Industrial forestry activities can increase landscape fragmentation, impacting wildlife populations, particularly Canada's woodland caribou, Rangifer tarandus caribou. To protect caribou in areas with forestry activities, the province of Ontario, Canada, implemented a Dynamic Caribou Harvest Schedule (DCHS). The DCHS spatially aggregates harvest disturbance into regions and distributes them across the landscape to maintain forest patch size–age distributions consistent with a natural variation range. However, the DCHS may negatively impact the cost of timber supply. We compared the DCHS with an alternative zoning approach that assigned the harvest deferral and operational management zones within a large forest area. We compared these approaches using an optimization model that combined harvest scheduling, access road construction, and caribou protection sub-problems. We formulated the protection of caribou habitat and road construction as network flow problems, while the harvesting problem incorporated the ecological constraints prescribed by the forest management plan. We compared the DCHS and zoning approaches in the Wabadowgang Noopming Forest of Ontario, a boreal area within the caribou distribution zone. For the same volume of sustainable harvest, the zoning approach protected less total area but more habitat and old-growth stands over the long term, and yielded lower timber costs by 1.2–2.2 $·m−3 than the DCHS. Healthy pregnancy requires a coordinated immune response, yet complications can arise, putting both the mother's and child's health at risk. Hypertensive disorders of pregnancy (HDP) and gestational diabetes mellitus (GDM) are pregnancy-related complications that account for most maternal morbidity and mortality. Cytokines are proteins released as part of the immune response to disease or infection and regulate inflammation. Certain pregnancy complications cause localized and systemic inflammation; however, cytokine profiles specific to such complications are not well understood. This study aims to examine associations between pregnancy complications of HDP and GDM and cytokine profiles in the second trimester of pregnancy. Data was obtained from the All Our Families birth cohort in Calgary, Alberta, Canada. The cohort collected questionnaires at the time of participant enrollment and maternal blood samples at 17–23 weeks gestation. Cases of HDP (n = 27) and GDM (n = 31) were matched to controls on BMI, maternal age, and smoking status in the preconception period at a 1:3 ratio. Cytokine levels were measured in blood samples using Luminex xMAP technology using a panel of 42 cytokines. Using R software, a Classification and Regression Tree (CART) analysis was conducted to identify cytokine profiles and levels associated with each complication. Four cytokines were identified in the HDP CART (in descending order of importance): Monocyte Chemoattractant Protein-1 (cut-off: <480pg/mL), Macrophage Inflammatory Protein-1β (cut-off: ≥26pg/mL), Eotaxin (cut-off: <27/≥27&<36/≥36pg/mL), and Soluble Cluster of Differentiation 40 Ligand (cut-off: <1342pg/mL). Six cytokine levels were identified in the GDM CART: Interleukin-1 Receptor Antagonist (IL-1Ra; cut-off: <25pg/mL), Interleukin-5 (cut-off: ≥0.4pg/mL), Interferon-γ (cut-off: <4.9pg/mL), IL-1Ra (cut-off: ≥111pg/mL), Eotaxin (cut-off: ≥21pg/mL), and Interleukin-18 (cut-off: ≥155pg/mL). By examining the complex inter-relationships between cytokines, findings of cytokine profiles guide further research in identifying biomarkers of pregnancy complications relevant to the design of the future management or prevention of these conditions. Background: Globally, pharmaceutical companies offer patient support programs in tandem with their products, which aim to enhance medication adherence and patient experience through education, training, support and financial assistance. We sought to identify the proportion and characteristics of such patient support programs in Canada and to describe the nature of supports provided. Methods: We conducted a crosssectional study to identify and characterize all marketed prescription drugs available in Canada as of Aug. 23, 2022, using the Health Canada Drug Product and CompuScript databases. To describe the nature of supports provided, we conducted a content analysis of publicly available patient support program websites and Web-based documents. Using logistic regression, we identified characteristics of drugs associated with having a patient support program including brand-name or branded generic (generic medications with a proprietary name), orphan (medications for rare diseases) or biologic drug status; estimated total cost of prescriptions dispensed at retail pharmacies; and price per unit. Results: Of the 2556 prescription drugs marketed by 89 companies in the study period, 256 (10.0%) had a patient support program in Canada. Many of the 89 drug manufacturers (n = 55, 61.8%) offered at least 1 patient support program, frequently relying on third-party administrators for delivery. Brandname and branded generic medications, biologic agents and drugs with orphan status were more likely to have a patient support program than generic drugs. Compared with drugs priced $1.01–$10.00 per unit, drugs priced $10.01–$100.00 per unit were nearly 8 times more likely to have a patient support program (adjusted odds ratio 7.54, 95% confidence interval 4.07– 14.64). Most sampled patient support programs included reimbursement navigation (n = 231, 90.2%) and