Objective: To describe the clinical-laboratory profile and analyze the factors associated with the severity of COVID-19., Methods: A prospective cohort study involving patients with COVID-19 admitted to a tertiary hospital in Recife, Brazil. All cases were confirmed by RT-PCR and classified according to severity criteria. A descriptive statistical analysis of the population's characteristics was conducted. Risk factors associated with the outcome of the case according to severity were analyzed by calculating the odds ratio (OR) using the general equation estimation (GEE) model., Results: Among the 75 cases included, 64% were female, and 62.7% were aged 65 years or older. The median length of stay was 9 days (6 - 14). Hypertension (65.3%) and Diabetes Mellitus (36%) were the most frequent comorbidities. Severe forms of COVID-19 constituted 41.3% of the sample. The factors associated with severity were a history of asthma (OR=4.58, 95%CI:1.13 - 18.7), report of anorexia (OR=1, 12, 95%CI:1.01-1.24), and laboratory changes that included elevated platelets (OR=1.00, 95% CI:1.00-1.01), elevated D'Dimer (OR=1, 26, 95% CI:1.04-1.52), elevated aspartate aminotransferase (OR=1.00, 95% CI:1.00-1.01), and gamma-glutamyl transferase (OR=1.22, IC95 %:0.98-1.51), hypernatremia (OR=1.31, 95%CI:1.12-1.52), and hyperkalemia (OR=1.21, 95% CI:1.04-1.41)., Conclusion: Multisystemic involvement with a tendency for thrombophilia, electrolyte disturbances, and hepatic aggression, reflected by laboratory changes, were factors associated with the severity of COVID-19., (© 2024. Objective: to identify the factors associated with death due to COVID-19 among Brazilian postpartum women in the first five months of the pandemic and five subsequent months, and describe the sociodemographic and clinical characteristics of postpartum women who developed the disease. Method: cross- sectional population-based study using a secondary database available in the Sistema de Informação de Vigilância Epidemiológica da Gripe -SIVEP-Gripe (Influenza Epidemiological Surveillance Information System), Brazilian Ministry of Health. A total of 869 postpartum women were included, and the analysis considered the first five months of the pandemic and subsequent five months. Association between the variables of interest and outcome (death due to COVID-19/cure) was investigated using logistic regression. Results: most participants were aged between 20 and 34, of mixed race or Caucasian, and lived in the urban/peri-urban area. The proportion of deaths was 20.2% in the first period and 11.2% in the second. The likelihood of death increased in both periods due to the presence of respiratory signs and symptoms: dyspnea, respiratory distress, and oxygen saturation below 95%, in addition to the need for ventilatory support and intensive care. Conclusion: the proportion of deaths among postpartum women was high and decreased in the second period under study. Respiratory signs and symptoms, mechanical ventilation, and intensive care were associated with death in both periods. [ABSTRACT FROM AUTHOR]

Deforestation and road construction have inflicted negative effects on wildlife populations. To contribute to the comprehension and preservation of wildlife species, this study aimed to evaluate collared anteater presented to a reference center located in São Paulo state, Brazil. Furthermore, the locations where the animals were most frequently found were determined. Data on collared anteaters (Tamandua tetradactyla) examined at the Center for Medicine and Research in Wild Animals (CEMPAS) were evaluated retrospectively for a period of 13 years. A total of 46 collared anteaters was examined, comprised of 14 males, 10 females, and 22 without identifying information. On average, adult males weigh 4.41 kg while adult females weigh 4.95 kg. The most common admission circumstance was vehicular strikes (36.96%), followed by dog bites (21.74%); 23.91% were considered clinically healthy. Animal outcomes were 36.96% (n=17) reintroduced, 34.78% (n=16) fatalities, 15.22% (n=7) transferred to other institutions, and 6.52% (n=3) escaped from the enclosure; and 6.52% (n=3) presented no medical record information. The collared anteaters were found mainly in the municipal areas of Botucatu (21.73%), São Manuel (17.39%), Lençóis Paulista (6.52%), and Bauru (4.34%), with SP-300 being the main highway associated with the occurrences. In conclusion, the main cause of lesion in the collared anteaters evaluated in this study was vehicular strike that that led to death. Objective: to relate complaints presented by emergency room patients, classified using the Manchester Triage System, with the final outcome (discharge/death/transfer). Methods: prospective cohort study, involving 509 patients who were admitted to the emergency room and remained there for more than 24 hours after admission, being monitored to the final outcome. Data were analyzed with a statistical program using descriptive and analytical statistics. Results: the mean age of the patients was 59.1 years and 59.3% were male. The main complaints were unwell adult (130 - 22.5%), shortness of breath in adults (81 - 14.0%), abdominal pain in adults (58 - 10.0%) and behaving strangely (34 - 5.9%), with 87% of the patients being discharged. More deaths were found in the patients classified in the severe colors, with 42.8% classified as red, 17.0% as orange and 