatments are generally less harsh and more successful. Knowing which factors to modify and by what amount that could shift breast cancer diagnoses to early stages of disease were studied in this project. Factors related to stage at diagnosis in a previous analysis of women diagnosed with breast cancer were assessed using data that mimicked the real data. The proportions of modifiable lifestyle factors in a population were increased or decreased, such as those ever having a mammogram, to see the effect on detecting more cancers in the earliest stages. Similarly, the average amounts of protein eaten per day or calories consumed were also increased and decreased. Increasing the total amount of protein eaten per day by even just 5 grams was the most important factor found in this study to improve early breast cancer diagnoses. The findings from this study could be helpful in breast cancer prevention programs that could target key modifiable factors and by other researchers studying other types of cancer. Background: Breast cancer is the most common cancer in Canadian women; nearly 25% of women diagnosed with cancer have breast cancer. The early detection of breast cancer is a major challenge because tumours often grow without causing symptom. The diagnosis of breast cancer at an early stage (stages I and II) improves survival outcomes because treatments are more effective and better tolerated. To better inform the prevention of and screening for breast cancer, simulations using modifiable rather than non-modifiable risk factors may be helpful in shifting the stage at diagnosis downward. Methods: Breast cancer stages were simulated using the data distributions from Alberta's Tomorrow Project participants who developed breast cancer. Using multivariable partial proportional odds regression models, modifiable lifestyle factors associated with the stage of cancer at diagnosis were evaluated. The proportions or mean levels of these lifestyle factors in the simulated population were systematically changed, then multiplied by their corresponding estimated odds ratios from the real data example. The effects of these changes were evaluated singly as well as cumulatively. Results: Increasing total dietary protein (g/day) intake was the single most important lifestyle factor in shifting the breast cancer stage downwards followed by decreasing total dietary energy intake (kcal/day). Increasing the proportion of women who spend time in the sun between 11 am and 4 pm in the summer months, who have had a mammogram, who have been pregnant or reducing the proportion who are in stressful situations had much smaller effects. The percentage of Stage I diagnoses could be increased by approximately 12% with small modifications of these lifestyle factors. Conclusion: Shifting the breast cancer stage at diagnosis of a population may be achieved through changes to lifestyle factors. This proof of principle study that evaluated multiple factors associated with the stage at diagnosis in a population can be expanded to other cancers as well, providing opportunities for cancer prevention programs to target specific factors and identify populations at higher risk. Background: COVID-19 pandemic has influenced health care delivery. We conducted an observational study to understand how obstetric medicine (ObM) physicians utilized home blood pressure monitoring (HBPM) to manage hypertension in pregnancy. Methods: Pregnant participants with risk factors or diagnosis of hypertensive disorders of pregnancy (HDP) were enrolled, May 2020–December 2021, and provided with validated home blood pressure (BP) monitor. ObM physicians completed questionnaires to elicit how home BP readings were interpreted to manage HDP. Results: We enrolled 103 people: 44 antepartum patients (33.5 ± 5 years, gestational age of 24 ± 5 weeks); 59 postpartum patients (35 ± 6 years, enrolled 6 ± 4 days post-partum). ObM physicians used range of home BP readings (70%) for management of HDP. Conclusions: HBPM to manage HDP is acceptable and can be used to manage hypertension during pregnancy. Further studies are needed to assess the generalizability of our findings and the safety of HBPM reliance alone in management of HDP. [ABSTRACT FROM AUTHOR]

To identify the contextual factors related to financial capability and financial well-being for adults living with acquired brain injury (ABI). We conducted a qualitative descriptive study using photovoice and included 17 adults who live with ABI in Manitoba, Canada. Over 3-to-5 weeks, participants took photos of their financial capability (i.e. knowledge, skills, and behaviors related to managing finances) or their financial well-being (i.e. subjective and objective financial outcomes). Participants were interviewed about their photos. Five researchers iteratively and thematically analyzed interview transcripts. Analysis identified the importance of the economic, social, technology, and physical or sensory context. Subthemes related to: (i) hard times finding financial resources; (ii) processes not making sense; (iii) getting help from the right person; and (iv) invisible disability bias and stigma. There is decreased literature about financial capability or financial well-being after ABI. The results of this study highlight the salience of finance to living with ABI and the