clinical case management (n = 223, 87.1%). Interpretation: About 1 in 10 drugs marketed in Canada has a manufacturersponsored patient support program, but these are concentrated around brand-name, branded generic, biologic and high-cost drugs, often for rare diseases. To understand the impact of patient support programs on health outcomes and sustainable access to cost-effective medicines, greater transparency and independent evaluation of patient support programs is necessary. Critical qualitative health researchers typically occupy and navigate liminal academic spaces and statuses, with one foot planted in the arts and social sciences and the other in biomedical science. We are at once marginalized and empowered, and this liminality presents both challenges and opportunities. In this article, we draw on our experiences of being (often the lone) critical qualitative health scholars on thesis advisory committees and dissertation examinations, as well as our experiences of publishing and securing funding, to illuminate how power and knowledge relations create conditions that shape the nature of our roles. We share strategies we have developed for standing our theoretical and methodological ground. We discuss how we use the power of our liminality to hold firm, push back, and push forward, to ensure that critical qualitative research is not further relegated to the margins and its quality and integrity sustained. The Hare Psychopathy Checklist-Revised (PCL-R; Hare, 2003) is a commonly used psychological test for assessing traits of psychopathic personality disorder. Despite the abundance of research using the PCL-R, the vast majority of research used samples of convenience rather than systematic methods to minimize sampling bias and maximize the generalizability of findings. This potentially complicates the interpretation of test scores and research findings, including the "norms" for offenders from the United States and Canada included in the PCL-R manual. In the current study, we evaluated the psychometric properties of PCL-R scores for all male offenders admitted to a regional reception center of the Correctional Service of Canada during a 1-year period (n = 375). Because offenders were admitted for assessment prior to institutional classification, they comprise a sample that was heterogeneous with respect to correctional risks and needs yet representative of all offenders in that region of the service. We examined the distribution of PCL-R scores, classical test theory indices of its structural reliability, the factor structure of test items, and the external correlates of test scores. The findings were highly consistent with those typically reported in previous studies. We interpret these results as indicating it is unlikely any sampling limitations of past research using the PCL-R resulted in findings that were, overall, strongly biased or unrepresentative. A personal narrative is presented which explores the author's experience of going to British Columbia Institute of Technology (BCIT), Vancouver/Burnaby, British Columbia, Concordia University College of Alberta, Edmonton and First Nations University in Regina Saskatchewan after winning a Canadian trip to explore the environmental health science (EHS) program in Canada.

The routine reporting of actionable statistics to improve system performance and prevent premature mortality has been promoted for decades. A key statistic produced nationally and globally is infant mortality. State governments define, collect and report vital events. In Canada, vital statistics is a provincial responsibility. The provinces, however, do not uniformly collect vital events for First Nations who are under Federal fiduciary responsibility or uniformly maintain a registration group field for disaggregation purposes. In 2008, Canada's Public Health Agency conceded that a lack of a First Nations identifier has obscured any understanding of First Nations perinatal health. Given these drivers of variability and the complex multi-jurisdictional vital statistics environments in which they occur, this paper demonstrates, using data linkage methods, a way to improve the estimation of infant mortality for the First Nations population in Manitoba, Canada. Objectives: Bystander resuscitation efforts, such as cardio-pulmonary resuscitation (CPR) and use of an automatic external defibrillator (AED), save lives in cardiac arrest cases. School training in CPR and AED use may increase the currently low community rates of bystander resuscitation. The study objective was to determine the rates of CPR and AED training in Toronto secondary schools and to identify barriers to training and training techniques. Methods: This prospective study consisted of telephone interviews conducted with key school staff knowledgeable about CPR and AED teaching. An encrypted Web-based tool with prespecified variables and built-in logic was employed to standardize data collection. Results: Of 268 schools contacted, 93% were available for interview and 83% consented to participate. Students and staff were trained in CPR in 51% and 80% of schools, respectively. Private schools had the lowest training rate (39%). Six percent of schools provided AED training to students and 47% provided AED training to staff. Forty-eight percent of schools had at least one AED installed, but 25% were unaware if their AED was registered with emergency services dispatch. Cost (17%), perceived need (11%), and school population size (10%) were common barriers