8.9% as yellow. Among the patients classified as green, 9.6% died. Conclusion: in the various colors of the Manchester Triage System, death prevailed in patients that presented the complaints of unwell adult, shortness of breath, head injury, major trauma, diarrhea and vomiting. American cutaneous leishmaniasis (ACL) may present different clinical manifestations, immune and therapeutic responses, depending on the Leishmania species, as well as inoculum size and factors inherent to the affected individual. Thus, the aim of this study was to carry out clinical-therapeutic follow-up of Brazilian patients with ACL caused by different Leishmania species. Between 2015 and 2018, patients with ACL from Amazonas and Pernambuco states (Brazil) were submitted to blood collection before and after treatment. The qPCR technique was used to quantify the parasite load. To identify the Leishmania species, one of the following techniques was employed: a conventional PCR performed from biopsy or blood DNA, followed by sequencing; or Multilocus Enzyme Electrophoresis from Leishmania isolated from biopsy/aspirated lesion. A total of 10.8% (23/213) of the patients included in positive cases were followed-up. All 23 patients were clinically and epidemiologically compatible with ACL and were also positive in parasitological tests (86.96%), molecular tests (73.91%) or both (60.87%). Seventeen samples collected before treatment and 11 collected after treatment were positive in the qPCR assay, with a mean parasite load (MPL) of 38.33 fg/μL and 11.81 fg/μL, respectively. Eight samples were positive in both collections. Thirteen patients (56.52%) were clinically cured (wound healing). Ten patients (43.47%) were not clinically cured at the time of return with the attending physician. Identification of Leishmania species was carried out in samples from nine patients, and six were identified as L. (Viannia) braziliensis , 2 as L (Viannia) guyanensis and 1 as L (Leishmania) amazonensis. One patient infected with L. guyanensis and other with L. braziliensis were not clinically cured and increased the mean parasite load after treatment. The data obtained from the followed-up patients and the relationship between clinical evolution and the infecting species demonstrate the need to understand its etiology to define the effective therapeutic protocol. Objective: To describe the clinical impact of the first year treatment with dornase alfa, according to age groups, in a cohort of Brazilian Cystic Fibrosis (CF) patients. Methods: The data on 152 eligible patients, from 16 CF reference centers, that answered the medical questionnaires and performed laboratory tests at baseline (T0), and at six (T2) and 12 (T4) months after dornase alfa initiation, were analyzed. Three age groups were assessed: six to 11, 12 to 13, and =14 years. Pulmonary tests, airway microbiology, emergency room visits, hospitalizations, emergency and routine treatments were evaluated. Student's t-test, chi-square test and analysis of variance were used when appropriated. Results: Routine treatments were based on respiratory physical therapy, regular exercises, pancreatic enzymes, vitamins, bronchodilators, corticosteroids, and antibiotics. In the six months prior the study (T0 phase), hospitalizations for pulmonary exacerbations occurred in 38.0, 10.0 and 61.4% in the three age groups, respectively. After one year of intervention, there was a significant reduction in the number of emergency room visits in the six to 11 years group. There were no significant changes in forced expiratory volume in one second (VEF1), in forced vital capacity (FVC), in oxygen saturation (SpO2), and in Tiffenau index for all age groups. A significant improvement in Shwachman-Kulczychi score was observed in the older group. In the last six months of therapy, chronic or intermittent colonization by P. aeruginosa was detected in 75.0, 71.4 and 62.5% of the studied groups, respectively, while S. aureus colonization was identified in 68.6, 66.6 and 41.9% of the cases. Conclusions: The treatment with dornase alfa promoted the maintenance of pulmonary function parameters and was associated with a significant reduction of emergency room visits due to pulmonary exacerbations in the six to 11 years age group, with better clinical scores in the =14 age group, one year after the intervention. Aim: to assess the clinical evolution of patients hospitalized due to the first episode of Acute Coronary Syndrome (ACS) according to its clinical manifestation. Methods: data were collected from 234 patients, hospitalized between May 2006 and July 2009 due to the first episode of an ACS, by consulting their medical records. Results: 234 patients were hospitalized, 140 (59.8%) due to Acute Myocardial Infarction (AMI). In the group with AMI, 19.3% presented complications, against 12.8% in the group with Unstable Angina (UA) (p=0.19). Angioplasty levels were higher among patients with AMI than with UA (p=0.02) and coronary artery bypass graft surgery was more frequent among UA patients (p=0.03). The majority (227; 97%) survived after the coronary event. Among the seven patients who died during the hospitalization, four had AMI (2.9%) and three UA (3.2%). Conclusions: A larger number of complications were found among infarction victims and the accomplishment of coronary artery bypass graft surgery differed between the groups. [ABSTRACT FROM AUTHOR]