importance of the context to addressing financial-related life participation for people living with ABI. Information about contextual factors related to finance can improve rehabilitation assessment and intervention practice as well as emphasize needed accessibility changes to financial environments. One of the most important and widely accepted pavement performance and ride quality indicators is the International Roughness Index (IRI). This study investigates the combined effect of pavement distress on flexible pavement performance in two climate regions (wet freeze and wet freeze) in the U.S. and Canada. The long-term pavement performance (LTPP) database was used to obtain pavement distress data. Data from forty-three of the LTPP pavement sections (333 observations) with no previous maintenance were collected. The proposed models predict the IRI as a function of pavement distress variables, namely the pavement age, rutting, fatigue cracking, block cracking, longitudinal cracking, transverse cracking, potholes, patching, bleeding, and ravelling. After the data were collected, modelling was conducted to predict IRI using two techniques: multiple linear regression (MLR) and artificial neural network (ANN). The coefficient of determination ( R 2), root mean squared error (RMSE) and mean absolute error (MAE) were used to examine the performance of the two techniques adopted in this study. The models' results revealed that both ANN and MLR models could predict IRI with good accuracy. The MLR models yielded the R 2 values of 77.7% and 89.3%, whereas the ANN models resulted in the R 2 values of 99.1% and 97.5% for wet freeze and wet no freeze climate regions, respectively. Background: South Asian (SA) Canadians are disproportionately affected by higher rates of mood and anxiety disorders. SA Canadians with depression report significant barriers to accessing mental health care and the highest proportion of unmet mental health needs. The Mental Health Commission of Canada (MHCC) advocates for culturally and linguistically relevant services for SA Canadians. Culturally adapted cognitive behavior therapy (CaCBT) has shown to be more effective than standard cognitive behavior therapy (CBT). Adapting CBT for the growing SA population in Canada will ensure equitable access to effective, culturally-appropriate mental health interventions. Method: The study used a qualitative design to elicit stakeholder consultation via in-depth interviews. This study is reported using the criteria included in Consolidated Criteria for Reporting Qualitative Studies (COREQ). The analysis follows an ethnographic approach and was informed by the principles of emergent design. Results: Five themes were identified from the analysis, (i) Awareness and preparation: factors that impact the individual's understanding of therapy and mental illness. (ii) Access and provision: SA Canadians' perception of barriers, facilitators, and access to treatment. (iii) Assessment and engagement: experiences of receiving helpful treatment. (iv) Adjustments to therapy: modifications and suggestions to standard CBT. (v) Ideology and ambiguity: racism, immigration, discrimination, and other socio-political factors. Conclusions: Mainstream mental health services need to be culturally appropriate to better serve SA Canadians experiencing depression and anxiety. Services must understand the family dynamics, cultural values and socio-political factors that impact SA Canadians to reduce attrition rates in therapy. [ABSTRACT FROM AUTHOR]

All forms of data are meant to be secured. The emergence of a variety of techniques and tactics utilized by malware authors has made recovery difficult. LockBit ransomware has been in the news and the APT behind deployment of this ransomware virus has compromised several organizations. It is important to understand the behavior of such malware, to identify the anti‐analysis stuff it performs and to block it. The APT involved compromised several health and medical facilities including AIIMS in India and Sick Kids Hospital in Canada. Ransomwares are highly impactful family of malware. Impacts may begin from confidential medical information being harvested over a C2 and can go up to rendering medical equipment useless. This article discusses compromised entities and defending strategies that can be used to block LockBit ransomware using any generic SIEM tool. It also explains the need for tools that can survive ransomware encryption, a restart command‐line and can detect the behavioral patterns of such malware. Several samples were collected from various sources; the identities of compromise were collected from strings and behavior of those samples. Emerging research suggests that students from equity-deserving groups (EDGs) may experience barriers within work-integrated learning (WIL) that other students may not face, and such barriers may negatively impact students’ participation in WIL. Guided by a social justice lens, this study used interviews of co-operative education (co-op) students (n = 30) from EDGs to explore barriers that such students experienced in one Canadian co-op program. Analyses of qualitative data showed that these students experienced non-structural barriers (those that are less explicit, such as internalized discrimination) and structural barriers (those related