to student training. Frequently employed training techniques were interactive (32%), didactic instruction (30%) and printed material (16%). Conclusions: CPR training rates for staff and students were moderate overall and lowest in private schools, whereas training rates in AED use were poor in all schools. Identified barriers to training include cost and student population size (perceived to be too small to be cost-effective or too large to be implemented). Vancouver is located in a temperate rain forest surrounded by mountains and water. Indigenous communities, including the Nations whose homelands the city occupies, have recognized that the abundance of this place can build healthy communities for many generations. But contemporary Vancouver is not a healthy city for all. Since 2014, Vancouver has adopted a Healthy City Strategy, a master social sustainability plan to address inequities in the social determinants of health at a population scale. This includes a number of population health targets and indicators. However, implementation has been slow, with substantive integration of health into policy not yet achieved. With support from the Partnership for Healthy Cities, we have translated the Healthy City Strategy’s indicators into an online dashboard, including disaggregated local data. In addition to public accountability, this tool enables data to be a platform for aligning city policy, action and investment toward common health outcomes. In parallel, we are also piloting more community-generated approaches to health data and indicators, with a focus on co-creating health indicators with Indigenous communities. Preliminary results have shown improvement in collaboration across city departments, leading to increased focus on upstream, preventive work. The dashboard is enabling frameworks for funding and working with the social service sector toward common health goals. Engagement efforts have shown the continued importance of a holistic strategy for health that is co-created with community knowledge and priorities. These interventions must be sustained and integrated into ongoing work. The technical work of developing the dashboard contributes to city-wide efforts toward data monitoring and reporting. Data has shown to be an important tool for beginning conversations, but sharing data about health inequities experienced by communities must be matched with an ongoing commitment to engagement and co-creation. Key messages: Population health data can be leveraged through interactive tools to better integrate and align city policies, plans and investments to address the social determinants of health. Unlike the iconic 1972 Summit Series, the 1974 Series between the Soviet Union and Team Canada—this time a national team consisting solely of World Hockey Association (WHA) players—remains largely a Canadian folklore footnote. In this essay, the authors hold up the 1974 Summit Series for critical reflection by focussing on three interrelated themes to reveal the competing political-economic, cultural, and diplomatic interests that were at stake in this largely forgotten event. First, they provide a political-economic analysis of the emergence of the WHA and the league's objectives for the 1974 Summit Series. The authors then offer an analysis of the role of the second Summit Series as an instance of "hockey diplomacy," a component of Prime Minister Trudeau's broader foreign policy agenda, which included improved bilateral relations with Moscow. Finally, the authors examine the significance of the 1974 Summit Series as part of the foreign policy initiatives of the Communist Party of the Soviet Union. Despite the stated diplomatic ambitions of both Canada and the Soviet Union, though, the 1974 Summit Series (like its 1972 predecessor) ended in violence, politicking, and accusations of unsportsmanlike behaviour, and would mark the finality of this specific bilateral model of sporting exchange to facilitate cultural contact and goodwill between both nations in the Cold War. Deinstitutionalization, changes in both criminal and civil law, and the use of the criminal justice system to manage problematic behavior by individuals with serious mental illnesses have affected the number of people entering forensic mental health systems. This review examines data revealing trends in demand for forensic services in Canada, the United States, and internationally. The number of beds and resources allocated to forensic mental health services has steadily increased not only in Canada, but in other countries across the world. In Canada, the number of new accused entering the system doubled annually from 1992 to 2004. In the United States, the number of individuals found not guilty by reason of insanity (NGRI), forensic beds and forensic expenditures have all increased dramatically in recent years. Furthermore, countries in Western Europe – Austria, Denmark, England, Germany, Ireland, Italy, the Netherlands, Spain and Switzerland – have reported an increase of 110%, on average, in the number of forensic beds between 1990 and 2006. These increases have implications for both forensic and non-forensic mental health services. Suggestions to reduce forensic demand and increase the successful reintegration of forensic clients into the community are discussed. [ABSTRACT FROM PUBLISHER]

The purpose of this study was to develop an expertise-based prognostic model for the treatment of complex posttraumatic stress disorder (PTSD) and dissociative identity disorder (DID). We developed a survey in 2 rounds: In the first round we surveyed 42 experienced therapists (22 DID and 20 complex PTSD therapists), and in the second round we surveyed a subset of 22 of the 42 therapists (13 DID and 9 complex PTSD therapists). First, we drew on therapists' knowledge of prognostic factors for stabilization-oriented treatment of complex PTSD and DID. Second, therapists prioritized a list of prognostic factors by estimating the size of each variable's prognostic effect; we clustered these factors according to content and named the clusters. Next, concept mapping methodology and statistical analyses (including principal components analyses) were used to transform individual judgments into weighted group judgments for clusters of items. A prognostic model, based on consensually determined estimates of effect sizes, of 8 clusters containing 51 factors for both complex PTSD and DID was formed. It includes the clusters lack of motivation, lack of healthy relationships, lack of healthy therapeutic relationships, lack of other internal and external resources, serious Axis I comorbidity, serious Axis II comorbidity, poor attachment, and self-destruction. In addition, a set of 5 DID-specific items was constructed. The model is supportive of the current phase-oriented treatment model, emphasizing the strengthening of the therapeutic relationship and the patient's resources in the initial stabilization phase. Abstract: Moraine and glacier dams bordering the Homathko Icefield in the southern British Columbia Coast Mountains failed in the 1980s and 1990s, causing catastrophic downstream floods. The largest of the floods occurred in August 1997 and was caused by overtopping and rapid breaching of the moraine dam that impounds Queen Bess Lake. The floodwaters from Queen Bess Lake eroded Holocene-age sedimentary deposits along the west fork of Nostetuko River and caused a steep rise in the hydrograph of Homathko River at the head of Bute Inlet, ∼115km downstream. A field investigation of the eroded valley fill in 2008, revealed multiple paraglacial valley-fill units, many of which are capped by in situ stumps and woody detritus. Dendrogeomorphological field techniques were employed to develop a chronology for the buried forests. A regional tree-ring chronology spanning the interval CE 1572–2007 was constructed from living subalpine fir (Abies lasiocarpa) trees at seven sites in the southern Coast Mountains. In cases where subfossil stumps and boles predated the regional chronology, relative death dates constrained by radiocarbon ages were assigned to floating chronologies. By combining these dendrogeomorphological dating methods, we identified six floodplain aggradation episodes within the past 1200years. Comparison to local and regional glacial histories suggests that these events reflect climate-induced Little Ice Age changes in local glacier cover. [Copyright &y& Elsevier]

Canadian labor arbitrations resolving disputes over employer discipline for alleged coworker sexual harassment were analyzed in order to explore how well women's right to a harassment-free workplace was protected in the arbitral assessment of the mitigating factors argued during hearings to support the male harasser. An analysis of the awards revealed a significant difference between reasoning that applied traditional, conventional, arbitral principles and reasoning acknowledging that the nature of sexual harassment, its seriousness, and its impact on the complainants and their female coworkers merited a creative, more independent line of reasoning. Examining arbitrations using a feminist lens, one that emphasized gendered power relationships in the workplace, enabled new insights indicating the gendering of some traditional arbitral jurisprudence and principles and some possibilities for legal reform. Nearly a quarter of all active U.S. physicians are international medical graduates (IMGs)—physicians trained outside the United States and Canada. We describe changes in characteristics of lMGs from 1981 to 2001 and compare them with their U.S. medical graduate (USMG) counterparts. Since 1981, the leading source countries for IMGs have included India, the Philippines, and Mexico. IMGs were more likely to be generalists and to practice in designated underserved areas than USMGs but slightly less likely to practice in isolated small rural areas and persistent-poverty counties. There are great movements of people taking place around the world and our societies are becoming more diverse and multiethnic in nature, especially in the urban environments of the larger cities. In most countries, schools still represent the ‘status quo’ and most principals are from the ‘majority culture’. There is little empirical evidence describing how schools respond to the changing demographics of the contemporary world. In this paper such empirical evidence is presented. The research question guiding this eight-nation study was: In what ways do principals facilitate access to schools for all children? Exploring matters such as school policies, recognition of difference and inclusion strategies, data are presented from the Canadian elements of this international collaborative study. This article examines the trends and determinants of union dissolution in Canada using data front the 1995 Canadian General Social Survey, looking at the aggregate trends' in Canadian unions and exploring the transition front cohabitation to either marriage or separation. A life-course