to policy and practice, both within their co-op program and their host organizations). While some barriers were specific to a given EDG, others were common across EDGs. These findings provide a fuller picture of the kinds of barriers experienced by WIL students within and across EDGs. Background: The prevalence of tuberculosis infection is critical to the design of tuberculosis prevention strategies, yet is unknown in Canada. We estimated the prevalence of tuberculosis infection among Canadian residents born abroad. Methods: We estimated the prevalence of tuberculosis infection by age and year of migration to Canada for people from each of 168 countries by constructing country-specific and calendar year–specific trends for annual risk of infection using a previously developed model. We combined country-specific prevalence estimates with Canadian Census data from 2001, 2006, 2011, 2016 and 2021 to estimate the overall prevalence of tuberculosis infection among foreign-born Canadian residents. Results: The estimated overall prevalence of tuberculosis infection among foreign-born people in Canada was 25% (95% uncertainty interval [UI] 20%–35%) for census year 2001, 24% (95% UI 20%–33%) for 2006, 23% (95% UI 19%–30%) for 2011, 22% (95% UI 19%–28%) for 2016 and 22% (95% UI 19%–27%) for 2021. The prevalence increased with age at migration and incidence of tuberculosis in the country of origin. In 2021, the estimated prevalence of infection among foreign-born residents was lowest in Quebec (19%, 95% UI 16%–24%) and highest in Alberta (24%, 95% UI 21%–28%) and British Columbia (24%, 95% UI 20%–30%). Among all foreign-born Canadian residents with tuberculosis infection in 2021, we estimated that only 1 in 488 (95% UI 185–1039) had become infected within the 2 preceding years. Interpretation: About 1 in 4 foreign-born Canadian residents has tuberculosis infection, but very few were infected within the 2 preceding years (the highest risk period for progression to tuberculosis disease). Background: To develop effective public health policies, programs, and services tailored to the unique sexual health needs of migrant populations, it is essential to understand the myriad socio-ecological factors that influence their sexual health. This qualitative community-based participatory study aimed to explore factors influencing migrants' sexual health at different socio-ecological levels in a Canadian setting. Methods: Participants (n = 34) from African, Caribbean, Black; Latin American; South Asian; Middle Eastern, as well as East and Southeast Asian communities were recruited across Manitoba using printed flyers, community organizations, and social media. Individual interviews, conducted in English, French, Mandarin, Cantonese, Tagalog, Arabic, Swahili, and Tigrinya languages, explored questions relating to sexual health and experiences with service providers. Data were analyzed using reflexive thematic analysis and socio-ecological systems theory. Results: The study uncovered a range of individual, interpersonal, institutional, and socio-structural factors that affect the sexual health of migrants in Manitoba. Individual factors such as sexual health knowledge and testing practices, interpersonal factors like the type of sexual partnerships, institutional factors such as sexual health information needs, language, and service access barriers, and structural-level factors like gender norms and HIV stigma exerted a significant influence on the sexual health practices of study respondents. Sexual health awareness was influenced by various factors including length of time in Canada and involvement in community-based services. Study respondents identified issues related to access to HIV testing and sexual health information, as well as language barriers, racism in healthcare, and HIV stigma. Gender and social norms played a significant role in discouraging communication about sex and safer sex practices. Conclusions: The study highlights the complex interplay of factors that influence the sexual health of migrants, and the need for targeted sexual health awareness campaigns and provision of sexual health information in languages spoken by migrants. Public health interventions focused on improving the sexual health outcomes for migrants should consider the socio-ecological elements identified in this study. Background: Despite the ongoing opioid epidemic, most patients are still prescribed a significant number of opioid medications for pain management after arthroscopic surgery. There is a need for consensus among orthopaedic surgeons and solutions to aid providers in analgesic strategies that reduce the use of opioid pain medications. Purpose: This position statement was developed with a comprehensive systematic review and meta-analysis of exclusively randomized controlled trials (RCTs) to synthesize the best available evidence for managing acute postoperative pain after arthroscopic surgery. Study Design: Position statement. Methods: The Embase, MEDLINE, PubMed, Scopus, and Web of Science databases were searched from inception until August 10, 2022. Keywords included arthroscopy, opioids, analgesia, and pain, and associated variations. We included exclusively RCTs on adult patients to gather the best available evidence for managing acute postoperative pain after arthroscopic