perspective is used to assess both marital and nonmarital union stability. Both marital and nonmarital unions are combined and various determinants of their dissolution are considered. In the latter half of the 1980s, when it was becoming apparent in Canada that previous equal pay policies had failed to close the gender wage gap, a number of provincial governments introduced pro–active pay equity policies. All these initiatives required the negotiation of pay equity in unionized workplaces. Leading up to the implementation of pay equity in Ontario, industrial relations specialists predicted an insurmountably conflictual process, whereas governments expected a new level of partnership with unions. Given these opposite expectations, the article aims to identify any significant patterns of cooperation and conflict in the pay equity bargaining process and to explore reasons for their dynamic. Based on a case study of Newfoundland’s health sector, neither prediction was correct as both conflict and cooperation occurred. It is argued that both the specificity and differences in the negotiations studied can be better understood by exploring the complex intertwinings of gender and class, namely, the ongoing articulation of their main manifestations: a reinforcing hierarchy and a transformative labour–feminist politic in the unions involved. The article concludes with some theoretical and policy observations concerning the importance of building in gender and class to current models of cooperative collective bargaining, as well as recognizing their importance in the pay equity process. This article assesses determinants of social support among the foreign-born elderly in Canada. We draw cross-sectional data from the second cycle of the National Population Health Survey, conducted by Statistics Canada in 1996-1997, and use ordinary least squares models for our empirical analysis. We focus on three measures of social support: perceived social support, social involvement, and social contact. Generally, we find that poor physical and/or mental health is negatively associated with our measures of support. Emotional problems, one of the most consistent predictors, erode social support. Also, social involvement declines with mobility problems but increases with self-reported health status, whereas social contact also increases with cognitive function. As with other studies, and consistent with relationships witnessed among the native-born population, our results indicate that the married/cohabiting are more likely than the separated/divorced or widowed to perceive social support. Utilizing Canadian longitudinal data, we find that exiting both marriage and cohabitation seems to have similar effects: Dissolving either union tends to be associated with a decrease in physical health, mental health, or both. With possible selection effects eliminated and protection effects held constant, we find that remaining in either type of union generally is associated with poorer health. We speculate that decreased union quality may account for this inverse relationship, and that protection effects may explain much of the reported health gains associated with union life. The substantial number of childless elderly in Canada and the United States raises important concerns about the nature of their family support, as well as their psychological well-being. Although the (mixed) effects of childlessness on the mental health of the elderly are well documented in the literature, little is known about the social distribution of psychological distress and depression within this population: how well the childless elderly live, the status of their mental health, and what factors affect their psychological well-being are all crucial questions that remain unanswered. This paper utilizes Pearlin et al.’s (1981) stress process model to examine psychological distress and depression among the childless elderly. Our empirical analysis uses a nationally representative sample of childless individuals aged 55 and over (N = 2,311) from the 1996-1997 National Population Health Survey (NPHS), and generalized linear model (GLM) techniques. Generally, our results support the stress process model. We find that individual-level stressors exert a strong and consistently negative impact on psychological distress and depression. A constitution has traditionally been seen as the documentary record of a settlement of conflict. This traditional constitution is an enactive document, consummating the creation of a polity. Constitution-making has been a widespread practice in the many conflicted and divided societies of the late twentieth century. The issues of recent conflicts are concerned with the recognition of identities as well as with provisions for the legitimate exercise of power. This agenda necessitates a process as important as the product, both open-ended and open to participation. I propose that we reconsider constitution-making as itself a part of the process of conflict transformation. Defining constitution-making as a forum for negotiation or a continuing conversation amid conflict and division draws attention to the distinctive characteristics of modern constitutionalism and to the ways in which this process helps or hinders the transformation of conflict. Historically, hunger was used as a tool of coercion and manipulation, and as a weapon to eradicate Indigenous populations. Through policy decisions, the support for and removal of Indigenous children, and other assimilative practices, social work has contributed to the perpetuation of 'helping practices' which damaged Indigenous cultures and well-being. Today, experiences of hunger are still tied to colonialism. There is a need to examine the complex history of feeding Indigenous peoples in Canada and to work to reclaim and heal Indigenous food systems. Background and purpose Minority stressors have been linked with alcohol use among transgender and gender diverse (TGD); however, no ecological momentary assessment studies have examined daily links between minority stress and alcohol use specifically among TGD. This study examined gender minority stressors and resilience as predictors of same-day or momentary alcohol-related outcomes. Feasibility and acceptability of procedures were evaluated. Methods Twenty-five TGD adults (mean age = 32.60, SD = 10.82; 88% White) were recruited Canada-wide and participated remotely. They completed 21 days of ecological momentary assessment with daily morning and random surveys (assessing alcohol outcomes, risk processes, gender minority stressors, resilience), and an exit interview eliciting feedback. Results Gender minority stress had significant and positive within-person relationships with same-day alcohol use (incidence risk ratio (IRR) = 1.12, 95% confidence interval [CI] [1.02, 1.23]), alcohol-related harms (IRR = 1.14, 95% CI [1.02, 1.28]), and coping motives (IRR = 1.06, 95% CI [1.03, 1.08]), as well as momentary (past 30-min) alcohol craving (IRR = 1.32, 95% CI [1.18, 1.47]), coping motives (IRR = 1.35, 95% CI [1.21, 1.51]), and negative affect (IRR = 1.28, 95% CI [1.20, 1.36]). Gender minority stress indirectly predicted same-day drinking via coping motives (ab = 0.04, 95% CI [0.02, 0.08]). Resilience was positively associated with same-day alcohol use (IRR = 1.25, 95% CI [1.03, 1.51]) but not harms. Conclusions TGD adults may use alcohol to cope with gender minority stress, which can increase the risk for alcohol-related harms. Caring for angry patients can be a threatening experience. Grounded theory research was used to explore female nurses' reactions and feelings as the recipients of patient anger. The data were collected by interviewing nine female registered nurses in two hospitals in south-western Nova Scotia. The participants were asked to discuss their feelings and responses to an intense encounter with an angry patient. Anger was defined as a multi-dimensional concept with negative cogitations. The authors suveyed a random sample of community-resident offenders in British Columbia to determine the prevalence of wife assault. The rate was high: About 1 in 5 men had a known history (current or lifetime). These findings support the view that wife assault is an important policy issue for provincial corrections agencies. The Outardes Estuary (Quebec) is a regressive sandy system developed in a sub-boreal climatic setting. The Outardes River drains an area of 1870 km[sup2] and was dammed for hydroelectric purposes in the 1960s and `70s. By reconstructing the hydrographs and mapping the morphology of the estuary from aerial photographs over a period of 21 years, we have documented the changes in the estuary resulting from the modification of the hydrologic regime. Until the mid-1960s, a catastrophic spring freshet (1800-2800 m[sup3]s[sup-1]) controlled the morphology of the estuary. The mouth was dominated by a disorganized, braided channel pattern, and large fluvial discharge-generated transverse bars. Peripheral channel areas in the upper estuary were zones of transport at flood stages. For 18 months in 1968 and '69 the river discharge was drastically reduced (50 m[sup3]s[sup-1]) to accomplish the main reservoir filling. Tidal currents and waves filled and remodeled the morphology of the mouth of the estuary, while the peripherial zones in the upper estuary experienced low-energy sedimentation. Since the generating plants were brought on-line in 1969, the fluvial discharge has been unable to modify substantially the sand body geometry in the mouth of the estuary. In the upper estuary the peripheral regions continue to be filled in, but the thalweg is being eroded. Based on the results of our coring programme, we suggest that major environmental changes may leave a sedimentary signature (addition or subtraction of size modes) which can be identified by the detailed analysis of grain size distributions. We explored attitudes of gay, bisexual, and other men who have sex with men (GBM) toward their amphetamine-use and associations with reduced use over time. We recruited sexually-active GBM aged 16+ years in Montreal, Toronto, and Vancouver, Canada, from 02-2017 to 08-2019, with follow-up visits every 6–12 months until November 2020. Among participants who reported past-six-month (P6M) amphetamine-use at enrollment, we used logistic regression to identify demographic, psychological, social, mental health, other substance-use, and behavioral factors associated with reporting needing help reducing their substance-use. We used mixed-effects logistic regression to model reduced P6M amphetamine-use with perceived problematic-use as our primary explanatory variable. We enrolled 2,449 GBM across sites. 15.5-24.7% reported P6M amphetamine-use at enrollment and 82.6 − 85.7% reported needing no help or only a little help in reducing their substance use. Reporting needing a lot/of help or completely needing help in reducing substance-use was associated with group sex participation (AOR = 2.35, 95%CI:1.25–4.44), greater anxiety symptomatology (AOR = 2.11, 95%CI:1.16–3.83), greater financial strain (AOR = 1.35, 95%CI:1.21–1.50), and greater Escape Motive scores (AOR = 1.07, 95%CI:1.03–1.10). Reductions in P6M amphetamine-use were less likely among GBM who perceived their amphetamine-use as problematic (AOR = 0.17 95% CI 0.10 − 0.29). Most amphetamine-using GBM did not feel they needed help reducing their substance use, and many reported reduced amphetamine-use at subsequent visits. Those who perceived their use as problematic were less likely to reduce their use. Further interventions to assist GBM in reducing their use are needed to assist those who perceive their use as problematic. [ABSTRACT FROM AUTHOR]