surgery. Patient characteristics, pain, and opioid data were extracted, data were analyzed, and trial bias was evaluated. Results: A total of 21 RCTs were identified related to the prescription of opioid-sparing pain medication after arthroscopic surgery. The following recommendations regarding noninvasive, postoperative pain management strategies were made: (1) multimodal oral nonopioid analgesic regimens—including at least 1 of acetaminophen—a nonsteroidal anti-inflammatory drug—can significantly reduce opioid consumption with no change in pain scores; (2) cryotherapy is likely to help with pain management, although the evidence on the optimal method of application (continuous-flow vs ice pack application) is unclear; (3) and (4) limited RCT evidence supports the efficacy of transcutaneous electrical nerve stimulation and relaxation exercises in reducing opioid consumption after arthroscopy; and (5) limited RCT evidence exists against the efficacy of transdermal lidocaine patches in reducing opioid consumption. Conclusion: A range of nonopioid strategies exist that can reduce postarthroscopic procedural opioid consumption with equivalent vocal pain outcomes. Background: Breastfeeding is not recommended for women living with HIV (WLWH) in Canada. We described the prevalence of breastfeeding and explored experiences of care, support, and stigma related to infant feeding. Setting: Quebec, Ontario, and British Columbia (Canada). Methods: Data were obtained from the HIV Women's Sexual and Reproductive Health Cohort Study (CHIWOS) surveys, conducted between 2013 and 2018. Results: Breastfeeding was reported by 73.5% of the 786 women who delivered before HIV diagnosis and 7.3% of the 289 women who delivered after HIV diagnosis. Among them, earlier year of delivery, delivery outside of Canada, and African, Caribbean, Black ethnicity were independently associated with increased odds of breastfeeding. Among WLWH who had a live birth during the last year, 77% (40/52) felt that they had received support regarding infant feeding practices, and 77% (23/30) were concerned that not breastfeeding could lead to them being identified as WLWH. Among 71 women within one year postpartum at any one of the study waves, 89% reported having an undetectable viral load. Conclusion: Breastfeeding experiences were common among WLWH, most often prior to HIV diagnosis. Fear of unintentional HIV status disclosure when not breastfeeding and challenges to maintain an undetectable HIV viral load are important issues to address during postpartum care. The military lifestyle can be defined by a triad of unique stressors: frequent relocation, parental absence, and risk of injury, illness and/or death of a serving member. Research has suggested that this unique triad of stressors can impact the mental health of children and youth living in military families. However, research focusing on the mental health of children and youth living in military families overwhelmingly focuses on the American context. Due to key contextual differences, it is unclear to what extent the American findings are representative of military-connected children and youth living in other geographical contexts, such as Canada. A large qualitative study was conducted to explore the mental health of military-connected children in Canada from three perspectives: child, parent, and service provider. This paper reports on the service provider findings. Using individual semi-structured interviews, data were collected from 11 service providers. Data analysis was guided by qualitative content analysis. Two main themes emerged: (1) the mental health of children living in military families may be impacted by the military lifestyle stressors, and (2) the mental health of children living in military families can be impacted by the accessibility and availability of mental health services. While this qualitative study builds upon recent Canadian work that has considered the service provider perspective, additional research is needed to better understand the experiences of service providers who support military-connected children and youth. Highlights: American research shows that the military lifestyle factors can impact the mental health of military-connected children. The mental health of military-connected children has yet to be extensively explored in Canada. The mental health of children living in Canadian military families may be impacted by the military lifestyle stressors. The mental health of Canadian military-connected children can be impacted by the accessibility and availability of mental health services. (1) Background: The COVID-19 pandemic has highlighted the critical role of medical laboratory technologists (MLTs) in the healthcare system. Little is known about the challenges MLTs faced in keeping up with the unprecedented demands posed by the pandemic, which contributed to the notable staff shortage in the profession. This study aims to identify and understand the stressors of MLTs in Canada and the lessons learned through their lived experiences during the pandemic. (2) Methods: In this descriptive qualitative study, we conducted five semi-structured focus groups with MLTs working during the pandemic. The focus group sessions were audio-recorded and then transcribed verbatim. Thematic analysis was used to