The COVID‐19 pandemic led to the widespread adoption of virtual care—the use of communication technologies to receive health care at home. We explored the differential impacts of the rapid transition to virtual care during the COVID‐19 pandemic on health‐care access and delivery for gay, bisexual and queer men (GBQM), a population that disproportionately experiences sexual and mental health disparities in Canada. Adopting a sociomaterial theoretical perspective, we analysed 93 semi‐structured interviews with GBQM (n = 93) in Montreal, Toronto and Vancouver, Canada, conducted between November 2020 and February 2021 (n = 42) and June‐October 2021 (n = 51). We focused on explicating how the dynamic relations of humans and non‐humans in everyday virtual care practices have opened or foreclosed different care capacities for GBQM. Our analysis revealed that the rapid expansion and implementation of virtual care during the COVID‐19 pandemic enacted disruptions and challenges while providing benefits to health‐care access among some GBQM. Further, virtual care required participants to change their sociomaterial practices to receive health care effectively, including learning new ways of communicating with providers. Our sociomaterial analysis provides a framework that helps identify what works and what needs to be improved when delivering virtual care to meet the health needs of GBQM and other diverse populations. In 2015/2016, Canada's largest provinces implemented publicly-funded human papillomavirus (HPV) vaccination programs for gay, bisexual, and other men who have sex with men (GBM) ≤ 26 years old. We sought to describe HPV vaccine uptake among GBM and determine barriers and facilitators to vaccine initiation with a focus on healthcare access and utilization. Engage is a cohort study among GBM aged 16 + years in three Canadian cities recruited from 2017 to 2019 via respondent driven sampling (RDS). Men completed a comprehensive questionnaire at baseline. By publicly-funded vaccine eligibility (≤26 years old = eligible for vaccination, ≥27 years old = ineligible), we described HPV vaccine uptake (initiation = 1 + dose, completion = 3 doses) and explored factors associated with vaccine initiation using Poisson regression. All analyses were weighted with the RDS-II Volz-Heckathorn estimator. Across the three cities, 26–35% and 14–21% of men ≤ 26 years and 7–26% and 2–9% of men ≥ 27 years initiated and completed HPV vaccination, respectively. Vaccine initiation was significantly associated with STI/HIV testing or visiting a HIV care specialist in the past six months (≤26: prevalence ratio[PR] = 2.15, 95% confidence interval[CI] 1.06–4.36; ≥27: PR = 2.73, 95%CI 1.14–6.51) and past hepatitis A or B vaccination (≤26: PR = 2.88, 95%CI 1.64–5.05; ≥27: PR = 2.03, 95%CI 1.07–3.86). Among men ≥ 27 years old, vaccine initiation was also positively associated with accessing PrEP, living in Vancouver or Toronto, but negatively associated with identifying as Latin American and increasing age. Vaccine initiation was twice as likely among men ≥ 27 years with private insurance versus no insurance. Sixty-five to 74% of men eligible for publicly-funded vaccine across the three cities remained unvaccinated against HPV by 2019. High vaccine cost may partly explain even lower uptake among men ≥ 27 years old. Men seeking sexual health care were more likely to initiate vaccination; bundling vaccination with these services may help improve HPV vaccine uptake. [ABSTRACT FROM AUTHOR]

Intersectoral processes that bring together public institutions, civil society organizations and affected community members are essential to tackling complex health equity challenges. While conventional wisdom points to the importance of human relationships in fostering collaboration, there is a lack of practical guidance on how to do intersectoral work in ways that support authentic relationship-building and mitigate power differentials among people with diverse experiences and roles. This article presents the results of RentSafe EquIP, a community-based participatory research initiative conducted in Owen Sound, Canada, in the midst of a housing crisis. The research explored the potential utility of equity-focused intersectoral practice (EquIP), a novel approach that invests in human relationships and knowledge co-creation among professionals and affected members of the community. The three-phase EquIP methodology centred the grounded expertise of community members with lived/living experience of housing inadequacy to catalyze reflexive thinking by people in professional roles about the institutional gaps and barriers that prevent effective intersectoral response to housing-related inequities. The research demonstrated that EquIP can support agency professionals and community members to (i) engage in (re)problematization to redefine the problem statement to better include upstream drivers of inequity, (ii) support reflexivity among those in professional roles to identify institutional practices, policies and norms that perpetuate stigma and impede effective intersectoral response and (iii) spark individual and collective agency and commitment towards a more equity-focused intersectoral system. Background: The Good Life with osteoArthritis: Denmark (GLA:DTM) program is an evidence‐based education and exercise program designed for individuals with symptomatic hip and knee osteoarthritis. Launched in Denmark, it has been implemented across Europe, Australia, and North America. The authors assessed the feasibility of GLADTM implementation in Canada (Alberta) by applying the RE‐AIM framework. An evaluation objective was to identify factors impacting the implementation of the program in both publicly funded and private rehabilitation settings, based on the experience of the initial cohort of providers and clinic leaders who set up and delivered the program. Methods: Semi‐structured telephone interviews were conducted with GLA:DTM‐trained providers, managers, or directors of clinics across Alberta. Braun and Clarke's thematic approach was used to code the data and identify emergent categories and themes. Those relevant to the implementation were identified and by consensus, categorized as facilitators of and challenges to the implementation process. Results: Eighteen GLA:DTM ‐trained providers and three clinic leaders from a range of clinical settings completed an interview. Seven common themes emerged in relation to implementation across the study settings. Three themes reflect facilitators of implementation (program acceptability by providers, multi‐level support mechanisms, and program flexibility) and four implementation challenges (direct and indirect costs, lack of external referrals, program access issues, and lack of suitable space). The initial implementation of the program was exploratory with limited focus on long‐term sustainability. Conclusions: The GLA:DTM program is a translatable program that can be implemented with relative ease in both public and private rehabilitation settings; however, costs, space constraints, and having an adequate referral base were noted challenges. Critical scholarship has illustrated how COVID-19 public health policies can enact racism, classism, and cis-heteronormativity, perpetuating harms among vulnerable communities. We sought to examine the accounts of gay, bisexual, and queer men (GBQM) in Canada on how normative ideologies played out in COVID-19 directives and what impacts these orders had on their lives. Two rounds of semi-structured interviews with GBQM in Montreal (n = 30), Toronto (n = 33), and Vancouver (n = 30) were conducted between November 2020-February 2021 and June-October 2021 (N = 93). Our reflexive thematic analysis drew on the frameworks of cis-heteronormativity and intersectionality to examine how normative assumptions about kinship, sociality, and privilege in COVID-19 public health directives were understood and experienced by GBQM. Our participants explicated how cis-heteronormativity was pervasive in COVID-19 public health messaging, noting that stay-at-home orders and limits on social gatherings reinforced heterosexual forms of kinship. The privileging of cis-heteronormative sociality had detrimental effects on the sense of belonging and identity formation of many participants due to restricted access to queer spaces during the pandemic. Others indicated that stay-at-home orders failed to account for the heterogeneity of queer people's experiences of homelessness and structural racism. These findings provide valuable insights into how public health efforts to control COVID-19 infections have overlooked the complex forms of kinship among GBQM, the importance of queer spaces and community organizations, and the varying vulnerabilities of diverse lesbian, gay, bisexual, trans, and queer (LGBTQ+) groups. There is mounting urgency regarding the mental health of gay, bisexual and other men who have sex with men (GBM). We examined how GBM are understanding the relationship between HIV and their mental health given the increasing biomedicalisation of HIV prevention and care. Our Grounded Theory analysis derived from qualitative interviews with 24 GBM living in Toronto, Canada, including both HIV‐negative and HIV‐positive men. Participants understood biomedical advances, such as undetectable viral load and pre‐exposure prophylaxis (PrEP), as providing some relief from HIV‐related distress. However, they offered ambivalent perspectives on the biomedicalisation of HIV. Some considered non‐HIV‐specific stressors (e.g. unemployment, racial discrimination) more significant than HIV‐related concerns. These men expressed HIV‐related distress as being under control due to biomedical advances or as always negligible when compared to non‐HIV‐specific stressors. Others emphasised the ongoing mental health implications of HIV (e.g. enduring risk and stigma). We describe a tension between optimistic responses to biomedicine's ability to ease the psychosocial burdens associated with HIV and the inability for biomedicine to address the social and economic determinants driving the dual epidemics of HIV and mental distress amongst GBM. We argue for more socio‐material analysis over further sexual behavioural analysis of GBM mental health disparities. [ABSTRACT FROM AUTHOR]