inductively code data and identify themes. (3) Results: A total of 27 MLTs across Canada participated in the study. Findings highlighted four key themes: (i) unexpected challenges navigating through the uncertainties of an ever-evolving pandemic; (ii) implications of staff shortage for the well-being of MLTs and quality of patient care; (iii) revealing the realities of the hidden, yet indispensable role of MLTs in predominantly non-patient-facing roles; and (iv) leveraging insights from the COVID-19 pandemic to enhance healthcare practices and preparedness. (4) Conclusion: The study provides in-depth insight into the experiences of MLTs across Canada during the pandemic. Based on our findings, we provide recommendations to enhance the sustainability of the laboratory workforce and ensure preparedness and resiliency among MLTs for future public health emergencies, as well as considerations as to combating the critical staff shortage. Social work education in Canada has struggled to identify and disrupt systems of oppression and marginalization, rooted in a history of colonialism and slavery, that continues to reverberate through neocolonial policies, pedagogy, and practice. Universities and social work programs remain white spaces that uphold white supremacy, while actively excluding nonwhite members and their ways of knowing and being in the world. It wasn't until the deepened retrenchment of anti-Black racism during the pandemic, punctuated by the killing of George Floyd, that a group of educators seized an opening to teach an anti-racist and anti-colonial praxis course for the Bachelor of Social Work students in spring of 2021, the first one of its kind in our faculty. Students were encouraged to engage with Indigenous and racialized community youth to imagine and integrate anti-racist and reconciliatory actions as part of their assignments. This paper shares our experiences co-designing and co-teaching this course together with community partners. We emphasize lessons from the applied pedagogical approaches as well as its implications for social work education and social work practice. [ABSTRACT FROM AUTHOR]

Radium-223 (Ra233) prolongs the survival of men with symptomatic bone-predominant metastatic castration-resistant prostate cancer (mCRPC). However, prostate-specific antigen (PSA) response patterns are not closely associated with Ra223 therapy outcomes. Herein, we sought to analyze the impact of Ra223-induced PSA flares on patient outcome. Using a retrospective cohort study of Ra223 treatment in four Ontario/Canada cancer centres, we identified 134 patients grouped into sub-cohorts according to distinct PSA response patterns: (i) initial PSA flare followed by eventual PSA decline; (ii) PSA response (≥30% PSA decrease within 12 weeks of treatment); and (iii) PSA non-response. We analyzed patient characteristics and outcome measures, including overall survival (OS), using the Kaplan-Meier method and log-rank testing. PSA flares were observed in 27 (20.2%), PSA responses in 11 (8.2%), and PSA non-responses in 96 (71.6%) patients. Amongst PSA flare patients, 12 presented with post-flare PSA decreases below baseline and 15 with PSA decreases below the flare peak but above baseline. Although only six flare patients achieved ≥30% PSA decreases below baseline, the median OS of all flare patients (16.8 months, 95% CI 14.9–18.7) was comparable to that of PSA responders and non-responders (p = 0.349). In summary, around 20% of mCRPC patients experience Ra223-induced PSA flares, whose outcome is similar to that of men with or without PSA responses. Background: There has been dramatic reduction in Haemophilus influenzae serotype b (Hib) since introduction of Hib vaccines, but children still experience serious invasive Haemophilus influenzae (Hi) disease caused by various serotype and non-typeable bacteria. The object of this study was to describe the serotype distribution and clinical spectrum of Hi bacteremia in children admitted to Canadian hospitals. Methods: All children with Hi bacteremia admitted 2013 through 2017 to 10 centres across Canada were included. Demographic, clinical, treatment and outcome data were collected. Results: Haemophilus influenzae bacteremia occurred in 118 children of median age 12 months (inter-quartile range: 7–48 months). Forty-three (36%) isolates were non-typeable (NTHi) and 8 were not typed. Of the 67 typeable (THi), Hia (H. influenzae serotype a) (n=36, 54%), Hif (serotype f) (n=19, 26%) and Hib (serotype b) (n=9, 13%) dominated. The THi was more likely than NTHi bacteremia to present as meningitis (p<0.001), particularly serotype a (p=0.04) and less likely to present as pneumonia (p<0.001). Complicated disease (defined as intensive care unit admission, need for surgery, long-term sequelae or death) occurred in 31 (26%) cases and were more likely to have meningitis (p<0.001) than were those with uncomplicated disease. Conclusion: In the era of efficacious conjugate Hib vaccines, NTHi, Hia and Hif have emerged as the leading causes of invasive Hi in Canadian children, with Hia being most likely to result in meningitis and complicated disease. A vaccine for all NTHi and THi would be ideal, but knowledge of the current disease burden from circulating strains will inform prioritization of vaccine targets. [ABSTRACT FROM AUTHOR]
